Abstract:The purpose of this study was to examine force transmission from one of the major multiarticular muscles of the finger, flexor digitorum profundus (FDP), to the index finger. Specifically, we examined whether the popular moment arm (MA)-joint torque technique of modeling muscle force transmission can accurately represent the effects of the FDP on finger movement. A dynamic finger model employing geometric MA values (model I) was compared with another model including realistic tendon force transformation mechanisms via pulley structures and joint reaction forces (model II). Finger flexion movements generated by these models were compared with those obtained from in vivo stimulation experiments. The model with the force transformation mechanisms (model II) resulted in more realistic joint spatial coordination (i.e., proximal interphalangeal > metacarpophalangeal > or = distal interphalangeal) than the MA-based model (model I) in relation to the movement patterns evoked by stimulation. Also, the importance of the pulley structures and passive joint characteristics was confirmed in the model simulation; altering/eliminating these components significantly changed the spatial coordination of the joint angles during the resulting movements. The results of this study emphasize the functional importance of the force transformation through various biomechanical components, and suggest the importance of including these components when investigating finger motor control, such as for examining injury mechanisms or designing rehabilitation protocols.
Identification and validation of the dynamic model of a tendon-driven anthropomorphic finger

Abstract:This study addresses the absence of an identification framework to quantify a comprehensive dynamic model of human and anthropomorphic tendon-driven fingers, which is necessary to investigate the physiological properties of human fingers and improve the control of robotic hands. First, a generalized dynamic model was formulated, which takes into account the inherent properties of such a mechanical system. This includes rigid-body dynamics, coupling matrix, joint viscoelasticity, and tendon friction. Then, we propose a methodology comprising a series of experiments, for step-wise identification and validation of this dynamic model. Moreover, an experimental setup was designed and constructed that features actuation modules and peripheral sensors to facilitate the identification process. To verify the proposed methodology, a 3D-printed robotic finger based on the index finger design of the Dexmart hand was developed, and the proposed experiments were executed to identify and validate its dynamic model. This study could be extended to explore the identification of cadaver hands, aiming for a consistent dataset from a single cadaver specimen to improve the development of musculoskeletal hand models.

Abstract:Thumb carpometacarpal joint space changes when the surrounding soft tissues including the capsule, ligaments, and tendons are stretched or pulled away. When at rest, joint forces originate from passive contraction of muscles and the involvement of joint capsule and ligaments. Previous biomechanical models of hand and finger joints have mostly focused on the assessment of joint properties when muscles were active. This study aims to present an experimental-numerical biomechanical model of thumb carpometacarpal joint to explore the contribution of tendons, ligaments, and other soft tissues in the passive forces during distraction. Five fresh cadaveric specimens were tested using a distractor device to measure the applied forces upon gradual distraction of the intact joint. The subsequent step involved inserting a minuscule sensor into the joint capsule through a small incision, while preserving the integrity of tendons and ligaments, in order to accurately measure the fundamental intra-articular forces. A numerical model was also used to calculate the passive forces of tendons and ligaments. Before the separation of bones, the forces exerted by tendons and ligaments were relatively small compared to the capsule force, which accounted for approximately 92% of the total applied force. Contribution of tendons and ligaments, however, increased by further distraction. The passive force contribution by tendons at 2-mm distraction was determined less than 11%, whereas it reached up to 74% for the ligaments. The present study demonstrated that the ligament-capsule complex plays significant contribution in passive forces of thumb carpometacarpal joint during distraction.

Estimating in Vivo Passive Forces of the Index Finger Muscles: Exploring Model Parameters.

Abstract:We compared predicted passive finger joint torques from a biomechanical model that includes the exponential passive muscle force-length relationship documented in the literature with finger joint torques estimated from measures in ten adult volunteers. The estimated finger joint torques were calculated from measured right index fingertip force, joint postures, and anthropometry across 18 finger and wrist postures with the forearm muscles relaxed. The biomechanical model predicting passive finger joint torques included three extrinsic and three intrinsic finger muscles. The values for the predicted passive joint torques were much larger than the values calculated from the fingertip force and posture measures with an average RMS error of 7.6Ncm. Sensitivity analysis indicated that the predicted joint torques were most sensitive to passive force-length model parameters compared to anthropometric and postural parameters. Using Monte Carlo simulation, we determined a new set of values for the passive force-length model parameters that reduced the differences between the joint torques calculated from the two methods to an average RMS value of 0.5Ncm, a 94% average improvement of error from the torques predicted using the existing data. These new parameter values did vary across individuals; however, using an average set for the parameter values across subjects still reduced the average RMS difference to 0.8Ncm. These new parameters may improve dynamic modeling of the finger during sub-maximal force activities and are based on in vivo data rather than traditional in vitro data.
Effectiveness of a novel finger range-of-motion brace for extensor tendon injury: a report of 10 patients

Abstract:Background: Extensor tendon injuries require surgical repair, followed by rehabilitation to ensure optimal outcomes. Immobilization has been the cornerstone of postoperative management. However, immobilization after surgery frequently makes the finger stiffness, often resulting in reduced functionality and quality of life for patients. Recent studies indicate that early controlled motion can significantly improve outcomes, but safe early range of motion (ROM) exercise is a significant clinical challenge. This article aims to check the efficacy of the novel designed finger ROM brace for preventing finger stiffness for extensor tendon injuries with case series. Methods: A finger ROM brace was designed based on the natural finger movement. Like a real finger, there are two tiny hinge joints and three round-shape body components. The design aimed to be ergonomic dynamic splint assisting controlled motion to promote early motion, thus reducing tendon tension and preventing stiffness. Elastic resistant ROM exercise could be by inserting a silicone band into the groove on the components and free movement could be achieved by removing a silicone band. Result: Between December 2022 and July 2023, 10 patients who underwent tenorrhaphy because of extensor tendon laceration were involved. Complete extensor tendon laceration was 3 patients, other seven patients had partial laceration of extensor tendons. Surgery was performed within 2 days of injury, and no infection was observed in all patients. After the extensor tendon was confirmed as healed state by ultrasound, the patients were permit the active exercise wearing finger ROM brace with a silicone band. Within 1-2 weeks after elastic resistant exercise, the patients could achieve free full ROM movement without any complication. Conclusion: The novel finger ROM brace combines the advantages of dynamic splinting and under-actuated mechanisms to offer a comprehensive solution for preventing stiffness after extensor tendon suture. Future studies should focus on clinical trials to validate the efficacy and safety of this brace in a larger population.
Estimation of joint contact pressure in the index finger using a hybrid finite element musculoskeletal approach

Abstract:The knowledge of local stress distribution in hand joints is crucial to understand injuries and osteoarthritis occurrence. However, determining cartilage contact stresses remains a challenge, requiring numerical models including both accurate anatomical components and realistic tendon force actuation. Contact forces in finger joints have frequently been calculated but little data is available on joint contact pressures. This study aimed to develop and assess a hybrid biomechanical model of the index finger to estimate in-vivo joint contact pressure during a static maximal strength pinch grip task. A finite element model including bones, cartilage, tendons, and ligaments was developed, with tendon force transmission based on a tendon-pulley system. This model was driven by realistic tendon forces estimated from a musculoskeletal model and motion capture data for six subjects. The hybrid model outputs agreed well with the experimental measurement of fingertip forces and literature data on the physiological distribution of tendon forces through the index finger. Mean contact pressures were 6.9 ± 2.7 MPa, 6.2 ± 1.0 MPa and 7.2 ± 1.3 MPa for distal, proximal interphalangeal and metacarpophalangeal joints, respectively. Two subjects had higher mean contact pressure in the distal joint than in the other two joints, suggesting a mechanical cause for the prevalence of osteoarthritis in the index distal joint. The inter-subject variability in joint contact pressure could be explained by different neuromuscular strategies employed for the task. This first application of an effective hybrid model to the index finger is promising for estimating hand joint stresses under daily grip tasks and simulating surgical procedures.

Abstract:Precise sustained force control of the fingers is important for achieving flexible hand movements. However, how neuromuscular compartments within a forearm multi-tendon muscle cooperate to achieve constant finger force remains unclear. This study aimed to investigate the coordination strategies across multiple compartments of the extensor digitorum communis (EDC) during index finger sustained constant extension. Nine subjects performed index finger extensions of 15%, 30%, and 45% maximal voluntary contractions, respectively. High-density surface electromyography signals were recorded from the EDC and then analyzed using non-negative matrix decomposition to extract activation patterns and coefficient curves of EDC compartments. The results showed two activation patterns with stable structures during all tasks: one pattern corresponding to the index finger compartment was named master pattern; whereas the other corresponding to other compartments was named auxiliary pattern. Further, the intensity and stability of their coefficient curves were assessed using the root mean square value (RMS) and coefficient of variation (CV). The RMS and CV values of the master pattern increased and decreased with time, respectively, while the corresponding values of the auxiliary pattern were both negatively correlated with the formers. These findings suggested a special coordination strategy across EDC compartments during index finger constant extension, manifesting as two compensations of the auxiliary pattern for the intensity and stability of the master pattern. The proposed method provides new insight into the synergy strategy across multiple compartments within a forearm multi-tendon during sustained isometric contraction of a single finger and a new approach for constant force control of prosthetic hands.
Pre- and post-treatment in flexor tendon tenolysis: An observational study

Abstract:Background Glide deficit of the distal flexors' tendons following primary repair in zone 1–3 are very common. Adhesions of tendons have multi factorial origins and are closely related to the healing of the affected tissues. The surgical practice used to resolve these complications is tenolysis. Purpose The purpose of this study was to identify and compare the Visual Analog Scale (VAS) relate to pain and Total Active Motion (TAM) of adult patients of both sexes undergoing tenolysis surgery. The results will then be compared to existing research to confirm their significance. Study Design Case-series. Methods Retrospective data for TAM and pain VAS were extracted from the medical records for 63 patients (73 fingers) who underwent flexor tenolysis between 2017 and 2019. Data were compared pre-operatively and 3 months after surgery. All patients underwent pre- and post-surgery therapy by hand therapists. Results The sample presented very encouraging improvements, except in the VAS and active range of motion (AROM) of thumb where some patients maintained the same assessment. The fingers reported statistically significant results, whereas the thumb group did not meet significant criteria. Overall, TAM improved from 134.6° to 196.7 and VAS decreased from 2.7 to 1.2. Discussion According to the results and the data change between pre- and post-treatment, the sample demonstrated improvements in all areas examined, reporting statistically significant results for the fingers with an improvement of TAM of 62.1° with a percentage value (%TAM) of 75.6%. Conclusions A specific treatment for this type of surgery is required for the patients so they can return to their daily and working activities. This article can be used as a starting point for further studies.

Abstract:The problem of modelling stresses incurred at the finger joints is critical to the design of durable joint replacements in the hand. The goal of this study was to characterise the forces and stresses at the finger and thumb joints occurring during activities such as typing at a keyboard, playing piano, gripping a pen, carrying a weight and opening a jar. The metacarpal and proximal phalanx were modelled using a COMSOL-based finite element analysis. Analysis of these activities indicates that joint forces in excess of 100 N may be common at the metacarpophalangeal joint (MCP) due to carrying objects such as groceries or while opening jars. The model predicted that stresses in excess of 2 MPa, similar to stresses at the hip, occur at the MCP with the properties of cancellous bone playing a significant role in the magnitude and distribution of stress.

A Probabilistic Finger Biodynamic Model Better Depicts the Roles of the Flexors During Unloaded Flexion.

Abstract:Previous deterministic finger biomechanical models predicted that the flexor digitorum superficialis (FDS) was silent and the flexor digitorum profundus (FDP) was the only active flexor during finger flexion. Experimental studies in vivo, however, recorded activities of both flexors. In this study, in an attempt to elucidate the roles of the flexors, a probabilistic biodynamic model of the index finger was constructed to estimate the muscle-tendon forces during an experimentally measured index finger flexion movement. A Monte-Carlo simulation was performed with four model parameters, including moment arms, physiological cross sectional areas (PCSA), passive torques, and anthropometric measures as independent random variables. The muscle-tendon forces at each time point were determined using a nonlinear optimization technique. The model predicted that both FDS and FDP contributed to sustaining the movement and the FDS was not necessarily silent. The two distinct force patterns observed in vivo in experimental studies were also corroborated by the simulation. These findings, contrary to previous deterministic models' predictions but in agreement with experimental measurements, explained the observed coactivation of FDS and FDP, and resolved the controversy regarding the roles of the flexors in finger movement dynamics.
Influence of Force-Length Relationship and Task-Specific Constraints on Finger Force-Generating Capacities

Abstract:Grip strength loss in extended and flexed wrist postures has been explained by reduced force-generating capacities of extrinsic finger flexor resulting from non-optimal length, owing to the force-length relationship. Recent works suggested that other muscles, especially wrist extensors, participate in this grip strength loss. The objective of this study was to clarify the role of the force-length relationship in finger force production. 18 participants performed maximal isometric finger force production during pinch grip (Pinch) and four-finger pressing (Press) tasks in four different wrist postures (extended, flexed, neutral, spontaneous). The maximum finger force (MFF), finger and wrist joint angles, as well as activation of four muscles were determined using dynamometry, motion capture, and electromyography. The force and length of the four muscles were estimated from joint angles and muscle activation using a musculoskeletal model. MFF decreased for flexed wrist during Pinch but remained stable across wrist postures during Press. The results suggested that the loss of pinch grip force in deviated wrist posture is partially related to force-length relationship of finger extensors. In opposition, MFF during Press was not influenced by the modulation of muscle capacities but was probably first limited by mechanical and neural factors related to finger interdependence

Three-dimensional finite element simulations of the mechanical response of the fingertip to static and dynamic compressions

Abstract:The analysis of the mechanics of the contact interactions of fingers/handle and the stress/strain distributions in the soft tissues in the fingertip is essential to optimize design of tools to reduce many occupation-related hand disorders. In the present study, a three-dimensional (3D) finite element (FE) model for the fingertip is proposed to simulate the nonlinear and time-dependent responses of a fingertip to static and dynamic loadings. The proposed FE model incorporates the essential anatomical structures of a finger: skin layers (outer and inner skins), subcutaneous tissue, bone and nail. The soft tissues (inner skin and subcutaneous tissue) are considered to be nonlinearly viscoelastic, while the hard tissues (outer skin, bone and nail) are considered to be linearly elastic. The proposed model has been used to simulate two loading scenarios: (a) the contact interactions between the fingertip and a flat surface and (b) the indentation of the fingerpad via a sharp wedge. For case (a), the predicted force/displacement relationships and time-dependent force responses are compared with the published experimental data; for case (b), the skin surface deflection profiles were predicted and compared with the published experimental observations. Furthermore, for both cases, the time-dependent stress/strain distributions within the tissues of the fingertip were calculated. The good agreement between the model predictions and the experimental observations indicates that the present model is capable of predicting realistic time-dependent force/displacement responses and stress/strain distributions in the soft tissues for dynamic loading conditions.

Abstract:Objective: Managing flexor tendon injuries surgically remains challenging due to the ongoing debate over the most effective suture technique and materials. An optimal repair must be technically feasible while providing enough strength to allow for early active mobilization during the post-operative phase. This study aimed to assess the biomechanical properties of three modified Kessler repair techniques using two different suture materials: a conventional two-strand and a modified four-strand Kirchmayr–Kessler repair using 3-0 Prolene® (2s-KK-P and 4s-KK-P respectively), and a four-strand Kessler–Tsuge repair using 4-0 FiberLoop® (4s-KT-FL). Methods: Human flexor digitorum profundus (FDP) tendons were retrieved from Thiel-embalmed prosections. For each tendon, a full-thickness cross-sectional incision was created, and the ends were reattached using either a 2s-KK-P (n = 30), a 4s-KK-P (n = 30), or a 4s-KT-FL repair (n = 30). The repaired tendons were tested using either a quasi-static (n = 45) or cyclic testing protocol (n = 45). Maximum force (Fmax), 2 mm gap force (F2mm), and primary failure modes were recorded. Results: In both quasi-static and cyclic testing groups, tendons repaired using the 4s-KT-FL approach exhibited higher Fmax and F2mm values compared to the 2s-KK-P or 4s-KK-P repairs. Fmax was significantly higher with a 4s-KK-P versus 2s-KK-P repair, but there was no significant difference in F2mm. Suture pull-out was the main failure mode for the 4s-KT-FL repair, while suture breakage was the primary failure mode in 2s- and 4s-KK-P repairs. Conclusions: FDP tendons repaired using the 4s-KT-FL approach demonstrated superior biomechanical performance compared to 2s- and 4s-KK-P repairs, suggesting that the 4s-KT-FL tendon repair could potentially reduce the risk of gapping or re-rupture during early active mobilization.

Abstract:Purpose: This study aimed to compare the outcome in terms of range of motion between early active flexion and extension (early active motion, [EAM]) and passive flexion using rubber bands followed by active extension (sometimes referred to as a Kleinert regimen) after flexor tendon repair in zones 1 and 2. Methods: Data were collected from the Swedish national health care registry for hand surgery (HAKIR). Rehabilitation regimens were decided by the preference of each caregiver. At 3 months, 828 digits (656 EAM and 172 passive flexion) and at 12 months, 448 digits (373 EAM and 75 passive flexion) were available for analysis. Thumbs were analyzed separately. Results: No notable difference in total active motion was found between the groups at 12 months of follow-up. Conclusions: This large registry study supports the hypothesis that EAM rehabilitation may not lead to better range of motion long-term than passive motion protocols. Abstract:The joints of the fingers play an important role in prehension. They must accomplish both great mobility in the sagittal plane to allow the fingers to roll up and great stability to ensure the grip is both precise and firm. The collateral ligaments and palmar plates are the main passive stabilizing structures between the interphalangeal (IP) and metacarpophalangeal (MCP) joints. Sprains with or without dislocation of the proximal IP joints of the fingers are common injuries in sports pathology and may involve not only the ligaments but also their bony insertion on the phalanges or extensor tendons. Certain entities are specific to the MCP joints: ligament ruptures with Stener-like effect under the sagittal bands and MCP flexion locking (locked finger). An investigation of tendon strains in jersey finger injury load cases using a finite element neuromuscular human body model

Abstract:Introduction: A common hand injury in American football, rugby and basketball is the so-called jersey finger injury (JFI), in which an eccentric overextension of the distal interphalangeal joint leads to an avulsion of the connected musculus flexor digitorum profundus (FDP) tendon. In the field of automotive safety assessment, finite element (FE) neuromuscular human body models (NHBMs) have been validated and are employed to evaluate different injury types related to car crash scenarios. The goal of this study is to show, how such a model can be modified to assess JFIs by adapting the hand of an FE-NHBM for the computational analysis of tendon strains during a generalized JFI load case. Methods: A jersey finger injury criterion (JFIC) covering the injury mechanisms of tendon straining and avulsion was defined based on biomechanical experiments found in the literature. The hand of the Total Human Model for Safety (THUMS) version 3.0 was combined with the musculature of THUMS version 5.03 to create a model with appropriate finger mobility. Muscle routing paths of FDP and musculus flexor digitorum superficialis (FDS) as well as tendon material parameters were optimized using literature data. A simplified JFI load case was simulated as the gripping of a cylindrical rod with finger flexor activation levels between 0% and 100%, which was then retracted with the velocity of a sprinting college football player to forcefully open the closed hand. Results: The optimization of the muscle routing node positions and tendon material parameters yielded good results with minimum normalized mean absolute error values of 0.79% and 7.16% respectively. Tendon avulsion injuries were detected in the middle and little finger for muscle activation levels of 80% and above, while no tendon or muscle strain injuries of any kind occurred. Discussion: The presented work outlines the steps necessary to adapt the hand model of a FE-NHBM for the assessment of JFIs using a newly defined injury criterion called the JFIC. The injury assessment results are in good agreement with documented JFI symptoms. At the same time, the need to rethink commonly asserted paradigms concerning the choice of muscle material parameters is highlighted.
