Abstract:Gastroschisis, a common congenital abdominal wall defect, is associated with abnormal development, and a high rate of mortality without prompt treatment. Compared with the trends in 1970s and 1980s, there has been an overall increased trend in the prevalence of this congenital birth defect, although differences in incidence have been reported among countries, with increases in some and decreases in others. Potential risk factors for gastroschisis include gender, mother's age, and a rural/urban residence. Few data exist on its prevalence and time trends in China.This study examined the time trend for prevalence of gastroschisis in China between 1996 and 2007, and also investigated the possible confounding effects of risk factors on changing trends. Data were obtained from the Chinese Birth Defects Monitoring Network on 6,308,594 hospital-delivered live birth and stillbirth infants born in the 28th gestational week or later during the 12-year study period. The seventh day after birth was the maximal diagnosis time assessed. The chi(2) test and the Poisson regression model were used to analyze overall secular prevalence trends during the study period, with adjustments for feature-specific risk factors, including gender, mother's age, and a rural/urban residence.The overall prevalence of gastroschisis in China between 1996 and 2007 was 2.54 per 10,000 births (1601/6,308,594 births). No significant changes were found in the overall prevalence of gastroschisis during the 12-year study period. There were no differences for feature-specific risk factors except for increased prevalence among infants of mothers who were 20 to 24 years old (P = 0.0498 and P = 0.0032, respectively, for the chi(2) test and the Poisson regression analysis) and decreased prevalence among infants of mothers 30 to 34 years old (P = 0.0177 for the Poisson regression analysis).These findings show no significant change in the overall prevalence of gastroschisis in China between 1996 and 2007. For specific risk factors, however, differences occur, with respect to the age of mothers of affected infants, with increase in risk for infants of 20- to 24-year-old mothers and a decrease among those with 30- to 34-year-old mothers.
