Fever, Abdominal Distension and Hepatomegaly

Abstract:A 3-year-old girl presented with fever, abdominal distension and anorexia for 3 days. Physical examination showed pallor, abdominal distension and hepatomegaly (4 cm below the costal margin). Laboratory tests showed granulocytosis (17.0–31.3 × 10/L), anaemia (Hb 60–85 g/L), eosinophilia (0.58–1.89 × 10/L), raised immunoglobulin E (IgE) (>400 IU/mL), C-reactive protein (CRP) (>1600 mg/L) and erythrocyte sedimentation rate (ESR) (135 mm/h). Chest computed tomography showed multiple small nodules with sharp borders and irregular size scattered in both lung fields (Fig. 1a). Magnetic resonance imaging showed multiple nodular lesions in the liver (Fig. 1b), and enlarged abdominal aortic and superior mesenteric lymph nodes. Laparoscopy revealed many 2–5-mm white nodules on the surface of the peritoneum, liver and small intestine. What is your diagnosis and differential? (Answer on page 463)
