Abstract:The etiology of tumor of the follicular infundibulum (TFI) is unknown. Eruptive forms of TFI are rare. We present the case of a 49-year-old woman with multiple lesions on the arms, shoulders, trunk, buttocks, and legs of more than 3 years' duration. On clinical and histologic examination, a diagnosis of multiple TFI was made. Additionally, the patient presented with other rare remarkable features including severe pruritus, the Köbner phenomenon, and underlying inflammatory cell infiltration of the tumors. These findings strongly suggest that eruptive TFI may represent a kind of cutaneous reaction.
An Ulcer on the Nipple

Abstract:A 39 year old woman presented with an asymptomatic ulcer on her left nipple. The ulcer had been present for five weeks. She also reported malaise. She denied any travel history in the past three months, but recalled a history of breast bites and breast-genital contact during sexual intercourse, which had occurred two weeks before the skin lesion appeared. Physical examination revealed demarcated ulceration with a border that was slightly elevated, crusted, and indurated (fig 1). Left axillary lymphadenopathy was noted. Abstract:A 26-YEAR-OLD WOMAN PRESENTS WITH5WEEKS OF VULVAR SWELLING,PRURITUS, and progressively enlarging vulvar plaque accompanied by 2 months of occasional constipation and recurrent bloody, mucoid diarrhea. She has no history of anogenital warts and is monogamous with her husband. She has had no bloodtransfusions, contactwith trichloroaceticacid, spermicides,oranyother toxic agents or irritants. Physical examination reveals bilateral purple swelling of the labium majora and multiple moist, edematous, 1to 8-mm wart-like skin nodules on the perineal and perianal surfaces (FIGURE 1). A 2-mm ulcer is observed inside the right labium minus. Digital rectal examination shows bloody mucus mixed with loose stool. White blood cell count is normal (4 10/L), as is neutrophil count (2.07 10/L);hemoglobin level is slightlydecreased(10.2g/dL).Erythrocyte sedimentation rate is 71 mm/h (reference range, 0-20 mm/h), and C-reactive protein level is 28 mg/L (reference range, 0-5 mg/L). Bacterial and fungal swab cultures from the ulceration are negative. Vaginal and urethral swabs are negative, with moderately increased polymorphonuclear leukocytes. Abstract:A 21-YEAR-OLD MALE STUDENT PRESENTS WITH A NODULE ON HIS CHEST THAT has been progressively enlarging over 1 month. He also has a 1-month history of upper abdominal pain associated with diarrhea but without fever or chills. He was diagnosed with acute gastroenteritis and treated with oral omeprazole and ofloxacin without relief. He has lost 5 kg during the past half year. He denies any history of chronic illness or family history of cancer. Physical examination reveals left supraclavicular and cervical lymphadenopathy. Abstract:A 25-year-old, 7-month pregnant woman presented with rapidly progressive multiple ulcerated nodules present for 5 months on her left thigh. The lesions began as small red-colored papules, becoming nodular within few days. Initially, she was treated with systemic antibiotics and analgesics, and later a local physician performed incision and drainage. The lesions further increased, becoming indurated and ulcerated. Two months after her delivery, the lesions had extended to the middle of the medial side of her left thigh. On admission, she had a history of fever and weight loss, but there was no trauma. She delivered a full-term healthy baby boy. Past and family history was not significant. Examination revealed 15-20 ill-defined, erythematous, immobile, and irregular nodules of varying sizes, measuring 4 × 5 cm2, and extending from the inguinal region to the middle portion on medial aspect of the left thigh (Figure 1). Some of the nodules ulcerated with yellow color discharge with crusting, attributed to secondary bacterial infection. Multiple palpable and matted inguinal lymph nodes were present in the left inguinal region.

Abstract:An 82-year-old woman presented to the department of dermatology at our hospital in February, 2016, with a 6-month history of widespread pruritic nodules on her neck, trunk, arms, and legs (figure), which were easily eroded by scratching. She had been diagnosed with prurigo nodularis in a local clinic and treated with triamcinolone acetonide acetate ointment and oral antihistamines without improvement. We took a biopsy sample from one of the lesions on her back. Haematoxylin and eosin staining showed hyperkeratosis, epidermal hyperplasia, acanthosis, papillomatosis, subepidermal blisters, and the infiltration of lymphocytes and eosinophils in the upper dermis (appendix). Subsequent direct immunofluorescence analysis of the skin from the lesion showed linear deposition of IgG along the basement membrane zone (figure). Laboratory analysis showed circulating IgG to BP180 and BP230 (134 U/mL and 104 U/mL, respectively; normal <9 U/mL), and ELISA test for HIV was negative. We made a diagnosis of pemphigoid nodularis.

Abstract:A 63-year-old male presented with widespread nodules and blisters with intense pruitus for half years. Brown nodules and lichenified lesions were distributed on his trunk and limbs diffusely. Superficial ulcers, crusts and hypopigmentation occurred in some lesions. A few tense vesicles were also found in his upper arm. The histopathology of a biopsy from his left upper arm showed that a subepidermal bulla with numerous eosinophils, and a perivascular infiltration of eosinophils and lymphocytes in the upper dermis. Direct immunouorescence demonstrated linear deposition of IgG and C3 at the basement membrane zone. The diagnosis of nodular pemphigoid was established.

Abstract:The patient was a 24-year old female who′s pregnant for 24+1w.She had a history of subacute,chronic,recurrent vulvar ulcer before pregnancy and suffered a gangrenous ulcer during the pregnancy.The smear of the ulcer secretion reveals many G+ bulky bacilli with some blastospores.Histopathology shows thicker epidermis with spongiosis,diffuse infiltration of the entire dermis and subcutaneous fat lobules with neutrophil infiltration in some regions while no manifestations of vasculitis.The diagnosis was Ulcus Vulvae Acutum which had been relieved by a combination therapy of etimicin 0.2g/d and prednisone 30mg/d for 1 week followed by a termination of the pregnancy.

Abstract:A 51-year-old Chinese man presented with a lump on his left buttock. Two years prior, he worked as a gold dredging contractor for one year in Banalia, Tshopo province, the Democratic Republic of the Congo, and received repeated unidentified insect bites. Three months prior, he found a bean-sized lump on his left buttock. He then developed an erythematous itchy rash on his legs that extended over his whole body, without fever or enlarged lymph nodes. The rash improved without treatment. The lump enlarged gradually over two months. On physical examination, the lump (diameter 2.5 cm) was not tender or movable. The patient was afebrile and did not have palpable inguinal lymphadenopathy. Laboratory data revealed normal neutrophils and eosinophils, C-reactive protein level, and erythrocyte sedimentation rate. Ultrasound revealed a low echogenic subcutaneous area (27 × 15 × 18 mm) and subcutaneous edema. Excisional biopsy and histopathological examination were performed (Fig 1). What is your diagnosis?

Abstract:An otherwise healthy woman in her 40s presented with a 2-year history of itching of the vulva. She had been diagnosed with dermatitis and prescribed topical corticosteroids, but to little effect. She denied high-risk sexual contacts or exposure to potential contact allergens. Physical examination revealed well-demarcated white-red and hypertrophic plaques, with scattered dotlike erosions. As shown in Figure 1, vulvar skin changes were visualized as the patient retracted the surrounding tissues. Histopathologic examination revealed a proliferation of Paget cells within the surface squamous epithelium (Figure 2), with the Paget cells displaying positive staining for carcinoembryonic antigen and cytokeratin (CK) 7, and negative staining for AE1/AE3, CK20, and melan-A. Abstract:AN 85-YEAR-OLD MAN IS ADMITTED FOR EVALUATION OF MULTIPLE PAPULES, plaques, and subcutaneous nodules on his face, trunk, and extremities. The patient has itching. There is no history of trauma. He has an 8-year history of diabetes and Sjogren syndrome with eye and mouth involvement and has been treated with varied doses of oral methylprednisolone for 5 years. During the 3 years prior to admission, he took 12 mg/d of methylprednisolone. Over the last 5 months, he received topical steroids and antifungal agents. Skin examination reveals multiple erythematous papules on his face and eyelids, clustered or solitary reddish or purple papules, indurated shining plaques, and subcutaneous nodules on his wrists (FIGURE 1A), arms, and lower limbs, mainly around the knees (Figure 1B) and thighs. Pus is not expressed with pressure. There are no lesions on his scalp, eyebrow, upper lip, chin, axillae, pubic area, palms, or soles. What Would You Do Next?

Abstract:Background Lichenoid vulvar dermatoses (LVD) are inflammatory diseases primarily affecting the vulva and anus. This study aims to evaluate the skin changes in patients with LVD using high‐frequency ultrasound. Methods Forty‐five patients with LVD, who attended Henan Provincial People's Hospital from November 2021 to March 2024, were selected. According to the pathological conclusions, patients were divided into two groups: the vulvar lichen sclerosus (VLS) group (n = 24) and the vulvar lichen simplex chronicus (VLSC) group (n = 21). Thirty age‐ and BMI‐matched healthy women were selected as the control group. We assessed the epidermal thickness, subepidermal low echogenic band (SLEB) thickness, dermal thickness, and vascular index (VI) among the three groups. Receiver operating characteristic curve (ROC) analysis was performed to determine the diagnostic efficacy of these ultrasound parameters for LVD. Binary logistic regression was used to investigate risk factors influencing LVD pathology in VLS patients. Results Epidermal thickness, SLEB thickness, dermal thickness, and VI were increased in the VLS and VLSC groups compared to the control group (p 0.05). The ROC curves showed that the area under the curve (AUC) value for the dermis (AUC = 0.882) was the largest for VLS, and VI (AUC = 0.917), it was the largest for VLSC. Binary logistic regression indicated that having an allergic disease was a risk factor for VLS between VLS and VLSC groups (OR = 6.797, p = 0.028). Conclusion High‐frequency ultrasound can detect thickening of the skin and increasing VI in patients with LVD, which can be helpful in the evaluation and management of LVD.

Abstract:A 56-year-old female who had a history for 10 years presented with vulvar papules and vesicles.On examination,the entire skin of the vulva were covered by clustered complexional papules and vesicles.Partial ectomy of skin lesions was done for biopsy,and histological examination revealed hyperplasia of the epidermis.The dermis showed numerous dilated lymphatic and angiomatous spaces and an extensive lymphocytic cell infiltration.The diagnosis of simple lymphangioma was confirmed.

Abstract:A 35‐year‐old woman presented with a nodular growth on her right buttock. She had more than a 20‐year history of multiple, asymptomatic, smooth, skin‐colored papules involving her right buttock. The lesions had increased in number over time and had become yellowish or pale red with coalescing nodules exhibiting irregular surfaces. There was no general malaise. Ten years earlier, a diagnosis of connective tissue nevus was made on the basis of a biopsy obtained at a local hospital, but the patient declined treatment at that time. During the past 10 years, the lesions had continued to develop. The medical and social history was otherwise unremarkable, and a general physical examination was negative except for the skin lesions. Cutaneous examination showed soft, yellowish or pale red to violaceous papules and nodules of various sizes with wrinkled surfaces, coalescing into plaques. The lesions were aggregated on her right buttock and extended onto the thigh. The biggest nodule, with a coarse and dry surface, was 40 mm in diameter (Fig. 1). Histopathologic examination of the largest nodule revealed hyperkeratosis and epidermal hyperplasia with elongated rete ridges. There were thin‐walled, ectatic papillary dermal vessels engorged with red blood cells and masses of mature adipose cells throughout the dermis (Fig. 2) (Tianwen G, Jianfang S. Non‐epidermal related.
