Abstract:BACKGROUND:Odor memory is an important field of clinical research for its distinctive characteristics, which differ from those of other sensory systems. To date, several tests have been implemented for the assessment of odor memory. Despite a range of studies demonstrating the importance of verbal mediation in odor memory, few have distinguished odor memory performance in different odor verbalization levels.NEW METHOD:We aimed to develop a standardized odor memory test toolbox with one group of odors that are easily verbally identified and the other group of odors that are difficult to identify. The test contained two odor categories (high- and low-verbalizability odors), each consisting of three subtests (short- and long-term memory and working memory).RESULTS:Satisfactory test-retest reliability and solid validity of the odor recognition and working memory test were shown in both odor categories. Moreover, people scored significantly better with high- than low-verbalizability odors. A negative age effect on odor memory performance was also found.COMPARISON WITH EXISTING METHODS:No previous odor memory test distinguished odor memory performance in different odor verbalization levels, while the Olfactory Memory Test Battery (OMTB) contains high- and low-verbalizability odors and each category has three subtests.CONCLUSIONS:The present study indicated the OMTB is a comprehensive assessment of odor memory with good reliability and validity. All subtests can be used separately or in combination with each other according to the clinical and research needs.
A computer-controlled olfactometer for a self-administered odor identification test

Abstract:Odor identification tests are widely used for the general screening of olfactory function. However, the administration of odor identification tests is often limited due to a lack of investigators’ time. Therefore, we attempted to design a computer-controlled olfactometer to present a self-administered odor identification test. The results produced by means of this olfactometer were evaluated in terms of validity and test–retest reliability. To test the validity, participants’ performance in the odor identification test using the olfactometer was compared with their performance in the odor identification test using the validated assessment of the “Sniffin’ Sticks” test. The ten-item odor identification test was performed two times using two different methods: (1) the self-administered test using the computer-controlled olfactometer and (2) the foreign-administered test using the “Sniffin’ sticks.” To examine test–retest reliability, 20 participants were asked to repeat these tests on a different day. Participants reached significantly higher scores on a foreign-administered odor identification test using the “Sniffin’ sticks” than on the olfactometer-based test; however, this effect was driven by two less correctly identified odors in the olfactometer-based test. The significant difference between both methods in the mean scores disappeared after excluding two odors from the analysis. In addition, both methods showed no significant difference in scores obtained during the first and second session, indicating that results were consistent between sessions. In conclusion, our findings demonstrate that the computer-controlled olfactometer designed in this study can be used for a self-administered odor identification test.

The Sniffin’ Sticks Odor Discrimination Memory Test: A Rapid, Easy-to-Use, Reusable Procedure for Testing Olfactory Memory

Abstract:Objectives: Neurodegenerative diseases can alter odor memory in addition to inducing quantitative impairment, and olfactory memory can be tested using the validated olfactory memory test with encapsulated odors (Odor Discrimination Memory Test [ODMT]). The aim of this study was to investigate the new Sniffin’ Sticks ODMT (SSODMT), which is comparable with the ODMT. Methods: The SSODMT was administered to 48 subjects (30 women, 18 men; mean age, 34.6 ± 16.3 years). Olfactory function was assessed using the 16-item Sniffin’ Sticks Identification Test. The ODMT was administered to a subgroup of 10 subjects to assess the comparability of the SSODMT and the ODMT. Test-retest reliability was studied in another 20 subjects (10 women, 10 men; mean age, 32.9 ± 16.4 years). Results: The mean SSODMT score was 10.7 ± 1.8 (range, 5-12). SSODMT test scores were significantly correlated with Sniffin’ Sticks Identification Test scores ( r 67 = 0.66, P < .001) and were significantly negatively correlated with subject age ( r 48 = −0.70, P < .001). SSODMT and ODMT scores were highly and significantly correlated, indicating that the test results were comparable ( r 10 = 0.75, P < .05). Test-retest analysis revealed a significant correlation ( r 20 = 0.68, P < .001). Conclusions: These results demonstrate the usefulness of the new SSODMT and its comparability with the ODMT. The SSODMT is an easy, rapid, and low-cost tool that is suitable for routine use in evaluating odor memory.
Self-assessment of olfactory function using the “Sniffin’ Sticks”

Abstract:Abstract Background A precise and reliable test of the olfactory function is indispensable for the diagnosis of the olfactory disorder (OD). Despite of this, in a clinical context, often there is no place in daily routine for time-consuming procedures. This study aimed to examine if the assessment of olfactory function using the “Sniffin’ Sticks” is suitable for self-assessment. Methods Participants comprised 84 healthy control subjects (HC) and 37 OD patients. The “Sniffin’ Sticks” test battery consisting of odor threshold (T), discrimination (D) and identification (I) tests was used for self- and assisted assessments. To save time, we applied the 8-item wide step version of the T test and the 8-item D test, whereas the I task remained the same as the original version. The whole test included two sessions, with each session comprising a self-assessment part performed by the participants themselves, and an assisted-assessment part performed by the examiner. Results Sniffin’ Sticks self-assessment was efficient in distinguishing between self-reported HC subjects and OD patients ( p ’s 0.05). In the self-administered I and TDI tests, there was a moderate to excellent test–retest reliability (ICC = 0.51–0.93, p ’s < 0.01), and a strong to excellent correlation with the assisted assessment ( r = 0.71–0.92, p ’s < 0.01). However, the self-administered T and D tests only exhibited low to moderate test–retest reliability (ICC = 0.30–0.72, p ’s < 0.05) and correlations with the assisted test ( r = 0.31–0.62, p ’s < 0.05). Conclusions The Identification self-test is appropriate to be solely applied, and is therefore an easy-to-use alternative for olfactory screening in a larger segment of patients. The whole “Sniffin’ Sticks” self-test also shows good measurement properties and is therefore a suitable backup in clinical practice, but improvement is needed due to the simplified D and T self-test.

Abstract:Background Early identification of deficits in our ability to perceive odors is important as many normal (i.e., aging) and pathological (i.e., sinusitis, viral, neurodegeneration) processes can result in diminished olfactory function. To realistically enable population-level measurements of olfaction, validated olfaction tests must be capable of being administered outside the research laboratory and clinical setting. Aim The purpose of this study was to determine the feasibility of remotely testing olfactory performance using a test that was developed with funding from the National Institutes of Health as part of a ready-to-use, non-proprietary set of measurements useful for epidemiologic studies (NIH Toolbox Odor ID Test). Materials and methods Eligible participants older than 39 years and active (within 6 months) in the Brain Health Registry (BHR), an online cognitive assessment platform which connects participants with researchers, were recruited for this study. Interested participants were mailed the NIH Toolbox Odor ID Test along with instructions on accessing a website to record their responses. Data obtained from subjects who performed the test at home was compared to the normative data collected when the NIH Toolbox Odor ID Test was administered by a tester in a research setting and validated against the Smell Identification Test. The age-range and composition of the population ensured we had the ability to observe both age-related decline and gender-related deficits in olfactory ability, as shown in the experimental setting. Results We observed that age-associated olfactory decline and gender-associated performance was comparable to performance on the administered test. Self-administration of this test showed the age-related loss in olfactory acuity, F(4, 1156)=14.564, p<.0001 as well as higher accuracy for women compared to men after controlling for participants’ age, F(1, 1160) = 22.953, p <.0001. The effect size calculated as Hedge’s g, was 0.41. Conclusion These results indicate that the NIH Toolbox Odor ID Test is an appropriate instrument for self-administered assessment of olfactory performance. The ability to self-administer an inexpensive olfactory test increases its utility for inclusion in longitudinal epidemiological studies and when in-person testing is not feasible.

Abstract:Cost-effective, noninvasive screening methods for preclinical Alzheimer's disease (AD) and other neurocognitive disorders remain an unmet need. The olfactory neural circuits develop AD pathological changes prior to symptom onset. To probe these vulnerable circuits, we developed the digital remote AROMHA Brain Health Test (ABHT), an at-home odor identification, discrimination, memory, and intensity assessment. The ABHT was self-administered among cognitively normal (CN) English and Spanish speakers (n=127), participants with subjective cognitive complaints (SCC; n=34), and mild cognitive impairment (MCI; n=19). Self-administered tests took place remotely at home under unobserved (among interested CN participants) and observed modalities (CN, SCC, and MCI), as well as in-person with a research assistant present (CN, SCC, and MCI). Olfactory performance was similar across observed and unobserved remote self-administration and between English and Spanish speakers. Odor memory, identification, and discrimination scores decreased with age, and olfactory identification and discrimination were lower in the MCI group compared to CN and SCC groups, independent of age, sex, and education. The ABHT revealed age-related olfactory decline, and discriminated CN older adults from those with cognitive impairment. Replication of our results in other populations would support the use of the ABHT to identify and monitor individuals at risk for developing dementia. Odor dilution sorting as a clinical test of olfactory function: normative values and reliability data

Abstract:Abstract Clinical assessment of an individual's sense of smell has gained prominence, but its resource-intensive nature necessitates the exploration of self-administered methods. In this study, a cohort of 68 patients with olfactory loss and 55 controls were assessed using a recently introduced olfactory test. This test involves sorting two odorants (eugenol, phenylethyl alcohol) in five dilutions according to odor intensity, with an average application time of 3.5 minutes. The sorting task score, calculated as the mean of Kendall's Tau between the assigned and true dilution orders and normalized to [0,1], identified a cutoff for anosmia at a score ≤ 0.7. This cutoff, which marks the 90th percentile of scores obtained with randomly ordered dilutions, had a balanced accuracy of 89% (78-97%) for detecting anosmia, comparable to traditional odor threshold assessments. Retest evaluations suggested a score difference of ± 0.15 as a cutoff for clinically significant changes in olfactory function. In conclusion, the olfactory sorting test represents a simple, self-administered approach to the detection of anosmia or preserved olfactory function. With balanced accuracy similar to existing brief olfactory tests, this method offers a practical and user-friendly alternative for screening anosmia, addressing the need for resource-efficient assessments in clinical settings.

Abstract:Abstract Clinical tests assessing olfactory performance have become indispensable for diagnosing olfactory dysfunction. As time and personnel resources are limited, it would be advantageous to have shorter protocols focusing on singular aspects of olfactory performance, such as odor identification. However, such a unidimensional approach is often inconclusive and needs further tests (and tools). Hence, new testing methods with high levels of sensitivity, specificity, and reproducibility are required for clinical practice. Here, we developed a S niffin’ S ticks o dor mix ture identification test method (SSomix), with emphasis on resource efficiency and simplicity of administration. SSomix consists of mixtures of two and three odors applied onto a piece of paper using 11 out of 16 items from the original Sniffin’ Sticks identification test kit. A total of 66 healthy subjects and 22 patients with olfactory dysfunction were included in the study. SSomix showed good to excellent test-retest reliability and validity. The area under the receiver operating characteristics curves indicated good diagnostic accuracy in identifying patients with reduced and severely impaired olfactory function. SSomix was a suitable downsizing of the original kit, especially regarding resource efficiency.

Abstract:Tools for measuring olfactory function in adults have been well established. Although studies have shown that olfactory impairment in children may occur as a consequence of a number of diseases or head trauma, until today no consensus on how to evaluate the sense of smell in children exists in Europe. Aim of the study was to develop a modified "Sniffin' Sticks" odor identification test, the "Sniffin' Kids" test for the use in children. In this study 537 children between 6-17 years of age were included. Fourteen odors, which were identified at a high rate by children, were selected from the "Sniffin' Sticks" 16-item odor identification test. Normative date for the 14-item "Sniffin' Kids" odor identification test was obtained. The test was validated by including a group of congenital anosmic children. Results show that the "Sniffin' Kids" test is able to discriminate between normosmia and anosmia with a cutoff value of >7 points on the odor identification test. In addition the test-retest reliability was investigated in a group of 31 healthy children and shown to be ρ = 0.44. With the 14-item odor identification "Sniffin' Kids" test we present a valid and reliable test for measuring olfactory function in children between ages 6-17 years.

Abstract:The decline of olfactory function is an early indicator of neurodegenerative diseases, such as Alzheimer’s and Parkinson’s disease. An olfactory function assessment instrument using ten familiar scents with multiple-choice answers has been developed, but several items were inapplicable and had not been standardized. This study aimed to identify various scents that have not been standardized for the aging population with normal cognitive function. This research was a preliminary study with a descriptive cross-sectional approach conducted in Kalianyar village, Jakarta, involving 23 elderly participants with normal cognitive function. An instrument consisting of a list of scents was employed in two steps, with or without assistance. Initially, participants had one chance to smell and try to guess the type of scent given. If the scent was not guessed correctly, participants were then provided with multiple-choice options as a hint. Based on the assessment of twelve scents from the existing research, only nine scents were recognizable. Among them, coffee, lemongrass, oranges, eucalyptus, and menthol were the scents with the most correct answers without assistance. Additionally, there were four scents recognized with or without assistance, such as jasmine, lemon, aromatic ginger, and lime. This study found that only nine scents were familiar and had the potential to be utilized in the future as a screening tool for cognitive impairment.

Abstract:The clinical diagnostics of olfactory dysfunction in children turns out to be challenging due to low attention span, insufficient linguistic development and lack of odor experiences. Several smell tests have been developed for adults. Most of these examinations take a relatively long time and require a high level of concentration. Therefore, the aim of the current study was to evaluate an odor discrimination and olfactory threshold test using the frequently used “Sniffin’ Sticks” in children and adolescents in a simplified two-alternative-forced-choice version (2AFC) and compare it to the original three-alternative-forced-choice test (3AFC). One-hundred-twenty-one healthy participants aged between 5 and 17 years took part in this study. Within each of the two sessions participants underwent olfactory testing using the modified 2AFC as well as the standard 3AFC method. A better test-retest reliability was achieved using the original 3AFC method compared to the modified 2AFC. This was true for the odor discrimination as well as the olfactory threshold. Age had a significant influence on both tests, which should be considered when testing young children. We discuss these findings with relation to the existing norms and recommend using the 3AFC version due to a better test-retest reliability to measure olfactory function in children.

Abstract:To improve our understanding of the perception of odors, researchers are often required to undertake experimental procedures with users exposed to multiple odors in a variety of settings, including to diagnose smell loss in clinics and care homes. Existing smell tests are typically administered using multiple sniffing pens, manually presented to patients by a highly specialized nurse using a time-consuming and complex testing paradigm. Automated odor delivery devices, such as olfactometer systems, exist but are expensive, bulky and typically lab based, making them difficult to use for on the ground odor delivery. Here we have developed a portable, affordable, odor delivery device that can deliver 24 odors through individual channels with high temporal precision and without cross-contamination. The device allows for the rapid, flexible sequencing of odors via digital control using a mobile application and has been experimentally validated in the lab, as well as tested on patients. The design provides several advantages for investigating olfactory perception and offers the possibility that users can one day self-administer smell tests in a range of settings, including at home, allowing smell healthcare services to evolve and become part of a routine practice and self-care culture.
Pediatric olfactory home testing using regular household items: A cross-over validation study

Abstract:Objective: To develop an olfactory test that can be conducted by an untrained person using common household items and to introduce a German version of the Chemosensory Pleasure Scale for Children (CPS-C(de)). Methodology: This olfactory home test was developed in phases including evaluation of odors for suitability in a home setting. Parents of 50 children (ages 6-17) were then equipped with instruction manuals and participants were tested twice in a cross-over design. A validated pediatric olfactory test (the Universal Sniff test (U-Sniff)) served as the comparative gold standard. Additionally, a Chinese-English-German "back-and-forth" translation was conducted to establish the CPS-C(de) and was tested for empirical validity. Results: Fourteen items were tested for feasibility, and all were identified on a sufficient rate (≥66%, mean ± SD of 93.3% ± 9.5%). Bland Altman Plot analysis between home testing and the U-Sniff test was nearly identical (bias = 0.04). CPS-C(de) showed similar results to the original Chinese version and a moderate correlation was found between CPS-C(de) scores and Body Mass Index of children (r30 = -0.527, p = 0.003). Conclusions: Remote olfactory testing in children using household items is feasible. The CPS-C(de) may be of value for future olfactory studies.
Development of a Simplified Smell Test to Identify Patients with Typical Parkinson's as Informed by Multiple Cohorts, Machine Learning and External Validation

Abstract:Background: Reduced olfaction is a common feature of patients with typical Parkinson disease (PD). We sought to develop and validate a simplified smell test as a screening tool to help identify PD patients and explore its differentiation from other forms of parkinsonism. Methods: We used the Sniffin' Sticks Identification Test (SST-ID) and the University of Pennsylvania Smell Identification Test (UPSIT), together with data from three case-control studies, to compare olfaction in 301 patients with PD or dementia with Lewy bodies (DLB) to 36 subjects with multiple system atrophy (MSA), 32 individuals with progressive supranuclear palsy (PSP) and 281 neurologically healthy controls. Individual SST-ID and UPSIT scents were ranked by area under the receiver operating characteristic curve (AUC) values for group classification, with 10-fold cross-validation. Additional rankings were generated by leveraging results from eight published studies, collectively including 5,853 unique participants. Lead combinations were further validated using (semi-)independent datasets. An abbreviated list of scents was generated based on those shared by SST-ID and UPSIT. Findings: We made the following five observations: (i) PD and DLB patients generally had worse olfaction than healthy controls, as published, with scores for MSA and PSP patients ranking as intermediate. (ii) SST-ID and UPSIT scents showed distinct discriminative performances, with the top odorants (licorice, banana, clove, rose, mint, pineapple and cinnamon) confirmed by external evidence. (iii) A subset of only seven scents demonstrated a similar performance to that of the complete 16-scent SST-ID and 40-scent UPSIT kits, in both discovery and validation steps. Seven scents distinguished PD/DLB subjects from healthy controls with an AUC of 0.87 (95%CI 0.85-0.9) and PD/DLB from PSP/MSA patients with an AUC of 0.73 (95%CI 0.65-0.8) within the three cohorts (n=650). (iv) Increased age was associated with a decline in olfaction. (v) Males generally scored lower than females, although this finding was not significant across all cohorts. Interpretation: Screening of subjects for typical Parkinson's-associated hyposmia can be carried out with a simplified scent identification test that relies on as few as seven specific odorants. There, the discrimination of PD/DLB subjects vs. age-matched controls is more accurate than that of PD/DLB vs. PSP/MSA patients. Abstract:Abstract“Sniffin’ Sticks” is a test of nasal chemosensory performance that is based on penlike odor-dispensing devices. It is comprised of three tests of olfactory function: tests for odor threshold, discrimination and identification. Previous work has already established its test-retest reliability and validity in comparison to established measures of olfactory sensitivity. The results of this test are presented as a composite TDI score – i.e., the sum of results obtained for threshold, discrimination and identification measures. The present multicenter investigation aimed at providing normative values in relation to different age groups. To this end, 966 patients were investigated in 11 centers. An additional study tried to establish values for the identification of anosmic patients, with 70 anosmics investigated in five specialized centers where the presence of anosmia was confirmed by means of olfactory evoked potentials. For healthy subjects, the TDI score at the 10th percentile was 24.5 in subjects younger than 15 years, 30.3 for ages from 16 to 35 years, 28.8 for ages from 36 to 55 years and 27.5 for subjects older than 55 years. While these data can be used to estimate individual olfactory abilities in relation to a subject’s age, hyposmia was defined as the 10th percentile score of 16- to 35-year-old subjects. Our latter study revealed that none of 70 anosmics reached a TDI score higher than 15. This score of 15 is regarded as the cut-off value for functional anosmia. These results provide the basis for the routine clinical evaluation of patients with olfactory disorders using “Sniffin’ Sticks.”

Abstract:IntroductionAssessment of olfactory performance is of high clinical interest in the contexts of smell loss as well as neurological diseases, and recently gained attention in obesity research. Available olfactory tests, especially for assessing olfactory sensitivity, are time-consuming and require high cognitive capacity. Therefore, we aimed to establish a short procedure for reliably testing olfactory sensitivity using a subtest of the “Sniffin’ Sticks” battery. Evaluation criteria are test duration, validity, and test-retest reliability.MethodsIn a preliminary study using a within-subject repeated-measures design, we measured olfactory sensitivity for n-butanol in 20 young and healthy participants. We compared sensitivity obtained with three different measures during two sessions in a pseudo-randomized order: a standard single-staircase three-alternative forced-choice procedure with seven reversals (SSP_7); an abbreviated version with five reversals (SSP_5); and an ascending presentation of 16 dilution steps from lowest to highest odor concentration (brief ascending procedure, BAP).ResultsCompared to the SSP_7, the BAP was 51%, and the SSP_5 26% shorter in duration. Both the BAP and SSP_5 scores were highly correlated with the SSP_7. The test-retest reliability in all three tests was similar to that typically reported in olfactory research.ConclusionThe abbreviated tests are valid measures of olfactory sensitivity. Especially, the BAP is as reliable as the standard method, but remarkably faster and easier to perform.ImplicationsThus, the short procedures bear potential for both research and clinical practice, especially for complex study designs with time constraints on olfactory testing and for patient populations with attention deficits.

Abstract:The development of the first standardized "scratch 'n sniff" olfactory test is described. Over 1600 subjects participated in five experiments. In Experiment 1, 50 microencapsulated odorants were rated as to their intensity, pleasantness, irritation, coolness, and familiarity, and two procedures for releasing them were compared. In Experiment 2, the results of the first experiment and other data were used in the development of the test, which was administered to a large number of subjects. Using multiple regression analysis, scores on this test were shown to be significantly related to the subjects' gender, ethnic background, and smoking behavior. Average test scores decreased as a function of age, with the greatest decline occurring between the sixth and tenth decades of life. These age-related changes were not correlated with scores on the Wechsler Memory Scale. Women performed better than men within all age categories. In Experiment 3, the test was shown to differentiate between subjects with known olfactory disorders (e.g., Kallmann's syndrome; Korsakoff's syndrome) and normal controls, and to reliably detect persons instructed to feign total anosmia. In Experiment 4, the test-retest reliability was established (6-month interval; r = 0.918, p less than 0.001), and in Experiment 5 the test was shown to correlate thresholds with odor detection (r = -0.794, p less than 0.001). This self-administratered test now makes it possible to rapidly and accurately assess general olfactory function in the laboratory, clinic, or through the mail without complex equipment or space-consuming stores of chemicals.

Abstract:Abstract Introduction Basic studies suggest that olfactory dysfunction and functional near-infrared spectroscopy (fNIRS) can be used as tools for the diagnosis of mild cognitive impairment (MCI); however, real-world evidence is lacking. We investigated the potential diagnostic efficacy of olfactory-stimulated fNIRS for early detection of MCI and/or Alzheimer disease (AD). Methods We conducted a patient-level, single-group, diagnostic interventional trial involving elderly volunteers (age >60 years) suspected of declining cognitive function. Patients received open-label olfactory-stimulated fNIRS for measurement of oxygenation difference in the orbitofrontal cortex. All participants underwent amyloid PET, MRI, Mini-Mental State Examination (MMSE), and Seoul Neuropsychological Screening Battery (SNSB). Results Of 97 subjects, 28 (28.9%) were cognitively normal, 32 (33.0%) had preclinical AD, 21 (21.6%) had MCI, and 16 (16.5%) had AD. Olfactory-stimulated oxygenation differences in the orbitofrontal cortex were associated with cognitive impairment; the association was more pronounced with cognitive severity. Olfactory-stimulated oxygenation difference was associated with MMSE (adjusted β [ aβ ] 1.001; 95% CI 0.540−1.463), SNSB language and related function ( aβ , 1.218; 95% CI , 0.020−2.417), SNSB memory ( aβ , 1.963; 95% CI , 0.841−3.084), SNSB frontal/executive function ( aβ , 1.715; 95% CI , 0.401−3.029) scores, standard uptake value ratio from amyloid PET ( aβ , −10.083; 95% CI , −19.063 to −1.103), and hippocampal volume from MRI ( aβ , 0.002; 95% CI , 0.001−0.004). Olfactory-stimulated oxygenation difference in the orbitofrontal cortex was superior in diagnosing MCI and AD (AUC, 0.909; 95% CI , 0.848−0.971), compared to amyloid PET (AUC, 0.793; 95% CI , 0.694−0.893) or MRI (AUC, 0.758; 95% CI , 0.644−0.871). Discussion Our trial showed that olfactory-stimulated oxygenation differences in the orbitofrontal cortex detected by fNIRS were associated with cognitive impairment and cognitive-related objectives. This novel approach may be a potential diagnostic tool for patients with MCI and/or AD. Trial registration CRIS number, KCT0006197 .

Abstract:BACKGROUND:Olfactory dysfunction appears prior to cognitive decline, and thus it has been suggested to be an early predictor of Alzheimer's disease. However, it is currently not known whether and how olfactory threshold test could serve as a quick screening tool for cognitive impairment. OBJECTIVE:To define olfactory threshold test for screening cognitive impairment in two independent cohorts. METHODS:The participants are comprised of two cohorts in China, 1,139 inpatients with type 2 diabetes mellitus (T2DM, Discovery cohort) and 1,236 community-dwelling elderly (Validation cohort). Olfactory and cognitive functions were evaluated by Connecticut Chemosensory Clinical Research Center test and Mini-Mental State Examination (MMSE), respectively. Regression analyses and receiver operating characteristic (ROC) analyses were carried out to determine the relation and discriminative performance of the olfactory threshold score (OTS) regarding identification of cognition impairment. RESULTS:Regression analysis showed that olfactory deficit (reducing OTS) was correlated with cognitive impairment (reducing MMSE score) in two cohorts. ROC analysis revealed that the OTS could distinguish cognitive impairment from cognitively normal individuals, with mean area under the curve values of 0.71 (0.67, 0.74) and 0.63 (0.60, 0.66), respectively, but it failed to discriminate dementia from mild cognitive impairment. The cut-off point of 3 showed the highest validity for the screening, with the diagnostic accuracy of 73.3% and 69.5%. CONCLUSION:Reducing OTS is associated with cognitive impairment in T2DM patients and the community-dwelling elderly. Therefore, olfactory threshold test may be used as a readily accessible screening tool for cognitive impairment.
