Abstract:Thank you for the opportunity to respond to insightful commentary on our systematic review ( Reference Poulter, Coe and Graham 1) on dietary interventions and the behaviour of prisoners. It is gratifying to see that our review is generating interest soon after publication. We are aware of the ground-breaking seminal work produced by Schoenthaler and colleagues and are grateful for the time taken to respond to our review. We agree that research in this field within this population group is often overlooked and that further discussion around this subject matter is important. We note the concern raised regarding our exclusion criteria, specifically our decision to include only studies with a control or the comparator group. Our intention was to ensure methodological rigour and consistency, aiming to synthesise evidence from studies with clearly defined control measures. This approach was chosen to enhance the reliability and validity of the findings by reducing potential biases and confounding factors. This did therefore lead to the exclusion of studies referenced in your letter, which involve quasi-experimental time-series designs, but we acknowledge the value of these formative studies to offer insights into the potential effects of dietary interventions on behaviour within correctional facilities. Moreover, the transformation of dietary policies in the Maine carceral system highlights significant research opportunities for investigating the impact of dietary interventions on behaviour and mental health. We agree that such investigations could contribute to multidisciplinary research efforts and facilitate a deeper understanding of the relationship between diet and prisoner health. Indeed our research group has recently received a UKRI KTP award to investigate the impact of a nutrition educational programme on the diet of UK prisoners, with important implications for both health and policy change (2) . We appreciate the opportunity to engage in constructive dialogue and remain committed to advancing the science of diet, behaviour and mental health in incarcerated population groups through rigorous research and collaboration, and your letter highlights further avenues for future exploration. This research has in part been supported through the Nigel Groome Scholarship for PhD candidates at Oxford Brookes University. A Systematic Review of the effect of Dietary and Nutritional Interventions on the Behaviours and Mental Health of Prisoners

Abstract:Prisoners experience a higher burden of poor health, aggressive behaviours and worsening mental health than the general population. This systematic review aimed to identify research that used nutrition-based interventions in prisons, focusing on outcomes of mental health and behaviours. The systematic review was registered with PROSPERO on the 26th of January 2022: CRD42022293370. Inclusion criteria comprised of current prisoners with no limit on time, location, age, sex, or ethnicity. Only quantitative research in the English language was included. PubMed/Medline, Web of Science, EMBASE, PsycINFO, and CINAHL were searched, retrieving 933 results, with 11 included for qualitative synthesis. Studies were checked for quality using the ROB 2 or ROBINS-I tool. Of the included studies, seven used nutritional supplements, three included diet changes, and one used education. Of the seven supplement-based studies, six included rule violations as an outcome, and only three demonstrated significant improvements. One study included mental health as an outcome, however, results did not reach significance. Of the three diet change studies, two investigated cognitive function as an outcome, with both reaching significance. Anxiety was included in one diet change study, which found a significant improvement through consuming oily fish. One study using diet education did not find a significant improvement in overall mental resilience. Overall, results are mixed, with the included studies presenting several limitations and heterogeneity. Future research should aim to consider increased homogeneity in research design, allowing for a higher quality of evidence to assess the role nutrition can play in improving the health of prisoners.

The Nutritional Content of Food in Carceral Institutions: A Systematic Review of Quantitative Studies

Abstract:The nutritional content of food in carceral institutions is important because it influences weight gain and health during incarceration. This systematic review assessed the available quantitative data and nutritional analyses of food in carceral institutions. Methodology is based on the Preferred Reporting Items for Systematic Reviews and Meta-Analyses statement with four phases (identification, screening, eligibility, and inclusion). Nine articles were selected. Most carceral institutions provided adequate nutrition for micronutrients, except for vitamin D (inadequate) and sodium (excessive). Most menus followed recommendations for macronutrients. Food from the commissary stores is high in calories, sugar, fat, and sodium. Most menus are adequate according to Dietary Reference Intakes. Changing menus drastically to meet nutritional targets may lead to consuming more food from commissary, potentially leading to poorer eating. For some menus, minor adjustments could bring the nutritional content closer to recommendations.

Abstract:In the ongoing series of interviews, Challenges Advisory Board member and Nova Institute for Health Fellow Alan C. Logan meets with thought leaders, scientists, scholars, healthcare professionals, artisans, and visionaries concerned about health at the scales of persons, places, and the planet. Here, Dr Stephen J. Schoenthaler of California State University, Stanislaus, responds to a set of questions posed by Challenges. For over forty years, Dr. Schoenthaler has been at the forefront of the research connecting nutrition to behavior and mental health. In particular, Dr. Schoenthaler’s work has examined relationships between dietary patterns, nutritional support, and behaviors that might otherwise be associated with criminality and aggression. Although the idea that nutrition is a factor in juvenile delinquency was popularized in the 1950s, the area received little scientific attention. In the 1970s, the idea that nutrition could influence behavior gained national attention in the US but was largely dismissed as “fringe”, especially by those connected to the ultra-processed food industries. Today, relationships between diet and behavior are part of the robust field called “nutritional psychiatry”; emerging studies demonstrate clear societal implications, including those within the criminal justice system. Here, Dr. Schoenthaler discusses how we got here and updates Challenges on where the field has moved, with an eye toward future possibilities. Dr. Schoenthaler reflects on the early influences that shaped his interest in the field and discusses the ways in which this research, especially in the context of criminal justice, is related to the many interconnected challenges of our time.
Healthy prison cultures: factors influencing male prisoners' engagement in exercise and subsequent healthy behaviours

Abstract:Purpose: The purpose of this paper is to identify the factors which influence male prisoners' motivation for, and engagement in, exercise and subsequent healthy behaviours. Design/methodology/approach: The first authors conducted 20 semi-structured interviews with male prisoners inside an English medium-security male prison. Interviews were recorded and transcribed, themes were identified using thematic analysis and a critical realist perspective applied to understand objective processes behind prisoners' experiences and shared meanings of exercise and engaging in healthy behaviours in prison. Findings: Emerging themes indicate that in the context of healthy behaviours male prisoners aspired to a masculine ideal that was characterised by a culture of either adaptive behaviours, or maladaptive behaviours. The former fostered an adaptive exercise culture which promoted psychological well-being through an autonomy-supportive environment, consequently internalising motivation and minimising perceived barriers to engaging in healthy behaviours. Conversely, a culture of maladaptive behaviours fostered a maladaptive exercise culture which led to negative psychological well-being, underpinned by external forms of motivation which emphasised barriers to engaging in healthy behaviours. Practical implications: Findings emphasise the need for prisons to promote an internal perceived locus of control for male prisoners when engaging in healthy behaviours. Originality/value: The authors adopt a rare interdisciplinary approach combining a psychological theory of motivation and criminological perspectives of prison culture to understand how best to minimise the impact of prisons as an institution on the psychological well-being of male prisoners.

Abstract:Recently, there has been increased scientific and clinical interest in the potential harms associated with ultra-processed foods, including poor mental health, aggression, and antisocial behavior. Research spanning epidemiology, mechanistic pre-clinical work, addiction science, microbiome and exposome science, and human intervention trials has underscored that nutrition is of relevance along the criminal justice continuum. As such, the emerging dietetics research is salient to the thousands of international psychologists and allied mental health professionals that are engaged in justice work, including forensics, prevention, and intervention. In addition, relationships between nutrition and behavior relate to “food crime”, an emergent area unifying criminal justice researchers with psychology, public health, and other interdisciplinary sectors. Food crime scrutinizes the vast harms, including non-communicable diseases and adverse behavioral outcomes, as influenced by the distribution of addictive ultra-processed food products. Here, we examine the emergent research, including biophysiological mechanisms, and evidence indicating that dietary patterns/components intersect with psychosocial vulnerabilities linked with risks of antisocial behavior and justice involvement. Viewed through a prevention lens, the study of nutrition and aggressive behavior should be prioritized, especially if the outcomes emerge as externalities of the global consumption of ultra-processed food. In the context of criminal justice and behavior, there is a need for forensic examination of how industry influence and power structures can undermine matters of food justice.
Nutritional interventions in prison settings: a scoping review

Abstract:Mounting evidence has shown that incarceration can affect the health and well-being of individuals and increase the risk of noncommunicable diseases (NCDs). Diet quality is known to be one of the main determinants of risk of NCDs, and dietary changes are the first approach used in primary care to reduce the incidence of NCDs. This scoping review aimed to summarize the evidence for (1) the diet quality of inmates, and (2) the effect of nutritional intervention in prison systems. In addition, we aimed to describe limitations in the current literature and to suggest potential future research areas. A systematic search was performed in 2 databases (PubMed and Web of Science) using predefined search terms and covering the period May 2023 to June 2023. Additionally, reference lists from the retrieved studies were hand-searched to identify any additional relevant publications. The identified literature was screened based on defined search strategies, criteria, and research questions defined using the PICo (population or problem, interest, and context) framework. The review was conducted referring to the PRISMA-ScR and the PICo framework. A total of 19 studies out of 63 initially identified records were included in this review (11 cross-sectional evaluations and 9 intervention-based studies). In almost all studies, assessment of the diet quality of menus showed the menus to be nutritionally adequate, except for having a higher-than-recommended intake of total energy, saturated fatty acids, sodium, cholesterol, and sugar. In addition, some studies reported a lower-than-recommended intake of fiber, magnesium, potassium, vitamins D, E, and A, and omega-3 fatty acids. Nutritional interventions were mainly planned in the form of workshops, seminars, and written material to deliver information on healthy dietary choices. Although no significant changes in inmates' dietary choices were observed in any of the studies, a high participation rate was detected. Inmates might require additional prevention intervention to reduce their susceptibility to cardiometabolic diseases by virtue of their isolation from community facilities. Interventions should be tailored to the characteristics of prison settings and inmates to increase adherence to nutritional recommendations.

Abstract:Introduction: Studies on food services are increasing actually in Spain. However, there still is very little information on how this service is organized in prisons, and even less about how it is perceived by its residents. Objectives: To analyze the food service and menu in the Modelo Prison in Barcelona, and confront it with the perception of prisoners. Methods: Semi-structured open interview with an official of the Division of Prisons, participant observation in the dining room and other spaces by one of the study researchers, and a specifically designed questionnaire adapted to this kind of institution. Nutrition and menu quality assessment was performed using the DIAL program and healthy eating index (IAS). Results: The supplied menus usually contain an excess of fat (41.3%) and carbohydrate deficit (41.7 %) even if is acceptable under IAS score (58.4 points). 75% of residents uses the dining room for daily main meals, spending less than 15 minutes on average per meal. The space is considered very noisy. The portions are considered adequate, but the taste, quality and service of food are negatively valued. Discussion and conclusions: Some gaps between institutional proposals and everyday practices and perceptions of users are clearly denoted. Some changes in food and dishes served in the menus -such as reducing meat and increasing consumption of legumes- could contribute to improve nutrition, perception and final cost of the menu. A greater variety of food and more possibility of choice in the dining room and in the shop could be also positive. A strategic reorganization of the use of time and space in the dining room that would reduce the feeling of discomfort and noise could contribute to a better and more enjoyable use of it, while contributing to a better perception of food and welfare in general.

Abstract:Criminal Justice and Behavior, Ahead of Print. The use of extended solitary confinement (ESC) has received domestic and international condemnation for its potential effects on the mental health of incarcerated persons. Despite the criticism, prison systems continue to rely on the practice. To advance understanding of why the housing is used and how correctional personnel view it, this study develops a theoretical model, based on concepts of human agency, empathy, and punitiveness, to examine perceptions of ESC and its effects on the mental health of incarcerated persons. The study examines survey responses from prison personnel who have worked in ESC and finds that personnel hold mixed views about ESC's harms. Structural equation modeling indicates that perceptions about agency and empathy influence views about ESC and whether it harms the mental health of incarcerated persons in the housing. These relationships are mediated by punitiveness and point to nuanced views that personnel hold about ESC's impacts.

Nutritional Characteristics of Menus in State Prisons

Abstract:Nutrition-related health disparities plague prisons in the United States. Unregulated and inadequate prison menus may contribute to noncommunicable chronic health conditions in this vulnerable population. The purpose of this research was to assess nutrition offerings provided by prison menus. Researchers requested the most current version of all master menus and associated nutrition analyses for gendered and age-centric (pediatric vs geriatric) menus. Menu and nutrient data were extracted and entered into a spreadsheet for analysis. Prisons serve gendered or nongendered menus to the general population, and 52.9% of prisons offer nongendered menus where males and females receive the same meals. This approach provides excess calories and saturated fat to females. Sodium was served in excess to both males and females. Fruit and vegetable servings on all gendered menus fell short of recommendations. The average prison menu inappropriately offers calories, sodium, and fruit and vegetable servings in a one-size-fits-all menu development method without considering gender, age, and physical activity. Interpretation and application of Dietary Guidelines for Americans are inconsistent. Nutrition guidelines recommending appropriate nutrients and food groups should be developed and available to corrections systems and dietitians. Current prison menu development practices yield inappropriate nutrition for prison populations.

The Rise, the Fall, and the Reinvention of the Prison Ethnography

Abstract:This chapter traces the emergence, maturation, and subsequent decline of ethnographic studies of prisons and jails in the United States. It provides a summary and overview of classic and contemporary prison ethnographies and identifies key issues and themes that animate qualitative research on prisons and carceral facilities. These include questions about the forms that punishment, surveillance, and control take, the ways that incarcerated men and women experience, resist, and make sense of the conditions of confinement, and the impact incarceration has for their relationships with families, communities, and one another. The chapter considers the dramatic reduction in the number of ethnographic studies of prisons and jails at century’s end and identifies how punitive policies associated with mass incarceration made it all but impossible for ethnographers to gain entry to carceral institutions. Contemporary ethnographers have reinvented the form by documenting practices and ideologies of control in alternate carceral spaces including visiting rooms, drug treatment programs, and group homes. A summary of recent work is included, along with a review of the ways that contemporary ethnographers foreground issues related to race and gender inequality. The chapter concludes with a discussion of prison ethnography in Europe where ethnographers enjoy greater access to carceral facilities and have considerable influence over public policy. For comparative purposes, I include a summary of ethnographic research from Ireland, the United Kingdom, and France.
"I was reaching out for help and they did not help me": Mental healthcare in the carceral state

Abstract:Background: Despite the limitations the carceral environment may impose on mental wellness, mental healthcare is increasingly becoming a carceral endeavor. Over the course of the last several decades, prisons and jails have become the de facto mental healthcare provider for thousands of incarcerated individuals. Furthermore, practices like mandated mental healthcare for supervised individuals further broaden the population experiencing mental healthcare within the criminal legal system at large. This study examines the perspectives of nine individuals who experienced mental healthcare within the carceral state, whether in prison or on parole or probation, with a special focus on how attributes of the carceral state create ideological and functional barriers to effective mental healthcare. Methods: Data for the parent study of this analysis was collected via in-depth, one-on-one interviews of about one hour's length, conducted at six-month intervals over the course of 2 years. These interviews were analyzed using an iterative process of open-coding, thematic code development, and code application to participant interviews. Results: The results showed a common perception of mental healthcare received within the carceral state as serving goals of the prison system, including control and punishment, rather than therapeutic goals of healing and empowerment. This often had negative implications for the quality of the treatment received, including patterns of diagnostic ambiguity, treatment that was ill-fitting to participants' needs, and treatment that was undermined by the new trauma created by the prison environment. The results also highlighted racial disparities prevalent within the carceral system. Despite the barriers created by the subjection of therapeutic practices to carceral goals, participants demonstrated resourcefulness and creativity in engaging with these treatment modalities to reap benefits where possible. Conclusions: Overall, these results highlight the inappropriateness of combining therapeutic and carceral spaces, the need for greater public attention to how carceral mechanisms disadvantage vulnerable populations, and the need for a cultural reconceptualization of mental illness such that it is met not with criminal punishment but appropriate care.
Where “Old Heads” Prevail: Inmate Hierarchy in a Men’s Prison Unit

Abstract:Research on inmate social order, a once-vibrant area, receded just as U.S. incarceration rates climbed and the country’s carceral contexts dramatically changed. This study returns to inmate society with an abductive mixed-methods investigation of informal status within a contemporary men’s prison unit. We collected narrative and social network data from 133 male inmates housed in a unit of a Pennsylvania medium-security prison. Analyses of inmate narratives suggest that unit “old heads” provide collective goods in the form of mentoring and role modeling that foster a positive and stable peer environment. We test this hypothesis with Exponential Random Graph Models (ERGMs) of peer nomination data. The ERGM results complement the qualitative analysis and suggest that older inmates and inmates who have been on the unit longer are perceived by their peers as powerful and influential. Both analytic strategies point to the maturity of aging and the acquisition of local knowledge as important for attaining informal status in the unit. In summary, this mixed-methods case study extends theoretical insights of classic prison ethnographies, adds quantifiable results capable of future replication, and points to a growing population of older inmates as important for contemporary prison social organization.

Abstract:There is mounting concern over the potential harms associated with ultra-processed foods, including poor mental health and antisocial behavior. Cutting-edge research provides an enhanced understanding of biophysiological mechanisms, including microbiome pathways, and invites a historical reexamination of earlier work that investigated the relationship between nutrition and criminal behavior. Here, in this perspective article, we explore how this emergent research casts new light and greater significance on previous key observations. Despite expanding interest in the field dubbed 'nutritional psychiatry', there has been relatively little attention paid to its relevancy within criminology and the criminal justice system. Since public health practitioners, allied mental health professionals, and policymakers play key roles throughout criminal justice systems, a holistic perspective on both historical and emergent research is critical. While there are many questions to be resolved, the available evidence suggests that nutrition might be an underappreciated factor in prevention and treatment along the criminal justice spectrum. The intersection of nutrition and biopsychosocial health requires transdisciplinary discussions of power structures, industry influence, and marketing issues associated with widespread food and social inequalities. Some of these discussions are already occurring under the banner of 'food crime'. Given the vast societal implications, it is our contention that the subject of nutrition in the multidisciplinary field of criminology—referred to here as nutritional criminology—deserves increased scrutiny. Abstract:Criminal Justice and Behavior, Ahead of Print. Drawing on qualitative interviews with 51 incarcerated adult men and nine correctional officers in a Western Canadian prison system, we ask why some incarcerated people find it appealing to be placed on correctional boot camp units and what such appeals tell us about broader conditions of incarceration. Participants on three boot camp units drew on narratives relating to (a) extrinsic benefits, (b) discipline and structure, (c) teamwork and positive relationships, and (d) an opportunity for self-improvement to construct symbolic boundaries between "normal" units and boot camps, as well as their former self and their transformed current self. By drawing symbolic boundaries between the past and present and between other units and their boot camp unit, our participants create narratives that allow them to partially mitigate some pains of imprisonment.

Abstract:As part of the rise of “get tough” punishment in recent decades, prison systems increasingly have relied on solitary confinement and what many contemporary accounts have termed “restrictive housing.” The latter includes an emphasis on some form of isolation and restrictions on privileges. Use of solitary-like confinement has engendered considerable debate because of differing views about whether it is moral or effective and whether it harms inmates. Despite this debate and the ubiquity of solitary-like confinement, there is much that remains unknown about its uses or effects. A central reason stems from inconsistent operationalizations of such housing in research and policy. This situation creates problems in generalizing the results of studies to diverse settings and populations. The goals of this article are to highlight these points and to advance scholarship and policy debates by presenting a conceptual framework for guiding and assessing research on restrictive housing.

An Examination of Mental Health and Psychiatric Care among Older Prisoners in the United States

Abstract:Despite that prisons in the United States (and other high-income countries) have witnessed an increase in the proportion of older inmates, and that prison populations exhibit high rates of psychiatric illness, there is limited knowledge on the nature of older inmates' psychological health and use/provision of psychiatric care. The present study addresses these gaps, analyzing a nationally representative sample of 1,907 male and female older inmates (age range = 50–84 years; <i>M</i> = 56) housed in U.S. state and federal prisons. The results highlight: (a) the prevalence of psychological issues among older prisoners; (b) factors associated with certain mental disorders and symptoms of mental health issues; (c) the prevalence of psychiatric treatment before and during imprisonment for those with (and without) reported psychological health issues; (d) similarities and differences between male and female older inmates in relation to psychological health, factors associated with psychological issues, and the use/provision of psychiatric care. Discussion points toward recommendations for managing inmate mental health, as well as direction for further research on older inmate mental health and psychiatric care.

Abstract:For nearly two centuries the practice of solitary confinement has been a recurring feature in many prison systems all over the world. Solitary confinement is used for a panoply of different reasons although research tells us that these practices have widespread negative health effects. Besides the death penalty, it is arguably the most punitive and dangerous intervention available to state authorities in democratic nations. These facts have spawned a growing international interest in this topic and reform movements which include, among others, doctors, psychologists, criminologists, sociologists, prisoners, families, litigators, human rights defenders, and prison governors. This chapter sets the scene by briefly describing this context and by presenting the structure of the book and the chapters that follow.

Abstract:There is broad consensus that the strains of imprisonment and unsupported release affect offenders’ mental health and operate to the detriment of their chances of successful reintegration. Drawing on data from 208 male inmates, the authors examine the mediating and moderating influences of social support on the links between inmates’ perceptions of prison conditions and other background variables on parolees’ feelings of hostility—a factor theoretically linked to reoffending—upon release. The results demonstrate that social support partially or completely mediates background characteristics and conditions the influence of prison perceptions on released inmates’ levels of hostility.
