Abstract:Journal of the American Psychoanalytic Association, Ahead of Print. Extensive clinical scholarship has described the application of object-relational principles, particularly the operation of projective identification, to psychodynamic psychotherapy with couples. The author explores the way in which a more complete depiction of projective processes, one that incorporates each partner's intrapersonal management of multiple internal object relations, interacting interpersonally in the couple therapy process, can explain the escalating cycles of conflict between couples that are elaborated in the family-systems literature, and be helpful in understanding the object-relational substrate of chronic conflict in couples more generally. A description of how to map each partner's internal object world through the identification of these cycles in the early couple therapy process is elaborated in a theoretical model and illustrated with case material.

Abstract:This response to Koshkarian and Larralde first clarifies the term "kinship object" placing it within its cultural and psychoanalytic context, then offers an alternative theoretical conceptualization based on the developmental achievement of the internalized good object as a necessary prerequisite for the experience of kinship. The response then connects the idea of kinship to the thinking of Searles and his work on the nonhuman environment. These themes emphasize the importance of the unconscious, its centrality to the psychoanalytic project, and our responsibility as psychoanalysts to bring the dark undercurrents of repressed thoughts and feelings into our awareness,

Abstract:The article reveals the key ideas of psychoanalysts of the «independent» group of the British Psychoanalytic Association. To date, the framework of the modern anthropological system consists of psychoanalytic theories of personality development. In addition to the orthodox (classical) psychoanalysis of S. Freud, individual psychology of A. Adler and analytical psychology of Jung, an important place in the system of psychoanalytic directions is occupied by the theory of object relations - psychoanalytic current, which is based on the idea that as much the satisfaction of internal urges, as Freud argued, as the successful search and establishment of relationships with others. However, in addition to the work of M. Klein and the followers of her scientific and practical school, the creative work of other representatives of British psychoanalysis remains little studied for Ukrainian science. On the basis of the theoretical analysis of literary sources the main principles of psychoanalytic pedagogy within the theory of object relations are revealed. The main features of the theory of object relations, around which the representatives of the «independent» group gathered, have been identified. Emphasis is placed on the common and different views of British psychoanalysts on the formation of the child's personality, as well as criticism of the ideas of classical psychoanalysis. In particular, it is proved that the representatives of the «independent» group departed from the classical Freudian idea of the dominant influence of the Oedipus complex and the rigid «Super Ego» on the formation of personality. It is proved that the representatives of the «independent» group adhered to three common ideas: 1) each person from birth seeks to object relations, rather than to meet needs; 2) the interaction of the newborn with the outside world, especially in the dyad with the mother, has a decisive influence on the evolution of his inner world, which is considered through the prism of introjected inner objects; 3) a person's attitude to external objects is determined by imaginary internal object relations, which were formed in the child's psyche at an early age. It was concluded that the activities of analysts of the «independent» group focused on the problems of interpersonal communication, the child's interaction with parents at an early age, as well as the problems of identification and self-perception. Prospects for further research are planned in revealing the peculiarities of the work of Balint’s groups and the organization of scientific and practical measures for the prevention of pedagogical burnout.
The Therapeutic Relationship: Intrapsychic and Behavioral Changes in Both Patient and Analyst

Abstract:This article explores how the therapeutic relationship facilitates intrapsychic and behavioral changes in both the patient and the analyst. Key elements of the therapeutic relationship are reviewed, including transference, countertransference, introjective and projective identification, as well as the "real relationship." Special attention is paid to the "transformative relationship," which is a special kind of bond that develops between analyst and patient that is unique and transformative. It consists of mutual respect, emotional intimacy, trust, understanding, and affection. Empathic attunement is a key element in the evolution of a transformative relationship. This attunement optimally promotes intrapsychic and behavioral changes in both the patient and the analyst. This process is illustrated by a case presentation.

Abstract:The transformative therapeutic relationship is a unique interpersonal and intrapsychic phenomenon between analyst and patient. It occurs within the therapeutic framework, and is characterized by specific boundaries of time, place, and verbal and behavioral interactions. Collectively, these interactions may facilitate mutual trust, caring, respect, and affection between analyst and patient. The shared experience of the transformative therapeutic relationship can result in significant intrapsychic and interpersonal changes for both patient and analyst. Abstract:The central aim of the current study is to further the understanding of how therapists identify and understand erotic transference in therapy, and how they perceive it to influence the therapeutic processes and alliance. Answers were analyzed thematically using an open-coding analysis. A total of 116 therapists participated in the study reflecting on how erotic transference is experienced in the therapeutic setting. Data analysis yielded three main themes: (1) threat; (2) boundaries; and (3) growth. The threat theme included two subthemes: Threat to the therapeutic process and threat to the therapist's professional-esteem. Growth also consisted of two subthemes: development of the therapist and opportunity for insight (into the inner world of the patient). The present study subscribes to the idea that erotic transference is a significant aspect of the therapeutic setting and, if addressed appropriately and more comprehensively, can have a great impact on the patient's healing process.

Abstract:A good physician-patient relationship is essential for the delivery of quality health care and is associated with better health outcomes. This study explored the association between patients' object relations and the physician-patient relationship. Primary care patients ( n = 72) and physicians ( n = 21) participated in the study. Patients' early memories were obtained prior to their medical visit and were coded with the Social Cognition and Object Relations–Global Method (SCORS-G). Patients and physicians independently rated the quality of the physician-patient encounter. Results indicated that object relations (SCORS-G Cognitive-structural and Self factors) incrementally predicted physician-patient relationship ratings, as rated by the patient, above and beyond psychological distress, somatization, and level of physician training. Patient education, psychological distress, and level of physician training predicted physician ratings of the medical encounter. The authors discuss the discrepancy between physician and patient ratings as they relate to patients' object relations.

The Psychoanalytic Study of the Child

Abstract:Therapists who work with very young and deeply troubled children stand on the shoulders of Anni Bergman, expanding her vision to ongoing work. This paper will describe a program of dyadic treatment for caregivers and young children in the foster care system, for whom multiple generations of trauma have prevented secure attachment and therefore disrupted the development of the confidence and trust needed for rapprochement and future ego development in the children. The program is based on concepts of Attachment and Mentalization, which are branches of the same tree as Bergman's pioneering work with Mahler and Pine on separation and individuation and Bergman's career-long exploration of the inner world of children during toddlerhood. Clinical examples will be presented of work with two children first seen at less than two years of age.

Abstract:Sigmund Freud asserted that a part of the child's narcissism is transferred upon his superego, and thus narcissistic tensions occur in the ego as it strives to live up to the ego ideal. The antithesis to narcissism is not the object relation but object love. An individual's profusion of object relations, in the sense of the observer of the social field, may conceal his narcissistic experience of the object world; and a person's seeming isolation and loneliness may be the setting for a wealth of current object investments. The concept of primary narcissism is a good case in point. Although it is extrapolated from empirical observations, it does not refer to the social field but to the psychological state of the infant. The balance of primary narcissism is disturbed by maturational pressures and painful psychic tensions which occur because the mother's ministrations are of necessity imperfect and traumatic delays cannot be prevented.

Abstract:Within the vibrant web of general Israeli psychoanalysis, the Israeli community of self psychology, grounded within Kohutian foundations, has developed an ethical-theoretical shift that can be termed selfobject psychology, a sort of bold proposal to spiritual psychoanalysis. This development enhances the Kohutian revolution of transforming psychoanalysis from the empirical sphere of the positivistic sciences onto an experiential realm of contemplative science. We believe that merging Buddhist philosophy and methodologies within a unique program of psychoanalytic training will impel an evolutionary process that will transform the whole domain of psychoanalysis – philosophically, paradigmatically, and clinically. 'Human Spirit' Psychoanalytic-Buddhist Training Program is a framework for psychoanalysts who are immersed in nurturing and refining the spiritual dimension of the mind. The program strives to cultivate a compassionate, transformed and unified psychoanalytic presence, oscillating freely between the personal, the interpersonal, and the transpersonal dimensions of the psychoanalytic space.

Abstract:More and more clinical studies demonstrate that theoretical approach has little to do with the effectiveness of psychotherapy; it is the quality of the therapeutic relationship that potentiates the beneficial effects. This prompted Clarkson's extensive research of the literature, which distinguished discourse about five modes or qualities of therapeutic relationship. These are: the working alliance, the transferential/countertransferential relationship, the reparative or developmentally needed relationship, the person-to-person or real relationship and the transpersonal relationship. This article presents Clarkson's framework and uses it to describe a case of brief dynamic therapy of a gay man with HIV. To show how this conceptualization of relationship provides 'a principle for integration between different approaches', the case is also described using mainly the perspectives of object relations and transactional analysis. The case describes how each mode of relationship came into focus at different times in the counselling process and how recognizing them permitted a more vibrant sense of, and respect for, the client's way of being, and his therapeutic journey. The article concludes that this relational perspective is not a theoretical approach in itself, but a way of reframing what goes on in the client-therapist encounter that transcends theoretical schools. As demonstrated in this case it provides a coherence and simplicity that concentrates and guides our understanding of the client's therapeutic needs.
The significance of Kleinian contributions to psychoanalysis. III. The Kleinian theory of ego psychology and object relations

Abstract:The Kleinian theory of ego psychology rests largely on the theory of internal object relations formed through projective identification and re-introjection, and how they undergo transformations from the paranoid-schizoid to the depressive positions via the capacity of the maternal container to transform her infant's "paranoid" content into sobering awareness of truth. It differs from the classical theory in terms of the initial emphasis on projective identification and the only gradually developing importance of introjection of the external world of reality. This contribution develops the Kleinian metapsychology of ego formation from the succession of the paranoid-schizoid and depressive positions, which are the phenomenological-experimental equivalents of Mahler's autistic, symbiotic, and separation-individuation phases of development. It seeks also to develop the Kleinian conception of narcissism and compare it to the classical one.
Psychoanalysis as a theory of therapy

Abstract:The development of psychoanalytic theory is traced from its origins in Freud's early thinking in terms of his advancing clinical experience. While the influence of clinical experience on the modifications of theory has long been accepted, little attention has been paid to the broad spectrum effects of changing theoretical perspectives on the technique of clinical psychoanalysis. This development is traced through the emergence of ego psychology and the dominance of the structural theory to more current developments having to do with object relations and the theory narcissism. The residual theoretical ambiguities and clinical perplexities are illustrated in two clinical cases. The development of psychoanalytic theory and the correlative aspects of technique are seen as manifesting an organic developmental process which has led to an increasing sophistication of analytic intervention and a broadening capacity for dealing with increasingly difficult and less classical types of cases. The conclusion is drawn that therapeutic lestening is equivalently an informed listening, and it is theory that lends understanding and sensitivity to the hearing process. It is theory that is to be put in the service of listening, rather than listening to be put in the service of theory.

Abstract:Argument The psychoanalytic setting counts today as one of the familiar therapeutic rituals of the Western world. Taking up some of the insights of the anthropology of science will allow us to account for both the social and the material arrangements from which Freud's invention emerged at the end of the nineteenth century out of the clinical laboratories and private consulting rooms of practitioners of hypnosis. The peculiar way of neglecting or forgetting the object world and the institution of the psychoanalyst as a “transference object” will be traced back to multiple reconfigurations in the history of hypnotism in France and in Germany. In this process, different practitioners tried to achieve a synthesis of clinical work and experimental psychology, with the aim of objectifying knowledge about human subjectivity. While Freud retained the claim of psychoanalysis performing an experimental situation, he set apart his own setting from the objectifying practices which were characteristic of this experimental psychology located in the clinic and the private consulting room. It is not easy to over-estimate the importance of the part played by hypnotism in the history of the origin of psychoanalysis. From a theoretical as well as from a therapeutic point of view, psycho-analysis has at its command a legacy which it has inherited from hypnotism. Psychoanalytic psychotherapy in times of social crisis: The impact on therapeutic relationship

Abstract:Discussion on defining therapy factors develops along two lines: one focusing on the interpretation and another focusing on the relationship. Changes in the socio-economic circumstances, cultural particularities, dismissal of institutions, constant negation, lack of boundaries, confusion of roles, various family secrets revealed due to "collapses," major conflicts, violence and aggression filling the individual as part of both the internal and the external reality - all are known risk factors causing the individual to experience trauma either due the nature of the received stimuli/threats or due to the loss of the enabling/supportive environment. Also, this situation affects the analyst's psychic structure as well. The analyst will have to strike a balance between their own internal objects, which under the circumstances activate the analyst's own suppressed conflicts, and the multiple intense projections of the analysand. First of all, the internal struggles taking place in the analyst's psychism regarding their own griefs, frustrations, and conflicts concerning their adjustment to the current reality, as well as individual griefs relating to their narcissistic doubts and the projections of omnipotence they receive. The question is whether the analyst will go through a destabilization process, being overwhelmed by psychic stimuli in multiple levels, or react with "manic defences" resulting, perhaps, in the prevalence of anti-psychoanalytic dynamics on the transference - countertransference axis. A second line of thought involves the internal struggles taking place in the psychic structure of the analysand, and often the "meeting" of the latter with the analyst through the "parallel process." Finally, we think about the multiple - due to psychic tensions - instances of enacting (or acting out) and the setting being put to the test in terms of frequency, fees, difficulty of symbolic processing, and aggressiveness towards the interpretations and demands of the analyst as object introjected to the superego and requiring "compliance" to certain standards, the setting principles. We refer to the internal processes resulting from conflicts in the therapists' psychic structure as regards their own internal objects, life experiences, frustrations in relation to the parent/authority, adolescent conflicts with the system, political views, life philosophy, and sense of fairness. Issues are more complex in the therapy of children and adolescents. Their mental condition is affected by that of their parents. When it is hard for the therapist to become an object of identification and idealization, since all institutions around them have been undermined, confirming the adolescent's guilt-ridden fantasy aggression? How, then, will the therapeutic process move forward when models crumble; when parents are being proved weak and unable to receive the aggressiveness of the independence-gaining process; and when reverie fantasies cease to exist? Today, more and more often we see adolescents who cannot develop because they are involved in their own parents' unprocessed situations. Despite the aforementioned difficulties, the therapy space as a setting with boundaries and empathetic functions could function as a "womb" that will give birth to new mental life. And this life will bloom as long as therapists preserve intact within them their values and principles - and their ability for reverie!
