Abstract:The targeted direct-to-customer marketing of unapproved stem cell treatments by a questionable online industry is directed at vulnerable users who search the Internet in the hope of a cure. This behavior especially poses a threat to individuals who find themselves in hopeless and desperate phases in their lives. They might show low reluctance to try therapies that solely promise a cure but are not scientifically proven to do so. In the worst case, they suffer serious side-effects. Therefore, this thesis examines the display of advertisements of unapproved stem cell treatments for Parkinson's Disease, Multiple Sclerosis, Diabetes on Google's results page. The company announced a policy change in September 2019 that was meant to prohibit and ban the practices in question. However, there was evidence that those ads were still being delivered. A browser extension for Firefox and Chrome was developed and distributed to conduct a crowdsourced Black Box analysis. It was delivered to volunteers and virtual machines in Australia, Canada, the USA and the UK. Data on search results, advertisements and top stories was collected and analyzed. The results showed that there still is questionable advertising even though Google announced to purge it from its platform.

Digital Manipulation of Images of Models' Appearance in Advertising: Strategies for Action Through Law and Corporate Social Responsibility Incentives to Protect Public Health

Abstract:Context: Widespread digital retouching of advertising imagery in the fashion, beauty, and other consumer industries promotes unrealistic beauty standards that have harmful effects on public health. In particular, exposure to misleading beauty imagery is linked with greater body dissatisfaction, worse mood, poorer self-esteem, and increased risk for disordered eating behaviors. Moreover, given the social, psychological, medical, and economic burden of eating disorders, there is an urgent need to address environmental risk factors and to scale up prevention efforts by increasing the regulation of digitally altered advertising imagery. Methods: This manuscript summarizes the health research literature linking digital retouching of advertising to increased risk of eating disorders, disordered weight and appearance control behaviors, and body dissatisfaction in consumers, followed by a review of global policy initiatives designed to regulate digital retouching to reduce health harms to consumers. Next, we turn to the US legal context, reporting on findings generated through legal research via Westlaw and LexisNexis, congressional records, federal agency websites, law review articles, and Supreme Court opinions, in addition to consulting legal experts on both tax law and the First Amendment, to evaluate the viability of various policy initiatives proposed to strengthen regulation on digital retouching in the United States. Findings: Influencing advertising practices via tax incentives combined with corporate social responsibility initiatives may be the most constitutionally feasible options for the US legal context to reduce the use of digitally alternated images of models' bodies in advertising. Conclusions: Policy and corporate initiatives to curtail use of digitally altered images found to be harmful to mental and behavioral health of consumers could reduce the burden of eating disorders, disordered weight and appearance control behaviors, and body dissatisfaction and thereby improve population health in the United States.
Communication Policy to Reduce Health Disparities: A Cross-Language Content Analysis of YouTube Television Advertising

Abstract:Introduction: This study aimed to identify disparate health-related marketing across English- and Spanish-language television networks in New York City, ultimately to inform policy that can counteract disproportionate health-related marketing that provides harmful content to and withholds beneficial information from Latinx populations. Methods: A 2-week composite sample of primetime English-language (National Broadcasting Company and Columbia Broadcasting System) and Spanish-language (Telemundo and Univision) television networks from YouTube television was randomly drawn from September 7, 2022 to September 27, 2022 in New York City. A total of 9,314 health-related television advertisements were identified for systematic media content analysis and coded into categories: alcohol, core or noncore foods/beverages, mental health/tobacco prevention, health insurance, medical centers, and pharmaceuticals. Analyses conducted in 2022-2024 included intercoder reliability and descriptive and rate difference estimates using total advertisement broadcasting time in the full sample and subsamples by language networks on YouTube television. Results: Spanish television networks broadcast greater health-adverse advertisements per hour for alcohol (rate difference=4.91; 95% CI=3.96, 5.85) and noncore foods/beverages (rate difference=13.43; 95% CI=11.52, 15.34) and fewer health-beneficial advertisements per hour for mental health/tobacco prevention (rate difference= -0.99; 95% CI= -1.45, -0.54), health insurance (rate difference= -1.00; 95% CI= -1.44, -0.57), medical centers (rate difference= -0.55; 95% CI= -1.23, 0.12), and pharmaceuticals (rate difference= -5.72; 95% CI= -7.32, -4.11) than New York City primetime English television networks. Conclusions: Multilevel policy innovation and implementation are required to mitigate primetime television marketing strategies that contribute to health inequities.

Abstract:Advertising to children and teenagers is a multibillion-dollar industry. This policy statement reviews the forms of advertising that children and teenagers encounter, including newer forms of digital marketing, such as sponsored content, influencers, data collection, persuasive design, and personalized behavioral marketing driven by machine learning. Parents and pediatric health care providers need to be aware of the ways different marketing messages reach children and teenagers, including Internet sites, social media, and mobile apps. Evidence suggests that exposure to advertising is associated with unhealthy behaviors, such as intake of high-calorie, low-nutrient food and beverages; use of tobacco products and electronic cigarettes; use of alcohol and marijuana; and indoor tanning. Children are uniquely vulnerable to the persuasive effects of advertising because of immature critical thinking skills and impulse inhibition. School-aged children and teenagers may be able to recognize advertising but often are not able to resist it when it is embedded within trusted social networks, encouraged by celebrity influencers, or delivered next to personalized content. This policy statement expresses concern about the practice of tracking and using children's digital behavior to inform targeted marketing campaigns, which may contribute to health disparities among vulnerable children or populations. Pediatricians should guide parents and children to develop digital literacy skills to prevent or mitigate negative outcomes, but it is equally important that policy makers and technology companies embrace digital design, data collection, and marketing practices within today's broad digital environment that support healthier decision-making and outcomes.

Abstract:Introduction In 2020, Google took voluntary action to restrict food and beverage advertising through its online channels in the European Union/United Kingdom using Google's own nutrient profiling model to identify products eligible to be marketed to children through its Google Display Network. The objective of this study was to evaluate the potential impact of the Google policy, if applied to the U.S. market, on restricting online advertising of the top-selling packaged foods and beverages in the U.S. Methods The top 25 U.S. food and beverage manufacturers were identified. Nutrient data for products from these manufacturers were sourced from Label Insight (a Nielsen IQ company) in 2021. Each product was examined against four nutrient profiling models: the Google nutrient profiling model, the WHO Europe nutrient profiling model, the Pan American Health Organization nutrient profiling model, and the Chilean government nutrient profiling model. Results Under Google's nutrient profiling model, 18% of 14,188 products were eligible to be advertised to children, representing 240 billion generated annually. The Google nutrient profiling model permitted the most products to be advertised to children of all four nutrient profiling models examined. Conclusions U.S. children engage extensively with online media. In place of government regulation, the Google advertising policy and related nutrient profiling model would limit online advertising of the most unhealthful products to children, if the policy were to be applied to the U.S. market. The effectiveness of the policy would be strengthened by refining the Google nutrient profiling model to better align with nutrient profiling models developed by authoritative health agencies, including the WHO.

Social Media Promotion of Health Tests With Potential for Overdiagnosis or Overuse: Protocol for a Content Analysis

Abstract:Background: In recent years, social media have emerged as important spaces for commercial marketing of health tests, which can be used for the screening and diagnosis of otherwise generally healthy people. However, little is known about how health tests are promoted on social media, whether the information provided is accurate and balanced, and if there is transparency around conflicts of interest. Objective: This study aims to understand and quantify how social media is being used to discuss or promote health tests with the potential for overdiagnosis or overuse to generally healthy people. Methods: Content analysis of social media posts on the anti-Mullerian hormone test, whole-body magnetic resonance imaging scan, multicancer early detection, testosterone test, and gut microbe test from influential international social media accounts on Instagram and TikTok. The 5 tests have been identified as having the following criteria: (1) there are evidence-based concerns about overdiagnosis or overuse, (2) there is evidence or concerns that the results of tests do not lead to improved health outcomes for generally healthy people and may cause harm or waste, and (3) the tests are being promoted on social media to generally healthy people. English language text-only posts, images, infographics, articles, recorded videos including reels, and audio-only posts are included. Posts from accounts with <1000 followers as well as stories, live videos, and non-English posts are excluded. Using keywords related to the test, the top posts were searched and screened until there were 100 eligible posts from each platform for each test (total of 1000 posts). Data from the caption, video, and on-screen text are being summarized and extracted into a Microsoft Excel (Microsoft Corporation) spreadsheet and included in the analysis. The analysis will take a combined inductive approach when generating key themes and a deductive approach using a prespecified framework. Quantitative data will be analyzed in Stata SE (version 18.0; Stata Corp). Results: Data on Instagram and TikTok have been searched and screened. Analysis has now commenced. The findings will be disseminated via publications in peer-reviewed international medical journals and will also be presented at national and international conferences in late 2024 and 2025. Conclusions: This study will contribute to the limited evidence base on the nature of the relationship between social media and the problems of overdiagnosis and overuse of health care services. This understanding is essential to develop strategies to mitigate potential harm and plan solutions, with the aim of helping to protect members of the public from being marketed low-value tests, becoming patients unnecessarily, and taking resources away from genuine needs within the health system. Using Social Media Data to Understand the Impact of Promotional Information on Laypeople's Discussions: A Case Study of Lynch Syndrome.

Abstract:BACKGROUND:Social media is being used by various stakeholders among pharmaceutical companies, government agencies, health care organizations, professionals, and news media as a way of engaging audiences to raise disease awareness and ultimately to improve public health. Nevertheless, it is unclear what effects this health information has on laypeople. OBJECTIVE:This study aimed to provide a detailed examination of how promotional health information related to Lynch syndrome impacts laypeople's discussions on a social media platform (Twitter) in terms of topic awareness and attitudes. METHODS:We used topic modeling and sentiment analysis techniques on Lynch syndrome-related tweets to answer the following research questions (RQs): (1) what are the most discussed topics in Lynch syndrome-related tweets?; (2) how promotional Lynch syndrome-related information on Twitter affects laypeople's discussions?; and (3) what impact do the Lynch syndrome awareness activities in the Colon Cancer Awareness Month and Lynch Syndrome Awareness Day have on laypeople's discussions and their attitudes? In particular, we used a set of keywords to collect Lynch syndrome-related tweets from October 26, 2016 to August 11, 2017 (289 days) through the Twitter public search application programming interface (API). We experimented with two different classification methods to categorize tweets into the following three classes: (1) irrelevant, (2) promotional health information, and (3) laypeople's discussions. We applied a topic modeling method to discover the themes in these Lynch syndrome-related tweets and conducted sentiment analysis on each layperson's tweet to gauge the writer's attitude (ie, positive, negative, and neutral) toward Lynch syndrome. The topic modeling and sentiment analysis results were elaborated to answer the three RQs. RESULTS:Of all tweets (N=16,667), 87.38% (14,564/16,667) were related to Lynch syndrome. Of the Lynch syndrome-related tweets, 81.43% (11,860/14,564) were classified as promotional and 18.57% (2704/14,564) were classified as laypeople's discussions. The most discussed themes were treatment (n=4080) and genetic testing (n=3073). We found that the topic distributions in laypeople's discussions were similar to the distributions in promotional Lynch syndrome-related information. Furthermore, most people had a positive attitude when discussing Lynch syndrome. The proportion of negative tweets was 3.51%. Within each topic, treatment (16.67%) and genetic testing (5.60%) had more negative tweets compared with other topics. When comparing monthly trends, laypeople's discussions had a strong correlation with promotional Lynch syndrome-related information on awareness (r=.98, P<.001), while there were moderate correlations on screening (r=.602, P=.05), genetic testing (r=.624, P=.04), treatment (r=.69, P=.02), and risk (r=.66, P=.03). We also discovered that the Colon Cancer Awareness Month (March 2017) and the Lynch Syndrome Awareness Day (March 22, 2017) had significant positive impacts on laypeople's discussions and their attitudes. CONCLUSIONS:There is evidence that participative social media platforms, namely Twitter, offer unique opportunities to inform cancer communication surveillance and to explore the mechanisms by which these new communication media affect individual health behavior and population health.

An Analysis of United States Online Political Advertising Transparency

Abstract:During the summer of 2018, Facebook, Google, and Twitter created policies and implemented transparent archives that include U.S. political advertisements which ran on their platforms. Through our analysis of over 1.3 million ads with political content, we show how different types of political advertisers are disseminating U.S. political messages using Facebook, Google, and Twitter's advertising platforms. We find that in total, ads with political content included in these archives have generated between 8.67 billion - 33.8 billion impressions and that sponsors have spent over $300 million USD on advertising with U.S. political content. We are able to improve our understanding of political advertisers on these platforms. We have also discovered a significant amount of advertising by quasi for-profit media companies that appeared to exist for the sole purpose of creating deceptive online communities focused on spreading political messaging and not for directly generating profits. Advertising by such groups is a relatively recent phenomenon, and appears to be thriving on online platforms due to the lower regulatory requirements compared to traditional advertising platforms. We have found through our attempts to collect and analyze this data that there are many limitations and weaknesses that enable intentional or accidental deception and bypassing of the current implementations of these transparency archives. We provide several suggestions for how these archives could be made more robust and useful. Overall, these efforts by Facebook, Google, and Twitter have improved political advertising transparency of honest and, in some cases, possibly dishonest advertisers on their platforms. We thank the people at these companies who have built these archives and continue to improve them.
Designing Effective Digital Advertisements to Prevent Online Consumption of Child Sexual Exploitation Material

Abstract:The necessity of developing effective and innovative primary prevention strategies is a pressing objective in stemming the harm caused by the production and consumption of child sexual exploitation material (CSEM). Digital media interventions - including search engine advertisements - hold promise as prevention tools to deflect potential users away from CSEM and redirect them to therapeutic services. This study aimed to establish an evidence base for effective advertising in this context by creating mock-up helpline advertisements and testing them in focus groups and interviews with staff and clients of New Zealand's major treatment service providers for harmful sexual behavior. Prior to testing these advertisements in situ (on online platforms), this paper reports on a foundational creative and qualitative investigation of effective advertisements - what imagery, text, and design features are most likely to succeed in diverting people away from seeking CSEM and encouraging them to reach out to a helpline? The findings of this study offer deeper understanding of the mindset, emotions, and barriers to reaching out that are experienced by onset CSEM users, as well as suitability of certain imagery and text for advertising in this context, and thereby contribute to strengthening primary prevention efforts in combatting CSEM consumption.
Auditing Digital Platforms for Discrimination in Economic Opportunity Advertising

Abstract:Digital platforms, including social networks, are major sources of economic information. Evidence suggests that digital platforms display different socioeconomic opportunities to demographic groups. Our work addresses this issue by presenting a methodology and software to audit digital platforms for bias and discrimination. To demonstrate, an audit of the Facebook platform and advertising network was conducted. Between October 2019 and May 2020, we collected 141,063 ads from the Facebook Ad Library API. Using machine learning classifiers, each ad was automatically labeled by the primary marketing category (housing, employment, credit, political, other). For each of the categories, we analyzed the distribution of the ad content by age group and gender. From the audit findings, we considered and present the limitations, needs, infrastructure and policies that would enable researchers to conduct more systematic audits in the future and advocate for why this work must be done. We also discuss how biased distributions impact what socioeconomic opportunities people have, especially when on digital platforms some demographic groups are disproportionately excluded from the population(s) that receive(s) content regulated by law.
Before Blue Birds Became X-tinct: Understanding the Effect of Regime Change on Twitter's Advertising and Compliance of Advertising Policies

Abstract:Social media platforms, including Twitter (now X), have policies in place to maintain a safe and trustworthy advertising environment. However, the extent to which these policies are adhered to and enforced remains a subject of interest and concern. We present the first large-scale audit of advertising on Twitter focusing on compliance with the platform's advertising policies, particularly those related to political and adult content. We investigate the compliance of advertisements on Twitter with the platform's stated policies and the impact of recent acquisition on the advertising activity of the platform. By analyzing 34K advertisements from ~6M tweets, collected over six months, we find evidence of widespread noncompliance with Twitter's political and adult content advertising policies suggesting a lack of effective ad content moderation. We also find that Elon Musk's acquisition of Twitter had a noticeable impact on the advertising landscape, with most existing advertisers either completely stopping their advertising activity or reducing it. Major brands decreased their advertising on Twitter, suggesting a negative immediate effect on the platform's advertising revenue. Our findings underscore the importance of external audits to monitor compliance and improve transparency in online advertising.

Unraveling the mirage of fairness cream commercials: A cross‐sectional analysis of commercials from South Asia

Abstract:Background Fairness products are an essential component of daily beauty routines for many individuals in subcontinental Asia. However, it is important to be aware that these products often contain ingredients that can be detrimental to the skin and are banned in several developed countries. Objective Our study aims to analyze the content of fairness cream commercials in order to gain a deeper understanding of the information used to persuade and influence consumers to use these products. Methods Fairness cream commercials originating from countries in subcontinental Asia, including India, Pakistan, Bangladesh, Sri Lanka, and Nepal, were specifically searched and analyzed on the YouTube platform. Results An analysis of 152 fairness cream commercials on YouTube identified 84.21% of commercials targeted female consumers, while only 15.79% targeted male consumers. 77.63% of commercials used celebrities in their commercials and 47.37% of commercials mentioned specific ingredients. Conclusions Based on our findings, it is crucial for dermatologists to take an active role in educating patients and consumers about the potential risks associated with certain ingredients found in fairness creams. Dermatologists should emphasize the importance of prioritizing overall skin health rather than solely focusing on skin lightening.

Abstract:Targeted advertising remains an important part of the free web browsing experience, where advertisers' targeting and personalization algorithms together find the most relevant audience for millions of ads every day. However, given the wide use of advertising, this also enables using ads as a vehicle for problematic content, such as scams or clickbait. Recent work that explores people's sentiments toward online ads, and the impacts of these ads on people's online experiences, has found evidence that online ads can indeed be problematic. Further, there is the potential for personalization to aid the delivery of such ads, even when the advertiser targets with low specificity. In this paper, we study Facebook -- one of the internet's largest ad platforms -- and investigate key gaps in our understanding of problematic online advertising: (a) What categories of ads do people find problematic? (b) Are there disparities in the distribution of problematic ads to viewers? and if so, (c) Who is responsible -- advertisers or advertising platforms? To answer these questions, we empirically measure a diverse sample of user experiences with Facebook ads via a 3-month longitudinal panel. We categorize over 32,000 ads collected from this panel ($n=132$); and survey participants' sentiments toward their own ads to identify four categories of problematic ads. Statistically modeling the distribution of problematic ads across demographics, we find that older people and minority groups are especially likely to be shown such ads. Further, given that 22% of problematic ads had no specific targeting from advertisers, we infer that ad delivery algorithms (advertising platforms themselves) played a significant role in the biased distribution of these ads.
Marketing to Children Through Online Targeted Advertising: Targeting Mechanisms and Legal Aspects

Abstract:Many researchers and organizations, such as WHO and UNICEF, have raised awareness of the dangers of advertisements targeted at children. While most existing laws only regulate ads on television that may reach children, lawmakers have been working on extending regulations to online advertising and, for example, forbid (e.g., the DSA) or restrict (e.g., the COPPA) advertising based on profiling to children. At first sight, ad platforms such as Google seem to protect children by not allowing advertisers to target their ads to users who are less than 18 years old. However, this paper shows that other targeting features can be exploited to reach children. For example, on YouTube, advertisers can target their ads to users watching a particular video through placement-based targeting, a form of contextual targeting. Hence, advertisers can target children by placing their ads in children-focused videos. Through a series of ad experiments, we show that placement-based targeting is possible on children-focused videos and enables marketing to children. In addition, our ad experiments show that advertisers can use targeting based on profiling (e.g., interest, location, behavior) in combination with placement-based advertising on children-focused videos. We discuss the lawfulness of these two practices concerning DSA and COPPA. Finally, we investigate to which extent real-world advertisers are employing placement-based targeting to reach children with ads on YouTube. We propose a measurement methodology consisting of building a Chrome extension to capture ads and instrument six browser profiles to watch children-focused videos. Realistic skin vs. flawless skin: explicating the appeal of retouch-free advertising among Chinese women

Abstract:Using retouched images of models in advertisements to embody idealized beauty prototypes (e.g., flawless skin) in various cultures has been a prevalent yet controversial practice. Guided by the overarching corporate moral responsibility framework, this study conducted two online experiments to assess the effects of ad retouching and disclaimers on advertising effectiveness. In Study 1, a 2 (model skin type: realistic skin vs. flawless skin) x 2 (retouch-free disclaimer: present vs. absent) between-subjects online experiment was conducted among Chinese female participants. The results revealed that Chinese female consumers' preference for flawless skin still drives purchase intentions toward a skincare brand. However, when an ad employed a retouch-free disclaimer, participants who saw an ad depicting a model with realistic skin indicated higher ad honesty than those who saw an ad depicting a model with flawless skin. Consumers' perceived ad honesty mediated the interaction effect between the model skin type and disclaimer on purchase intentions. In Study 2, we found a similar interaction effect and moderated mediation effects. Such that, when a retouch-free disclaimer was present, participants rated an ad featuring realistic skin to have significantly higher ad honesty than an ad featuring flawless skin, which in turn, led to higher purchase intentions. The findings provide theoretical and practical insights into how brands can better appeal to contemporary Chinese female consumers.

Abstract:In this study we analyzed content and marketing tactics of digital medicine companies to evaluate various types of cross site tracking middleware used to extract health information from users without permission. More specifically we examine how browsing data can be exchanged between digital medicine companies and Facebook for advertising and lead generation purposes. The analysis was focused on a small ecosystem of companies offering services to patients within the cancer community that frequently engage on social media. Some companies in our content analysis may fit the legal definition of a personal health record vendor covered by the Federal Trade Commission, others are HIPAA covered entities. The findings of our analysis raise policy questions about what constitutes a breach under the Federal trade Commission's Health Breach Notification Rule. Several examples demonstrate serious problems with inconsistent privacy practices and reveal how digital medicine dark patterns may elicit unauthorized data from patients and companies serving ads. Further we discuss how these common marketing practices enable surveillance and targeting of medical ads to vulnerable patient populations, which may not be apparent to the companies targeting ads.
Not Just Skipping. Understanding the Effect of Sponsored Content on Users' Decision-Making in Online Health Search

Abstract:Advertisements (ads) are an innate part of search engine business models. Advertisers are willing to pay search engines to promote their content to a prominent position in the search result page (SERP). This raises concerns about the search engine manipulation effect (SEME): the opinions of users can be influenced by the way search results are presented. In this work, we investigate the connection between SEME and sponsored content in the health domain. We conduct a series of user studies in which participants need to evaluate the effectiveness of different non-prescription natural remedies for various medical conditions. We present participants SERPs with different intentionally created biases towards certain viewpoints, with or without sponsored content, and ask them to evaluate the effectiveness of the treatment only based on the information presented to them. We investigate two types of sponsored content: 1. Direct marketing ads that directly market the product without expressing an opinion about its effectiveness, and 2. Indirect marketing ads that explicitly advocate the product's effectiveness on the condition in the query. Our results reveal a significant difference between the influence on users from these two ad types. Though direct marketing ads are mostly skipped by users, they can tilt users decision making towards more positive viewpoints. Indirect marketing ads affect both the users' examination behaviour and their perception of the treatment's effectiveness. We further discover that the contrast between the indirect marketing ads and the viewpoint presented in the organic search results plays an important role in users' decision-making. When the contrast is high, users exhibit a strong preference towards a negative viewpoint, and when the contrast is low or none, users exhibit preference towards a more positive viewpoint.

Abstract:Summary 1. The blood picture of 14, 15 and 16 day White Leghorn chick embryos injected into the allantois with aqueous adrenal cortex extract, shows no significant change in the number of lymphocytes but a market absolute polymorph leukocytosis, 24 hours after injection. The erythrocytes are not affected by the extract. 2. Large individual variation in the lymphocyte count and greater difficulty involved in their identification may have obscured the effect of the hormonal preparation on the lymphocyte count. 3. ACE treatment induced a tendency (not statistically significant) toward weight loss in the 14 day embryos, a significant decrease in the hatchability rate, and an apparent defective feather development. However the blood picture and weight in comparison with the controls was normal after hatching.
