Stressing the journey: using life stories to study medical student wellbeing

Abstract:While previous studies have considered medical student burnout and resilience at discrete points in students' training, few studies examine how stressors and resilience-building factors can emerge before, and during, medical school. Our study focuses on students' life stories to comprehensively identify factors contributing to student wellbeing. We performed a secondary analysis of life-story interviews with graduating fourth year medical students. These interviews were originally conducted in 2012 as part of the Project on the Good Physician, and then re-analyzed, focusing on student wellbeing. Respondents were encouraged to identify turning points in their life stories. De-identified transcripts were then coded using a consensus-based iterative process. 17 of 21 respondents reported feeling burned out at least once during medical school. Students identified three major stressors: negative role models, difficult rotations, and the United States Medical Licensing Examination Step 1. Two "motivational stressors"-financial concerns and personal life events-emerged as sources of stress that also motivated students to persevere. Finally, students identified four factors-positive role models, support networks, faith and spirituality, and passion-that helped them reframe stressors, making the struggle seem more worthwhile. These findings suggest that a life-story approach can add granularity to current understandings of medical student wellbeing. Initiatives to reduce stress and burnout should extend beyond the immediate medical school context and consider how past challenges might become future sources of resilience. This study also provides an example of secondary analysis of qualitative data, an approach which could be useful to future research in medical education.

Abstract:This study addresses medical student burnout, the physical and emotional exhaustion experienced due to chronic stress, which is prevalent among medical students. The relationships between spirituality and personal well-being were assessed across the 4 years of a medical school in southeastern USA using the Maslach Burnout Inventory General Survey, World Health Organization Quality of Life-Spiritual, Religious, and Personal Beliefs Survey, and the narrative HOPE Spiritual Assessment Tool. The instruments measured exhaustion, cynicism, and professional efficacy pertaining to medical students’ work environment as well as levels of spirituality extracted from facets such as personal practices, sources of hope and strength, and meaning and connection in the world. A significant inverse relationship was found between spirituality and exhaustion as well as spirituality and cynicism. Conversely, spirituality shared a significantly positive relationship with professional efficacy. A significantly higher level of exhaustion and cynicism and lower reported professional efficacy were found in M2 students compared to M4 students. Important sources of student support identified in thematic analysis of narrative were family, friends, a connection to God or a higher power, being part of a religious or spiritual community, prayer/meditation, music, and time spent outdoors. The data suggest that spirituality may be a protective coping tool against burnout in medical students; students nearing the end of pre-clerkship training were in the most need of coping resources. The integration of spirituality into medical school wellness programs may prove beneficial to students specifically before the stressful second year curriculum and can serve students well into their medical profession.

Abstract:Exosomes are nano-sized biovesicles of endocytic origin physiologically released by nearly all cell types into surrounding body fluids. They carry cell-specific cargos of protein, lipids, and genetic materials and can be selectively taken up by neighboring or distant cells. Since the intrinsic properties of exosomes are strictly influenced by the state of the parental cell and by the cellular microenvironment, the analysis of exosome origin and content, and their cell-targeting specificity, make them attractive as possible diagnostic and prognostic biomarkers. While the possible role of exosomes as messengers and a regenerative tool in cardiovascular diseases (CVDs) is actively investigated, the evidence about their usefulness as biomarkers is still limited and incomplete. Further complications are due to the lack of consensus regarding the most appropriate approach for exosome isolation and characterization, both important issues for their effective clinical translation. As a consequence, in this review, we will discuss the few information currently accessible about the diagnostic/prognostic potential of exosomes in CVDs and on the methodologies available for exosome isolation, analysis, and characterization.
Assessing Physiological and Psychological Factors Contributing to Stress among Medical Students: Implications for Health

Abstract:Background: Physiological responses to stress disturb internal homeostasis, leading to serious health consequences. Medical students experience high stress levels that should be managed promptly to prevent stress-related impacts on students' health and education. Aim: This study aims to identify the relationship between stress factors, general health, and academic performance. Methods: This study recruited 421 medical students of all academic years. Participants completed an online survey assessing stress levels using a validated 10-item Perceived Stress Scale. Stress was also compared with students' health and academic performance. Results: We found that 93.6% of our sample experienced moderate to severe stress, and 31% reported increased stress due to the coronavirus disease (COVID-19) pandemic. Except for internship students, stress significantly decreases as students progress each academic year (p < 0.05). Students with higher GPAs and with comorbidities are more stressed. Comorbidities were primarily reported in students in their final years of education with a 4% lower GPA than healthy students. Although we had three stress-related themes (general, academic, and pandemic), students' perceptions of stress factors were primarily academically related. Conclusions: Students experience high stress levels in their final educational years, which might increase the risk of health issues and low academic performance. It is essential to innovate stress-coping strategies specially designed for medical students and mandatorily provided by all medical colleges and to educate students on the effects of stress on their health.

Abstract:Background: Understanding medical students' mental health using subjective well-being indicators is important because it will allow for the creation of positive psychology-based intervention techniques as well as the implementation of main and secondary psychiatric disorder prevention procedures for medical students. These indicators may help to improve the overall quality of health care provided by these future professionals to the general public. Aims: The aim of the present study was to assess the levels of stress and subjective well being as well as factors related to it and their link to the academic performance amongst medical students of a tertiary care setting. Materials and Methods: This is a 3-month cross-sectional survey that took place amongst medical students at a tertiary care medical college in Salem, India, between January and March 2022. Students from all academic years, from first year to internship, were included in this study. The study included all students who were willing to participate and gave their informed consent. Results: Of the total 492 respondents, 288 (58.5%) were females and 204 (41.5%) were males. 29.7% of the study participants belonged to the first year of the MBBS course. 43.9% of the study participants possess a very good academic performance and majority (86.6%) of the students have regular attendance towards classes. Conclusion: Taken together, the findings of studies like these could inform preventive interventions aimed at reducing medical students' distress and promoting future doctors' well-being.

Abstract:BACKGROUND: Burnout is prevalent in doctors and can impact on job dissatisfaction and patient care. In medical students, burnout is associated with poorer self-rated health; however, it is unclear what factors influence its development. This study investigated whether health behaviours predict burnout in medical students.METHODS: Medical students (n=356) at the Universities of St Andrews and Manchester completed an online questionnaire assessing: emotional exhaustion (EE), depersonalisation (DP), personal accomplishment (PA), alcohol use, physical activity, diet, and smoking.RESULTS: Approximately 55% (54.8%) of students reported high levels of EE, 34% reported high levels of DP, and 46.6% reported low levels of PA. Linear regression analysis revealed that year of study, physical activity, and smoking status significantly predicted EE whilst gender, year of study, and institution significantly predicted DP. PA was significantly predicted by alcohol binge score, year of study, gender, and physical activity.CONCLUSIONS: Burnout is present in undergraduate medical students in the United Kingdom, and health behaviours, particularly physical activity, predict components of burnout. Gender, year of study, and institution also appear to influence the prevalence of burnout. Encouraging medical students to make healthier lifestyle choices early in their medical training may reduce the likelihood of the development of burnout.

Abstract:Over the next 10 years (1994-2004), NASA plans to launch several satellites for studying Earth's ecosystems. This Earth Observing System is expected to provide about a terabyte of data per day. The authors have worked with Earth scientists at NASA Ames to encode the selection and preparation of this data as an AI-based planning task. They extended a domain independent planner called Collage, to meet the requirements of this domain, and are building a prototype application that selects and prepares data for ecosystem models. Collage is a successor to the Gemplan planner. Both systems were designed for large, complex planning domains and have been applied to standard toy problems as well as more realistic domains, such as building-construction planning. Two features distinguish Collage from other domain-independent planners. First, instead of state-based planning algorithms, Collage uses a broad repertoire of domain independent, action-based plan-construction algorithms. It can also be extended to include new planning methods, including domain-specific ones. Second, Collage uses localized search: using information about domain structure, the planner partitions a potentially intractable, global search space into smaller, more manageable ones, each working on a local subproblem of the overall task.<<ETX>>

Abstract:BACKGROUND: Medical student exposure to stressors is associated with depression, burnout, somatic distress, decreases in empathy, serious thoughts about dropping out of medical school, suicidal ideation, and poor academic performance. Despite this, there have been no recent, multicenter, qualitative studies assessing medical students' perceptions of their greatest stressor(s).OBJECTIVE: The goal of this study was to identify the most significant stressors noted by medical students themselves, in order to inform the development of programs and policies to reduce medical student distress.DESIGN: Medical students from the nine schools in the state of Florida were invited to complete an anonymous online questionnaire assessing wellness and distress. Students were notified that all responses were voluntary and that individual responses would not be linked to themselves or their program. This paper focuses on students' responses to fixed-response items regarding their experience of stress and open-ended responses to the following question: 'What do you consider to be the greatest stressor(s) facing medical students?' Qualitative data were analyzed using the Grounded Theory method of data analysis.RESULTS: Results confirmed the impact of several stressors highlighted in previous studies (e.g., excessive workload, difficulties with studying and time management, conflicts in work-life balance and relationships, medical school peer relations, health concerns, and financial stressors). However, students also reported unique system-level concerns that have not consistently been highlighted in past research (e.g., medical school administrative failures, concerns about lack of assistance with career planning, and assessment-related performance pressure.CONCLUSIONS: Though individually focused interventions have demonstrated some success, medical students self-report stressors that may be better addressed through system-level changes.

Burnout and stress amongst interns in Irish hospitals: contributing factors and potential solutions

Abstract:BackgroundThe transition from medical school to internship can be daunting for newly qualified doctors. High rates of stress and burnout have been reported, with negative impacts on patient care and physician wellbeing.AimsWe surveyed interns in our hospital group to evaluate rates of stress and burnout, as well as identify the causative factors and propose potential solutions to these.MethodsA hundred and one interns working in four different hospitals over a 2-year period were invited to participate in an anonymous survey. The survey collected basic demographic details and surveyed aspects of mental health using the burnout scale, Maslach Burnout Inventory (MBI) and the stress scale and 12-item General Health Questionnaire (GHQ-12). Interns were also asked to rate a variety of workplace factors on a Likert scale based on the degree of stress caused. Finally, they were surveyed on their awareness of support services available to them.ResultsOur results showed that 37% of interns met the criteria for psychological distress, high levels of emotional exhaustion, high depersonalisation and a low sense of personal accomplishment were reported in 55.4, 51.5 and 41.6%, respectively. Inadequate preparation for practice, financial worries, poor role definition and sleep deprivation were reported as significant stressors. Most were unaware of available support services and expressed interest in leaving Ireland after internship.ConclusionsBurnout and stress are significant problems amongst doctors in Irish hospitals. Ensuring better preparation for clinical practice and awareness of support services is vital to tackle this issue.

Abstract:Medical students are highly susceptible to distress and burnout, raising concern about how this can carry over into their careers, impacting wellness, professionalism, and patient care. The aims of this study are to assess the mental health and wellness status of medical students in Ontario. This study electronically surveyed medical students across M1-M4 attending a medical school in Ontario, Canada, using the Brief COPE, DASS21, and Copenhagen Burnout Inventory (CBI) to examine their coping mechanisms, levels of stress, anxiety, and depression, the prevalence of personal, work-related, and patient-related burnout, and perceived support. Participants were recruited through mass email containing a survey link. BriefCOPE results show that students tend to rely on self-distraction, emotional support, instrumental support, and acceptance as coping mechanisms. DASS21 demonstrates that M4 students report severe stress and anxiety and mild depression. Results of CBI show moderate personal and work-related burnout, and low patient-related burnout. Qualitative analysis of students' opinion on how the institution can enhance their wellness resulted in three themes: enhanced student scheduling, more communication from the administration, and more elective wellness sessions and social events. The data presented in this study will provide insight into the overall mental health and wellness of medical students studying in Ontario and may have important implications for medical education, healthcare institutions, and health policymaking.
Beyond burnout: a four-year survey of osteopathic medical student mental health and the implications for the development of wellness and mental health programs

Abstract:Context: The mental health crisis in medicine cannot be explained by burnout alone. Physicians are not immune to this crisis and are known to have higher rates of suicide and depression than the general population. A high prevalence of mental health symptoms has been observed in early medical training. Objectives: This study was completed to characterize medical students' mental well-being and provide guidance for timely intervention. Methods: An annual prospective, voluntary, anonymous, cross-sectional survey of medical students was completed over a 4-year period in medical school from 2016 to 2019. The survey was created based on standardized psychiatric screening tools assessing symptoms of depression, anxiety, burnout, and sleep problems. In each of those years, 1,257 (2016), 1,254 (2017), 1,221 (2018), and 1,220 (2019) enrolled students, respectively, were invited to participate. Data on students' mental health were analyzed in relation to their year of school separately for each survey year utilizing SAS 9.4. Results: A total of 973 students in 2016, 889 students in 2017, 547 students in 2018, and 606 students in 2019 participated in the study. For depression and burnout subscales, an increase in symptom scores were observed every survey year (2016, 2017, 2018, and 2019) by the second or third year of medical school with a clinically significant effect size. Persistently high levels of anxiety were observed throughout medical school, with significant increases after the first year noted in the 2016 and 2017 surveys, but not in the 2018 or 2019 surveys. Similarly, significant changes in sleep disturbance were found in the 2016 and 2017 surveys, but not in 2018 or 2019. Conclusions: Symptoms of burnout, depression, and anxiety were observed throughout all four years of medical school, with increases starting after the first year. Early intervention is needed to support students' mental health and increase access to care and resources.
The use of mucin-specific monoclonal antibodies and lectins in the detection of tumor-associated serum markers in gynecological cancer.

Abstract:The use of the mucin-specific lectin from Sambucus sieboldiana (SSAM) in the detection of tumor-associated serum antigens produced by patients with ovarian, cervical, and uterine cancer was investigated. Two-site assays were developed which used either SSAM or the MUC1 core protein-specific monoclonal antibody (mab) BC2 as capture, and biotinylated SSAM to detect bound mucin (SSAM and BC2SSAM assays respectively). These new assays were compared to the CA125 assay, and another assay for MUC1 (CASA), which utilizes the core protein reactive mabs BC2 and BC3. some asymptomatic women and patients with benign disease showed very high levels in the SSAM assay, while this was not the case in the other assays. When cutoff levels were set to exclude healthy women and patients with benign disease, the levels of detection in patients with ovarian cancer were 51% with CASA (> 6.7 units/ml), 71% with CA125 (> 250 units/ml), and 38% with BC2SSAM (> 8.6 units/ml). The levels of detection in cervical and uterine cancer patients were 28% and 25% with CASA, 0% and 8% with CA125, and 28% and 25% with BC2SSAM respectively. Of particular interest was the very different spectrum of reactivity observed with the CASA and BC2SSAM assays which use the same capture mab, indicating that each assay detects different glycoforms of the MUC1 mucin. Indeed, when used in combination, the CASA and BC2SSAM assays gave 62% of ovarian cancer patients, and 50% of cervical or uterine cancer patients with elevated marker levels. The additional use of BC2SSAM gave no advantage over the combined use of the CASA and CA125 assays in ovarian cancer, with 80% of patients detected, but the CASA/BC2SSAM combination was particularly useful in the cervical and uterine cancers due to the low level of detection with CA125. In fact, the additional use of CA125 gave no advantage over the CASA/BC2SSAM combination in these patients. Furthermore, the BC2SSAM assay may also be useful in monitoring patients with high preoperative BC2SSAM levels (> 10 units/ml), since this assay predicted recurrence in 5/5 cases, and was negative in all cases with no evidence of disease. Furthermore, the performance of this assay in monitoring these patients was equal or superior to CA125 and CASA.
How Student and Faculty Perceptions Differ on the Stressors that Medical Students Face

Abstract:Objectives: Medical education is notorious for the stress that students face as they strive to succeed both academically and clinically. This stress has been linked to declining academic performance and worsening mental health. To combat these negative outcomes, it is essential for medical school faculty and administration to address common stressors among medical students. No studies have addressed whether medical school faculty and students perceive stressors similarly, however. Methods: In this two-part study, data collected from medical students in 2021 to 2022 to identify their most significant sources of stress were used to create a survey that queries the frequency and intensity of these stressors. This survey was distributed to medical students and faculty at the same institution. The responses between students and faculty were compared and student data also were analyzed by academic year to observe changes in perception that accompany progression through the medical curriculum. Results: The results showed that faculty overestimated the impact of certain stressors on medical students (eg, in-house examinations, US Medical Licensing Examination Steps 1 and 2 examinations, and patient interactions). In addition, preclinical students were more concerned with finding extracurricular activities, missing opportunities, and performing research compared with clinical students. Conclusions: This study demonstrated that although faculty anticipated most medical student stressors, there are significant gaps that still need to be addressed to better reduce and respond to the stress experienced by medical students.
Trends in medical students' stress, physical, and emotional health throughout training

Abstract:Background: Medical student wellness, including physical health, emotional health, and levels of perceived stress, appears to decline during training, with students reporting high levels of depression, anxiety, and burnout as early as the first year of medical school. The impact of curricular changes on health and stress remains unclear, and a modified curriculum that compresses training of the foundational sciences and its effect on wellness has not been studied. Oregon Health & Science University School of Medicine has recently instituted a unique competency-based model, which provides an important opportunity to assess the effects of curricular change on student wellness.Objective: Assess the effects of curricular change on student wellness.Design: Medical students at a single institution were administered the SF-8, an 8-item health-related quality of life survey, as well as the Perceived Stress Scale, a 10-item scale that measures the degree to which life situations are appraised as stressful, at baseline (matriculation) and at the end of Year 1, 2 and 3. Individual variables were assessed over time, as well as a trend analysis of summary domain scores over the 4 time periods.Results: Physical, emotional, and overall health were highest at baseline and lowest at the end of Year 1, after which they improved but never again reached baseline levels. Physical health declined less than emotional health. Perceived stress levels did not change over time but remained moderately high. There were no differences in health or perceived stress based on demographic variables.Conclusions: In a competency-based curriculum, physical, emotional and overall health significantly worsened during Year 1 but improved thereafter, while perceived stress remained unchanged. Early in training, stress and poor overall health may be related to concerns about self-efficacy and workload. Although advanced students show improved wellness, concerns remained about emotional difficulties, such as anxiety and irritability, and feeling a lack of control.
Systematic Review of Depression, Anxiety, and Other Indicators of Psychological Distress Among U.S. and Canadian Medical Students

Abstract:PURPOSE: To systematically review articles reporting on depression, anxiety, and burnout among U.S. and Canadian medical students.METHOD: Medline and PubMed were searched to identify peer-reviewed English-language studies published between January 1980 and May 2005 reporting on depression, anxiety, and burnout among U.S. and Canadian medical students. Searches used combinations of the Medical Subject Heading terms medical student and depression, depressive disorder major, depressive disorder, professional burnout, mental health, depersonalization, distress, anxiety, or emotional exhaustion. Reference lists of retrieved articles were inspected to identify relevant additional articles. Demographic information, instruments used, prevalence data on student distress, and statistically significant associations were abstracted.RESULTS: The search identified 40 articles on medical student psychological distress (i.e., depression, anxiety, burnout, and related mental health problems) that met the authors' criteria. No studies of burnout among medical students were identified. The studies suggest a high prevalence of depression and anxiety among medical students, with levels of overall psychological distress consistently higher than in the general population and age-matched peers by the later years of training. Overall, the studies suggest psychological distress may be higher among female students. Limited data were available regarding the causes of student distress and its impact on academic performance, dropout rates, and professional development.CONCLUSIONS: Medical school is a time of significant psychological distress for physicians-in-training. Currently available information is insufficient to draw firm conclusions on the causes and consequences of student distress. Large, prospective, multicenter studies are needed to identify personal and training-related features that influence depression, anxiety, and burnout among students and explore relationships between distress and competency.

Abstract:Aim: This study concerns the evaluation of study-related psychosocial risk factors and resources in medical students at the beginning and before the end of their course of studies. Methods: Written questionnaires were filled out by medical students in Lübeck and Freiburg in their first and fifth years of study and analysed with three standard instruments (AVEM, SAM, F-Sozu). The response rate was 84.5% (n=435) in the first and 83.0% (n=351) in the fifth year of study. Results: At the outset of their course of study, most of the students evinced behaviour and subjective experience patterns which were not deleterious to their health. Nevertheless, even at this point in time, 22.9% of the students showed a risk constellation with an excessive commitment to work and readiness to overstrain themselves. With 17.9% resignatory exhaustion with a highly restrictive subjective quality of life was found. In the fifth year of study, this proportion had increased to 23.3%, while the quality of health behaviour and subjective experience patterns deteriorated. Self-awareness and social support were augmented by comparison with norm samples. Conclusion: A portion of the medical students showed a risk constellation in behaviour and subjective experience at the very beginning of their studies. At the end, this tendency has significantly strengthened. The integration of teaching contents dealing with health promotion and successful coping with stress for the medical students themselves would therefore seem essential for the conservation of the health and working capacity of the students, as well as in order to prevent subsequent profession-specific mental stress and burn-out.
Perceived Stress and Stressors among Undergraduate Medical Students of a Nigerian Institution

Abstract:Aim: To identity stressors and measure the intensity of stress perceived by clinical students in a Nigerian institution. Methods: This study was a cross-sectional study of fifth and sixth-year medical students using the 40-item Medical Student Stressor Questionnaire (MSSQ). Students marked their responses to each of the 40 questions on a Likert scale ranging from-causing no stress at all (0) to causing severe stress (4). Results: The median stress scores for the six domains were as follows: Academic related stressor (ARS)- 2.1, Teaching and Learning related stress (TLRS)-1.29, Desire related stressors (DRS)- 1.00, Group activities related stressors (GARS)- 1.00, Social related stressor (SRS) - 0.83, and Interpersonal related stressor (IRS)- 0.57. Overall, ARS was perceived to cause high-level stress in 51.6%, and severe stress in 7.8% of students. Specifically, 'Heavy workload' and 'large amount of content to be learnt' caused severe stress in 45.3% and 40.6% of students respectively. Skipping meals was frequent and associated with high stress scores in IRS, SRS and GRS domains. Conclusion: Academic related stressors cause high-severe stress among a considerable proportion of medical students studied, while interpersonal related stress caused mild stress. 'Heavy workload', 'Tests/Examinations', and 'lack of time to review what has been learnt' are some major stressors identified. Universities need to prioritise accessibility to healthy meals, improved students' living environment, provision of psychological support and formal training on time management and other soft skills, to reduce stress and promote better academic performance. There may be a need to review medical students' curriculum to prioritise relevance over breadth of content.

Abstract:Background Medical students’ rate of depression, suicidal ideation, anxiety, and burnout have been shown to be higher than those of the same-age general population. However, longitudinal studies spanning the whole course of medical school are scarce and present contradictory findings. This study aims to analyze the longitudinal evolution of mental health and burnout from the first to the last year of medical school using a wide range of indicators. Moreover, biopsychosocial covariates that can influence this evolution are explored. Method In an open cohort study design, 3066 annual questionnaires were filled in by 1595 different students from the first to the sixth year of the Lausanne Medical School (Switzerland). Depression symptoms, suicidal ideation, anxiety symptoms, stress, and burnout were measured along with biopsychosocial covariates. The longitudinal evolution of mental health and burnout and the impact of covariates were modelled with linear mixed models. Results Comparison to a same-aged general population sample shows that medical students reported significantly more depression symptoms and anxiety symptoms. Medical students’ mental health improved during the course of the studies in terms of depression symptoms, suicidal ideation, and stress, although suicidal ideation increased again in the last year and anxiety symptoms remained stable. Conversely, the results regarding burnout globally showed a significant worsening from beginning to end of medical school. The covariates most strongly related to better mental health and less burnout were less emotion-focused coping, more social support, and more satisfaction with health. Conclusion Both improvement of mental health and worsening of burnout were observed during the course of medical school. This underlines that the beginning and the end of medical school bring specific challenges with the first years’ stressors negatively impacting mental health and the last year’s difficulties negatively impacting burnout.
