Abstract:Objective To explore the risk factors of lower extremity deep venous thrombosis (DVT) after orthopedic surgery in orthopedic patients.Methods We retrospectively analyzed 500 patients who underwent surgery in orthopedics due to degenerative diseases of the spine from September 2016 to May 2019.All patients underwent ultrasound examination of the lower extremities before surgery to confirm the absence of thrombosis Double lower extremity ultrasound on the 3th,6th,9th and 12th postoperative day.According to the results of ultrasound,the patients were divided into DVT group and non-DVT group.A total of 65 cases of DVT were diagnosed and the DVT formation rate was 13％.The age,D-dimer,body mass index (BMI),comorbidity,preoperative and postoperative coagulation,operation time,Logistic multivariate regression analysis of hospitalization time,intraoperative blood loss,etc.to explore risk factors associated with DVT.Results A total of 500 patients were included for analysis,According to the final results,65 patients were included in the DVT group,and 435 were included in the non-DVT group.The incidence of DVT was 13％.Univariate analysis showed statistically significant differences in age,BMI,and hypertension between the two groups (P ＜ 0.05).Logistic regression analysis showed D-dimer [odds ratio (OR) =7.059,95％ confidence interval (CI) =2.864-17.401],OR =1.836,95％ CI =1.022-3.301) and operative time (OR =1.513,95％ CI =1.039-2.203) is a risk factor for postoperative lower extremity DVT.Conclusion Obesity,D-dimer and long operation time are independent risk factors for lower extremity DVT.

Abstract:Deep vein thrombosis (DVT) after total hip or knee arthroplasty is relatively common and is associated with significant morbidity and mortality, presenting a large clinical burden. The objective of this study was to investigate the incidence and risk factors for DVT after total joint arthroplasty. This retrospective review was conducted in a single institution. The patients received postoperative DVT chemoprophylaxis (Fraxiparine or Rivaroxaban), followed by venography to check for DVT on the third to ninth postoperative day. The clinical characteristics were summarized and analyzed using SPSS version 17.0. From March 2010 to July 2013, 963 patients were enrolled in the study. DVT chemoprophylaxis was administered to all of the 963 patients. DVT occurred in 173 subjects (17.96%). Univariate analysis showed that age (P<0.001), BMI (P<0.01), cholesterol (P<0.001), high-density lipoprotein (P<0.001), apolipoprotein A (P<0.01), history of tumor (P<0.05), visual analogue scale (VAS) pain score (P<0.001), and operative duration (P<0.05) were significant risk factors for DVT. The incidence of DVT was 17.96% after total joint arthroplasty with prophylactic antithrombotic therapy. Age, BMI, cholesterol, high- density lipoprotein, apolipoprotein A, history of tumor, VAS score, and operative duration were significant risk factors for DVT. Copyright (C) 2017 The Author(s). Abstract:OBJECTIVE:To analyze the clinical risk factors for deep vein thrombosis (DVT) after total hip and knee arthroplasty in Chinese patients who received prophylactic treatment for DVT.METHODS:We evaluated 128 total hip arthroplasty (THA) and total knee arthroplasty (TKA) in 95 patients performed at our center from April 2004 to August 2004, which included 48 THAs in 43 patients and 80 TKAs in 52 patients. There were 27 men and 68 women with a mean age of 59.77 years (range, 23-78 years). All patients had been given low-molecular-weight heparin before operation and for 7-10 days post-operation to prevent DVT. Color Doppler ultrasonography was used to detect DVT of bilateral lower extremities in all patients before operation and at 7-10 days after operation. Nineteen clinical factors were examined preoperation and 7-10 days post-operation in order to analyze their influences on DVT formation after surgery.RESULTS:There were 45 patients who developed DVT after operation. The incidence of DVT in all patients was 47.4% (45/95) and the incidence of proximal DVT was 3.2%. There were more asymptomatic DVT (57.8%, 26/45) than symptomatic ones, and some patients without DVT (14%, 7/50) presented some of the DVT symptoms. Logistic regression analysis demonstrated a definite association of female, obesity (representative by BMI), cement usage and diagnosed RA with DVT with odds ratio of 10.008, 3.094, 8.887, and 0.194 respectively. Other clinical factors had no statistically significant association with DVT.CONCLUSIONS:Female, obesity, and cement usage were the risk factors for DVT after THA and TKA, and diagnosed RA was the protecting factors for DVT after THA and TKA. Other clinical factors such as age, OA, type of implant, monolateral or bilateral operation, duration of anesthesia, surgery and bandage usage for blood control, time for immobilization et al were not the risk factors for DVT.

Abstract:Objective To investigate the incidence rate and prevention of deep vein thrombosis (DVT) after total hip arthroplasty (THA). Methods From May 2007 to December 2008, 141 patients with THA underwent deep vein anterograde venagraphy of lower limb in order to find the location, size and num-ber of thrombus. There were 53 males and 88 females in our study. The patients ranged from 17 to 86 years old, with an average age of (63.20±13.78) years. The patients were randomly divided into anticoagulation group and non-anticoagulation group according to the use of low-molecular-weight heparin (LMWH) or not. There were 82 patients in non-anticoagulation group and 59 ptients in non-anticoagulation group. The diag-nosis of DVT was according to the criterion recommended by Rabinov and Paulin in 1972, combining with the experience of the civil scholars. Depending on the finding of deep vein anterograde venography, the DVT was classified into proximal type, distal and mixed ones. Results The incidence rate of DVT after THA was 32.62% (46 patients). There were proximal DVT in one case, mixed DVT in two cases, distal DVT in 43 cases. Thirty-four patients (73.9%) had not obvious clinical signs and symptoms. There was no difference in incidence rate between the non-anticoagulation group (59 patients) and anticoagulation group (82 patients). Conclusion The incident of DVT after THA can be found accurately by digital lower limb venography technology. There is no direct relation between clinical DVT and clinical evidence. LHMN can decrease the incident rate, but there is no statistical significance. It is necessary to adopt individual thromboembolism prophylaxis measures.

Abstract:Background: Deep vein thrombosis (DVT) remains a life-threatening complication of arthroplasty. It remains controversial for anticoagulation strategies after total hip arthroplasty (THA). Methods: A retrospective study was conducted to determine whether clinical risk factors and/or genetic variants might predict the development of DVT and find efficient anticoagulation strategies. Nanjing DVT study (NJDVTS), a retrospective and largest study conducted in a single center included 908 subjects who received knee or hip arthroplasty. Results: A total of 328 subjects (215 women and 113 men) who underwent uncemented THA were studied. Patients were followed up 3 months later after surgery. DVT was tested by contrast venography (CV). We checked genetic variants for DVT by a case-control study. Logistic regression analysis was performed to predict of the ability of each covariate and genetic factor. We also used logistic regression analysis with variable selection for obtaining the prediction model of DVT. Conclusions: DVT after THA was affected by personal (age) and clinical factors (mechanical compression, duration of surgery). THA with short duration of surgery did not require prophylactic anticoagulation. Extended anticoagulation was unnecessary for non-DVT patients after discharge from hospital.
Risk Factors Associated with Thromboembolic Complications After total Hip Arthroplasty: An Analysis of 1,129 Pulmonary Emboli

Abstract:Introduction: Venous thromboembolism (VTE) remains a dangerous complication after total hip arthroplasty (THA), despite advances in chemoprophylactic measures. This study aimed to identify risk factors of developing pulmonary embolism (PE) and deep vein thrombosis (DVT) after THA using a modern cohort of patients reflecting contemporary practices. Methods: The Premier Healthcare Database was queried for primary, elective THAs from January 1st, 2015, to December 31st, 2021. Patients who developed PE or DVT within 90 days of THA were compared with patients who did not develop any postoperative VTE. Differences in patient demographics, comorbidities, hospital factors, perioperative medications, chemoprophylactic agents, and allogeneic blood transfusion were compared between cohorts. Multivariable logistic regression models were used to identify independent risk factors of PE and DVT. In total, 544,298 THAs were identified, of which 1,129 (0.21%) developed a PE and 1,799 (0.33%) developed a DVT. Results: Patients diagnosed with a PE had significantly higher rates of in-hospital death (2.6% vs 0.1%, P < 0.001) compared with those without a PE. Age (adjusted odds ratio: 1.02 per year, 95% confidence interval [CI]: 1.01 to 1.03) and Black race (aOR: 1.52, 95% CI: 1.24 to 1.87) were associated with an increased risk of PE. Comorbidities associated with increased risk of PE included chronic pulmonary disease (aOR: 1.58, 95% CI: 1.36 to 1.84), pulmonary hypertension (aOR: 2.06, 95% CI: 1.39 to 3.04), and history of VTE (aOR: 2.38, 95% CI: 1.98 to 2.86). Allogeneic blood transfusion (aOR: 2.40, 95% CI: 1.88 to 3.06) was also associated with an increased risk of PE while dexamethasone utilization was associated with a reduced risk (aOR: 0.83, 95% CI: 0.73 to 0.95). Discussion: Increasing age; Black race; allogeneic blood transfusion; and comorbidities, including chronic pulmonary disease, pulmonary hypertension, and history of VTE, were independent risk factors of PE after THA. Given the increased mortality associated with PE, patients should be carefully evaluated for these factors and managed with an appropriate chemoprophylactic regimen.

Abstract:Objective:To investigate the risk factors of deep venous thrombosis in patients before hip arthroplasty, and to explore the possible causes of postoperative thrombosis.Methods:The clinical data of 361 patients with hip arthroplasty treated in the Department of Orthopaedic Trauma of Xi′an Honghui Hospital from September 2015 to December 2019 were studied retrospectively, including 102 males and 259 females, aged 65 to 94 years, and the average age was 72.25 years old. All fracture patients were given subcutaneous injection of low molecular weight heparin calcium to prevent lower extremity thrombosis. The deep veins of both lower extremities were examined before and after operation. The general data of the two groups of patients were collected and recorded, including age, sex, whether complicated with medical diseases (essential hypertension, type 2 diabetes, coronary heart disease), serological indexes, time from injury to admission, and time from admission to operation. The software of SPSS 19.0 was used for statistical analysis.Results:The incidence of lower limb DVT, before operation was 29.92%, including 26 males (24.07%) and 82 females (75.93%). The results of multivariate logistic regression analysis showed that diabetes mellitus ( OR=2.127, 95% CI: 1.134-3.989, P=0.019), coronary heart disease ( OR=1.692, 95% CI: 1.056-2.713, P=0.029) and the time from injury to admission ( OR=1.677, 95% CI: 1.037-2.712, P=0.035) were independent risk factors for DVT in elderly patients undergoing hip arthroplasty. The incidence of lower limb DVT, after operation was 46.54%. After operation, proximal thrombus were occurred in 2 cases (1.19%), distal thrombus in 143 cases (85.12%), and mixed thrombus in 23 cases (13.69%). Postoperative thrombus was ipsilateral to the fracture limb in 84 cases (50.00%), thrombus was located in the healthy side of the fracture in 19 cases (11.31%), and DVT occurred in 65 cases (38.69%) in both lower limbs. Conclusions:Delayed admission longed than 48 hours, coronary heart disease and diabetes mellitus are the risk factors for the formation of DVT. The thrombus that existed before operation and did not disappear after operation accounted for 48.81% of the total incidence of postoperative thrombosis, and the new thrombus accounted for 51.19% of the total incidence of postoperative thrombosis. For the elderly patients with femoral neck fracture undergoing hip arthroplasty, ultrasonic examination of both lower limbs should be performed before and after operation to find the changes of thrombus in time and do a good job of prevention and treatment.

Abstract:Objective To investigate hemodynamic changes of deep vein in lower limb during the perioperative period of total hip arthroplasty (THA) and early diagnosis of deep vein thrombosis (DVT). Methods Doppler ultrasound, hemorheology detection and plasma D-dimer testing were done on 62 patients treated with THA. Statistical analysis was carried out on the data of patients with or without DVT to study the early diagnosis of DVT. Results The results of Doppler ultrasound showed DVT in 8 patients. Compared with postoperative concentration of plasma D-dimer, the preoperative con-centration of plasma D-dimer was significandy higher in patients with or without DVT (P < 0.05). The levels of hemorheological indices were significantly increased at postoperative day 7 (P < 0.05). Con-clusions Doppler ultrasound combined with plasma D-dimer testing and hemorheology detection are helpful in early diagnosis of DVT.
Risk Factors for Postoperative Deep Venous Thrombosis and Pulmonary Embolism Following Primary Total Hip Arthroplasty and Primary Total Knee Arthroplasty

Abstract:Pulmonary embolism (PE) and deep vein thrombosis (DVT) are common postoperative complications. This study retrospectively analyzes preoperative attributes as risk factors for DVT or PE following primary total hip arthroplasty (THA) and total knee arthroplasty (TKA). Preoperative transfusion, age 65+, dyspnea with moderate exertion, body mass index (BMI) 24.9+ were independent risk factors for PE following THA. Cardiac comorbidities were related with reduced risk. Bleeding disorder, operative time > 110 minutes, perioperative transfusion, age 65+, dyspnea at rest, and BMI < 18.5 were independent risk factors for DVT following THA. Perioperative transfusion, age 57+, dyspnea at rest and with moderate exertion, and BMI 24.9+ were independent risk factors for PE following TKA. Smoking status was related to reduced risk. Perioperative transfusion, age 65+, dyspnea with moderate exertion, and male sex were independent risk factors for DVT following TKA. Cardiac comorbidities were related with reduced risk. Abstract:Objective:To analyze the incidence of deep venous thrombosis after total knee arthroplasty in patients with rheumatoid arthritis and to analyze the risk factors.Methods:One hundred and sixteen cases of rheumatoid arthritis patients undergoing total knee arthroplasty in the Department of Sports Medicine and Adult Reconstructive Surgery, Nanjing Drum Tower Hospital from March 2010 to December 2018 were retrospectively analyzed, including 12 males and 104 females. Inclusion criteria were definite diagnosis, standard total knee arthroplasty, and available for preoperative and postoperative examination of thrombosis. Those without definite diagnosis, treated with unicompartmental knee arthroplasty, with preoperative thrombosis or unable to accept relating examinations were excluded. Age, gender, body mass index, hypertension, diabetes, triglycerides, postoperative bed rest time and pain visual analogue scale score were recorded. The patients were divided into thrombosis group and non-thrombosis group according to the incidence of deep vein thrombosis, and the difference of figures were compared. Chi-square test was used to examine categorical variables, and paired t test was adopted for metering data. Logistic regression analysis was used to verify the correlation of various risk factors and thrombosis.Results:Seventeen cases developed deep vein thrombosis, and the incidence rate was 14.7% (17/116). All the thrombosis was stable and no pulmonary embolism occurred. Thrombosis group had higher average age than the non-thrombosis group[(62.8±8.8) vs (56.4±11.1)years, t =2.272, P=0.025], and the proportion of people over 65 years old in thrombosis group was 47.1%, higher than 21.2% of the non-thrombosis group (χ2=3.88, P=0.049). Multivariate regression analysis revealed that age ≥65 years was an independent risk factor of postoperative deep vein thrombosis (OR=1.265, P =0.032).Conclusions:The incidence of deep vein thrombosis after total knee arthroplasty in patients with rheumatoid arthritis is not low. Age ≥ 65 years is an independent risk factor for deep vein thrombosis after total joint replacement in patients with rheumatoid arthritis and it is suggested to conduct more aggressive thromboprophylaxis to those patients.

Abstract:Objective To explore the risk factors and predictors of deep venous thrombosis(DVT) after hip and knee arthroplasty,and to find out the effective method for DVT diagnosis. Method One hundred and sixteen patients were selected and analyzed through self-designed questionnaire.Result The risk factors for DVT included sex,age,operation type,anesthesia way,disease type,intensive care history and BMI.The diagnosis effect of lower limb swelling was much closer to that of color ultrasound.Most of our common Lab examination items about the diagnosis revealed no significance.Conclusion Sex,age,operation type,anesthesia way,disease type,intensive care history and BMI may be the risk factors of deep venous thrombosis after hip and knee arthroplasty.Swelling of lower limb has much value about the diagnosis of deep venous thrombosis.
Deep Vein Thrombosis After Total Hip Arthroplasty And Total Knee Arthroplasty In Patients With Previous Ischemic Stroke

Abstract:The present study evaluated the prevalence and therapy of deep vein thrombosis (DVT) after total hip arthroplasty (THA) and total knee arthroplasty (TKA) in 57 patients with previous ischemic stroke. Postoperative anticoagulants were used for DVT prophylaxis, and batroxobin and prolonged anticoagulants were used for thrombolysis in DVT subjects. The incidence of DVT after THA and TKA in patients with previous ischemic stroke was 16.2% and 20%, respectively. No bleeding complications were observed and no new ischemic stroke occurred during the following 3 months. The prevalence DVT after THA and TKA in patients with previous ischemic stroke was not specific, and the treatment of DVT with batroxobin and anticoagulants was effective and safe.

Abstract:BACKGROUND:Deep vein thrombosis (DVT) is common after total joint arthroplasty (TJA), and can cause the sequela of post-thrombotic syndrome (PTS), which is associated with decreased quality of life and increased treatment cost. The purpose of this study is to determine the incidence and risk factors for PTS in patients with DVT following primary unilateral total knee and hip arthroplasty. METHODS:We conducted this follow-up study involving patients developing DVT after primary unilateral total knee arthroplasty or total hip arthroplasty at our institution between April 2010 and March 2017. Each patient received a follow-up clinical interview regarding PTS-related symptoms and signs. We introduced demographic, clinical, and surgical data into the analysis to identify the risk factors for PTS. RESULTS:A total of 182 patients with postoperative DVT were enrolled with a mean follow-up time of 3.6 years. The incidence of PTS was 9.3% in patients developing DVT after TJA. Malignancy (P = .033), previous surgery in ipsilateral lower extremity (P = .013), and blood transfusion (P = .022) appeared to be the risk factors for PTS. CONCLUSION:We determined the incidence and risk factors for PTS in patients with DVT following TJA. Preventive measures should be used for patients at high risk of PTS.
Venous thromboembolism following primary total hip arthroplasty

Abstract:Aim: In the absence of thromboprophylaxis, venographically detected deep vein thrombosis (DVT) occurs in approximately 50% of patients undergoing primary total hip arthroplasty. Despite the existence of national and international guidelines, thromboprophylaxis may be underused. Methods: A retrospective review was performed of the clinical incidence of venous thromboembolism (VTE) and thromboprophylactic practice patterns over a nine year period. Patient baseline characteristics, VTE risk factors, prophylactic modalities (mechanical and pharmacological), operation duration, type of prosthesis and fixation, mode of anesthesia, hospital length of stay (LOS) were analyzed. The main efficacy outcome was DVT and/or pulmonary embolism (PE). The primary safety outcome was major bleeding. Results: In-hospital incidence of VTE was 2.5% and 3.8% up to three months post hospital discharge. Median time to postoperative VTE development in-hospital and after discharge was 6.5 days (IQR: 5.0 to 8.0 days) and 29.0 days (IQR: 19.5 to 38.0 days) respectively. 66.7% (95% CI: 30.0 to 90.3%) of all readmissions for VTE occurred within one month post-operatively. There were no readmissions for VTE in patients discharged on extended pharmacological prophylaxis. Conclusions: The use of prophylactic protocols was associated with relatively low VTE rates up to three months with minimal bleeding complications. A more intense in-hospital and extended prophylaxis beyond hospitalization is recommended in this high risk group of patients.

Abstract:[Objective] To analyze the common risk factors of deep venous thrombosis( DVT) after joint arthroplasty due to knee or hip osteoarthritis( OA). [Methods] From August 2010 to February 2012,456 patients who had undergone total knee or hip joint arthroplasty underwent venography of lower- extremity deep vein on postoperative days 3 to 5. Based on the results of angiography,patients were divided into DVT group( 22 males and 121 females,mean age 66. 97 years) and non- DVT group( 68 males and 245 females,mean age 65. 93 years). The potential associations of DVT risk and age,gender,diabetes,hypertension,history of cardiovascular disease,VTE history,history of malignancy,body mass index( BMI),smoking history,and types of surgery were analysed. [Results] In OA patients,a BMI value of or more than 25 kg /m2( OR =1.716;95% CI = 1. 100- 2. 675; P = 0. 017) and cardiovascular disease history( OR = 3. 403; 95% CI = 1. 129- 10. 260; P =0. 030) significantly increased DVT risk after joint arthroplasty. [Conclusion] A higher BMI value and cardiovascular disease history are risk factors of DVT following joint arthroplasty in patients with knee or hip OA.

Abstract:Objective To analyze the characteristics of proximal deep vein thrombosis (DVT) after total joint arthroplasty.Methods 40 cases of proximal DVT were collected from May 2007 to October 2016 which were all diagnosed by lower limb venography or sonography.There were 9 males and 31 females aged from 38 to 86 years,average 67 years,and body mass index was from 16.94 to 31.25kg/m2,with an average of 24.3 kg/m2.29 cases of THA patients and 11 cases of TKA patients.The anatomical distribution,size,radiological performance,local physical findings were recorded and multivariable analysis was performed to evaluate the relationship between distribution of proximal DVT and related parameters including age,gender (male/female),side of leg(left/right),surgery type(knee and hip),preoperative diagnosis(femoral neck fracture/other disease),time of diagnosis with DVT (during hospitalization/after discharge).Results Among 40 proximal DVT cases,31 cases diagnosed by phlebography and 9 cases were diagnosed by ultrasound.2 cases were isolated proximal DVT while 38 cases connected with distal DVTs.9 cases of blood clots in the proximal and distal thrombosis connected discontinuously.There was no isolated proximal DVT in TKA group and 10 cases (90.9％) of blood clots in the proximal and distal thrombosis connected continuously while there were 2 cases (6.9％) isolated proximal DVTs in THA group and 21 (77.8％) of blood clots in the proximal and distal thrombosis connected discontinuously,and there was no statistic significant difference between the two groups.The average length of proximal was 8.85±9.3 1cm (range from 2-35cm) with 5.0±2.05 cm in TKA group and 10.31 ± 10.55 cmin THA group.A significant difference was found between the 2 groups (P=0.014).13 cases located in femoral veins and upper venous region which were all from THA group.There was a significant difference in the distribution between surgery type and preoperative diagnosis.However,there was no significant difference between other potential factors and distribution of proximal DVT.25 cases accepted the evaluation of symptom of DVT before scanning.However,no significant difference were found in edema,VAS score,Homans sign,Neuof sign between 9 TKA and 16 THA cases.Conclusion Proximal DVTs after knee and hip arthroplasty are more preferred to connect with distal DVT continuously.Surgery type and preoperative diagnosis have a significant effect on the distribution of proximal DVT.Proximal DVTs in THA and patients with preoperative femoral neck fracture could be more easily involving femoral veins and upper parts.

Abstract:Venous thromboembolism (VTE) is a common complication after major orthopedic surgery. However, the reported rates of deep venous thrombosis (DVT) vary widely. Our aim was to study the incidence of DVT in the nonoperated leg after primary major lower extremity arthroplasty using bilateral venography. The records of patients who underwent total hip arthroplasty (THA) or total knee arthroplasty (TKA) at our institution between August 2013 and August 2014 were reviewed. We examined the records for a total of 380 patients, of which 244 had undergone bilateral venography 3-5 days after THA or TKA. A total of 244 patients undergoing TKA (n=119) or THA (n=125) were recruited for this study. DVT was diagnosed in 42 (17.2%) of the 244 patients; nine patients developed DVT in both legs, and four developed DVT only in the nonoperated leg. All of the DVTs were located in calf veins. DVT occurred more frequently after TKA than THA, especially in muscular veins. Female sex and older age were found to be positive risk factors for DVT after THA. The incidence of DVT after major lower arthroplasty is high. DVT in nonoperated legs should be carefully considered because of its potential risks, especially in left. Distal DVT also has a high risk to develop pulmonary embolism (PE). The combination of anticoagulant and pneumatic compression is a good measure to avoid proximal DVT, and it is well tolerated to stop anticoagulant if the patients were diagnosed as having no DVT. Risk factors of deep vein thrombosis in patients after artificial joint replacement surgery

Abstract:Objective To investigate the incidences and risk factors of deep venous thrombosis in patients after artificial joint replacement surgery. Methods Clinical data of 123 patients receiving artificial joint replacement surgery from July 2009 to May 2011 were retrospectively analyzed, among these 123 cases, DVT occurred in 98 patients. Results The incidence of DVT was 20.3% , univariate analysis showed that age, gender,body mass index (BMI), surgical sites, surgical time, the use of bone cement were significantly correlated with DVT. The incidences of DVT in obese women with 60 -70 year-old and in patients receiving bone cement werehigher than that in controls. Conclusion There were complex risk factors of DVT after artificial joint replacement surgery. For 60-70 year-old obese woman, the preoperative preparation and care should be prepared to avoid the incidence of DVT.
