Abstract:The esophageal foreign body with dangerous vascular complications is a rare but usually fatal disease, including aortoesophageal fistula (AEF), arterio-esophageal fistula, penetrating aortoesophageal injury, artery dissection etc. Although surgical intervention is mandatory to these entities, there is little consensus about the optimal management of these conditions due to rare cases reported. With advent of the endovascular surgical strategy, the notion of treatment has been changed, and the successful cases reported had increased. Herein, we report three cases with dangerous vascular complications. First case is a 31-year-old man accidentally swallowing a pill with an oblong plastic packaging one day before admission and clinical manifestation of retrosternal pain and worsening dysphagia, with the diagnose of potential AEF. The second case is a 58-year-old woman swallowing a duck bone accidentally five days before admission and clinical manifestation of mild odynophagia, with the diagnose of aortic intramural hematoma. The third case is a 46-year-old-man swallowing a fish bone accidentally one day before admission and clinical manifestation of mild odynophagia and abnormal foreign body sensation, with the diagnose of one penetrating aortoesophageal injury. They were all successfully managed by combination of an emergent endovascular stent graft and endoscopic technique, and uneventful with follow-up of 14 months, 19 months and 23 months respectively.

Abstract:This is a case of plasma pseudocholinesterase deficiency in a patient who was unable to extubate and had a prolonged mechanical ventilation after receiving succinylcholine for esophagogastroduodenoscopy (EGD). Pseudocholinesterase deficiency is a rare genetic as well as an acquired disorder that affects the body's ability to metabolize choline esters such as succinylcholine and mivacurium. In most cases of pseudocholinesterase deficiency, no signs or symptoms of the condition exist. It is first suspected after a prolonged recovery from paralysis following general anesthesia in which succinylcholine or mivacurium are administered.

Abstract:Background: Acute pancreatitis is a common cause of hospitalisation characterised by inflammation of the pancreas. While mechanical, toxic and iatrogenic factors typically cause it, post-oesophagogastroduodenoscopy (EGD) pancreatitis is extremely rare. This report examines a case of acute pancreatitis following EGD, aiming to highlight this rare but significant complication. Case description: A 46-year-old woman with a history of breast cancer, anxiety, vitamin D deficiency and gastro-oesophageal reflux disease underwent an EGD, which revealed and led to the removal of duodenal polyps. Six hours post-procedure, she presented with severe abdominal pain radiating to her back, accompanied by nausea. Laboratory results indicated elevated lipase levels, and a computed tomography (CT) scan confirmed acute pancreatitis. The patient was managed with aggressive fluid resuscitation, bowel rest and pain management, leading to an improvement in her condition and subsequent discharge. We believe that the pancreatitis was likely caused by the use of cautery during the endoscopic mucosal resection of duodenal polyps. Conclusion: This case underscores the need for clinicians to recognise acute pancreatitis as a potential complication of EGD, especially in the absence of other common risk factors. Learning points: Acute pancreatitis following oesophagogastroduodenoscopy (EGD) is an uncommon but significant complication.Possible mechanisms include mechanical trauma, gas insufflation or electrical injury during endoscopic mucosal resection.Clinicians should be aware of this potential complication, especially when no typical aetiological factors for pancreatitis are present.

Abstract:Esophageal rupture, known as Boerhaave's syndrome, is a rare but life-threatening condition characterized by a transmural tear in the esophageal wall, typically resulting from a sudden increase in intraesophageal pressure, commonly due to forceful vomiting. Eosinophilic esophagitis (EoE) characterized by eosinophilic infiltration and chronic inflammation of the esophagus leading to esophageal stenosis and fibrosis. Patients with uncontrolled EoE have an increased risk of esophageal food impaction predisposing them to esophageal injury.

Abstract:Gastroparesis is the functional delay in gastric emptying, without a known obstructive cause. Common etiologies include diabetes mellitus, post-surgical, post-viral and medications; however, most cases remain idiopathic. Diet changes, prokinetic agents, or endoscopic interventions such as botulinum toxin injection and pyloric muscle dilatation have been used with limited efficacy. Gastric Per Oral Endoscopic pyloromyotomy (G-POEM) has been shown to be a safe and effective method for treatment of medically-refractory gastroparesis. Herein, we report the use of G-POEM for the management of gastroparesis, followed by a rare complication of gastric flap necrosis presenting as acute upper Gastrointestinal (GI) hemorrhage.

Abstract:Respiratory arrest secondary to megaoesophagus is a rare complication of achalasia. We treated an 85-year-old female with a history of achalasia who presented with sudden respiratory arrest and cardiopulmonary resuscitation in the community. In the emergency department, she was intubated for respiratory distress secondary to upper airway obstruction and reduced consciousness. Flexible nasal endoscopy revealed a retropharyngeal bulge, and computed tomography (CT) demonstrated megaoesophagus with distal tapering. She was managed with gastric decompression and percutaneous endoscopic gastrostomy (PEG) feeding with an uncomplicated hospital course. This case provides a rare differential for a patient with acute upper airway obstruction and cardiopulmonary arrest and is the first such case described in the literature in Aotearoa New Zealand.

Acute noncardiogenic pulmonary edema after neostigmine administration during the recovery period of general anesthesia: A case report

Abstract:Rationale: Acute non-cardiogenic pulmonary edema (ANPE) is a rare but challenging complication which occurs during the perioperative period, mainly before and after the extubation in the course of the recovery period of general anesthesia. It is characterized by increased fluid in extravascular pulmonary spaces, preventing gas exchange and further resulting in respiratory failure. Patient concerns: A 12-year-old boy who had undergone island skin flap in the right upper limb and who developed a unilateral pulmonary edema after the administration of neostigmine during the recovery period of general anesthesia. The neostigmine was administered to reverse neuromuscular block. Diagnoses: Acute non-cardiogenic pulmonary edema. Interventions: The patient was transferred to intensive care unit (ICU) and treated with mechanical ventilation (controlled mode ventilation pattern, CMV) and other supportive treatment. Outcomes: The oxyhemoglobin saturation of the patient returned to the normal level with symptoms and signs of ANPE significantly alleviated. The mechanical ventilation was withdrawn by the fourth day, and no sequela of vital organs was observed. Lessons: Although neostigmine is widely used for recovery from neuromuscular block and exhibits satisfactory effect in most cases, there is a potential risk of ANPE in some cases, which is rare but potentially fatal and could affect the patient outcomes. Therefore, it is necessary for anesthetists to closely monitor the vital signs of patients after administration of neostigmine in the perioperative period.

Abstract:Esophageal food bolus impaction is a medical emergency. While most foreign objects and food boluses pass spontaneously, 10% to 20% may require endoscopic removal. Delaying esophagogastroduodenoscopy (EGD) is discouraged, as prompt removal within 24 hours is recommended by the American Society for Gastrointestinal Endoscopy (ASGE). Current gastrointestinal treatment guidelines advise against obtaining radiographs before endoscopic evaluation in suspected uncomplicated, nonbony esophageal food bolus cases.

Abstract:Cerebral air embolism (CAE) is a rare entity and is more often secondary to iatrogenic causes. We present a rare case of CAE post esophagogastroduodenoscopy (EGD) with a shower of emboli resulting in diffuse cortical infarction. An 80-year-old man with gastroesophageal reflux disease underwent an elective EGD for esophageal dilatation. During the procedure, there was significant arterial and venous bleeding which subsided with treatment. After the procedure, the patient became comatose with the decerebrate extensor posturing to painful stimulation. Computed tomography without contrast revealed multiple foci of CAE. Diffusion-weighted magnetic resonance images performed at the same time showed numerous areas of acute ischemic lesions affecting primarily the Gray Matter, demonstrating both cortical micro air emboli and bi-hemispheric, global hypoperfusion leading to cortical ribboning pattern. There have been less than 22 cases of CAE after EGD in the literature, most of which were middle cerebral arterial territory infarctions. Our clinical images represent one of the extremely rare cases showing a shower of emboli and global hypoperfusion-induced gray matter infarction due to CAE-causing brain herniation.
Acute cricopharyngeal achalasia after general anesthesia in myotonic dystrophy: A case report

Abstract:Rationale: Myotonic dystrophy type 1 (DM-1) is a progressive multisystem genetic disorder that causes myotonia and both distal limb and facial/neck muscle weakness by expanding the CTG repeats of the DMPK gene in chromosome 19q13.3. General anesthesia is indicated in DM-1 patients owing to their sensitivity to anesthetic drugs such as opioids, hypnotics, and neuromuscular blocking agents. Patient concerns: A 48-year-old male patient underwent a laparoscopic cholecystectomy for gallstones under general anesthesia. He experienced sudden cardiac arrest and respiratory failure the day after surgery. After a thorough review of past medical history, we recognized that 15 years prior, he had been diagnosed with classic type DM-1, but the diagnosis was not self-reported before general anesthesia. Symptoms of severe dysphagia developed subsequently. In a videofluoroscopic swallowing study (VFSS), we observed abrupt aggravation of myotonic dysphagia after general anesthesia. VFSS revealed cricopharyngeal opening dysfunction, with a remaining large residue in the pyriform sinus, resulting in a severe cricopharyngeal achalasia pattern. Diagnosis: Acute cricopharyngeal achalasia after general anesthesia. Intervention and outcome: The patient underwent a dysphagia rehabilitation program that included cricopharyngeal opening exercises and functional electrical stimulation. However, no significant improvement was observed in the cricopharyngeal achalasia in a 3-month follow-up VFSS. Lessons: Low body temperature and anesthetic medications such as opioids and hypnotic agents can induce myotonia in the cricopharyngeal muscle.

Abstract:Background: Gelatin foam has been regularly used the complex neuro and spinal surgeries for a long time. Apart from their hemostatic properties, these are inert and provide form an inert membrane that prevents scar adhesions o vital structures such as the brain or spinal cord. Case presentation: We present a case of cervical myelopathy due to an ossified posterior longitudinal ligament that underwent the instrumented posterior decompression and had neurological worsening 48 h after the index surgery. An magnetic resonance imaging showed a hematoma compressing the spinal cord which on exploration was confirmed to be a gelatin sponge. It represents the rare phenomenon of mass effect due to their osmotic properties, especially in a closed space causing neurologic deterioration. Conclusion: We emphasize the rare cause of early onset quadriparesis after the posterior decompression due to the swollen gelatin sponge over the neural elements. The patient recovered with timely intervention.

Abstract:Perfusion, Ahead of Print. IntroductionAn infrequent yet known complication of ECMO is abdominal compartment syndrome requiring emergency laparotomy. Also, the need for prolonged enteral nutrition while on ECMO may require endoscopic gastrostomy to maintain adequate nutritional status. Here we describe our experience with emergency laparotomy and endoscopic gastrostomy in patients on ECMO support.MethodsWe retrieved patient histories from our clinical archives and performed a retrospective description of all patients taken to an emergency laparotomy or endoscopic gastrostomy while on ECMO support at our cardiovascular referral center from July 2019 through June 2024.ResultsDuring the research period of 5 years a total of 401 patients were placed on ECMO support for either cardiogenic shock or respiratory failure. A total of 27 (7%) patients required an abdominal intervention while on ECMO. 14 (3.5%) patients required emergency laparotomy and 13 (3.2%) of patients required endoscopic gastrostomy tube placement. Overall 30-day mortality of all patients requiring a general surgery procedure while on ECMO support was 33%.ConclusionECMO support can result in many complications despite its many benefits. Patients who require emergency laparotomy while on ECMO have lower survival-to-discharge and higher mortality at 30 days. Endoscopic gastrostomy however, can be safely performed on ECMO with little to no bleeding complications despite anticoagulation.

Abstract:Endoscopic retrograde cholangiopancreatography is a complex procedure with a significant risk of severe consequences. We herein report a 56-year-old Middle Eastern female who was diagnosed with acute ascending cholangitis. Endoscopic retrograde cholangiopancreatography was performed with gallstone absorption and stent implanting. However, the patient developed significant pneumothorax; pneumomediastinum, pneumoperitoneum, pneumoretroperitoneum, and subcutaneous emphysema of the abdomen, chest, right arm and shoulder, face and right orbital area. Radiological studies demonstrated no evidence of perforation on bowel obstruction. The patient was treated successfully with good results and post-operative follow-up was unremarkable. In conclusion, air leakage following endoscopic retrograde cholangiopancreatography without evidence of perforation is extremely rare. While pneumothorax development usually requires thoracostomy; pneumomediastinum, pneumoperitoneum, pneumoretroperitoneum, and subcutaneous emphysema are usually treated conservatively.
Resolution of severe gastroparesis induced by parasympathetic surge following facial trauma: a case report

Abstract:Background: Gastroparesis is a condition that affects the motility of the gastrointestinal (GI) tract, causing a delay in the emptying process and leading to nausea, vomiting, bloating, and upper abdominal pain. Motility treatment along with symptom management can be done using antiemetics or prokinetics. This study highlights the diagnostic and therapeutic challenges of gastroparesis and suggests a potential link between facial trauma and symptom remission, indicating the need for further investigation. Case presentation: A 46-year-old Hispanic man with hypertension, type 2 diabetes (T2D), and hyperlipidemia on amlodipine 10 mg, lisinopril 5 mg, empagliflozin 25 mg, and insulin glargine presented with a diabetic foot ulcer with probable osteomyelitis. During hospitalization, the patient developed severe nausea and vomiting. The gastroenterology team advised continuing antiemetic medicine and trying very small sips of clear liquids. However, the patient didn't improve. Therefore, the gastroenterology team was contacted again. They advised having stomach emptying tests to rule out gastroparesis as the source of emesis. In addition, they recommended continuing metoclopramide, and starting erythromycin due to inadequate improvement. Studies found a 748-min stomach emptying time. Normal is 45-90 min. An uneventful upper GI scope was done. Severe gastroparesis was verified, and the gastroenterology team advised a percutaneous jejunostomy or gastric pacemaker for gastroparesis. Unfortunately, the patient suffered a mechanical fall resulting in facial trauma. After the fall, the patient's nausea eased, and emesis stopped. He passed an oral liquids trial after discontinuation of erythromycin and metoclopramide. Conclusion: This case exemplifies the difficulties in diagnosing and treating gastroparesis. An interesting correlation between parasympathetic surges and recovery in gastroparesis may be suggested by the surprising remission of symptoms following face injuries.

Abstract:We present a case involving a patient with laparoscopic banded sleeve gastrectomy (BSG) with a 3-month history of persistent vomiting, decreased tolerance for fluids, and limited intake of soft food items. Upon investigation, an eroded band and gastric dilatation were identified. The treatment involved the removal of the eroded band and a segment of the stomach, followed by the restoration of gastric continuity through a gastrogastrostomy.
