Abstract:Objective: There is a scarcity in tools to predict postpartum depression (PPD). We propose a machine learning framework for PPD risk prediction using data extracted from electronic health records (EHRs). Methods: Two EHR datasets containing data on 15,197 women from 2015 to 2018 at a single site, and 53,972 women from 2004 to 2017 at multiple sites were used as development and validation sets, respectively, to construct the PPD risk prediction model. The primary outcome was a diagnosis of PPD within 1 year following childbirth. A framework of data extraction, processing, and machine learning was implemented to select a minimal list of features from the EHR datasets to ensure model performance and to enable future point-of-care risk prediction. Results: The best-performing model uses from clinical features related to mental health history, medical co morbidity, obstetric complications, medication prescription orders, and patient demographic characteristics. The model performances as measured by area under the receiver operating characteristic curve (AUC) are 0.937 (95% CI 0.912 - 0.962) and 0.886 (95% CI 0.879-0.893) in the development and validation datasets, respectively. The model performances were consistent when tested using data ending at multiple time periods during pregnancy and at childbirth. Limitations: The prevalence of PPD in the study data represented a treatment prevalence and is likely lower than the illness prevalence. Conclusions: EHRs and machine learning offer the ability to identify women at risk for PPD early in their pregnancy. This may facilitate scalable and timely prevention and intervention, reducing negative outcomes and the associated burden.

Abstract:Gestational diabetes mellitus (GDM) is common during pregnancy, with the prevalence reaching as high as 31.0％ in some European regions (McIntyre et al., 2019). Dysfunction of the glucose metabolism in pregnancy can influence fetal growth via alteration of the intrauterine environment, resulting in an increased risk of abnormal offspring birth weight (McIntyre et al., 2019). Infants with abnormal birth weight will be faced with increased risks of neonatal complications in the perinatal period and chronic non-communicable diseases in childhood and adulthood (Mitanchez et al., 2015;McIntyre et al., 2019). Therefore, accurate esti-mation of birth weight for neonates from women with GDM is crucial for more sensible perinatal decision-making and improvement of perinatal outcomes. Timely antenatal intervention, with reference to accurately estimated fetal weight, may also decrease the risks of adverse long-term diseases.

Abstract:Defensins are antimicrobial peptides that exhibit direct microbicidal activity as well as mediator-like functions by, for example, activating immature dendritic cells. This review focuses on defensins and other antimicrobial peptides that are present in periodontal tissues. Their antimicrobial capacity against periodontal microorganisms, their regulation and their expression profiles during periodontal diseases is discussed. As antimicrobial peptides may possess great potential for new diagnostic, preventive and therapeutic strategies, a better understanding of how antimicrobial peptides are regulated as part of the innate host immune response is crucial.

Abstract:This study reviews the current status and future prospective of knowledge on the use of artificial intelligence for the prediction of spontaneous preterm labor and birth (“preterm birth” hereafter). The summary of review suggests that different machine learning approaches would be optimal for different types of data regarding the prediction of preterm birth: the artificial neural network, logistic regression and/or the random forest for numeric data; the support vector machine for electrohysterogram data; the recurrent neural network for text data; and the convolutional neural network for image data. The ranges of performance measures were 0.79–0.94 for accuracy, 0.22–0.97 for sensitivity, 0.86–1.00 for specificity, and 0.54–0.83 for the area under the receiver operating characteristic curve. The following maternal variables were reported to be major determinants of preterm birth: delivery and pregestational body mass index, age, parity, predelivery systolic and diastolic blood pressure, twins, below high school graduation, infant sex, prior preterm birth, progesterone medication history, upper gastrointestinal tract symptom, gastroesophageal reflux disease, Helicobacter pylori, urban region, calcium channel blocker medication history, gestational diabetes mellitus, prior cone biopsy, cervical length, myomas and adenomyosis, insurance, marriage, religion, systemic lupus erythematosus, hydroxychloroquine sulfate, and increased cerebrospinal fluid and reduced cortical folding due to impaired brain growth.

Abstract:Objective: This study sought to develop a multifactorial predictive model for preterm birth risk, with the goal of providing clinical practitioners with early prevention. Methods: This retrospective cohort study utilized 2022 and 2018 National Vital Statistics System (NVSS) birth data, with the 2022 cohort arbitrarily split into training (70%) and internal verification (30%) subsets, and the 2018 cohort for external validation. Four machine learning algorithms-logistic regression, adaptive lasso regression, bootstrap forest, and boosted trees-identified features associated with preterm birth. The study then integrated the consensus features identified across the four models to construct a logistic regression-based preterm birth prediction nomogram. To evaluate the model's efficacy, calibration, receiver operating characteristic (ROC), and decision curve analysis were applied to both the internal and external validation sets. Results: The study included 2 567 040 mother-infant pairs from the 2022 cohort and 2 688 568 mother-infant pairs from the 2018 cohort. All four machine learning models demonstrated high accuracy (area under the curve [AUC] >0.7) in predicting preterm birth, and the internal validation results indicated good model generalizability. Feature selection identified nine common risk factors associated with preterm birth. The prediction nomogram based on these nine common features achieved AUCs of 0.701, 0.702, and 0.704 in the training, internal validation, and external validation sets, respectively. The calibration curves showed good agreement, and the decision curve analysis confirmed the model's net clinical benefits. Conclusion: This study developed a reliable preterm birth prediction tool using large-scale birth cohort data, filling the gap of lacking external validation for existing preterm birth prediction models.

Abstract:Biomedical Engineering: Applications, Basis and Communications, Ahead of Print. The World Health Organization (WHO) estimates that over 15 million infants are born before the entire period of pregnancy. Over a million neonatal deaths occurred in 2015 as a result of preterm delivery, which is the prime cause for most deaths among children under the age of five. Although preterm birth has no hereditary link, only 10% of preterm deliveries in high-income settings result in deaths, compared to a mortality rate of up to 90% in low-income nations. When preterm cases are discovered, in addition to enhanced care provided at home for the expecting mother, the growing foetus may also benefit from medication, hospitalization for the duration of the pregnancy, or both. Low-income countries also struggle with a lack of access to a comprehensive healthcare system, which makes it difficult to proactively detect premature births. Machine learning techniques have a great potential to improve this situation by providing an intelligent framework for the detection of critical situations and consequently alerting the individual in cases of anomalies. This will require further follow-up with medical specialists. In this work, we investigate the application of deep learning and machine learning techniques for the analysis of electrohysterogram (EHG) data, which are uterine electrical impulses used to detect preterm deliveries. The TPEHG dataset is used to train a variety of machine learning classifiers, such as Support Vector Machines, Logistic Regression, and Decision Trees, as well as Deep Neural Networks like Convolutional Neural Networks and LSTMs. Additionally, we use plurality voting to build an ensemble of various neural networks and binary classifiers that are trained to classify EHG signals. The ensemble machine learning classifier with five base classifiers produced the best results overall, with an accuracy of 98.99%, sensitivity of 98.3%, and specificity of 97.9% outperforming several state-of-the-art algorithms for preterm birth detection.
Prediction of preterm birth in multiparous women using logistic regression and machine learning approaches

Abstract:To predict preterm birth (PTB) in multiparous women, comparing machine learning approaches with traditional logistic regression. A population-based cohort study was conducted using data from the Ontario Better Outcomes Registry and Network (BORN). The cohort included all multiparous women who delivered a singleton birth at 20–42 weeks' gestation in an Ontario hospital between April 1, 2012 and March 31, 2014. The primary outcome was PTB < 37 weeks, with spontaneous PTB analyzed as a secondary outcome. Stepwise logistic regression and the Boruta machine learning were used to select the important variables during the first and second trimester. For building prediction models, the whole data set were divided for the two independent parts: two-third for training the classifiers (Logistic regression, random forests, decision trees, and artificial neural networks) and one-third for model validation. Then, the training data set were balanced by random over sampling technique. The best hyper parameters were obtained by the tenfold cross validation. The performance of all models was evaluated by sensitivity, specificity, positive predictive value, negative predictive value, and the area under the receiver operating characteristics (AUC). The cohort included 145,846 births, of which 8125 (5.57%) were preterm. In first-trimester models, the strongest predictors of PTB were previous PTB, preexisting diabetes, and abnormal pregnancy‐associated plasma protein-A. In the testing data set, the highest predictive ability was seen for artificial neural networks, with an area under the receiver operating characteristic curve (AUC) of 68.8% (95% CI 67.6–70.1%). In second-trimester models, addition of infant sex, attendance at first-trimester appointment, medication exposure, and abnormal alpha-fetoprotein concentrations increased the AUC to 72.1% (95% CI 71.1–73.1%) with logistic regression. With the inclusion of the variable complications during pregnancy, the AUC increased to 80.5% (95% CI 79.6–81.5%) using logistic regression. For both overall and spontaneous PTB, during both the first and second trimesters, models yielded negative predictive values of 97%. Overall, machine learning and logistic regression produced similar performance for prediction of PTB. For overall and spontaneous PTB, both first- and second-trimester models provided negative predictive values of ~ 97%, higher than that of fetal fibronectin.

Abstract:Prediction analysis of preterm neonate mortality is necessary and significant for benchmarking and evaluating healthcare services in Hospitals and other medical centers. Application of artificial intelligence and machine learning models, which is a hot topic in medicine/healthcare and engineering, may improve physicians skill to predict the preterm neonatal deaths. The main purpose of this research article is to introduce a preterm neonatal mortality risk prediction by means of machine learning/ML predictive models to survive infants using supervised ML models if possible. Moreover, this paper presents some effective parameters and features which affect to survive the infants directly. It means, the obtained model has an accuracy of about 91.5% to predict the status of infant after delivery. After recognizing the critical status for an infant, physicians and other healthcare personnel can help to infant for possible surviving using special medical NICU cares. It has been tried to get some suitable models with high accuracy and comparing the results. In a word, a survival prediction analysis of preterm neonate mortality has been carried out using machine learning methods via Python programming (possible surviving infants after delivery in the hospital).

Estimation of preterm birth markers with U-Net segmentation network

Abstract:Preterm birth is the most common cause of neonatal death. Current diagnostic methods that assess the risk of preterm birth involve the collection of maternal characteristics and transvaginal ultrasound imaging conducted in the first and second trimester of pregnancy. Analysis of the ultrasound data is based on visual inspection of images by gynaecologist, sometimes supported by hand-designed image features such as cervical length. Due to the complexity of this process and its subjective component, approximately 30% of spontaneous preterm deliveries are not correctly predicted. Moreover, 10% of the predicted preterm deliveries are false-positives. In this paper, we address the problem of predicting spontaneous preterm delivery using machine learning. To achieve this goal, we propose to first use a deep neural network architecture for segmenting prenatal ultrasound images and then automatically extract two biophysical ultrasound markers, cervical length (CL) and anterior cervical angle (ACA), from the resulting images. Our method allows to estimate ultrasound markers without human oversight. Furthermore, we show that CL and ACA markers, when combined, allow us to decrease false-negative ratio from 30% to 18%. Finally, contrary to the current approaches to diagnostics methods that rely only on gynaecologist's expertise, our method introduce objectively obtained results.

Abstract:Background Machine learning could be used for prognosis/diagnosis of maternal and neonates' diseases by analyzing the data sets and profiles obtained from a pregnant mother. Purpose We aimed to develop a prediction model based on machine learning algorithms to determine important maternal characteristics and neonates' anthropometric profiles as the predictors of neonates' health status. Methods This study was conducted among 1280 pregnant women referred to healthcare centers to receive antenatal care. We evaluated several machine learning methods, including support vector machine (SVM), Ensemble, K-Nearest Neighbor (KNN), Naïve Bayes (NB), and Decision tree classifiers, to predict newborn health state. Results The minimum redundancy-maximum relevance (MRMR) algorithm revealed that variables, including head circumference of neonates, pregnancy intention, and drug consumption history during pregnancy, were top-scored features for classifying normal and unhealthy infants. Among the different classification methods, the SVM classifier had the best performance. The average values of accuracy, precision, recall, F1-score, and area under the receiver operating characteristic curve (AUC) in the test group were 75%, 75%, 76%, 76%, and 65%, respectively, for SVM model. Conclusion Machine learning methods can efficiently forecast the neonate's health status among pregnant women. This study proposed a new approach toward the integration of maternal data and neonate profiles to facilitate the prediction of neonates' health status.

Predicting Preterm Birth Using Explainable Machine Learning in a Prospective Cohort of Nulliparous and Multiparous Pregnant Women.

Abstract:Preterm birth (PTB) presents a complex challenge in pregnancy, often leading to significant perinatal and long-term morbidities. "While machine learning (ML) algorithms have shown promise in PTB prediction, the lack of interpretability in existing models hinders their clinical utility. This study aimed to predict PTB in a pregnant population using ML models, identify the key risk factors associated with PTB through the SHapley Additive exPlanations (SHAP) algorithm, and provide comprehensive explanations for these predictions to assist clinicians in providing appropriate care. This study analyzed a dataset of 3509 pregnant women in the United Arab Emirates and selected 35 risk factors associated with PTB based on the existing medical and artificial intelligence literature. Six ML algorithms were tested, wherein the XGBoost model exhibited the best performance, with an area under the operator receiving curves of 0.735 and 0.723 for parous and nulliparous women, respectively. The SHAP feature attribution framework was employed to identify the most significant risk factors linked to PTB. Additionally, individual patient analysis was performed using the SHAP and the local interpretable model-agnostic explanation algorithms (LIME). The overall incidence of PTB was 11.23% (11 and 12.1% in parous and nulliparous women, respectively). The main risk factors associated with PTB in parous women are previous PTB, previous cesarean section, preeclampsia during pregnancy, and maternal age. In nulliparous women, body mass index at delivery, maternal age, and the presence of amniotic infection were the most relevant risk factors. The trained ML prediction model developed in this study holds promise as a valuable screening tool for predicting PTB within this specific population. Furthermore, SHAP and LIME analyses can assist clinicians in understanding the individualized impact of each risk factor on their patients and provide appropriate care to reduce morbidity and mortality related to PTB.

Abstract:BACKGROUND:Postpartum hemorrhage (PPH) is a leading cause of maternal morbidity worldwide and placenta previa is one of the major risk factors for PPH in overall population. However, the clinical prediction of PPH remains challenging. This study aimed to investigate an ideal machine learning-based prediction model for PPH in placenta previa parturients with cesarean section.METHODS:The clinical data of 223 placenta previa parturients who underwent cesarean delivery in our hospital from 2016 to 2019 were retrospectively collected for analysis. An artificial neural network model was designed to predict PPH, defined as blood loss exceeding 1000 mL with 24h after delivery. Twenty clinical variables were selected as predictors. We also applied six conventional machine learning methods as reference models, including support vector machine, decision tree, random forest, gradient boosting decision tree, adaboost and logistic regression. All the models were validated using 5-fold cross-validation. The area under the receiver operating characteristic curve (AUC), precision, recall and the prediction accuracy of each model were reported.RESULTS:A total of 223 pregnant women were enrolled in this study, including 101 cases (45.29%) experienced PPH. The proposed model achieved superior prediction performance with an AUC of 0.917, an accuracy of 0.851, a precision score of 0.829 and a recall score of 0.851, which outperformed other six conventional machine learning methods.CONCLUSIONS:Compared to the conventional machine learning approaches, artificial neural network model shows discriminative ability in identifying women's risk of PPH with placenta previa during cesarean section.

Abstract:Objective: To determine whether deep learning algorithms are suitable for predicting preterm birth.Methods: A retrospective study was conducted at Peking University Third Hospital from January 2018 to June 2023. Birth data were divided into two parts based on the date of delivery: the first part was used for model training and validation, while real world viability was evaluated using the second part. Four machine learning algorithms (logistic regression, random forest, support vector machine, and transformer) were employed to predict preterm birth. Receiver operating characteristic curves were plotted, and the area under the curve (AUC), sensitivity, specificity, positive predictive value (PPV), negative predictive value (NPV), and accuracy were calculated.Results: This research included data on 30,965 births, where 24,770 comprised the first part, and included 3164 (12.77%) in the preterm birth group, with 6195 in the second part, including 795 (12.83%) in the preterm birth group. Significant differences in various factors were observed between the preterm and full-term birth groups. The transformer model (AUC = 79.20%, sensitivity = 73.67%, specificity = 72.48%, PPV = 28.21%, NPV = 94.95%, and accuracy = 72.61 % in the test dataset) demonstrated superior performance relative to logistic regression (AUC = 77.96% in the test dataset), support vector machine (AUC = 71.70% in the test dataset), and random forest (AUC = 75.09% in the test dataset) approaches.Conclusion: This study highlights the promise of deep learning algorithms, specifically the transformer algorithm, for predicting preterm birth.
A prediction model for short-term neurodevelopmental impairment in preterm infants with gestational age less than 32 weeks

Abstract:Introduction: Early identification and intervention of neurodevelopmental impairment in preterm infants may significantly improve their outcomes. This study aimed to build a prediction model for short-term neurodevelopmental impairment in preterm infants using machine learning method. Methods: Preterm infants with gestational age < 32 weeks who were hospitalized in The Maternal and Child Health Hospital of Guangxi Zhuang Autonomous Region, and were followed-up to 18 months corrected age were included to build the prediction model. The training set and test set are divided according to 8:2 randomly by Microsoft Excel. We firstly established a logistic regression model to screen out the indicators that have a significant effect on predicting neurodevelopmental impairment. The normalized weights of each indicator were obtained by building a Support Vector Machine, in order to measure the importance of each predictor, then the dimension of the indicators was further reduced by principal component analysis methods. Both discrimination and calibration were assessed with a bootstrap of 505 resamples. Results: In total, 387 eligible cases were collected, 78 were randomly selected for external validation. Multivariate logistic regression demonstrated that gestational age( p = 0.0004), extrauterine growth restriction ( p = 0.0367), vaginal delivery ( p = 0.0009), and hyperbilirubinemia (0.0015) were more important to predict the occurrence of neurodevelopmental impairment in preterm infants. The Support Vector Machine had an area under the curve of 0.9800 on the training set. The results of the model were exported based on 10-fold cross-validation. In addition, the area under the curve on the test set is 0.70. The external validation proves the reliability of the prediction model. Conclusion: A support vector machine based on perinatal factors was developed to predict the occurrence of neurodevelopmental impairment in preterm infants with gestational age < 32 weeks. The prediction model provides clinicians with an accurate and effective tool for the prevention and early intervention of neurodevelopmental impairment in this population.

Drawing on a long-term research project in the field of games-based learning, the article describes the methodology adopted during the in-field experiments carried out with the aim of developing young children's creativity. The results of the study, which are presented and discussed, confirm the hypothesis that digital tools can contribute to fostering creativity. Improving Prediction of Survival for Extremely Premature Infants Born at 23 to 29 Weeks Gestational Age in the Neonatal Intensive Care Unit: Development and Evaluation of Machine Learning Models

Abstract:Background: Infants born at extremely preterm gestational ages are typically admitted to the neonatal intensive care unit (NICU) after initial resuscitation. The subsequent hospital course can be highly variable, and despite counseling aided by available risk calculators, there are significant challenges with shared decision-making regarding life support and transition to end-of-life care. Improving predictive models can help providers and families navigate these unique challenges. Objective: Machine learning methods have previously demonstrated added predictive value for determining intensive care unit outcomes, and their use allows consideration of a greater number of factors that potentially influence newborn outcomes, such as maternal characteristics. Machine learning–based models were analyzed for their ability to predict the survival of extremely preterm neonates at initial admission. Methods: Maternal and newborn information was extracted from the health records of infants born between 23 and 29 weeks of gestation in the Medical Information Mart for Intensive Care III (MIMIC-III) critical care database. Applicable machine learning models predicting survival during the initial NICU admission were developed and compared. The same type of model was also examined using only features that would be available prepartum for the purpose of survival prediction prior to an anticipated preterm birth. Features most correlated with the predicted outcome were determined when possible for each model. Results: Of included patients, 37 of 459 (8.1%) expired. The resulting random forest model showed higher predictive performance than the frequently used Score for Neonatal Acute Physiology With Perinatal Extension II (SNAPPE-II) NICU model when considering extremely preterm infants of very low birth weight. Several other machine learning models were found to have good performance but did not show a statistically significant difference from previously available models in this study. Feature importance varied by model, and those of greater importance included gestational age; birth weight; initial oxygenation level; elements of the APGAR (appearance, pulse, grimace, activity, and respiration) score; and amount of blood pressure support. Important prepartum features also included maternal age, steroid administration, and the presence of pregnancy complications. Conclusions: Machine learning methods have the potential to provide robust prediction of survival in the context of extremely preterm births and allow for consideration of additional factors such as maternal clinical and socioeconomic information. Evaluation of larger, more diverse data sets may provide additional clarity on comparative performance. Trial Registration:

Abstract:Abstract In recent decades, preterm birth (PTB) has become a significant research focus in the healthcare field, as it is a leading cause of neonatal mortality worldwide. Using five independent study cohorts including 1290 vaginal samples from 561 pregnant women who delivered at term ( n = 1029) or prematurely ( n = 261), we analysed vaginal metagenomics data for precise microbiome structure characterization. Then, a deep neural network (DNN) was trained to predict term birth (TB) and PTB with an accuracy of 84.10% and an area under the receiver operating characteristic curve (AUROC) of 0.875 ± 0.11. During a benchmarking process, we demonstrated that our DL model outperformed seven currently used machine learning algorithms. Finally, our results indicate that overall diversity of the vaginal microbiota should be taken in account to predict PTB and not specific species. This artificial-intelligence based strategy should be highly helpful for clinicians in predicting preterm birth risk, allowing personalized assistance to address various health issues. Abstract:Multiunit activity of paraventricular nucleus and fornix was studied during activation and inhibition of the pituitary--adrenocortical system in chronic experiments on rabbits. Immobilization causing blood corticosteroid levels to rise was followed by the excitation of the neurons in paraventricular nucleus and by inhibition in the fornix. Administration of cortisol (100 micrograms/kg before the immobilization) causing inhibition of pituitary--adrenocortical system was followed by the inhibition of neurons in paraventricular nucleus and fornix. The activation of the paraventricular nucleus seems to be responsible for the activation of the pituitary--adrenocortical system whereas its inhibition induced the inhibition of this system.
