Abstract:Background: Eye tracking (ET) is a powerful methodology for studying attentional processes through quantification of eye movements. The precision, usability, and cost-effectiveness of ET render it a promising platform for developing biomarkers for use in clinical trials for autism spectrum disorder (ASD). Methods: The autism biomarkers consortium for clinical trials conducted a multisite, observational study of 6-11-year-old children with ASD (n = 280) and typical development (TD, n = 119). The ET battery included: Activity Monitoring, Social Interactive, Static Social Scenes, Biological Motion Preference, and Pupillary Light Reflex tasks. A priori, gaze to faces in Activity Monitoring, Social Interactive, and Static Social Scenes tasks were aggregated into an Oculomotor Index of Gaze to Human Faces (OMI) as the primary outcome measure. This work reports on fundamental biomarker properties (data acquisition rates, construct validity, six-week stability, group discrimination, and clinical relationships) derived from these assays that serve as a base for subsequent development of clinical trial biomarker applications. Results: All tasks exhibited excellent acquisition rates, met expectations for construct validity, had moderate or high six-week stabilities, and highlighted subsets of the ASD group with distinct biomarker performance. Within ASD, higher OMI was associated with increased memory for faces, decreased autism symptom severity, and higher verbal IQ and pragmatic communication skills. Limitations: No specific interventions were administered in this study, limiting information about how ET biomarkers track or predict outcomes in response to treatment. This study did not consider co-occurrence of psychiatric conditions nor specificity in comparison with non-ASD special populations, therefore limiting our understanding of the applicability of outcomes to specific clinical contexts-of-use. Research-grade protocols and equipment were used; further studies are needed to explore deployment in less standardized contexts. Conclusions: All ET tasks met expectations regarding biomarker properties, with strongest performance for tasks associated with attention to human faces and weakest performance associated with biological motion preference. Based on these data, the OMI has been accepted to the FDA's Biomarker Qualification program, providing a path for advancing efforts to develop biomarkers for use in clinical trials.
Investigation of Eye-Tracking Scan Path as a Biomarker for Autism Screening Using Machine Learning Algorithms

Abstract:Autism spectrum disorder is a group of disorders marked by difficulties with social skills, repetitive activities, speech, and nonverbal communication. Deficits in paying attention to, and processing, social stimuli are common for children with autism spectrum disorders. It is uncertain whether eye-tracking technologies can assist in establishing an early biomarker of autism based on the children’s atypical visual preference patterns. In this study, we used machine learning methods to test the applicability of eye-tracking data in children to aid in the early screening of autism. We looked into the effectiveness of various machine learning techniques to discover the best model for predicting autism using visualized eye-tracking scan path images. We adopted three traditional machine learning models and a deep neural network classifier to run experimental trials. This study employed a publicly available dataset of 547 graphical eye-tracking scan paths from 328 typically developing and 219 autistic children. We used image augmentation to populate the dataset to prevent the model from overfitting. The deep neural network model outperformed typical machine learning approaches on the populated dataset, with 97% AUC, 93.28% sensitivity, 91.38% specificity, 94.46% NPV, and 90.06% PPV (fivefold cross-validated). The findings strongly suggest that eye-tracking data help clinicians for a quick and reliable autism screening.

Abstract:Autism spectrum disorder (ASD) is a complex brain developmental disorder characterized by troubles with social interaction and communication, and by restricted interest and repetitive stereotypical behaviors. Childhood is a critical development period in a human life associated with physical, cognitive and social-emotional development, and has also been considered as a key time window for early diagnosis and intervention of autism. In this article, we focus on investigating the development rules of autistic children and typically developing (TD) children using electroencephalography (EEG) signal analysis and eye tracking analysis by conducting the following two independent experiments, respectively. In the first experiment, the resting-state EEG data of 351 children aged ranging from 3 to 9 years (80 children with autism, 271 TD children) are collected. We calculated the relative power spectrum of EEG using the well-known Pwelch’s method and then established a linear model to explore the main effects of age and diagnosis group at each frequency band. The results showed that the relative power spectrum in the slow-wave band decreased with age both in children with autism and TD children. Notably, autistic children demonstrated that significant differences in the relative power in the alpha band from children in TD group. In the second experiment, a total of 293 children aged ranging from 3 to 9 years (104 children with autism, 189 TD children) were recruited to acquire the eye gaze data when watching a dynamic social scene video. Different areas of interest, including eye, mouth, body, background and joint attention, were considered, and the total fixation time of each area of interest was statistically computed for children with autism and TD children with an interval of one year. The results demonstrated that the proportion of fixation time of the joint attention and the mouth in children with autism was significantly reduced compared to TD children. Specifically, autistic children were the common attention deficit, while paying more attention to the background and the body. In summary, two experiments above have been performed to explore the potential objective biomarkers and behavior markers from different perspectives of neural oscillation and statistical features of eye-tracking fixation patterns. Both results have proven that EEG oscillation activity and eye gaze behavior can provide an objective evaluation of brain development for children with autism, and furthermore can reveal the developmental rules of EEG and eye gaze behavior patterns with age, which is helpful for clinical diagnosis and early intervention of ASD.

Abstract:Eye tracking is a promising tool for Autism Spectrum Disorder (ASD) detection in both children and adults. An important aspect of social communication is keeping eye contact, which is something that people with ASD frequently struggle with. Eye tracking can assess the duration of eye contact and the frequency and direction of gaze movements, offering quantifiable indicators of social communication deficits. People with ASD may also demonstrate other abnormalities in visual processing, such as an increased concentration on detail, sensory sensitivity, and trouble with complicated visual activities. These variations can be measured via Eye tracking, which offers critical information for the planning of therapy and diagnosis. The primary objective of this work is to provide a thorough description of the most recent studies that use Eye tracking combined with various Machine Learning (ML) and Deep Learning (DL) models for the detection of ASD. This will provide insights into the identification, and behavioral assessment, and distinguish between autistic people and those who are Typically Developing (TD). A detailed review of the various ML and DL models with their datasets and performance criteria is presented. Different types of eye movement datasets with diagnostic standards and eye tracker devices are also discussed. Finally, the study addresses the potential of gaze prediction in ASD patients for the design of interventions.

Abstract:Abstract Few clinically validated biomarkers of ASD exist which can rapidly, accurately, and objectively identify autism during the first years of life and be used to support optimized treatment outcomes and advances in precision medicine. As such, the goal of the present study was to leverage both simple and computationally-advanced approaches to validate an eye-tracking measure of social attention preference, the GeoPref Test, among 1,863 ASD, delayed, or typical toddlers (12–48 months) referred from the community or general population via a primary care universal screening program. Toddlers participated in diagnostic and psychometric evaluations and the GeoPref Test: a 1-min movie containing side-by-side dynamic social and geometric images. Following testing, diagnosis was denoted as ASD, ASD features, LD, GDD, Other, typical sibling of ASD proband, or typical. Relative to other diagnostic groups, ASD toddlers exhibited the highest levels of visual attention towards geometric images and those with especially high fixation levels exhibited poor clinical profiles. Using the 69% fixation threshold, the GeoPref Test had 98% specificity, 17% sensitivity, 81% PPV, and 65% NPV. Sensitivity increased to 33% when saccades were included, with comparable validity across sex, ethnicity, or race. The GeoPref Test was also highly reliable up to 24 months following the initial test. Finally, fixation levels among twins concordant for ASD were significantly correlated, indicating that GeoPref Test performance may be genetically driven. As the GeoPref Test yields few false positives (~ 2%) and is equally valid across demographic categories, the current findings highlight the ability of the GeoPref Test to rapidly and accurately detect autism before the 2nd birthday in a subset of children and serve as a biomarker for a unique ASD subtype in clinical trials.

Abstract:Social attention deficits are a hallmark characteristic within autism spectrum disorder (ASD) and have been hypothesized to have cascading effects on emotion recognition. Eye-tracking methodology has emerged as a potentially reliable, feasible, and sensitive biomarker for examining core phenotypic features of ASD; however, these findings are mixed with regards to measuring treatment change in clinical trials. The present study aimed to assess the utility of an eye-tracking paradigm to discriminate between clinical groups in social attention and emotion recognition through face scanning and pupillometry. The present study also assessed the reliability of this paradigm within the ASD sample to further our understanding of the utility of eye-tracking for future clinical trials. Participants included 42 individuals with ASD, 29 developmental disability (DD) controls, and 62 typically developing (TD) controls between 3 and 25 years of age. An emotional faces eye-tracking paradigm was administered to all participants, with the ASD group completing the paradigm a second time approximately 2 months later. Participants' average proportion of looking and number of fixations to specific areas of interest (AOI) were examined along with changes in pupil reactivity while viewing different emotional faces. Results suggest atypical face-scanning through a reduced proportion of looking and the number of fixations toward the eyes in the ASD group regardless of the emotion that was presented. Further, pupillometry measures were able to detect increases in pupil dilation to happy faces in the ASD group. Lastly, test-retest reliability coefficients varied between the poor and excellent range based on the mechanism assessed, with the proportion of looking demonstrating the highest reliability coefficients. These findings build on the promise of eye-tracking as a feasible and reliable biomarker for identifying social attention and emotion recognition deficits in ASD. Detecting differences in emotion recognition explicitly through facial scanning was not as clear. Specific mechanisms within the eye-tracking paradigm may be viable options for assessing treatment-specific outcomes.

Abstract:Altered patterns of visual social attention preference detected using eye-tracking and a variety of different paradigms are increasingly proposed as sensitive biomarkers for autism spectrum disorder. However, few eye tracking studies have compared the relative efficacy of different paradigms to discriminate between autistic compared with typically developing children and their sensitivity to specific symptoms. To target this issue, the current study used three common eye tracking protocols contrasting social versus non-social stimuli in young (2-7 years old) Chinese autistic (n = 35) and typically developing (n = 34) children matched for age and gender. Protocols included dancing people vs . dynamic geometrical images, biological motion (dynamic light point walking human or cat) vs . non-biological motion (scrambled controls) and child playing with toy vs . toy alone. Although all three paradigms differentiated autistic and typically developing children, the dancing people versus dynamic geometry pattern paradigm was the most effective, with autistic children showing marked reductions in visual preference for dancing people and correspondingly increased one for geometric patterns. Furthermore, this altered visual preference in autistic children was correlated with the ADOS social affect score and had the highest discrimination accuracy. Our results therefore indicate that decreased visual preference for dynamic social stimuli may be the most effective visual attention-based paradigm for use as a biomarker for autism in Chinese children. Abstract:Introduction: Early identification of Autism Spectrum Disorder (ASD) is critical for effective intervention. Restricted interests (RIs), a subset of repetitive behaviors, are a prominent but underutilized domain for early ASD diagnosis. This study aimed to identify objective biomarkers for ASD by integrating electroencephalography (EEG) and eye-tracking (ET) to analyze toddlers' visual attention and cortical responses to RI versus neutral interest (NI) objects. Methods: The study involved 59 toddlers aged 2-4 years, including 32 with ASD and 27 non-ASD controls. Participants underwent a 24-object passive viewing paradigm, featuring RI (e.g., transportation items) and NI objects (e.g., balloons). ET metrics (fixation time and pupil size) and EEG time-frequency (TF) power in theta (4-8 Hz) and alpha (8-13 Hz) bands were analyzed. Statistical methods included logistic regression models to assess the predictive potential of combined EEG and ET biomarkers. Results: Toddlers with ASD exhibited significantly increased fixation times and pupil sizes for RI objects compared to NI objects, alongside distinct EEG patterns with elevated theta and reduced alpha power in occipital regions during RI stimuli. The multimodal logistic regression model, incorporating EEG and ET metrics, achieved an area under the curve (AUC) of 0.75, demonstrating robust predictive capability for ASD. Discussion: This novel integration of ET and EEG metrics highlights the potential of RIs as diagnostic markers for ASD. The observed neural and attentional distinctions underscore the utility of multimodal biomarkers for early diagnosis and personalized intervention strategies. Future work should validate findings across broader age ranges and diverse populations.

New Eye Tracking Metrics System: The Value in Early Diagnosis of Autism Spectrum Disorder

Abstract:Background Eye tracking (ET) is emerging as a promising early and objective screening method for autism spectrum disorders (ASD), but it requires more reliable metrics with enhanced sensitivity and specificity for clinical use. Methods This study introduces a suite of novel ET metrics: Area of Interest (AOI) Switch Counts (ASC), Favorable AOI Shifts (FAS) along self-determined pathways, and AOI Vacancy Counts (AVC). These metrics were applied to toddlers and preschoolers diagnosed with ASD. The correlation between these new ET metrics and Autism Diagnostic Observation Schedule, Second Edition (ADOS-2) scores was assessed using linear regression. Sensitivity and specificity of the cut-off scores were also evaluated to predict diagnosis. Results Our findings indicate significantly lower FAS and ASC and higher AVC (P < 0.05) in children with ASD compared to their non-ASD counterparts within this high-risk cohort. There were no significant differences in total fixation time or pupil size (p > 0.05). Additionally, FAS was negatively correlated with ADOS-2 total scores and the social affect (SA) subscale (p < 0.05). Among these new ET metrics, AVC yielded the best sensitivity (88-100%) and specificity (80-88%) with a cut-off score of 0.305-0.306, followed by FAS and ASC for distinguishing ASD from non-ASD for diagnosis. Conclusions This study confirms the utility of innovative ET metrics FAS, AVC, and ASC, which exhibit markedly improved sensitivity and specificity, enhancing ASD screening and diagnostic processes.

Abstract:Importance: Atypical eye gaze is an early-emerging symptom of autism spectrum disorder (ASD) and holds promise for autism screening. Current eye-tracking methods are expensive and require special equipment and calibration. There is a need for scalable, feasible methods for measuring eye gaze. Objective: Using computational methods based on computer vision analysis, we evaluated whether an app deployed on an iPhone or iPad that displayed strategically designed brief movies could elicit and quantify differences in eye-gaze patterns of toddlers with ASD vs typical development. Design, setting, and participants: A prospective study in pediatric primary care clinics was conducted from December 2018 to March 2020, comparing toddlers with and without ASD. Caregivers of 1564 toddlers were invited to participate during a well-child visit. A total of 993 toddlers (63%) completed study measures. Enrollment criteria were aged 16 to 38 months, healthy, English- or Spanish-speaking caregiver, and toddler able to sit and view the app. Participants were screened with the Modified Checklist for Autism in Toddlers-Revised With Follow-up during routine care. Children were referred by their pediatrician for diagnostic evaluation based on results of the checklist or if the caregiver or pediatrician was concerned. Forty toddlers subsequently were diagnosed with ASD. Exposures: A mobile app displayed on a smartphone or tablet. Main outcomes and measures: Computer vision analysis quantified eye-gaze patterns elicited by the app, which were compared between toddlers with ASD vs typical development. Results: Mean age of the sample was 21.1 months (range, 17.1-36.9 months), and 50.6% were boys, 59.8% White individuals, 16.5% Black individuals, 23.7% other race, and 16.9% Hispanic/Latino individuals. Distinctive eye-gaze patterns were detected in toddlers with ASD, characterized by reduced gaze to social stimuli and to salient social moments during the movies, and previously unknown deficits in coordination of gaze with speech sounds. The area under the receiver operating characteristic curve discriminating ASD vs non-ASD using multiple gaze features was 0.90 (95% CI, 0.82-0.97). Conclusions and relevance: The app reliably measured both known and new gaze biomarkers that distinguished toddlers with ASD vs typical development. These novel results may have potential for developing scalable autism screening tools, exportable to natural settings, and enabling data sets amenable to machine learning.

Abstract:BackgroundAttention-deficit/hyperactivity disorder (ADHD) is a common neurodevelopmental disorder in school-aged children. The lack of objective biomarkers for ADHD often results in missed diagnoses or misdiagnoses, which lead to inappropriate or delayed interventions. Eye-tracking technology provides an objective method to assess children’s neuropsychological behavior. ObjectiveThe aim of this study was to develop an objective and reliable auxiliary diagnostic system for ADHD using eye-tracking technology. This system would be valuable for screening for ADHD in schools and communities and may help identify objective biomarkers for the clinical diagnosis of ADHD. MethodsWe conducted a case-control study of children with ADHD and typically developing (TD) children. We designed an eye-tracking assessment paradigm based on the core cognitive deficits of ADHD and extracted various digital biomarkers that represented participant behaviors. These biomarkers and developmental patterns were compared between the ADHD and TD groups. Machine learning (ML) was implemented to validate the ability of the extracted eye-tracking biomarkers to predict ADHD. The performance of the ML models was evaluated using 5-fold cross-validation. ResultsWe recruited 216 participants, of whom 94 (43.5%) were children with ADHD and 122 (56.5%) were TD children. The ADHD group showed significantly poorer performance (for accuracy and completion time) than the TD group in the prosaccade, antisaccade, and delayed saccade tasks. In addition, there were substantial group differences in digital biomarkers, such as pupil diameter fluctuation, regularity of gaze trajectory, and fixations on unrelated areas. Although the accuracy and task completion speed of the ADHD group increased over time, their eye-movement patterns remained irregular. The TD group with children aged 5 to 6 years outperformed the ADHD group with children aged 9 to 10 years, and this difference remained relatively stable over time, which indicated that the ADHD group followed a unique developmental pattern. The ML model was effective in discriminating the groups, achieving an area under the curve of 0.965 and an accuracy of 0.908. ConclusionsThe eye-tracking biomarkers proposed in this study effectively identified differences in various aspects of eye-movement patterns between the ADHD and TD groups. In addition, the ML model constructed using these digital biomarkers achieved high accuracy and reliability in identifying ADHD. Our system can facilitate early screening for ADHD in schools and communities and provide clinicians with objective biomarkers as a reference.

Abstract:Background Early identification of autism spectrum disorder (ASD) improves long-term outcomes, yet significant diagnostic delays persist. Methods A retrospective cohort of 449 children (ASD: 246, typically developing [TD]: 203) was used for model development. Eye-movement data were collected from the participants watching videos that featured eye-tracking paradigms for assessing social and non-social cognition. Five machine learning algorithms, namely random forest, support vector machine, logistic regression, artificial neural network, and extreme gradient boosting, were trained to classify children with ASD and TD. The best-performing algorithm was selected to build the final model which was further evaluated in a prospective cohort of 80 children. The Shapley values interpreted important eye-tracking features. Results Random forest outperformed other algorithms during model development and achieved an area under the curve of 0.849 (< 3 years: 0.832, ≥ 3 years: 0.868) on the external validation set. Of the ten most important eye-tracking features, three measured social cognition, and the rest were related to non-social cognition. A deterioration in model performance was observed using only the social or non-social cognition-related eye-tracking features. Limitations The sample size of this study, although larger than that of existing studies of ASD based on eye-tracking data, was still relatively small compared to the number of features. Conclusions Machine learning models based on eye-tracking data have the potential to be cost- and time-efficient digital tools for the early identification of ASD. Eye-tracking phenotypes related to social and non-social cognition play an important role in distinguishing children with ASD from TD children.

Abstract:Autism spectrum disorder (ASD) is one of the most common childhood developmental disorders. Early detection and intervention for ASD are critical for increasing child success. In the past decade, utilizing the abnormal eye gaze characteristics of children with autism in regard to certain visual stimuli is emerging as a screening approach due to its cost-efficiency and promising accuracy. However, the effect of visual stimulus on children with ASD has not been considered as a diagnostic consideration in the past. In this paper, we first create a visual stimuli database based on an extensive literature review, then we examine the impact of picture stimuli and exposure time on the quantitative accuracy of screenings for ASD. This is done by extracting gaze distribution in a 2D space and comparing children with ASD to typical peers using the 1st Wasserstein distance. A group of 32 participants with ASD and typical development (TD) were recruited for the study. The f-score accuracy results demonstrate the impact of implementing visual stimulus on screening for ASD. Our study demonstrates that the parsing of "social scene" stimulus with 5-second exposure time has the best performance at 98.24%.

Abstract:While the number of children with diagnosed autism spectrum disorder (ASD) continues to rise from year to year, there is still no universal approach to autism diagnosis and treatment. A great variety of different tools and approaches for the on-site diagnostic are available right now, however, a big percent of parents have no access to them and they tend to search for the available tools and correction programs on the Internet. Lack of money, absence of qualified specialists, and low level of trust to the correction methods are the main issues that affect the in-time diagnoses of ASD and which need to be solved to get the early treatment for the little patients. Understanding the importance of this issue our team decided to investigate new methods of the online autism diagnoses and develop the algorithm that will be able to predict the chances of ASD according to the information from the gaze activity of the child. The results that we got during the experiments show supported our idea that eye-tracking technology is one of the most promising tools for the early detection of the eye-movement features that can be markers of the ASD. Moreover, we have conducted a series of experiments to ensure that our approach has a reliable result on the cheap webcam systems. Thus, this approach can be used as an additional first screening tool for the home monitoring of the early child development and ASD connected disorders monitoring. The further development of eye-tracking based autism diagnosis has a big potential of usage and can be further implemented in the daily practice for practical specialists and parents.

Abstract:Children with autism spectrum disorder (ASD) and specific language impairment (SLI) exhibit serious social-emotional difficulties from early infancy which hamper the differential diagnosis of these two conditions. This review aims at analysing differences between ASD and SLI disorders in children from 0 to 12 years regarding emotional competence abilities using eye tracking methodology. The findings show specific markers in the ASD population (e.g. difficulties in facial emotion recognition, atypical visual scanning patterns, or non-social attentional orientation) and a notable scarcity of studies involving SLI children. Comparative research points at potential differences in visual scanning patterns. The conclusions support the use of this methodology for comparative studies between clinical populations.

Abstract:Over the past years, researchers have been using head-mounted eye-tracking systems to study young children's gaze behaviors in everyday activities through which children learn about the world. This method has great potential to further our understanding of how millisecond-level gaze behaviors create multisensory experiences and fluctuate around social environments. While this line of work can yield insight into early perceptual experiences and potential learning mechanisms, the majority of the work is exclusively conducted with typically-developing children. Sensory sensitivities, social-communication difficulties, and challenging behaviors (e.g., disruption, elopement) are common among children with developmental disorders, and they may represent potential methodological challenges for collecting high-quality data.

Abstract:Background: Children with autism spectrum disorder (ASD) have been observed to be associated with fixation abnormality as measured eye tracking, but the dynamics behind fixation patterns across age remain unclear. Materials and Methods: In this study, we investigated gaze patterns between toddlers and preschoolers with and without ASD while they viewed video clips and still images (i.e., mouth-moving face, biological motion, mouthing face vs. moving object, still face picture vs. objects, and moving toys). Results: We found that the fixation time percentage of children with ASD showed significant decrease compared with that of TD children in almost all areas of interest (AOI) except for moving toy (helicopter). We also observed a diagnostic group (ASD vs. TD) and chronological age (Toddlers vs. preschooler) interaction for the eye AOI during the mouth-moving video clip. Support vector machine analysis showed that the classifier could discriminate ASD from TD in toddlers with an accuracy of 80% and could discriminate ASD from TD in preschoolers with an accuracy of 71%. Conclusion: Our results suggest that toddlers and preschoolers may be associated with both common and distinct fixation patterns. A combination of eye tracking and machine learning methods has the potential to shed light on the development of new early screening/diagnosis methods for ASD.

Abstract:Background: The Extension for Community Health Outcomes (ECHO) Autism Program trains clinicians to screen, diagnose, and care for children with autism spectrum disorder (ASD) in primary care settings. This study will assess the feasibility and impact of integrating an artificial intelligence (AI)–based ASD diagnosis aid (the device ) into the existing ECHO Autism Screening Tool for Autism in Toddlers and Young Children (STAT) diagnosis model. The prescription-only Software as a Medical Device, designed for use in children aged 18 to 72 months at risk for developmental delay, produces ASD diagnostic recommendations after analyzing behavioral features from 3 distinct inputs: a caregiver questionnaire, 2 short home videos analyzed by trained video analysts, and a health care provider questionnaire. The device is not a stand-alone diagnostic and should be used in conjunction with clinical judgment. Objective: This study aims to assess the feasibility and impact of integrating an AI-based ASD diagnosis aid into the ECHO Autism STAT diagnosis model. The time from initial ECHO Autism clinician concern to ASD diagnosis is the primary end point. Secondary end points include the time from initial caregiver concern to ASD diagnosis, time from diagnosis to treatment initiation, and clinician and caregiver experience of device use as part of the ASD diagnostic journey. Methods: Research participants for this prospective observational study will be patients suspected of having ASD (aged 18-72 months) and their caregivers and up to 15 trained ECHO Autism clinicians recruited by the ECHO Autism Communities research team from across rural and suburban areas of the United States. Clinicians will provide routine clinical care and conduct best practice ECHO Autism diagnostic evaluations in addition to prescribing the device. Outcome data will be collected via a combination of electronic questionnaires, reviews of standard clinical care records, and analysis of device outputs. The expected study duration is no more than 12 months. The study was approved by the institutional review board of the University of Missouri-Columbia (institutional review board–assigned project number 2075722). Results: Participant recruitment began in April 2022. As of June 2022, a total of 41 participants have been enrolled. Conclusions: This prospective observational study will be the first to evaluate the use of a novel AI-based ASD diagnosis aid as part of a real-world primary care diagnostic pathway. If device integration into primary care proves feasible and efficacious, prolonged delays between the first ASD concern and eventual diagnosis may be reduced. Streamlining primary care ASD diagnosis could potentially reduce the strain on specialty services and allow a greater proportion of children to commence early intervention during a critical neurodevelopmental window. Abstract:We tested the potential for Gazefinder eye-tracking to support early autism identification, including feasible use with infants, and preliminary concurrent validity of trial-level gaze data against clinical assessment scores. We embedded the ~ 2-min 'Scene 1S4' protocol within a comprehensive clinical assessment for 54 consecutively-referred, clinically-indicated infants (prematurity-corrected age 9–14 months). Alongside % tracking rate as a broad indicator of feasible assessment/data capture, we report infant gaze data to pre-specified regions of interest (ROI) across four trial types and associations with scores on established clinical/behavioural tools. Most infants tolerated Gazefinder eye-tracking well, returning high overall % tracking rate. As a group, infants directed more gaze towards social vs. non-social (or more vs. less socially-salient) ROIs within trials. Behavioural autism features were correlated with increased gaze towards non-social/geometry (vs. social/people) scenes. No associations were found for gaze directed to ROIs within other stimulus types. Notably, there were no associations between developmental/cognitive ability or adaptive behaviour with gaze towards any ROI. Gazefinder assessment seems highly feasible with clinically-indicated infants, and the people vs. geometry stimuli show concurrent predictive validity for behavioural autism features. Aggregating data across the ~ 2-min autism identification protocol might plausibly offer greater utility than stimulus-level analysis alone.

Abstract:Purpose: Developmental language disorder (DLD) and autism sometimes appear as overlapping conditions in behavioral tests. There is much literature on the visual scanning pattern (VSP) of faces in autistic children, but this is scarce regarding those with DLD. The purpose of this study was to compare the VSP of faces in young children with DLD, those with autism, and typically developing peers, assessing the effect of three variables. Method: Two eye-tracking experiments were designed to assess the effect of the emotion and the poser's gender (Experiment 1) and the poser's age (Experiment 2) on the VSP of participants (Experiment 1: N = 59, age range: 32–74 months; Experiment 2: N = 58, age range: 32–74 months). We operationalized the VSP in terms of attentional orientation, visual preference, and depth of processing of each sort of face. We developed two paired preference tasks in which pairs of images of faces showing different emotions were displayed simultaneously to compete for children's attention. Results: Data analysis revealed two VSP markers common to both disorders: (a) superficial processing of faces and (b) late orientation to angry and child faces. Moreover, one specific marker for each condition was also found: typical preference for child faces in children with DLD versus diminished preference for them in autistic children. Conclusions: Considering the similarities found between children with DLD and those with autism, difficulties of children with DLD in attention to faces have been systematically underestimated. Thus, more effort must be made to identify and respond to the needs of this population. Clinical practice may benefit from the potential of eye-tracking methodology and the analysis of the VSP to assess attention to faces in both conditions. This would also contribute to the improvement of early differential diagnosis in the long run.
