Abstract:Pulmonary embolism (PE) is a potentially lethal disease. Syncope or seizure as a presenting symptom of PE is very difficult to diagnose. We report a patient with PE presenting recurrent transient loss of consciousness including both syncope and seizure. A previously healthy 51-year-old woman with unremarkable family history and personal history was referred to our hospital for evaluation of recurrent transient loss of consciousness. Half a month before admission, she had approximately 60 seconds of loss of consciousness with urinary and fecal incontinence when getting up in the morning. She had no tonic-clonic activity. She was brought to the local hospital. There in the afternoon she experienced another transient loss of consciousness the same as the previous episode in the morning. Three days before admission, she encountered a third lapse in consciousness. Her husband witnessed the 60 seconds spell characterized by tonic bilateral upgaze with head twitching to the right upper and extensor posturing of all four extremities. Then she was transferred to our hospital from the local hospital. On physical examination, she was alert without evidence of respiratory distress and showed no signs of cyanosis. Her blood was 110/70 mmHg without orthostatic change. The chest examination revealed no abnormal findings. There was no hepatomegaly or splenomegaly on abdominal examination. And neurologic examination was also normal. Objective tests including a head CT and brain MRI were normal. The electroencephalogram (EEG) was also unremarkable. The level of D-dimer was 1018 μg/L. A Doppler scan of the lower extremities revealed no deep venous thrombosis. A chest CT showed vague opacities in the upper lobe of right lung and bilateral small pleural effusions. The ECG revealed a sinus tachycardia and an echocardiogram showed elevated pulmonary artery pressure with normal left and right ventricles. And Hotler did not reveal long Q-T interval or any arrhythmias. On the sixth hospital day during defecating in the morning, she suddenly fell to the ground and was witnessed by an approximately 30 seconds spell characterized by tonic bilateral upgaze and extensor posturing of all four extremities without urinary and fecal incontinence. Postictally, she complained of weakness and denied chest pain or dyspnea. On examination, she was tachycardic, tachypneic, and normotensive. She had a pulse oximetry value of 100% while receiving 30% oxygen. Two hours later, she became hypoxic and hypotensive and required intravenous administration of pressor agents. She had a pulse oximetry value of 86% while receiving 100% oxygen by face mask, becoming obviously cyanotic and progressive dyspnea. Emergent CT angiograms confirmed PE (Figure 1). She was subsequently treated with bolus urokinase and followed by subcutaneous injection of low molecular weight heparin, which resulted in clinical improvement. She was discharged home on the eighteenth hospital day with a regimen of warfarin.Figure 1.: Contrast-enhanced CT angiograms show bilateral pulmonary embolism (arrows).Here we report a patient with recurrent PE which resulted in recurrent attacks of syncope and seizure, as emboli were detected in both left and right pulmonary arteries. PE triggered vaso-vagal reflex, ventricular failure, impaired left ventricular filling, and arterial hypotension were the mechanisms involved in our patient.1,2 D-dimer test and CT angiograms are now widely used in diagnostic algorithms for the evaluation of PE, and prompt medications with thrombolytic therapy and anticoagulation might be life-saving.3
Lifestyle Effects on an Unusual Presentation of Syncope: A Case Report

Abstract:We present a 53-year-old Hispanic male with a history of palpitations and chronic marijuana use coming to the emergency department (ED) with three episodes of sudden loss of consciousness that occurred after starting his job as a night shift worker, which led to severe chronic sleep deprivation. These episodes lacked prodromal (chest pain, shortness of breath, palpitations, diaphoresis) and postictal (drowsiness, nausea, confusion, headache) symptoms. Electrocardiograms (EKGs) performed in the ED revealed sinus bradycardia with a heart rate of 54 beats per minute (bpm), which dropped to 37 bpm during admission. Overnight telemetry exhibited sinus pauses, characterized by a delay in atrial activity for at least three seconds. A repeat EKG showed an incomplete right bundle branch block (RBBB). The patient received a final diagnosis of recurrent syncope and was given an implantable loop recorder (ILR). The ILR revealed several sinus pauses over the span of three months with no syncopal episodes. The patient was educated on dietary and lifestyle modifications to reduce the risk of experiencing syncopal episodes. This case study explores a unique presentation of syncope with a multifactorial etiology and discusses the impact of lifestyle behaviors on syncope exacerbation.

Abstract:A woman in her mid 70s presented to the emergency department following a sudden loss of consciousness during a meal. When paramedics arrived, the patient had a palpable pulse, but an out-of-hospital cardiac arrest (OHCA) developed in the ambulance. Cardiopulmonary resuscitation (CPR) was initiated, achieving return of spontaneous circulation (ROSC) after 19 minutes without a shockable rhythm. The reported medical history included hypertension. The patient underwent an electrocardiogram (ECG) on arrival (Figure, A).

[High risk syncope in a young patient: An uncommon cause]

Abstract:Syncope is a frequent consult. It's responsible for 1 to 3% of consults to the emergency room in the United States. The cause is generally benign; however, high-risk cases are potentially deadly and must be identified. We present a case report of a 40-year-old female who presents with syncope, Torsade de Pointes, and ventricular fibrillation. The coronary angiography showed no disease of the coronary arteries. The ventriculography, transthoracic echocardiogram, and cardiac magnetic resonance revealed a mid-ventricular Takotsubo cardiomyopathy. The ECG showed a prolonged QT interval with an inverted T wave. During the first 48 hours, she presented multiple episodes of non-sustained polymorphic ventricular tachycardia. The patient evolved with diminished QTc interval and regression of the wall motion abnormalities. At the 6-month follow-up the patient was asymptomatic, without new episodes.
A Case of Syncope Induced in the Supine Position

Abstract:We experienced a reproducible supine syncope followed by upper abdominal pain. A 66-year-old man was transferred to our hospital after an episode of syncope during sleep. He had a history of acute pancreatitis, diabetes, hypertension, and dyslipidemia, but no history of presyncopal attack. One night, his wife noticed he was snoring abnormally in bed, and he did not respond to her voice until after she tried many times to wake him. The same attack was reproduced three times in the same situation. One of the attacks was recorded under a continuous ECG and radial tonometry. In this case, a presyncopal attack and a sense of ill-feeling were provoked by the patient lying in a prolonged supine position. He was eventually diagnosed as metastatic liver tumor 5 months after the first attack. Because few cases of syncopal attack have been reported in the supine position, its underlying mechanisms deserve consideration.

Abstract:In this work, we report a rare case of vasovagal syncope presenting with ventricular arrest for approximately 15 s, which was successfully cured by cardiac nerve ablation. A 50‐year‐old female patient, presented with repeated syncope for more than 2 years. Prior assessments were conducted at different hospitals, but no definite abnormalities were found. The patient's fear and anxiety about possible future attacks were escalating. Through a Head‐up tilt test, the cause was finally identified as vasovagal syncope. Following a 5‐min administration of nitroglycerin, the patient reported palpitations, nausea, and deep, rapid breathing. The electrocardiogram initially showed a first‐degree atrioventricular block, progressing swiftly to a second‐degree type I atrioventricular block—high atrioventricular block. Immediate intervention was undertaken, but blood pressure was not instantly ascertainable, coinciding with an abrupt loss of consciousness. Subsequent electrocardiographic findings included paroxysmal third‐degree atrioventricular block, sinus arrest, and complete cardiac arrest, prompting the initiation of external cardiac compressions. The longest recorded ventricular arrest approximated 15 s, with sinus rhythm resuming post 10 s of cardiac compressions and the patient regaining consciousness. The patient underwent vagal ablation and no longer experienced syncope.

Recurrent Syncope in an 84-Year-old Man

Abstract:An 84-year-old man with hypertension and type 2 diabetes presented with recurrent transient loss of consciousness within 2 hours after dinner at home. Physical examination, electrocardiogram, and laboratory studies were unremarkable except hypotension. Blood pressures were measured in different postures and within 2 hours after meal, but neither orthostatic hypotension nor postprandial hypotension was detected. Further, history taking revealed that the patient was tube-fed with a fluid food pump with an inappropriate rapid infusion rate of 1500 mL per minute at home. He was eventually diagnosed as having syncope due to postprandial hypotension, which was caused by the inappropriate way of tube feeding. The family was educated about appropriate way of tube-feeding and the patient did not develop any episode of syncope during a two-year follow-up. This case highlights the importance of careful history taking in the diagnostic evaluation of syncope and the increased risk of syncope due to postprandial hypotension in the elderly.

Abstract:Vasospastic angina is caused by sudden occlusive vasoconstriction of a segment of an epicardial artery, which can present with a wide spectrum of clinical scenario. We report the cases of two patients diagnosed with vasospastic angina, with one of which presenting with sudden cardiac arrest, while the other presenting with a relatively benign syncope. But both of them have J waves formation on ECG during active ischemia. The diagnosis and management of vasospastic angina, as well as the proposed clinical significance of J waves during coronary spasm are discussed.

Abstract:Steal syndrome is characterized by the disruption of blood flow from specific vascular territories due to perfusion via collateral vessels bypassing an obstructed artery segment. In the neurovascular setting, this can lead to entities such as subclavian steal syndrome or the less prevalent double steal phenomenon, primarily associated with atherosclerosis. We present a case of an early 80s patient with cerebrovascular insufficiency who exhibited neurological deficits during mastication. The angiographic evaluation showed occlusion at the origins of the left common carotid and of the brachiocephalic trunk. This configuration caused the left vertebral artery to be the only vessel filling anterograde. The flow in the right vertebral artery is retrograde, filling the subclavian and the right common carotid artery. Endovascular revascularization of the brachiocephalic trunk was successfully performed using a Gore Viabahn VBX Balloon Expandable Stent (W.L. Gore & Associates, Flagstaff, AZ, USA), leading to symptom resolution.

Abstract:Stretch syncope (SS) is a benign, uncommon, distinct condition described mainly in adolescent males. It is responsible for paroxysmal events started by stereotyped stretching actions with neck hyperextension, culminating in alteration of consciousness. Motor manifestations are often present and may be associated with a generalized slowing of the electroencephalographic activity, challenging the diagnosis. Despite a few cases reported in the literature, different mechanisms have been implied in the pathogenesis, involving both local and systemic hemodynamic phenomena. Here, we report on an 8-year-old girl with self-induced SS, providing new insights into the related neurophysiological profile and discussing the possible etiology. Our evidence of transient and dynamic vascular impairment supports the hypothesis of SS as a multifactorial disorder.

Abstract:Background: Ischemia with non-obstructive coronary artery disease is a prevalent form of ischemic heart disease. The majority of ischemia with non-obstructive coronary artery disease cases are attributed to underlying factors such as coronary microvascular dysfunction (CMD) and/or coronary artery spasm. Ischemia with non-obstructive coronary artery disease can present with various clinical manifestations. Recurrent syncope is an atypical complaint in patients with ischemia with non-obstructive coronary artery disease. Case presentation: This case report describes the presentation of a 58-year-old Chinese male patient who experienced repeated episodes of syncope. The syncope was found to be caused by concomitant coronary artery spasm and presumptive coronary microvascular dysfunctionc suggested by "slow flow" on coronary angiography. The patient was prescribed diltiazem sustained-release capsules, nicorandil, and atorvastatin. During the three-month follow-up conducted on our outpatient basis, the patient did not experience a recurrence of syncope. Conclusion: This study highlights the importance of considering ischemia with non-obstructive coronary artery disease as a potential cause of syncope in the differential diagnosis. It emphasizes the need for early diagnosis of ischemia with non-obstructive coronary artery disease to facilitate more effective management strategies.
Spontaneous Atraumatic Rupture of a Liver Hemangioma as a Rare Cause of Syncope

Abstract:Background: Syncope is common in emergency medicine, but only a minority of syncopes is caused by hemorrhage. Liver hemangioma is the most frequent benign liver tumor, and they rarely lead to symptoms or complications. Case Presentation. We describe the case of an 81-year-old man with hemorrhagic shock due to an atraumatic rupture of a hepatic hemangioma while on oral anticoagulation. The patient presented to the emergency department after three episodes of syncope before admission, nausea, vomiting, mild epigastric abdominal pain, but with clinical signs of peritonitis. On admission, the patient had a mild tachycardia with a heart rate of 107/min and a blood pressure of 102/83 mmHg. Initial hemoglobin was 122 g/L, and lactate was slightly elevated (2.5 mmol/L). Bedside sonography revealed free intraabdominal fluid. The subsequent computed tomography showed a ruptured hemangioma of the liver with ongoing hemorrhage. After the CT scan, the patient became increasingly tachycardic and the blood pressure dropped to 94/62 mmHg. After administration of blood products and intravenous fluids, the patient responded with improved hemodynamics and was transferred to angiology for emergency embolization. After the intervention, the patient spent two days in the intermediate care unit and was discharged after 10 days of hospitalization. Conclusion: Atraumatic rupture of a hemangioma with consecutive hemorrhagic shock is extremely rare. In selected cases of spontaneously ruptured hemangiomas with hemoperitoneum, endovascular embolization can be an alternative to surgery. Furthermore, this case emphasizes the importance of sonographic examination as an additional diagnostic tool in syncope and concomitant abdominal pain.

Abstract:Transient loss of consciousness (TLOC) is defined as a brief episode of unconsciousness with a subsequent complete recovery. The etiology can be diverse, encompassing both traumatic and nontraumatic reasons. Nontraumatic causes of TLOC can be further categorized into (a) syncopes, (b) epileptic reasons, (c) a psychogenic origin and (d) rare ones [ 1 ]. Syncopes, defined as a decrease in blood pressure with a subsequent global cerebral hypoperfusion and loss of postural tone, only represent a subset of TLOCs and can be named as such only after a thorough prior diagnostic evaluation [ 1 ]. They can be further classified based on their etiology or the risk they carry for an underlying serious condition. The diagnostic tools used in this context are typically simple and straightforward in their application [ 1 , 2 ]. However, identifying and interpreting the root causes of the occurred TLOC in an adequate, chronologically structured manner is complex and differentiating syncopes from other nonsyncopal TLOCs can, thus, often be difficult. Doing so is of major relevance and a common challenge particularly in emergency departments (EDs) [ 3 ]; a setting where, by nature, the most unselected and undefined patient cohort presents.

59 THE SHARK-FIN THAT EMERGES AND IMMERGES: A CASE REPORT OF TRANSIENT AND RIPETITIVE TRIANGULAR QRS-ST-T.

Abstract:Abstract Introduction Shark fin sign, also known as triangular QRS-ST-T waveform (TW), is characterized by a giant R wave (amplitude ≥ 1 mV) resulting from the fusion of the QRS complex, the ST-segment and the T wave and showing triangular morphology. TW pattern is quite uncommon and reveals a life-threatening condition that needs aggressive management. Early recognition is important to prevent acute complications such as ventricular arrhythmias and cardiogenic shock. Case presentation A 45-years old black man with history of arterial hypertension presented with out-of-hospital cardiac arrest (OHCA) at his workplace. Extracorporeal cardiopulmonary resuscitation started immediately and return of spontaneous circulation occurred after 11 minutes and 2 DC shocks. Then he was transferred to our emergency department (ED). At arrival he was hemodynamically stable, ECG showed atrial fibrillation without any repolarization abnormalities and echocardiogram revealed left ventricular hypertrophy (IVS 12mm, PW 12mm) and normal biventricular function. Normal electrolytes and high-sensitivity troponin level of 14 (n.v. 0.01-19.8 ng/L) were found at the blood test. Sedation in the ED was needed due to psychomotor agitation. He was then transferred to the CCU where spontaneous cardioversion to sinus rhythm occurred, and no additional arrhythmias were noted at continuous ECG monitoring. After ten hours, while the patient was still under sedation, ECG monitoring revealed a progressive QRS-ST segment elevation in inferior leads up to a triangular QRS-ST-T giant waveform (“Shark fin sign”), without hemodynamic deterioration. After approximately 3 minutes the ECG showed a progressive and complete regression to a normal waveform. An emergent beside echocardiogram demonstrated inferior wall hypokinesia, but normal pump function. The episode repeated 5 more times in the following hour, waiting for cath lab personnel. Coronary angiography revealed a sub-occlusive mid RCA stenosis without any thrombotic lesion. Percutaneous coronary intervention (PCI) with the implantation of one drug eluting stent (DES) was performed. After the procedure the patient was hemodynamically and electrically stable. ECG did not show any other abnormality and no episodes of recurrent ST-elevation occurred. Sedation was suspended and no focal neurological deficits were noted. Discussion Shark fin sign is the expression of abrupt total deprivation of blood flow in large areas of myocardium that cause massive slowing of transmural electrical conduction. These electrical differences may be at the base of the triangular R waves and predispose to VF induction and cardiogenic shock. This sign is very rare, with an annual incidence of 0.7% in patients admitted for STEMI, and in most of the cases is described in the occlusion of the LAD. In our case we assisted to transient but repetitive inferior triangular ST-elevation with complete regression to normal ECG. The coronary angiogram showed non-obstructive lesion of the RCA. This phenomenon was probably the expression of RCA's plaque coronary vasospasm that caused the ECG shark fin sign and cardiac arrest. Conclusion Shark fin electrocardiographic waveform is an uncommon pattern of STEMI and may be the expression of thrombotic coronary occlusion as well as massive coronary vasospasm. This sign correlates with high risk of cardiogenic shock, electrical instability, and cardiac arrest, thus requiring prompt and aggressive management.

Abstract:Diagnosing patients presenting to the emergency department with self-limited episodes of isolated dizziness (the episodic vestibular syndrome) requires a broad differential diagnosis that includes posterior circulation transient ischemic attack. Because these patients are, by definition, asymptomatic without new neurologic findings on examination, the diagnosis, largely based on history and epidemiologic context, can be challenging. We review literature that addresses the frequency of posterior circulation transient ischemic attack in this group of patients compared with other potential causes of episodic vestibular syndrome. We present ways of distinguishing posterior circulation transient ischemic attack from vestibular migraine, the most common cause of episodic vestibular syndrome. We also present a diagnostic algorithm that may help clinicians to work their way through the differential diagnosis.

Abstract:The differential diagnosis for pediatric patients with apparent transient loss of consciousness(TLOC)is a frequent challenge.True TLOC refers to a state of unawareness of a short duration(usually less than 5 min)that terminates spontaneously,during which the patient is unresponsive to speech,touch or pain and may exhibit abnormal motor activity,including jerking movements of the extremities[1].In general,the patients cannot recall the complete course of the episode,and any physical findings during the episode can only be obtained through the description of witnesses.
STEMI in a Patient With Recent Intracranial Hemorrhage

Abstract:Vestibular paroxysmia is an episodic vestibular disorder resulting from compression or irritation of the eighth cranial nerve. This disorder is a rare and difficult diagnosis in children. We report the case of a 16-year-old adolescent male with a history of syncope and coronavirus disease 2019 infection four months prior who presented with intermittent episodes of vertigo and unsteadiness several times a week. These events started abruptly, and he appeared frozen. However, he remained conscious and was able to answer questions. He subsequently resumed normal activity in less than a minute without seizure stigmata or postictal period. His general and neurological examinations were unremarkable. Extensive diagnostic evaluation yielded negative results, except for an electrocardiogram consistent with Wolff-Parkinson-White syndrome. However, his symptoms persisted after cardiac ablation, suggesting they were not related to this arrhythmia. Following unsuccessful trials with various medications, his symptoms resolved with carbamazepine. Early recognition and appropriate treatment of this condition could substantially improve the quality of life for affected individuals.
