Abstract:This study aimed to assess the clinical efficacy of the bandage contact lenses in the treatment of acute and impending corneal perforations.Patients who underwent the treatment of bandage contact lenses for acute and impending corneal perforations in the ophthalmology of the Second Affiliated Hospital of Zhejiang University School of Medicine from June 2014 to June 2015 were selected and,their subjective symptoms before and after wearing lenses were evaluated.Each subject underwent a comprehensive ophthalmic examination,including slit-lamp microscopy,fluorescein,digital tonometry,anterior segment photography,and best spectacle-corrected visual acuity (BSCVA).In addition,the complications of wearing the bandage contact lenses throughout the entire follow-up period were recorded.All the patients were followed up for more than 3 months after stopping wearing the bandage contact lenses.During the study,19 patients (19 eyes) were qualified according to the requirments,including 4 cases of corneal ulcer perforations suffered from exposure keratitis,4 cases ofcomeal perforations due to trauma,3 cases of neurotrophic corneal ulcer perforations caused by recurrent viral keratitis,3 cases of corneal perforations led by graft-versus-host reaction disease,1 case of corneal ulcer perforations after pterygium,3 cases of corneal ulcer perforations after cataract surgery,and 1 case of corneal perforations caused by chemical bums.The efficacy of 19 patients included:the corneal ulcers of 10 cases of impending corneal perforations healed,and perforating wound recovered,without corneal perforations.The corneal wound of 8 cases of full-thickness corneal perforations sealed,the depth of anterior chamber was recovered,and the morphology of ocular surface was restored effectively.1 case of full-thickness corneal perforations wore offcomeal bandage lens due to instability.There were no secondary infection or other complications during wearing corneal bandage lens.The bandage contact lenses could be an effective method for the treatment of acute and impending corneal perforations,which might be a safe and effective adjuvant therapy.
Treatment of corneal perforation with lenticules from small incision lenticule extraction surgery: a preliminary study of 6 patients.

Abstract:Purpose: To evaluate the use of lenticules extracted using small incision lenticule extraction (SMILE) surgery as a surgical alternative for wound closure in corneal perforation. Methods: Corneal lenticules obtained through SMILE surgery with central thickness >100 mu m were fixed over corneal perforation sites using 10-0 nylon interrupted stitches. Patients were monitored for a minimum of 1 year and were assessed using slit-lamp microscopy, fluorescein, digital tonometry, and best spectacle-corrected visual acuity (BSCVA) measurements. Postoperative complications throughout the study period were recorded. Results: Corneal perforations were successfully sealed in all 6 patients; 3 patients (50%) exhibited improved postoperative BSCVA. Between 3 and 4 weeks after the operation, part of the lenticules became incorporated into the corneal stroma and complete reepithelialization was achieved. During the follow-up period of 12 months, no evidence of infection, relapse, or perforation was detected in any patient. Conclusions: These preliminary findings suggest that the use of corneal lenticules may be a safe and effective surgical alternative for corneal perforation closure, with potential clinical application as relatively simple and inexpensive temporary measures to improve the condition of the cornea for further definitive interventions.
A novel sandwich technique of minimally invasive surgery for corneal perforation

Abstract:This study aimed to describe a novel sandwich technique of minimally invasive surgical implantation by using corneal stromal lenticules for corneal perforation. This prospective observational study included nine patients aged 23–79 years (mean, 54 ± 9) from Tianjin Eye Hospital. Corneal stromal lenticules with a central thickness of 120 μm were obtained from small incision lenticule extraction(SMILE). With the corneal perforation as the central point, an iris-repositor was used to manually create a mid-stromal pocket; the stromal lenticular button was gently inserted into the intrastromal pocket and flattened. The graft was sandwiched between the anterior and posterior corneal stromal pockets; the incision was closed using two sutures. The primary health outcome measured in this study was the logMAR best spectacle-visual acuity at 6 months postoperatively. Corneal perforation closure and anterior chamber formation occurred immediately after surgery. No epithelial implantation, infection, or allogeneic rejection was observed during the follow-up. No patients required a penetrating keratoplasty. Anterior segment optical coherence tomography revealed a mean thickness of the grafted tissue of 122 ± 21 μm, with a clear interface in all cases. Comparing the baseline and postoperative 6-month values, the mean logMAR best spectacle-corrected visual acuity was improved from 0.18 ± 0.12 to 0.52 ± 0.31 ( P < 0.001). There was significant reduction in the mean manifest spherical equivalent, refractive cylinder, and mean keratometry readings. The novel minimally invasive surgical method for corneal implantation. It was an effective alternative treatment that could restore corneal integrity and improve vision in patients with corneal perforations. This method appears safe, easy, cost- and time-effective, and reliable.

Novel drug delivery methods for the treatment of keratitis: moving away from surgical intervention

Abstract:Introduction: Corneal ulceration is one of the leading causes of blindness especially in low- and mid-income countries (LMICs). Surgical treatment of microbial keratitis is associated with multiple challenges that include non-availability of donor corneal tissues, lack of trained corneal surgeons, and poor compliance to follow up care. As a result, the surgery fails in 70-90% cases. Therefore, improving outcome of medical treatment and thereby avoiding the need for the surgery is an unmet need in the care of corneal ulcer cases.Areas covered: In this review article, the authors have tried to compile information on the novel drug-delivery systems that have potential to enhance success of medical management. We have discussed the following systems: cyclodextrins, gel formulations, colloidal system, nanoformulations, drug-eluting contact lens, microneedle patch, and ocular inserts.Expert opinion: The goals of corneal ulcer treatment are as follows: rapid eradication of causative microorganisms, control of damage from induced inflammation and microbial toxins, and facilitation of repair. The ocular surface anatomy poses several challenges for drug delivery using standard topical therapy. The novel drug-delivery systems mentioned above aim to enhanced tear solubility; superior stability; improved bio-availability; reduced toxicity; besides facilitating targeted drug delivery and convenience of administration.
A Novel Approach: Treating Corneal Perforation with Corneal Lenticule

Abstract:To the editor: Corneal perforation requires an emergency surgical repair. Currently in China we do not have well-developed corneal bank, which makes it difficult for a surgeon to get grafts to perform emergency repair. Recent breakthrough in femtosecond laser (FS) technology has resulted in a novel “all-in-one” refractive procedure called refractive lenticule extraction (ReLEx).1 A corneal stromal lenticule is removed during the procedure. We presented a surgical case of a large corneal perforation repaired with acellular glycerolpreserved multilayer corneal lamellar lenticules. A 40-year-old male patient presented at emergency room of Jinling Hospital complaining of right eye pain with vision loss. His right eye vision was limited to hand movements due to neutrophic corneal perforation (Figure 1A). Iridocorneal touch with iris incarceration was shown on optical coherence tomography (OCT) exam (Visante, Carl Zeiss Meditec, Inc. Dublin, CA, USA) (Figure 1B). The diameter of the perforation was measured 3.61 mm by a Fourier-domain OCT system (Optovue, Inc., Fremont, CA, USA) (Figure 1C). Due to the unavailability of regular corneal graft, an emergent repair was carried out using acellular corneal tissue. The acellular tissue used to repair was harvested from myopic patients during ReLEx surgeries using a VisuMax femtosecond laser system (Carl Zeiss Meditec AG). Informed consent was obtained and blood test and donor eligibility were performed and determined according to guidelines of FDA Regulation 21 CFR-1271 (http://www.accessdata.fda.gov/scripts/cdrh/cfdocs/cfCFR/CFRSearch.cfm?fr=1271.75).Figure 1. Lenticule diameter (optical zone) was measured as 6.0–6.5 mm with central thickness varying from 80 to 120 μm. Three layers of lenticules were stacked up and stored at -78°C in pure sterile glycerin. During the procedure, the edge of the necrotic tissue from recipient cornea was debrided. A 4-mm-diameter lenticule button was prepared using a corneal trephine. The graft was secured in place using a single running 10/0 nylon suture (Figure 1D). Tobramycin ophthalmic ointment was used postoperatively. The following day, slit-lamp and OCT examination showed a deep anterior chamber without signs of inflammation (Figure 1D and 1E). The visual acuity was 20/200. The three layers of the corneal stromal lenticule graft were clearly identified on the OCT (Figure 1F). Suture removal was performed 3 months after the surgery (Figure 1G). Uncorrected visual acuity was 20/80, anterior chamber was deep with residue of iridocorneal touch (Figure 1H), and the junction lines of the three layers inside the graft were invisible (Figure 1I). Shortage of corneal donors is a common problem in developing countries including China.2 ReLEx is a recent breakthrough in FS technology. During ReLEx procedure, it is possible to harvest corneal lenticule for future use. Lenticules can be combined together to create a multilayered graft. The thickness of this graft can be varied by stacking different numbers of lenticules. The availability of these grafts is rising due to the increased volume of ReLEx surgeries. Penetrating keratoplasty can be performed under more favorable conditions in some cases. To our knowledge, this is the first reported case study to reuse FS corneal lenticules. More clinical study using this novel approach will be performed.

Abstract:Introduction: Complex corneal conditions present surgical challenges and necessitate innovation. Here, we present two cases where we performed intraocular lens trans-scleral fixation using the double-needle Yamane technique, followed by penetrating keratoplasty and vitrectomy using a temporary Landers wide-field keratoprosthesis. Case presentation: Case 1 involved a 70-year-old man with an aphakic eye of bullous keratopathy and corneal opacity owing to multiple penetrating and endothelial keratoplasty, endophthalmitis, and herpetic keratitis. His visual acuity was counting fingers at 20 cm before surgery. Penetrating keratoplasty with vitrectomy and intraocular lens scleral fixation was performed using the double-needle Yamane technique, and 10 months postoperatively, his best-corrected visual acuity improved to 0.6, presenting a clear cornea. Case 2 involved a 62-year-old man who underwent penetrating keratoplasty twice for corneal perforation and therapeutic penetrating keratoplasty with vitrectomy for traumatic globe rupture, resulting in the loss of the intraocular lens. The patient exhibited graft failure, and his best-corrected visual acuity was 0.03. Utilizing a temporary Landers wide-field keratoprosthesis, we performed penetrating keratoplasty and intraocular lens trans-scleral fixation without complications. His final best-corrected visual acuity improved to 0.15 with a clear cornea. Conclusions: Trans-scleral fixation of intraocular lens with penetrating keratoplasty, using temporary Landers wide-field keratoprosthesis, yielded positive clinical outcomes without serious complications.

Abstract:The scarcity of corneal tissue to treat deep corneal defects and corneal perforations remains a challenge. Currently, small incision lenticule extraction (SMILE)-derived lenticules appear to be a promising alternative for the treatment of these conditions. However, the thickness and toughness of a single piece of lenticule are limited. To overcome these limitations, we constructed a corneal stromal equivalent with SMILE-derived lenticules and fibrin glue. In vitro cell culture revealed that the corneal stromal equivalent could provide a suitable scaffold for the survival and proliferation of corneal epithelial cells, which formed a continuous pluristratified epithelium with the expression of characteristic markers. Finally, anterior lamellar keratoplasty in rabbits demonstrated that the corneal stromal equivalent with decellularized lenticules and fibrin glue could repair the anterior region of the stroma, leading to re-epithelialization and recovery of both transparency and ultrastructural organization. Corneal neovascularization, graft degradation, and corneal rejection were not observed within 3 months. Taken together, the corneal stromal equivalent with SMILE-derived lenticules and fibrin glue appears to be a safe and effective alternative for the repair of damage to the anterior cornea, which may provide new avenues in the treatment of deep corneal defects or corneal perforations.
A novel technique of penetrating keratoplasty to prevent intraocular contents extrusion for infectious keratitis

Abstract:Purpose: To evaluate the safety and the effectiveness of our novel penetrating keratoplasty for infectious keratitis. Methods: Retrospective, noncomparative, interventional case series of patients with infectious keratitis who received the novel penetrating keratoplasty technique were analyzed. A prepared plastic sheet was located between the diseased cornea and iris-lens diaphragm. After the diseased lesions were removed, the graft was positioned on the plastic sheet and sutured to the recipient bed. The plastic sheet was pulled out from the anterior chamber before the all interrupted sutures were placed. The intra- and post-operative complications, the outcome of the graft and the number of corneal endothelial cells were analyzed. Results: A total of 82 eyes of 82 patients was included. The mean follow-up period was 29 ± 16 months (range from 13 to 45 months). No intraocular content extrusion, simultaneous cataract extraction and suprachoroidal hemorrhage occurred. Direct contact between the infectious cornea and the graft was successfully avoided. Greater than expected endothelial cell reduction or complications were not found. Conclusions: This modified technique effectively prevents the extrusion of intraocular contents while avoiding the direct contact with donor endothelium during the procedure. The occurrence rate of complications such as endothelial cell loss is not higher than the conventional methods.

Contact Lens–Assisted DMEK for Corneal Decompensation With an Anterior Chamber Intraocular Lens: A Novel Technique

Abstract:Purpose: To describe a novel technique in which a contact lens was used to facilitate Descemet membrane endothelial keratoplasty (DMEK) in an eye with an anterior chamber intraocular lens (ACIOL). Methods: A 71-year-old man with a long-standing ACIOL developed bullous keratopathy. As the patient was at high risk of complications due to multiple ocular comorbidities, DMEK was performed with the ACIOL left in the anterior chamber. An inserted cosmetic contact lens was used as a platform for the graft unfolding. Results: Partial graft detachment on the third postoperative day required rebubbling. This successfully cleared up the cornea after a few weeks. At the one-month follow-up, the uncorrected distance visual acuity improved to 20/50, with an endothelial cell loss of 35.2%. Steroid-induced ocular hypertension necessitated a trabeculectomy by the fifth postoperative month. At the sixth follow-up month, endothelial cell loss was 64%. Conclusions: Contact lens–assisted DMEK is a safe procedure for eyes with an ACIOL. However, good foresight is needed regarding whether the ACIOL should be kept or exchanged.

Design and Biocompatibility of a Novel, Flexible Artificial Cornea

Abstract:Purpose: We sought to introduce the materials, design, and biocompatibility of a flexible and suturable artificial corneal device. Methods: Single-piece, fully synthetic, optic-skirt design devices were made from compact perfluoroalkoxy alkane. The skirt and the optic wall surfaces were lined with a porous tissue ingrowth material using expanded polytetrafluoroethylene. Full-thickness macroapertures around the skirt perimeter were placed to facilitate nutrition of the recipient cornea. Material properties including the skirt's modulus of elasticity and bending stiffness, optic light transmission, wetting behavior, topical drug penetrance, and degradation profile were evaluated. Results: The final prototype suitable for human use has a transparent optic with a diameter of 4.60 mm anteriorly, 4.28 mm posteriorly, and a skirt outer diameter of 6.8 mm. The biomechanical and optical properties of the device closely align with the native human cornea with an average normalized device skirt-bending stiffness of 4.7 kPa·mm4 and light transmission in the visible spectrum ranging between 92% and 96%. No optical damage was seen in the 36 devices tested in fouling experiments. No significant difference was observed in topical drug penetrance into the anterior chamber of the device implanted eye compared with the naïve rabbit eye. Conclusions: The flexibility and biocompatibility of our artificial cornea device may offer enhanced tissue integration and decreased inflammation, leading to improved retention compared with rigid keratoprosthesis designs. Translational relevance: We have developed a fully synthetic, flexible, suturable, optic-skirt design prototype artificial cornea that is ready to be tested in early human feasibility studies.

Abstract:Corneal wound closure for surgical eye surgeries or accidents is typically performed to prevent pathogens from the sterile intraocular environment and avoid potential postoperative complications. Tissue adhesives are increasingly employed for corneal wound closure with superior treatment efficiency and less adverse effects. In this study, we successfully develop a novel corneal adhesive based on functionally coupled PEG-lysozyme (PEG-LZ) hydrogels for wound closure after surgical eye surgeries. PEG-LZ hydrogels have plenty of micropores and gradually decreased pore size with increasing concentration from 10%, 15% to 20% (w/v), in which PEG-LZ (15%) represents the suitable pH value, gelation time and elastic modulus. PEG-LZ hydrogels have no in vitro cytotoxicity and excellent ex vivo wound closure effectiveness in porcine eyes. The in vivo wound sealant in rabbit eyes by PEG-LZ hydrogels presents a superior therapeutic effect compared with the conventional methods of stromal hydration and suture, in terms of the wound closure percent, mean corneal thickness, percent of wound gaping, and the Descemet membrane detachment. PEG-LZ hydrogels do not induce obvious histological pathology changes. The PEG-LZ corneal adhesive is considered as a tissue adhesive alternative for wound closure after surgical eye surgeries.

Abstract:OBJECTIVE:To describe a novel surgical technique for the treatment of large corneal perforations by using acellular multilayer of corneal stromal lenticules.METHODS:Prospective study. The acellular tissue used for the repair was harvested from myopic patients during the femtosecond laser (FS) refractive surgery. Informed consent, blood test and donor eligibility were obtained in each case. Three or four layers of lenticules were stacked up and stored at -80°C in pure sterile glycerin. The diameter is 6.0 to 6.5 mm and central thickness was 300 to 400 µm. If the diameter of the corneal ulcer perforation was larger than 3 mm and corneal grafts were not available, we used this kind of patches to seal the perforations. It was a retrospective case series study. Five cases of corneal ulcer perforation were enrolled in this study. One was neuropathic keratitis, one was atopic keratoconjunctivitis, and the other three were fungal keratitis. Acellular multilayer of stromal lenticules were used in these cases for emergent therapy.RESULTS:The sealing of the perforation and the re-establishment of the anterior chamber were achieved successfully in all the cases. For the pericentral perforations, visual recovery was achieved. And efficient palliative management was done for the central perforations.CONCLUSIONS:The reported technique seems to represent a good alternative emergency procedure for the management of large corneal perforations. It is a very useful method for Chinese hospitals where the shortage of cornea donors is a very serious problem and the amount of FS surgeries are increasing.
Scleral Lens Applications Focused on Korean Patients with Various Corneal Disorders

Abstract:Purpose: We aimed to report on the clinical outcomes of scleral lens applications in Korean patients with various corneal disorders. Methods: This retrospective review was conducted for 62 eyes of 47 patients who had been fitted with scleral lenses for various corneal disorders. The patients were referred for inadequate spectacle-corrected visual acuity and rigid gas permeable (RGP) or soft contact lens intolerance. Uncorrected visual acuity, habitually corrected visual acuity, best lens-corrected visual acuity, topographic indices, keratometry indices, and lens parameters were evaluated. Results: Twenty-six eyes of 19 patients with keratoconus were enrolled. Other conditions included corneal scar (13 eyes of 12 patients), phlyctenules (three eyes), laceration (four eyes), chemical burn (one eye), keratitis (one eye), Peters' anomaly (one eye), fibrous dysplasia (one eye), ocular graft-versus-host disease (two eyes of one patient), irregular astigmatism (18 eyes of 12 patients), and corneal transplant status (five eyes of four patients). The mean topographic values of the eyes include flat keratometric value (43.0 ± 6.1 diopters [D]), steep keratometric value (48.0 ± 7.4 D), and astigmatism (4.9 ± 3.6 D). Of the eyes fitted with scleral lenses, best lens-corrected visual acuity (0.10 ± 0.22 logarithm of the minimum angle of resolution [logMAR]) was significantly better than the habitually corrected visual acuity (0.59 ± 0.62 logMAR, p < 0.001). Conclusions: Scleral contact lenses are a good alternative for patients with corneal abnormalities and those who are intolerable to RGP contact lenses, resulting in both successful visual outcomes and patient satisfaction, especially concerning keratoconus, corneal scar, and corneal transplant status.
Contact lens induced bacterial keratitis in LCD II: Management and multimodal imaging: a case report and review of literature

Abstract:Purpose: To describe the management and multimodal imaging of lattice corneal dystrophy type II (LCD-II) complicated by an infectious keratitis due to a bandage contact lens and to review current literature. Observation: A 50-year-old female was diagnosed with Meretoja's Syndrome by the triad of facial palsy, loose skin (cutix laxa), and stromal corneal dystrophy. At slit lamp, bilateral lattice corneal dystrophy (LCD) was characterized by multiple linear refractile lines and subepithelial fibrosis along with Neurotrophic keratitis Mackie grade I. Findings of anterior segment optical coherence tomography (AS-OCT) were epithelial irregularity, subepithelial fibrosis, hyperreflectivity on anterior stromal layer, lobulated stromal surface. In vivo confocal microscopy (IVCM) showed hyperreflected deposits on the basal and Bowman layers, visible keratocytes; fine lines and streaks between corneal lamella. The sub-basal nerve plexus and the stromal nerves were no longer visible. She presented in emergency with a left red eye. A severe bacterial keratitis was diagnosed as a complication of a bandage contact lens used to treat recurrent epithelial erosion. Corneal anesthesia was complete. Corneal neovascularization was evident 10 weeks later and topical bevacizumab (5 mg/ml) was introduced twice daily. Partial regression of deep stromal vessels was noticed at 3 months. Conclusion: In Meretoja's syndrome, neurotrophic keratopathy secondary to polyneuropathy due to systemic amyloid deposits is present in the advanced stages, promotes recurrent corneal erosions. Corneal sensitivity test, AS-OCT and IVCM are crucial in the diagnosis behind any recurrent corneal erosion. The use of bandage contact lens should be avoided in Meretoja's syndrome to prevent a possible infectious keratitis.

Abstract:Keratoconus is a corneal disorder characterized by the progressive thinning and bulging of the cornea. Currently, the major goal of management is to halt its progression, restore normal corneal strength, prevent acute complications, and save vision. Penetrating keratoplasty and deep anterior lamellar keratoplasty as conventional surgical methods for advanced keratoconus are limited by relatively high rates of immune intolerance, slow post-operational recovery, high costs, and shortage of donor corneas. Recently, the development of lenticule addition keratoplasty enables the restoration of corneal thickness simply by implanting a lenticule into the stromal pocket created with the femtosecond laser, which can originate from cadaver corneas or more appealing, be extracted from patients via a small-incision lenticule extraction (SMILE) surgery. As the first systematic review in this field, we critically review publications on lenticule addition keratoplasty and provide our perspectives on its clinical application and the focus of future research.

Observation and Assessment of the Immediate Use of a Silicon Hydrogel Contact Lens after Transepithelial Corneal Cross Linking: a Prospective Study

Abstract:Transepithelial corneal crosslinking (CXL) is a novel surgical approach for the treatment of keratoconus, which is a bilateral asymmetrical ophthalmological disease accompanied by progressive corneal ectasia. Silicon hydrogel (SiH) contact lenses have been extensively used in clinical ophthalmologic medicine, as a postoperative ophthalmological intervention. However, the ideal lens application duration after transepithelial CXL remains uncertain. Here, we aimed to investigate the effects and comfort of immediate corneal contact lens use after transepithelial CXL for keratoconus. In this prospective study, 60 patients with keratoconus who underwent transepithelial CXL treatment were enrolled from September 2021 to January 2023 with a male:female ratio of 39:21, and an average age of 25.42 ± 5.47 years. The patients were divided randomly into two groups: group A contained 30 patients wearing silicone hydrogel contact lenses for 7 days postoperatively, and group B contained 30 patients wearing the same contact lenses for 3 days. Ten subjective ophthalmologic symptoms were surveyed by the patients, including pain, photophobia, foreign body sensation, tearing, burning, blurred vision, dry eyes, difficulty opening the eyes, astringency, and stinging. Ophthalmologic signs, including corneal edema and conjunctival congestion, were recorded by a single clinician on postoperative days 1, 3, and 7. Each surgical procedure was readily performed without complications, and both groups postoperative day 7 (P = 0.04), where group B scored (0.01 ± 0.41) lesser than group A (0.12 ± 0.29), whilst corneal edema in both groups recorded significantly different on postoperative days 5 and 7 (group A demonstrated the result of 0.17 ± 0.14 and 0.08 ± 0.11 for the respective days, whereas group B indicated 0.10 ± 0.13 and 0.03 ± 0.07 at the corresponding times). Immediate use of silicone hydrogel corneal lenses after transepithelial CXL effectively alleviates postoperative ocular distress, particularly with a three-day use period as the ideal duration.

Abstract:Background True Keratoconus Suspect (KCS) is an absolute contraindication to LASIK or Photorefractive Keratectomy (PRK) due to postoperative ectasia. Objective To evaluate the effectiveness of laser asymmetric keratectomy with collagen cross-linking (L-LAK-CXL) in myopic patients with suspected keratoconus (KCS). Methods This study included 40–44-year-old four myopic (-2.50 to -5.50 D) patients (4 eyes), of KCS with focal corneal steepening over +47.0 D and peripheral asymmetric corneal thickness. L-LAK-CXL was performed for both original ablation of refractive errors and crescentic customized ablation of the thicker peripheral cornea selectively and myopic changes due to the ablation of the peripheral thicker cornea simultaneously, followed by CXL without the epithelium. We compared preoperative and postoperative ocular findings, including corneal symmetry (total differences of the corneal thickness in four directions (SUM) and decentration of the thinnest point (DISTANCE)) and tear break-out time (TBUT). Results From preoperative to postoperative, spherical equivalent (D, average) decreased from -3.38 to -0.34, uncorrected distance visual acuity (LogMAR) increased from 0.53 to 0.00, and Kmax (average D) had decreased from +48.3 to +43.95, central pachymetry (CP, μm, average) decreased from 574 to 511. Postoperative corneal symmetry increased markedly owing to decreased SUM and DISTANCE scores. TBUT increased over 15 s postoperatively. No post-operative corneal ectasia was observed. Conclusion L-LAK-CXL improved corneal symmetry in myopic patients with KCS by reducing SUM, decreasing DISTANCE and Kmax, increasing TBUT, and demonstrating good postoperative visual outcomes.
