Late Breaking Abstract - Detection of bronchopulmonary dysplasia (BPD) in preterm infants with an artificial neural network (ANN) trained using air flow time series (TS) measured during tidal breathing (Tb)

Abstract:Background: Currently, assessing lung function in infants is based on the multiple-breath washout technique or on capnography, approaches requiring sophisticated equipment to measure the concentration of CO 2 or artificially introduced gases in the exhaled air. On the other hand, the measurement of the inspiratory and expiratory flow during Tb is technically much simpler. Aims and objectives: To train and test an ANN capable of detecting BPD in preterm infants. To this end, the ANN simply processes a TS of air flow values measured during Tb. Methods: Ten minutes of Tb were recorded in term and preterm infants recruited within a prospective birth cohort study [Fuchs O. et al. Cohort profile: the Bern infant lung development cohort. Int J Epidemiol 41: 366–376, 2012]. Here, data from 139 term born infants and from 190 preterm infants was used. Among the 190 preterm infants, 47 were diagnosed with mild, 54 with moderate, and 31 with severe BPD. For each patient, 100 consecutive regular breaths, carefully inspected to exclude sighs or artefacts, were used for training, validating, and testing a Long Short-Term Memory recurrent ANN. The data was randomly split into 60% for training, 20% for validation, and 20% for testing. The model's parameters were tuned by minimizing the binary cross-entropy using gradient optimization. Results: The tuned model's performance on unseen test data was 96% accuracy, 100% specificity, 96% sensitivity, 98% precision and 97% F1-score. Conclusion: Sufficiently long TS of inspiratory and expiratory flow measured in infants during Tb can be used to train an ANN capable of accurately detecting BPD.
