The Unspoken Factors of Physician Mental Health: A Qualitative Survey Amplifying Voices of Physicians and Physicians-in-Training

Abstract:Introduction Mental health includes our emotional, psychological, and social well-being, and it affects how we act, think, and feel. This state enables people to cope with the stressors of life, realize one's abilities, learn well, work well, and be a contributor to our community. When your mental health is no longer working at its optimal level these functions are disrupted. In the United States today, mental health illness affects 1 in 4 adults, which is about 26% of adults, 46% of teenagers, and 13% of children every year. Despite these statistics, about 50% of these individuals will not seek help due to the stigmas and discrimination surrounding mental health1. This stigma is heightened when it comes to struggling with these issues as professionals. Although recently education on the effects of mental health and the availability of services is being increased in professional settings, the stigma and fear remains2-3. In this study, we conducted a survey of upcoming and current physicians to determine the factors that contribute to poor mental health of these physicians and the coping mechanisms that they employ instead of receiving professional help. Methods An anonymous electronic survey was created via SurveyMonkey to measure mental health perceptions among physicians and physicians-in-training. Survey was created by the Medical College of Georgia's Student Educational Enrichment Program (SEEP) Advanced Scholars in the summer of 2023. Survey questions were adapted from domestic and international surveys regarding the health and wellbeing of physicians and physicians-in-training which analyzed if individuals within the field of medicine were supplied with adequate resources for their mental health, felt comfortable in disclosing their mental health status to colleagues or a supervisor, or felt comfortable seeking help for their mental health4-7. SEEP Advanced Scholars and Instructor distributed survey to their networks via email, individual texts and group platforms specific to pre-medical and medical students, residents, attendings and retired physicians. Survey responses were collected for one week before being closed for analyzation. Two open-ended questions: 1) factors that contribute to poor mental health and 2) utilization of coping strategies were analyzed. Factors were separated and scored based on the themes of a) Internal vs b) External Stressors. Coping strategies were scored by prevalence of use among respondents. Results 171 participants (63.16% woman; 36.26% male; 0.58% nonbinary/genderqueer) completed the survey. Majority of respondents identified as Black or African American (53.22%) and lived in the South Atlantic region of the USA (80.70%), with the least number of reported respondents from American Indian or Alaskan Native heritage (0.00%) and those living in the Mountain region of the USA (0.59%). Average age of respondents was 28.5 years (SD = 39.00) with the majority ranging from 20 to 30 years of age (62.75%).As shown in Figure 2, external factors (69.76%) caused more stress in respondents than internal factors (30.24%). Workload was the number one external stressor while lack of sleep was the number one internal stressor for respondents. As shown in Figure 4, familial and platonic relationships play a major role in coping across career stages for physicians and physicians-in-training. Healthy coping strategies, identified as not taking a toll on mental or physical health, were utilized more often than unhealthy coping strategies such as internet use, sleeping, eating, avoidance, and isolation. Conclusions Given that external factors related to workload caused the most stress for physicians and physicians-in-training, structural changes at the university and medical school level and clinical settings are necessary to mitigate the potential negative effects of stress on the body and mind. Promotion of positive coping strategies should be implemented at all career stages. Increasing normalization of conversations regarding physician mental health is recommended from the earliest stages. Medical instructors and clinical supervisors should undergo training in supporting students and employees with their mental health8. Limitations of this research included limited timeframe to participate in study and the inability to determine at what stage in a person's medical career that a person had the most struggle with mental health. Additionally, the majority of the population from this survey identified as being Black or African American, residing in the Southeast region of the United States. Thus, the results cannot be generalized to a wider range of individuals.
