Abstract:Background and Aims: After endoscopic full-thickness resection (EFTR), reliable closure of the perforation is critical. However, it is technically difficult to close some defects by using metal clips alone or by purse-string suturing, which may lead to unreliable closure. Inspired by the process of pulling up the 2 ends of the incision in the surgical suture, we developed a new endoscopic closure technique, the "internal traction-assisted suspended closure" technique. This pilot study was performed as an initial evaluation of the feasibility and safety of this new endoscopic closure technique. Methods: Data from patients in whom this suspended closure technique was used to close full-thickness defects after EFTR were retrospectively reviewed. The primary outcome was successful closure rate. Secondary outcomes were closure time, length of postprocedural hospital stay, and incidence of postprocedural adverse events. Defect size and tumor characteristics were also analyzed. Results: Eight patients who underwent the suspended closure technique after EFTR were included. All patients were successfully treated with the suspended closure technique, and no patient developed serious adverse events. The median length of the defect was 3.25 cm (range, 2.5-9.0) and the median width was 2.8 cm (range, 1.8-6.0). The median closing time was 13 minutes (range, 6-24). Conclusions: The internal traction-assisted suspended closure technique is a simple, reliable, and easy-to-use technique for large full-thickness defects after endoscopic resection. Abstract:Background and study aim: Successful closure of wall defects is the key procedure following endoscopic full-thickness resection (EFTR). In this report we describe a new method similar to interrupted suture to repair gastric defects by means of endoloops and metallic clips.Patients and methods: We retrospectively analyzed 20 patients who presented at our institute between March 2011 and February 2012 with gastric submucosal tumors and who consequently underwent EFTR, with the resulting large gastric defects being closed using endoloops and metallic clips. Tumor characteristics, en bloc resection rates, and postoperative complications were evaluated in all the patients.Results: The median age of the 20 patients was 47 years. The mean maximum size of lesions was 1.47 +/- 0.72cm (range 0.4-3cm). All lesions were diagnosed by endoscopic ultrasound as tumors in the muscularis propria layer. The en bloc resection rate was 100%. Five patients developed fever and complained of slight abdominal pain in the first day after treatment. No patient had severe complications such as peritonitis or abdominal abscess. In all cases, wounds healed after 1 month. We observed the persistence of the clips for over 3 months in 6 out of 19 cases without any clinical manifestations or large injury to the intestine.Conclusions: Our study provides evidence that the use of metallic clips and endoloops is a relatively safe, easy, and feasible method for repairing gastric defects resulting from EFTR.

Abstract:Endoscopic submucosal dissection is established as a curative endoscopic method for gastrointestinal epithelial neoplasms with a high possibility of complete en bloc resection; however, postoperative adverse events of bleeding and delayed perforation remain. To prevent or minimize them, several techniques for endoscopic mucosal closure have been introduced, such as using endoscopic clips, combined use of hemoclips and supplement devices, and specially designed endoscopic suturing devices. Furthermore, endoscopic full-thickness suturing technique for gastrointestinal wall defect has been developed based on the concept in natural orifice transluminal endoscopic surgery and endoscopic full-thickness resection. Several closure techniques, including over-the-scope clip, threaded bars inserted in a hollow needle, stitches or staplers, and a curved needle and thread are reported. Secure closure of the iatrogenic defect may further expand the range of therapeutic endoscopy. Accumulation of evidence for the efficacy of endoscopic closure and establishment of these techniques are desired.
A New Endoscopic Closure Method for Gastric Mucosal Defects: Feasibility of Endoscopic Hand Suturing in an Ex Vivo Porcine Model (with Video)

Abstract:Background and study aims: More secure endoscopic closure techniques for iatrogenic gastric defects are required for safe endoscopic surgery. We developed a novel endoscopic suturing method, endoscopic hand suturing (EHS), of gastric mucosal defects and determined its feasibility and efficacy ex vivo. Materials and methods: We created 24 mucosal defects (each 2 cm in diameter) by endoscopic submucosal dissection. The following three techniques were tested: EHS with a 3 – 0 barbed suture that was grasped with biopsy forceps (n = 6) or a prototype through-the-scope needle holder (n = 6) by endoscopy, looping with endoloops (n = 6) by endoscopy, and clipping with hemoclips (n = 6) by hand. The mucosal edges were attached to each other at three points. The closure strength was compared among the three groups, and the procedural duration was compared between the EHS and looping groups. Results: All 12 lesions were completely closed by EHS. The median strength of the closure, measured with a spring scale, was significantly greater in the EHS group (0.74 kg) than in the looping group (0.33 kg, P = 0.0012) or clipping group (0.07 kg, P = 0.0009). The median procedural duration did not significantly differ between the EHS and looping groups (19.7 vs. 19.8 minutes, P = 1.0000). The use of the needle holder significantly reduced the procedural duration compared with the biopsy forceps. Conclusion: Mucosal defects can be firmly closed with EHS, which may be helpful for establishing a safer and more secure endoscopic surgery.
Using Endoloop and Metallic Clip to Repair the Gastric Defects after Endoscopic Full-Thickness Resection

Abstract:Introduction: With the increase of experience in endoscopic submucosal dissection, a new endoscopic technique named endoscopic full-thickness resection (EFTR) without laparoscopic assistance has been applied to resect gastric submucosal tumors originating from the muscularis propria layer and/or having close proximity to the serosa.1 The key stage of the EFTR procedure is the effective closure of the wall defect left after resection, to prevent peritonitis and the need for surgical intervention. Here we describe a new method to repair gastric defects by using endoloops and metallic clips. Methods: This is a 65-year-old patient, who felt epigastric discomfort since May 2013. Then, she went to hospital and had gastroscopy, the result showed that there was a tumor at the gastric antrum. The endoscopic ultrasonography showed the tumor was originating within the muscularis propria and no lymph node metastasis was detected. On November 13, 2013, we decided to use EFTR to treat this lesion. After a short anesthesia induction, the submucosal tumor was initially detected by endoscopy. First, we departed the lesion from the mucosal layer to the serosa using an electric knife. The tumor together with its surrounding muscularis propria and serosa was removed by snare resection. Then, the omentum could be seen through the gastric wall defect and we started using the endoloop and metallic clip interrupted suture method to repair a gastric defect. An endoloop was anchored with a clip to normal mucosa near the proximal resection margin, and then the maneuver was repeated to anchor the same endoloop at the distal resection margin. After that, the endoloop was tightened slightly to approximate the border of the defect. We totally used three endoloops in this case to repair the defect. After EFTR, the patient received gastrointestinal decompression and antibiotic treatment. Results: The patient woke up in half hour after EFTR and was discharged 6 days after the endoscopic resection. The antibiotic treatment was taken until the patient went home. After 1-month follow-up visits, the metallic clips and endoloops fell off spontaneously and the patient had no symptoms to report. Conclusion: At this time, the EFTR of small gastrointestinal stromal tumors and other submucosal tumors was a therapeutic technique under scientific evaluation. The use of the endoloop and metallic clip interrupted suture method seemed to be a safe, easy, and feasible new procedure for repairing gastric defects left after EFTR. Abstract:Objective:To evaluate the feasibility and short-term efficacy of a novel and simplified closure method developed by our team for the defect closure after endoscopic full-thickness resection (EFTR) for gastric submucosal tumors (SMT) in the gastric wall.Methods:A prospective single-arm clinical study was used. Inclusion criteria: (1) the lesion was located in the fundus or the greater curvature of the stomach, and was confirmed to originate from the muscularis propria layer; (2) the diameter of the tumor was ≤3.5 cm, and the tumor had no extensive adhesion to the peritoneal tissues and organs in extraperitoneal cavity; (3) the tumor had no malignant features under ultrasound endoscopy; (4) the patient agreed to participate in the study; (5) patients with severe complications were excluded. Based on the above criteria, 20 patients with gastric SMT at the Endoscopy Center of Zhongshan Hospital of Fudan University from January 2015 to March 2018 were enrolled in this study, including 5 males and 15 females with mean age of 61.1 (38 to 70) years. Grasping forceps-assisted endo-loop snare ligation device which is called "Shao-Mai" method was used to close the defect site. All the patients underwent EFTR and "Shao-Mai" method to perform defect closure. After successful tumor resection by EFTR, an endo-loop was anchored onto the edge of the gastric defect with grasping forceps assistance and closed tightly. The observation indicators included tumor size, en bloc resection, operation time, postoperative complications and hospital stay. The follow-up indicators included tumor residual, local recurrence, and metachronous lesions. Results:All the 20 lesions were located in the muscularis propria with a size of 0.5-3.5 (mean 1.4) cm. Three of them were located in the greater curvature of the mid-upper gastric body, 17 were located in the fundus. The endoscopic "Shao-Mai" closure was successfully performed after EFTR in all the 20 cases. Endoscope was used uniquely through the entire process, without laparoscopic assistance. The operative time was 20-100 (mean 43.8) minutes, while the "Shao-Mai" closure procedure took a range of 3-30 (mean 10.1) minutes. The en bloc resection rate was 100%. The pathological diagnosis included 17 gastrointestinal stromal tumors and 3 leiomyomas. No major complications occurred during or after surgery. All the patients were discharged 1-11 (mean 3.1) days after operation. The wounds of all the cases were healed completely six months after operation and only scar was observed without ulcer. No residual lesion, tumor recurrence or metastasis, leakage or fistula of digestive tract were found during the follow-up period of 15-54 (median 41) months. Conclusion:The endoscopic "Shao-Mai" closure method is a simplified novel way, which is feasible, effective, and safe for closing the gastric defect after EFTR.

Abstract:Nowadays, endoscopic submucosal dissection (ESD) is commonly performed for the removal of large gastrointestinal lesions. Endoscopic mucosal defect closure after ESD is vital to avoid adverse events. In recent years, many innovative instruments have emerged and proved to be beneficial. In this paper, we conducted a thorough literature review and summarized the closure methods for large‐size post‐ESD mucosal defects over decades. We separated these methods into five categories based on the operational principle: "side closure" method, "ring closure" method, "layered closure" method, "hand suturing closure" method, and "specially designed device closure" method. Side closure with clips assisted by instruments such as threads or loops is applicable for each segment of the gastrointestinal tract to prevent postoperative bleeding. If the defect tension is too large to close with the traditional side closure methods, zigzag closure and ring closure could be applied to gather the bilateral defect edges together and achieve continuous closure. In the stomach and rectum with a high risk of submucosal dead space between the submucosa and muscular layers, side closure methods with muscle layer grasping clip or layered closure methods could enable the involvement of the deep submucosa and muscle layers. The ring closure method and specially designed devices including over‐the‐scope clip, Overstitch, and X‐tack could resolve perforation effectively. Individual closure method requires endoscope reinsertion or sophisticated operation, which may be limited by the deep location and the narrow lumen, respectively. Although specially designed devices are expected to offer promising prospectives, the cost‐effectiveness remains to be a problem.

Abstract:Objective To evaluate the efficacy of a new endoscopic suturing device(OverStitchTM, Apollo Endosurgery, USA)for gastrointestinal wall defect after endoscopic full?thickness resection (EFTR). Methods Clinical data of the patients with submucosal tumors who underwent endoscopic suturing closure of gastrointestinal wall defect at the Endoscopy Center of Zhongshan Hospital from April 2015 to June 2015 were retrospectively analyzed. Results Five patients were included in this research with 4 lesions located in stom?ach and 1 lesion located in descendent duodenum. The mean diameter of lesions was 3?? 2 cm (ranging 2?? 0?5?? 0 cm). All patients underwent complete closure and none of them was converted to laparoscopic surgery or laparotomy. The mean repairing time was 16?? 6 min (ranging 11?? 0?22?? 0 min). The mean postoperative hospi?talization time was 5 days ( ranging 4?6 d). Two patients complained about slight abdominal pain and elevated body temperature. CT showed minor pleural effusion in one case. These patients received antibiotics, were on gastrointestinal decompression and asked to maintain the semi?supine position and discharged after 3 to 4 days. Conclusion Endoscopic suturing closure of gastrointestinal wall defect with the OverStitchTM en?doscopic suturing device is technically fast, feasible and safe.

Abstract:Objective To evaluate the clinical efficacy and safety of different stitching methods, over-the-scope-clip (OTSC) and metal clips combined with nylon rope(King closure), for full-layer gastric wall defect. Methods Data of 75 cases, who underwent endoscopic full-thickness resection (EFTR) of gastric SMTs from May 2015 to May 2018 in our endoscopy center were retrospectively analyzed. According to the closure method, the patients were divided into the OTSC group (20 cases) and the King closure group (55 cases). Comparison was made in gender, age, the largest diameter of tumor, the location of tumor, defect surface diameter, total operating time, defect closure time, closure success rate, the length of hospital stays, cost and postoperative complications between the two groups. Results The baseline data were comparable, and there were no significant differences in age, gender, tumor location, tumor diameter, and defect surface diameter between the two groups(all P>0.05). The success rate of closure was 100% in both groups. In terms of length of hospital stay, there was no significant difference between the two groups (t=1.13, P=0.268). The total operating time was 63.24±43.22 min in the King closure group versus 47.60±18.13 min in the OTSC group (t=2.20, P=0.030). The closure time of the defect surface was 20.85±16.35 min in the King closure group versus 10.95±5.20 min in the OTSC group (t=2.65, P=0.010). Hospitalization costs were 24 200±800 yuan in the King closure group versus 36 200±2 350 yuan in the OTSC group (t=6.21, P<0.001). Postoperative abdominal elevation radiographs in both groups indicated a small amount of subphrenic free gas, and no intervention was given due to the small amount of gas and no obvious symptoms. No late bleeding, recurrent perforation, infection or other complications occurred after operation, and all patients were discharged successfully. Six months after surgery, 15 patients (27%) in the King closure group developed metal clips or nylon rope residue, which were successfully removed by endoscopy. The anastomosis clamp of nighteen patients (95%) in the OTSC group were in the original position. None of the patient received open surgery. Conclusion OTSC and King closure are both safe and effective in the treatment of full-thickness defect of gastric wall. OTSC has the advantages of short total operation time and short closure time, but with high cost. Abstract:Abstract Background A 5–7 day hospital stay is usually needed after endoscopic submucosal dissection (ESD) of gastric tumor because of the possibility of delayed perforation or bleeding. The aim of this study was to evaluate the efficacy of combined use of a single over-the-scope clip (OTSC) and through-the-scope clips (TTSCs) to achieve complete closure of artificial gastric ulcer after ESD. Methods We prospectively studied 12 patients with early gastric cancer or gastric adenoma. We performed complete closure of post-ESD artificial gastric ulcer using a combination of a single OTSC and TTSCs. Results Mean size of post-ESD artificial ulcer was 54.6 mm. The mean operating time for the closure procedure was 15.2 min., and the success rate was 91.7 % (11/12). Patients who underwent complete closure of post-ESD artificial gastric ulcer could be discharged the day after ESD and the closing procedure. Conclusions Complete closure of post-ESD artificial gastric ulcer using a combination of a single OTSC and TTSCs is useful for shortening the period of hospitalization and reducing treatment cost.

Abstract:Background and study Aims: To prevent complications after colonic endoscopic submucosal dissection (ESD), we developed a new closure technique using repositionable clips. Patients and methods: The closure of post-ESD mucosal defects was attempted in 19 cases. Mucosal defects were linearly closed by holding and dragging the anal mucosal edge towards the oral mucosal edge using repositionable clips. Standard hemoclips were additionally placed to complete the closure. We retrospectively assessed the feasibility of this technique. Results: Defect closure was successfully completed in 18 cases (94.7%). The mean defect size and the procedural time were 40.212.0mm (range, 24-71mm) and 10.7 +/- 7.2min (range, 4.0-29.9min), respectively. The mean number of repositionable clips and standard clips required for closure was 1.6 +/- 0.8 (range, 1-3) and 7.3 +/- 3.7 (range, 3-16), respectively. No adverse events occurred during procedures and thereafter (95% confidence interval, 0-17.6%). Conclusions: The new closure technique for large mucosal defects after colonic ESD using repositionable clips was feasible and appeared effective for preventing subsequent adverse events.
Novel clip method for endoscopic submucosal dissection defect closure reducing submucosal dead space in antithrombotic gastric patients

Abstract:Wound suture after endoscopic submucosal dissection (ESD) is anticipated to be beneficial in the stomach, particularly for patients taking antithrombotic medications, as they face an increased risk of posterior bleeding [1] [2]. However, defect closure in the stomach presents challenges due to its relatively thick mucosa and muscular layer [3]. This was achieved without leaving any submucosal dead space by using a novel clip that features sharp claws at the tip for a strong grasping force ([Fig. 1]). The video shows the procedure ([Video 1]). A man in his 80s with a history of stroke taking 30 mg/day of edoxaban underwent ESD for a 20-mm lesion on the anterior wall of gastric antrum. Edoxaban was discontinued only on the day of the treatment. Before suturing, the visible vessels were cauterized using hemostatic forceps. The clip was used to grasp the muscle layer in the middle of the wound margin and then brought to the contralateral side of the margin ([Fig. 2] a ). Once close enough, the muscle layer was pulled in with suction and then clipped ([Fig. 2] b,c ). The top and bottom of the first clip were clipped in the same manner ([Fig. 2] d,e ). The wound was sutured to ensure that the muscle layer was crimped, preventing any occurrence of submucosal dead space. Additional regular clips were used to close the space between the previously placed clips ([Fig. 2] f ). Following ESD, the patient resumed taking edoxaban on the day after the procedure without experiencing any posterior bleeding or delayed perforation. While there have been several reports of suture methods using clips or endoscopic instruments, these approaches tend to cause submucosal dead space because they draw the mucosal layer [4] [5]. The method minimizes submucosal dead space by crimping the muscular layer instead of pulling the mucosa. This technique is valuable for achieving strong wound closure in the stomach. Abstract:Endoscopic submucosal dissection (ESD) is an effective treatment for early-stage gastrointestinal cancer [1] [2]. However, difficult site lesions (e. g., colon flexion, right-side colon) and large mucosal defects (greater than 5 cm) result in post-ESD defect closures as an ongoing challenge in clinical practice [3]. Therefore, we report a novel method for the efficient closure of large mucosal defects after ESD using nylon loops combined with titanium clips.

Abstract:Objective We sought to investigate the clinical efficacy and safety of a novel endoscopic closure technique in repairing gastric wall defects after endoscopic full-thickness resection (EFTR) of gastric submucosal tumors (SMTs) originating from the muscularis propria layer. Methods From December 2016 to December 2019, patients with gastric submucosal tumors were enrolled and clinicopathological data were collected. All SMTs were resected by EFTR and gastric wall defects were closed using a novel endoscopic closure technique. The feasibility, efficacy, and safety were evaluated. Results A total of 21 patients with gastric SMTs were included in this study. Among the included SMTs, 15 tumors were located in the fundus of the stomach, and 6 were located in the upper body of the stomach. The average size of the lesions was 2.3 cm (range: 1.9-2.5 cm). All patients underwent EFTR and the gastric wall defect was closed by endoscopic closure. The average endoscopic closure time was 9 min (range: 7-15 min) and the average hospitalization stay length was five days (range: 4-6 days). One patient developed abdominal pain on the first day after the procedure and their body temperature increased; he received treatments such as anti-infection, antacid, and gastrointestinal decompression and was cured and discharged after 4 days. No instance of delayed bleeding, postoperative gastrointestinal fistula, or abdominal infection occurred. No case was transferred to surgery. The postoperative pathology profile included 18 stromal tumors and 3 leiomyomas. During the follow-up period (6-24 months), no case of residual or recurrence was recorded. Conclusion The described endoscopic loop ligation technique is feasible, effective, and safe for repairing gastric wall defects after EFTR for gastric submucosal tumors originating from the muscularis propria layer.

Abstract:Endoscopic closure has drawn attention owing to the development and clinical introduction of various endoscopic interventions in the gastrointestinal tract. A mucosal closure is expected to prevent postoperative adverse events, such as bleeding and delayed perforation, in endoscopic intraluminal resection, for example endoscopic mucosal resection (EMR) and endoscopic submucosal dissection (ESD). Endoscopic full-thickness resection always necessitates a robust and sustained defect closure. Furthermore, gastrointestinal fistula requires a closure method that is less invasive than surgical interventions. Intentionally created defects become wider, deeper, and harder to close with only conventional methods using endoscopic clips owing to therapeutic endoscopic expansion. Therefore, dedicated suturing devices/techniques need to be further developed.

Abstract:Background and study aims Mucosal suturing enables reliable and optimal defect closure after endoscopic treatments. We developed and investigated the feasibility of endoscopic hand-suturing (EHS) after gastric endoscopic submucosal dissection (ESD) in porcine in vivo models and in human clinical cases. Patients and methods EHS involving continuous suturing of the mucosal layers using a through-the-scope needle-holder and absorbable barbed suture, was performed after gastric ESD in six live pigs and in eight consecutive patients. Success rates, adverse events, and suture maintenance were subsequently investigated. Results EHS was successfully completed in all six pigs and eight patients (100%) without severe adverse events. However, at 1 week postoperatively the suture closures had not been maintained in all six porcine cases and in the first four clinical cases. In the later four clinical cases, wider and tight suturing of the mucosal edges ensured that the defects remained closed until postoperative week 4. Conclusion EHS is a feasible procedure that may facilitate secure and refined endoscopic surgeries. Mucosal closure in such cases can be maintained using firm suturing.

Abstract:A 58-year-old woman was found to have a submucosal bulging lesion in the anterior wall of the gastric fundus during a screening esophagogastroscopy. Endosonographic evaluation revealed it to be a 3.1×2.5cm, hypoechoic mass originating from muscularis propria (MP). Endoscopic full-thickness resection (EFR) was attempted. After submucosal injection, a four-fifth circumferential mucosal incision was made around the lesion. Submucosal dissection was performed to unroof the overlying mucosa, which was preserved via the remaining one-fifth circumferential mucosal edge. Thus a mucosal flap was created and turned aside to expose the mass. En bloc resection of the lesion resulted in a 3.5*3.0cm full-thickness defect . The mucosal flap was flipped back and the defect was almost fully covered. Therefore, closure of the defect was accomplished by simply clipping the two edges of the initially incised mucosa. The patient was discharged 2 days later without discomfort. Histopathology confirmed a gastrointestinal stromal tumor (GIST), prognostic group 1.
