Abstract:Objective To evaluate the efficacy of mini- implant used for maxil ary anterior teeth intrusion to improve gum-my smile of patients. Methods Mini- implants were applied between the upper lateral incisor and the canine on both sides after the maxil ary and mandible dentition were regularly arranged in 20 patients. Then 100g force was applied to intrude the upper anterior teeth for 5- 6 months. Photos, panoramic radiograph and lateral cephalometric X ray were taken before and after the treatment. Vertical changes on maxil ary first molars and maxil ary central incisor and soft tissue changes on upper lip were recorded. Results The facial profile and gummy smile of patients improved significantly after treatment. The clinical crowns of anterior teeth were significantly reduced and the pulp vitality of anterior teeth was normal. No obvious root absorption was found. U1- PP and U1- SN decreased significantly (P<0.01). ULL increased while ULP decreased (P<0.05). No significant increases were observed in U6- PP and U6- SN (P>0.05). Conclusion Gummy smile can be corrected effectively and safely by intruding upper anterior teeth with mini- implant.

Abstract:Today the dental esthetic rehabilitation is a very hot subject in dental clinics. Gummy Smile Correction by Intruding Upper Incisors with Mini-Screw Implant: an Esthetic Evaluation by the Golden Facial Mask

Abstract:Objective To use a golden facial mask to classify the gummy smile and then to evaluate the effectiveness of correction gummy smile by intruding upper incisors and canines with mini-screw implants. Methods Sixteen female and 1 male gummy smile patients with an age range of 16-40 years, 26. 2 years on average, were involved in the study. The mini-screw implants were inserted buccally between upper lateral incisors and canines on both sides for intruding upper incisors together with fixed appliance. The golden facial mask was introduced to analyze the frontal view of smile pre- and post-treatment. Results Gummy smile of all patients involved in the study was reduced by intruding upper incisors and canines, and the average period for intruding about 9. 8 months. According to the analysis of the golden facial mask, the reasons of gummy smile in different patients were different before treatment. However, the lips, teeth and gingiva were almost coincident with the golden facial mask after treatment. Conclusions Analyzing the relationship among lip, teeth and gingiva by the golden facial mask, the inprovement of gummy smile by using mini-screw implants to intrude the upper incisors can be evaluated.
Efficacy of digital guide-assisted implant restoration in anterior teeth aesthetics and its impact on labial bone mass

Abstract:Objective: To analyze the efficacy of digital guide-assisted implant restoration technique in enhancing the anterior teeth aesthetics and its impact on labial bone mass. Methods: We retrospectively analyzed clinical data from 90 patients who underwent maxillary anterior teeth implant restoration at The First People's Hospital of Fuyang, Hangzhou, from January 2021 to September 2023. The patients were divided into two groups: a conventional group (n=45, 45 implants, standard implant restoration) and a digital group (n=45, 45 implants, digital guide-assisted implant). We compared implant positional deviations, changes in dental plaque index (PLI), aesthetic effect scores, labial bone mass differences, and the occurrence of adverse reactions post-treatment between the two groups. Results: The digital group exhibited significantly less deviation in root position in the buccolingual and vertical directions, less neck deviation in the buccolingual and vertical directions, and less apical deviation than the conventional group (P=0.021, P=0.005, P=0.016, P=0.008, P=0.026, respectively). Three months postoperatively, the digital group demonstrated a significantly lower mean PLI (P<0.001), higher white and pink aesthetic scores (P=0.021, P=0.005), and increased alveolar ridge height and coronal and middle labial bone mass (P=0.006, P=0.015, P=0.008). Additionally, this group experienced lower incidence of adverse reactions (4.44% vs. 17.78%) compared with the conventional group (P=0.044). Conclusion: The digital guide-assisted implant restoration significantly enhances implant accuracy, reduces bone resorption, improves aesthetic outcomes, and ensures higher safety.
Aesthetic and Functional Rehabilitation for Dentogingival Asymmetry Using Zirconia Restorations

Abstract:In an effort to improve dentogingival aesthetics, scientific research has been developing non-surgical techniques and dental restorative materials. A 45-year-old female reported to the hospital with a complaint of an unpleasant aesthetic in the anterior region of the maxilla. She has an irregular contour of the gingiva in her anterior region and no temporomandibular problems. There is a history of trauma, but she did not take any treatment for it, which caused discolouration. Plaque index was retained by the surface roughness. Due to this reason, the patient's appearance, self-esteem, and quality of life have decreased. The purpose of this case is to provide a system-based clinical example of oral rehabilitation for anterior teeth. The patient didn't like the way she looked, and hence aesthetic and functional rehabilitation was planned. Treating the dentogingival asymmetry was the first step in the therapeutic approach. An in-ceram zirconia permanent porcelain metal-free crown was positioned after the temporary crowns were cemented into place. It should be mentioned that in clinical situations, the integration of periodontal and prosthetic treatment is crucial. Additionally, it is stated that the in-ceram zirconia system can produce satisfactory results when applied appropriately. For the main anterior teeth, zirconia crowns have offered an alternate treatment option that addresses aesthetic concerns and facilitates the installation of extra-coronal restorations. This article describes a case involving the aesthetic and functional restoration of severely damaged maxillary incisors using zirconia crowns.

Abstract:Dental trauma mostly damages the maxillary anterior teeth, affecting the individual’s esthetics, functionality, and psychological well-being. Restorative dentistry treatment is required to obtain a natural appearance based on the smile design approach. The aim of this report is to characterize the procedure of complicated cosmetic restoration of the six maxillary anterior teeth, which results in straighter and more beautiful natural-looking smiles. A 32-year-old male patient presented with multiple crown fractures in the anterior maxillary teeth, resulting in an unaesthetics smile. The patient also had a deep bite. A comprehensive aesthetics dental treatment was carried out. The dental treatment plan had to be carefully executed by considering several factors such as dental convexities and contours, re‐establishment of function, and esthetics. Smile design should always be approached as a multi-factor decision-making, which enables the doctor to treat patients using a personalized and interdisciplinary approach.
CAD/CAM techniques help in the rebuilding of ideal marginal gingiva contours of anterior maxillary teeth: A case report

Abstract:Background and overview: "Pink esthetics," which are considered to be as important as "white esthetics," have attracted increasing attention. To date, clinicians rarely have applied computer-aided design and computer-aided manufacturing (CAD/CAM) techniques in the rebuilding of the contour of the marginal gingiva in the esthetic zone. Case description: In this case report, the authors describe a female patient who had gingival inflammation and an asymmetrical contour of the marginal gingiva of the anterior maxillary teeth because previously placed ceramic crowns violated the biological width. The authors used a 3-dimensional-printing surgery template to guide precise crown-lengthening surgery to expose subgingival shoulders and to obtain an ideal marginal gingival contour. Then the authors used interim CAD/CAM crowns to induce the growth of the interdental papilla by 0.5 to 1.5 millimeters. Finally, the patient had a symmetrical and well-balanced contour of the marginal gingiva. In addition, the authors reduced the patient's "black triangle" areas to the greatest possible extent. Conclusions and practical implications: This case report illustrates that CAD/CAM products, including 3-dimensional-printing surgery templates and CAD/CAM interim crowns, are helpful in shaping and rebuilding the ideal contour of the marginal gingiva in the esthetic zone, such as the anterior maxillary teeth.
Clinical Evaluation on Aesthetic Outcome of Anterior Tooth Restoration Using Crown Lengthening Surgery

Abstract:OBJECTIVE:To evaluate methods and clinical performance of crown lengthening surgery for aesthetic restoration improvement of short anterior teeth. Various surgery methods, proper restoration time after surgery, and indications for crown lengthening surgery were discussed.METHODS:Crown lengthening surgery and prosthetic treatment were applied to 148 short anterior teeth of 22 cases. Modified Ryge criteria were used to assess the gingival health after treatment. Restoration results before and after treatment were evaluated by use of Kay's altered dental esthetics classification.RESULTS:When patients were re-examined after restoration of two weeks, obvious inflammation was found in 7.4% teeth. After providing the treatment and instruction on oral hygiene, 98.6% gingival health was significantly improved during the follow-up period. Two cases were examined during more than 4-year period and no obvious gingival inflammation and change in dental crown ratio were found. After treatment, all of 22 cases met Kay dental esthetics classification Class II-I (cosmetic) or Class II-I criteria.CONCLUSIONS:Right selection of indications, accurate restoration design, delicate surgery operation, enough recover period for the gingival after surgery, and proper oral hygiene training method, all contribute to the satisfactory aesthetic restoration of anterior teeth.
Orthodontic Correction of a Skeletal Class II Malocclusion with Severe Gummy Smile by Total Intrusion of the Maxillary Dentition

Abstract:A 22-year-old woman visited the hospital complaining of lip protrusion, crowded teeth, and a gummy smile. The clinical examination showed a convex profile with a hyperdivergent mandible and a severe gummy smile in both anterior and posterior regions. The unstable mandible position was considered during treatment planning, and a targeted mechanic system was carefully designed. Temporary skeletal anchorage devices in the posterior dental region and a transpalatal arch were introduced as anchorage for the intrusion of the entire maxillary dentition and controlled retraction of the anterior teeth. Based on effective and simple mechanisms, we successfully eliminated the severe gummy smile and improved the facial aesthetics with the aid of conventional appliances. This approach provided an alternative option to orthognathic surgery or bulky invasive miniscrews for treatment of skeletal Class II malocclusion with severe gummy smile.

Abstract:The new method for esthetic rehabilitation of complicated anterior teeth esthetic defects accompanied by soft tissue morphology, including patient-involved digital esthetic analysis, design, esthetic outcome prediction, 3D printing surgical guide for periodontic surgery and digital fabrication is a practical technology. This method is useful for improvement of clinical communication efficiency between doctor-patient, doctor-technician and doctors from different departments, and is conducive to multidisciplinary treatment of this complicated anterior teeth esthetic problem.
The Influence of the Maxillary Posterior Region on Smile Aesthetics in a Chinese Cohort

Abstract:Objectives: The aim of this research was to establish standard norms for posterior smile -related characteristics including posterior smile line (PSL), the most posterior teeth dis-played, buccal corridor ratio (BCR), and buccal corridor symmetry (BCS) and investigate its aesthetic contributions to smile attractiveness in a Chinese population. Materials and methods: From digitally recorded dynamic smile videos of young Chinese par-ticipants, 188 standardised full-smile images were captured and then aesthetically evalu-ated by 22 laypersons using a visual analog scale (VAS). Four smile-related variables in the posterior region were analysed. VAS data were compared between subgroups to test the influence of these variables on smile aesthetics with significance level of P < .05. Results: The VAS scores of participants with high PSL were significantly lower than those with aver-age or low PSL (P < .01), especially when they display average or low anterior smile line (P < .05). Smiles with the second premolar displayed obtained the highest VAS amongst the 3 subgroups (P < .05). No significant differences were found between the VAS scores of BCR and BCS (P > .05). Conclusions: Maxillary posterior gingiva and teeth displayed influence smile attractiveness evaluated by laypersons, which should be given attention when treating patients with compromised aesthetics, especially those displaying average or low anterior smile lines. Abstract:Implant has been redefined,influenced by contemporary patients' increasing expectations for improved comfort and esthetic outcomes.Patients with continuous tooth loss in maxillary aesthetic zone because of severe periodontitis often associated with hard and soft tissue defects,increasing the risk of aesthetics and the operation difficulty.Techniques are needed to reconstruct hard and soft tissue around the implant to create favorable conditions for reconstruction function,and reflecting appearance.This article illustrates a novel approach of reconstruct hard and soft tissue around maxillary implants using guided bone regeneration (GBR),free gingival graft (FGG)and rolling flap surgery,which achieve a good aesthetic effect.This issue provides an experience of treating patients with continuous front teeth missing due to severe periodontitis by using a combination of surgical procedures in clinic.

Abstract:OBJECTIVE:To explore a new method of patient-involved digital design, esthetic outcome prediction and fabrication for the esthetic rehabilitation of anterior teeth, and to provide an alternative choice for the restoration of anterior teeth.METHODS:In this study, 32 patients with esthetic problems in their anterior teeth were included and divided into two groups randomly: the experimental group (16 patients) and control group (16 patients). In the experimental group, the dentition and facial images were obtained by intra-oral scanning and Three-dimensional (3D) facial scanning and then calibrated. The design of the rehabilitation and the esthetic outcome prediction were created by computer-aided design (CAD) software. After morphologic modification according to the patients' opinions, prostheses were fabricated according to the final design by computer-aided manufacturing (CAM) equipment. As for the control group, the regular design method was applied to restore their anterior teeth. The time consuming in the first insertion of each restoration in both groups was recorded. The quality of the prostheses was assessed by another prosthedontist. The satisfaction to prostheses and the facial appearance were evaluated by the patients.RESULTS:The process of the patient-involved digital design and outcome anticipation was successfully established. The patients were satisfied with the esthetic effects of the anterior restoration made by the digital technique. The acceptance rate of the patients on the digital rehabilitation in the experimental group was 100%. There was no significant difference of the quality of the prostheses between the two groups. The satisfaction rate of the patients on prostheses and facial appearance was significantly higher in the experimental group than in the control group (P < 0.05). In addition, the time consuming in the first insertion of the experimental group was much shorter than that in the control group (P < 0.01).CONCLUSION:The new method of the patient-involved digital design, esthetic outcome prediction and fabrication for the esthetic rehabilitation of anterior teeth is a practical technique. This method is useful in shortening the time consuming of the restoration of anterior teeth and improving the patient satisfaction with the esthetic outcome.
[Improvement of the mastication function of orthodontics combined with restoration in the treatment of deep overbite with severe lower anterior teeth attrition]

Abstract:Purpose: To observe the effect of orthodontics combined with restoration on masticatory function in deep overbite patients with severe lower anterior teeth attrition. Methods: From January 2018 to January 2022, a total of 164 deep overbite patients with severe lower anterior teeth attrition were collected and divided into two groups according to different treatment plans: control group(72 patients, with restoration treatment) and experimental group(92 patients, with orthodontics combined with restoration treatment). The chewing efficiency of the two groups was evaluated, temporomandibular joint dysfunction index (DI), muscle palpation index (PI) and cranio-mandibular index (CMI) were calculated. The satisfaction with facial esthetic, the Chinese version of Oral Health Impact Scale-14(OHIP-14) and the repair satisfaction score were evaluated, the occurrence of adverse events between the two groups was compared. SPSS 23.0 software package was used for statistical analysis. Results: After treatment, the chewing efficiency of the experimental group was significantly improved compared to the control group, while the DI, PI, and CMI were significantly reduced compared to the control group(P＜0.05). Compared with the control group, the satisfaction degree with facial esthetic and restoration in the experimental group was significantly higher, while the OHIP-14 score was significantly lower after treatment(P＜0.05). The incidence of adverse events in the experimental group was significantly decreased compared with the control group (6.52% vs 25.00%, P＜0.05). Conclusions: Combination of orthodontics and restoration treatment can enhance the effectiveness of restoration treatment for deep overbite with severe lower anterior teeth attrition, improve the mastication function and temporomandibular joint balance,satisfaction and quality of life of patients, as well as reduce the risk of adverse events.
[Evaluation of red and white aesthetic effects of porcelain veneers in the restoration of developmental anterior dental gaps]

Abstract:Purpose: To evaluate the red and white aesthetic effect of porcelain veneer in the restoration of developmental anterior interdental spaces. Methods: A total of 152 anterior teeth in 64 patients with developmental anterior dental gaps were restored using porcelain veneers, the aesthetic effects before and after restoration were evaluated by pink aesthetic index (PES) and white aesthetic index(WES), the aesthetic effect of gingival papilla filling and reconstruction was evaluated by interdental gingival papilla index (PIS), and visual analogue score (VAS) was used to compare the satisfaction of patients before and after restoration. SPSS 203.0 software package was used for statistical analysis. Results: The overall mean scores of PES before and after restoration of 152 developmental anterior interdental teeth were 9.63±2.23 and 13.64±0.88, respectively. The average scores of WES before and after restoration were 6.85±1.87 and 9.81±0.58, respectively. There were significant differences of PES and WES scores before and after restoration(P＜0.01). According to the requirements of "near" perfect restoration(PES≥13 points, WES≥9 points), the red and white aesthetic effect after restoration was near the standard. The scores of PIS before and after restoration were 1.86±0.67 and 2.97±0.18, the interdental gingival papilla was completely filled with space, and the shape was ideal, there were significant statistical differences before and after restoration(P＜0.01). Patients were more satisfied with the smile curve and morphology of the restored anterior teeth than other parameters. Conclusions: The aesthetic effect of using porcelain veneers to repair developmental anterior interdental gaps is ideal, among which the filling of the papillae between the teeth can meet the expectations of patients, and the aesthetic effect evaluation of PIS and PES/WES applied to porcelain veneers to repair developmental anterior interdental spaces has practical guiding significance in clinical practice.

Abstract:OBJECTIVES: To investigate the influence of a single infrapositioned ankylosed tooth or implant supported restoration, in smile attractiveness.MATERIAL AND METHODS: A series of 48 digital modified images, simulating a varying degree of infraposition(from 0.25 to 2.0 mm, with a step of 0.25 mm), was created for each one of the maxillary anterior tooth, by altering the full-portrait image of a smiling male adult, in medium and high smile lines. For the model with the high smile line, also a series of 24 digital modified images simulating infraposition of a single anterior tooth with a restored incisal edge was created. Smile attractiveness for each one of the images was evaluated from 160 participants (80 dentists and 80 laypersons), implementing a visual analog scale.RESULTS: For the images with the high smile line, an infraposition ≥ 0.25mm in central incisor's region and ≥ 0.5mm in the region of the lateral incisor or the canine, had a negative effect on the perceived smile attractiveness both for the dentists and the laypersons. Regarding the medium smile line, an infraposition ≥ 0.5mm in central and lateral incisor's region, had a negative effect on the perceived smile attractiveness for both groups of observers. In the area of the canine, infraposition ≥ 0.5mm for the dentists and ≥ 0.75mm for the laypersons, had a negative impact on the smile attractiveness.CONCLUSIONS: Even a minor infraposition of a single maxillary anterior ankylosed tooth or implant supported restoration, can reduce the perceived attractiveness of the face. Infraposition in canine's area can be better tolerated in a medium, compared to a high smile line. In patients with a high smile line prosthetic intervention in order to restore the incisal edge of an infrapositioned tooth, without to harmonize the gingival contour, can be beneficial for the lateral incisor, ineffective for the central incisor and unfavorable for the canine.

Abstract:Orthodontics is a discipline that is based on aesthetics. It coordinates the relationship among nose, lip, and chin through the movement of teeth and the adjustment of jaw relationship. Orthodontists need to fully analyze the patient's soft tissue profile before treatment. They also need to analyze the existing problems and those that may be solved or caused during orthodontics to guide the design of orthodontic treatment and effectively improve the clinical effect of orthodontics. This article reviews the aesthetic preconditions that guide the design of orthodontic treatment from the perspective of orthodontists.
