Abstract:Objective To summarize the experience of urethroplasty of hypospadias in adults.Methods 22 hypospadias patients aged from 18 to 29 years were underwent urethroplasty,including 12 of penile type,9 of penoscrotal type and 1 of scrotal type.The urethroplasty procedures were scrotal septal flap technique in 14 patients,the Duckett technique in 7 and Duckett plus Duplay technique in 1.No urinary diversion was used in all cases.Results Within 5 to 24 months follow-up,14(63.6%) cases were cured.Urinary fistula occurred in 6 cases(27.3%) and were cured by reoperation.Urethral fieldstone formation were observed in 2 cases(9.1%) and were removed under cistoscope.Conclusion The postoperative complication of hypospadias is easy emergence in adults.The improvement of surgical technique,material and drainage method can increase the cure rate and decrease the complication rate.

Abstract:Objectives: To discuss the characteristics in adult with hypospedias and try to increase the achievement ratio in hypospadias repair. Methods: From Sept. 2001 to Dec. 2005 we have 29 patients age from 16 years old to 62 years old (average 22.9 y r) with hypospadias undergoing 39 times operations. The procedures include 34 urethroplasty and 5 fistula repair. We evaluated the procedures and discussed how to management the adult patients with hypospedias. Result: 19 of 28 (67.9%) of primary cases were successful. Fistula developed in 10 of 34 (29.4%) cases. Urethra stricture developed in 6 of 34 (17.6%) in all urethroplasty surgery. Conclusion: Urethroplasty methods is the key point for hypospadias repair surgery in adults. TIP urethroplasty is suitable for most single cases. Inlay labial mucosal graft with TIP urethroplasty is a good choice for complicated hypospadias repair with or without urethrostenosis. Dressing technique also play an important role in management of hypospadias in adults.

Abstract:Hypospadias is the most common congenital anomaly of the penis. The problem usually develops sporadically and without an obvious underlying cause. The ectopically positioned urethral meatus lies proximal to the normal site and on the ventral aspect of the penis, and in severe cases opens onto the scrotum or perineum. The foreskin on the ventral surface is deficient, while that on the dorsal surface is abundant, giving the appearance of a dorsal hood. Chordee is more common in severe cases. Cryptorchidism and inguinal hernia are the most common associated anomalies. The frequency of associated anomalies increases with the severity of hypospadias. For isolated anterior or middle hypospadias, laboratory studies are not usually necessary. Screening for urinary tract anomalies should be considered in patients with posterior hypospadias and in those with an anomaly of at least one additional organ system. The ideal age for surgical repair in a healthy child is between 6 and 12 months of age. Most cases can be repaired in a single operation and on an outpatient basis. Even patients with a less than perfect surgical result are usually able to enjoy a satisfactory sexual life.

[Long-term outcomes after hypospadias surgery: Sexual reported outcomes and quality of life in adulthood]

Abstract:Objective: To evaluate outcomes and long-term sexual quality of life after hypospadias surgery. Seventeen-years-old patients operated for a posterior hypospadias in childhood were included in a transversal study. Patients and methods: Fifteen patients, among the forty children treated since 1997, accepted to participate. These young men (mean age at the first surgery was 27.9±20months) were clinically reviewed and responded to questionnaires (EUROQOL 5, IIEF15 and non-validated questionnaire). This study arises about 8.4±5years after the last visit in paediatric department. Results: Mean study age was 21.2±4.7years. One third of patients thought that global quality of life was distorted. Although 33% of the patients had erectile dysfunction, 80% were satisfied with their sexual quality of life. The most important complains were relative to the penile appearance. Number of procedures was not predictive of patient's satisfaction about penile function and appearance. Thirty-three percents of the patients would have been satisfied to have psychological and medical support. They would be interested in having contact with patients who suffered from the same congenital abnormality. Conclusion: These patients had functional and esthetical disturbances. This visit leads to a specific visit in 20% cases. In this study, medical follow-up does not seem to be counselling and had to be adapted. Adequate follow-up transition between paediatric and adult departments especially during adolescence seems to be necessary.

Abstract:BACKGROUND AND OBJECTS: The patient's age at the first hypospadias repair may be an important factor for determining postoperative outcomes. Age at the first procedure differs between Western countries and medical centers in China. This review examines the differences between the incidence of surgical complications and surgical age in boys receiving hypospadias repairs in North America, Europe and China.Literature reports were reviewed in PubMed and WanFang databases using the key terms and phrases; 'hypospadias outcomes', 'complications of hypospadias repair' and 'timing of hypospadias repair'. All peer-reviewed articles published over the past decade (2001-2011) were considered if; a full text was available, the article included age at the first hypospadias procedure and surgical complications.In total, 16 131 patients were reported in 113 papers from North America, Europe and China according to our inclusion criteria. There was a significant difference in age at the first hypospadias surgery (P < 0.0005) and in the incidence of complications (P <0.001) between the different global regions investigated, with the earliest surgeries occurring in North American patients. Urethral fistulas were the most common complication reported in all of the regions included in this study.Fellowship training in pediatric urology could improve surgical outcomes, particularly in young children. Younger children experience fewer complications following hypospadias surgeries, independent of training and access to resources.
Hypospadias surgery: when, what and by whom?

Abstract:Hypospadias is repaired by paediatric surgeons, paediatric urologists, adult reconstructive urologists and plastic surgeons. This review is unique in representing all four specialities, to provide a unified policy on the management of hypospadias. The surgeon of whichever speciality should have a dedicated interest in this challenging work, ideally having an annual volume of at least 40-50 cases. The ideal time for primary repair is at 6-12 months old, although when this is not practicable there is another opportunity at 3-4 years old. A surgical protocol is presented which emphasises both functional and cosmetic refinement. Using a logical progression of a very few related procedures allows the reliable correction of almost any hypospadias deformity. A one-stage repair is used when the urethral plate does not require transection and its axial integrity can be maintained. Occasionally, when the plate is of adequate width and depth, it can be tubularized directly using the second stage of the two-stage repair. When (usually) the urethral plate is not adequately developed and requires augmentation before it can be tubularized, then that second-stage procedure is modified by adding a dorsal releasing incision +/- a graft (alias Snodgrass and 'Snodgraft' procedures). The two-stage repair offers the most reliable and refined solution for those patients who require transection of the urethral plate and a full circumferential substitution urethroplasty. From available evidence this protocol combines excellent function and cosmesis with optimum reliability. Nevertheless, it would be complacent to assume that these gratifying results will be maintained into adult life. We therefore recommend that there is still a need for active follow-up through to genital maturity.

Abstract:Objectives The exact incidence of failed hypospadias repair (FHR) in adult patients remain unknown, but the number is rapidly increasing. The study aims to evaluate the most common issues in adult patients requiring additional surgical treatment due to FHR in childhood. Methods From January 2015 to January 2023 patients with previous hypospadias repair in childhood, who underwent additional surgery in adulthood, were evaluated. The most common complications, total number of surgical procedures and time span between childhood and adulthood surgery were recorded. Postoperative follow-up included 1-, 3-, and 6-months evaluation and annually afterward. Results Fifty-five patients were treated with a mean age of 26 years. One stage repair was performed in 47 (85.5%) and staged repairs in 8 (14.5%) patients. The most common reasons for surgical repair were: urethral stricture in 8 (14.5%) patients, failed urethroplasty with abnormal meatal position in 10 (18.2%) patients, residual curvature or deformity in 10 (18.2%) patients and combination of several complications in 27 (49.1%) patients. Urethral repair was performed using buccal mucosa graft (BMG), penile skin flap, or combination of BMG and skin flap. Penile curvature and deformities were corrected by plication procedures. Mean time interval between the last surgical procedure in childhood and the definitive correction in adulthood was 16 years. Twenty-four (43.6%) of 55 patients underwent one surgical and 31 (56.4%) patients underwent two or more surgical procedures in childhood. Mean number of procedures was 4. The mean postoperative follow-up period was 18 months. The overall successful outcome was achieved in 50 (90.9%) patients while 5 (9.1%) patients were considered failures, as additional procedures were necessary. Conclusions Surgical treatment of failed hypospadias repair is challenging due to poor quality of available tissue and excessive scar formation. Regular and long-term follow-up after primary repair is necessary and these patients should be referred to a center of excellence. Conflicts of Interest The authors declare no conflict of interest.

Abstract:Hypospadias is one of the most common congenital malformations, affecting about 4-6 males per 1000 male births, and ranging in severity from a urethral meatus that is slightly off-center to a meatus in the perineal area. Over the past three decades its prevalence may have increased due to changes in reporting of mild cases and/or increased survival of low birth weight infants due to improved neonatal care. However, despite the increasing numbers of males with hypospadias, the overall etiology remains unclear and likely multifactorial in nature. The purpose of this review article is to provide a comprehensive overview of the various factors implicated in hypospadias etiology, including genetic and environmental factors. In addition, we list syndromes in which hypospadias is a relatively common association and delineate the areas that require further investigation in an effort to understand this condition.

Abstract:This study aimed to assess the cosmetic and sexual outcomes of childhood hypospadias repair after puberty. Among 672 patients who underwent hypospadias repair between 2001 and 2017, 243 sexually active patients were included in the study. At their last visit, cosmetic and sexual evaluation were done through the Penile Perception Score, the Hypospadias Objective Scoring Evaluation, IIEF , MSHQ-EjD. The levels of erectile dysfunction(ED) severity were determined according to erectile function domain of IIEF . The median follow-up after the last surgery was 16 [IQR (13–18)] years, and median patient age at the time of study was 19.67 [IQR(18–22)] years. Patients were divided into two subgroups as follows: Group I; Repairs using grafts ( n = 120, 49.4%) and Group II; Graft-free repairs ( n = 123, 50.6%). The rate of dissatisfaction with penile appearance was higher in Group I (16.7% vs. 4.9%, p = 0.003). Mild ED was more frequent in Group I (14.2% vs. 3.3%, p = 0.007); while none of the patients had either moderate or severe ED. Other domains of sexual function were observed to be similar in both groups. The need for unwanted interventions due to complications, and surgeries requiring grafts were found to be risk factors for ED development and dissatisfaction with penile appearance in adulthood. Those who underwent graft-free hypospadias repair experience better cosmetic and sexual outcomes when compared to grafted repairs, yet, satisfactory results were achieved in almost 90% of the grafted repairs.

Complications Following Primary Repair of Non-proximal Hypospadias in Children: A Systematic Review and Meta-Analysis

Abstract:Purpose: The aim of this study was to systematically review the literature on the complications and postoperative outcomes of children with non-proximal hypospadias. Methods: Electronic databases including PubMed, Embase, and Cochrane Library CENTRAL were searched systematically from January 1990 to June 2020 for the literature that reported the postoperative outcomes of patients with non-proximal hypospadias. Non-proximal hypospadias encompassed distal and mid-penile hypospadias. Results: We included 44 studies involving 10,666 subjects. Urethrocutaneous fistula (UCF) was the most common complication with an incidence of 4.0% (95% CI, 3.1–5.0%). Incidence of overall complications was 8.0% (95% CI, 6.3–9.8%). Meta-regression analysis revealed that length of urethral stent indwelling (coefficient 0.006; 95% CI, 0.000–0.011; p = 0.036) and penile dressing (coefficient 0.010; 95% CI, 0.000–0.021; p = 0.048) were two risk factors for UCF. Multivariate meta-regression analysis did not identify any independent risk factors for UCF. No differences were found between stent and stentless groups in non-proximal hypospadias regarding incidences of UCF (OR, 0.589; 95% CI, 0.267–1.297), meatal stenosis (OR, 0.880; 95% CI, 0.318–2.437), and overall complications (OR, 0.695; 95% CI, 0.403–1.199). No differences were found between foreskin preservation and circumcision in terms of complications either. Conclusions: UCF is the most common complication following hypospadias repair with an incidence of 4.0%. Independent risk factors for UCF were not identified in the current research. Distal hypospadias repair without stent indwelling is not likely to compromise the postoperative outcome. Further studies should be designed to explore the differences between different surgical approaches and the potential risk factors for complications following hypospadias repair.

Penile Appearance, Sexual Function and Sexual Satisfaction of Hypospadias Patients after Surgery:A Long-Term Follow-Up Study

Abstract:OBJECTIVE:To evaluate the long-term outcomes of hypospadias surgery relating to penile appearance, sexual function and sexual satisfaction, and analyze the influencing factors by comparing them with those of healthy male adults.METHODS:We conducted follow-up visits to 128 hypospadias patients surgically treated in our hospital between January 1990 and June 1994. We retrospectively analyzed their clinical data in comparison with 136 healthy male adults.RESULTS:The rate of satisfaction with penile appearance was significantly lower in the hypospadia patients than in the healthy men, and the main reason for the patients'dissatisfaction was inadequate penile size. There were no statistically significant differences between the two groups in sexual function and sexual satisfaction. The rate of sexual satisfaction was lower in the patients treated for proximal and middle shaft hypospadias but higher in those treated for distal shaft hypospadias than in the healthy adults, particularly high in those successfully treated by one-stage surgery at the age of < or = 3 years. The factors influencing postoperative sexual satisfaction included penile appearance, severity of hypospadias, surgical complications, surgical stage, age of surgery, and premature ejaculation. However, sexual satisfaction was not affected by surgical complications and surgical staging in those successfully treated by one-stage surgery at the age of < or = 3 years.CONCLUSION:Importance should be attached to the long-term follow-up of hypospadias patients after surgery. Sexual satisfaction could be achieved by successful one-stage urethroplasty at the age of < or = 3 years. Appropriate surgical procedures based on the characteristics of different cases may help the patients to establish confidence in their future sexual activities.

Abstract:Hypospadias refers to a congenital pathology of the genitourinary system. This bibliographical review analyzes and systematizes historical, epidemiological, clinical, populational and genetic data, highlights the problem of reproductive health of adult patients. An expert evaluation of materials related to the modern classification of hypospadias for both genders, diagnosis, surgical treatment options, family planning and childbearing taken from the PubMed database of the scientific periodicals was carried out. Authors noted the increased dynamics of the disease in the population. Hereditary predisposition of hypospadias was confirmed as well as the inverse proportionality of the severity of the disease and the ability of a patient to enter the relationships and the reproductive function in the event that hypospadias is not aggravated by testicular dysfunction, reduction of the vas deferens and other factors.

Abstract:Hypospadias is one of the most common congenital anomalies defined by abortive development of the urethral spongiosum, the ventral prepuce and in more severe cases penile chordee. The etiology of hypospadias remains unknown with environmental exposure in the form of endocrine disruptors the most likely explanation for the worldwide increase in incidence in the last three decades. There are five sequential steps for the successful repair of hypospadias: 1) Orthoplasty or penile straightening, 2) Urethroplasty, 3) Meatoplasty and Glanuloplasty, 4) Scrotoplasty, and 5) Skin Coverage. The major technical advances in hypospadias surgery that have improved surgical outcomes are 1) Preservation of the urethral plate, 2) Incision of the urethral plate, 3) Dorsal midline plication, 4) Deepithelized urethroplasty dartos flap coverage, and 5) Two stage alternative techniques. This article reviews the pertinent embryology, anatomy and the most common hypospadias reconstructive operations to accomplish a successful repair.

Two-Stage Repair With Buccal Mucosa For Severe And Complicated Hypospadias In Adults

Abstract:Objectives:To report the outcomes of two-stage repair for adult hypospadias using a buccal mucosa graft as a tube.Methods:A total of 57 adult patients who underwent surgery for severe urethral defect at our institution from January 2002 to March 2010 were included in this analysis. The urethroplasty procedure included penile urethra reconstruction with the buccal mucosa graft used as a tube in the first stage, anastomosis of the unobstructed urethra in the second stage and a scrotal fascio-cutaneous flap transferred to resurface the reconstructed urethra for waterproofing.Results:No complications occurred after the first stage, whereas postoperative infections occurred in six cases and distal necrosis of the scrotal flap occurred in two cases after the second stage. All eight of these patients had the complication of urethrocutaneous fistula, which healed after 2-4 weeks by cleaning and changing dressings in six patients. Twelve patients complained of straining at voiding in the early postoperative period. However, nine of these patients had an obvious increase in maximum flow rate without any intervention 3 months after the operation. Follow up ranged from 6 to 62 months (mean 28 months) in 46 patients. Most of the patients presented terminal meatus, no chordee, no urethral stricture and normal voiding. The majority of patients achieved excellent cosmetic and functional results.Conclusions:A buccal mucosa graft as a tube is a reliable and durable method of two-stage repair for severe and complicated hypospadias in adults. The use of scrotal flaps seems to contribute to a good vascularized waterproof cover.
General Considerations in Hypospadias Surgery

Abstract:Preoperative evaluation is an integral part of hypospadias surgery. It includes the type of hypospadias, size of urethral plate and glans, chordee and penile torque, hypoplastic urethral length and dorsal hood. Severe hypospadias should be evaluated radiologically for lower and upper urinary tract, hormonal status, and intersex disorders. Six to 18 months of age is the golden time to operate the hypospadias patients. Patients are operated on under general anaesthesia, regional and local blocks for postoperative pain. The microsurgical instruments, magnification, and gentle tissue handling help to have better results. Bladder drainage with an appropriate catheter helps in improving the outcomes. The use of local or systemic hormones is useful for improving the growth and vascularity of the urethral plate in the patients with poorly developed urethral plate. The urethral plate preservation urethroplasty results are better, and the addition of spongioplasty to TIPU reconstructs a near-normal urethra. The algorithms are proposed with a rational approach for both one- and two-stage urethroplasty. Surgical training, experience, and type of hypospadias guide the surgeon to choose one-stage or two-stage repair. Applying the basic microsurgical principles, choice of one- or two-stage in a given case, use of the magnification and fine suture material, surgery at desired age, and proper postoperative care yield good results of hypospadias repair.
Staged Hypospadias Surgery for Adult Males

Abstract:OBJECTIVE:To explore the necessity of staged hypospadias surgery for adult men in order to improve the success rate of operation.METHODS:We retrospectively analyzed 52 cases of hypospadias treated in our department from January 2004 to January 2012. The patients were adult males at the mean age of 22 years and all had a history of urethroplasty, with curvature of the penis and scar tissues on the penile skin. We removed the scarred fibrous tissues on the ventral cavernosa and cut off the urethral plate following foreskin-degloving. For those still with penile curvature, we straightened the penis by plication of the dorsal tunica albuginea, with the length of the anterior urethral defect > 50% of that of the penis after penis-straightening. The patients were assigned to group 1 (n = 20) to receive stage-I foreskin vascular pedicle flap urethroplasty and group 2 (n = 32) to undergo foreskin-shaping at the ventral aspect of the penis following penile straightening to prepare the urethra plate for stage-II Duplay urethroplasty after 6-12 months.RESULTS:The success rates of urethroplasty were 25 and 56.3% in groups 1 and 2, respectively, with the post-urethroplasty incidence rates of urinary fistula of 50 and 21.9%, urethral stricture of 15 and 9.4%, local wound infection of 30 and 25%, and urethral rupture of 20 and 12.5%. There were statistically significant differences between the two groups in the incidence of urinary fistula and the success rate of urethroplasty, but not in urethral stricture, local wound infection and urethral rupture.CONCLUSION:For adult hypospadias patients with a history of urethroplasty, especially those with obvious penile curvature, long urethral defect and insufficient foreskin, staged hypospadias surgery is preferable, which can dramatically increase the success rate of second-stage urethroplasty.
Hypospadias repair: a single centre experience

Abstract:Objectives. To determine the demographics and analyze the management and factors influencing the postoperative complications of hypospadias repair. Settings. Hayatabad Medical Complex Peshawar, Pakistan, from January 2007 to December 2011. Material and Methods. All male patients presenting with hypospadias irrespective of their ages were included in the study. The data were acquired from the hospital's database and analyzed with Statistical Package for Social Sciences (SPSS). Results. A total of 428 patients with mean age of 8.12 ± 5.04 SD presented for hypospadias repair. Midpenile hypospadias were the most common. Chordee, meatal abnormalities, cryptorchidism, and inguinal hernias were observed in 74.3%, 9.6%, 2.8%, and 2.1% cases, respectively. Two-stage (Bracka) and TIP (tubularized incised urethral plate) repairs were performed in 76.2% and 20.8% of cases, respectively. The most common complications were edema and urethrocutaneous fistula (UCF). The complications were significantly lower in the hands of specialists than residents (P-value = 0.0086). The two-stage hypospadias repair resulted in higher complications frequency than single-stage repair (P value = 0.0001). Conclusion. Hypospadias surgery has a long learning curve because it requires a great deal of temperament, surgical skill and acquaintance with magnifications. Single-stage repair should be encouraged wherever applicable due to its lower postoperative complications.

Abstract:Introduction Proximal hypospadias surgery is impacted by a high complication rate. The goal of this work was to assess the overall composite complication rate, fistula rate and stenosis rate following proximal hypospadias surgery realized according to onlay urethroplasty, Duckett, Koyanagi and Bracka techniques. Methods The databases MEDLINE, EMBASE, SCOPUS, Cochrane Library, the Cochrane Database of Systematic Reviews and the Cochrane Central Register of Controlled Trials (CENTRAL) and Sciencedirect were searched. Studies had to report data about the mean age of population, the average duration of patient follow-up and the number of procedures required for surgical treatment of primary and proximal hypospadias. Two independent including one urologist reviewers screened all the articles and selected the articles to be included. Results Overall composite complication rates were 32%, 34%, 49%, and 43%, for Onlay urethroplasty, Duckett's tubularized flaps urethroplasty, Koyanagi repair and Bracka 2 stages repair, respectively. Fistula rates were 13%, 18%, 21% and 23% respectively. The heterogeneity of complication rates reported in the different studies was not moderated by age, country, or patient's continent origin. Discussion The classifications of complications used in articles were disparate and make comparisons between techniques difficult. The report of post-surgical complications in the literature is often poorly coded and follow-up times were often too short. Conclusion This meta-analysis attempts to determine to the extent possible, given the serious weaknesses in the hypospadias literature, plausible estimates of complication rates after skin flap urethroplasty. The patched onlay skin flap, the Duckett's tubularized skin flap technique, the Koyanagi's technique, and the Bracka's two-stage urethroplasty procedure lead to very high complication rates. Reported complication rates are comparable across techniques.
