Abstract:Background: The Chinese government officially announced the universal two-child policy in 2015 and three-child policy in 2021. The launching of the new birth policy has brought issues of shortages of midwifery workforce and imbalances in maternal and newborn care which may lead to adverse childbirth outcomes and negative experiences for women.Objective: The study aimed to investigate the status of midwifery workforce and childbirth services in the context of China’s new birth policy, and to identify the association between midwifery staffing and childbirth outcomes.Study design: A descriptive, multi-center cross-sectional survey was conducted in China from July to December 2021. The national baby-friendly hospitals from the eastern, central and western regions of China were selected via stratified sampling. Descriptive statistics were used to analyze the characteristics of midwifery workforce and childbirth services. A linear regression model was conducted to analyze the association between midwifery staffing and childbirth outcomes.Results: A total of 180 baby-friendly hospitals were selected and investigated, staffed with 4,159 midwives, 412 obstetric nurses and 1,007 obstetricians at the labour and delivery units. The majority of midwives (69.06%) were aged under 36 years old, with undergraduate degree (70.79%), junior professional titles (62.18%) and less than 10 years of working experience (61.19%). The average efficiency index of midwifery services was 264 deliveries per midwife in China (318 in the eastern region, 112 in the central region, and 283 in the western region). The overall childbirth intervention rates of the 180 hospitals were 44.90% (923,062/2,055,692) in cesarean delivery, 6.73% (81,776/1,214,406) in intrapartum cesarean section, and 22.10% (242,619/1,097,964) in episiotomy. Of the 180 hospitals, 138 (76.66%) provided continuous labour support, 108 (60.00%) had midwives’ antenatal clinic service, and 125 (69.44%) during the second stage of labour. Midwifery staffing was significantly related to two childbirth interventions with increased midwifery staffing indicating reduced rates of instrumental vaginal delivery (adjusted β -0.032, 95% CI -0.115~-0.012, P<0.05) and episiotomy (adjusted β -0.171, 95% CI -0.190~-0.056, P<0.01).Conclusions: The educational levels and professional qualifications of Chinese midwives need to be further improved under China's new birth policy. The rates of childbirth interventions including overall cesarean section rate in China and the episiotomy rate especially in the central region still remain relatively high. Improved midwifery staffing was associated with reduced rates of instrumental vaginal delivery and episiotomy, indicating that further investments in midwifery workforce could produce better childbirth outcomes.
