Abstract:Uveitic glaucoma (UG) is secondary glaucoma, present as a clinical challenge in both diagnosis and management.We report a case of complex UG, which initially presented as pupillary block and rupture of the anterior lens capsule. We performed cataract extraction with preservation of posterior capsule. Then, the case turned to aphakic malignant glaucoma. We performed anterior vitrectomy with posterior capsule resection in this case. After the second operation, the patient had a satisfactory recovery. Specifically, ultrasonographic biomicroscopy was useful during the diagnosis process and follow-up period in this case.UG presenting as pupillary block, rupture of the anterior lens capsule, and aqueous misdirection seldom presents in clinical practice. Earlier and more active surgical intervention may be necessary for effective preservation of visual function in complex cases of UG.

Abstract:Purpose: To report a rare case of delayed dislocation of a novel posterior chamber phakic intraocular lens into the vitreous cavity, which was successfully treated by a reformed technique. Case PresentationA 29-year-old female received Ejinn phakic refractory lens (EPRL) implantation to correct her high myopia. Spontaneous dislocation into the vitreous cavity occurred 26-months post-operatively without traumatic history. Pars plana vitrectomy combined with cutting the EPRL into two equal pieces was performed to remove the dislocated EPRL. ConclusionDislocation into the vitreous cavity of EPRL can be successfully and easily removed by our reformed technique. Concerns about zonules-related complications pre-operatively, intraoperatively, and post-operatively must be raised in the practice of EPRL implantation.
Two Techniques of Posterior Chamber Intraocular Lens (IOL) Implantation by a 4-Point Ciliary Sulcus Suture Fixation Technique Combined with Vitrectomy

Abstract:OBJECTIVE:To observe the results of two techniques of posterior chamber intraocular lens (IOL) implantation by a 4-point ciliary sulcus suture fixation combined with vitrectomy.METHODS:Lensectomy and vitrectomy combined with posterior chamber IOL implantation by a 4-point scleral fixation was performed in 23 cases with complicated ocular trauma, including 9 eyes had eyeball rupture, vitreous hemorrhage, retinal contusion and traumatic cataract; 6 eyes having eyeball rupture, vitreous hemorrhage, retinal contusion, intraocular foreign body and traumatic cataract; 8 eyes having ocular blunt trauma, vitreous hemorrhage, traumatic cataract and lens dislocation. Two techniques of IOL were randomly assigned: Alcon CZ70BD PMMA suspensory IOL was performed in 12 cases (Group I); Bausch & Lomb Akreos Adapt four-haptic hydrophilic acrylic foldable IOL was performed in 11 cases (Group II). After completed vitrectomy, removed intraocular foreign body and (or) cataract extracted, one-stage IOL implantation by 4-point scleral fixation was performed and the knots of fixation sutures were rotated and buried under the sclera for all patients. The IOL position was adjusted by suture for the best centration.RESULT:The mean follow-up time was (12.4 +/-1.7) months. The postoperative best-corrected visual acuity (BCVA) was improved in 11 cases (91.7%) of Group I; and BCVA was > or =0.5 in 3 patients(25.0 %). The postoperative BCVA was improved in 11 cases (100 %) of Group II; and BCVA was > or =0.5 in 4 eyes (36.4%). There was no IOL decentration or tilting in two groups.CONCLUSION:Lensectomy and vitrectomy combined with posterior chamber IOL implantation by a 4-point scleral fixation technique is a good method to treat complicated ocular trauma. Two techniques of posterior chamber IOL have both advantages and disadvantages, and can be used accordingly.

Abstract:The present study describes a surgical technique for the management of complex dislocation and subluxation of the intraocular lens (IOL)-capsular bag (CB)-capsular tension ring (CTR) complex. During the procedure, one double-curved suture was inserted through the sclera into the posterior chamber, passing through the anterior capsules and then into the anterior chamber. The suture lines were pulled out from above and below the IOL-CB-CTR complex and a knot was tied 1 mm away from the corneoscleral incision. The IOL-CB-CTR complex was repositioned by adjusting the suture length. This novel method does not require explanting of the original IOL, allowing the IOL-CB-CTR complex to be immobilized in a simple and effective way.

Abstract:The core of the modern surgical management for the lens subluxation is the retention and remodeling of the septum of capsule and zonule to the greatest extent , in order to reduce the incidence of complications related to vitreous and retina . With the auxiliary apparatus of capsular retractors , capsular tension rings , capsular tension segments, capsular anchor and femtosecond laser-assisted phacoemulsification cataract surgery makes operation more mini-invasive and controllable .The stability of capsular bag is getting better and better and the new functional intraocular lens is expected to be used in only the most ideal cases .

Abstract:Cataract surgery for the subluxated crystalline lens is challenging. A thorough preoperative evaluation is important to determine the appropriate surgical approach for lens removal and the subsequent technique of intraocular lens placement. Important considerations include the extent and location of zonular weakness, and whether the zonular deficiency is caused by a static or progressive disease. The capsular bag should be preserved where possible. Creating a good-sized and centred continuous curvilinear capsulorhexis is crucial to facilitate the use of capsular retractors and capsular tension devices, which provide capsular stability. Nucleus sculpting and rotation should be minimised to reduce zonular stress. Being cognisant of the possible intraoperative complications that may occur at each stage of the surgery and knowing how to reduce the risk of these complications occurring will enable surgeons to perform safe cataract surgery in these complex cases.

Safety and Effect of Femtosecond Laser-Assisted Cataract Surgery Combined with Cionni Capsular Tension Ring Implantation in the Management of Traumatic Lens Subluxation

Abstract:AIM:To investigate the safety and effect of femtosecond laser-assisted cataract surgery with Cionni modified capsular tension ring(MCTR)implantation in the management of traumatic lens subluxation.METHODS: Totally 11 patients(11 eyes)with traumatic lens subluxation were divided into three groups according to the severity of lens dislocation, ranging from 90° to 120°(4 eyes), 120° to 180°(5 eyes)and 180° to 270°(2 eyes). The contact LenSx femtosecond laser cataract surgery platform was applied to create the capsulotomy, prepare nuclear fragmentation and make corneal wound creation. Anterior vitrectomy was performed in some patients during the surgery. After capsular retractors insertion and phacoemulsification, the MCTR was inserted to the capsular bag and fixed to the sclera. Finally, the IOL was implanted into the capsular bag. Postoperative visual acuity, intra- and post-operative complications, anterior capsular opening, IOL and MCTR position and intraocular pressure(IOP)were assessed.RESULTS:The duration of follow-up was 2mo. All the operations were completed successfully. Five eyes underwent cataract surgery combined with anterior vitrectomy. Four eyes had been inserted with 2-eyelet MCTR and seven eyes with 1-eyelet MCTR. The best corrected visual acuity(BCVA)after operation was better than 0.5 in 4 eyes, between 0.3 and 0.5 in 3 eyes, between 0.1 and 0.3 in 3 eyes, and less than 0.1 in 1 eye. Compared with preoperative BCVA, the difference was statistically significant(P<0.05). All the IOLs were stably centered and the eyelet of MCTR was fixated steadily between the iris and the anterior capsule. The common intra- and post-operative complications were subconjunctival hemorrhage, incomplete capsulotomy, residual cortex, secondary glaucoma and posterior capsular opacification. CONCLUSION:Femtosecond laser-assisted cataract surgery can improve the success rate of capsulorhexis, and reduce the difficulty of nuclear fragmentation. Femtosecond laser-assisted cataract surgery combined with MCTR implantation is an ideal surgical method for traumatic lens subluxation.
Femtosecond laser-assisted successful management of subluxated cataractous lens with vitreous in anterior chamber

Abstract:Femtosecond laser-assisted cataract surgery was performed in a case of posttraumatic cataract with six clock hours subluxation and vitreous in the anterior chamber (AC). Femtosecond laser pretreatment allowed a closed-chamber creation of corneal incisions, capsulotomy, and lens fragmentation with minimal sudden lens-diaphragm movements and zonular stress. Integrated imaging systems allowed customization of the size and position of capsulotomy and nuclear fragmentation, based on the extent and site of subluxation. Presence of vitreous in AC did not hamper femtosecond laser application. Triamcinolone-assisted vitrectomy was performed before phacoemulsification and after implanting the intraocular lens (IOL). Postoperative uncorrected visual acuity was 20/20 with a stable IOL.
Combined Surgical Approaches in the Management of Traumatic Dislocated Lenses

Abstract:Objective:To evaluate the efficacy of combined surgical approaches in the management of dislocated lenses.Methods:Clinical records were reviewed retrospectively for a series of consecutive 36 patients with traumatic dislocated lens (including subluxation) underwent lens and vitreous surgery from October 1993 to March 2000.According to various clinical features,different surgical approaches were performed to these cases,including extraction of dislocated lens by snare through a limbal incision,lensectomy or phacofragmentation through pars plana incisions and vitrectomy.Some cases combined with trabeculectomy or valved tube implantation and vitreoretinal surgery to treat complications caused by dislocated lens,and in 17 cases,primary anterior or posterior chamber intraocular lens were implanted.Results:All dislocated or subluxated lenses were removed completely,33 of 36 eyes(91.7%) achieved improved visual acuity,19 eyes (52.8%) got 0.3 or better visual acuity,IOP in all eyes were normal all,6 eyes which had retinal detachment gained anatomic retinal attachment after surgery.Conclusions:The combination of different surgical approaches in the management of traumatic dislocated lenses and the complications caused by injury gives good visual rehabilitation.Valved tube implantation is an effective surgical method to treat the refractory glaucoma resulted from severe ocular trauma.
Managing dislocated hard lens nuclei: 23-gauge vitrectomy and lens extraction via a corneoscleral limbal incision versus 23-gauge vitrectomy and phacofragmentation

Abstract:Purpose: To compare 23-gauge vitrectomy and lens extraction via a corneoscleral limbal incision (CSLI) with 23-gauge vitrectomy and phacofragmentation to treat dislocation of hard lens nuclei. Setting: Ningbo Eye Hospital, Zhejiang, China. Design: Retrospective case series. Methods: The study included consecutive patients with complete posterior dislocation of a hard nucleus (grade ≥ IV) into the vitreous cavity. All patients received 23-gauge 3-channel vitrectomy. Some patients also had phacofragmentation and others had lens extraction through a CSLI. Results: The CSLI group comprised 21 eyes of 21 patients and the phacofragmentation group, 22 eyes of 22 patients. The median follow-up was 10.8 months (range 6 to 24 months) and 11.3 months (range 5 to 18 months), respectively. Demographic characteristics, reason for lens dislocation, preoperative corrected distance visual acuity (CDVA), preoperative intraocular pressure (IOP), lens nucleus grade, and comorbidities were similar between groups. The CSLI group had a shorter mean surgical time than the phacofragmentation group (42.5 ± 7.2 minutes versus 68.2 ± 16.5 minutes); less frequent use of perfluorocarbon liquid, octafluoropropane, or air tamponade; lower incidence of retinal tears (9.5% versus 31.8%); and better CDVA but worse astigmatism 1 day and 1 week postoperatively (P < .05). The postoperative IOP did not differ between groups. Corneal edema and recurrent retinal detachment were less common in the CSLI group than in the phacofragmentation group. Conclusion: The 23-gauge vitrectomy with lens extraction through a CSLI might have advantages over 23-gauge vitrectomy with phacofragmentation for management of dislocated hard lens nuclei.
Managing Intraocular Lens Dislocation with One-Port Pars Plana Vitrectomy under Direct Vision with Microscope Illumination

Abstract:AIM: To describe a technique of managing intraocular lens (IOL) with deep dislocation in the vitreous cavity by performing pars plana vitrectomy (PPV) with only one pars plana incision under the direct illumination of the surgical microscope. METHODS: Patients who had in-the-bag or out-of-the-bag (spontaneous) IOL dislocation after uneventful phacoemulsification cataract extractions, with the dislocated IOL or IOL-capsular bag complex dropping completely into the vitreous since 2013 were included in our studies. The postoperative patients were followed up for 6mo. Detailed description of technique and retrospective description of eight typical cases were demonstrated in this study. RESULTS: A total of 40 surgeries were conducted using this technique. The main possible predisposing conditions included: post-vitrectomy, posterior capsule rupture or broken zonules, a history of ocular trauma, long axial length, secondary IOL implantation, chronic uveitis, retinitis pigmentosa and post-glaucoma surgery. In all eyes, the IOLs were successfully removed. No intraoperative or postoperative complications related to the procedures occurred. The preoperative corrected distance visual acuity (CDVA) ranged from 20/133 to 20/25, and at 6mo postoperatively, the CDVA was similar or the same. The intraocular pressure was all within the normal range. CONCLUSION: One-port PPV under direct vision with microscope illumination is a simple and safe surgical technique to managing IOL dislocation, which shortens the surgical time, and largely avoids surgical complications.

Abstract:Purpose: To evaluate the efficacy and safety outcomes of our newly developed skewer technique in the management of lens dislocation. Methods: A total of 21 eyes with lens dislocation were treated with this surgical technique. We focused on best-corrected visual acuity (BCVA), intraocular pressure, and complications of retinal damage at 3 months post-surgery. Results: The mean age of the patients was 60.20 ± 10.73 (range: 41-85) years, and the average axial length was 25.40 ± 2.35 (range: 22.30-32.35) mm. The BCVA at 3 months post-surgery showed a significant improvement compared to preoperative measurements. No intraoperative or postoperative retinal damage occurred in any eyes treated with this new technique. Conclusion: The "skewer" technique we developed is an innovative solution for addressing lens dislocation. Phacoemulsification can be performed in the anterior chamber utilizing a light guide to control the position and movement of the lens. This approach eliminates the need for perfluorocarbon liquid or extensive pars plana scleral sclerotomy. Based on postoperative outcomes from a series of patients who underwent this procedure, the technique is highly safe and feasible.
Management of Complex Retinal Detachment with Combined Vitrectomy and Intravitreal Perfluorocarbon Liquid Injection

Abstract:Objective: To evaluate the advantage of perfluorocarbon liquid as a hydrokinetic manipulation tool during complex vitreoretinal surgery. Methods: Forty-two eyes with complicated retinal detachment were treated with vitrectomy and intravitreal perfluorocarbon liquid injection. Postoperative tamponade were obtained with silicon oil or gas. Results: Retinas were successfully reattached in all eyes intraoperatively, after a follow-up period of 3-6 months,34 of the 42 retinas (81%) remain attached . Conclusion: Perfluorocarbon liquid is an ideal hydrokinetic manipulation tool during complex vitreoretinal surgery.
Effect of Subluxated Cataract Extraction by Phacoemulsification with Intraocular Lens Implantation

Abstract:Objective To assess the technique and effect of subluxated cataract extraction by phacoemulsification with intraocular lens implantation. Methods A series of 21 eyes of 21 patients had subluxated cataract extraction by phacoemulsification and intraocular lens implantation through sclera tunnel incision with an endocapsular ring inserted into the capsular bag in 2 eyes. Results Anterior chamber intraocular lens was implanted in one eye due to extension of capsular-zonular dialysis at the time of operation. The operation of 20 eyes was successful with primary posterior chamber intraocular lens implantation. Seventeen eyes (81% ) restored ≥0.5 visual acuity 3 months post-operatively. No complication occurred. Conclusion Phacoemulsification for subluxated cataract is an effective method. It has few complications and may improve post-operative visual acuity satisfactorily. The endocapsular ring is a safe and effective device that may facilitate subluxated cataract extraction by phacoemulsification and intraocular lens implantation.
Modified Intrascleral Fixation for Repositioning the Dislocated Single-Piece, Rigid PMMA Intraocular Lens

Purpose: To describe a modified intrascleral fixation technique for repositioning single-piece rigid polymethyl methacrylate intraocular lenses (IOLs). Methods: Four patients with dislocated IOLs were enrolled. Surgical modifications included using ≤1-mm scleral incision for haptic externalization, placing the IOL haptic placement in scleral tunnels, and using 8-0 absorbable sutures. Patients were followed up for 6 months with routine ophthalmic examinations, corneal endothelial cell counts, and ultrasound biomicroscopy (UBM). Results: The mean follow-up time was 13.5 ± 5.45 months. The IOL was well centered and the spherical refraction improved (+10.25 ± 2.21 vs. −0.81 ± 1.59 D, P < 0.05), whereas the best-corrected visual acuity (pre 20/43 Snellen, 0.42 ± 0.33 logarithm of the minimum angle of resolution equivalent and post 20/36 Snellen, 0.31 ± 0.22 logarithm of the minimum angle of resolution equivalent; P = 0.235), intraocular pressure (pre 13.8 ± 3.21, post 13.55 ± 5.14 mmHg), corneal endothelium density (pre 2,423.8 ± 279.6/mm2, post 2,280.25 ± 350.7/mm2), and total astigmatism (pre −1.94 ± 0.43, post −1.69 ± 0.59 D) remained unchanged. The average horizontal and vertical IOL tilt was 0.33 ± 0.22° and 0.81 ± 0.38°, respectively. Intraocular lens decentration was 0.10 ± 0.03 mm horizontally and 0.13 ± 0.06 mm vertically. Conclusion: The modified intrascleral fixation technique shows encouraging midterm results in patients with dislocated single-piece, rigid polymethyl methacrylate IOLs. Larger samples and longer follow-up are required to confirm the outcomes of this technique.

[Phacofragmentation of dislocated lens in vitreous cavity].

Abstract:OBJECTIVE:To discuss the surgical techniques and therapeutic effects of phacofragmentation of the dislocated lens in the vitreous cavity. METHODS:Phacofragmentation was performed on 23 eyes of 23 patients with dislocated lens or subluxated lens in the vitreous cavity. With high suction and moderate ultrasound, the nucleus could be emulsified. Energy and time of emulsification: 50%-60% of energy was set up and emulsification spent 43 s to 3 min 23 s (mean 1 min 57 s). The pressure of maximum vacuum aspiration was 110-120 mm Hg (1 mm Hg = 0.133 kPa). In 11 eyes, anterior chamber intraocular lens (ACIOL) was implanted. Cryoapplications and encircling procedure were also performed on one eye. RESULTS:Phacofragmentation was successful in all patients. After follow-up for 5-19 months (mean 7.2 months), the corrected visual acuity was obviously improved in the cases of ACIOL and Marfan's syndrome, with 12 patients achieving visual acuity of 0.12-0.6. The major complications were corneal edema and damage of iris. CONCLUSIONS:It is demonstrated that phacofragmentation in the central vitreous cavity is safe and fairly easy procedure without adverse effects on retina and sclera. It helps to reduce intraoperative risks of alternative methods, avoiding a large opening of the anterior chamber and hypotony of the globe.

Abstract:Objetive To investigate the safety and efficacy of the capsular tension ring implantation in phacoemulsification of lens subluxation.Methods From 2009 to 2011,26patients(28 eyes)with lens subluxation were enrolled.After clear cornea incision and continuous curvilinear capsulorhexis,the tension ring was implanted in the capsular bag,to make sure the capsular bag was centered and fixed.Following irrigation/aspiration,an intraocular lens was then inserted into the capsule bag.Results The phacoemulsification and intraocular lens implantation was successful in all patients.The best corrected visual acuity in patients followed up improved significantly(P<0.05).The intraocular lenses were well centered in 23 eyes;mild tilt in 3 eyes,but no obvious eccentric;mildly eccentric in 2 eyes.No serious complications occurred.Conclusions Capsular tension ring implantation can stabilize the capsular bag,ensure the conduct of phacoemulsification and intraocular lens implantation in capsular bag,maintain long-term stability of the intraocular lens, which is safe and effective.

Abstract:Objective To investigate the effectiveness and safety of anterior vitrectomy combined with capsular retractor and Cionni modified capsular tension ring (MCTR) in traumatic lens subluxation surgery with anterior vitreous prolapse.Methods This study comprised 16 patients (16 eyes) of traumatic lens subluxation with anterior vitreous prolapse,in which iridodialysis was in 3 eyes,express implantation in 1 eye and Ahmed glaucoma valve implantation in 1 eye.According to the severity of lens subluxation,the patients were divided into three levels,including 90°-120° (7 eyes),120°-180° (5 eyes) and 180°-270° (4 eyes).Considering the anterior vitreous prolapse,anterior vitrectomy or/and pars plan vitrectomy was conducted with assistance of triamcinolone acetonide.After continuous curvilinear capsulorhexis,two to four capsular retractors were placed in the capsulorhexis to support and center the capsule.MCTR was inserted with scleral suture fixation after phacoemulsification.At last,the foldable IOL was implanted to capsular bag.Postoperative visual acuity,intra-and post-operative complications,anterior capsular opening,IOL positions and intraocular pressure (IOP) were assessed during 3 months' follow up.Results All patients had successfully undergone phacoemulsification,MCTR insertion and IOL implantation.Seven eyes were inserted with 2-eyelet MCTR and nine eyes were inserted with 1-eyelet MCTR.The postoperative visual acuity was better than 0.5 in 9 eyes,between 0.3 and 0.5 in 4 eyes,between 0.1 and 0.3 in 2 eyes,and less than 0.1 in 1 eye.Compared with pre-operation,the difference in the visual acuity was statistically significant (x2 =17.503,P =0.000).The scheimpflug images obtained from the Pentacam,which demonstrated that the IOL was well centered,and no MCTR and IOL decentration occurred.The common intraand post-operative complications were residual cortex,aqueous misdirection syndrome,anterior capsular tears,anterior capsular phimosis and posterior capsular opacification.Conclusion The effective application of anterior vitrectomy is the key point to reduce surgical complications.Anterior vitrectomy combined with insertion of capsular retractor and MCTR is an ideal surgical option for patients of traumatic lens subluxation with anterior vitreous prolapse.

Abstract:Many systemic or localized ocular conditions can lead to the weakening of the zonules that support the intraocular lens, subsequently leading to lens subluxation or complete dislocation into the vitreous cavity. Some of these conditions include pseudoexfoliation syndrome, Marfan syndrome, Weill-Marchesani syndrome, intraocular neoplasia, aniridia, trauma, microspherophakia, and iatrogenic causes such as prior vitrectomy or filtration procedures. Traumatic zonular dialysis is a major cause of lens subluxation and dislocation. Blunt trauma may cause transient shortening of the eye in the anterioposterior direction causing equatorial expansion, which can stretch or break the zonules. In a similar manner, perforating and penetrating injury of the eye can damage the zonules directly or indirectly. Patients may develop traumatic cataract or be symptomatic purely from the subluxed lens. Symptoms of a subluxed or dislocated lens include hyperopic or myopic shift, severe astigmatism, monocular diplopia, glare, and functional aphakia. The routine phacoemulsification techniques used today for cataract surgery may not be suitable in cases with weak zonular support. Here we review the current management strategies for traumatic crystalline lens subluxation and dislocation.

Long-Term Postoperative Outcomes Following Cionni Ring and In-the-Bag Intraocular Lens Implantation in Eyes With Subluxated Lenses

Abstract:Purpose: Evaluate long-term outcomes following Cionni modified capsule tension ring (MCTR) and in-the-bag intraocular lens (IOL) implantation in subluxated lenses. Design: Retrospective case series. Methods: Forty-one eyes of 31 patients from Raghudeep Eye Hospital, India, with subluxated lenses who had completed a minimum of 5 years' postoperative follow-up were included. Lens extraction, capsular bag fixation with MCTR using 9-0 polypropylene suture, and in-the-bag IOL implantation were performed in all eyes. Corrected distance visual acuity (CDVA), IOL centration, posterior capsule opacification, glaucoma, and retinal complications were documented at final follow-up. Results: The mean age of the cohort was 20.48 ± 16.46 (SD) years. Twenty-four eyes (58%) were below 15 years of age at the time of surgery. Marfan syndrome accounted for 37% cases. Mean follow-up was 9.89 ± 3.81 (SD) years. Thirty-two eyes (74%) had a CDVA of ≥0.3 logMAR at final follow-up. IOL decentration was noted in 7 eyes (17%), requiring a secondary surgery. The mean duration from primary surgery to resurgery was 8.79 years. Seventeen eyes (41%) required a laser capsulotomy, 88% of which were pediatric eyes. Retinal detachment occurred in 4 eyes (10%), 3 of which had Marfan syndrome. Conclusion: Capsular bag fixation with an MCTR using 9-0 polypropylene and in-the-bag IOL implantation had good long-term visual outcomes with an acceptable rate of serious postoperative complications in eyes with subluxated lenses. This approach allows preservation of the natural compartments of the eye and placement of an IOL in its most physiological position. However, considering a 17% rate of IOL decentration requiring surgical intervention, long-term stability with nonbiodegradable suture materials such as polytetrafluoroethylene as well as decentration rates following sutured or sutureless scleral fixation should be compared.
