Abstract:The skin lightening (SL) industry has a global reach and is projected to continue to grow over the coming decade. While SL treatments may be safely prescribed for treatment of some dermatologic conditions, many over-the-counter SL products contain ingredients that can cause harm to the skin and other organ systems. Given a lack of transparent information to patients and the historical colorist foundation that contextualizes a component of the cosmetic SL industry, dermatologists need to navigate biomedical and ethical concerns when explaining SL products to patients. This commentary briefly outlines the medical ethical issues surrounding this topic and describes avenues by which dermatologists may provide informed patient care that best supports beneficence, justice, autonomy, and nonmaleficence.

Skin Bleaching and Dermatologic Health of African and Afro-Caribbean Populations in the US: New Directions for Methodologically Rigorous, Multidisciplinary, and Culturally Sensitive Research

Abstract:Skin-bleaching practices, such as using skin creams and soaps to achieve a lighter skin tone, are common throughout the world and are triggered by cosmetic reasons that oftentimes have deep historical, economic, sociocultural, and psychosocial roots. Exposure to chemicals in the bleaching products, notably, mercury (Hg), hydroquinone, and steroids, has been associated with a variety of adverse health effects, such as Hg poisoning and exogenous ochronosis. In New York City (NYC), skin care product use has been identified as an important route of Hg exposure, especially among Caribbean-born blacks and Dominicans. However, surprisingly sparse information is available on the epidemiology of the health impacts of skin-bleaching practices among these populations. We highlight the dearth of large-scale, comprehensive, community-based, clinical, and translational research in this area, especially the limited skin-bleaching-related research among non-White populations in the US. We offer five new research directions, including investigating the known and under-studied health consequences among populations for which the skin bleach practice is newly emerging at an alarming rate using innovative laboratory and statistical methods. We call for conducting methodologically rigorous, multidisciplinary, and culturally sensitive research in order to provide insights into the root and the epidemiological status of the practice and provide evidence of exposure-outcome associations, with an ultimate goal of developing potential intervention strategies to reduce the health burdens of skin-bleaching practice.

Abstract:Women in Pakistan are obsessed with the concept of glowing whitish complexion to get good marriage proposals, jobs and desired status in society. Most of these fairness creams contain very high amount of mercury and hydroquinone. These act by inhibiting the tyrosinase enzyme responsible for melanin production. The use of skin fairness products that frequently contain toxic ingredients is associated with significant adverse health side effects, resulting in immense challenges for dermatologists. Despite current regulations, lightening agents continue to dominate the cosmetic industry. In this review, our international team of dermatologists tackles the topic of skin lightening as a global public health issue, one of great concern for both women’s health and racial implications. We aim to inspire a global discourse on how modern dermatologists can utilize scientific evidence and cultural competency to serve and protect patients of diverse skin types and backgrounds. In doing so, we hope to promote healthy skin and inclusive concepts of beauty in our patients and society.
Perceptions of Sun Protection, Skin Tone, Colorism, and Dermatologic Care Among South Asians in the USA

Abstract:South Asians (SAs) are among the fastest growing populations in the USA. Colorism — the system of inequality that views lighter skin as more advantageous in society — is prevalent in SA culture. This study evaluates motivations of sun protection use, attitudes of colorism, and skin lightening (SL) practices among SA Americans. Two-hundred-four participants recruited from online forums and ResearchMatch completed a questionnaire. Over half (111/204) reported use of sunscreen, of which 39.6% (44/111) reported daily or frequent use. Nearly half of respondents (98/204) believed that they are not at risk for skin cancer, with 37.7% (77/204) reporting minimal knowledge of skin cancers and only 4.9% (10/204) receiving a total body skin exam. One-third (65/204) reported being more concerned about prevention of tanning than skin cancer. In total, 38.2% (78/204) of respondents reported use of SL products, of which 33.3% (26/78) reported hydroquinone-based products and 26.9% (21/78) were unaware of the ingredients in their SL product. Only 16.7% (13/78) consulted a medical professional before using SL products. While many agreed that SA culture places high importance on light skin with regards to beauty standards (82.3%, 168/204), less noted that lighter skin is more beautiful (37.0%, 74/204). SL users more strongly agreed with colorism attitudes than non-users. Limitations include a small sample size with younger participants. Dermatologists must be mindful of the cultural motivations for skin tone preferences, sun protection habits, and SL behaviors and provide culturally relevant education on sunscreen, skin cancer, and risks of SL for the SA community.

Impact of Digital Advertising Policy on Harmful Product Promotion: Natural Language Processing Analysis of Skin-Lightening Ads

Abstract:Introduction: Starting June 30, 2022, Google implemented its revised Inappropriate Content Advertising Policy, targeting discriminatory skin-lightening ads that suggest superiority of certain skin shades. This study evaluates the ad content changes from 2 weeks before to 2 weeks after the policy's enforcement. Methods: Text ads from Google searches in eight countries (Bahamas, Germany, India, Malaysia, Mexico, South Africa, United Arab Emirates, and United States) were collected in 2022, totaling 1,974 prepolicy and 3,262 post-policy ads, and analyzed in 2023. A gold standard database was established by two coders who labeled 707 ads, which trained five natural language processing models to label the ads, covering content and target demographics. The descriptive statistics and multivariable logistic models were applied to analyze content before versus after policy implementation, both globally and by country. Results: Vertex AI emerged as the best natural language processing model with the highest F1 score of 0.87. There were significant decreases from pre- to post-policy implementation in the prevalence of labels of "Racial or Ethnic Identification" and "Ingredients: Natural" by 47% and 66%, respectively. Notable differences were identified from pre- to post-policy implementation in India, Mexico, and Germany. Conclusions: The study observed changes in skin-lightening product advertisement labels from pre- to post-policy implementation, both globally and within countries. Considering the influence of digital advertising on colorist norms, assessing digital ad policy changes is crucial for public health surveillance. This study presents a computational method to help monitor digital platform policies for consumer product advertisements that affect public health.

Abstract:BACKGROUND: Skin bleaching is a widespread phenomenon in spite of their potentially toxic health effects.OBJECTIVES: This study aimed to determine if such products are used in Sweden in particular by pregnant women, furthermore to explore immigrant women's view skin bleaching.METHODS: 455 pregnant women completed a questionnaire, which were statistically analysed. Focus groups and individual interviews were conducted with immigrant women, content analysis was used to assess the data.RESULTS: Skin bleaching products were used by 2.6% of pregnant women, significantlly more by women born in non-European countries. Motivating factors were associated with the concept of beauty together with social and economic advantages. The women had low awareness of the potential health risks of the products. Regulations on the trade of skin bleaching products have not effectively reduced the availability of the products in Sweden nor the popularity of skin bleaching.CONCLUSION: There is need for further research especially among pregnant women and possible effects on newborns. Products should be tested for toxicity. Public health information should be developed and health care providers educated and aware of this practice, due to their potential negative health implications.
Worldwide internet trends in the public interest related to skin whitening and bleaching creams

Abstract:Skin whitening is a practice that is used to obtain lighter skin tone and is most prevalent in Africa and Asia. Substances used for this procedure, such as hydroquinone or mercury have a variety of side effects and are banned in several countries. This study examined the popularity of internet searches for terms related to skin whitening and bleaching creams with the use of GoogleTrends (GT). GT was searched globally for the topic "skin whitening" and two terms "hydroquinone cream" and "mercury cream" throughout a 10-year period (01.09.2013-31.08.2023). The popularity of searches increased during the analyzed period. The topic "skin whitening" was most popular in Sudan, Vietnam, and Sri Lanka. The searches were higher for "hydroquinone cream" than "mercury cream" in almost all countries, besides the Philippines and Indonesia. Our study confirms that skin whitening practices are popular, especially among populations with darker skin tone. Despite potentially toxic side effects, creams with hydroquinone and mercury are increasingly searched worldwide. Education about skin whitening and the usage of bleaching substances should be implemented, especially in the regions of Africa and Asia.

In the current issue of the Journal, we highlight four papers on the epidemiology and clinics of skin of color patients but also emphasize the importance of education and research in the dermatopathology of SOC patients.

A novel skin brightening topical technology

Abstract:Background: Effective skin lightening remains an unmet need in over-the-counter formulations. Aims: This research examined a topical facial formulation containing hexylresorcinol, silymarin, 20% vitamin C, and 5% vitamin E in a proprietary anhydrous vehicle in skin explants for UVB photoprotective effects and clinical benefits. Patients/method: In vitro investigation examined 12 skin explants to assess the test product and vehicle. Six skin explants received 10 μL of the study product, and six skin explants received the 10 μL of the vehicle. After 96 hours, half the skin samples were exposed to 250 mJ/cm2 of UVB radiation while the other half unexposed. Clinically, 42 female subjects with normal or dry skin 35-55 years with skin types I-VI were enrolled possessing discoloration, uneven skin tone, and fine lines. The dermatologist investigator evaluated brightening, evenness, fine lines, wrinkles, and global appearance. Results: Explants treated with the study product experienced no significant change in gene marker expression of pro-collagen and pro-inflammatory gene markers upon UVB exposure. In contrast, skin explants treated with the vehicle experienced significant decreases in pro-collagen expression and significant increases in pro-inflammatory gene marker expression. Clinically, the greatest improvement as compared to baseline was seen at week 12 (P < .001) with 45% improvement in brightening, 27% improvement in evenness, 25% improvement in lines, and 25% improvement in global facial appearance. Conclusion: Hexylresorcinol, silymarin, 20% vitamin C, and 5% vitamin E in a proprietary anhydrous vehicle are effective in decreasing UVB-induced photodamage in skin explants while clinically producing skin brightness improvement.

Abstract:Skin whitening, or also referred to as bleaching or lightening, is a skin regiment that works to reduce pigmentation in the skin, to commonly treat skin issues like discoloration, acne scars, hyperpigmentation, age spots, or whiten naturally dark skin. The purpose of this study was to understand Black women’s knowledge and how they communicate about health impacts of skin whitening, providing insight into users’ health literacy of skin whitening products and use, based on levels of knowledge of ingredients in products. An autoethnographic approach, triangulated with in-depth interviews, and field and participant observations, reveal what Black women know about, and how Black women use skin whitening products; how Black women conceptualize the risks relative to the knowledge of products content and potential impacts, and strategies of communication that could enhance health literacy.

Abstract:Deliberate lightening of skin among African women is a complex phenomenon that intersects with cultural identity, health, beauty, and societal influence. The scientific literature has scarcely explored a comprehensive approach by interviewing users of skin lightening products. This article aims to analyze, through a comprehensive approach, the motivations underlying the deliberate lightening of skin among Burkinabe women. The research encompasses a narrative literature review and a qualitative field study in Bobo-Dioulasso, Burkina Faso. It targeted 59 women, categorized into current users, former users, and non-users of skin lightening products. Individual interviews and focus groups were utilized to gather qualitative data. The comprehensive approach enabled contextualization of the phenomenon, focusing on personal and collective motivations, while adhering to ethical principles. Participants gived various motivations for deliberate lightening of skin, including the pursuit of beauty, seduction, and social valorization. The majority used fairthese products to achieve a lightly pigmented, equating it with beauty and allure. Some aimed to enhance their seductive capital or improve their social status. Reasons for discontinuing the practice included awareness of health risks, societal pressure, and unmet objectives. Non-users cited reasons such as attachment to their natural skin tone, health concerns, and financial constraints. Deliberate lightening of skin can be viewed as a strategy to increase various forms of capital: aesthetic, seductive, social, and symbolic. This practice reflects socio-cultural dynamics and environmental influences, emphasizing the role of the body as capital in contemporary society. The findings reveal a heightened awareness among women of their body as a multifaceted capital, convertible into other forms of capital under certain conditions.

The role of systemic treatments for skin lightening

Abstract:Background: Pigmentation of the skin occurs as a result of increased melanin production or deposition due to various reasons including age, hormonal imbalances, endocrine disease, inflammation, and/or exposure to damaging radiation, resulting in dermatologic conditions such as lentigines, melasma, or postinflammatory hyperpigmentation. Although numerous topical therapies exist for skin lightening, they are limited by efficacy and pigmentation recurrence after treatment cessation. New research into systemic therapies for hyperpigmentation has been promising. Objective: To summarize the current literature for systemic skin lightening therapies. Methods: A review of the literature surrounding systemic skin lightening therapies was completed using PubMed (US National Library of Medicine). Results: Multiple systemic therapies for skin lightening exist including oral carotenoids, glutathione, melatonin, Polypodium leucotomos hydrophilic extract, procyanidin, and tranexamic acid. Preliminary data for the treatment of hyperpigmentation are promising, and currently, these oral treatments appear safe. It is not suggested to use intravenous glutathione for skin lightening due to the increased risk of adverse events. Conclusion: With the patient population seeking effective systemic treatments for skin pigmentation, it is important for dermatologists to understand the properties, the efficacy, and the adverse events profile of each compound, thus ensuring proper use by patients, and that patients are appropriately counseled regarding treatment expectation and safety.
Skin ‘Bleaching’ Practices and Associated Adverse Health Effects in Zimbabwe: A Canonical Correlational Study of Harare

Abstract:The study aimed to identify the ingredients, processes and practitioners involved in the skin lightening practices in Zimbabwe and to identify the associated, adverse health effects as well as to establish the user demographics. A technical field survey was carried out in Harare, Zimbabwe from October 2022 to March 2023. Out of a total of 450 potential participants, 382 individuals in 3 broad groups were successfully interviewed; these included 150 skin lightening practitioners and traders, 118 skin lightening products users and 114 individuals who have never practiced skin bleaching. A semi structured technical questionnaire was used for each group of participants and the collected data were analysed qualitatively and quantitatively. The study observed that both sexes are involved in skin lightening in Zimbabwe, the majority (52%) being women aged between 31-45 years. The established industry, is divers, including registered healthcare providers, commercial trade, informal markets and others. The majority of practitioners (65%) have less than 5 practising years. There are over 30 different skin lightening products available either as prescription medications, OTC treatments, commercial products, network marketing products as well as illegal bootleg concoctions. The products extends from creams, lotions and serums, to oral and injectable dosage forms. The most prevalent products being corticosteroid creams. 75% of users reported both reversible and non-reversible adverse health side effects and outcomes including skin irritations, inflammation, rashes, erythema, oedemas, ochronosis, dermal atrophy, photophobia, insomnia and unexplained weight gain. The most prevalent side effect being pruritus and irritations. It was observed that there is no formal registration, regulation or legislation covering the products, processes and practice of skin lightening in Zimbabwe.
Racial/Ethnic Variations in Acne: Implications for Treatment and Skin Care Recommendations for Acne Patients With Skin of Color.

Abstract:BACKGROUND Acne vulgaris is among the most common dermatologic diagnoses observed, including skin color (SOC) populations. This project sought to help clarify the existing published data and provide consensus statements on acne presentation, prevention, treatment, and maintenance in SOC populations to help improve patient outcomes. METHODS Six SOC dermatologists convened for a virtual meeting and used a modified Delphi process to address: 1) Are there racial/ethnic differences in the clinical presentation and sequela of acne? 2) Are there racial/ethnic differences in the therapeutic endpoint of acne treatment and patient expectations? 3) Is there a need for specialized approaches to therapeutic options and skincare in acne patients with SOC? The results of a literature review and the outcome of discussions, coupled with the panel's expert opinion and experience, are intended for health care providers caring for acne patients and clinician-researchers. RESULTS Racial/ethnic differences in the clinical presentation, sequelae, and desired treatment outcomes for acne have been reported. Notwithstanding limitations in the number, size, and methodologies of studies to date, the available data suggest that strategies that improve outcomes in acne patients with SOC include: Early initiation and maintenance of treatment regimens and careful consideration of tolerability of active ingredients, vehicles, and dosing. Using pH-balanced, non-irritating cleansers and non-comedogenic ceramides containing moisturizers help minimize irritation or dryness. CONCLUSIONS There a need for specialized approaches to therapeutic options and skincare in acne patients with SOC. Abstract:Background: Visual casts and discoloration are common barriers to sunscreen use in melanin-rich populations. However, photoprotective measures are essential for individuals with all skin types, including darker skin. Methods: Single-center, 7-day, open-label study of healthy adult females with Fitzpatrick Skin Types (FST) IV to VI and sensitive skin treated with once-daily daily facial moisturizer sun protection factor 35 (DFM SPF35). Subjects completed a cosmetic acceptability questionnaire at days 1 and 7. Photography using VISIA CR was performed at day 7. Adverse events were monitored throughout the study. Results: Thirty-two (32) subjects participated; 31.3% had FST IV, 53.1% V, and 15.6% VI skin. DFM SPF35 was viewed as cosmetically elegant. At day 1, 96.7% of subjects agreed product was easy to apply; 90.0% reported soft skin after product use; 86.7% said it had a lightweight, non-greasy feel and hydrated the skin. At day 7, 93.7% reported no visible white residue on their skin and said the product applied easily/absorbed well. The majority (90.6%) would continue using and would recommend the product; and 87.5% reported the product blended seamlessly into their skin, which agreed with clinical photography. Responses were consistent among subjects with normal, oily, or combination skin. No adverse events were reported. Conclusions: DFM SPF35 blended well into the skin and was perceived favorably among subjects with SOC after 1 and 7 days of use. Subjects felt it had good cosmetic acceptability without unacceptable white residues or a greasy feeling. Representation of Skin of Color in Social Media Discussions on Melasma, Keloids, and Vitiligo

Abstract:Background: The prevalence of social media as a source of medical information has grown substantially in recent years, especially for skin conditions disproportionately affecting individuals with skin of color, such as melasma, keloids, and vitiligo. Objective: This study aims to evaluate the nature of content related to these conditions on social media platforms, Instagram and TikTok. Methods: In March 2023, the top five hashtags for melasma, keloid, and vitiligo were identified on both platforms. For each hashtag, the 10 most popular posts were selected, based on Instagram and TikTok algorithms. A content analysis was conducted, categorizing posts as Educational, Promotional, or Inspirational. Posts were further classified by content creator type. Results: For the top 50 posts related to melasma on Instagram, the majority were promotional (58%), with the most common source being non-dermatologist social media influencers (50%). Dermatologists were the primary content creators for specific hashtags, such as #Melasma on TikTok, where the content was predominantly educational. Conclusion: Considering the high prevalence of dermatologist-creator content on TikTok, it is crucial to continue this shift toward dermatologist-driven educational content, as social media platforms continue to grow. Abstract:Antioxidants are deemed useful in controlling oxidative stress associated with extrinsic skin aging and pigmentation disorders. Resveratrol is a polyphenol compound found in many edible plants such as Vitis vinifera, and its inhibitory effects on the catalytic activity, gene expression, and posttranslational modifications of tyrosinase, a key enzyme in the melanin biosynthetic pathway, provide a mechanistic basis for its antimelanogenic effects seen in melanocytic cells, three-dimensionally reconstituted skin models, and in vivo animal models. As a potent antioxidant and a modulator of nuclear factor erythroid 2-related factor 2 (Nrf2), and sirtuin 1, resveratrol can also regulate multiple signaling pathways associated with inflammation and premature aging. Recent clinical studies have supported the efficacy of resveratrol and its analogs, such as resveratryl triacetate (RTA) and resveratryl triglycolate (RTG), in human skin lightening and antiaging. These findings suggest that resveratrol and its analogs are potentially useful as skin lightening and antiaging agents in cosmetics.

Efficacy of hydroquinone-free skin-lightening cream for photoaging

Abstract:Background: Hyperpigmentation and solar damage remains a difficult problem to treat with topical agents. Aims: To evaluate a novel skin-lightening complex (SLC) comprising four actives targeting melanin formation at multiple levels, namely disodium glycerophosphate, L-leucine, phenylethyl resorcinol, and undecylenoyl phenylalanine, in an oil-in-water emulsion cream. Patients/methods: Skin-lightening complex was evaluated in 80 female subjects of skin types I-III with at least moderate mottled hyperpigmentation. After a wash-out period of 1 month with a sunscreen, the subjects added a cream containing the SLC for 12 weeks twice daily to entire face and continued the sunscreen use. Results: Whereas there was no significant change during the wash-out period, the primary endpoint mottled hyperpigmentation decreased by 32% after the 12-week treatment period with the SLC cream. Secondary endpoints such as severity and number of lentigines, skin tone, and skin brightness also improved. In all, 57% of the subjects showed at least a moderate response, 17% did not improve, and 3% got worse after the treatment. Conclusions: The SLC cream was well tolerated, in particular when comparing with exfoliating or peeling agent containing skin-lightening products. When used with a daily sunscreen, this study confirms that the SLC represents an alternative to hydroquinone, retinoids, and many other skin-lightening actives.
Skin color reporting in squamous cell carcinoma-related randomized controlled trials in top dermatology journals: a systematic review

Abstract:The objectives are to determine the frequency that skin color is reported in randomized controlled trials (RCTs) involving squamous cell carcinoma (SCC) detection and treatment in leading dermatology journals. A systematic review of RCTs involving SCC was conducted among the top ten most impactful dermatology journals from inception to July 10th, 2023. Studies were included if they reviewed the treatment, prevention, or detection of SCC, involved patients directly and were classified as traditional RCTs. Studies were considered positive for reporting SOC if there was any demographic data in the methods or results of the following terms: Fitzpatrick scale, race, ethnicity, sunburn tendency, or skin of color. Of the 39 studies which were identified, 23 reported data related to skin color data (59.0%). White individuals were the most reported in these studies (56.5%). Subgroup analysis was conducted, and no statistical significance was found for study location, year of publication, or funding source. Skin color impacts skin cancer detection, predominant location of tumors, and recurrence. Less than 60% of high-quality RCTs related to SCC in top global dermatology journals included skin color among the demographic traits of study participants. Subgroup analysis demonstrated no improvement in reporting over the past 2 decades. Further research is needed to understand the reason for low skin color reporting rates among SCC-related RCTs and the impact this has on society.
