Abstract:Objective To evaluate the effects of Laparoscopic cholecystectomy (LC) combined with intraoperative cholanggiography (IOC)and intraoperative endoscopic sphincterotomy (IOEST) for the treatment of cholecystocholedocolithiasis. Methods 106 cases with cholecystocholedocolithiasis diagnosed and treated by LC combined with IOC and IOEST were reviewed retrospectively. Results 60 40%(64/106)patients diagnosed cholelithiasis pre-LC were found choledocholithisis; LC combined with IOC and IOEST was successfully performed on 99 of 106 cases(93 40%),and their CBD stones were completely cleared in 98 of 99 cases(99 00%). Six cases(6 1%) were complicated with mild acute pancreatitis, 1 case(0 01%) with duodenal perforation,1 case(0 01%) with bile leakage, and 2 cases(0 02%) with stomach stasis. Conclusions LC combined with IOC and IOEST was a safe ,effective approach to diagnosis and minimally invasive treatment of cholecystocholedocholithiasis.

Abstract:Objectives: Although peroral cholangioscopy has improved the endoscopic treatment of difficult stones, the treatment of intrahepatic stones remains challenging. The incidence of cholangitis is high when peroral cholangioscopy is used to treat intrahepatic stones. This study aimed to investigate the efficacy and safety of endoscopic treatment with peroral cholangioscopy for intrahepatic and common bile duct stones. Methods: Patients aged ≥20 years, who underwent endoscopic treatment with peroral cholangioscopy for intrahepatic or common bile duct stones at Tottori University Hospital from January 2016 to December 2022, were retrospectively evaluated to determine the efficacy and safety of the treatment. Results: Overall, 70 patients were included in this study: 22 in the intrahepatic stone group and 48 in the common bile duct stone group. Stones were smaller (8 vs. 17.5 mm, p < 0.001) and more numerous (p = 0.016) in the intrahepatic stone group than in the common bile duct stone group. Although the common bile duct stone group exhibited a higher rate of complete stone clearance in the first session, no significant differences were observed in the final results. The intrahepatic stone group had a higher incidence of cholangitis (36% vs. 8%, p = 0.007); however, all cases were mild. Conclusions: Endoscopic treatment with peroral cholangioscopy for intrahepatic stones may be associated with a higher incidence of cholangitis than that for common bile duct stones. Since saline irrigation may contribute to the development of cholangitis, it is important to be aware of intraductal bile duct pressure when performing peroral cholangioscopy.

Abstract:A 69-year-old man visited our hospital with chief complaints of abdominal pain and vomiting. He was diagnosed as having choledocholithiasis and admitted for treatment. Although endoscopic retrograde cholangiopancreatography was attempted with a duodenoscope, the scope could not be advanced because of a stricture caused by esophageal cancer. A forward-viewing endoscope was passed through the esophagus, allowing observation up to the duodenum. Employing the forward-viewing endoscope, we performed endoscopic sphincterotomy and endoscopic papillary balloon dilation, followed by stone removal with a balloon. No complications occurred such that we were subsequently able to make a smooth transition to esophageal cancer treatment. Because there are cases in which stones can be removed with a forward-viewing endoscope, its use appears to be a possible therapeutic approach.

Recent advances in endoscopic management of difficult bile duct stones

Abstract:Endoscopic treatment is now recognized worldwide as the first-line treatment for bile duct stones. Endoscopic sphincterotomy combined with basket and/or balloon catheter is generally carried out for stone extraction. However, some stones are refractory to treatment under certain circumstances, necessitating additional/other therapeutic modalities. Large bile duct stones are typically treated by mechanical lithotripsy. However, if this fails, laser or electrohydraulic lithotripsy (EHL) is carried out under the guidance of conventional mother-baby cholangioscopy. More recently, direct cholangioscopy using an ultrathin gastroscope and the newly developed single-use cholangioscope system - the SpyGlass direct visualization system - are also used. In addition, extracorporeal shock wave lithotripsy has also been used for stone fragmentation. Such fragmentation techniques are effective in cases with impacted stones, including Mirizzi syndrome. Most recently, endoscopic papillary large balloon dilationhas been introduced as an easy and effective technique for treating large and multiple stones. In cases of altered anatomy, it is often difficult to reach the papilla; in such cases, a percutaneous transhepatic approach, such as EHL or laser lithotripsy under percutaneous transhepatic cholangioscopy, can be a treatment option. Moreover, enteroscopy has recently been used to reach the papilla. Furthermore, an endoscopic ultrasound-guided procedure has been attempted most recently. In elderly patients and those with very poor general condition, biliary stenting only is sometimes carried out with or without giving subsequent dissolution agents.

Abstract:Objective To explore the therapeutic strategy and clinical value of intraoperative chole-dochoscopy and electrohydraulic lithotripsy for refractory intrahepatic bile duct stones.Methods Liver pa-renchyma,intrahepatic bile duct and bile duct stones were explored under direct vision and intraoperative choledochoscope in 1 1 cases of refractory intrahepatic bile duct stones.Electrohydraulic lithotripsy and lithot-omy were performed to remove the stones and protect the liver parenchyma.If the stones could not be re-moved once,a secondary lithotripsy and lithotomy was performed through the fistula tract.Results All re-fractory calculi were crushed after one or two procedures and the clearance rate were 100%.No complica-tions occurred.Ten patients were followed up from 1 to 3 years except one.Three cases revealed recurrent stone during follow-up due to withdrawal of ursodeoxycholic acid capsules in 1 to 2 years.Seven others showed no stone recurrence within follow-up time.Conclusion Intraoperative choledochoscopy and electro-hydraulic lithotripsy is an easy technique and can effectively protect the liver parenchyma.The life quality of patients can be improved with low surgical risk and postoperative complications.

Abstract:Objective To explore the feasibility and safety of laparoscopic transcystic common bile duct stone extraction and cholecystectomy in pregnant patients with choledocholithiasis.Methods A retrospective analysis of 4 pregnant patients with choledocholithiasis was performed.The clinicoradiologic,perioperative and follow up data were analyzed.Results The 4 patients were admitted for acute cholangitis at their second trimester of pregnancy.Preoperative MRCP demonstrated that the diameter of the common bile duct stone was less than that of the cystic duct.Laparoscopic transcystic common bile duct stone extraction and cholecystectomy were successfully conducted.Plasma shock wave lithotripsy was applied in one patient with impacted gallstone,intraoperative cholangiography was conducted in one patient with suspected residual stone.There was not major post-op complications nor stone recurrence in the follow up postoperatively.They all delivered a healthy baby on the expected date of childbirth.Conclusions Simutaneously laparoscopic transcystic common bile duct stone extraction and cholecystectomy in pregnant patients with choledocholithiasis is feasible and safe.

Abstract:OBJECTIVE:To assess the effects of approach-intraoperative endoscopic sphincterotomy (IOES) for common bile duct (CBD) stones during laparoscopic cholecystectomy (LC).METHODS:Twenty-seven patients with secondary CBD stones were treated by IOES during LC. Therapeutic effects were evaluated on the basis of the cure rate, early complications, and days of hospitalization.RESULTS:IOES was successfully performed in 26 (96.30%) of 27 cases, and their CBD stones were cleared completely. Two cases (7.69%) were complicated by mild acute pancreatitis.CONCLUSION:IOES as an alternative to the treatment of CBD stones during LC is safe and avoids reoperation.

Abstract:Introduction: To investigate the feasibility and safety of laparoscopic hepatectomy combined with choledochoscopy without T-tube drainage in the treatment of intrahepatic and common bile duct stones. Method: From January 2014 to May 2018, 18 patients with hepatolithiasis in the left intrahepatic duct and common bile duct stones underwent laparoscopic hepatectomy combined with choledochoscopy and stone extraction. Of which 12 patients have left lateral heatic calculi, 6 patients have left hepaic calculi, all patients were complicated with common bile duct stones.After laparoscopic hepatectomy ,common bile duct stones were removed under video-assisted choledochoscopy through the left hepatic duct without choledochotomy and T-tube drainage. Result: Twelve patients underwent successfully laparoscopic left lateral lobectomy,six patients underwent successfully laparoscopic left hemihepatectomy, all 18 patients with common bile duct stones were explored by choledochoscope through hepatic cross section.5 patients were used laser lithotripsy due to incarceration of common bile duct stones. The mean operation time was 148 min (range ,108∼212 min), the mean intraoperative blood loss was 160ml (range,80∼320ml).There was one case of bile leakage, which was resolved by conservative treatment.The mean postoperative hospital stay was 5 days(range,3-9 days).Over follow-up of 6 months to 60 months,mean 26 months,no residual or recurrent stone formation were found. Conclusion: Laparoscopic hepatectomy combined with choledochoscopy without T-tube drainage for the treatment of intrahepatic and common bile duct stones is safe and effective in selected patients. This procedure has advantages of reapid rehabilitation and short postoperative hospital stay.
Single-operator holmium laser lithotripsy under direct peroral cholangioscopy using an ultra-slim upper endoscope in a patient with a large stone in the common bile duct

Abstract:In patients with complicated stones (e.g., large stones) in the common bile duct (CBD), advanced treatment modalities are generally needed. Here we present an interesting case of a large CBD stone treated with holmium laser lithotripsy, which was performed under direct peroral cholangioscopy using an ultra-slim endoscope. An 86-year-old woman underwent endoscopic retrograde cholangiopancreatography for a symptomatic large CBD stone. After the failure of sphincterotomy, balloon sphincteroplasty, and mechanical and extracorporeal shock wave lithotripsy, we attempted to fragment the stone by peroral cholangioscopy using the holmium laser system through the operating channel of an ultra-slim endoscope. No complication was observed at the end of this procedure. Thus, holmium laser lithotripsy under direct peroral cholangioscopy performed using an ultra-slim endoscope is a feasible and safe endoscopic method for removing large CBD stones.
Ultrasound-guided double-tract percutaneous cholecystostomy combined with a choledochoscope for performing cholecystolithotomies in high-risk surgical patients

Abstract:BackgroundCholecystolithiasis is the most common disease treated by general surgery, with an incidence of about 0.15–0.22 %. The most common therapies are open cholecystectomy (OC) or laparoscopic cholecystectomy (LC). However, with a greater understanding of the function of the cholecyst, more and more patients and surgeons are aware that preserving the functional cholecyst is important for young patients, as well as patients who would not tolerate anesthesia associated with either OC or LC. Based on these considerations, we have introduced a notable, minimally invasive treatment for cholecystolithotomy.MethodsWe performed a retrospective review of patients with cholecystolithiasis who were unable to tolerate surgery or who insisted on preserving the functional cholecyst. Our particular approach can be simply described as ultrasound-guided percutaneous cholecystostomy combined with a choledochoscope for performing a cholecystolithotomy under local anesthesia.ResultsTen patients with cholecystolithiasis were treated via this approach. All except one patient had their gallbladder stones totally removed under local anesthesia, without the aggressive procedures associated with OC or LC. The maximum number of gallbladder stones removed was 16, and the maximum diameter was 13 mm without lithotripsy. After the minimally invasive surgery, the cholecyst contractile functions of all patients were normal, confirmed via ultrasound after a high-fat diet. Complications such as bile duct injury, biliary fistula, and bleeding occurred significantly less often than with OC and LC. The recurrence rates for each of 2 post-operative years were about 11.11 % (1/9, excluding a failure case) with uncertainty surrounding recurrence or residue, and 22.22 % (2/9, including one non-recurrence patient with follow-up time of 22 months), respectively.ConclusionsUltrasound-guided percutaneous cholecystostomy combined with choledochoscope is a safe, efficient, and minimally invasive cholecystolithotomy method. We recommend this technique for the management of small stones (less than 15 mm) in high-risk surgical patients.

Abstract:Objective To introduce a new method that utilizes a leak-proof stent plus cholangioscopy in the diagnosis and treatment of common bile duct stones.Method The method was used in 78 cases of cholelithiasis,which represented all the cases admitted in our center between January 2003 and March 2005.Results All the gallstones were successfully removed in 67 cases of common bile duct stone.Residual stones of the intrahepatic bile ducts were found in 3 cases postoperatively in 19 cases of hepatolithiasis.The stone clearance ratio was 96.15%(75/78) and abdominal infection occurred in none of the cases.Hepatocholangiocarcinoma was detected by pathology in 3 of the cases postoperatively.Conclusions It is a safe and effective approach by using the leak-proof stent plus cholangioscopy during the operation for common bile duct stones;and it is a new method for stone clearance and early hepatocholangiocarcinoma diagnosis.
Holmium Laser Lithotripsy Combined with Choledochoscope for Postoperative Refractory Stone in the Intrahepatic Bile Duct

Abstract:Objective To discuss the value of holmium laser lithotripsy combined with choledochoscope for the treatment of postoperative refractory stone in the intrahepatic bile duct.Methods The clinical data of 37 patients with postoperative refractory stone in the intrahepatic bile duct,who received holmium laser lithotripsy(0.8-1.2 J/5-10 Hz) combined with choledochoscope from June 2010 to June 2012 were reviewed retrospectively.Results The patients totally received 1-12 times of holmium laser lithotripsy in each,with a mean of 2.6 times.In one patient the stones were not removed completely by choledochoscopy.The other 36 patients were cured by the treatment with the stones being removed completely.The success rate of the procedure was 97.3%(36/37).No bile duct wall burns or perforation occurred during the procedure.The 36 patients,whose stones were removed completely,were followed up for 3 to 25 months with a mean of 12.3 months,during which re-examination by B-ultrasonography revealed no recurrent stones.Conclusion Holmium laser lithotripsy combined with choledochoscopy is safe and effective for choice for postoperative refractory stone in the intrahepatic bile duct.

Abstract:Background: As the use of the robotic platform increases among general surgeons, the amount of robotic cholecystectomies is expected to increase as well. The use of intraoperative cholangiography is valuable in assessing for choledocholithiasis. We describe our technique of performing robotic intraoperative cholangiograms with choledochoscopy. Out technique aids in efficiency since no undocking is required. Methods: Preoperatively, the decision to perform a cholangiogram is made based on physical exam, labs, and imaging findings. The procedure begins with obtaining a critical view of safety. The robotic arms are positioned in a manner that allows all 4 robotic arms to remain docked. A ductotomy is made and the cholangiocatheter is introduced. The cholangiogram images are then interpreted and if a stone is seen in the common bile duct we will then perform a transcystic common bile duct exploration using the SpyGlass Discover digital. A complete cholangiogram is then performed. The cystic duct is secured and the gallbladder is removed from the liver bed. The patients are watched overnight and discharged on postoperative day 1. Conclusions: A robotic approach to performing a transcystic common bile duct exploration is a safe and reproducible treatment method for choledocholithiasis. Our approach offers an advantage since no undocking is required.

Abstract:Therapeutic endoscopic ultrasound has become widespread as an effective procedure for biliary drainage; however, it is rarely used to remove foreign bodies such as a biliary stent. A 57-year-old man was referred to our hospital for a benign biliary stricture in the left hepatic duct after hepatectomy. Initially, a 7-Fr plastic stent was placed in the left hepatic duct with the distal end set above the papilla, and it was replaced with an 8.5-Fr stent as the stricture remained after 3 months. Endoscopic retrograde cholangiopancreatography was performed to retrieve the plastic stent 3 months later; however, the stent could not be moved because the proximal flap was caught in the stricture. Attempts using various devices failed to retrieve the stent; thus, endoscopic ultrasound-guided hepaticogastrostomy was performed to create a route for stent retrieval. Eventually, the plastic stent was successfully retrieved with biopsy forceps through a fully covered self-expandable metallic stent located in a transgastric fistula. We propose our new method involving endoscopic ultrasound-guided hepaticogastrostomy for endoscopic stent retrieval that fails via the transpapillary route.

Laparoscopic Ultrasound-Guided Transcystic Approach for the Treatment of Common Bile Duct Stones

Abstract:Background: The treatment of choledocholithiasis with nondilated common bile duct (CBD) is a challenge for surgeons who often choose endoscopic retrograde cholangiopancreatography with laparoscopic cholecystectomy (LC) staging surgery instead of simultaneous laparoscopic CBD exploration with LC because of the small CBD diameter. This study aims to introduce and assess the clinical applicability of a technique we developed to identify and extract CBD stones using laparoscopic ultrasound (LUS). Methods: We retrospectively reviewed surgical procedures and clinical data of 13 patients who underwent LC and CBD exploration using LUS between May 2022 and August 2023. The cystic duct was used for CBD stone removal. Results: Ten patients were successfully treated; 2 patients with residual stones were treated with ursodeoxycholic acid, whereas 1 patient required a microincision near the CBD and choledochoscopy because of stone incarceration in the duodenal papilla. The CBD diameter was 6 mm (5-9 mm). There were less than three CBD stones, with diameters of 2-6 mm; the median operative time was 105 minutes (range, 52-155 minutes). One patient developed postoperative cholangitis. The median postoperative hospital stay was 6 days (3-8 days). The stone clearance rate was 76.9%, and the CBD stone detection rate was 100%. No intraoperative complications, postoperative bile leakage, and mortality occurred. Conclusions: CBD exploration and transcystic stone extraction under LUS guidance are safe and effective approaches for patients with choledocholithiasis; strict control over surgical indications is necessary. This study could provide new strategies for effectively treating choledocholithiasis.

Abstract:ERCP is the standard treatment for common bile duct stones (CBD stones). On the other hand, in approximately 10% of patients with CBD stones, the complete removal of the stones cannot be achieved by conventional ERCP, which performs endoscopic sphincterotomy followed by stone extraction. Additional advanced and complex procedures are often necessary to remove these "difficult bile duct stones", including endoscopic papillary large balloon dilatation or mechanical lithotripsy. Advances in cholangioscopy have made electrohydraulic or laser lithotripsy under direct endoscopic visualization possible during ERCP.
