Abstract:OBJECTIVES:Mild cognitive impairment (MCI) is a transitional stage between normal cognitive aging and dementia that increases the risk of progressive cognitive decline. Early prediction of MCI could be beneficial for identifying vulnerable individuals in the community and planning primary and secondary prevention to reduce the incidence of MCI.DESIGN:A narrative review and cohort study.SETTING AND PARTICIPANTS:We review the MCI prediction based on the assessment of sociodemographic factors. We included participants from 3 surveys: 8915 from wave 2011/2012 of the China Health and Retirement Longitudinal Study (CHARLS), 9765 from the 2011 Chinese Longitudinal Healthy Longevity Survey (CLHLS), and 1823 from the 2014 Rugao Longevity and Ageing Study (RuLAS).METHODS:We searched in PubMed, Embase, and Web of Science Core Collection between January 1, 2019, and December 30, 2022. To construct the composite risk score, a multivariate Cox proportional hazards regression model was used. The performance of the score was assessed using receiver operating characteristic (ROC) curves. Furthermore, the composite risk score was validated in 2 longitudinal cohorts, CLHLS and RuLAS.RESULTS:We concluded on 20 articles from 892 available. The results suggested that the previous models suffered from several defects, including overreliance on cross-sectional data, low predictive utility, inconvenient measurement, and inapplicability to developing countries. Our empirical work suggested that the area under the curve for a 5-year MCI prediction was 0.861 in CHARLS, 0.797 in CLHLS, and 0.823 in RuLAS. We designed a publicly available online tool for this composite risk score.CONCLUSIONS AND IMPLICATIONS:Attention to these sociodemographic factors related to the incidence of MCI can be beneficially incorporated into the current work, which will set the stage for better early prediction of MCI before its incidence and for reducing the burden of the disease.
The Prevalence Of Mild Cognitive Impairment In China: A Systematic Review

Abstract:The aim of this study was to analyze the prevalence of mild cognitive impairment (MCI) among the aging population (60 years of age and above) in China. Epidemiological investigations on MCI in online Chinese journals were identified manually using the CQVIP, CNKI, and Wanfang databases. Articles from journals published in English were identified using PubMed and Web of Science. Original studies that included prevalence surveys of MCI were selected. Forty-eight relevant studies were included in the analysis, covering 22 provinces in China. Our results showed that the pooled prevalence of MCI in the older Chinese population was 14.71% (95% confidence interval [CI], 14.50-14.92%). The prevalence was 16.72% (95% CI, 15.68-17.71%) in clinical samples vs. 14.61% (95% CI, 14.40-14.83%) in nonclinical samples (chi 2=16.60, P<0.01), and 15.20% (95% CI, 14.91-15.49%) in screened samples vs. 14.16% (95% CI, 13.85-14.46%) in diagnosed samples (chi 2=22.11, P<0.01). People of older age, of female sex, or living in rural areas or western China were associated with a higher prevalence of MCI. The prevalence of MCI was high in Chinese older adults, and even higher in those who were older, female, or living in rural areas or western China. Future studies are recommended to address the prevalence of MCI in the other 12 provinces of China. Furthermore, diagnostic assessments should be included in the identification of MCI.

Abstract:The combination and integration of multimodal imaging and clinical markers have introduced numerous classifiers to improve diagnostic accuracy in detecting and predicting AD; however, many studies cannot ensure the homogeneity of data sets and consistency of results. In our study, the XGBoost algorithm was used to classify mild cognitive impairment (MCI) and normal control (NC) populations through five rs-fMRI analysis datasets. Shapley Additive exPlanations (SHAP) is used to analyze the interpretability of the model. The highest accuracy for diagnosing MCI was 65.14% (using the mPerAF dataset). The characteristics of the left insula, right middle frontal gyrus, and right cuneus correlated positively with the output value using DC datasets. The characteristics of left cerebellum 6, right inferior frontal gyrus, opercular part, and vermis 6 correlated positively with the output value using fALFF datasets. The characteristics of the right middle temporal gyrus, left middle temporal gyrus, left temporal pole, and middle temporal gyrus correlated positively with the output value using mPerAF datasets. The characteristics of the right middle temporal gyrus, left middle temporal gyrus, and left hippocampus correlated positively with the output value using PerAF datasets. The characteristics of left cerebellum 9, vermis 9, and right precentral gyrus, right amygdala, and left middle occipital gyrus correlated positively with the output value using Wavelet-ALFF datasets. We found that the XGBoost algorithm constructed from rs-fMRI data is effective for the diagnosis and classification of MCI. The accuracy rates obtained by different rs-fMRI data analysis methods are similar, but the important features are different and involve multiple brain regions, which suggests that MCI may have a negative impact on brain function.

Abstract:We aimed to investigate the use of advanced functional MRI (fMRI) techniques such as proton magnetic resonance spectroscopy (1H-MRS) and the apparent diffusion coefficient (ADC) value in diffusion weighted imaging (DWI), in the diagnosis of mild cognitive impairment (MCI). Multiple indicators were combined in order to improve the early diagnostic value of MRS and ADC. We administered MRS and DWI-ADC to 13 patients with Alzheimer’s disease (AD), 9 patients with MCI, and 13 control patients. Changes in N-acetylaspartate/creatine and phosphocreatine (NAA/Cr), myoinositol/creatine (mI/Cr), and the ADC values in the hippocampus and the temporoparietal region were compared among groups. The sensitivity and specificity of different markers were analyzed individually and combined with others. All participants were evaluated by the mini mental state examination (MMSE), and the correlation between NAA/Cr, MI/Cr, ADC and the score of MMSE were analyzed separately. The NAA/Cr, mI/Cr and ADC values in the hippocampus among AD, MCI patients, and controls were significantly different (p<0.05). At a fixed specificity of 84.6%, the high sensitivity of 100% and 92.9% in differentiating AD and MCI from normal controls were obtained by combining the three indicators. The receiver operating characteristic plots illustrated that the area under the multimarker curve was the biggest among the all four curves, and the sensitivity of the multimarkers was highest. The best correlation was obtained between ADC and MMSE, rather than between NAA or mI and MMSE. Thus, we found that changes in NAA/Cr, mI/Cr and ADC in the hippocampus and the temporoparietal regions were helpful in the clinical diagnosis of MCI. Furthermore, these changes showed potential in predicting the progression of MCI to AD if the multimarkers were combined.

Abstract:OBJECTIVE:To assess whether quick cognitive screening test (QCST) could quickly identify mild cognitive impairment (MCI).METHODS:QCST and a full set of standardized neuropsychological tests, including mini-mental state examination (MMSE) and montreal cognitive assessment (MoCA) were performed. A total number of 121 cases of MCI [41 cases of amnestic MCI-single domain (aMCI-s); 44 of amnestic MCI-multiple domain (aMCI-m); 36 of nonamnestic MCI (naMCI)], 79 cases of mild Alzheimer's disease (AD) and 186 healthy elderly volunteers were employed in the present study. All the participants (55-85 years old) had an educational level no less than 5 years. QCST subtests included word list recall, naming test, animal fluency test, similarity test, color trail-1min, clock drawing test, finger construction test, and digit span test. The total score of QCST was 90 points, 10 points for each index of subtests.RESULTS:The total scores of QCST in MCI, AD and the control groups were (58.13+/-8.18), (44.53+/-10.54) and (72.92+/-6.85) points, respectively. According to the educational level, the cut off scores of participants with an educational level of 5-8 years, 9-12 years and more than 13 years were 63, 65 and 68 points, respectively. The sensitivity and specificity of QCST in detection of MCI were 87.6% (85.7% for aMCI-s, 90.1% for aMCI-m and 89.5% for naMCI) and 84.3%, respectively. The area under the curve was 0.923 (95% CI: 0.892-0.953). Delayed memory, color trail-1min and similarity test could help distinguish between aMCI and naMCI.CONCLUSION:QCST may have a good sensitivity and specificity for MCI detection, which warrants its further clinical application.

Abstract:Objective:To analyze identify method of mild cognitive impairment. Methods:The Wechsler Adult Intelligence Scale-Chinese Revised (WAIS-RC) and Wechsler Memory Scale-Chinese Revised (WMS-RC) were examined in 150 patients with memory complaints. 63 individuals whose intelligence quotient (IQ) and memory quotient (MQ) over 85 were designated as normal elderly group;and 33 individuals whose IQ scores over 85 and MQ score between 70 and 84 as mild cognitive impairment(MCI-1) group; and 25 individuals whose IQ scores over 85 and MQ scores between 55 and 69 as MCI-2 group; 29 individuals whose IQ scores and MQ scores less than 85 and conformed with DSM-Ⅳ as Alzheimer disease group.Results: Picture short memory and mental control (B)showed the highest identification between the 2 MCI groups and normal elderly group. Besides, visual reproduction played an important role in mild memory decline (MCI-1) group. Tactile memory, logical memory and associated learning played an important role in moderate memory decline (MCI-2) group.Conclusion: Picture short memory and mental control (B) contribute to identify MCI speedily and effectively.
Mild Cognitive Impairment: a Concept Useful for Early Detection and Intervention of Dementia

Abstract:As life expectancy increases around the globe, dementia has become an increasingly important public health issue that has created new challenges for communities' social service networks[1]. The early detection and prevention of dementia, particularly Alzheimer's disease (AD), necessitate the development of effective public health education programs about the early symptoms and preclinical changes of dementia. To promote this effort the concept of mild cognitive impairment (MCI) – which some experts consider an early form of AD – has been created. Despite tremendous progress over the last 20 years in understanding the neuropsychological, neuroimaging, and neurobiological characteristics of MCI, there remains controversy about the value of this construct. [2] Despite the problems of proving that MCI is a distinct disease entity, we contend that this clinical construct plays a crucial role in addressing the public health challenges of dementia that exist in contemporary societies. First, MCI is strongly related to dementia, especially AD. Studies in China and other countries have consistently found that elderly people with MCI are much more likely to develop dementia than those without MCI.[3] The cognitive regression and the patterns of cerebral atrophy in elderly persons with MCI are similar to those seen in individuals with AD, and the similarity is most pronounced in individuals with MCI who subsequently develop AD.[4] Brain imaging studies indicate that the structural and functional changes found in the brains of elderly persons with MCI are intermediate between those in normal elderly people and in elderly individuals with AD.[5] Second, interventions for persons with MCI can improve brain functioning. Cognitive or memory training can improve memory functioning in elderly people with MCI.[6],[7] Neuroimaging studies have shown that cognitive training can enhance brain activity in the frontal, temporal, and parietal areas.[8] And several randomized controlled trials have found that specific Chinese traditional medicines can reduce the transition from MCI to AD.[9],[10]These findings suggest that brain plasticity is preserved in elderly people with MCI and, thus, suggest that targeted interventions should be able to improve the cognitive functioning of individuals with MCI. Lastly, the recognition of MCI can help in the early detection of dementia. Recent diagnostic criteria of AD and MCI[11],[12] stress the value of pathological biomarkers including the neurotoxic β-amyloid protein (Aβ) and tau protein which either directly damage neurons or are the indicators of damaged neurons. Subsequent research focused on these biomarkers hold the promise of substantially improving the prevention and management of AD.[13] Research on the sensitivity and specificity of these biomarkers may find that their occurrence in individuals with MCI is predictive of progression to AD and, thus, merits more intensive follow-up and treatment. And if drugs targeting Aβ or tau protein prove effective, screening for these biomarkers in persons with MCI will identify those who are responsive to treatment and, thus, can prevent the progression to AD in these individuals. In clinical practice the use of cognitive tests to screen for MCI may be more feasible than the routine assessment of biomarkers. Several instruments from high-income counties (e.g the Montreal Cognitive Assessment [MoCA][14]) have been translated and adapted for use in China. But not all such screening tools are appropriate for use in mainland China where the cultural and educational background of the elderly, particularly those from rural areas, is quite different from that of the elderly in high-income countries. Thus, we designated a patent screening tool, the Screening Scale for Mild Cognitive Impairment (ZL201010508406.2)[15] which might overcome the problem and is more appropriate to use with Chinese elderly. Cultural and lifestyle factors also play a role in the adaptation of the cognitive training programs for elderly persons with MCI that have been primarily developed in high-income countries. It will also be important to develop China-specific programs for promoting life styles in the elderly that are conducive to brain health and the prevention of dementia. In summary, active research about the identification of MCI and about different interventions for persons with MCI is one of the most promising avenues that communities around the world have for facing the tremendous public health challenge of AD.
Assessment implement and its related factors in type 2 diabetic patients with mild cognitive impairment

Abstract:Objective To study the assessment implement and its related factors in type 2 diabetic patients with mild cognitive impairment(MCI).Methods Montreal Cognitive Assessment(MoCA)(Beijing Version)was chosen as cognition assessment implement.58 type 2 diabetic patients with MCI were enrolled as the research group and 30 type 2 diabetic patients with normal cognitive function as control.HbA_(1C),blood lipid,urine microalbumin,liver and renal functions were measured in all subjects.Results Compared with control group,the blood levels of HbA_(1C)[(10.48±2.38 vs 9.28±2.19)%,P<0.05],total cholesterol[(4.87±1.18 vs 4.18±1.04)mmol/L,P<0.01],and low-density lipeprotein-cholesterol[LDL-C,(2.97±0.87 vs 2.37±0.61)mmol/L,P<0.01]increased,and high-density lipoprotein-cholesterol decreased[(1.084±0.34 vs 1.25±0.33)mmoL/L,P<0.05]in MCI group.There were significant differences in the duration of diabetes mellitus,diabetic retinopathy,body mass index,and abdominal circumference between MCI group and control group(all P<0.05).There were no significant differences in blood triglycerides,alanine aminotransferase(ALT),aspartate aminotransferase(AST),creatinine,and urine microalbumin between the two groups.MoCA scores were negatively correlated with HbA_(1C)(r=-0.396,P=0.002)and LDL-C(r=-0.275,P=0.036)in MCI group.Multiple regression analysis showed that HbA_(1C) was a significantly independent determinant for the MoCA scores.Conclusion The risk factors such as longer duration of diabetes mellitus.more diabetea mellitus complications,obesity,dyslipidemia,and inefficient control of blood glucoge all contribute to the development and aggravation of cognitive impairment.Therefore,good control of blood glucose and lipids,and reduction of complication and body weight may help to improve the cognitive function.

Abstract:Background: Callosal Angle (CA) and Evans Index (EI) are considered as imaging biomarkers to diagnose normal-pressure hydrocephalus using traditional MR measurement methods. Objective: The current study was aimed to evaluate the differential diagnostic value of CA and EI in the mild cognitive impairment (MCI) and Alzheimer’s disease (AD). Methods: Five-hundred and two subjects were selected from the Alzheimer's Disease Neuroimaging Initiative (ADNI) database, which included 168 Normal Controls (NC), 233 MCI and 101 AD patients. The structural MR images were interactively applied with multiplanar reconstruction to measure the CA and EI. Results: CA presented no significant difference among NC, MCI and AD groups (H value = 3.848, P value = 0.146), and EI demonstrated higher value in MCI and AD groups than that in NC groups (P = 0.000 and 0.001, respectively). MCI group had significant larger EI (0.29±0.04) than (0.27±0.03) NC group in 70-75 years old sub-groups. ROC showed that the area under the curve was 0.704±0.045 for NC-MCI in 70-75 years old groups. The correlation analysis indicated that EI was significantly negatively related to MMSE scores of MCI patients (r = -0.131, P = 0.046). Conclusion: EI might serve as a screening imaging biomarker for MCI in 70-75 years old, and show limited differential value for the diagnosis of AD. CA could present no diagnostic value for MCI and AD in the current study.

Abstract:BACKGROUND: Mild cognitive impairment (MCI) represents early status of Alzheimer's disease. Magnetic resonance spectroscopy (MRS) has been used in early diagnosis and identification as well as monitoring of Alzheimer's disease. However, little data is available concerning relationship between cognitive impairment and changes of biochemical metabolites. OBJECTIVE: To investigate the relationship between cognitive impairment and changes of biochemical metabolites as well as volumes of hippocampal formation (HPF) in MCI patients, accordingly to evaluate the roles of MRS in the diagnosis and the state monitoring of MCI. DESIGN, TIME AND SETTING: Case-contrast observation. The study was performed at the Outpatient Department and ward of Department of Neurology, Lianyungang Hospital of Xuzhou Medical College between September 2006 and December 2008. PARTICIPANTS: A total of 20 MCI patients were selected and 20 elderly with normal cognition, matched in gender, age and education, were selected. METHODS: Before volumes of the left HPF were measured by MRS, mini-mentalstate examination (MMSE), the Montreal Cognitive Assessment (MoCA), activities of daily living (ADL), clinical dementia rating (CDR) and Wechsler memory scale (WMS) were used to examine each participant. In addition, the N-acetylaspartate (NAA)/creatine (Cr), choline (Cho)/Cr, myo-inositol (ml)/Cr of HPF were obtained. Moreover, the correlations between the NAA/Cr, Cho/Cr, ml/Cr and the volume of HPF in MCI group were analyzed. MAIN OUTCOME MEASURES: Volume of HPF, NAA/Cr, Cho/Cr, and ml/Cr. RESULTS: Compared with control group, reduced NAA/Cr (P < 0.01), increased ml/Cr (P < 0.01) and reduced volume of HPF (P < 0.05) were found in the MCI subjects. No significant difference was observed in the Cho/Cr between two groups (P > 0.05). In the MCI group, the NAA/Cr remarkably correlated with MoCA, memory quotient, and volumes of left HPF (P < 0.01); Cho/Cr significantly correlated with memory quotient (P < 0.05); ml/Cr has remarkably negative correlation with MoCA and volumes of left HPF (P < 0.01), but positive correlation with memory quotient (P < 0.01). The volumes of left HPF also remarkably correlated with MoCA and memory quotient (P < 0.01). CONCLUSION: An increased ml/Cr ratio, a decreased left HPF volumes and NAA/Cr ratio were observed in the MCI patients. Significant correlations between biochemical metabolites of HPF, HPF volumes and MoCA as well as memory quotient were observed. MRS plays an important role in the diagnosis and state monitoring of MCI.
Neuropsychological investigation and analysis of elderly patients with mild cognitive impairment

Abstract:Objective:To explore the neuropsychological characteristics of the patients with mild cognitive impairment (MCI) in order to provide the basis for the development of cognitive training.Methods:60 elderly patients who had check-up in geriatrics outpatient department were given examination with neuropsychological scale and they were divided into normal group,mild cognitive impairment group (MCI) and Alzheimer's disease group (20 cases for each group).The specially trained nurses mensurated the patients in these 3 groups and the investigation survey was conducted by mini mental status examination (MMSE),activities of daily living scale (ADL).Results:The MMSE scores of neuropsychological scale were respectively 29.300±0.923 in normal group,25.300±1.261 in MCI group and 13.000±2.675 in AD group,and the significant differences were found among the three groups (P<0.05);the scores in MCI group were between the scores in AD and normal group;ADL scores were 22.500 ± 1.604 in normal group,22.800 ± 2.308 in MCI group and 53.700 ± 13.191 in AD group respectively.The scores among the three groups were significantly different (P<0.05).The scores in MCI group were slightly higher than the scores in normal group and significantly lower than the scores in AD group.Conclusion:The patients with MCI have a number of damages in cognitive function and the decline in different degree was found in short-term memory,visual space,calculating power and the executive function.The patients with MCI have the similar neuropsychological characteristics with AD patients in early stage.
The Distinct Roles Ofdifferent Memory Tests in Identifying Mild Cognitive Impairment

Abstract:Objective To find the distinct roles of different meomory tests in identifying mild cognitive impairment(MCI).Methods Seven hundred and twelve patients from the dysmnesia clinic in our hospital from May 2004 to May 2006 underwent brain CT scan or MRI.Among these patients, 329 were allocated into research group, who had complain of memory impairment, symptoms lasting more than 3 months, were aged between 50--80 years old, at an 8-year or up educational level, with MMSE≥24 and unknown etiology, normal auditory and visual sense, thus did not meet the diagnostic criteria of dementia according to NINCDS-ADRDA.A battery of neuropsychological tests comprised of auditory verbal learning test(AVLT), logical memory test(LM), Rey-Osterrieth complex figure test(CFT), MMSE, clock drawing test, five point test, verbal fluency test, Stroop color word test, trail making test and clinical dementia rating scale.Results(1)The identified rate of MCI was 51% for AVLT-Ⅱ31% for AVLT-Ⅰ27% for recall of CFT21% for LM-Ⅰ=21% for LM-Ⅱ.(2)The patients who had lower cut off score of AVLT- Ⅱ and LM-Ⅱ were classified into MCI group(63/329,19%).The patients whose score of AVLT-Ⅱ was lower and LM-Ⅱ was higher than the cut off score were classified to incongruent group(104/329, 32%). (3)Analyzing of the clinical manifestation and psychological tests revealed patients in incongruent group, unlike the normal aged, were MCI patients whose cognitive impairment was comparatively mild.Conclusions There exists significant differences between the different episode memory tests in identifying MCI in Chinese culture.The delayed memory of AVLT, being the most sensitive, could be used to determine whether the patient had the objective evidence of memory impairment.
A Multi-Modal and Multi-Atlas Integrated Framework for Identification of Mild Cognitive Impairment

Abstract:Background: Multi-modal neuroimaging with appropriate atlas is vital for effectively differentiating mild cognitive impairment (MCI) from healthy controls (HC). Methods: The resting-state functional magnetic resonance imaging (rs-fMRI) and structural MRI (sMRI) of 69 MCI patients and 61 HC subjects were collected. Then, the gray matter volumes obtained from the sMRI and Hurst exponent (HE) values calculated from rs-fMRI data in the Automated Anatomical Labeling (AAL-90), Brainnetome (BN-246), Harvard–Oxford (HOA-112) and AAL3-170 atlases were extracted, respectively. Next, these characteristics were selected with a minimal redundancy maximal relevance algorithm and a sequential feature collection method in single or multi-modalities, and only the optimal features were retained after this procedure. Lastly, the retained characteristics were served as the input features for the support vector machine (SVM)-based method to classify MCI patients, and the performance was estimated with a leave-one-out cross-validation (LOOCV). Results: Our proposed method obtained the best 92.00% accuracy, 94.92% specificity and 89.39% sensitivity with the sMRI in AAL-90 and the fMRI in HOA-112 atlas, which was much better than using the single-modal or single-atlas features. Conclusion: The results demonstrated that the multi-modal and multi-atlas integrated method could effectively recognize MCI patients, which could be extended into various neurological and neuropsychiatric diseases.

Reliability and Validity of the Chinese Version of Mild Behavioral Impairment Checklist in Mild Cognitive Impairment and Mild Alzheimer's Disease.

Abstract:BACKGROUND Mild behavioral impairment (MBI) has been proposed as an early manifestation of dementia. The Mild Behavioral Impairment Checklist (MBI-C) may help identify MBI in prodromal and preclinical dementia. OBJECTIVE The study aimed to evaluate the reliability and validity of the Chinese version of MBI-C in mild cognitive impairment (MCI) and mild Alzheimer's disease (AD), and to explore the structure of the five factors of the MBI-C in Chinese culture. METHODS Sixty dyads of MCI and mild AD (MCI, n = 33; mild AD, n = 35) were recruited. The informants completed the MBI-C and Neuropsychiatric Inventory Questionnaire (NPI-Q) and were interviewed for clinician rating of the NPI. The Cronbach's coefficient was used to measure the structural reliability of the MBI-C. The criterion-validity was evaluated with the correlation coefficient between the MBI-C and the total scores of NPI-Q and NPI. Exploratory factor analysis was conducted to investigate the structure of the MBI-C. RESULTS The Cronbach's α coefficient was 0.895. The MBI-C total score was positively correlated with all five domains (r = 0.577∼0.840). The total score of MBI-C was significantly correlated with the total scores of NPI-Q (r = 0.714) and NPI (r = 0.749). Similarly, the five domain scores of MBI-C were significantly correlated with the factor and total scores of NPI-Q (r = 0.312∼0.673) and NPI (r = 0.389∼0.673). The components of each factor in Chinese version of MBI-C were slightly different from those of the a priori defined domains (χ2 = 1818.202, df = 496, p < 0.001). CONCLUSION The Chinese version of MBI-C has good reliability and validity, and can be used to evaluate the psychological and behavioral changes in MCI and mild AD.
Investigation on the Alteration of Brain Functional Network and Its Role in the Identification of Mild Cognitive Impairment

Abstract:Mild cognitive impairment (MCI) is generally regarded as a prodromal stage of Alzheimer's disease (AD). In coping with the challenges caused by AD, we analyzed resting-state functional magnetic resonance imaging data of 82 MCI subjects and 93 normal controls (NCs). The alteration of brain functional network in MCI was investigated on three scales, including global metrics, nodal characteristics, and modular properties. The results supported the existence of small worldness, hubs, and community structure in the brain functional networks of both groups. Compared with NCs, the network altered in MCI over all the three scales. In scale I, we found significantly decreased characteristic path length and increased global efficiency in MCI. Moreover, altered global network metrics were associated with cognitive level evaluated by neuropsychological assessments. In scale II, the nodal betweenness centrality of some global hubs, such as the right Crus II of cerebellar hemisphere (CERCRU2.R) and fusiform gyrus (FFG.R), changed significantly and associated with the severity and cognitive impairment in MCI. In scale III, although anatomically adjacent regions tended to be clustered into the same module regardless of group, discrepancies existed in the composition of modules in both groups, with a prominent separation of the cerebellum and a less localized organization of community structure in MCI compared with NC. Taking advantages of random forest approach, we achieved an accuracy of 91.4% to discriminate MCI patients from NCs by integrating cognitive assessments and network analysis. The importance of the used features fed into the classifier further validated the nodal characteristics of CERCRU2.R and FFG.R could be potential biomarkers in the identification of MCI. In conclusion, the present study demonstrated that the brain functional connectome data altered at the stage of MCI and could assist the automatic diagnosis of MCI patients.

Abstract:Background: The pathogenesis of dementia often starts several years prior to clinical onset during which the individual is asymptomatic. Existing strategies for the accurate diagnosis of early dementia are limited by high cost and the invasive nature of the procedures. Eye movement parameters associated with cognitive functions may be helpful in the early identification of dementia and in the development and evaluation of preventive and therapeutic strategies.Objective: We aimed to assess differences in eye movement parameters between healthy elderly individuals and patients with mild cognitive impairment (MCI). Furthermore, we examined the correlations between eye movement parameters with cognitive functions and specific hemispheric region and neural structures in individuals with MCI.Method: Eighty individuals with MCI without dementia (based on DSM-IV criteria) identified by community screening and 170 healthy controls were administered Chinese versions of MoCA and NTB, and a long (20 min) or short (5 min) version of a visual paired comparison (VPC) task. Two weeks later, 44 MCI patients and 107 healthy controls completed a retest of the VPC task, 44 MCI patients and 43 healthy controls among them administered a MRI. At the end of 1-year follow-up, a subset of 26 individuals with MCI and 57 healthy controls were administered the long version of VPC task and MoCA test again. Eye movement parameters and the relationship of eye movement parameters with cognitive functions and with changes in neural structures were compared between groups.Results: Patients with MCI were older, had less education, and had lower scores on cognitive tests than healthy controls. After adjustment for age and level of education, patients with MCI had lower novelty preference scores on the VPC than healthy controls. Using the logistic regression model, the amount of time that subjects focused on these novel images could predict MCI patients from normal elderly with an out of sample area under the receiver operator characteristic curve of 0.62. Furthermore, the cognition score of subjects whose novelty preference score was low decreased more remarkably in 1 year. For both the patient and control groups, VPC novelty preference was significantly correlated with verbal fluency and delayed and short-term memory function. Novelty preference score was also significantly correlated with the cortical thickness of several structures in the right hemisphere.Conclusion: Eye movement parameters are stable indicators to distinguish patients with MCI and cognitively normal subjects and are not affected by different testing versions and numbers. Additionally, the patients' cognitive deficits and eye movement indices were correlated. Future longitudinal studies should further explore the clinical utility of eye movement parameters as early markers of MCI.

Abstract:Objective To explore the neuropsychological and clinical characters of amnestic mild cognitive impairment(MCI) and search the risk factor of MCI.Methods Cognitive function of 65 elders in community (above 60 years old) were assessed by using Cambridge cognitive examination Chinese version (CAMCOG-C), Mini mental state examination (MMSE), Clinical dementia rating (CDR), Geriatric depression scale (GDS), Activity of Daily Living Scale (ADL) and Logical memory of Wechsler memory scale Chinese version (WMS-C). Among these elders 20 were diagnosed as having amnesic mild cognitive impairment and their cognitive function was compared with 23 normal elderly controls.Results There are more females (80%) and illiterate persons (85%) in the MCI group. The values of all scales in the MCI group were lower than those in the control group. Of which the difference in total scores of MMSE, CAMCOG-C, orientation, language, memory, and perception; as well as the subscale in place orientation, linguistic expression, action expression, and visual perception were significantly different (P<0.05). Conclusion Lower education background is a risk factor for elders to take place MCI. Elders with amnestic MCI also display deficits in orientation, language and perception.
The Prevalence of Mild Cognitive Impairment among Chinese People: A Meta-Analysis

Abstract:Background: Mild cognitive impairment (MCI) induced the majority number of dementia patients. The prevalence of MCI in China varied across studies with different screening tools and diagnostic criteria. Objective: A systematic review and meta-analysis was conducted to estimate the pooled MCI prevalence among the population aged 55 years and older in China. Methods: PubMed, EMBASE, CNKI, Wanfang, CQVIP, and CBMdisc were searched for studies on prevalence of MCI among Chinese elderly between January 1, 1980, and February 10, 2020. The quality assessment was conducted via external validity, internal validity, and informativity, the pooled prevalence was calculated through the random-effect model, and the homogeneity was evaluated by Cochran’s Q test and I 2 . Results: Fifty-three studies with 123,766 subjects were included. The pooled prevalence of MCI among Chinese elderly was 15.4% (95% CI: 13.5–17.4%). Subgroup analyses indicated that the prevalence calculated with different screening tools was 20.2% (95% CI: 15.1–25.9%) for Montreal Cognitive Assessment (MoCA) and 13.0% (95% CI: 10.7–15.5%) for Mini-Mental State Examination (MMSE). According to different diagnostic criteria, the prevalence was 14.8% (95% CI: 12.2–17.6%) for Petersen criteria, 15.0% (95% CI: 12.7–17.5%) for DSM-IV, and 21.2% (95% CI: 17.5–25.2%) for Chinese Expert Consensus on Cognitive Impairment (CECCI). Besides, women, older adults, illiterate people, rural residents, and those who lived with unhealthy lifestyles and morbidity showed higher prevalence. Conclusions: The prevalence of MCI in China was 15.4%, which varied by demographics, lifestyles, morbidity, screening tools, and diagnostic criteria. In further studies, screening tools and diagnosis criteria should be considered when estimating MCI prevalence.
