Abstract:OBJECTIVE:To analyze the incidence, trends and related factors of birth defects in Huai'an from 2008 to 2020. METHODS:The surveillance data from maternal and child health system of Huai'an from 2008 to 2020 and Huai'an Statistical Yearbook were used for analysis. Taking the annual change percentage and average annual change percentage (AAPC) as the main outcome indicators, the JoinPoint regression analysis was performed to estimate the changing trend of birth defects from 2008 to 2020. Spearman correlation analysis was used to examine the association between birth defects and birth rate, marriage rate, proportion of women with advanced maternal age. RESULTS:During 2008 to 2020, a total of 3414 cases of neonatal birth defects occurred in Huai'an, with an incidence of 4.6‰ (3414/736 608). The rate of perinatal birth defects in Huai'an showed an increasing trend (AAPC=8.8%, t=3.2, P<0.01), and the year of 2016 was a significant changing point. Among 24 types of birth defects, the incidence of congenital heart disease rose and became the most prevalent defect, while the incidence of neural tube malformations such as anencephaly, encephalocele and spina bifida was declined. The incidence of birth defect was negatively correlated with the birth rate ( r=-0.751, P<0.01), not correlated with marriage rate ( r=-0.516, P>0.05), and positively correlated with the proportion of women with advanced maternal age ( r=0.726, P<0.01). CONCLUSION:The incidence of birth defects in Huai'an shows an increasing trend from 2008 to 2020 with congenital heart disease as the most common type of birth defect, and the increase of birth defects incidence is closely related with the increase of the proportion of women with advanced maternal age.

Abstract:Abstract Background To investigate the prevalence and prenatal diagnosis rate of chromosomal abnormalities (CA) in Zhejiang Province, China. Methods We estimated the annual changes in the detected prevalence of CA and prenatal diagnosis rate among 681,590 births in Zhejiang Province, China, between 2014 and 2020. Data were derived from the provincial birth defects surveillance system, which represents 30% of annual births in Zhejiang Province. The effect of maternal age was also evaluated. Results The detected prevalence of sex chromosomal abnormalities (1.70–7.30 per 10,000 births, Ptrend < 0.001) and microdeletion and microduplication (0.30–6.81 per 10,000 births, Ptrend < 0.001) gradually increased, contributing to an upward trend in overall CA (12.09–39.22 per 10,000 births). The diagnosis rate before 22 gestational weeks constantly increased from 20.8 to 70.1% for trisomy 21 (Ptrend = 0.003). The prevalence rate ratio for maternal age of ≥ 35 years was higher than that for maternal age of 25–29 years for trisomy 21 (5.40, 95% confidence interval [CI] 4.59–6.35) and sex chromosomal abnormalities (3.28, 95% CI 2.48–4.33). Conclusions The rising prevalence of CA in China may be attributable to the elevated maternal age and the innovation of prenatal diagnosis tools, Thus, studies should pay attention to the rare CA that were previously ignored, and select rational screening tools.
Epidemiology of Birth Defects in Teenage Pregnancies: Based on Provincial Surveillance System in Eastern China.

Abstract:BackgroundHealthcare for adolescents and birth defects (BD) prevention are highlighted public health issues. The epidemiology of birth defects in teenage pregnancies has not been studied extensively. ObjectivesTo investigate the prevalence trend and spectrum of BDs among teenage mothers. MethodsThis observational study covered all births registered in the BD surveillance system in Zhejiang Province, China, during 2012-2018. The annual change in the prevalence of BDs among adolescent mothers was estimated. Crude relative ratios using the BD categories in teenage pregnancies were calculated and compared with those in women aged 25-29 years. ResultsOverall, 54,571 BD cases among 1,910,977 births were included in this study, resulting in an overall prevalence of 234.64 to 409.07 per 10,000 births from 2012 to 2018 (P-trend < 0.001) in total population. The prevalence of birth defects in teenage pregnancies increased from 247.19 to 387.73 per 10,000 births in 2012-2018 (P-trend = 0.024). The risks of neural tube defects (relative risk [RR] = 3.15, 95% confidence interval [CI] 2.56, 3.87), gastroschisis (RR = 7.02, 95% CI 5.09, 9.69), and multiple birth defects (RR=1.27, 95% CI 1.07, 1.52) were higher in teenage pregnancies than those in women aged 25-29 years. ConclusionsWe found a distinctive spectrum of BDs, with higher proportions of fatal or multiple anomalies in infants born to teenage mothers than in those born to adults aged 25-29 years. These results emphasize the importance of providing adolescents with better access to reproductive and prenatal care.

Abstract:BackgroundCongenital heart defect is the leading malformation in China. There may have been changes in congenital heart defect incidence because of birth policy shift in China over past years. This study aimed to investigate the epidemiology, prenatal diagnosis, and outcomes of congenital heart disease to improve medical and policy decisions.MethodsData on cases of congenital heart disease identified during 2014-2018 were taken from the Zhejiang provincial birth defects surveillance system. Chi-square test, odds ratio (OR) and 95% confidence interval (CI) were used to explore epidemiology, prenatal diagnosis, and birth outcomes of congenital heart disease.ResultsThe average incidence of congenital heart disease was 16.0 per 1000 births, which increased by 62.2% during 2014-2018(chi (2)(trend)=181.41, P<0.001). However, the average critical congenital heart incidence was 1.6 per 1000 births, which remained stable over time. Women aged 20years (OR2.1, 95% CI 1.9-2.3) or >= 35years (OR 1.2, 95% CI 1.2-1.3) were at higher risk of having babies with congenital heart disease than women aged 21-34years. Women who gave birth in urban areas (OR 1.2, 95% CI 1.2-1.3), had a son (OR 1.3, 95% CI 1.3-1.4), or had multiple births (OR 4.0, 95% CI 3.7-4.4) were also at higher risk than those giving birth in rural areas, to girls, or single births, respectively. The three major subtypes of congenital heart disease were atrial septal defect (67.9%), patent ductus arteriosus (34.7%), and ventricular septal defect (6.4%). The prenatal detection rate of critical congenital heart disease was 90.0%, which was far higher than total congenital heart disease, at 22.2% (chi (2)=1687.67, P<0.001). There were 1457 (17.1%) stillbirths, 106 (1.2%) early neonatal deaths, and 6983 (81.7%) live births associated with congenital heart disease.ConclusionsThe high incidence of congenital heart disease in Zhejiang might be attributable to the large proportion of mild congenital heart disease. The incidence of critical congenital heart disease, the prenatal detection rate, and perinatal deaths from congenital heart disease are comparable to those in other studies.
Changes in the Incidence of Birth Defects in Sichuan Province, Southwest China from 2006 to 2019: A Retrospective Analysis Based on Hospital Birth Defect Monitoring

Abstract:Abstract Background: As a province with a large population in Southwest China, Sichuan has a relatively high incidence of birth defects (BDs), but the current status and characteristics of BDs are unclear. This study aimed to analyse the changing trends of BDs in the past 14 years and propose countermeasures.Methods: Data were obtained from the Sichuan Provincial Birth Defects Hospital Surveillance Network (SPBDHSN); the surveillance objects were perinatal infants (PIs) and BD infants who were hospitalized in the surveillance hospital from 2006 to 2019. SPSS 22.0 software was used for relevant statistics and analysis; descriptive and chi-square test analyses were used to calculate the incidence of BDs in the whole province, region of the mother, sex of the baby and maternal age.Results: From 2006 to 2019, 1,799,914 PI cases were monitored, and 26,797 newborns were diagnosed with BDs. The average incidence of BDs was 148.88 per 10,000 births. The average incidence of BDs based on region (urban, rural) and sex (male, female) was 237.01, 87.6, 166.4, and 129.63 per 10,000 births, respectively. The average rates of birth defects for maternal age (<20, 20-25, 25-30, 30-35, >35 years) were 142.77, 133.01, 147.96, 170.39 and 180.37 per 10,000, respectively. P values according to the chi-square test for the incidence of BDs by region, sex of the baby, and maternal age were all less than 0.01, with significant differences. The incidence of BDs in the province varied from year to year, with the first deformity gradually transitioning from body surface deformity to visceral deformity.Conclusions: The prevention and treatment of BDs in Sichuan has achieved remarkable results, and the incidence of birth defects has begun to show a downward trend in recent years. However, due to the imbalance of economic and social development between regions in the province, the prevention and treatment of birth defects still faces huge challenges.
Changes in Maternal Age and Prevalence of Congenital Anomalies During the Enactment of China's Universal Two-Child Policy (2013-2017) in Zhejiang Province, China: an Observational Study.

Abstract:BACKGROUND:China implemented a partial two-child policy (2013) followed by a universal two-child policy (2015), replacing the former one-child policy mandated by the government. The changes affect many aspects of China's population as well as maternal and infant health, but their potential impact on birth defects (BDs) remains unknown. In this study, we investigated the associations of these policy changes with BDs in Zhejiang Province, China. METHODS AND FINDINGS:We used data from the BD surveillance system in Zhejiang Province, China, which covers 90 hospitals in 30 urban districts and rural counties, capturing one-third of the total births in this province. To fully consider the time interval between conception and delivery, we defined the one-child policy period as data from 2013 (births from October 2012 to September 2013), the partial two-child policy period as data from 2015 (births from October 2014 to September 2015), and the universal two-child policy period as data from 2017 (births from October 2016 to September 2017). Data from 2009 and 2011 were also used to show the changes in the proportion of births to women with advanced maternal age (35 years and older) prior to the policy changes. Main outcome measures were changes in the proportion of mothers with advanced maternal age, prevalence of BDs, rankings of BD subtypes by prevalence, prenatal diagnosis rate, and live birth rate of BDs over time. A total of 1,260,684 births (including live births, early fetal losses, stillbirths, and early neonatal deaths) were included in the analyses. Of these, 644,973 (51.16%) births were to women from urban areas, and 615,711 (48.84%) births were to women from rural areas. In total, 135,543 (10.75%) births were to women with advanced maternal age. The proportion increased by 85.68%, from 8.52% in 2013 to 15.82% in 2017. However, it had remained stable prior to policy changes. Overall, 23,095 BDs were identified over the policy changes (2013-2017). The prevalence of BDs during 2013, 2015, and 2017 was 245.95, 264.86, and 304.36 per 10,000 births, respectively. Trisomy 21 and other chromosomal defects increased in both risk and ranking from 2013 to 2017 (crude odds ratio [95% confidence interval] 2.13 [1.75-2.60], from ranking 10th to 5th, and 3.63 [2.84-4.69], from ranking 16th to 6th, respectively). The prenatal diagnosis rate increased by 3.63 (2.2-5.1) percentage points (P < 0.001), from 31.10% to 34.72%, and identification of BDs occurred 1.88 (1.81-1.95) weeks earlier (P < 0.001). The live birth rate for infants with BDs born before 28 gestational weeks increased from 1.29% to 11.45%. The major limitations of this observational study include an inability to establish causality and the possible existence of unknown confounding factors, some of which could contribute to BDs. CONCLUSIONS:In this study, we observed significant increases in maternal age and the prevalence of total and age-related anomalies following China's new two-child policy. Increases in live birth rate for infants with BDs born before 28 gestational weeks suggest that healthcare for very preterm births with BDs may be warranted in the future, as well as updating the definition of perinatal period.

Abstract:OBJECTIVE:To study the prevalence rates of birth defects in high and low risk areas in China.METHODS:A population-based surveillance system on birth defects was used to obtain the prevalence rates of 24 kinds of major external birth defects from > or = 20 weeks of gestation to 7 days of life in selected areas in Shanxi and Jiangsu provinces.RESULTS:The birth prevalence of birth defects (232.4 per 10,000 births) and neural tube defects (NTDs) (138.7 per 10,000 births) in four counties of Shanxi province were significantly higher than that in Taiyuan city (75.3 and 28.2 per 10,000 births, respectively). There was no significant difference for all selected birth defects between Wuxi city and Xishan counties in low risk areas. There was a 6.1-fold of higher prevalence for NTDs in Taiyuan city compared with that in Wuxi areas (4.6 per 10,000 births). In four counties of Shanxi province, the prevalence rates of anencephaly, spina bifida, hydrocephaly, cleft palate alone and polydactyly were significantly higher than in Wuxi areas. The NTDs prevalence rate in four counties of Shanxi was 30.2 times higher than in Wuxi areas. When compared with previous surveillance data, the NTDs prevalence rate did not present obvious declining trend in high risk areas. The birth prevalence rate had a 31.8% decrease when births were calculated after 28 gestational weeks and compared with those from 20 gestational weeks.CONCLUSION:NTDs remained to be the most common birth defect seen in Shanxi province. The birth prevalence rate of NTDs in some areas of Shanxi province was among the highest that ever reported in the world in comparison with data from other countries and regions. The current prevalence rate in high risk areas in Shanxi province did not clearly show a declining trend. Programs on surveillance and prenatal diagnosis were proved to have made big impact on the rates of major external birth defects.

Abstract:Objective: This study analyzed the prevalence, epidemiological characteristics and risk factors of birth defects among livebirths in central China, aiming to provide evidences for the prevention of birth defects and government Decision-makings. Methods: Birth data from China's Hubei Province between 2015 and 2022 were collected, including basic information of the livebirths, the mothers and the fathers, as well as information about delivery and each prenatal examination. The livebirths prevalence of birth defects was calculated and the trends were mapped. The basic characteristics of birth defects were evaluated by the difference analysis between case and health groups. Univariate and multivariate Poisson regression was performed to examine the independent risk factors for birth defects. Results: Among 43,568 livebirths, 166 livebirths were born with birth defects, resulted in a total prevalence rate of 3.81 per 1,000 livebirths, showing a remarkable uptrend from 0.41per 1,000 livebirths in 2015 to 9.23 per 1,000 livebirths in 2022. The peak of the prevalence was in January and February. Congenital malformation of the musculoskeletal system was the main type of birth defect in central China livebirths, followed by cleft lip and cleft palate. Overall, newborns with birth defect had significantly earlier delivery gestational age, poorer health and higher proportion of infants with low birth weight than healthy births. The gender of livebirths, excess weight at delivery (≥80 kg) of mothers, more than 2 times of gravidity or parity of mothers, and advanced paternal age (≥40 years) were independent risk factors for birth defects (or specific birth defects). Conclusion: The livebirths prevalence of birth defects shows increasing trend in central China, which deserves the attention of the government and would-be parents. Elevated paternal age, excess maternal weight, gravidity and parity should be considered when planning their families.
Epidemiology of Birth Defects in High-Prevalence Areas of China

Abstract:OBJECTIVE:To study the levels of prevalence and to describe the epidemiological features of birth defects in high-prevalence areas in China.METHODS:A population-based retrospective study was conducted in two counties of Shanxi province that including birth defects among fetuses and live births born after 20 weeks' gestational age in the study areas from 2002 through 2004.RESULTS:During 2002 - 2004, the prevalence of birth defects was 844.2 per 10 000 births in study areas and the first five main birth defects were inguinal hernia (182.2 per 10,000 births), anencephaly (104.4 per 10 000 births), congenital mental retardation (79.4 per 10,000 births), congenital heart diseases (73.2 per 10,000 births) and spina bifida (63.9 per 10,000 births). These five main birth defects accounted for about 60% of total birth defect cases. Inguinal hernia, undescended testicle, congenital mental retardation were usually not included for statistical analysis on birth defect. Excluding these birth defects, the prevalence of birth defects in study areas was 537.2 per 10,000 births and the first five main birth defects were anencephaly, congenital heart diseases, spina bifida, hydrocephaly (40.5 per 10 000 births) and encephalocele (31.2 per 10 000 births). The male prevalence of birth defects was 966.2 per 10 000 births, which was significantly higher than prevalence in females. The prevalence rates of birth defects for maternal age groups under 20 years old and over 30 years old were remarkably higher than those for 20-24 year and 25-29 year old mothers. The teenage mothers were at higher risk of having a child with neural tube defects(NTDs) or congenital heart diseases. Prevalence rates under 20 year maternal age group were 408.8 and 188.7 per 10 000 births for NTDs and congenital heart diseases respectively, whereas corresponding rates were 204.8 and 91.0 per 10 000 births for groups over 30 years of age. The prevalence rates increased with increasing gravidity or parity.CONCLUSION:This study showed that the patterns of birth defects in study areas were very different from those of other areas, with an unusually high prevalence of NTDs. The study also suggested that both young and advanced mothers were at increased risk of having a child with a birth defect.

Abstract:Objective:To obtain the actual birth prevalence rate and the distribution of major external birth defects in high risk areas of Shanxi province, China. Methods:The population-based birth defects surveillance system was used to monitor 24 kinds of major external birth defects in four selected counties (Pingding, Xiyang, Taigu and Zezhou counties) in Shanxi Province. Results:A total of 11534 births (from 20 weeks of gestation to 7 days of life) were monitored in four counties in year 2003. The prevalence rate of birth defects, including 24 kinds of major external structural defects, was 232.4 per 10,000 births. The birth prevalence of NTDs was 138.7 per 10,000 births, which accounted for 59.7% of all cases. There were 7 kinds of birth defects with prevalence rate more than 10 per 10,000 births, including in turn: anencephaly(65.9/10,000),Spina bifida (58.1/10,000),hydrocephaly (47.7/10,000),polydactyly (26.0/10,000),cleft palate (21.7/10,000),encephalocele (14.7/10,000),cleft lip (10.4/10,000). The prevalence of NTDs among off spring of women with lower education and the female infants/fetuses was significantly higher. There was also increasing trend for prevalence of birth defects and NTDs among women living in rural area, or with age less than 20 or more than 30 years. Conclusion: Neural tube defects are still the most common birth defects in high risk areas of Shanxi province,and the birth prevalence rate of NTDs are among the highest level worldwide.Women living in rural area,with lower education,too young or older pregnant age may lave the higher risk of having a child with birth defects.

Abstract:What is already known about this topic?: The Shanxi Province, located in northern China, holds the highest prevalence of birth defects (BDs) across the country. Following the implementation of a nationwide folic acid supplementation program in 2009, a significant reduction of 53.89% in the prevalence of neural tube defects (NTDs) was observed in Shanxi from 2012 to 2017. However, despite this decrease, the prevalence rate for congenital heart defects (CHDs) in 2017 was over four times that of the 2012 rate. Since 2014, CHDs have emerged as the most predominant BD in Shanxi.What is added by this report?: The present study has identified a marked reduction in the prevalence of both total BDs and NTDs in five counties within Shanxi over the past two decades. As of 2017-2022, NTDs continue to be the most prevalent BDs recorded in this region. Contrarily, there has been a noteworthy increase in the prevalence of CHDs, ranking them among the top five most common BDs in the region between 2017 and 2022, though their rate remains below the national average. Additionally, the proportion of external anomalies remains high. Nevertheless, due to constrained access to primary healthcare services and diagnostic facilities, the early detection rate for internal anomalies, particularly CHDs, may be underestimated in the region.What are the implications for public health practice?: The results of this study underscore the necessity for augmented efforts in promoting folic acid supplementation as a preventive measure for NTDs. Moreover, improvements in the distribution of medical resources within this region are recommended, particularly the introduction and enforcement of local training programs aimed at enhancing CHD screening and diagnostic processes in these respective counties.

Abstract:Introduction:The prevalence of birth defects (BDs) at ≥28 gestational weeks in China has declined significantly in recent years. However, a few studies have implied that the prevalence is underestimated due to pregnancies with severe BDs often being terminated before 28 weeks of gestation. This study sought to contribute to this data conflict by determining the total prevalence of BDs throughout the entirety of pregnancies, depicting the epidemiological distribution of BDs in Shaanxi Province, and examining the impact of pregnancy termination before 28 weeks of gestation on overall BD prevalence.Methods:Based on data extracted from the provincial Birth Defects Surveillance Network in Shaanxi Province, from 2014 to 2020, the trends of 18 frequent, major BDs at any gestational age were analysed.Results:The total prevalence of BDs throughout pregnancy in Shaanxi Province increased significantly from 2014 to 2020, partly due to the inclusion of slight congenital heart diseases. It was also shown that the prevalence of all major types of BD had been previously underestimated, particularly severe external BDs, anywhere from 10% to 100% due to the previous sole measurement of terminations after 28 gestational weeks. Neural tube defects, however, remained one of the top five BDs with the highest total prevalence.Conclusions:The exclusion of pregnancy terminations <28 weeks of gestation resulted in a severe underestimation of the total birth prevalence of BDs, particularly severe external defects. To estimate the true population-level prevalence of BD better, the current BD surveillance system should include pregnancy terminations before 28 weeks of gestation.

Abstract:OBJECTIVE:This research was to compare the occurrence levels of birth defect, to describe the distribution of primary birth defects in different range of monitored ages and to provide data to China birth-defect monitoring system.METHODS:A retrospective study on birth defect was conducted in two counties, Shanxi province, China, which covered birth defects among fetuses after 20 weeks' gestational age from 2002 through 2004. Data collected on birth defect cases mainly included extrinsic and visceral anomaly.RESULTS:The occurrence rates of the monitored structural birth defects significantly increased with the increase of age. The occurrence rates were 17.6, 34.0, 43.6, and 53.7 per 1000 births, for different statistical range, from 20-week to 27-week gestational age, 7 days, 1 year and 3 years after birth, respectively. The range from 28-week gestational age to 7 days after birth was usually regarded as the routinely monitored range. If the occurrence rate was calculated from the 20-week gestational age, it appeared a 2.1-time increase. However, if the range was changed to 1 or 3 years after birth, the occurrence rate increased to 2.7 or 3.3 times high, respectively. The distribution of time when birth-defect was identified was significantly different by categories with majority of neural tube defect cases diagnosed at antepartum or 7 days after birth. Visceral defects were mainly found at 7 days after birth but increased with age, even some were diagnosed at 1 year after birth.CONCLUSION:The routine Chinese monitoring program might detect approximately 1/3 of those structural birth defects with the base of current technique and monitoring range from 28-week gestational age to 7 days after birth. The result of our findings should be of help to other related studies.

Abstract:What is already known about this topic?Shanxi Province in northern China has been identified as a region with the highest prevalence of birth defects nationwide. With large amounts of financial support devoted for prevention and related interventions for birth defects, huge progress has been made as a 60% decrease in its prevalence was observed from 2009 to 2014.What is added by this report?The study presented a recent trend of adverse pregnancy outcomes (APOs) in Shanxi Province, a region with high prevalence of birth defects in China from 2007-2019. The prevalence of serious APOs including birth defects, stillbirths, and neonatal deaths remained at a relatively low level, yet macrosomia, low birth weight (LBW), preterm births, and spontaneous abortion comprised a majority of all APOs, with macrosomia showing an obvious upward trend from 2007 to 2012.What are the implications for public health practice?These findings provide new evidence for prevention and intervention strategies of APOs in northern China. Future research should focus on comprehensive interventions for multiple APOs, especially macrosomia, LBW, preterm births, and spontaneous abortion.
Prevalence of neural tube defects at birth in 30 counties and cities of China, 1993-2000

Abstract:Objective:To describe the prevalence rates of neural tube defects (NTD) and yearly trends (1993－2000) in China. Methods: All the data were obtained from the Birth Defects Surveillance System in thirty counties/cities of China. The calculation of NTD prevalence rates and time trend analyses were based on the data for live (L) and stillbirths (S) only, the denominator used was total births (L+S). Linear regression analysis of the prevalence rate over years has been used to measure time trends. Results: In the period of 1993－2000, there were 1 264 neural tube defects among 1 189 126 total births in 30 counties/cities in China. The overall neural tube defects rate was 10.63 per 10000 births, and rates were higher (18.99 per 10 000 births) in the year 1993, lower (6.05 per 10 000 births) in 1998. The prevalence rates of anencephaly, spina bifida and encephalocele were 4.71,4.39 and 1.53 per 10 000 births respectively. Of all index NTD cases, anencephaly and spina bifida were most frequent, making up 44.3% and 41.3%, and encephalocele represented 14.4%. The stillbirth proportion was 69.3% over all index NTD cases, 95.4% in anencephaly cases, 43.7% in spina bifida, and 62.6% in encephalocele cases. Linear trend analysis indicated a significant decline for NTD rate (F=11.818, β=－0.814,P=0.014). Analysis by specific defect showed significant declines for the rates of anencephaly (P=0.004) and spina bifida (P=0.026), but no significant annual variation of encephalocele (P=0.227). Results of comparing with data reported from other surveillance systems (1994－1999) showed that the highest NTD rate (9.41 per 10000 births) was seen in China, nearly7 times the lowest rate (1.44 per 10000 births) in England and Wales. Conclusion: The overall neural tube defects rate in 30 counties/cities of China presented a significant downward trend between 1993 and 2000, and China still had the higher neural tube defects prevalence. 

Abstract:Objective To compare the birth defects data from two different birth defect(BD) survey methods in high-prevalence areas of China.Methods Two counties of Shanxi province with high prevalence of BDs were selected as survey fields.The BDs among fetuses and live births born after 20 weeks' gestational age from 2002 through 2004 were assessed by two survey methods,population-based and hospital-based BDs surveys.The population-based data were collected from hospital records and field survey,while the hospital-based data from registry records of department of obstetrical,B ultrasonics in the local hospitals and maternal and child health centers.Results There were 4 855 births reported in hospital-based birth source and 6 420 births in population-based birth source.For pregnancy from prenatal 20th week to postnatal 7 days,the incidence of BDs was 232.7 per 10 000 births in hospital-based survey,which was significantly lower than that from population-based survey(232.7 vs 347.4 per 10 000 births,P0.001).Considering the category of BDs during prenatal 20th week to postnatal 7 days,the prevalence of some malformations including the malformation involved in nervous system,ear,urinary system and skin malformation was higher in population-based survey compared with hospital-based survey.The top ranked BDs were same at this monitoring period in two different surveys,with anencephalus,spina bifida,congenital hydrocephalus,encephalocele and lip and/or cleft palate being the top five.With the survey monitoring period extending to postnatal 3 years,the BD prevalence from population-based survey source increased by 2.43 folds,and 3.63 times of that from hospital-based survey source.Inguinal hernia ranked top of BDs,followed by anencephalus,congenital mental retardation,congenital heart disease and spina bifida.Conclusions The incidence of BDs and ranking were impacted by the monitoring method and range of monitoring ages.It may need to extend the monitoring period to 3 years old from the routine Chinese monitoring in order to obtain complete BDs data and provide a reliable basis for the BD prevention.
[A Population-Based Surveillance System on Birth Defects and Its Application].

Abstract:OBJECTIVE:To describe a unique surveillance system on birth defects in the People's Republic of China.METHODS:The system was instituted in 1992 as a component for the evaluation on the effectiveness of a community intervention program using periconceptional folic acid supplementation to prevent neural tube defects and currently carries out surveys on a birth cohort of approximately 150 000 infants per year. Information was collected in the form of detailed written descriptions by local health care providers and photographs of affected infants. The system allowed for detection of birth defects at the local level with later definitive classification and coding; however, information was limited to those structural anomalies which were visible on physical examination.RESULTS:This surveillance system on birth defects provides an extensive database of infants with major and minor external structural anomalies. From 1993 to 1996, more than one million births were monitored, with more than 10 000 birth defects and 30 000 photos submitted. Among them 5 000 were identified with major birth defects. Neural tube defects, oral clefts and extremity abnormalities were the first few defects with most cases. Data were used in birth defect surveillance reports and scientific papers.CONCLUSIONS:This system can be utilized for studies on etiology, descriptive epidemiology, and identification of unusual trends which is unique with multiple advantages in the People's Republic of China.
Spatio-Temporal Distribution and Demographic Characteristics of Congenital Heart Defects in Guangdong, China, 2016–2020

Abstract:Background Congenital heart defects are the most common type of birth defects and bring a heavy disease burden in China. Examining the temporal and spatial trends of congenital heart defects epidemics can give some elementary knowledge for succeeding studies. Objective To characterize the spatial-temporal patterns of the prevalence of congenital heart defects based on a substantial cohort of the perinatal fetus in south China in 2016–2020. Methods This study was a retrospective population-based cohort study conducted in Guangdong, China from 2016 to 2020. Pregnant women and their infants received birth defect surveillance during pregnancy and seven days after delivery in more than 1,900 midwifery hospitals in 21 cities. Perinatal infants with congenital heart defects were identified and enrolled. The prevalence of congenital heart defects was calculated according to cities, years, urban and rural areas, regions of Guangdong, categories of maternal age at delivery, seasons of delivery, and infant's gender. Results A total of 8,653,206 perinatal infants and 53,912 total congenital heart defects were monitored in Guangdong, including 46,716 (86.65%) without other defects and 7,736 (13.35%) with other defects. The average prevalence of total congenital heart defects was 62.30/10,000 (95% CI, 61.78/10,000–62.83/10,000), congenital heart defects without other defects was 53.36/10,000 (95% CI, 52.88/10,000–53.85/10,000), and congenital heart defects with other defects was 8.94/10,000 (95%CI, 8.74/10,000–9.14/10,000). From 2016 to 2020, the prevalence of total congenital heart defects was 54.92/10,000, 54.23/10,000, 63.79/10,000, 73.11/10,000, 68.20/10,000, respectively. We observed geographical variations within the prevalence of congenital heart defects. The prevalence of congenital heart defects was much higher in the Pearl River Delta region than in the non-Pearl River Delta region, as well as higher in urban areas than in rural areas. Conclusion The findings of this study are helpful to the understanding of the etiology and epidemiology characteristics of congenital heart defects in south China. Our data likely reflect a better estimate of the spatiotemporal trends in congenital heart defects prevalence than reported previously.

Abstract:Objective As the only hospital-based national surveillance spot of birth defects (BDs) in Changzhou city located in the economically developed eastern part of China, Changzhou Maternal and Child Health Care Hospital has encountered serious challenges in BD prevention. This study aimed to describe the epidemiology of total BDs born in the hospital from 2014 to 2018. Methods The data were collected from the national hospital-based birth defect surveillance system. BD prevalence was calculated by Poisson distribution. Trends of prevalence and the associations regarding information with BDs were analyzed by Poisson regression. Results The reported prevalence of total BDs was 313.92 (95% confidence interval [CI]: 299.59–328.76) per 10,000 perinatal infants (PIs), while the perinatal prevalence of BD was 160.19 (95% CI: 150.00–170.89) per 10,000 PIs. A remarkable uptrend in the prevalence of BDs was noticed with a prevalence rate ratio (PRR) of 1.09 (95% CI: 1.04–1.14) and 1.13 (95% CI: 1.09–1.16), respectively. Congenital heart disease (CHD), cleft lip with or without cleft palate (CL/P), congenital malformation of the kidney (CMK), polydactyly, Down syndrome (DS), cystic hygroma, neural tube defect (NTD), and congenital talipes equinovarus (CTE) were common types of total BDs. Mothers living in the urban area (PRR = 1.67, 95% CI:1.50–1.87), male fetuses (PRR = 1.16, 95% CI: 1.05–1.28), and maternal age younger than 20 (PRR = 2.28, 95% CI: 1.60–3.25) and 25 years (PRR = 1.41, 95% CI: 1.22–1.63) or older than 35 years (PRR = 1.18, 95% CI: 1.00–1.40) were risk factors for BD occurrence. Conclusion The reported prevalence of total BDs was nearly two times higher than the perinatal prevalence of BDs in PIs, and the ranks of total BDs and BDs in PIs were different. Mothers living in the urban area, male fetuses, and maternal ages younger than 25 or older than 35 years were risk factors for BD incidence. Thus, improving prenatal examination technology, expanding the surveillance time quantum of BDs, and keeping maternal health may be warranted.
