Abstract:Background: Computer-aided designed 3D printing guide plate can be used to assist the screw implantation. At present, there is still lack of clinical study on the application of 3D printing guide plate to assist multiple segment pedicle screw implantation in the treatment of cervical spinal canal tumor. Objective: To investigate the clinical outcomes of 3D printing guide plate assisted pedicle screw implantation in the operation of multiple segmental primary tumor of cervical spi-nal canal. Methods: A total of 47 patients with primary tumor of cervical spinal canal were treated with multiple segmental pedicle screw implantation from July 2010 to April 2015 were enrolled in this retrospective study. There were 28 males and 19 females with a mean age of (56.3±12.2) years old (range, 39-79 years). The 3D printing guide plate was used in 20 of them (3D printing group), but not used in the other 27 patients (traditional group). The operative time, bleeding volume, screw accuracy (Kawaguchi grade evaluation), visual analogue scale (VAS) score, and Japanese Orthopaedic Association (JOA) cervical nerve function score were compared between the two groups. Results: The operation was successfully per-formed in all the patients, and the lesions were completely removed. The operative time was (191±25) min in 3D printing group, which was significantly shorter than that in traditional group ([229±27] min, t=-5.077, P=0.000). The amount of intra-operative bleeding was (383±83) ml in 3D printing group, which was significantly less than that in traditional group ([445± 76] ml, t=-2.627, P=0.012). The accuracy of the screw implantation was as follows according Kawaguchi grade evaluation:in 3D printing group, grade 0 accounted for 93% (119/128), grade 1 was 5.4% (7/128), grade 2 was 1.6% (2/128); in tradi-tional group, grade 0 accounted for 84.3% (118/140), grade 1 was 7.9% (11/140) and grade 2 was also 7.9% (11/140). There was statistical difference in the accuracy of the screw implantation (grade 0) between the two groups (P=0.026). At 3 months after surgery, the average VAS score was 2.5±0.5 and 2.3±0.5 in the 3D printing group and traditional group, respec-tively (t=1.143, P=0.259); the average JOA score was 13.6±1.4 and 13.1±1.7 in the 3D printing group and traditional group, respectively (t=0.862, P=0.393). Conclusions: The accuracy of 3D printing guide plate assisted cervical pedicle screw place-ment is high, which can obviously shorten the operation time and the amount of bleeding.
