Abstract:The aim of this study was to evaluate the stress distributions and deformations of the temporomandibular joint (TMJ) during different periods before and after sagittal split ramus osteotomy (SSRO). A three-dimensional finite element model of the mandible and TMJ was established, based on the preoperative CT of a patient with mandibular prognathism. Numerical SSRO was performed and the models of three postoperative periods were established. Contact elements were used to simulate the interaction between the articular discs and the articular cartilages. Nonlinear cable elements were used to simulate the disc attachments and the ligaments. Muscle forces and boundary conditions corresponding to the central occlusion were applied on all the models. The results showed that the stress distributions of the patient's TMJs were not the same as those of asymptomatic subjects. The stress distributions and deformations of the disc, condylar and temporal cartilage were changed at different periods after SSRO. The bio-mechanical parameters of TMJ were improved after SSRO. And the postoperative results showed that appropriate functional training could help to avoid TMJ diseases. Therefore SSRO could improve the stress distributions of the TMJ and relieve the symptoms of temporomandibular disorder (TMD).

Abstract:Sagittal split ramus osteotomy (SSRO) is the representative orthognathic surgery for the patients with mandibular prognathism. It is essential to understand the biomechanical environment of temporomandibular joint (TMJ) to analyse the effects of SSRO. This study aimed to investigate the influence of SSRO on the stress distributions in the TMJs of the patients with mandibular prognathism under symmetric occlusions. Thirteen patients with mandibular prognathism and ten asymptomatic volunteers were recruited. Finite element models of the asymptomatic, preoperative, and postoperative subjects were established, based on the CT of the asymptomatic volunteers and preoperative and postoperative scans of patients with mandibular prognathism. Contact was used to simulate the interactions between discs and articular surfaces, and between the upper and lower dentition. Muscle forces and boundary conditions corresponding to the centric and anterior occlusions were applied on the models. Under both occlusions, the maximum and minimum principal stresses of the articular disc and condyle in the preoperative group were significantly greater than those in the control group. After SSRO, the maximum and minimum principal stresses of the articular disc and condyle of the patients under both occlusions were greatly reduced, but the principal stresses of the articular disc were not significantly higher than those in the control group. Mandibular prognathism led to excessive stress on the TMJ. Severe asymmetric compression between the TMJs on both sides might cause temporomandibular disorder (TMD) symptoms. SSRO could improve the stress distribution of the TMJ and partially relieve the symptoms of TMD.

Abstract:To investigate the characteristic changes of the TMJ stress distributions, before and after SSRO for treating the skeletal class III malocclusion, being during centric occlusion.CT scan was used to obtain 2-D transverse image of maxilla and mandible. Finite element models were established before and after SSRO. An analysis of the stress, strain, and displacement of TMJ configurations was done by three-finite element method. Paired t test was used to compare the stress before and after operation.The Von Mises stress on TMJ configurations was found to decrease significantly after mandibular set-back SSRO; The maximal and minimal principal stress and maximal displacement were also found to decrease after surgery; The stress of both sides of the TMJ was found to distribute symmetrically.Oral and maxillofacial deformity was treated by orthognathics surgery, jaws were repositional that induce the changes of the TMJ stress, the upper and lower further study should be done to determine the stress changes effecting the function and health of TMJ.

Abstract:Purpose: The study aims to analyse the effects of condylar rotation on the biomechanical environment of the TMJ after bilateral sagittal split ramus osteotomy (BSSRO) through the finite element method (FEM). Methods: Thirteen patients with mandibular prognathism and twenty-three normal subjects were recruited. The three-dimensional (3D) models were reconstructed. 13 representative morphological parameters were measured for comparison. A patient was selected to perform virtual BSSRO surgery by rotating the condyles in MIMICS. The preoperative and postoperative 3D models of the patient were subsequently imported into ABAQUS for finite element analysis. The preoperative and postoperative stresses and joint spaces in the TMJs were investigated. Results: The maxillofacial morphologies of the patients with mandibular protrusion was significantly different from those of the asymptomatic subjects (P<.05). Stresses in the postoperative group were lower than those in the preoperative group. The rotation of the condyle could cause the variations in stress levels and joint spaces within the TMJs. Inward and upward rotation of the condyle was associated with higher stress in the TMJ, whereas the lowest stress was observed when the condyle remained stationary following surgical intervention. Significance: Lateral, medial and superior joint spaces were more related to the stresses in the TMJs. The condyle should be kept in place as much as possible to avoid disrupting the balance of the TMJ in patients with mandibular protrusion.

Abstract:Objective: The purpose of this study is to assess the impact of Sagittal Split Ramus Osteotomy (SSRO) on the alignment of the condyle and orientation of the Ramal segment following surgery in patients operated for the correction of mandibular asymmetry. Methods: A total of 27 patients who underwent SSRO were enrolled in this study, and study groups were defined as asymmetric (study group) and symmetric (control group) using linear measurements from the dental midline based on a three-dimensional coordinate system. Differences on preoperative and postoperative values of anterior (AJS), posterior (PJS) and superior joint spaces (SJS), condylar axial angle (CAA), Sagittal Ramal Angle (SRA), and Coronal Ramal Angles (CRA) of study and control groups were measured and compared. The data was analyzed using the "Wilcoxon signed-ranks test" to identify differences between groups. Results: Differences between preoperative and postoperative values of CRA and SRA of the contralateral group showed statistically significant results with p-values of 0.007 and 0.005, respectively. A statistically significant change in CAA angle was found in the control and deviation groups (p = 0.018 and p = 0.010, respectively). Conclusions: SSRO has inevitable effects on the condylar and ramal orientation. Individuals with asymmetry require particular attention throughout the planning process and beyond. Conjunctive modalities and modifications should be considered and utilized when necessary. Future studies with larger sample sizes, homogenous follow-up periods, and more comprehensive clinical data are needed to substantiate understanding of the response of the condylar segment. Clinical relevance: Orthognathic surgeries inevitably alter the alignment and harmony of temporomandibular structures and may result in change of AJS, PJS, SJS, CAA, SRA, and LRA, which may change the biomechanics of joint and lead to several complications like temporomandibular disorders. Especially in cases with midline asymmetry needs special consideration from planning till the end of the treatment to achieve best results. In severe cases, conjunctive modalities and modifications and other alternatives such as inverted-L osteotomies should be considered.

Abstract:Asymmetric mandibular prognathism is a clinically common skeletal dentomaxillofacial deformity. Unilateral sagittal split ramus osteotomy (USSRO) is an effective alternative procedure to bilateral sagittal split ramus osteotomy (BSSRO) for some patients. However, the biomechanical effect of temporomandibular joint (TMJ) of USSRO has not been fully studied. This study aims to evaluate the stress distribution changes in the TMJ of asymmetric mandibular prognathism treated with BSSRO/USSRO, to validate the clinical feasibility of USSRO. Nineteen patients with mandibular prognathism patients who were treated with BSSRO (n=12) and USSRO (n=7) had preoperative and postoperative computed tomographic scanning. Preoperative and postoperative 3-dimensional finite element analysis (FEA) of functional TMJ movements were made on one BSSRO patient and one USSRO patient. In all patients, the ANB angle and mandibular deviation were significantly improved postoperatively. There was no significant difference in the postoperative ANB angle and mandibular deviation between the BSSRO group and the USSRO group. In two preoperative FEA models, the maximum stresses of non-deviation side TMJ structures were greater than the deviation side during functional movements. The unbalanced stress distribution was corrected postoperatively in both BSSRO/USSRO FE models. Both BSSRO/USSRO can improve the ANB angle and mandibular deviation. The bilateral TMJ structure in patients with asymmetric mandibular prognathism had unbalanced stress, which could be significantly improved with the USSRO as effectively as BSSRO.
Fixation Methods for Mandibular Advancement and Their Effects on Temporomandibular Joint: A Finite Element Analysis Study

Abstract:Objectives: Bilateral sagittal split osteotomy (BSSO) is a common surgical procedure to correct dentofacial deformities that involve the mandible. Usually bicortical bone fixation screw or miniplates with monocortical bone fixation screw were used to gain stability after BSSO. On the other hand, the use of resorbable screw materials had been reported. In this study, our aim is to determine first stress distribution values at the temporomandibular joint (TMJ) and second displacement amounts of each mandibular bone segment. Methods: A three-dimensional virtual mesh model of the mandible was constructed. Then, BSSO with 9 mm advancement was simulated using the finite element model (FEM). Fixation between each mandibular segment was also virtually performed using seven different combinations of fixation materials, as follows: miniplate only (M), miniplate and a titanium bicortical bone fixation screw (H), miniplate and a resorbable bicortical bone fixation screw (HR), 3 L-shaped titanium bicortical bone fixation screws (L), 3 L-shaped resorbable bicortical bone fixation screws (LR), 3 inverted L-shaped titanium bicortical bone fixation screws (IL), and 3 inverted L-shaped resorbable bicortical bone fixation screws (ILR). Results: At 9 mm advancement, the biggest stress values at the anterior area TMJ was seen at M fixation and LR fixation at posterior TMJ. The minimum stress values on anterior TMJ were seen at L fixation and M fixation at posterior TMJ. Minimum displacement was seen in IL method. It was followed by L, H, HR, M, ILR, and LR, respectively. Conclusion: According to our results, bicortical screw fixation was associated with more stress on the condyle. In terms of total stress value, especially LR and ILR lead to higher amounts.

Abstract:As one of the most commonly used surgical methods in orthognathic surgery, bilateral sagittal split ramus osteotomy (BSSRO)has been widely used in kinds of mandibular deformities because of its great flexibility in the reposition of displacement mandibular. The postoperative fixation of BSSRO has two distinct phases: non-rigid internal fixation and rigid internal fixation. In recent years, with the introduction and development of rigid fixation techniques, many new fixation methods occurs. And with the development of radiography and digital technology, there are further discussion about the influence of different fixations on temporomandibular joint.In this paper,the fixation methods after BSSRO and the influence on temporomandibular joint were reviewed briefly.
Evaluation of stress distribution values at the temporomandibular joint and distalization of teeth during en-masse retraction of the mandibular dentition using buccal shelf bone screws: A FEM study

Abstract:Introduction: Present Finite-element-method (FEM) study evaluates the stress distribution values at the Temporomandibular-Joint (TMJ) during en-masse retraction of the mandibular dentition using buccal shelf bone screws with varying magnitudes of forces. Material and methods: Nine replicas of a pre-existing three-dimensional finite element model of the craniofacial skeleton and articular disc from Cone-Beam-Computed-Tomography (CBCT) data and Magnetic-Resonance-Imaging (MRI) data of a patient were used. Buccal shelf (BS) bone screws were inserted buccal to the mandibular second molar region. Forces of magnitudes 250 gm, 350 gm and 450 gm were applied through NiTi coil springs along with stainless-steel archwires of sizes 0.016 × 0.022-inch, 0.017 × 0.025-inch and 0.019 × 0.025-inch. Results: On the articular disc, the maximum stress was observed at the inferior region and the inferior part of the anterior and posterior zone at all force levels. The stress on the articular disc and displacement of teeth increased with increase in force levels in all the 3 archwires. The maximum stress at the articular disc and displacement of teeth was observed for 450 gm force and the least for 250 gm force. No significant difference in the amount of displacement of teeth or the stresses produced at the articular disc was seen with increasing the size of the archwire. Conclusion: With the present FEM study, we can deduce that it is better to use forces of lower levels on patients with temporomandibular-disorders (TMD) so as to reduce the stresses produced at the TMJ to avoid worsening of the TMD condition.
Orthognathic Surgery Effects on Temporomandibular Joint Compressive Stresses.

Abstract:INTRODUCTION This study tested orthognathic surgery effects on temporomandibular joint (TMJ) compressive stresses. METHODS Pre- (T1) and post-surgery (T2) cone-beam computed tomography images were collected from consenting subjects aged ≥15 years. Anatomical data were used to measure surgical changes in anteroposterior mandibular position and occlusal plane angle (FH-OP), estimate condylar loading areas (mm2 ), and to calculate T1 and T2 TMJ and jaw muscle forces (N) during canine biting via numerical modeling. Analysis of covariance tested for sex and biting angle differences in T2-T1 TMJ compressive stresses (TMJ force/loading area, MPa). Principal Component Analyses identified jaw muscle forces that accounted for changes in T2-T1 TMJ loads. Regression analyses tested the correlations between surgical changes in mandibular position, FH-OP, TMJ loads, and muscle forces. RESULTS Of 148 cases screened, 28 females and 16 males provided complete records. Condylar loading areas were significantly smaller (P=0.024) for females versus males (124±5 versus 144±7 mm2 ). T2-T1 differences in TMJ compressive stresses varied by surgical change, biting angle, and sex. Overall, the largest increases in TMJ compressive stresses post-surgery were for females with mandibular setbacks where FH-OP angle decreased. T2-T1 changes in jaw muscle forces had moderate (ipsilateral, λ=4.59; η2 =0.071) to large (contralateral, λ=1.49; η2 =0.31) effects on TMJ loads. CONCLUSIONS T2-T1 differences in TMJ compressive stresses during canine biting were affected by surgical changes in mandibular position and occlusal plane angle, biting angle, and sex. Surgical changes altered jaw muscle forces for the same biting conditions and, thus, affected TMJ loads and compressive stresses.

Abstract:OBJECTIVETo investigate the regulation of stress distribution on TMJ when the location of occlusal load on the mandibular second molar was changed by using the 3-dimensional Finite Element Method (FEM).METHODSA 3-dimensional FEM TMJ model including left mandibular second molar and mandible was established. In finite element analysis software ANSYS the same boundary constraints were applied to the TMJ FEM model with different load cases. The occlusal loads were applied to simulate ICO load, distal load, and buccal load and the stress values of condyles of the FEM model were calculated.RESULTSAs the occlusal load was applied to different tooth contact areas with various shapes, the stress distributions of bilateral condyles were found different and asymmetrical. The condyle stress on TMJ FEM model in three loading cases was diverse, not only in stress character but also in stress distribution style. The maximum value of Von Mises in the opposite condyle was 51.513 MPa when ICO load was applied. But when the distal load or the buccal load was applied, the maximum value of Von Mises in the opposite condyle increased to 72.145 MPa or 69.566 Mpa respectively.CONCLUSIONSThe same level but in different direction of occlusal load that resulted from difference of loading spots by changing the occlusal contact area, would induce different stress features on bilateral condyles.

Abstract:This study was to investigate the changes in condylar position and structure of the temporomandibular joint (TMJ) after mandibular setback using 2 forms of the ramus osteotomy.Twelve adult male rhesus monkeys were randomly divided into groups A (n = 6) and B (n = 6) for mandibular setback surgical procedure. An intraoral vertical ramus osteotomy (IVRO) was performed in group A, whereas a sagittal split ramus osteotomy (SSRO) was performed in group B. Changes in condylar position were quantified by computed tomography (CT) preoperatively and postoperatively. All animals in groups A and B were killed at 12 weeks after surgical procedure. The TMJ specimens were harvested and processed for histological examination.In group A, the CT examinations showed a significant anteroinferior displacement of the condyle after surgical procedure. In group B, slight posterior displacement and lateral tilting of the condyle were noted after surgical procedure. Thickened cartilage layer and endochondral ossification were seen in the condyles of group A, but only minimal alteration in articular cartilage was found in group B.Both SSRO and IVRO can be biologically sound procedures for correction of mandibular prognathism. Intraoral vertical ramus osteotomy procedure leads to condylar adaptive remodeling, which may have favorable effects on TMJ, and it could be considered as a preferred surgical treatment for those patients with preoperative TMJ disorders.

Abstract:Objective To record the electromyography(EMG) of mandibular prognathism patients before and after orthognathic surgery,analyse the change process of masticatory muscles system after sagittal split ramus osteotomy(SSRO) and further explore the relationship between the electromyography changes and the adaptive remodeling of masticatory muscles.Methods We selected 18 simply mandibular prognathism patients who will accept SSRO.Recorded the electrophysiological parameters of masticatory muscles before the operation,after 3 months,after 1 year,and then calculated and analysed the parameters.Results The mandibular prognathism patients had lower EMG indexes under normal conditions.3 months after SSRO,most of EMG indexes came down which were significantly lower than preoperative group.1 year after SSRO,most of the EMG indexes improved a lot,while some of them were still lower than control group.Conclusion Reconstruction of masticatory muscle system after SSRO is a long-term process.The performance of masticatory muscles was improved by SSRO.

Abstract:Background: Stress relieving corticoto mies during the treatment of maxillary expansion are needed in adult patients. Methods: Three-dimensional (3D) finite element model was prepared, and finite element analysis was processed to evaluate the stress distributions within the skull and maxillary teeth during surgically assisted rapid maxillary expansion (SARME) treatment. Results: Expansion forces generated more stress on the corticotomy-applied part of the maxilla. The stress levels decreased dramatically above the corticotomy line. Conclusion: Asymmetric transveral maxillary expansion might be achieved from a symmetric force generating screw during SARME treatment. SARME osteotomies may concentrate the stress in the expanding maxilla and reduce the pain in other parts of the cranium.

Abstract:Objective To built a 3D-finite element model(FEM) of mandible bilateral sagittal split ramus osteotomy(BSSRO).Methods Mandible CT scan pictures of were taken with a adult man with normal dentition and occlusion,and a 3D-FEM was then established with professional software.With the same soft ware,we simulated BSSRO operation and various fixation patterns.Results A vivid 3D-FEM was made and it possessed the same mechanical character under mechanics experiment.Conclusion By means of professional soft ware and CT scanning picture,an emulational 3D-FEM of mandible can be produced.

Abstract:PurposeThis study aims at analysing the stresses distribution in the temporomandibular joint (TMJ) using a complete high-resolution finite element model (FE Model). This model is used here to analyse the stresses distribution in the discs during a closing jaw cycle. In the end, this model enables the prediction of the stress evolution in the TMJ disc submitted to various loadings induced by mandibular trauma, surgery or parafunction.Materials and methodsThe geometric data for the model were obtained from MRI and CT scans images of a healthy male patient. Surface and volume meshes were successively obtained using a 3D image segmentation software (AMIRA®). Bone components of skull and mandible, both of joint discs, temporomandibular capsules and ligaments and dental arches were meshed as separate bodies. The volume meshes were transferred to the FE analysis software (FORGE®). Material properties were assigned for each region. Boundary conditions for closing jaw simulations were represented by different load directions of jaws muscles. The von Mises stresses distribution in both joint discs during closing conditions was analyzed.ResultsThe pattern of von Mises stresses in the TMJ discs is non-symmetric and changed continuously during jaw movement. Maximal stress is reached on the surface disc in areas in contact with others bodies.ConclusionsThe three-dimension finite element model of masticatory system will make it possible to simulate different conditions that appear to be important in the cascade of events leading to joint damage.

Abstract:The stress distributions of the temporomandibular joint(TMJ) under centric occlusion,anterior occlusion and unilateral occlusion were analyzed.A three-dimensional finite element model of the TMJ with mandible and teeth was developed according to CT images of a volunteer with asymptomatic joints.Contact elements were used to simulate the interaction between the articular discs and the mandibular condyles and between the articular discs and the articular fossas.The muscle forces and boundary conditions corresponding to the three occlusions were applied on the model.It was observed that the stresses of the TMJs in the models with anterior occlusion and unilateral occlusion are higher than those in the model with centric occlusion.In the three models,the maximum and minimum stresses of the discs occur in the intermediate zones and the anterior bands,respectively.Moreover,the stresses on the ventral disc are higher than the dorsal disc.The high stresses of the discs are located at the intermediate zones under the three occlusions.The stress of the contralateral TMJ is much higher than the ipsilateral TMJ under the unilateral occlusion.

Abstract:Temporomandibular joints (TMJs) constitute a pair of joints that connect the jawbone to the skull. TMJs are bilateral joints which work as one unit in conducting daily functions such as speaking, mastication, and other activities associated with the movement of the jaw. Issues associated with the TMJs may arise due to various factors—one such factor being the internal load on the TMJ. These issues may contribute to temporomandibular disorders (TMD). This study aims to evaluate the mandibular trajectories and the associated stress changes during the process of opening the mouth on the TMJs of an asymptomatic subject. The mouth opening motion was recorded by a motion capturing system using models of the mandible and maxilla constructed based on the computed tomography (CT). Two discs constructed based on magnetic resonance imaging (MRI). Finite element analysis was performed on the relative motion of the mandible to the maxilla and validated. The process modelled by these displacements provided less than 10% error in terms of deformation. The simulation results indicate that the lateral intermediate zone—the head and neck of the mandible—and the articular eminence sustained the most significant stresses during the mouth opening motion. The results also suggested that the stresses increase as the range of opening increases with the greatest von Mises stress, tensile, and compressive stress found at the position of maximal opening.This article is protected by copyright. Abstract:The aim of this study was to evaluate stress distributions in the temporomandibular joints (TMJs) with temporomandibular disorders (TMDs) for comparison with healthy TMJs. A model of mandible and normal TMJs was developed according to CT images. The interfaces between the discs and the articular cartilages were treated as contact elements. Nonlinear cable elements were used to simulate disc attachments. Based on this model, seven models of various TMDs were established. The maximum stresses of the discs with anterior, posterior, medial and lateral disc displacement (ADD, PDD, MDD and LDD) were 12.09, 9.33, 10.71 and 6.07 times magnitude of the identically normal disc, respectively. The maximum stresses of the posterior articular eminences in ADD, PDD, MDD, LDD, relaxation of posterior attachments and disc perforation models were 21, 59, 46, 21, 13 and 15 times greater than the normal model, respectively. TMDs could cause increased stresses in the discs and posterior articular eminences.
