A delay in correlation between cyclosporine dosage and trough level measurements

Abstract:The adjustment of cyclosporine trough levels within a desired therapeutic range is complex because of the weak correlation between drug dose and immediate measurements. The correlation is significantly improved if a time interval is imposed between the two parameters. The time required to establish a correlation or the delay between dosage and trough measurements varied considerably between patients. This study consisted of applying the linear regression analysis to data from the dose and the corresponding trough level curves followed sequentially by trough measurements from succeeding days. Regardless of the assay used and the wide variability between patients, an initially weak correlation improved significantly after a period of delay. The delay before trough levels corresponded with given dosages indicated an indirect relationship and suggested a linkage between circulating cyclosporine and a bound or storage compartment. A period of instability in trough level measurements is observed upon initiation of cyclosporine immunosupression and after changes in dosages indicating a time interval required to establish a more stable relationship between the circulating and bound compartments.
