Abstract:In May 2024, we requested all obstetric institutes (n = 1970) that are members of the Japan Association of Obstetricians and Gynecologists (JAOG) to provide information concerning the collaboration between obstetric institutes and psychiatrists in Japan, such as (1) pregnancy and delivery management of all pregnant women with psychiatric complications, and (2) availability of smooth collaboration with associated psychiatry (or psychiatrists) regarding pregnant women with psychiatric complications. Of the 1970 obstetric institutes, 1483 (75.2%) responded with valid information (completion of data submitted). In Japan, only 21% (308/1483) of all obstetric institutes are able to see all pregnant women with psychiatric complications who attend their hospital, and 32% (480/1483) of the institutes responded that they have smooth cooperation with associated psychiatrists. The response to pregnant women with mental health problems in Japanese obstetric institutes is not adequately equipped yet.

Abstract:Journal of International Medical Research, Volume 52, Issue 5, May 2024. The Postpartum Care Services (PCS) programme in Japan is intended to promote physical recovery and psychological rest for mothers and their children after discharge from the delivery facility, as well as nurture the mothers' own self-care skills and support healthy childrearing for mothers, children and their families. The subsidies for PCS are based on cooperation between psychiatry and obstetrics and between multiple professions, including the local government. The services should also be implemented based on the instruction to medical institutions and the local governments that they should actively screen and approach pregnant women in need of support. This narrative review describes the challenges of expanding the PCS programme nationwide in Japan.

Abstract:Perinatal mood disorders affect both parents, impacting their children negatively. Little is known on the association between parental perinatal mood disorders and pediatric outcomes in Japan considering relevant covariates. Our objective was to investigate the association between paternal and maternal perinatal mood disorders and adverse physical and psychological child outcomes by the age of 36 months, adjusting for covariates such as the child's sex, age of parent at child's birth, perinatal mood disorders of the other parent, and perinatal antidepressant use.

Expanding the international conversation with fathers’ mental health: toward an era of inclusion in perinatal research and practice

Abstract:Paternal mental health is beginning to be recognized as an essential part of perinatal health. Historically, fathers were not recognized as being at risk for perinatal mental illnesses or relevant to maternal and infant health outcomes. The purpose of this paper is to provide an overview of paternal perinatal mental health, leading tools to assess paternal depression and anxiety, the impact of paternal mental health on mother and child health, and future directions for the field. An international team of paternal perinatal mental health experts summarized the key findings of the field. Fathers have an elevated risk of depression and anxiety disorders during the perinatal period that is associated with maternal depression and can impact their ability to support mothers. Paternal mental health is uniquely associated with child mental health and developmental outcomes starting from infancy and continuing through the child lifespan. Tailored screening approaches for paternal mental health are essential to support fathers early in the perinatal period, which would offset health risks for the family. Recommendations on paternal mental health are provided on four key areas to support father perinatal mental health: (1) intervention research, (2) clinical training, (3) national policy, and (4) the inclusion of fathers in the focus of the International Marcé Society for Perinatal Mental Health.

Abstract:In Japan, many fathers consider their spouse to be their children's primary parent while casting themselves in a supporting role. Yet, in the majority of reported child maltreatment cases in Japan, the child's father is recorded as the perpetrator. This may seem somewhat puzzling, given that primary caregivers are recorded as the perpetrator of maltreatment in other cultures. This study qualitatively analyses the parenting experience of 11 Japanese fathers and their reflections on child maltreatment risks. Semi-structured interviews were conducted with fathers of pre-school aged children from middle-class families who had no reported history of child maltreatment. Using qualitative content analysis through a process of condensing, coding and categorising, we arrived at the following theme: fathers aspire to be an active parent, while respecting and supporting their spouses, but anxiety and stress trigger impatience and frustration during parenting. The fathers reported that they are more likely to maltreat their children, especially boys, in situations which triggered anxiety and frustration. Anxiety is particularly heightened when they feared public embarrassment. These findings are discussed with reference to the Japanese social and cultural context, and contrasted with previous research into the parenting experiences of Japanese mothers. The findings indicate that fathers may benefit from tailored support programmes which strengthen their self-efficacy before building resilience for the challenging situations they may encounter as fathers.

The experiences of husbands of primiparas with depressive or anxiety disorders during the perinatal period

Abstract:OBJECTIVES: During the perinatal period, husbands take the key role as essential supporter of wives with mental illness. The aim of this study was to explore the experiences of husbands of primiparas with depressive or anxiety disorders.METHODS: A qualitative descriptive design was used in the study. In-depth interviews were held one to two months after childbirth and results were analyzed using a constant comparative method.RESULTS: We approached ten couples and seven husbands agreed to be interviewed. From interviews, four categories emerged. They are "Husband is committed to decision making by exploring the impact of pregnancy", "The husband's burden depends on his wife's mental status and the relationship between her parents", "The preciousness of baby offsets the new burden," and "Continuous process of trial and error dealing with wife's mental status".CONCLUSIONS: For many years prior to pregnancy, husbands had been alone in trying to help their wives through trial and error. After childbirth they accepted the new child-centered lifestyle and supported their wives' mental health. Healthcare providers are needed to become advisors not only to pregnant women but also to their husbands and build a stable support system with members such as psychiatrists, obstetricians and midwives.

Ten-year trajectories of postpartum depression of Japanese mothers and fathers

Abstract:Background: Perinatal depression is a significant concern affecting both women and men during pregnancy and postpartum periods. While maternal postpartum depression has been extensively studied, paternal depression remains under-researched despite its prevalence and impact on family well-being. This study aimed to estimate the trajectories of perinatal and postpartum depression in Japanese parents over ten years and to determine the details of the symptoms of postpartum depression for each trajectory group, considering reciprocal effects between maternal and paternal depression. Methods: A total of 789 couples used the Edinburgh Postnatal Depression Scale to rate their depressive symptoms prenatally; at 5 weeks, 3 months, 6 months, and 1 year postpartum; and then yearly thereafter until the 10th year. Parallel-process latent class growth analysis was used to group participants according to their longitudinal patterns of depressive symptoms. Results: For both mothers and fathers, four depressive symptom trajectories fit the data best and were most informative (escalating: 6.5 %; mothers low and fathers moderate: 17.2 %; mothers high and fathers low: 17.9 %; low: 58.4 %). A variance analysis showed significant class-parent interactions across anhedonia, anxiety, and depression subscales, indicating distinct patterns of depressive symptomatology. Discussion: Tailored mental health programs and universal screening using the Edinburgh Postnatal Depression Scale are recommended to address the specific needs of each trajectory class. This study contributes to the understanding of long-term depressive symptom trajectories in parents and emphasizes the necessity of comprehensive support strategies to enhance family well-being and resilience.
Postpartum Mental Status in Women with Social Problems at a Japanese perinatal center

Abstract:We examined the association between ‘specified expectant mothers (SEMs; pregnant women with social problems)’ at our institute and their mental status at one month after delivery. We examined 890 pregnant women who delivered at ≥37 weeks of gestation at our institute between January 2019 and October 2019. We excluded mothers with mental disorders requiring medication and whose children were hospitalized in the neonatal intensive care unit. During the study period, there were 148 SEMs with healthy babies (16.6%). The frequency of screening positive on the Japanese version of the Mother-to-Infant Bonding Scale and the Edinburgh Postnatal Depression Scale was higher in SEMs than in non-SEMs (P=0.04 and 0.02). In addition, there was a strong positive correlation between scores on the two scales in SEMs (r2=0.41, P<0.01). The present results indicate the quantitatively clarify of the mental vulnerabilities in SEMs.
The Impact of Authoritative, Authoritarian, and Permissive Parenting Styles on Children’s Later Mental Health in Japan: Focusing on Parent and Child Gender

Abstract:Few studies in Japan have examined whether children’s later mental health is influenced by the way parental authority is exercised, specifically with a focus on gender. Our main purposes of this study were to confirm the three-factor structure of the Japanese version of the Parental Authority Questionnaire (PAQ) and to verify its reliability and validity. We then used structural equation modeling to examine whether children’s later mental health was influenced by parents’ authoritative, permissive, and authoritarian parenting. The subcategories included in the Japanese version of the Clinical Outcomes in Routine Evaluation-Outcome Measure were chosen as mental health indices. A total of 1,320 people in Japan, including company employees, university students, and hospital staff members, were asked to recall and evaluate the parenting they received and to report on their current mental health. Confirmatory factor analyses verified the three-factor structure of the Japanese version of the PAQ, with a reduction in the number of items from the original version. The respondents, regardless of gender, evaluated their mothers as having been more authoritative than their fathers. Concerning the impact of parenting styles on respondents’ later mental health, both maternal and paternal authoritarian parenting styles worsened respondents’ later mental health, including symptomatic problems, risk to self and others, life functioning, and psychological well-being. Both maternal and paternal authoritative parenting had a beneficial impact on respondents’ later mental health. Simultaneous analyses of multi-groups demonstrated that the nature of these influences did not vary with respondent gender. We present the above results in detail, and discuss them from psycho-socio-cultural viewpoints.

Abstract:Background Previous studies have not elucidated the factors affecting marriage and pregnancy/childbirth. This study identifies whether work characteristics and conditions affect marriage and pregnancy/childbirth among women physicians. Methods This nationwide survey was conducted on women members of three major Japanese medical societies: internal medicine, obstetrics and gynecology, and surgery. Results A total of 7747 women participated in this study. Although the mean age at marriage was the same, the age at the first childbirth in all specialties, especially in surgery, was significantly higher than that for the general Japanese population. In addition, being in a junior position, being board‐certified, and working in a workplace other than a university hospital were positively associated with the experience of pregnancy/childbirth, whereas working hours showed no impact. Conclusions We should change the mindset regarding pregnancy/childbirth and build effective strategies in academic institutions to improve the environment for safer pregnancy/childbirth while advancing careers.

Characteristics of stepfamilies and maternal mental health compared with non-stepfamilies in Japan

Abstract:Background: Stepfamilies remain poorly understood in Japanese society, and the support needs of stepfamily mothers are unclear. This study aimed to identify characteristics of stepfamilies and maternal mental health as compared with non-stepfamilies in Japan to utilize as a primary resource for providing effective support through community-based health care for stepfamilies. Methods: From December 2011 to July 2012, we conducted this questionnaire survey with mothers at 3- and 4-month checkups for infants. The response rate was 75.1%. The sample for analysis included responses of 2246 mothers, excluding single mothers. Results: Respondents comprised 47 (2.1%) stepfamilies and 2199 (97.9%) non-stepfamilies. There were significantly higher rates of parents with not more than a high school education and ≥3 children among stepfamilies compared with non-stepfamilies. Stepfamily mothers had significantly higher rates of feeling a lack of economic resources, absence of participation in childbirth education classes, smoking during pregnancy, and unplanned pregnancy. Furthermore, they also had significantly higher rates of depression and a lack of confidence in the parent role. Maternal depression was associated with factors such as maternal age, self-perceived health, stress level, confidence in breastfeeding, confidence in the parent role, and number of children. Conclusions: These findings suggest that stepfamilies exhibit many characteristics related to social disadvantage and problems with community-based health care in Japan. Healthcare providers should be aware of stepfamily mothers' support needs and should put in place a support system for stepfamilies. Moreover, compared with non-stepfamily mothers, stepfamily mothers have a significantly higher prevalence of depression. However, stepfamily composition does not necessarily increase the risk of maternal depression. Therefore, healthcare providers should put in place a system for obtaining more thorough information about stepfamilies and conduct an early assessment to identify their support needs.
Factors associated with new onset of father-to-infant bonding failure from 1 to 6 months postpartum: an adjunct study of the Japan environment and children's study

Abstract:Purpose: This study aimed to determine the factors associated with new onset father-to-infant (paternal) bonding failure from 1 to 6 months postpartum. Methods: This was a prospective birth-cohort study. Paternal bonding failure was evaluated using the Japanese version of the Mother-to-Infant Bonding Scale (MIBS-J) at 1 and 6 months postpartum. For cut-off scores, overall bonding failure, MIBS-J total scores ≥ 5; subscale for lack of affection, MIBS-J_LA scores ≥ 3; and subscale for anger/rejection, MIBS-J_AR scores ≥ 3 were used in this study. Multivariate regression analysis was performed to analyze relative variables. Results: We analyzed 872 fathers. The frequency of new-onset overall bonding failure, lack of affection, and anger/rejection was 5.6%, 4.9%, and 6.3%, respectively. For new-onset overall bonding failure, significant associated factors were paternal childcare leave (adjusted odds ratio [AOR] 3.192; 95% confidence interval [CI] 1.203-8.469), paternal new-onset depression symptoms (AOR 3.181; 95% Cl 1.311-7.716), and maternal new-onset overall bonding failure (AOR 4.595; 95% Cl 1.119-18.866). For new-onset lack of affection, significant associated factors were preterm birth (AOR 4.189; 95% Cl 1.473-11.913) and paternal new-onset depression symptoms (AOR 3.290; 95% Cl 1.294-8.362). For new-onset anger and rejection, significant associated factors were paternal childcare leave (AOR 3.142; 95% Cl 1.138-8.676), paternal new-onset depression symptoms (AOR 2.829; 95% Cl 1.133-7.068), and maternal new-onset anger/rejection (AOR 7.064; 95% Cl 2.300-21.700). Conclusions: The factors associated with new-onset paternal bonding failure from 1 to 6 months postpartum were paternal childcare leave, preterm birth, paternal postpartum depression symptoms, and maternal bonding failure.
Paternal Postpartum Depression and Associated Factors Among Partners of Women Who Gave Birth in Seka Town, Southwest Ethiopia

Abstract:Despite epidemiologic studies demonstrating the increased incidence of paternal postpartum depression, their emotional health is overlooked throughout their partner's pregnancy and postpartum period as postpartum depression has been traditionally construed as a disease of women. Traditional masculinity norms also result in a lack of recognition and barriers to the treatment of depression in men. This study is aimed to determine the prevalence and factors of paternal postpartum depression among fathers whose wives gave birth. A community-based cross-sectional study was conducted from July 7 to 17, 2023. The 288 sample size was estimated using a single population proportion formula and selected by random sampling technique. The data were exported from Kobo Toolbox and analyzed using SPSS version 26. Candidate variables were identified in bivariate at p < .25 for the multivariate analysis. A p < .05 and adjusted odds ratio (AOR) were used to determine the significance. A total of 280 partners participated making a 97.22% response rate. Paternal postpartum depression was 19.6%(95% confidence interval [CI] = [15.4%, 24.3%]). It was significantly associated with history of depression (AOR = 4.4, 95% CI = [1.7, 10.9]), unplanned pregnancy (AOR = 4.7, 95% CI = [1.9, 11.3]), alcohol consumption (AOR: 3.0, 95% CI = [1.3, 7.4]), infant sleeping problem (AOR: 3.0, 95% CI = [1.1, 8.9]), and mode of delivery (AOR: 3.0, 95% CI = [1.3, 7.6]). This study concluded that paternal postpartum depression was high. The researchers recommended the inclusion of men's mental health services like screening into women's postnatal health care.
Current state and future prospects for psychosomatic medicine in Japan

Abstract:In this article, we describe the history and current state of psychosomatic medicine (PSM) in Japan and propose measures that could be considered based on our view of the future prospects of PSM in Japan. The Japanese Society of PSM (JSPM) was established in 1959, and the first Department of Psychosomatic Internal Medicine in Japan was established at Kyushu University In 1963. PSM in Japan has shown a prominent, unique development, with 3,300 members (as of March 2016), comprised of 71.6% of medical doctors including psychosomatic internal medicine (PIM) specialists, general internists, psychiatrists, pediatricians, obstetricians and gynecologists, dentists, dermatologists, and others. Most of the non-physician members include psychology and nursing staff specialists.The Japanese Society of Psychosomatic Internal Medicine (JSPIM), founded in 1996, is another major society with more than 1,200 physicians that is mainly composed of internists. The first joint congress of the five major PSM societies from each field was held in 2009. They included the Japanese Society of Psychosomatic Medicine, Psychosomatic Obstetrics and Gynecology, Psychosomatic Pediatric Medicine, Psychosomatic Dental Medicine, and Psychosomatic Internal Medicine. Several subdivided societies in related medical fields have also been established for cardiovascular, digestive, dermatological, and oriental medicine and for eating disorders, pain, fibromyalgia, stress science, behavioral medicine, and psycho-oncology. JSPM and JSPIM participate in international activities including publishing BioPsychoSocial Medicine (BPSM) and the establishment of a sister society relationship with the Germany College of PSM. PSM in Japan has adopted a variety of professional psychotherapies, including transactional analysis, autogenic therapy, and cognitive behavioral therapy. Mutual interrelationship has been promoted by the Japanese Union of Associations for Psycho-medical Therapy (UPM).Although PSM in Japan is functioning at a high level, there remain areas that could be improved. Among the 81 medical schools in Japan, just eight university hospitals have an independent department of PSM and of 29 dental schools only three dental university hospitals have a department of psychosomatic dentistry.Further accumulation of evidence regarding the mind-body relationship in clinical and basic science that is based on the latest advanced technology is necessary. The psychosomatic medicine community needs to make an even greater contribution to meeting the needs of modern society. The possibilities for the future development of PSM in Japan must be widely discussed.

Abstract:BACKGROUND: Much attention has been paid to the problem of postpartum depression in women. However, there is some indication that men also experience depression after the birth of a child, and that paternal depression is linked to maternal depression.AIMS: The purpose of this integrative review was to examine current knowledge about postpartum depression in fathers. Specific aims were (1) to examine the incidence of paternal depression in the first year after the birth of a child, (2) to identify the characteristics and predictors of paternal postpartum depression, (3) to describe the relationship between maternal and paternal postpartum depression, and (4) to discuss the influence of paternal depression on the family and infant.METHODS: A literature search from 1980 to 2002 was carried out using the CINAHL, PsychInfo, and Medline electronic databases. Twenty research studies were identified that included incidence rates of paternal depression during the first year postpartum. These were further examined and synthesized regarding onset, severity, duration, and predictors of paternal depressive symptoms, and for information about the relationship between maternal and paternal depression.FINDINGS: During the first postpartum year, the incidence of paternal depression ranged from 1.2% to 25.5% in community samples, and from 24% to 50% among men whose partners were experiencing postpartum depression. Maternal depression was identified as the strongest predictor of paternal depression during the postpartum period. The implications of parental depression for family health were discussed.CONCLUSIONS: Postpartum depression in men is a significant problem. The strong correlation of paternal postpartum depression with maternal postpartum depression has important implications for family health and well-being. Consideration of postpartum depression in fathers as well as mothers, and consideration of co-occurrence of depression in couples, is an important next step in research and practice involving childbearing families.

Abstract:Abstract The proportion of women who experience a depressive state after delivery differs between primiparas and multiparas, so it is important to clarify the different factors related to depression between the two groups. In this study, we confirmed the differences in depressive states, the perinatal period, and social support between primiparas and multiparas, and clarified their characteristics. Data were extracted from a prospective cohort questionnaire survey conducted on pregnant women in Japan that included sociodemographic questions, the Edinburgh Postnatal Depression Scale, and the Japanese version of the Social Support Questionnaire. We carried out the chi-square test, Student’s t -test, and analysis of covariance to compare responses between primiparas and multiparas. A total of 1138 primiparas and 380 multiparas provided valid responses. We found that primiparas had higher rates of experiencing maternity blues and postpartum depression than multiparas. We also found that primiparas had higher anxiety scores than multiparas. Primiparas with postpartum depression perceived a lower number of persons available to provide social support than primiparas without postpartum depression. These findings suggest that it is important to provide pregnant women, especially for primiparas, with information that allows them to increase the number of people who can provide them with support.

Abstract:The perinatal period (from conception to 1 year after birth) is one of biological, psychological, and social transition, requiring adjustment, support, and resources. While the mental health of perinatal women is receiving welcome attention, the absence of clinical guidelines means that the mental health of new fathers (and other non-birthing parents) is likely underassessed. Although around 10% of fathers are reported to experience depression during the perinatal period,1 the true prevalence is likely to be higher given the lack of screening and barriers to disclosure. Paternal perinatal depression may present as difficulties including balancing work and home life, perceived financial strain, and relationship difficulties between fathers and their partners.2 While fathers' partners are their main social support,2 poor marital relationship satisfaction and maternal depression are risk factors for paternal depression.3 Men may be less likely than women to seek help for perinatal depression, normalising their symptoms or not recognising it as something affecting fathers.2 Yet, paternal depression is associated with child emotional and behavioural difficulties,4 as well as postnatal depression in mothers,1 and so attention to the mental health of (expectant) fathers and other non-birthing parents is required. Abstract:Japan is not among the countries with a high ratio of beds and admissions into psychiatric hospitals to total population, but the average length of hospitalization in Japan is among the longest in the world. The ie characteristic of Japanese culture has taken care of the elderly and the mentally disturbed even though it regarded them as a nuisance, thus serving to keep the number of beds and admissions in Japan lower than in Western countries. But those treated as a nuisance who sever all face-to-face ties upon being admitted to hospitals tend to be expected culturally to regard the hospitals as their homes, thus prolonging the length of their hospitalization.

Abstract:Recent years have seen increased attention to the needs and support of siblings of children with chronic illness, and reports of intervention studies on siblings are gradually increasing worldwide. In Japan, the basic policy approved by the Cabinet in 2021 of The Basic Law for Child and Maternal Health and Development stipulates promoting support for the siblings of children with chronic illness, medical care, and disabilities. Simultaneously, practical reports are emerging. However, reports on the actual state of sibling support at medical institutions in Japan are limited. This study aimed to describe the actual state of support for siblings of children with illness in Japanese medical institutions using a cross-sectional design. Responses were obtained from 207 of 484 registered training facilities for Board-Certified Pediatricians of the Japan Pediatric Society through anonymous questionnaires investigating the actual state of siblings' support. Descriptive statistics were calculated, and the state of siblings' support was described. Fifty-two participants (25.1%) answered that the entire ward, including two outpatient departments, provided siblings' support, while 37 (17.9%) answered some staff made an effort, and 117 (56.5%) did not. Support mentioned included conversing with siblings, actively speaking to siblings, calling siblings' names, and counseling care through the parents. Of the 45 cases (21.7%) where siblings were invited to events and gatherings, 10 (22.2%) were siblings-centered events. Some cases involved collaboration with local sibling support groups such as non-profit organizations. This study clarified the actual state of siblings' support, and further expansion of this support is required.
