Abstract:Objective To evaluate the efficacy of mini- implant used for maxil ary anterior teeth intrusion to improve gum-my smile of patients. Methods Mini- implants were applied between the upper lateral incisor and the canine on both sides after the maxil ary and mandible dentition were regularly arranged in 20 patients. Then 100g force was applied to intrude the upper anterior teeth for 5- 6 months. Photos, panoramic radiograph and lateral cephalometric X ray were taken before and after the treatment. Vertical changes on maxil ary first molars and maxil ary central incisor and soft tissue changes on upper lip were recorded. Results The facial profile and gummy smile of patients improved significantly after treatment. The clinical crowns of anterior teeth were significantly reduced and the pulp vitality of anterior teeth was normal. No obvious root absorption was found. U1- PP and U1- SN decreased significantly (P<0.01). ULL increased while ULP decreased (P<0.05). No significant increases were observed in U6- PP and U6- SN (P>0.05). Conclusion Gummy smile can be corrected effectively and safely by intruding upper anterior teeth with mini- implant.

The focus is on orthodontic treatment approaches tailored to address moderate to severe gummy smiles. The communication aims to enhance clinicians' understanding of diverse orthodontic strategies applicable to correct gummy smiles stemming from different etiological factors, facilitating more precise and effective treatment outcomes aligned with patient expectations.

Abstract:Skeletal class III malocclusion with severe skeletal disharmonies and arch discrepancies is usually treated via the conventional orthodontic-surgical approach. However, when associated with tooth impaction and periodontal risks, the treatment is more challenging and complex. The esthetic, occlusal, and periodontal stability of the treatment outcome is more difficult to obtain. The 16-year-old female patient in this case was diagnosed with dental and skeletal Class III malocclusion, bilateral impacted maxillary canines, and scalloped thin gingiva. The multidisciplinary management included a segmental arch technique, extracting two premolars, a subepithelial connective tissue graft surgery, and orthognathic surgery. The esthetic facial profile, pleasant smile, appropriate occlusion, and functional treatment results were obtained and maintained in 8-year follow-up.
Gummy Smile Correction by Intruding Upper Incisors with Mini-Screw Implant: an Esthetic Evaluation by the Golden Facial Mask

Abstract:Objective To use a golden facial mask to classify the gummy smile and then to evaluate the effectiveness of correction gummy smile by intruding upper incisors and canines with mini-screw implants. Methods Sixteen female and 1 male gummy smile patients with an age range of 16-40 years, 26. 2 years on average, were involved in the study. The mini-screw implants were inserted buccally between upper lateral incisors and canines on both sides for intruding upper incisors together with fixed appliance. The golden facial mask was introduced to analyze the frontal view of smile pre- and post-treatment. Results Gummy smile of all patients involved in the study was reduced by intruding upper incisors and canines, and the average period for intruding about 9. 8 months. According to the analysis of the golden facial mask, the reasons of gummy smile in different patients were different before treatment. However, the lips, teeth and gingiva were almost coincident with the golden facial mask after treatment. Conclusions Analyzing the relationship among lip, teeth and gingiva by the golden facial mask, the inprovement of gummy smile by using mini-screw implants to intrude the upper incisors can be evaluated.

Abstract:Excessive gingival display (EGD) is one of the most common aesthetic concerns, and its correction often presents a challenge to periodontists. It has a multifactorial etiology, and this article describes a case involving hypermobile upper lip (HUL), altered passive eruption (APE), and vertical maxillary excess (VME). Upon investigation, a positive collum angle and marked subnasal skeletal depression were observed. In this context, it is noted that during a spontaneous smile, the upper lip retracts and gets lodged in this depression. The rehabilitation plan includes included aesthetic crown lengthening via gingivectomy using Chu's proportional gauge for altered passive eruption and filling the subnasal depression by PMMA (polymethylmethacrylate) bone cement. The entire treatment plan was digitalised using cutting edge methods such as computed tomography (CT), cone beam computed tomography (CBCT), and 3D printers for virtual planning of the defect's position, size, and shape. No postoperative complications were reported. After six months, the patient exhibited a harmonious smile with reduced exposed gingiva.
Orthodontic Correction of a Skeletal Class II Malocclusion with Severe Gummy Smile by Total Intrusion of the Maxillary Dentition

Abstract:A 22-year-old woman visited the hospital complaining of lip protrusion, crowded teeth, and a gummy smile. The clinical examination showed a convex profile with a hyperdivergent mandible and a severe gummy smile in both anterior and posterior regions. The unstable mandible position was considered during treatment planning, and a targeted mechanic system was carefully designed. Temporary skeletal anchorage devices in the posterior dental region and a transpalatal arch were introduced as anchorage for the intrusion of the entire maxillary dentition and controlled retraction of the anterior teeth. Based on effective and simple mechanisms, we successfully eliminated the severe gummy smile and improved the facial aesthetics with the aid of conventional appliances. This approach provided an alternative option to orthognathic surgery or bulky invasive miniscrews for treatment of skeletal Class II malocclusion with severe gummy smile.
Dose and injection site of botulinum toxin type A for gummy smile management: A systematic review and bibliometric analysis

Abstract:Introduction: Botulinum toxin type A (BoNT/A) is an effective non-surgical method for treating gummy smile (GS). This systematic review evaluated the efficacy, duration, and safety of different BoNT/A injections. Methods: Four electronic databases were searched for relevant literature, generating 1106 references. Results: The review included 13 prospective, controlled clinical trials. The mean pre-injection anterior gingival exposure ranged from 3.5 mm to 6.8 mm, reaching maximum effect at 2-4 weeks post-injection. Most studies indicated complete improvement in gingival exposure post-injection, with gingival exposure reduced to ≤3 mm. The dosage of BoNT/A was determined by the severity of gingival exposure, with effects lasting up to 12-24 weeks. Levator labii superioris alaeque nasi (LLSAN), levator labii superioris (LLS), and zygomaticus minor (ZMi) were the main targeted muscles. Next, bibliometric analysis was conducted to provide an overview of the existing publications on managing gummy smiles. Conclusions: This data demonstrates that BoNT/A can effectively treat various types of GS triggered by muscle hyperactivity. It is a non-intrusive treatment with significant improvement, high safety, minimal side effects, and high patient satisfaction. This study was preregistered in the Prospective Register of Systematic Reviews (CRD42024509183). Clinical significance: This study systematically reviewed and compared previous results on efficacy, duration, patient satisfaction, and adverse effects of different botulinum toxin type A doses and injection sites, laying a solid foundation for further studies that use BoNT/A in the management of gummy smiles.

Abstract:OBJECTIVESThis study aimed to classify the dynamic smile and to quantify the gingival line (GL), as well as apico-coronal displacement of the gingival zenith (GZ), in the maxillary anterior dentition in a young Chinese population.METHODSTwo-hundred young Chinese subjects (100 men and 100 women; 20-35 years of age) with healthy dentogingival tissue were recruited. The dynamic smile process was captured using a digital camera. The smile type, GL type, the vertical distance of the GZ between the canine and the central incisor on the same side and the GZ of the lateral incisor-GL relationship were measured using a self-developed smile-analysis method. The kappa statistics was used to examine the reliability of the data recorded by the rater. The Pearson chi-square test was used to analyse the differences between subjects regarding the frequencies of smile type and GL type at α = 0.05.RESULTSData revealed that 45.5% of subjects had a high smile and 45.5% had an average smile; 58.2% of the subjects presented an upwards GL. The GZ of canine teeth was 0.33 mm apical to the corresponding central incisor and no significant difference between both sides of the GZ was observed. The GZ of the lateral incisor was located coronal to the GL in 87.9% of samples. The vertical distance between the GZ of the lateral incisor and the GL was 0.59 mm and no statistically significant difference was detected.CONCLUSIONSThe most common findings were a high or average smile type, combined with an upward GL. In the majority of subjects, the GZ of the lateral incisor is coronal to the GL. The apico-coronal displacement of the GZ showed bilateral symmetry.

Abstract:Gingival hyperpigmentation and the condition known as gummy smile are very common dental cosmetic problems. Gingival hyperpigmentation arises due to the excess presence of melanin in certain regions of the gums. In the case of gummy smile, more than the required amount of gingival tissue is exposed upon smiling. An aesthetically pleasing smile should expose only a negligible amount of gingival tissue. Gummy smile and gingival hyperpigmentation can have detrimental effects on the aesthetic quality of a smile, and thereby a wide variety of treatment options must be taken into consideration depending patient outcome objectives. The use of a laser as a treatment modality is considered to be a promising option for such cases. We aim to explain the effects of using a laser on the gingiva and discuss the advantages and disadvantages of this type of treatment and the resulting alteration of the genetic composition of the gingival tissue. This article reviews the histological aspects and biological effects of a laser treatment for oral hyperpigmentation and gummy smile and analyzes the use of the laser as a modality to improve the smiles of people with hyperpigmentation and excessive gingival display. We also attempt to provide insight into the use of plasma as a novel technology for medical and dental research and its future implications with regard to, dental soft tissue procedures.
Application of Botulinum Toxin at the Yonsei Point for the Treatment of Gummy Smile: A Randomized Controlled Trial.

Abstract:BACKGROUND:Demand for less-invasive procedures for treating gummy smile, such as botulinum toxin A injections, has increased substantially over the years. Meanwhile, the optimal injection site for botulinum toxin A injection is debated. The authors aimed to investigate the efficacy of botulinum toxin A injection at the Yonsei point for treating gummy smile.METHODS:In this double-blind, single-site, randomized clinical trial, healthy participants with a gummy smile (anterior gingival exposure of ≥3.0 mm) were enrolled and randomized (1:1 ratio) into two groups. The experimental group was administered 6 U of botulinum toxin A at the Yonsei point (a single-site injection of 3 U to the right Yonsei point and 3 U to the left Yonsei point), and the control group received the same dose in the bilateral levator labii superioris alaeque nasi muscle sites. The patients were assessed at baseline and 4, 12, 24, and 48 weeks after the first injection using a digital vernier caliper.RESULTS:A total of 49 participants were enrolled. Anterior and bilateral posterior gingival exposure were reduced at 4, 12, and 24 weeks ( P ≤ 0.05) and returned to baseline at 48 weeks in both groups; there was no difference between the groups at these time points. The increase in satisfaction among patients was significant, and few adverse events were observed.CONCLUSION:Both the Yonsei point and the levator labii superioris alaeque nasi muscle site can be used as botulinum toxin A injection sites for treating gummy smile.CLINICAL QUESTION/LEVEL OF EVIDENCE:Therapeutic, I.
Nonsurgical treatment of an adult with a skeletal Class II gummy smile using zygomatic temporary anchorage devices and improved superelastic nickel-titanium alloy wires

Abstract:Patients with a severe gummy smile and a skeletal Class II profile are difficult to treat. This case report describes an effective treatment alternative for improving a gummy smile in a patient with a severe Class II molar relationship, severe crowding, and lip protrusion using zygomatic anchorage devices and improved superelastic nickel-titanium wires. A 36-year-old woman had an excessive overjet and a deep overbite with a bilateral Angle Class II molar relationship. The cephalometric analysis demonstrated a Class II skeletal relationship (ANB, 9.5°), retroclination of the mandible (FMA, 38.4°), and severe labial inclination of the mandibular incisors (IMPA, 101.9°). The main treatment objectives included normalizing the overjet and overbite, improving the gummy smile, and establishing a satisfactory occlusion. During treatment with fixed appliances, intrusion of the total maxillary dentition using skeletal anchorage and elimination of the bimaxillary protrusion were achieved. Improvement of the lateral profile and gummy smile enhanced facial esthetics. Intrusion and distalization of the maxillary dentition with skeletal anchorage and improved superelastic nickel-titanium wires provided a satisfactory dental occlusion, esthetic improvement, and adequate function. This approach should be considered as an alternative treatment option to orthognathic surgery for adults with high-angle skeletal Class II malocclusion and a gummy smile.

Abstract:OBJECTIVE To evaluate the effectiveness of mini-screw anchorage on intruding upper incisors in patients with gummy smile. METHODS Sixteen female patients (mean age: 23.8 years) were involved in this study. The mini-screws were placed between upper lateral incisors and canines on both sides for intruding upper incisors together with fixed appliance. The cephalometric analysis was carried out. RESULTS Totally 63 teeth in 16 patients were intruded successfully and the deep overbite was decreased by (2.74 +/- 2.00) mm (P < 0.001). The distance from the edge of the upper incisor to palatal plane was reduced by (5.00 +/- 2.76) mm (P < 0.001), and the incisors were proclined by (9.34 +/- 10.06) degrees (P < 0.01). The duration for intrusion was 10.8 months on average. CONCLUSIONS It was practicable and effective to improve gummy smile by intruding upper incisors with mini-screw implant.
Digital Planning to Enhance Diagnosis and Precision in Correcting Excessive Gingival Display in the Presence of Asymmetrical Maxillary Position: A Case Report

Abstract:This case report addresses the treatment of excessive gingival display (EGD) in the context of maxillary asymmetry, zenith irregularities, and occlusal plane inclination. Digital planning was pivotal in formulating a precise treatment strategy by incorporating facial photographs, digital models, and cone-beam computed tomography data. Parameters, including occlusal plane inclination, teeth position, and lip-to-gingival margin relationships, were considered to ensure treatment alignment with the patient's facial characteristics. Notably, during the planning phase, it was evident that the conventional approach using the cementoenamel junction as the apical limit for incisions would result in asymmetry. Consequently, the gingival margin position was defined in accordance with facial and lip features. The chosen treatment, flapless crown lengthening, was tailored to the patient's thin phenotype and guided by measurements derived from digital planning. Postsurgery, the patient experienced a swift and painless recovery. A harmonious smile with a stable gingival margin position was achieved at the 1-year follow-up, seamlessly complementing the patient's facial attributes. This case underscores the importance of personalized EGD treatment and the value of digital planning in enhancing diagnostic accuracy and precise treatment planning, ultimately facilitating optimal treatment strategies.
The Influence of the Maxillary Posterior Region on Smile Aesthetics in a Chinese Cohort

Abstract:Objectives: The aim of this research was to establish standard norms for posterior smile -related characteristics including posterior smile line (PSL), the most posterior teeth dis-played, buccal corridor ratio (BCR), and buccal corridor symmetry (BCS) and investigate its aesthetic contributions to smile attractiveness in a Chinese population. Materials and methods: From digitally recorded dynamic smile videos of young Chinese par-ticipants, 188 standardised full-smile images were captured and then aesthetically evalu-ated by 22 laypersons using a visual analog scale (VAS). Four smile-related variables in the posterior region were analysed. VAS data were compared between subgroups to test the influence of these variables on smile aesthetics with significance level of P < .05. Results: The VAS scores of participants with high PSL were significantly lower than those with aver-age or low PSL (P < .01), especially when they display average or low anterior smile line (P < .05). Smiles with the second premolar displayed obtained the highest VAS amongst the 3 subgroups (P < .05). No significant differences were found between the VAS scores of BCR and BCS (P > .05). Conclusions: Maxillary posterior gingiva and teeth displayed influence smile attractiveness evaluated by laypersons, which should be given attention when treating patients with compromised aesthetics, especially those displaying average or low anterior smile lines. Abstract:Objective To investigate the smile esthetic elements of Chinese Han-nationality female based on high definition (HD) digital camera with computer-image measuring technique.Methods HD digital camera was used to capture spontaneous smile process combined with computer-image measuring technique to establish the analysis method of smile.Ninety-nine Han-nationality female with normal gingiva,aged from 20 to 30 years were recruited.The lip-incisor edge distance,gingival display were measured.The data was statistically analyzed.Results A standard and objective method to analyze spontaneous smile was established.Forty-three percent(43/99) of the subjects studied were classified as having a high smile and average smile respectively,13％ (13/99) as having a low smile. Gingival display accounted for (1.34 ±0.98) mm in high smile type.Conclusions Average smile and high smile type were most common in the Chinese Han-nationality female studied.
Analysis of Dynamic Smile and Upper Lip Curvature in Young Chinese

Abstract:During smile evaluation and anterior esthetic construction, the anatomic and racial variations should be considered in order to achieve better matching results. The aims of this study were to validate an objective method for recording spontaneous smile process and to categorize the smile and upper lip curvature of Chinese Han-nationality youth. One hundred and eighty-eight Chinese Han-nationality youths (88 males and 100 females) ranged from 20 to 35 years of age were selected. Spontaneous smiles were elicited by watching comical movies and the dynamics of the spontaneous smile were captured continuously with a digital video camera. All subjects' smiles were categorized into three types: commissure, cuspid and gummy smile based on video editing software and final images. Subjects' upper lip curvatures were also measured and divided into three groups: upward, straight and downward. Reliability analysis was conducted to obtain intra-rater reliabilities on twice measurements. The Pearson Chi-square test was used to compare differences for each parameters (α=0.05). In smile classification, 60.6% commissure smile, 33.5% cuspid smile and 5.9% gummy smile were obtained. In upper lip measurement, 26.1% upward, 39.9% straight and 34.0% downward upper lip curvature were determined. The commissure smile group showed statistically significant higher percentage of straight (46.5%) and upward (40.4%) in upper lip curvatures (P<0.05), while cuspid smile group (65.1%) and gummy smile group (72.7%) showed statistically significant higher frequency in downward upper lip curvature (P<0.05). It is evident that differences in upper lip curvature and smile classification exist based on race, when comparing Chinese subjects with those of Caucasian descent, and gender.
Analysis of the Factors Influencing the Subjective Smile Evaluation in Females after Orthodontic Treatment

Abstract:Objective To understand the factors influencing subjective smile evaluation and investigate the consistence between orthodontists and laypersons on smile evaluation.Methods 80 female patients with skeletal Class Ⅰ facial pattern (aged 14 to 27 years old) were included in the study after finishing orthodontic treatment.All subjects presented with Class Ⅰ canine and molar relationship,normal overbite and overjet and well-aligned teeth.The subjects were divided into extraction (42 subjects) and non-extraction groups(38 subjects).Posttreatment smile photographs of lower 1/3 of the face were taken.The smile photos were randomly numbered and distributed with a fixed sequence to 10 orthodontists and 10 laypersons for subjective smile evaluation (VAS).Results Significant difference was noted in smile scores when smile photographs were evaluated by orthodontists and laypersons.(P＜0.001).No significant difference was found in smile scores when smile photos in extraction group were compared with that in non-extraction group.Correlation analysis indicated that smile index was positively correlated with orthodontists and layperson's score,smile height,gingival exposure,canine width ratio,visible dentition width ratio was negatively correlated with their score.Conclusions Extraction or non-extraction decision itself might not significantly affect subjective smile evaluation of a patient.Gingival exposure may play a more important role in subjective evaluation of smile.
Measurement and Analysis of Smile Line of 62 Han-Chinese

Abstract:OBJECTIVE:To analyze smile features in maximum lip-dynamic in Chinese.METHODS:Sixty-two Han-Chinese, travelling in Germany, with a mean age of 28.5 years were enrolled and photographed. Standardized digital photos were made to measure the height of displayed maxillary gingiva, papilla and tooth during an enjoyment smile. Statistical analysis was performed using Mann-Whitney-U Test and non-parametric analysis.RESULTS:The mean height of tooth display for the central incisors was 10 mm, with no significant difference between the gender (P > 0.05). The mean height of gingival display was 1.3 mm and the mean papilla height was 3.4 mm. There was no significant difference between the genders (P > 0.05). 31% (19/62) of the subjects belonged to high smile line type, 50% (31/62) to medium smile line type and 19% (12/62) to low smile line type.CONCLUSIONS:The red esthetics is a paramount factor for Chinese men and women in the esthetic restoration. 81% of the Chinese studied showed various degree of gingival exposure from central incisor to the premolars, which defined the esthetic area for Chinese people. Papilla is a critical parameter for esthetic evaluation and treatment design. The ratio of high smile line in Chinese may be higher than that in Caucasian.
