Clinical distribution and drug resistance analysis of carbapenem-resistant pseudomonas aeruginosa

Abstract:Objective To monitor the distribution and analyze drug resistance of carbapenem-resistant Pseudomonas aeruginosa,and to provide a basis for the rational use of clinical drugs. Methods Pathogens isolated from the specimens of inpatient in our hospital since January 2012 to December 2013 were cultured,pathogens identification and drug susceptibility test were performed using VITEK-2 compact microorganism analysis system. The distribution and drug resistance of Imipenem and / or Meropenem-resistant type Pseudomonas aeruginosa were analyzed. Results Totally 247 strains of carbapenem-resistant Pseudomonas aeruginosa were isolated from 2012 to 2013,the resistance rates of Pseudomonas aeruginosa to Imipenem were rised from 9. 8% to 23. 4%,to Meropenem were rised from 6. 4% to 11. 6%,66. 4% of Pseudomonas aeruginosa came from patients over 80-year-old and 80. 0% of them came from phlegm specimens. Totally 247 strains of Pseudomonas aeruginosa were mainly distributed in geriatrics department( 64. 4%),ICU department( 16. 2%) and respiratory department( 6. 5%). Carbapenem-resistant Pseudomonas aeruginosa had three kinds of resistance patterns,the R / R were 118 strains( 47. 8%),the R / S were 121 strains( 49. 0%),and the S / R were 8 strains( 3. 2%). The drug resistance rate of R/R type bacteria to 9 kinds of common anti-pseudomonas drug was significantly higher than that of R / S type bacteria. The widespread use of imipenem and meropenem increased their resistance rates and also increased the resistance rates of other antimicrobial. The infection caused by carbapenem-resistant Pseudomonas aeruginosa should be treated by combination therapy based on susceptibility testing. Conclusion Clinicians should use broad-spectrum antimicrobial agents rationally,strengthen the monitoring of high incidence and prevalent department,develop reasonable and effective measures to control infection,to prevent and reduce nosocomial infection outbreak caused by carbapenem-resistant Pseudomonas aeruginosa.
