Abstract:Background On 7 October 2023, hundreds of armed Hamas fighters breached the security border fence and entered Israel from the Gaza Strip. More than 1400 Israeli citizens were murdered, and 239 individuals were kidnapped. Many Israeli citizens experienced these occurrences as psychologically traumatic events that caused stress and uncertainty. Objectives The present study aimed to examine the relationship between exposure to war (in more distant circles), intolerance of uncertainty (IU) and disengaged coping on acute stress symptoms (ASS). First, we examined whether exposure to war and IU were directly associated with ASS. Second, we tested the mediating role of disengaged coping in the relationship among war exposure, IU and ASS. Methods This cross‐sectional study involved 393 Israeli citizens. Participants answered questionnaires on exposure to war, IU, coping strategies and ASS. Results The study results indicate that higher exposure and higher levels of IU were directly associated with more intensive ASS, and this association was partially mediated by higher use of disengaged coping strategies. Conclusions Individuals during wartime are at risk of experiencing high levels of ASS and developing ASD. However, degree of exposure to war alone was not associated with ASS, but it was related to personal resources and coping strategies.

Abstract:The study explored the intricate relationship between political armed conflict, mental health outcomes, and emotion regulation strategies, focusing on cognitive reappraisal and emotion suppression. Drawing on extensive literature, we examined the impact of the Israel-Hamas war on mental health, particularly post-traumatic stress symptoms (PTSS) and anxiety. Using data from a nationally representative sample of 517 Israeli adults surveyed four weeks after the October 7 war began, we conducted path analysis to examine the direct and indirect effects of conflict exposure on mental health outcomes mediated by emotion regulation. Cognitive reappraisal was positively associated with well-being, while emotion suppression was linked to higher levels of PTSS and anxiety and lower levels of well-being. Subjective exposure to war events was associated with higher levels of anxiety and PTSS, and cognitive reappraisal served as a protective factor for overall well-being. We also observed ethnicity, age, and gender differences in exposure levels and mental health outcomes. Emotion regulation emerges as a crucial factor in navigating the psychological challenges posed by war exposure, with cognitive reappraisal associated with positive mental health outcomes and emotion suppression linked to adverse effects.
Exposure to the 2014 Gaza War and Support for Militancy: The Role of Emotion Dysregulation

Abstract:How do wars shape emotions and attitudes in intractable conflicts? In two studies conducted in the aftermath of the 2014 Gaza War in the Middle East, we tested a new theoretical model wherein the ability to regulate emotions is central in determining the influence of war exposure on emotions (i.e., group-based humiliation) and support for militancy, through posttraumatic stress symptoms (PSS). Results supported our model: (a) higher exposure to the war predicted group-based humiliation in both studies and in Study 2 also greater support for militancy; in both studies, (b) higher exposure predicted more PSS only among participants high in emotion dysregulation, and, for them, (c) higher exposure predicted greater group-based humiliation, through increased levels of PSS. Results from Study 2 suggest that (d) group-based humiliation will ultimately lead to greater support for militancy. The findings' contribution to the different literatures and their integration is discussed.
Peritraumatic Reaction Courses During War: Gender, Serious Mental Illness, and Exposure

Abstract:OBJECTIVE: We assessed in vivo symptom courses of early psychological responses during war and investigated the influence of exposure, gender, and a prior diagnosis of severe mental illness (SMI).METHOD: Participants were 181 highly exposed individuals from the general population and community psychiatric rehabilitation centers. A 30-day twice-daily Internet-smartphone-based intensive assessment two weeks into the 2014 Israel-Gaza war estimated peritraumatic symptom clusters, sense of threat, negative emotions and cognitions, and siren exposure during two periods that varied in exposure level. Piecewise growth curve modeling procedures were performed.RESULTS: We found different courses for most variables, gender, and SMI status. Women were more reactive two weeks into the war but reduced their reactivity level at a faster pace than males, reaching lower symptom levels one month later. Women's courses were characterized by arousal, negative emotionality, sense of threat, and reactivity to siren exposure. No-SMI men had a stable course followed by a significant reduction in arousal, negative emotions, avoidance, and perceived threat during a "return to routine" lower-level intensity period of the war. Individuals with SMI had higher reactivity levels at study onset; but while women with SMI improved over time, men with SMI worsened. SMI reactivity was characterized by negative cognitions, intrusions, and avoidance.CONCLUSIONS: Early reactions during prolonged exposure to war are variable, dynamic, and affected by exposure context. Symptoms, emotions, and cognitions develop differentially over time and are affected by gender and mental health status. The identification of various early stress courses should inform primary intervention strategies.
PTSD symptoms and subjective traumatic outlook in the Israel-Hamas war: Capturing a broader picture of posttraumatic reactions

Abstract:This study examined the unique associations of both probable posttraumatic stress disorder (PTSD) and subjective traumatic outlook (STO) with psychiatric and psychological concomitants among Israelis during the Israel-Hamas War. While PTSD reflects phenomenological psychiatric reactions to traumatic events, STO reflects subjective evaluations that one is traumatized. A convenience sample of 375 Israelis aged 20-87 from around Israel completed an online survey in the first two months of the war. A series of univariate logistic and linear regressions showed that while both probable PTSD and STO were associated with significant depression and anxiety symptoms, STO showed unique associations with hope and revenge, psychological variables that may be more sensitive following exposure to human-made and war trauma. Theoretical and practical considerations regarding these results suggest that a broader picture of post-trauma reactions can be captured by accounting for both the observed symptoms (PTSD) and one's introspective perception of being traumatized (STO).

Abstract:Journal of Social Work, Ahead of Print. SummarySocial workers have had a pivotal role in handling the challenges faced by people dealing with the coronavirus disease 2019 (COVID-19) pandemic. Supporting clients who have undergone traumatic experiences during a global pandemic may increase the risk of experiencing secondary traumatic stress. This study examined two protective factors that may be key in the association between stress and social workers' secondary traumatic stress: Psychological capital (internal protective factor) and satisfaction with supervision (external factor). The study sample included 104 Israeli social workers who worked in their profession during the first year of the COVID-19 pandemic. A regression-based path analysis was conducted to test the moderation model with the PROCESS software.FindingsSelf-efficacy significantly moderated the association between perceived stress and intrusion; increasing self-efficacy decreases perceived stress's effect on intrusion. Although no significant moderating effect was found for the existence of supervision, the interaction between perceived stress and satisfaction with supervision was significant in predicting secondary trauma.ApplicationsSocial workers' unions must work to improve the quality of supervision and raise awareness of the importance of supervision in general and particularly during times of crisis. Manageable workloads, a reasonable balance between work and home, and verbal and financial encouragement to seek professional psychological assistance would all be to the advantage of social workers at risk for secondary trauma.

The impact of political violence on posttraumatic stress symptomology: a longitudinal analysis

Abstract:Objective: The current paper uses the Conservation of Resources Theory to frame an examination of the impact of exposure to political violence on posttraumatic stress symptomology among three groups within Israeli society: (1) Native Born Jews; (2) Foreign Born Jews; and (3) Palestinian Citizens of Israel. Methods: The study population was a large nationally representative sample of 1613 respondents collected during The Second Intifada. The sample consists of approximately 40% Jews born in Israel (n = 652), 30% (n = 484) were Jews who immigrated to Israel, and close to 30% (n = 477) were Palestinian Citizens of Israel. Mediation analyses explored the role of resource loss in the relationship between social status and posttraumatic stress disorder (PTSD) symptom severity. Results: Compared to native born Jews, foreign born Jews and Palestinian Citizens of Israel reported greater PTSD symptom severity at wave III. These relationships were not mediated by psychosocial resource loss or economic resource loss. Conclusions: We discuss the importance of tailored interventions with minority groups in the context of ongoing political violence.
Post-traumatic Recovery to Distress Symptoms Ratio Mediates Relations of Resilience Fostering Resources and Their Predictors

Abstract:A large-scale study investigated the direct and indirect effects of demographic predictors and traumatic experiences on resilience fostering resources following a war. The sample consisted of 829 Israeli adults, living in a border town, 1 year after experiencing the 2006 war with Lebanon. Resilience was assessed by measures of individual and public resilience and low sense of danger. Results show that as hypothesized the proportion of post-traumatic recovery to post-war distress symptoms predicts these indices of resilience and partly mediates the direct links between these indices and demographic predictors (age, gender and economic condition) and exposure to war. Research of resilience has proposed several prototypical trajectories that characterize responses to potentially traumatic events. Our discussion suggests that these trajectories and their changes over time can be accounted for by the balance of post-war recovery to symptoms level. Copyright © 2014 John Wiley & Sons, Ltd.

Abstract:The ongoing Israel-Hamas war is posing additional challenges for mental health workers in an already stressful workplace. This study centres on the psychological effects of the shared traumatic reality on mental health workers, arising from the Israel-Hamas war. One month after exposure to the terrorist attack of 7 October 2023 and the outbreak of war following this event, 147 mental health workers completed questionnaires regarding a variety of variables such as demographics, anxiety symptoms, acute stress symptoms, media-induced secondary trauma, personal resilience, National resilience (NR), and post-traumatic growth (PTG). The study found that mental health workers with previous trauma displayed higher anxiety symptoms, acute stress symptoms, and media-induced secondary trauma. Additionally, acute stress and anxiety were positively correlated with media-induced secondary trauma. Religiosity, personal resilience, and NR were found associated with lower anxiety and acute stress symptoms. Religiosity was also positively correlated with personal resilience, NR, and PTG. The PTG of mental health workers working with trauma survivors and evacuees was higher compared to that of other mental health workers. Both adverse and adaptive reactions were evident among mental health workers. While traumatic stress is expected, individual, professional, and NR factors may mitigate its effects. Providing training, social support, regulated media exposure, stress management, and meaning-focused coping strategies can help safeguard workers' well-being.

Abstract:This study examined the moderating role of self-complexity (SC) on well-being (WB) and psychopathology among Israeli adolescents exposed to the Second Lebanon War (2006). Adolescents (N=584, mean age 16.41) completed a SC measure, Political and Negative Life Events (NLE) scales, Brief Symptom Inventory and Satisfaction with Life Scale. The theoretical model analyzed the function of SC as a moderator of exposure effects to political life events (PLE), while controlling for general NLE. Results corroborated the model with SC moderating the effects of the war-related PLE. Adolescents with low SC are at risk for damaged WB and psychiatric consequences from political violence exposure. This opens a diagnostic avenue for identification of at-risk adolescents in this socio-political context toward whom clinical programs should be directed.

Abstract:This study examined the moderating effect couple forgiveness (as perceived by the wives/mothers) may have on the intergenerational transmission of posttraumatic stress symptoms (PTSS) among families of combat veterans and former prisoners of war (ex-POWs). The sample included 123 (79 ex-POWs and 44 control combatants) Israeli father-mother-adult offspring triads. Self-report measures were administered at 2008 to veterans, at 2010-2011 to wives, and at 2013-2014 to offspring. The findings indicated that ex-POWs' PTSS were positively related to their offspring's PTSS, while couple forgiveness buffered this effect, particularly, and detrimentally, when couple forgiveness was low. These findings imply that apprehending the quality of the forgiving atmosphere within the marital relationship may be important for understanding the apparatus of intergenerational transmissions of trauma.

Abstract:OBJECTIVES Depression is a prevalent outcome of traumatic experiences, such as combat and war captivity. This study explores the heterogeneity of changes over time and assesses the contribution of trauma exposure (combat vs. war captivity), hardiness, and social support for depression trajectories. METHODS Two groups of Israeli veterans were assessed in 1991, 2003, 2008, and 2015: 149 former prisoners-of-war (ex-POWs) and 107 combat veterans. Protective factors were evaluated in 1991. Group-based trajectory modeling was conducted to identify latent trajectories of change. RESULTS Four trajectories of "resiliency" (62.8%), "delayed onset" (25.1%), "exacerbation" (6.2%), and "chronicity" (5.9%) were found. The majority of the resilient group were combat veterans whereas the clinical groups consisted primarily of ex-POWs. Lower hardiness and social support were related to more deleterious trajectories. CONCLUSIONS Spirals of loss involving hardiness and social support, normative experiences, and contextual factors may present explanations for the various depression trajectories.
War-Related Stress among Israeli College Students Following 7 October 2023 Terror Attack in Israel

Abstract:Background: Warfare represents a significant source of stress in contemporary times, with enduring implications beyond the immediate casualties, fostering a pervasive atmosphere of danger and anxiety within affected populations. The Israel-Hamas war, marked by ongoing armed incursions and missile attacks, stands as a recent example of such turmoil, inflicting widespread trauma and disruption. Methods: This study, conducted among students at the Ashkelon Academic College in southern Israel, aimed to investigate the stress levels and associated factors amidst the ongoing conflict. Utilizing a cross-sectional survey design, data were collected from 625 participants between January and February 2024, approximately four months after the commencement of the initial attack. The survey encompassed demographic information, perceived stress levels, sleep quality, and social media usage. Results: The findings revealed moderate to high stress levels among participants, with significant differences observed based on gender, parental status, and residency in conflict zones. Moreover, poorer sleep quality was reported among students residing in conflict-affected areas. Regression analysis identified several predictors of elevated stress, including gender, parental status, sleep quality, residency in conflict zones, and social media usage. Conclusions: These findings underscore the profound impact of ongoing conflict on college students' mental well-being, highlighting the need for tailored interventions and support services within higher education institutions. The limitations include the study's focus on a specific college population and the timing of the data collection relative to the onset of the war. Nonetheless, this research contributes valuable insights concerning the stress dynamics within the unique context faced by Israeli students amidst ongoing warfare.
The Relation Between Veterans' Posttraumatic Stress Disorder and Their Wives' Posttraumatic Growth: Testing of a Multiple Mediating Model

Abstract:Objective To examine the multiple mediating roles of veterans' disclosure and intimacy as well as their wives' posttraumatic stress disorder (PTSD) in the relation between veterans' PTSD and wives' posttraumatic growth (PTG). Methods Self-report questionnaires were used to investigate 287 Israel veterans who fought in the 1973 Yom Kippur War, and 166 wives of these veterans. A structure equation model analyzed the multiple mediating effects. Results Veterans' PTSD had a significant and positive effect on their wives' PTG before inclusion of the mediators. However, after inserting the mediators (i.e., veterans' disclosure and intimacy and wives' PTSD) the direct effect of veterans' PTSD on their wives' PTG became non-significant. In addition, veterans' PTSD had a 1-step indirect effect on their wives' PTG through wives' PTSD, and veterans' PTSD also had a 2-step indirect effect on their wives' PTSD through the path from veterans' disclosure to veterans' intimacy. Conclusion The results indicated that veterans' disclosure and intimacy as well as their wives' PTSD mediate the relation between veterans' PTSD and their wives' PTG. Abstract:Background Exposure to potentially morally injurious events (PMIEs) during military service is associated with heightened suicidal ideation (SI). However, no longitudinal study has established temporal associations between these variables and examined the possible mediating roles of depression and loneliness in this effect. Methods Participants were 374 active-duty Israeli combatants who participated in a five-year longitudinal study with four measurement points: T1- one year before enlistment, T2- one month before discharge from army service, and then again six months and twelve months following their discharge (T3 and T4, respectively). Data were assessed through semi-structured interviews and validated self-report questionnaires. Results Above and beyond pre-enlistment personal characteristics (T1) and combat exposure (T2), PMIEs-'betrayal' (T2) predicted more frequent SI over the past twelve months (T4) through the mediation of depression. Moreover, both PMIEs-'betrayal' and 'self' (T2) predicted more frequent SI over the past twelve months (T4) through the mediation of loneliness. Limitations We used self-report measures to assess PMIEs and SI, which may suffer from various biases. Conclusions Our findings are the first to provide evidence of longitudinal, temporal associations between exposure to PMIEs and SI. Notably, potential interventions might consider addressing the loneliness experienced following exposure to PMIEs during military service, among recently discharged traumatized veterans.

Mental Health Outcomes of Arab and Jewish Populations in Israel a Month after the Mass Trauma Events of October 7, 2023: A Cross-Sectional Survey of a Representative Sample

Abstract:On October 7, 2023, a war erupted in Israel following a mass terror attack including mass shootings, violent events, civilian abductions, and numerous fatalities, ranking as the third most deadliest terror attack. This cross-sectional, population-based study evaluated the impact on the mental health and utilization of mental health services in the Arab and Jewish populations. Conducted through a virtual platform, the study compared demographic factors, exposure to war-related events, anxiety, and post-traumatic stress symptoms among 517 participants in a representative sample of the adult population in Israel (79.1% Jewish, 20.9% Arab). Jews reported higher exposure to war-related events, but a majority expressed a lack of interest in mental help. In contrast, more Arabs desired mental help but faced barriers like stigma and scarce resources. Arabs showed a greater preference for group therapy and medical treatment. Trust in official bodies was consistently higher among Jews. Both populations exhibited similar levels (12-15.4%) of probable post-traumatic stress disorder (PTSD). This study equips clinicians, researchers and policymakers with real-time insights into improving mental health support for the culturally diverse needs of Jewish and Arab communities following exposure to mass trauma.
The contribution of exposure to potentially morally injurious events to trajectories of posttraumatic stress symptoms among discharged veterans - a five-year study

Abstract:Purpose: Combatants and veterans are at risk of developing post traumatic stress symptoms (PTSS). The long-term responses to traumatic events are variable and can be classified into distinct PTSS trajectories. In this prospective study, we evaluated PTSS trajectories among combat veterans during the initial year after discharge from military service. Subsequently, we analyzed how combat exposure and PMIEs contributed to these trajectories. Methods: Our study encompassed 374 combat veterans who participated in a five-year prospective study, with four waves of measurements, T1 - one year before enlistment, T2 - one month prior to discharge from military service (July 2021), and then again at six months (T3 - February 2022) and twelve months after discharge (T4, July-August 2022) . Results: The utilization of Latent Profile Analysis (LPA) revealed a diverse array of PTSS trajectories. Predominantly, a resilient trajectory emerged as the most frequently observed (69.3%), with 'delayed onset'(13.6%), 'improving'(9.9%) and 'chronic'(6.1%) trajectories following in order. Importantly, multinominal regression analysis indicated that combat exposure and PMIE-betrayal contributed to alignment with symptomatic trajectories. Conclusions: This study represents the first of its kind to establish longitudinal, time-dependent associations between PMIEs and PTSS trajectories. These results emphasize the critical importance of ongoing screening and the development of tailored interventions for combat veterans.

The research, which analyzed data from 2028 Israeli residents following the traumatic events of October 7, 2023, investigated the perception of non-existent sirens and their association with acute PTSD and CPTSD symptoms. The findings reveal that (pseudo)hallucinations were more prevalent among individuals with acute CPTSD symptoms compared to those with PTSD symptoms alone. Additionally, auditory (pseudo)hallucinations were significantly associated with a higher likelihood of CPTSD versus PTSD. These results were consistent for those directly and indirectly exposed individuals to the October 7 attack. Despite its cross-sectional nature, the study provides valuable insights into trauma-related auditory (pseudo)hallucinations in wartime contexts.
