Abstract:We report a 38-year-old man who presented with bilateral conjunctival congestion, hoarseness, and progressively growing pruritic, infiltrated skin lesions that had first begun over the face and neck, and later spread to the trunk and the limbs in 4 months. The clinical appearance of the lesions mimics granulomatous rosacea, acne vulgaris, or pityrosporum folliculitis. Histopathologic examination of the lesions from the face and chest both revealed dense dermal nodular lymphohistiocytic infiltrates which were positive for CD68 and S-100, but negative for CD1a. A systemic work-up for him detected no lymphadenopathy or other systemic involvement. A diagnosis of extranodal Rosai-Dorfman disease was made, and the patient received systemic glucocorticoids, with considerable improvement after 4 months of therapy.

Abstract:A 57-year-old woman presented with erythematous papules arranged in an annular on both arms,neck,legs and abdomen.Histopathologic analysis of a skin specimen revealed the granulomatous infiltrate and altered collagen in the upper dermis.Mucin accumulation in collagen fibers was obvious by an alcian blue stain.Mild elastic fiber fragmentation was observed by elastic stain.Diagnosis:generalized granuloma annulare.

Abstract:Chronic lymphocytic leukemia (CLL) infiltrating the skin is uncommon and can present in many different ways. The present study reports a case of CLL infiltrating multiple body areas. A 57-year-old male with a 10-year history of subclinical B-cell chronic lymphocytic leukemia (B-CLL) presented with skin hypertrophic changes of the ears, eyebrows, tip of the nose, toes and fingers. In addition, the patient had erythematous plaques on the buttocks. Histopathology revealed a lymphocytic infiltrate. The patient rejected the recommended chemotherapy and, following a three-year follow-up, remained alive with mildly aggravated symptoms. It has previously been reported that infiltrative CLL can involve the head and neck; however, involvement of multiple body areas, particularly toes and fingers is rare. This case highlights the importance of considering leukemia cutis in patients with underlying CLL who present with unusual clinical features.

Abstract:AN 85-YEAR-OLD MAN IS ADMITTED FOR EVALUATION OF MULTIPLE PAPULES, plaques, and subcutaneous nodules on his face, trunk, and extremities. The patient has itching. There is no history of trauma. He has an 8-year history of diabetes and Sjogren syndrome with eye and mouth involvement and has been treated with varied doses of oral methylprednisolone for 5 years. During the 3 years prior to admission, he took 12 mg/d of methylprednisolone. Over the last 5 months, he received topical steroids and antifungal agents. Skin examination reveals multiple erythematous papules on his face and eyelids, clustered or solitary reddish or purple papules, indurated shining plaques, and subcutaneous nodules on his wrists (FIGURE 1A), arms, and lower limbs, mainly around the knees (Figure 1B) and thighs. Pus is not expressed with pressure. There are no lesions on his scalp, eyebrow, upper lip, chin, axillae, pubic area, palms, or soles. What Would You Do Next?

Abstract:Recently, we described a case of generalized pure cutaneous Rosai-Dorfman disease in a 43-year-old Asian man in JAMA. The lesions distributed on nearly all of the skin of the whole body, except for mucous sites. Molecular, immunophenotypic, and sequencing analyses seem to define it as a histiocytic-mesenchymal transition and intermediate proliferative histiocytosis not associated with mtDNA large deletion and pathogenic mutation, as well as the SLC29A3 gene mutation.

Abstract:A 51-year-old Chinese man presented with a lump on his left buttock. Two years prior, he worked as a gold dredging contractor for one year in Banalia, Tshopo province, the Democratic Republic of the Congo, and received repeated unidentified insect bites. Three months prior, he found a bean-sized lump on his left buttock. He then developed an erythematous itchy rash on his legs that extended over his whole body, without fever or enlarged lymph nodes. The rash improved without treatment. The lump enlarged gradually over two months. On physical examination, the lump (diameter 2.5 cm) was not tender or movable. The patient was afebrile and did not have palpable inguinal lymphadenopathy. Laboratory data revealed normal neutrophils and eosinophils, C-reactive protein level, and erythrocyte sedimentation rate. Ultrasound revealed a low echogenic subcutaneous area (27 × 15 × 18 mm) and subcutaneous edema. Excisional biopsy and histopathological examination were performed (Fig 1). What is your diagnosis?

Abstract:Clinical Introduction An elderly Asian woman presented to the ED with month-long fever, rhinorrhea and productive cough. She subsequently developed tender bruise-like skin nodules over her trunk and limbs, which quickly progressed to form cavity ulcers 2 weeks before ED visit (figure 1). Abstract:A 49-year-old female presented with subcutaneous panniculitis-like T-cell lymphoma is reported. The patient had nodules and tumors on her extremities and trunk for one year. Tumors on the left leg had ulcerated for over two months and the patient had low fever, hypodynamia and anemia for nearly 2 months. Histopathologic examination showed the diffuse infiltration of atypical lymphocytes in subcutaneous tissue with local lipid necrosis and hemophagocytic phenomenon. Immunohistologic examination showed CD3(+), CD43(+), CD56(+), CD68(+), CD30(-), CD20(-).

Abstract:Diagnosis of nodular red lesions is challenging. The differential diagnosis includes dermal nevus, angioma, pyogenic granuloma, amelanotic melanoma, eccrine poroma, Kaposi's sarcoma, skin malignancy or metastasis. Erythema nodosum is one of the common consideration of the red skin nodules, however fully work up should be done to find the right diagnosis.A 60 years old female admitted to our hospital due to pain dark reddish skin nodules since one month. She had continuously high grade fever of 39 Celsius accompanied by arthralgia and fatigue since two months prior to admission and she lost 6 kg of weight in 2 months. On admission, physical examination revealed slight fever, pale conjunctiva, mild hepatosplenomegaly, tender dark red nodules 0.3 to 2 cm, firm edge, at her cheek, abdominal area and both lower extremities. No lymph nodes enlargement was noticed. Her laboratory test showed haemoglobin 9,1 g/dl, WBC 3,040/mL, PLT 149,000/mL, SGOT 48 U/L, SGPT 43 U/L, urea 12.5 mg/dL, creatinine 0.67 mg/dL. She was found to be non-reactive for HBsAg, HCV, and HIV antigens. Urine routine and microscopic examination was unremarkable.Her histopathology of left foot nodule biopsy revealed cutaneous lymphoma. The immunohistochemical (IHC) stain of CD45, CD20, and CD10 were positive, Ki67 were also positive with >70% tumor cells, while CD3,CD56, CD30, and Granzyme were negative. Her final diagnosed was Cutaneous Diffuse large B cell lymphoma.Primary cutaneous lymphomas of B-cells occur less frequently than primary cutaneous T-cells lymphomas. Primary extra-nodal diffuse large B-Cell lymphoma (DLBCL) can be seen in up to 40% of cases. However skin involvement is less common and in a large cohort of DLBCL cases, skin involvement at presentation was seen only in 3.3% of cases.It characterized by few lesions, in general showing nodules or infiltrations of relatively fast growth and have no itching. The diagnosis is made by the immunohistochemical findings, clinicopathological correlation, and molecular pathology. The lymphomas have different clinical behaviours despite being identical in morphological appearance. The primary lymphomas presents with local recurrence in up to 68% of the cases and with rare extra-cutaneous dissemination, with an average rate of 5-year survival varying from 89 to 96%. Cutaneous lymphoma should be always become one of considered diagnosed of skin red nodules even it is rare.

Abstract:A man in his 60s presented with a 20-year history of progressive pruritic skin lesions on his face and trunk. The lesions first appeared on his back without identifiable cause and spread, reaching the abdomen and eventually the head and face. Skin examination revealed multiple edematous red plaques distributed over the patient's forehead, eyelids, and chin that were accompanied by alopecia of the right eyebrow (Figure, A). These plaques were ill-defined, ranging from 3 cm to 6 cm in diameter. Simultaneously, we could identify generalized follicular red papules on his abdomen, back, and both sides of the chest (Figure, B). The papules were 2 mm in size and partially fused into red patches. Punch biopsies were performed from the chin and back, the results revealing dense and deep infiltration of lymphocytes in and around hair follicles. Immunohistochemical staining showed that the infiltrating cells expressed CD3 and CD4, while CD8 and CD56 were negative. His baseline laboratory investigations yielded normal results. His workup results were negative for extracutaneous involvement. The diagnosis of skin-limited folliculotropic mycosis fungoides (FMF) was established. The skin lesions achieved partial remission following treatment with interferon α (IFNa) injection and UV-B phototherapy.

Abstract:A 3-year-old girl presented with fever, abdominal distension and anorexia for 3 days. Physical examination showed pallor, abdominal distension and hepatomegaly (4 cm below the costal margin). Laboratory tests showed granulocytosis (17.0–31.3 × 10/L), anaemia (Hb 60–85 g/L), eosinophilia (0.58–1.89 × 10/L), raised immunoglobulin E (IgE) (>400 IU/mL), C-reactive protein (CRP) (>1600 mg/L) and erythrocyte sedimentation rate (ESR) (135 mm/h). Chest computed tomography showed multiple small nodules with sharp borders and irregular size scattered in both lung fields (Fig. 1a). Magnetic resonance imaging showed multiple nodular lesions in the liver (Fig. 1b), and enlarged abdominal aortic and superior mesenteric lymph nodes.
