Abstract:Background: To evaluate the population-based frequency and severity of multiple sclerosis (MS)-related ocular diseases. Methods: Retrospective, population-based study examining patients with MS between January 1, 1998 and December 31, 2011. Patients were identified using the Rochester Epidemiology Project, which is a record-linkage system of medical records for all patient–physician encounters among Olmsted County, Minnesota residents. Diagnosis of MS was confirmed based on neuroimaging, cerebrospinal fluid studies, and serum studies for each patient according to the 2017 McDonald criteria. Patient data were obtained using the medical records and followed through April 1, 2018. Results: Of the 116 patients with MS, 66% were female and the median age of onset was 36 years (interquartile range 27.5–43.5 years). About half (61/116, 53%) had MS-related neuro-ophthalmic manifestations during their disease course, and about one-fourth (33/116, 28%) had visual symptoms as their presenting symptom of MS, most commonly as optic neuritis (26/116, 22%). Optic neuritis was the leading MS-related ocular condition (37%), followed by internuclear ophthalmoplegia (16%) and nystagmus (13%). Optic neuritis was mostly unilateral (40/43, 93%), with 16% (6/43) having a visual acuity of 20/200 or worse at nadir but ultimately 95% (35/37) improving to a visual acuity of 20/40 or better. Conclusions: This study provides the population-based frequency of MS-related ocular disease, which demonstrates a high frequency of ocular manifestations in MS both at disease onset and during the disease course, emphasizing the utility of neuro-ophthalmologists, or collaboration between neurologists and ophthalmologists, in the care of patients with MS.

Abstract:Objectives: The objectives of the study are to assess the peripapillary choroidal thickness (PPCT), retinal nerve fiber layer (RNFL), and optic nerve head (ONH) parameters in multiple sclerosis (MS) patients compared to healthy subjects. Methods: One hundred and twenty-eight eyes from 64 patients were included in this cross-sectional study. Eighty-two eyes of 41 MS patients and 46 eyes of 23 healthy subjects were examined. PPCT and RNFL were measured using spectral-domain optical coherence tomography (OCT). PPCT was measured from the four quadrants around the optic disc at a distance of 1 mm (PPCT-1) and 2 mm (PPCT-2) from the edge of the ONH and the beginning of the retinal pigment epithelium. ONH parameters were measured with Heidelberg retinal tomography (HRT-3). Disease duration, the number of episodes, MS subtypes, and Expanded Disability Status Scale (EDSS) scores were recorded. Results: The RNFL measurements and the mean PPCT-1 and PPCT-2 were significantly lower in MS patients compared to healthy individuals. PPCT-1 and PPCT-2 were measured as the thickest in the temporal quadrant, followed by the superior, nasal, and inferior quadrants, respectively, in both groups. When the ONH parameters were evaluated, cup/disc area ratio, cup area, and cup volume values were significantly higher, whereas optic rim volume and rim area values were significantly lower in MS patients compared to healthy subjects (p<0.05). We observed significant changes in RNFL and ONH parameters of MS patients in parallel with disease severity determined by EDSS scores and the presence of optic neuritis. Conclusion: There were significant changes in RNFL thickness, PPCT, and ONH parameters when MS patients were compared with healthy subjects. Assessment of RNFL and PPCT with OCT and ONH with HRT-3 may be useful in the follow-up of MS patients.

Abstract:Multiple Sclerosis (MS) is a chronic autoimmune demyelinating disease of the central nervous system (CNS) characterized by inflammation, demyelination, and axonal damage. Early recognition and treatment are important for preventing or minimizing the long-term effects of the disease. Current gold standard modalities of diagnosis (e.g., CSF and MRI) are invasive and expensive in nature, warranting alternative methods of detection and screening. Oculomics, the interdisciplinary combination of ophthalmology, genetics, and bioinformatics to study the molecular basis of eye diseases, has seen rapid development through various technologies that detect structural, functional, and visual changes in the eye. Ophthalmic biomarkers (e.g., tear composition, retinal nerve fibre layer thickness, saccadic eye movements) are emerging as promising tools for evaluating MS progression. The eye's structural and embryological similarity to the brain makes it a potentially suitable assessment of neurological and microvascular changes in CNS. In the advent of more powerful machine learning algorithms, oculomics screening modalities such as optical coherence tomography (OCT), eye tracking, and protein analysis become more effective tools aiding in MS diagnosis. Artificial intelligence can analyse larger and more diverse data sets to potentially discover new parameters of pathology for efficiently diagnosing MS before symptom onset. While there is no known cure for MS, the integration of oculomics with current modalities of diagnosis creates a promising future for developing more sensitive, non-invasive, and cost-effective approaches to MS detection and diagnosis.

Abstract:Background: Multiple sclerosis (MS) is a prevalent chronic inflammatory and neurodegenerative disease of the central nervous system. The main evolving forms, relapsing–remitting MS (RRMS) and secondary progressive MS (SPMS), lack clear delineation. Methods: We conducted an observational study on 523 Caucasian RRMS patients receiving first-line disease-modifying therapies (DMTs), analyzing demographic, clinical, and geographical data. Results: RRMS patients experienced a statistically significant reduction in relapse rates post-DMT initiation. Significant differences in time to reach an Expanded Disability Status Score (EDSS) of 3.0 and 6.0 were observed based on demographics and onset topography. Kaplan–Meier analysis revealed that the onset with optic or supratentorial symptoms is linked to a longer time until EDSS = 3.0 is reached. Urban origin correlated with a prolonged time until EDSS = 3.0. Gender and environment showed no significant associations with the hazard of reaching an EDSS = 6.0. Cox regression analysis revealed no significant impact of relapses on the time to reach EDSS scores of 3.0 and 6.0 in our study cohort. Conclusions: Multivariate analysis identified several predictive factors for disability progression, including environment, age at onset, and disability level at DMT initiation.

Retinal changes in multiple sclerosis: An optical coherence tomography and angiography study

Abstract:Background: Multiple sclerosis (MS) is an autoimmune demyelinating disease of the central nervous system with neuroaxonal damage. It is the principal driver of non-traumatic disability in young adults. Visual symptoms are common and optic neuritis (ON) may be the revealing feature in up to 30% of cases. Structural optical coherence tomography (OCT) represents a biomarker of central nervous system neurodegeneration in MS. OCT-angiography (OCT-A) is a noninvasive tool allowing the study of retinal vasculature and the detection of microvascular damage in neuro-retinal diseases. In this study, we aimed to assess structural and microvascular retinal changes in patients with MS with and without ON and to correlate the findings with visual function and MS disability. Methods: We conducted a cross-sectional study including patients diagnosed with MS according to the 2017 McDonald criteria. All patients underwent complete neurological examination with evaluation of the Expanded Disability Status Scale (EDSS) and the Multiple Sclerosis Severity Score (MSSS) and an ophthalmological examination including OCT and OCT-A. Patients were compared with age- and sex-matched healthy subjects. The primary endpoints were assessment of retinal nerve fiber layer (RNFL) thickness, ganglion cell layer (GCL+), and ganglion cell complex (GCL++) thicknesses on OCT. Vascular densities in the superficial capillary plexus (SCP), deep capillary plexus (DCP), and choriocapillaris (CC) were assessed on OCT-A, as well as central avascular zone (CAZ) parameters, lacunarity and fractal dimension. Results: A total of 160 MS eyes with and without a previous history of ON and 64 age- and gender-matched healthy eyes were analyzed. Among 160 eyes with MS, 69 had a history of ON. We observed a decrease in RNFL and GCL++ thickness in all 12 quadrants in MS patients when compared to healthy controls. Multivariate analysis by linear regression noted a significant correlation for temporal GCL++ and inferonasal RNFL thickness that were decreased in the MS group. A greater decrease in retinal layers thickness was identified in MS patients with a history of ON. On OCT-A, vascular density in (SCP) was significantly reduced in the MS group (P<0.002). A significant correlation between RNFL thickness and retinal vascular density was found but only in less than half of the hourly quadrants. A significant correlation was noted between visual acuity and CC density (P<0.0001). We also noted an inverse correlation between EDSS scores and CC density (P=0.02 and r=-0.275) and between MSSS and RNFL/GCL++ thicknesses. Conclusions: RNFL and GCL++ layers were thinner in MS patients with a history of ON and were reversely correlated with disease severity. Moreover, retinal vascular changes were observed in MS even in eyes without ON, and CC was reversely correlated with visual function and current disability. Thus, structural OCT coupled with OCT-A could represent a noninvasive and dynamic biomarker of MS severity and progression.

Abstract:OBJECTIVES : Multiple sclerosis (MS), which is known as a young-adult age disease, is called late-onset MS (LOMS) when it occurs at the age of 50 and older. In our study, we aimed to analyse the clinical and demographic characteristics, comorbidities, diagnostic and treatment challenges and prognosis of LOMS. METHODS : In a retrospective analysis of 136 patients diagnosed with multiple sclerosis (MS) after the age of 50, based on the 2017 McDonald criteria, and who were under observation in eight distinct MS centers across Turkey; demographic information, clinical characteristics of the disease, oligoclonal band (OCB) status, initial and current Expanded Disability Status Scale (EDSS) values, administered treatments, and the existence of spinal lesions on magnetic resonance imaging (MRI) were investigated. RESULTS : The mean age of the 136 patients was 60.96±6.42 years (51–79), the mean age at diagnosis was 54.94±4.30 years, and 89 (65.4%) of the patients were female. Most of the cases, 61.1% (83) had at least one comorbidity. In 97 patients who underwent lumbar puncture (LP), OCB positivity was observed in 63.6%. In 114 patients (83.8%), spinal lesions were detected on MRI. Eighty-seven patients had relapsing-remitting MS (RRMS) (64%), 27 patients had secondary progressive MS (SPMS) (19.9%), and 22 patients had primary progressive MS (PPMS) (16.2%). The mean EDSS at the time of diagnosis was 2.44±1.46, and the mean current EDSS was 3.15±2.14. CONCLUSIONS : In LOMS patients, the rates of delay in the diagnostic process, treatment disruption and progressive disease are higher than in the general MS population. The high rates of LP applying and OCB positivity of this study may indicate the habit of looking for clear evidences in advanged age in our country. This situation and comorbidities may cause a delay in diagnosis and eliminates the window of opportunity for early diagnosis. Although the high number of spinal lesions is a known marker for progressive disease, it is an issue that needs to be discussed whether the increased frequency of progressive course at older ages is due to the nature of the disease or immune aging itself.

Abstract:Aims/Purpose: We present a comprehensive evaluation of machine learning (ML) algorithms using Optical coherence tomography (OCT) and optical coherence tomography angiography (OCT‐A) for the diagnosis of Multiple sclerosis (MS). Methods: Prospective observational study using OCT volumes acquired with Cirrus HD‐OCT 5000 (Carl Zeiss, Meditec, Dublin, CA, EEUU) using the scanning protocols Optic Disc Cube 200 × 200 and Macular Cube 512 × 128, and OCT‐A macular volumes 175 × 350 with FOV 6 × 6 mm and OCT‐A of the Optic Nerve Head (ONH) with FOV 4.5 × 4.5 mm. The total data set consisted of 79 eyes of 40 patients with MS and 54 eyes of 27 control subjects. We evaluated classification algorithms such as Support Vector Machines, k‐Nearest Neighbour, Decision Trees, Random Forest, Extra Trees Classifier and Gaussian NB using data extracted from Ganglion Cell Analysis (8 parameters), Macular Thickness Analysis (7 parameters) and Optic Disc Analysis (5 parameters) from the OCT volumes and 5 parameters from the analysis of the macular OCT‐A volumes and 6 parameters from the analysis of the ONH. We studied all the ML algorithms considering three scenarios: OCT (20 parameters), OCT‐A (11 parameters), and OCT + OCT‐A (31 parameters). The models were trained using a 5‐fold cross‐validation strategy. We calculated the accuracy, F1‐score for each model and the area under the curve (AUC) was used to select the best ML algorithm. We applied SHAP (SHapley Additive exPlanations) to explain the output of the best ML model. Results: For the MS diagnosis, the best results were obtained for OCT with Gaussian NB (accuracy 0.865 ± 0.085, F1‐score 0.864 ± 0.086 and AUC 0.871 ± 0.081), for OCT‐A with Random Forest (accuracy 0.832 ± 0.032, F1‐score 0.832 ± 0.043 and AUC 0.831 ± 0.043) and for OCT + OCT‐A with Gaussian NB (accuracy 0.885 ± 0.028, F1‐score 0.886 ± 0.028 and AUC 0.891 ± 0.034). The input parameters with the highest contribution to the model's predictions were the minimum ganglion cell thickness and the temporal flow index in the ETDRS grid. Conclusions: This study highlights the efficacy of machine learning techniques in utilizing combined parameters from OCT and OCT‐A images of the macula and ONH tests to facilitate early diagnosis of MS. Moreover, it underscores the significance of ophthalmic examination in the comprehensive management of the disease.

A retrospective study of multiple sclerosis in Siriraj Hospital, Bankok, Thailand

Abstract:Objective: To determine the demographic and clinical data of Thai multiple sclerosis (MS) patients. Methods: A retrospective study of 72 patients attending the MS clinic at Siriraj Hospital, Mahidol University, Thailand between January 1997 and June 2004. Results: Fifty-eight patients (81%) were classified as clinically definite MS, 5 (7%) as Devic's syndrome, and 9 (13%) as possible MS. There were 62 females (86%) and 10 males (14%). Age at onset was 33 +/- 12 years with a mean relapse rate of 1.2 +/- 1.0 attacks per annum. None had a family history of MS. Visual impairment (53%) was the most common manifestation. Only 16% had classic (western) form of MS. Positive oligoclonal bands were found in 21%, visual evoked potentials with a typical delayed latency in 28%. MRI brain lesions compatible with McDonald's criteria were seen in only 24%, and spinal MRI brain longer than 2 vertebral bodies in 62%. The mean Kurtzke's Expanded Disability Status Scale (EDSS) was 3.0. Conclusions: Thai MS patients had much more female occurrence, no family history, common optico-spinal form, long spinal MRI lesions and low positive CSF oligoclonal bands.
Comparison of clinical characteristics between patients with opticospinal and conventional multiple sclerosis

Abstract:Objective To compare the clinical characteristics between patients with opticospinal multiple sclerosis (OS-MS) and those with conventional multiple sclerosis (C-MS). Methods Clinical materials of 99 patients with MS diagnosed according to the McDonald criteria in Suzhou area were collected (C-MS: 73 cases; OS-MS: 26 cases) and analyzed in coordination with results of laboratory examinations. Results Compared to patients with C-MS, female was more preponderant and prognosis was poorer in patients with OS-MS, initial symptoms were more frequently with visual deterioration and motor weakness, and the prevalence of motor weakness, sensory symptoms and visual deterioration during the course of disease was higher. No significant difference was found in age at onset and course of disease between the two groups. Conclusions There are some differences of certain clinical characteristics between patients with C-MS and OS-MS.
Everyday memory impairment, neuroradiological findings and physical disability in multiple sclerosis

Abstract:Thirty-five multiple sclerosis (MS) patients diagnosed according to the Poser criteria were examined in a variety of cognition and memory tasks. Their performance was compared to age- and gender-matched healthy individuals. While there was no difference in overall intellectual performance and short-term memory between the MS group and control subjects, the patient group showed significant deficits in several everyday memory tasks including story recall, object and face recognition, procedural, topographical and prospective memory tasks. An intra-group comparison in the MS cohort revealed that those patients with a chronic-progressive MS course exhibited the most substantial everyday memory impairments and highest EDSS scores. These findings coincide with the neuroradiological, semiquantitative lesion analysis which was also performed. In this analysis, the latter MS subgroup also had the highest lesion score, as a sign of an overall brain involvement.

Abstract:IMPORTANCE: The American Academy of Neurology Axon Registry® provides real-world data for patients with multiple sclerosis and neuro-myelitis optica. However, some data are incomplete (e.g. demographics) and some relevant outcomes are not systematically captured in neurology documentation (e.g. visual acuity). The American Academy of Ophthalmology IRIS® Registry (Intelligent Research in Sight) contains demographic and visual function data that may complement Axon Registry-derived data to enhance understanding of real-world visual outcomes in neurological disease.OBJECTIVE: To combine Axon Registry and IRIS Registry data to reduce missingness of demographic information and characterize visual outcomes in patients with multiple sclerosis and neuro-myelitis optica.DESIGN: Cross-sectional study.SETTING: Outpatient neurology and ophthalmology clinical practices.PARTICIPANTS: Patients participating in both registries between January 1, 2014 through December 10, 2021 were included if they had repeat ICD-9/10 codes for with multiple sclerosis or neuro-myelitis optica in the Axon registry.EXPOSURE: Diagnosis (multiple sclerosis or neuro-myelitis optica).MAIN OUTCOME AND MEASURE: Age, sex, race and ethnicity were assessed in the individual registries and classified as conflicting, missing, or not missing in the combined data set. The IRIS Registry contributed visual acuity data.RESULTS: Among 60,316 patients with multiple sclerosis and 1,068 patients with neuro-myelitis optica in the Axon Registry, 14,085 and 252 had temporal overlap in the IRIS Registry. Combining data reduced missing or conflicting data for race and ethnicity by 15-19 % (absolute reduction, all p ≤ 0.0005), but not age (p = 1.0) or gender (p = 0.08). 10,907 patients with MS and 142 with NMO had visual acuity data in the IRIS Registry. Visual acuity averaged between eyes was worse in patients with NMO after adjusting for age and gender (0.17 logMAR, 95 %CI 0.12,0.21, p < 0.0005).CONCLUSION AND RELEVANCE: Using data from two registries reduced missing data for race and ethnicity and enabled examination of outcomes captured in the IRIS Registry for conditions that are diagnosed more frequently in the Axon Registry, demonstrating the utility of a multi-registry analysis.

Abstract:Background/Objectives: This study investigated the frequency and timing of optic neuritis (ON) episodes in relation to the onset of multiple sclerosis (MS) and examined the occurrence of Uhthoff's phenomenon and Lhermitte's sign to understand their roles in early diagnosis and disease progression. Methods: A longitudinal study was conducted with 127 MS patients. Clinical data, including ophthalmological examinations for ON, were collected and questionnaires assessed the presence of Uhthoff's phenomenon and Lhermitte's sign. Results: Results showed that 37% of patients experienced demyelinating retrobulbar ON, with 25.53% having ON as the initial symptom of MS. Residual visual acuity impairment (below 20/40) and dyschromatopsia were reported by 25.53% and 17.02% of patients, respectively. Uhthoff's phenomenon and Lhermitte's sign were present in 26.77% and 36.22% of patients, respectively. The findings underscore the importance of early ophthalmological assessments in diagnosing MS, as ON can be an initial indicator of the disease. Conclusions: The study highlights the need for precise diagnostic tools and personalized therapeutic strategies focused on specific biomarkers and pathways involved in neuroinflammation and demyelination. Early diagnosis through vigilant ophthalmologic evaluation can lead to interventions that significantly alter disease progression, improving patient outcomes and quality of life.

Abstract:Background/Aims: Retinal nerve fiber layer (RNFL) thinning has been observed on histopathology and time-domain optical coherence tomography in many diseases of the central nervous system. In this study, with a higher resolution of spectral-domain optical coherence tomography (SDOCT), we detected RNFL changes in patients with multiple sclerosis (MS) in China, and compared RNFL thickness between eyes with and without optic neuritis (ON). Methods: In this retrospective, nonrandom case study, the patients were recruited from the Affiliated Sir Run Run Shaw Hospital of Zhejiang University. RNFL thickness was measured for each eye using SDOCT. The controls were recruited from the healthy population. Results: Peripapillary RNFL thickness of 24 eyes in 12 patients was detected by SDOCT. The average RNFL thickness of the MS patients was 81.9 ± 17.8 µm compared to the control value of 102.1 ± 8.1 µm (p = 0.00). The average RNFL of the patients with a history of ON was thinner than that of patients without ON (71.8 ± 19.2 µm vs. 92.0 ± 8.5 µm, p = 0.001). Conclusion: The RNFL thinning in Chinese patients with MS can be detected by SDOCT. The SDOCT scan represents a high-resolution, objective, noninvasive and easily quantifiable in vivo biomarker of MS.

Abstract:Background: Cognitive impairment (CI) affects approximately one-third of the patients with early multiple sclerosis (MS) and clinically isolated syndrome (CIS). Little is known about factors predicting CI and progression after initial diagnosis. Methods: Neuropsychological screening data from baseline and 1-year follow-up of a prospective multicenter cohort study (NationMS) involving 1123 patients with newly diagnosed MS or CIS were analyzed. Employing linear multilevel models, we investigated whether demographic, clinical and conventional MRI markers at baseline were predictive for CI and longitudinal cognitive changes. Results: At baseline, 22% of patients had CI (impairment in ≥2 cognitive domains) with highest frequencies and severity in processing speed and executive functions. Demographics (fewer years of academic education, higher age, male sex), clinical (EDSS, depressive symptoms) but no conventional MRI characteristics were linked to baseline CI. At follow-up, only 14% of patients showed CI suggesting effects of retesting. Neither baseline characteristics nor initiation of treatment between baseline and follow-up was able to predict cognitive changes within the follow-up period of 1 year. Conclusions: Identification of risk factors for short-term cognitive change in newly diagnosed MS or CIS is insufficient using only demographic, clinical and conventional MRI data. Change-sensitive, re-test reliable cognitive tests and more sophisticated predictors need to be employed in future clinical trials and cohort studies of early-stage MS to improve prediction.
Cellular-Level Visualization of Retinal Pathology in Multiple Sclerosis With Adaptive Optics

Abstract:Purpose: To apply adaptive optics-optical coherence tomography (AO-OCT) to quantify multiple sclerosis (MS)-induced changes in axonal bundles in the macular nerve fiber layer, ganglion cell somas, and macrophage-like cells at the vitreomacular interface. Methods: We used AO-OCT imaging in a pilot study of MS participants (n = 10), including those without and with a history of optic neuritis (ON, n = 4), and healthy volunteers (HV, n = 9) to reveal pathologic changes to inner retinal cells and structures affected by MS. Results: We found that nerve fiber layer axonal bundles had 38% lower volume in MS participants (1.5 × 10-3 mm3) compared to HVs (2.4 × 10-3 mm3; P < 0.001). Retinal ganglion cell (RGC) density was 51% lower in MS participants (12.3 cells/mm2 × 1000) compared to HVs (25.0 cells/mm2 × 1000; P < 0.001). Spatial differences across the macula were observed in RGC density. RGC diameter was 15% higher in MS participants (11.7 µm) compared to HVs (10.1 µm; P < 0.001). A nonsignificant trend of higher density of macrophage-like cells in MS eyes was also observed. For all AO-OCT measures, outcomes were worse for MS participants with a history of ON compared to MS participants without a history of ON. AO-OCT measures were associated with key visual and physical disabilities in the MS cohort. Conclusions: Our findings demonstrate the utility of AO-OCT for highly sensitive and specific detection of neurodegenerative changes in MS. Moreover, the results shed light on the mechanisms that underpin specific neuronal pathology that occurs when MS attacks the retina. The new findings support the further development of AO-based biomarkers for MS.

Abstract:Abstract Objective To investigate the relationship between coping mechanisms in people with multiple sclerosis (MS, pwMS) and cognitive, physical, and psychosocial factors such as socio-demographic characteristics, disability, personality, stigma, quality of life, depression, and anxiety. Method One hundred and two pwMS were enrolled in this cross-sectional study. Demographics and clinical characteristics were recorded. Coping with the MS Scale (CMSS), including seven subscales, which are problem-solving, physical assistance, acceptance, avoidance, personal health control, energy conservation, and emotional release, was used to measure coping. Anxiety and depression levels, stigma, neuropsychological symptoms, and personality were measured by the Hospital Anxiety and Depression Scale (HAD), EuroQol-5D Quality of Life Scale (EQ-5D), Quality of Life in Neurological Diseases (NeuroQoL) -Stigma Scale, Multiple Sclerosis Neuropsychological Questionnaire (MSNQ), and Revised Eysenck Personality Questionnaire-Abbreviated Form (EKA-GGK), respectively. Results There was a weak statistically significant positive correlation between the physical support subscale and age and the disease duration and a strong positive correlation with EDSS (r = .214, p = .035; r = .213, p = .036; r = .582, p ≤ .0001, respectively). There was a moderate negative relationship between the physical support subscale and the EQ-5D mobility, self-care, pain, and health subscales (r = −.434, p = .000; r = −.482, p = .000; r = −.526, p ≤ .001, respectively), a weak negative correlation with anxiety, and a strong negative relationship with usual activities (r = −.379, p ≤ .001; r = −.243, p = .017; r = −.384, p ≤ .001, respectively). Conclusion It has been shown that coping with MS can be affected by cognitive, physical, and psychosocial factors.

Abstract:Background: Late-onset multiple sclerosis (LOMS or L; MS) and early-onset MS (EOMS or E) are less common, and their prognosis can be different. To characterize the demographic and clinical features, and clinical outcomes of LOMS and EOMS patients, comparing them to adult-onset MS (AOMS or A) patients. Methods: The study was conducted as a secondary analysis of a prospective study. The participants were divided into three groups according to age of MS onset: early onset (<18 years of age), adult-onset (20-40 years of age), and late-onset (>55 years of age). Demographic variables, oligoclonal bands, IgG index, and Expanded Disability Status Scale (EDSS) score in admission, first year, second year and current EDSS were evaluated. The Timed 25-Foot Walk Test (T25FW), Timed Up and Go (TUG), Multiple Sclerosis Walking Scale-12, Single Leg Standing Test, Activity-Specific Balance Confidence Scale, Nine-Hole Peg Test, Epworth Sleepiness Scale and Restless Legs Syndrome Severity Scale were performed. Appropriate statistical analysis was made. Results: A total of 658 pwMS was included in the study and divided into three groups: EOMS (n = 117), AOMS (n = 499), and LOMS (n = 42). Statistically significant differences were determined between groups in terms of age [L (mean:59.86±5.45 years-y-)> A (36.87±9.12 y)> E(26.56±8.85 y), p < 0.001], education level, current EDSS score (L > E, p < 0.001), EDSS score in first admission, EDSS score in the first year, EDSS score in the second year (L > A > E, p < 0.001), reached an EDSS score 6 (E > L p = 0.001, E > A p = 0.015), disease duration (E > A, E > L, mean E = 11.66±9.7 y, A = 7.99±7.4 y, L = 6.31±4.67 y) time switching second-line treatment to the third line (E > L p < 0.001, A > L p = 0.002, mean E = 171.73±83.29 months-m-, A = 136.13±65.75 m, L = 65.85±45.96 m), number of relapses (A > E > L, median E = 4.0, A = 3.0, L = 2.0), distribution of MS type and oligoclonal band types. Significant differences were found in T25FW and TUG. Post-hoc analysis showed that participants in the LOMS group have longer T25FW (mean L = 7.8 ± 6.11, A = 6.25±5.09, E = 5.72±3.13, p = 0.011) and TUG (mean L = 11.01±5.53, A = 9.57±8.04, E = 8.38±5.51, p = 0.007) times than the AOMS and EOMS groups. Conclusion: Our result revealed that individuals with LOMS face elevated disability levels and a heightened propensity to transition from first-line treatments to more advanced therapeutic interventions. LOMS have worse lower extremity functional status than AOMS and EOMS patient. Clinical evaluations and treatment choices require more attention in LOMS. However, according to the low number of LOMS in our cohort, these results were considered cautious, and more wide and multi-center studies must be designed.

Abstract:Multiple sclerosis (MS) is a chronic disease of the central nervous system characterized by inflammation and neurodegeneration. It represents the leading cause of nontraumatic disability in young adults in many places around the world. Currently, for the objective quantification of the damage of the visual pathway swept‐source OCT (SS‐OCT) is considered the most appropriate technique. Additionally, during the course of the disease multiple neurological deficiencies can appear, including eye motility disorders. Patients may develop various ocular motor disorders (OMDs) affecting visual function and significantly increasing disability. These alterations involve visual function and may decrease quality of life perceived by patients because they affect, among other functions, reading capability. OMDs, can be analysed by several recording systems, although they have not been incorporated into the practice routine clinic due to its complexity, high costs and also because the value of these alterations is probably not known. It is possible that an accurate examination of motility eye, may have a substantial impact on the diagnosis and management of those patients. OMDs has been suggested as potential biomarker of early neurological deterioration and its evaluation could contribute to obtaining progression and response to treatments, facilitating translational development of so‐called precision medicine. Eye trackers are state‐of‐the‐art tools that can obtain valuable information of OMDs in neurological degenerations. In MS, oculomotor alterations as saccadic dysmetria, saccadic intrusions, nystagmus, fixational deficits, smooth pursuit impairment, skew deviation, optokinetic and vestibulo‐ocular reflex impairments, may go unnoticed because systematic examination of the oculomotor systems do not use routinely these techniques. As mentioned, objective recordings of eye movements through eye‐tracking systems are not currently performed widely, despite that they have been described as an important adjunct in diagnosis, documentation, and management of different neurologic conditions. In addition, eye tracking is also a capable method to investigate the relationship between behaviour and neural mechanisms. Eye motion recordings provide a measure of functional preservation of the nervous system, which should be recognized to be of significant importance in assessing a disease in which disability is manifested by disruption of normal neuronal functions. In fact, it has been described that eye tracking tasks (especially saccadic tasks) are often used as an adjunct to traditional scales for cognitive assessment. In addition, eye tracking data confirmed that MS is characterized by attention deficits. In conclusion, there is a growing interest in the use of eye tracking systems as biomarkers for MS. Specifically in this work we have analysed a large series MS patients to detect if any of the anomalies that patients refer to, such as reading difficulties, are related to disorders of the ocular motor systems.

Abstract:Background. Multiple sclerosis (MS) is an inflammatory demyelinating and neurodegenerative disease characterized by a variety of neurological symptoms. Cognitive impairment is an important clinical aspect of MS and require careful interviewing and neuropsychological testing to detect it. Contrast-enhanced magnetic resonance imaging (MRI) is the primary diagnostic method in MS, which helps confirm the presence and assess the extent of structural damage to the brain and spinal cord. The purpose of the study was to determine the frequency and severity of cognitive impairment in patients with relapsing-remitting multiple sclerosis, taking into account the localization of brain lesions according to MRI. Materials and methods. Seventy-two patients with a diagnosis of relapsing-remitting MS according to the McDonald criteria (2017) were examined. All of them underwent a thorough neurolo-gical, neuropsychological, and instrumental examination. The average duration of the disease was 8.2 ± 0.8 years. The average score on the Expanded Disability Status Scale was 3.38 ± 0.18 points. Symbol Digit Modalities Test (SDMT), Paced Auditory Serial Addition Test 3 (PASAT-3), and Montreal Cognitive Assessment (MoСA) were used to assess the neuropsychological status of patients. All participants underwent a 1.5T MRI of the brain with dotavist contrast enhancement. Results. A thorough survey of examined patients with MS revealed the presence of complaints of cognitive decline in 91.67 % of cases (66 patients). The average score on the SDMT was 32.6 ± 1.6 (controls 57.1 ± 0.9), on the PASAT-3 — 30.3 ± 1.8 (controls 56.8 ± 0.5), the evaluation score on the MoСA was 22.4 ± ± 0.5 points compared to the control group (28.4 ± 0.2 points). The conducted correlation analysis revealed statistically significant negative correlations in the corpus callosum (MoCA: r = –0.255; SDMT: r = –0.263; PASAT-3: r = –0.171; p < 0.05) and in the brainstem (MoCA: r = –0.191; SDMT: r = –0.209; PASAT-3: r = –0.214; p < 0.05). The results of the analysis of variance confirmed the correlations described above between the localization of brain lesions and the results of neuropsychological testing and revealed statistically significant differences in the scores on the MoCA and SDMT depending on the localization of lesions (MoCA: F(14, 57) = 5.052, p < 0.001; SDMT: F(14, 57) = 2.609, p < 0.05). Conclusions. Patients with relapsing-remitting multiple sclerosis quite often have cognitive impairment, which is confirmed by the results of neuropsychological testing. Correlation analysis revealed a significant negative relationship between the localization of foci in the corpus callosum and the brainstem and cognitive decline according to the results of tests (MoCA, SDMT, PASAT-3). The results of our study confirm the presence of structural and functional connections between the localization of focal brain lesions and the development of cognitive disorders, which has important diagnostic and prognostic value.
