Abstract:A current shortage of pediatric dermatologists limits access to dermatologic care among the pediatric population, yet comprehensive and updated data are lacking regarding access among the pediatric Medicaid population. This cross‐sectional study characterized Medicaid acceptance among actively practicing board‐certified pediatric dermatologists in the United States and revealed that of the 352 physicians compiled, 275 (78.1%) accept Medicaid. Significant differences in Medicaid acceptance status were observed based on practice type, region of practice, practice county median household income, and density of pediatric dermatologists. While the majority of practicing board‐certified pediatric dermatologists accept Medicaid, our findings suggest that differences in access to Medicaid‐accepting pediatric dermatologists exist based on practice type, geographic location, and density of pediatric dermatologists per county.

Disparities in State Medicaid Coverage of Tretinoin for Pigmentary Disorders Compared to Acne Vulgaris

Abstract:Background: Melasma and post-inflammatory hyperpigmentation (PIH) are common cosmetic dermatologic conditions that predominantly affect patients with skin phototypes III-VI. Comparing treatment coverage for these pigmentary disorders to treatment coverage for acne vulgaris may demonstrate disparities in insurance coverage for diseases that primarily affect patients of color. Objective: Describe differences in Medicaid coverage for topical tretinoin for melasma and PIH vs. acne vulgaris in all 50 states and the District of Columbia. Methods: This is a cross-sectional study of Medicaid insurance plans in all 50 states and the District of Columbia conducted between February 1 and 28, 2023. Data was collected from online publicly available preferred drug lists, prior authorization criteria, and email/telephone inquiries. Information was collected regarding coverage restrictions, including age restrictions, diagnostic restrictions, preferred drug status, and prior authorization requirements. Results: Complete coverage data for all three clinical indications was retrieved from 30 (58.8%) states; partial coverage data for acne vulgaris was retrieved from 16 (31.4%) states; no coverage data was retrieved from 5 (9.8%) states. Of states reporting coverage data, topical tretinoin is covered in 45 (97.8%) states for acne vulgaris and 10 (33.3%) states for melasma and post-inflammatory hyperpigmentation. There was decreased Medicaid coverage of topical tretinoin for acne vulgaris compared to melasma and PIH (P<0.05).  Conclusion: There is differential Medicaid coverage for acne vulgaris compared to pigmentary disorders which disproportionately affect patients of color. Abstract:Regular application of over‐the‐counter (OTC) sunscreen is considered the foundation of skin cancer prevention, yet OTC sunscreen is not eligible for reimbursement in almost all state Medicaid benefit plans. On review of 111 Medicaid preferred drug lists (PDLs) across 50 states and the District of Columbia (DC), only five plans were identified that incorporate coverage of sunscreen. Thus, many recipients of Medicaid, the majority of whom are individuals and families of lower socioeconomic status, may encounter financial difficulty and thus forego utilizing sun protective measures due to financial constraints. Here, we compare current Medicaid coverage of OTC sunscreen and discuss calculated and theoretical annual costs of this skin cancer prevention method.

Abstract:Among 454 North American patients referred for photopatch testing between 2009 and 2020, nearly 20% were diagnosed with photoallergic contact dermatitis. Sunscreens, particularly benzophenones, were by far the most common photoallergens (almost 90% of positive photopatch tests). Background Photoallergic contact dermatitis (PACD) is a delayed hypersensitivity reaction to allergens only in the presence of ultraviolet radiation in sunlight. Photopatch testing (PhotoPT) is necessary to confirm the diagnosis of PACD. There are few published studies of PhotoPT in North America. Objective To summarise the results of patients photopatch tested by members of the North American Contact Dermatitis Group (NACDG), 2009–2020. Methods Retrospective analysis of patient characteristics and PhotoPT results to 32 allergens on the NACDG Photopatch Test Series. Results Most of the 454 tested patients were female (70.3%), 21–60 years old (66.7%) and White (66.7%). There were a total of 119 positive photopatch tests. Sunscreen agents comprised 88.2% of those, with benzophenones responsible for over half of them. Final diagnoses included PACD in 17.2%, allergic contact dermatitis (ACD) in 44.5%, polymorphous light eruption (PMLE) in 18.9% and chronic actinic dermatitis (CAD) in 9.0% of patients. Conclusions In 454 patients with suspected photosensitivity referred for photopatch testing in North America, approximately one‐fifth had PACD. Sunscreen agents, especially benzophenones, were the most common photoallergens. Other common diagnoses included ACD, PMLE and CAD. Photopatch testing is an important tool for differentiating these conditions.

Abstract:Importance: Office-based phototherapy is cost-effective for psoriasis but difficult to access. Home-based phototherapy is patient preferred but has limited clinical data, particularly in patients with darker skin. Objective: To compare the effectiveness of home- vs office-based narrowband UV-B phototherapy for psoriasis. Design, setting, and participants: The Light Treatment Effectiveness study was an investigator-initiated, pragmatic, open-label, parallel-group, multicenter, noninferiority randomized clinical trial embedded in routine care at 42 academic and private clinical dermatology practices in the US. Enrollment occurred from March 1, 2019, to December 4, 2023, with follow-up through June 2024. Participants were 12 years and older with plaque or guttate psoriasis who were candidates for home- and office-based phototherapy. Interventions: Participants were randomized to receive a home narrowband UV-B machine with guided mode dosimetry or routine care with office-based narrowband UV-B for 12 weeks, followed by an additional 12-week observation period. Main outcomes and measures: The coprimary effectiveness outcomes were Physician Global Assessment (PGA) dichotomized as clear/almost clear skin (score of ≤1) at the end of the intervention period and Dermatology Life Quality Index (DLQI) score of 5 or lower (no to small effect on quality of life) at week 12. Results: Of 783 patients enrolled (mean [SD] age, 48.0 [15.5] years; 376 [48.0%] female), 393 received home-based phototherapy and 390 received office-based phototherapy, with 350 (44.7%) having skin phototype (SPT) I/II, 350 (44.7%) having SPT III/IV, and 83 (10.6%) having SPT V/VI. A total of 93 patients (11.9%) were receiving systemic treatment. At baseline, mean (SD) PGA was 2.7 (0.8) and DLQI was 12.2 (7.2). At week 12, 129 patients (32.8%) receiving home-based phototherapy and 100 patients (25.6%) receiving office-based phototherapy achieved clear/almost clear skin, and 206 (52.4%) and 131 (33.6%) achieved DLQI of 5 or lower, respectively. Home-based phototherapy was noninferior to office-based phototherapy for PGA and DLQI in the overall population and across all SPTs. Home-based phototherapy, compared to office-based phototherapy, was associated with better treatment adherence (202 patients [51.4%] vs 62 patients [15.9%]; P < .001), lower burden of indirect costs to patients, and more episodes of persistent erythema (466 of 7957 treatments [5.9%] vs 46 of 3934 treatments [1.2%]; P < .001). Both treatments were well tolerated with no discontinuations due to adverse events. Conclusions and relevance: In this randomized clinical trial, home-based phototherapy was as effective as office-based phototherapy for plaque or guttate psoriasis in everyday clinical practice and had less burden to patients. A Combination Treatment of Drug‐laser‐photon for Melasma: A Retrospective Study of Clinical Cases

Abstract:Background Combinational therapy such as taking tranexamic acid while using laser treatment has been proved potential efficacy by many experiments. However, there is few research which contains large samples and consistent observations. Objective We evaluated clinical efficacy and safety of a new systemic treatment of drug-laser-photon therapy. Methods Retrospective and randomized investigator-blinded study of 75 patients with mixed type melasma was analyzed. At each visit, standardized photographs were taken using VISIA. Modified melasma area and severity index (mMASI) scores were marked using photographs by two dermatologists. Results The mMASI score decreased significantly from 6.92 to 3.84 after the treatment. The VISIA analyze right cheek data shows: Spots (from 49.67 +/- 3.43 to 56.09 +/- 3.31), UV spots (from 41.39 +/- 24.45 to 44.56 +/- 25.86), and Brown spots (from 23.97 +/- 17.89 to 28.16 +/- 21.28) are statistically increased (p = 0.035, p = 0.018, p = 0.07). All patients feel varying degrees of improvement, about 10.17% felt very much improved, 30.51% felt much improved (51%-75%), 45.76% felt moderately improved (26%-50%), and 13.56% felt little improved (1%-25%). Limitations This study was no control group. Conclusion The efficacy and safety profile of the combination of drug-laser-photon therapy systemic treatment in melasma patients has been proved. It has potential possibility to become a new, reliable, widely suitable therapy strategy.
Accuracy of Medicaid Physician Directories of Board‐Certified Pediatric Dermatologists

Abstract:Patients may rely on health insurance physician directories to determine who is in‐network and inaccuracies in these directories may lead to delayed access to treatment, unexpected out‐of‐pocket costs, and increased barriers to care. This cross‐sectional study quantifies the accuracy of state‐specific Medicaid directories regarding board‐certified pediatric dermatologists, and compares physicians listed accurately with those listed inaccurately by type of practice and region of practice. The accuracy of all state Medicaid physician directories was 84.7%. Most states with board‐certified pediatric dermatologists had directories that contained inaccuracies and directory accuracy varied significantly by practice type, but not region of practice.

Abstract:Photobiomodulation (PBM) is an emerging treatment modality in dermatology with increasing office and home-based use. PBM is the use of various light sources in the red light (620-700 nm) and near-infrared (700-1440 nm) spectrum as a form of light therapy. PBM is often administered through low-level lasers or light-emitting diodes. Studies show that PBM can be used effectively to treat conditions secondary to cancer therapies, alopecia, ulcers, herpes simplex virus, acne, skin rejuvenation, wounds, and scars. PBM offers patients many benefits compared to other treatments. It is noninvasive, cost-effective, convenient for patients, and offers a favorable safety profile. PBM can be used as an alternative or adjuvant to other treatment modalities including pharmacotherapy. It is important for dermatologists to gain a better clinical understanding of PBM for in-office administration and to counsel patients on proper application for home-use devices to best manage safety and expectations as this technology develops. PBM wavelengths can induce varied biological effects in diverse skin types, races, and ethnicities; therefore, it is also important for dermatologists to properly counsel their skin of color patients who undergo PBM treatments. Future clinical trials are necessary to produce standardized recommendations across conditions and skin types.

The effect of phototherapy on Demodex density: a case–control study

Abstract:Abstract Background Human Demodex mites, Demodex folliculorum and Demodex brevis , are microorganisms that reside in the pilosebaceous units, usually without causing symptoms. Phototherapy has been linked to demodicosis in previous studies. We aimed to determine whether there was an increase in the frequency of demodicosis and Demodex density after 20 phototherapy sessions. Methods A case–control study was conducted with 32 participants who received narrowband ultraviolet B or ultraviolet A‐1 therapy for various dermatological indications. Standardized skin surface biopsies were performed before and after phototherapy to assess Demodex density. The presence of Demodex‐related skin conditions was assessed before phototherapy. A statistical analysis was performed to compare the Demodex densities and prevalence of demodicosis between the baseline and 20th session of phototherapy. Results No significant change was observed in Demodex density after 20 sessions of phototherapy. The average Demodex density before treatment was 2.75 ± 4.48 (/cm 2 ), and after treatment, it was 2.85 ± 4.81 (/cm 2 ), indicating no significant difference ( P = 0.879). The percentage of patients with demodicosis in at least one region of the face was 28.1% (9/32) before treatment, and after treatment, it was 31.3% (10/32), with no significant difference ( P = 1.00). Conclusions Our findings contradict previous studies that suggested an increased Demodex density and demodicosis prevalence after phototherapy. The data from previous studies are open to debate due to their selected samples, designs, and interpretations regarding the phototherapy–immunosuppression–Demodex relationship. Larger‐scale longitudinal studies conducted on a homogeneous sample are warranted to better understand the relationship between phototherapy and demodicosis.

Abstract:Abstract Photodermatoses comprise a heterogeneous group of disorders characterized by abnormal skin responses following physiological exposure to solar radiation. Previous investigations have established that photodermatoses are associated with significant psychological morbidity and poor health-related quality of life (HRQoL), although few studies have quantified this phenomenon in large patient cohorts. Therefore, the aim of this study was to perform a cross-sectional evaluation of patient-reported psychological morbidity and HRQoL in a large tertiary photobiology unit. Patients aged ≥ 16 years who attended outpatient appointments at a tertiary photobiology unit between October 2022 and August 2023 were issued a survey delivered directly to their smartphone using an online platform (DrDoctor). This survey used both free-text questions and established questionnaire tools to measure psychological morbidity and HRQoL. The questionnaires included General Anxiety Disorder-7 (GAD-7), Patient Health Questionnaire-9 (PHQ-9), Brief Illness Perception Questionnaire, Patient-Reported Outcome Measurement System-Profile 57, and Dermatology Life Quality Index (DLQI). Based on screening questions that identified the patient as new or follow-up and their diagnosis (if known), a tailored selection of questionnaires was administered. Formal thematic analysis was performed on free-text answers using NVivo (QSR International, Doncaster, Australia) and WordCloud. Descriptive statistical analyses were performed using SPSS (IBM, Armonk, NY, USA). During the study period, 1538 surveys were sent to patients, including those attending for repeat follow-ups. Data from 176 new patients were analysed and, following exclusion of surveys derived from repeat attendances, data from 414 follow-up patients were analysed. Among new patients, 86.9% were aware of their skin’s sensitivity to sunlight, with 61.3% reporting symptoms year round and 27.8% in summer only. Additional perceived triggers included stress, genetics, heat and medication, while patients’ aims for their appointments were to identify the cause of their condition, their diagnosis, a potential cure, and the best treatment options. Of 414 follow-up patients, 75% reported knowing their diagnosis. These were chronic actinic dermatitis (19%), erythropoietic protoporphyria (19%), polymorphic light eruption (15%), solar urticaria (12%), photoaggravated eczema (12%), lupus or other connective tissue disorders (7%), and other diagnoses (16%). In total, 32.6% of follow-up patients had a GAD-7 score ≥ 5 (consistent with clinically relevant anxiety) and 36.7% had a PHQ-9 score ≥ 5 (consistent with clinically relevant depression). Among conditions for which DLQI was measured, the highest median DLQI over the past year was 13, reported by patients with chronic actinic dermatitis. This study highlights the significant psychological impact of photodermatoses and their detrimental effects on HRQoL, which affect patients with chronic actinic dermatitis in particular.

Abstract:Summary Despite the development of highly effective biologics for skin diseases such as psoriasis or atopic dermatitis, UVA and UVB therapy, alone or in combination, are still essential components of various guidelines. Phototherapy is not only a first‐line treatment and highly effective for a number of skin diseases, but is also economical and has few side effects. The targeted use of UVA and UVB, if necessary, in combination with the photosensitizer psoralen in the context of PUVA therapy, enables the dermatologist to effectively treat a wide variety of skin diseases. Indications for phototherapy include epidermal diseases such as atopic dermatitis, psoriasis and vitiligo, as well as photodermatoses, mycosis fungoides, graft‐versus‐host disease and deep dermal diseases such as scleroderma. This article reviews the physical principles, molecular mechanisms, current treatment regimens, and individual indications for phototherapy and photochemotherapy.
Adoption of newly FDA-approved targeted immunomodulatory therapies by dermatologists: a cross-sectional analysis of Medicare Part D claims from 2013 to 2018

Abstract:Although targeted immunomodulatory medications are increasingly utilized for inflammatory skin conditions like plaque psoriasis, little is known of the trends in the adoption of newly Federal Drug Administration (FDA)-approved immunomodulators by dermatologists. We performed a retrospective, cross-sectional analysis of Medicare Part D Prescriber datasets to identify dermatologists filing Medicare prescription claims for immunomodulatory drugs FDA-approved for plaque psoriasis between 2013 and 2018. Differences in dermatologist characteristics were determined between dermatologists prescribing a psoriasis treatment within two years of its FDA approval, "early adopters" and non-prescriber dermatologists over the same time period. Biologics approved for psoriasis from 2013 to 2018 included certolizumab pegol, secukinumab, brodalumab, ixekizumab, guselkumab, and apremilast. Early adopter dermatologists (n = 783) accounted for 5% of all Medicare Part D prescribing dermatologists. Early adopters were more likely to be male, in practice longer, and had a greater number of average annual beneficiaries than dermatologists who did not. Only six (< 1%) early adopters practiced in a small town or rural areas. We believe these data show that the adoption of novel biologic treatments for psoriasis by dermatologists to Medicare beneficiaries may be associated with clinician experience and practice volume. Additionally, we identified low absolute numbers of dermatologists prescribing biologics overall in non-metropolitan areas, which may represent delayed access to novel psoriasis treatments for many Medicare beneficiaries.

Abstract:Abstract Phototherapy is commonly recognized as a convenient treatment modality for inflammatory skin diseases (ISDs). It has seen significant technological advancements over the last 2 decades. With variable success, high running costs and demand for trained professionals, treatment outcomes in ISDs using cabinet phototherapy are limited. Due to its high efficacy and safety profile, 308-nm excimer pulsed light (EPL) represents an exciting and practical option in moderate-to-severe ISDs that are treatment resistant and difficult to reach, where only affected areas are targeted and overall fewer patient visits are required for clearance than with conventional phototherapy. A prospective single-centre cohort study was conducted on 36 patients with ISDs treated with EPL, including psoriasis (n = 14), palmoplantar pustular psoriasis (n = 6), vitiligo (n = 2), necrobiosis lipoidica (n = 3), vitiligo (n = 2) and dermatosis (n = 9). The number of sessions required ranged from 4 to 18, with a maximum fluence between 250 and 850 mJ cm−2 among patients. The primary outcome involved assessing lesions post-EPL treatment using Physician’s Global Assessment (PGA) scores including ‘much better’, ‘better’, ‘no difference’ or ‘worsened’. Secondary outcomes included occurrence of complications and comparison of Dermatology Life Quality Index (DLQI) scores before and after EPL. At the end of each completed EPL treatment course, lasting an average of six sessions in total, and requiring an average maximum fluence of 350 mJ cm−2, 72% (n = 26) of patients achieved clearance and were found to have ‘much better’ or ‘better’ lesion outcomes when reviewing PGA scores. A significant reduction in post-EPL DLQI in 28% (n = 10) of patients was also found, with an average score reduction by 4. No complications were reported in 94% of patients (n = 34), with minor post-EPL burn found in 6% of patients (n = 2) but high overall patient satisfaction. In conclusion, our study contributes further supporting evidence to the scarce dermatological literature surrounding the successful use of EPL as a powerful treatment modality in inflammatory skin disease, especially if recalcitrant. Large studies including randomized controlled trials are necessary to help validate these promising results and determine optimal dosing regimens, long-term efficacy and patient satisfaction.
