Abstract:Background/Aims: Retinal nerve fiber layer (RNFL) thinning has been observed on histopathology and time-domain optical coherence tomography in many diseases of the central nervous system. In this study, with a higher resolution of spectral-domain optical coherence tomography (SDOCT), we detected RNFL changes in patients with multiple sclerosis (MS) in China, and compared RNFL thickness between eyes with and without optic neuritis (ON). Methods: In this retrospective, nonrandom case study, the patients were recruited from the Affiliated Sir Run Run Shaw Hospital of Zhejiang University. RNFL thickness was measured for each eye using SDOCT. The controls were recruited from the healthy population. Results: Peripapillary RNFL thickness of 24 eyes in 12 patients was detected by SDOCT. The average RNFL thickness of the MS patients was 81.9 ± 17.8 µm compared to the control value of 102.1 ± 8.1 µm (p = 0.00). The average RNFL of the patients with a history of ON was thinner than that of patients without ON (71.8 ± 19.2 µm vs. 92.0 ± 8.5 µm, p = 0.001). Conclusion: The RNFL thinning in Chinese patients with MS can be detected by SDOCT. The SDOCT scan represents a high-resolution, objective, noninvasive and easily quantifiable in vivo biomarker of MS.
Retinal changes in multiple sclerosis: An optical coherence tomography and angiography study

Abstract:Background: Multiple sclerosis (MS) is an autoimmune demyelinating disease of the central nervous system with neuroaxonal damage. It is the principal driver of non-traumatic disability in young adults. Visual symptoms are common and optic neuritis (ON) may be the revealing feature in up to 30% of cases. Structural optical coherence tomography (OCT) represents a biomarker of central nervous system neurodegeneration in MS. OCT-angiography (OCT-A) is a noninvasive tool allowing the study of retinal vasculature and the detection of microvascular damage in neuro-retinal diseases. In this study, we aimed to assess structural and microvascular retinal changes in patients with MS with and without ON and to correlate the findings with visual function and MS disability. Methods: We conducted a cross-sectional study including patients diagnosed with MS according to the 2017 McDonald criteria. All patients underwent complete neurological examination with evaluation of the Expanded Disability Status Scale (EDSS) and the Multiple Sclerosis Severity Score (MSSS) and an ophthalmological examination including OCT and OCT-A. Patients were compared with age- and sex-matched healthy subjects. The primary endpoints were assessment of retinal nerve fiber layer (RNFL) thickness, ganglion cell layer (GCL+), and ganglion cell complex (GCL++) thicknesses on OCT. Vascular densities in the superficial capillary plexus (SCP), deep capillary plexus (DCP), and choriocapillaris (CC) were assessed on OCT-A, as well as central avascular zone (CAZ) parameters, lacunarity and fractal dimension. Results: A total of 160 MS eyes with and without a previous history of ON and 64 age- and gender-matched healthy eyes were analyzed. Among 160 eyes with MS, 69 had a history of ON. We observed a decrease in RNFL and GCL++ thickness in all 12 quadrants in MS patients when compared to healthy controls. Multivariate analysis by linear regression noted a significant correlation for temporal GCL++ and inferonasal RNFL thickness that were decreased in the MS group. A greater decrease in retinal layers thickness was identified in MS patients with a history of ON. On OCT-A, vascular density in (SCP) was significantly reduced in the MS group (P<0.002). A significant correlation between RNFL thickness and retinal vascular density was found but only in less than half of the hourly quadrants. A significant correlation was noted between visual acuity and CC density (P<0.0001). We also noted an inverse correlation between EDSS scores and CC density (P=0.02 and r=-0.275) and between MSSS and RNFL/GCL++ thicknesses. Conclusions: RNFL and GCL++ layers were thinner in MS patients with a history of ON and were reversely correlated with disease severity. Moreover, retinal vascular changes were observed in MS even in eyes without ON, and CC was reversely correlated with visual function and current disability. Thus, structural OCT coupled with OCT-A could represent a noninvasive and dynamic biomarker of MS severity and progression.
Macular ganglion cell complex parameters by optical coherence tomography in cases of multiple sclerosis without optic neuritis compared to healthy eyes

Abstract:Purpose: To compare different macular thickness parameters and peripapillary retinal nerve fiber layer (RNFL) thickness between recently diagnosed cases of multiple sclerosis (MS) without optic neuropathy (ON) and healthy individuals. Methods: This cross-sectional study was performed between June 2014 and June 2015. All subjects underwent ocular and retinal examination. Spectral domain optical coherence tomography (SD-OCT) was used to measure the thickness of different layers of the retina at macular and peripapillary regions and at different quadrants. Between groups comparison was performed with P < 0.05 indicating statistical significance. Results: There were 32 eyes in the MS group and 74 eyes in the control group. The MS group was significantly younger than the control group ( P < 0.001). The mean ganglion cell complex (GCL++) thickness in superior macular area was 64.1 ± 8.9 μ in the MS group and 71.1 ± 5.9 μ in the control group. The thickness of the RNFL did not statistically differ in each of the quadrants between groups. Despite controlling for age, the macular thickness parameters were significantly thinner in eyes with MS compared to healthy eyes ( P < 0.01). Conclusion: The macular ganglion cell complex (mGCC) parameters were significantly reduced in recently diagnosed cases of MS as compared to healthy individuals.

Abstract:BACKGROUND: Reduced peripapillary retinal nerve fiber layer (pRNFL) and combined ganglion cell and inner plexiform layer (GCIP) thicknesses as measured by optical coherence tomography (OCT) have been observed in multiple sclerosis (MS) patients. The purpose was to determine the most associative OCT measure to level of cognitive and physical disability in MS.METHODS: Data were collected from 546 MS patients and 175 healthy controls (HCs). We compared the average pRNFL, temporal pRNFL (T-pRNFL), overall inner ganglion cell/inner plexiform layer (GCIP), and the overall ganglion cell complex (GCC) including macular RNFL and GCIP thicknesses measurements in differentiating MS subtypes from HCs. The association between OCT measures, Expanded Disability Status Scale (EDSS), and Symbol Digit Modalities Test (SDMT) were assessed using generalized estimating equations models.RESULTS: Both peripapillary and macular OCT measurements could differentiate all MS subtypes from HCs. The SDMT score was significantly associated with reduced thickness of all OCT measures, mostly in average pRNFL (0.14 µm, P = 0.001) and T-pRNFL (0.17 µm, P < 0.001). The EDSS score was significantly associated with reduced inner retinal layer thickness. The largest reduction was seen in T-pRNFL (-1.52 μm, P < 0.001) and inner GCC (-1.78 μm, P < 0.001).CONCLUSION: The T-pRNFL is highly sensitive and associated with level of both cognitive and physical disability.

Abstract:BACKGROUND:Fundus changes associated with high myopia (HM) may mask those associated with primary open-angle glaucoma (POAG). This study aim to determine the characteristics of RNFL thickness changes in patients with both POAG and HM and compare these to changes in patients with only HM. The diagnostic capabilities of both OCT and GDxVCC in this subset of patients are also evaluated.METHODS:Twenty-two eyes with POAG and HM (spherical equivalent (SE) between -6.0 and -12.0 D) were evaluated, and 22 eyes with HM were used for comparison. Characteristic retinal nerve fiber layer (RNFL) thickness profiles in patients with POAG and HM were examined using optical coherence tomography (OCT) and scanning laser polarimetry with variable corneal compensation (GDxVCC), and the diagnostic capabilities of these imaging modalities were compared. RNFL parameters evaluated included superior average (Savg-GDx), inferior average (Iavg-GDx), temporal-superior-nasal- inferior-temporal (TSNIT) average, and nerve fiber indicator (NFI) on GDxVCC and superior average (Savg-OCT), inferior average (Iavg-OCT), nasal average (Navg-OCT), temporal average (Tavg-OCT), and average thickness (AvgThick-OCT) on OCT (fast RNFL scan). Visual field testing was performed and defects were evaluated using mean defect (MD) and pattern standard deviation (PSD).RESULTS:The RNFL parameters (P < 0.05) significantly different between groups included Savg-GDx, Iavg-GDx, TSNIT average, NFI, Savg-OCT, Iavg-OCT, Tavg-OCT, and AvgThick-OCT. Significant correlations existed between TSNIT average and AvgThick-OCT (r = 0.778), TSNIT average and MD (r = 0.749), AvgThick-OCT and MD (r = 0.647), TSNIT average and PSD (r = -0.756), and AvgThick-OCT and PSD (r = -0.784). The area under the receiver operating characteristic curve (AUROC) values of TSNIT average, Savg-GDx, Iavg-GDx, NFI, Savg-OCT, Iavg-OCT, Navg-OCT, Tavg-OCT, and AvgThick-OCT were 0.947, 0.962, 0.973, 0.994, 0.909, 0.917, 0.511, 0.906, and 0.913, respectively. The NFI AUROC was the highest value.CONCLUSIONS:RNFL thickness was significantly lower in all but the nasal quadrant in patients with POAG and HM, compared to patients with only HM. Measurements with OCT and GDxVCC were well-correlated, and both modalities detected RNFL thickness changes. However, GDxVCC was better than OCT in detecting POAG in HM patients.

Abstract:Objective To study the feature of retinal nerve fiber layer( RNFL) thickness and its relationship with disease course,the Expanded Disability Status Scale and visual evoked potentials( VEP) in patients with multiple sclerosis.Methods 12 patients and 12 controls matched for gender,age and visual acuity were enrolled in the study.The thickness of RNFL was measured in all participants by optical coherence tomography.The feature of RNFL thickness and its relationship with disease course,the Expanded Disability Status Scale and VEP were obtained by statistics analysis.Results The RNFL thickness showed a thinner trend in patients than in controls.The difference was statistically significant in temporal upper quadrants( P = 0.030).RNFL thickness of superior quadrants in the left eye of patients was significantly correlated with the latency period of VEP( P = 0.037) and RNFL thickness of inferior quadrants and temporal quadrants in the left eye of patients was significantly correlated with the amplitude of VEP( P = 0.004,P = 0.008).Conclusion The RNFL thickness measured by OCT showed a thinner trend in patients with multiple sclerosis.RNFL thickness could be a new biomarker of axonal loss in multiple sclerosis.

Abstract:Aims/Purpose: Structural and physiological abnormalities have been reported in the retina of people with multiple sclerosis (pwMS). We aimed to investigate whether change in the peripapillary retinal nerve fibre layer (RNFL) thickness is associated with difference in retinal oxygen (O 2 ) saturation in newly diagnosed pwMS. Methods: Retinal oxygen saturation was measured using imaging by spectrophotometric non‐invasive retinal oximeter. RNFL thickness was acquired by RS‐3000 NIDEK OCT. Twenty‐five newly diagnosed pwMS were included. The measurements were performed primarily on the right eye; 25 healthy individuals (HCs) were age‐ and gender‐ matched with the pwMS. Arterio‐venular difference (A‐V diff) was calculated from the mean arteriolar and venular O 2 saturation values based on measurements obtained from all vessels. Data were analysed by Spearman correlation. Results: There was a significant positive correlation between RNFL and A‐V diff in pwMS ( p = 0.0014; rho = 0.4949), and no significant difference in the O 2 saturation in retinal venules and arterioles in pwMS (mean: 60.0% and 93.7%; p = 0.5980) compared to HCs (mean: 59.3% and 91.5%; p = 0.8934), respectively. Conclusions: Our findings indicate that early in MS, changes in RNFL and A‐V diff are detectable, before patients develop any apparent/severe disease. We recommend inclusion of the peripapillary RNFL and A‐V diff in MS monitoring and research.

Abstract:Objective: To investigate the changes in the retinal microcirculation in multiple sclerosis (MS) patients by swept-source optical coherence tomography angiography (SS-OCTA). Patients and Methods: Thirty-seven patients with relapsing-remitting MS and 40 healthy volunteers were included into this crosssectional study. Clinical history, Expanded Disability Status Scale and duration of MS were collected. SS-OCTA by deep range imaging (DRI) OCT measurements were performed on all subjects. Macular perfusion parameters including superficial and deep foveal avascular zones (FAZs, FAZd, respectively) (%), vascular densities of superficial capillary plexus (SCP) (%), deep capillary plexus (DCP) (%) and choriocapillaris (CC) (%) were compared with healthy subjects. Results: Vascular densities of SCP, DCP and CC were found to be statistically lower in the study group compared to the control group (p = 0.02, p = 0.03, p = 0.03, respectively). FAZs and FAZd, areas were significantly higher in the study group (p = 0.02, p = 0.02, respectively). Central macular thickness and subfoveal choroidal thickness were significantly lower than in the control group (p = 0.015, p = 0.047, respectively). Conclusion: Evaluation of retinal blood flow in patients with MS is useful both for understanding the physiopathology of the disease and in the clinical follow-up.
Optimal Intereye Difference Thresholds in Retinal Nerve Fiber Layer Thickness for Predicting a Unilateral Optic Nerve Lesion in Multiple Sclerosis

Abstract:Background: The optic nerve is a frequent site for involvement in multiple sclerosis (MS). Optical coherence tomography (OCT) detects thinning of the retinal nerve fiber layer (RNFL) in eyes of patients with MS and in those meeting criteria for clinically or radiologically isolated demyelinating syndromes. Current international diagnostic criteria for MS do not include the optic nerve as an imaging lesion site despite the high prevalence of acute optic neuritis (ON), or occult optic neuropathy, among early MS and clinically isolated syndrome patients; as well as most MS patients over the course of the disease. We sought to determine optimal thresholds for intereye difference in peripapillary RNFL thickness that are most predictive of a unilateral optic nerve lesion. Methods: We analyzed spectral domain OCT data of 31 healthy volunteers and 124 patients with MS at a single center as part of an ongoing collaborative investigation of visual outcomes. Intereye differences in peripapillary (360°) RNFL thickness were calculated as the absolute value of the difference. First, we determined the 95th percentile value of intereye difference for the healthy volunteers. This value was applied to the convenience sample group of MS patients as a validation cohort determining how well this threshold could distinguish patients with vs without a history of unilateral ON. The relation of intereye differences in peripapillary RNFL thickness to binocular low-contrast letter acuity scores was also examined. Results: Among healthy volunteer participants (n = 31), the 95th percentile value for intereye difference (upper boundary of expected for normal controls) was 6.0 μm. This value was applied to the convenience sample group of MS patients (n = 124, validation cohort). Positive predictive value, negative predictive value, sensitivity, and specificity for identifying MS patients with a history of unilateral ON were calculated for the 6-μm threshold value in a 2 × 2 table analysis with the application of χ2 tests (P < 0.0001). The 6-μm threshold was predictive of worse binocular low-contrast acuity scores at 2.5% (P = 0.03) and 1.25% (P = 0.002 by linear regression analyses). A receiver operating characteristic curve analysis demonstrated an optimal intereye difference threshold of 5 μm for identifying unilateral ON in the MS cohort. Conclusions: An intereye difference of 5–6 μm in RNFL thickness is a robust structural threshold for identifying the presence of a unilateral optic nerve lesion in MS.

Cellular-Level Visualization of Retinal Pathology in Multiple Sclerosis With Adaptive Optics

Abstract:Purpose: To apply adaptive optics-optical coherence tomography (AO-OCT) to quantify multiple sclerosis (MS)-induced changes in axonal bundles in the macular nerve fiber layer, ganglion cell somas, and macrophage-like cells at the vitreomacular interface. Methods: We used AO-OCT imaging in a pilot study of MS participants (n = 10), including those without and with a history of optic neuritis (ON, n = 4), and healthy volunteers (HV, n = 9) to reveal pathologic changes to inner retinal cells and structures affected by MS. Results: We found that nerve fiber layer axonal bundles had 38% lower volume in MS participants (1.5 × 10-3 mm3) compared to HVs (2.4 × 10-3 mm3; P < 0.001). Retinal ganglion cell (RGC) density was 51% lower in MS participants (12.3 cells/mm2 × 1000) compared to HVs (25.0 cells/mm2 × 1000; P < 0.001). Spatial differences across the macula were observed in RGC density. RGC diameter was 15% higher in MS participants (11.7 µm) compared to HVs (10.1 µm; P < 0.001). A nonsignificant trend of higher density of macrophage-like cells in MS eyes was also observed. For all AO-OCT measures, outcomes were worse for MS participants with a history of ON compared to MS participants without a history of ON. AO-OCT measures were associated with key visual and physical disabilities in the MS cohort. Conclusions: Our findings demonstrate the utility of AO-OCT for highly sensitive and specific detection of neurodegenerative changes in MS. Moreover, the results shed light on the mechanisms that underpin specific neuronal pathology that occurs when MS attacks the retina. The new findings support the further development of AO-based biomarkers for MS.

Abstract:Multiple sclerosis (MS) is an inflammatory and neurodegenerative, potentially disabling disease of the central nervous system. OCT (Optical Coherence Tomography) and OCT-A (Optical Coherence Tomography with Angiography) are imaging techniques for the retina and choroid that are used in the diagnosis and monitoring of ophthalmological conditions. Their use has recently expanded the study of several autoimmune disorders, including MS. Although their application in MS remains unclear, the results seem promising. This review aimed to provide insight into the most recent OCT and OCT-A findings in MS and may function as a reference point for future research. According to the current literature, the retinal nerve fibre layer (RNFL) and ganglion cell-inner plexiform complex (GC-IPL) are significantly reduced in people with MS and are inversely correlated with disease duration. The use of OCT might help distinguish between MS and neuromyelitis optica spectrum disorders (NMOSD), as the latter presents with more pronounced thinning in both the RNFL and GC-IPL. The OCT-A findings in MS include reduced vessel density in the macula, peripapillary area, or both, and the enlargement of the foveal avascular zone (FAZ) in the setting of optic neuritis. Additionally, OCT-A might be able to detect damage in the very early stages of the disease as well as disease progression in severe cases.

Abstract:<section class="article-section__content"><h3 class="article-section__sub-title section1"> Objective</h3><p>Therapeutic development in progressive multiple sclerosis (PMS) has been hampered by a lack of reliable biomarkers to monitor neurodegeneration. Optical coherence tomography (OCT)‐derived retinal measures have been proposed as promising biomarkers to fulfill this role. However, it is unclear if retinal atrophy persists in PMS, exceeds normal aging, or can be distinguished from relapsing–remitting MS (RRMS).</p></section><section class="article-section__content"><h3 class="article-section__sub-title section1"> Methods</h3><p>178 RRMS, 186 PMS, and 66 control participants were followed with serial OCT for a median follow‐up of 3.7 years.</p></section><section class="article-section__content"><h3 class="article-section__sub-title section1"> Results</h3><p>The estimated proportion of peri‐papillary retinal nerve fiber layer (pRNFL) and macular ganglion cell+inner plexiform layer (GCIPL) thinning in MS attributable to normal aging increased from 42.7% and 16.7% respectively at age 25 years, to 83.7% and 81.1% at age 65 years. However, independent of age, PMS was associated with faster pRNFL (−0.34 ± 0.09%/year; p < 0.001) and GCIPL (−0.27 ± 0.07%/year; p < 0.001) thinning, as compared to RRMS. In both MS and controls, higher baseline age was associated with faster inner (INL) and outer nuclear layer (ONL) thinning. INL and ONL thinning were independently faster in PMS, as compared to controls (INL:‐0.09 ± 0.04%/year, p = 0.03; ONL:‐0.12 ± 0.06%/year, p = 0.04), and RRMS (INL:‐0.10 ± 0.04%/year, p = 0.01; ONL:‐0.13 ± 0.05%/year, p = 0.01), while they were similar in RRMS and controls. Unlike RRMS, disease‐modifying therapies (DMTs) did not impact rates of retinal layer atrophy in PMS.</p></section><section class="article-section__content"><h3 class="article-section__sub-title section1"> Interpretation</h3><p>PMS is associated with faster retinal atrophy independent of age. INL and ONL measures may be novel biomarkers of neurodegeneration in PMS, that appear to be unaffected by conventional DMTs. Macular choroidal thickness and peripapillary retinal nerve fiber layer thickness in normal adults and patients with optic atrophy due to acute idiopathic demyelinating optic neuritis

Abstract:PURPOSE: To evaluate the association between macular choroidal thickness and peripapillary RNFL thickness in patients with optic atrophy due to acute idiopathic demyelinating optic neuritis and in normal controls using spectral domain optical coherence tomography (SD-OCT).METHODS: We performed SD-OCT peripapillary RNFL circular scan centered on the optic disc with horizontal and vertical crosshair scans through the fovea using the enhanced depth technique in 62 eyes with optic atrophy due to acute idiopathic demyelinating optic neuritis and 86 eyes of normal controls. The association between RNFL thickness and macular choroidal thickness measurements was assessed.RESULTS: The mean age was 43 ± 14 years (mean ± SD) in patients with optic atrophy and 45 ± 16 years in healthy controls (p = 0.791). There was a significant association between nasal peripapillary RNFL thickness and choroidal thickness at 3.0 mm nasal to the foveal center in patients with optic atrophy in multivariate analysis (estimate = 1.398, p = 0.011). In controls, there were significant associations between global average, superior, and inferior peripapillary RNFL thickness and choroidal thickness at 3.0 mm superior to the foveal center (estimate = -60.112, p = 0.044, estimate = 15.821, p = 7.312, and estimate = 15.203, p = 7.222, respectively).CONCLUSIONS: Our SD-OCT data revealed that there was a significant association between peripapillary RNFL thickness and macular choroidal thickness in patients with optic atrophy due to acute idiopathic demyelinating optic neuritis and in controls, although the mechanism remained unclear. The difference in the pattern of association between patients with optic atrophy and controls suggests that optic atrophy caused by acute idiopathic demyelinating optic neuritis could affect the pattern of association between peripapillary RNFL thickness and macular choroidal thickness.

Abstract:Background To assess quantitatively the microvascular changes in the retina and choroid of healthy eyes, eyes of subjects with primary angle-closure glaucoma (PACG), primary open-angle glaucoma (POAG) and neuromyelitis optica spectrum disorder (NMOSD) using optical coherence tomography angiography (OCTA). Methods A series of healthy individuals and subjects with PACG, POAG and NMOSD were recruited in this cross-sectional study. OCTA was performed to capture the optic nerve head and macula images and the vessel density (VD) and retinal nerve fiber layer (RNFL) thickness were quantified. The choriocapillary flow density (CFD) was calculated as the percentage of flow area to the whole selected area. Results A total of 68 PACG subjects, 25 POAG subjects, 51 NMOSD subjects and 37 healthy controls were enrolled. Significant decreases in peripapillary VD and RNFL thickness were observed in PACG ( p <0.001) and POAG ( p <0.001) eyes and also in NMOSD subjects with an optic neuritis history ( p <0.001) compared with healthy controls. Unaffected eyes in PACG and POAG subjects had lower baseline peripapillary VD than healthy controls ( p =0.002 and p =0.011, respectively). PACG eyes showed a lower baseline CFD than POAG ( p =0.0027) and the CFD in early and advanced PACG eyes showed a significantly much more decrease than POAG ( p =0.002 and p <0.001,respectively). Conclusions The peripapillary vessel density and RNFL thickness was reduced in glaucomatous and NMOSD eyes compared with healthy controls. PACG eyes showed a lower CFD than POAG and the distinct peripapillary and choriocapillaris microvasculature changes may be correlated to different PACG and POAG pathogenesis.

Abstract:Optical coherence tomography (OCT) is a non-invasive imaging technique based on the principle of low-coherence interferometry that captures detailed images of ocular structures. Multiple sclerosis (MS) is a neurodegenerative disease that can lead to damage of the optic nerve and retina, which can be depicted by OCT. The purpose of this pilot study is to determine whether macular OCT can be used as a biomarker in the detection of retrochiasmal lesions of the visual pathway in MS patients. We conducted a prospective study in which we included 52 MS patients and 27 healthy controls. All participants underwent brain MRI, visual field testing, and OCT evaluation of the thicknesses of the peripapillary retinal nerve fiber layer (pRNFL), macular ganglion cell layer (GCL), and macular inner plexiform layer (IPL). OCT measurements were adjusted for optic neuritis (ON). VF demonstrated poor capability to depict a retrochiasmal lesion identified by brain MRI (PPV 0.50). In conclusion, the OCT analysis of the macula appears to excel in identifying retrochiasmal MS lesions compared to VF changes. The alterations in the GCL and IPL demonstrate the most accurate detection of retrochiasmal visual pathway changes in MS patients.

Abstract:OBJECTIVE: To evaluate choroid plexus (CP) volume as a biomarker for predicting clinical disability and retinal layer atrophy in relapsing remitting multiple sclerosis (RRMS).METHODS: Ninety-five RRMS patients and 26 healthy controls (HCs) underwent 3 T whole brain MRI, expanded disability status scale (EDSS) and optical coherence tomography (OCT). Fully automated intra-retinal segmentation was performed to obtain the volumes of the retinal nerve fiber layer (RNFL), combined ganglion cell layer -inner plexiform layer (GCIPL), inner nuclear layer (INL), outer plexiform layer (OPL), outer nuclear layer (ONL), retinal pigment epithelium (RPE), total macular volume (TMV) and papillomacular bundle (PMB). Automated segmentation of the CP within the lateral ventricles was performed and the choroid plexus volume (CPV) was normalized by total intracranial volume (TIV). Linear regression analysis and generalized estimating equation (GEE) models were applied to evaluate relationships between nCPV and EDSS, T2 lesion volume, disease duration, and retinal layer volumes, followed by Bonferroni correction analysis for multiple comparisons.RESULTS: RRMS patients had larger tChPV compared to HCs (p < 0.001). After Bonferroni correction, there was a significant positive correlation between tChPV and EDSS (r<sup>2</sup> = 0.25, p = 0.0002), disease duration (r<sup>2</sup> = 0.30, p = 0.01), and T2 lesion volume (r<sup>2</sup> = 0.39, p = 0.0000). A robust negative correlation was found between tChPV and RNFL (p < 0.001), GCIPL (p = 0.003), TMV (p = 0.0185), PMB (p < 0.0001), G (p = 0.04), T(p = 0.0001).CONCLUSIONS: Our findings support the association of tChPV with disability and altered retinal integrity in RRMS.

Abstract:Objective:We used ultra-high resolution spectral domain optical coherence tomography (UHR-OCT) to analyze the subclinical changes in retinal neurons,axon injury,and retinal thickness in neuromyelitis optica spectrum disorders (NMOSD) patients.Methods:This case-control study included 21 cases of NMOSD patients and 20 matched healthy volunteers (control group).The NMOSD patients were divided into a subgroup without optic neuritis (NO-ON) (13 eyes) and a subgroup with optic neuritis (ON) group (13 eyes).The thickness of the total retina of each patient and control subject was evaluated by using UHR-OCT.Data of each layer were analyzed using one-way ANOVA.Results:Except at the central region,the total retinal thickness of the ON group in whole mapping images was significantly thinner than the control group and NO-ON group.There were significant differences in total thickness between the NO-ON group and the control group in some regions (nasal interior [NI],P=0.011;temporal interior[TI],P=0.003;superior exterior [SE],P=0.019;inferior exterior [IE],P=0.002).The thicknesses of the retinal nerve fiber layer (RNFL) and the combined ganglion cell layer and inner plexiform layer (GCL+IPL) in the ON group was significantly thinner in each quadrant compared with the control group (P＜ 0.001).The RNFL thickness of the NO-ON group was thinner in the nasal and temporal areas compared with the control group (NI,P=0.049;TI and temporal exterior [TE],P＜ 0.001).The GCL+IPL thickness in the NO-ON group was thinner than control group (SE,P＜ 0.001;IE,P=0.002).The thickness of inner nuclear layer (INL) in the ON group was thicker than in the control group (superior interior [SI],P=0.001;inferior interior [II],P=0.003;TE,P=0.043);however the thickness of the INL in the NO-ON group was thinner than in the control group (SE,P=0.015;IE,P=0.012).The thickness of Henle fiber layer and outer nuclear layer (HFL+ONL) in the NO-ON group was thinner than for the control group (SI,P=0.009;SE,P=0.018;Ⅱ,P=0.001;IE,P=0.001),but there were no significant differences between the ON group and the control group.Conclusions:There were significant changes in the different retinal layers of NMOSD patients compared to healthy controls.The thicknesses of RNFL and GCIPL in the NO-ON subgroup were decreased,suggesting that NMOSD patients with no optic neuritis potentially have structural damage in the retinal nerve.
Peripapillary and macular choroidal thickness in both eyes of patients with acute unilateral retrobulbar optic neuritis

Abstract:PURPOSE: The purpose of this study was to examine macular and peripapillary choroidal thickness (CT) in patients with acute unilateral retrobulbar optic neuritis.MATERIALS AND METHODS: In this cross-sectional study, 19 patients with acute unilateral retrobulbar optic neuritis were examined. A control group was matched with patients for sex and age. Enhanced depth imaging optical coherence tomography in macula and peripapillary areas in both eyes was performed for evaluation of CT. The CT was measured in subfoveal and other six points of macula and four points of peripapillary areas with a 3.4-mm scan circle centered on the optic nerve head.RESULTS: The mean subfoveal CT was 384.7 ± 101.6 μm, 380.5 ± 109 μm, and 401.2 ± 84.6 μm for affected eye, unaffected fellow eye, and healthy control, respectively. All measurements of macular CT were thinner in the patient group compared with healthy controls. Global peripapillary CT in affected eyes, unaffected fellow eyes, and healthy controls were 202 ± 43.3, 195.1 ± 42.9, and 234 ± 71.2, respectively. The difference between the three groups was statistically significant in the nasal point of peripapillary area (<i>P</i> = 0.023). No correlation was seen between CT and initial visual acuity or duration from symptom onset to medical survey in acute phase of retrobulbar optic neuritis.CONCLUSION: Patients with acute retrobulbar optic neuritis showed no significantly thinner macular and peripapillary CT in both eyes compared with healthy controls.

Abstract:As part of ongoing efforts to design improved nerve agent antidotes, two X-ray crystal structures of Torpedo californica acetylcholinesterase (TcAChE) bound to the bis-pyridinium oxime, Ortho-7, or its experimental bis-imidazolium analogue, 2BIM-7, were determined. Bis-oximes contain two oxime groups connected by a hydrophobic linker. One oxime group of Ortho-7 binds at the entrance to the active-site gorge near Trp279, and the second binds at the bottom near Trp84 and Phe330. In the Ortho-7-TcAChE complex the oxime at the bottom of the gorge was directed towards the nucleophilic Ser200. In contrast, the oxime group of 2BIM-7 was rotated away from Ser200 and the oxime at the entrance induced a significant conformational change in the peripheral anionic site (PAS) residue Trp279. The conformational change alters the surface of the PAS and positions the imidazolium oxime of 2BIM-7 further from Ser200. The relatively weaker binding and poorer reactivation of VX-inhibited, tabun-inhibited or sarin-inhibited human acetylcholinesterase by 2BIM-7 compared with Ortho-7 may in part be owing to the unproductively bound states caught in crystallo. Overall, the reactivation efficiency of 2BIM-7 was comparable to that of 2-pyridine aldoxime methyl chloride (2-PAM), but unlike 2-PAM the bis-imidazolium oxime lacks a fixed charge, which may affect its membrane permeability.
