Abstract:Background: Lung cancer remains a critical public health issue, presenting multifaceted challenges in prevention, diagnosis, and treatment. This article aims to review the current landscape of lung cancer research and management, delineate the persistent challenges, and outline pragmatic solutions. Materials and methods: Global experts from academia, regulatory agencies such as the Food and Drug Administration (FDA) and the European Medicines Agency (EMA), the National Cancer Institute (NCI), professional societies, the pharmaceutical and biotech industries, and patient advocacy groups were gathered by the New York Lung Cancer Foundation to review the state of the art in lung cancer and to formulate calls to action. Results: Improving lung cancer management and research involves promoting tobacco cessation, identifying individuals at risk who could benefit from early detection programs, and addressing treatment-related toxicities. Efforts should focus on conducting well-designed trials to determine the optimal treatment sequence. Research into innovative biomarkers and therapies is crucial for more personalized treatment. Ensuring access to appropriate care for all patients, whether enrolled in clinical trials or not, must remain a priority. Conclusions: Lung cancer is a major health burden worldwide, and its treatment has become increasingly complex over the past two decades. Improvement in lung cancer management and research requires unified messaging and global collaboration, expanded education, and greater access to screening, biomarker testing, treatment, as well as increased representativeness, participation, and diversity in clinical trials.

Abstract:Lung cancer, a multifaceted disease, demands tailored therapeutic approaches due to its diverse subtypes and stages. This comprehensive review explores the intricate landscape of lung cancer research, delving into recent breakthroughs and their implications for diagnosis, therapy, and prevention. Genomic profiling and biomarker identification have ushered in the era of personalised medicine, enabling targeted therapies that minimise harm to healthy tissues while effectively combating cancer cells. The relationship between pulmonary tuberculosis and lung cancer is examined, shedding light on potential mechanisms linking these two conditions. Early detection methods, notably low-dose computed tomography scans, have significantly improved patient outcomes, emphasising the importance of timely interventions. There has been a growing interest in segmentectomy as a surgical intervention for early-stage lung cancer in recent years. Immunotherapy has emerged as a transformative approach, harnessing the body's immune system to recognise and eliminate cancer cells. Combining immunotherapy with traditional treatments, such as chemotherapy and targeted therapies, has shown enhanced efficacy, addressing the disease's heterogeneity and overcoming drug resistance. Precision medicine, guided by genomic profiling, has enabled the development of targeted therapies like tyrosine kinase inhibitors, offering personalised treatments tailored to individual patients. Challenges such as drug resistance and limited accessibility to advanced therapies persist, emphasising the need for collaborative efforts and innovative technologies like artificial intelligence. Despite challenges, ongoing interdisciplinary collaborations and technological advancements offer hope for a future where lung cancer is treatable and preventable, reducing the burden on patients and healthcare systems worldwide.

Abstract:The management of lung cancer has evolved very rapidly in the past decade. The introduction of targeted and immunotherapies have significantly improved outcomes especially in advanced stage disease (1), endobronchial technology to improve early diagnostic yields (2), robotic surgery to perform challenging surgery in more precise manner (3), and liquid biopsies to diagnose and guide treatments. However, these past great leaps in lung cancer management, as Sir Winston Churchill once said, "... is not the end. It is not even the beginning of the end. But it is perhaps, the end of the beginning." In this special series of "Lung Cancer Management—The Next Decade", we have invited some of the top clinician scientists in the field, many of them pioneers and trailblazers in their topics, to provide us with their unique perspectives. The articles cover the latest developments in: (I) the use of circulating tumour DNA and the potentials it holds [Lam et al. (4)]; (II) the current and future technologies of using robotic bronchoscopy [Ho et al. (5)] and cone beam computed tomography (CT) scan for accurate tissue biopsy acquisition [Verhoeven et al. (6)]; (III) the state of the art in non-invasive and minimally invasive surgical approaches to treat lung cancer by transbronchial ablation therapies [Chan et al. Are We Losing the Final Fight against Cancer?

Abstract:Despite our increasing understanding of the biology and evolution of the cancer process, it is indisputable that the natural process of cancer creation has become increasingly difficult to cure, as more mutations are found with age. It is significantly more difficult to challenge the curative method when there is heterogeneity within the tumor, as it hampers clinical and genetic categorization. With advances in diagnostic technologies and screening leading to progressive tumor shrinkage, it becomes more difficult over time to evaluate the effects of treatment on overall survival. New treatments are often authorized based on early evidence, such as tumor response; disease-free, progression-free, meta-static-free, and event-free survival; and, less frequently, based on clinical endpoints, such as overall survival or quality of life, when standard guidelines are not available to approve pharmaceuticals. These clearances usually happen quite rapidly. Although approval takes longer, relative survival demonstrates the genuine worth of a novel medication. Pressure is being applied by pharmaceutical companies and patient groups to approve "new" treatments based on one of the above-listed measures, with results that are frequently insignificantly beneficial and frequently have no impact on quality of life.

Abstract:Purpose of review Current risk stratification and treatment decision-making for bladder cancer informed by histopathology as well as molecular diagnostics face limitations. This review summarizes recent advancements in single-cell and spatial omics methodologies for understanding bladder cancer biology and their potential impact on development of novel therapeutic strategies. Recent findings Single-cell RNA sequencing and spatial omics techniques offer unprecedented insights into various aspects of tumor microenvironment (TME), bladder cancer heterogeneity, cancer stemness, and cellular plasticity. Studies have identified multiple malignant cell subpopulations within tumors, revealing diverse transcriptional states and clonal evolution. Additionally, intratumor heterogeneity has been linked to tumor progression and therapeutic response. Immune cell composition analysis has revealed immunosuppressive features in the TME, impacting treatment response. Furthermore, studies have elucidated the role of cancer-associated fibroblasts and endothelial cells in shaping the tumor immune landscape and response to therapy. Summary Single-cell and spatial omics technologies have revolutionized our understanding of bladder cancer biology, uncovering previously unseen complexities. These methodologies provide valuable insights into tumor heterogeneity and microenvironmental interactions, with implications for therapeutic development. However, challenges remain in translating research findings into clinical practice and implementing personalized treatment strategies. Continued interdisciplinary collaboration and innovation are essential for overcoming these challenges and leveraging the full potential of single-cell and spatial omics in improving bladder cancer diagnosis and treatment.

Abstract:Abstract Lung cancer carcinogenesis is the outcome of a field of premalignant changes that occur in the bronchial tree leading to the overt emergence of the malignant lung cancer (field cancerization). A tissue or blood proteomic signature of these premalignant changes represent a novel concept of risk biomarkers that can predict the probability of future emergence of Lung Cancer. In this study, we analyzed two independent cohorts of non-small cell lung cancer (NSCLC) patients. One cohort consisted of 18 patients who donated blood before and after tumor resection surgery and matched tissue samples (UAMS Medical Center). The second set derived from a longitudinal case-control study of plasma sample collected during the physician health study that were donated at different times prior to the diagnosis of Non-Small Cell Lung Cancer (N=272). We used affinity-enriched mass spectrometry analysis and statistical learning techniques to identify a set of common markers that discriminated plasma cases and controls and were conserved in different bronchial tissue compartments including tumor and adjacent tissue. The tumor microenvironment adjacent to and surrounding cancer cells undergoes to progressive stages of carcinogenesis during field cancerization. We identified 22 proteins involved in cell adhesion, protein folding, immune response, angiogenesis, and cancer proliferation. This is a novel class of functional risk biomarkers to determine the probability of NSCLC onset. In conclusion, our findings shed light on a new class of functional biomarkers that underly tumor pathogenesis mechanisms and can provide hints for new strategies of tumor prevention therapy or lifestyle modification. Abstract:Colon cancer (CC) and rectal cancer (RC) are synonymously called colorectal cancer (CRC). Based on our experience in basic and clinical research as well as routine work in the field, the term CRC should be abandoned. We analyzed the available data from the literature and results from our multicenter Research Group Oncology of Gastrointestinal Tumors termed FOGT to confirm or reject this hypothesis. Anatomically, the risk of developing RC is four times higher than CC, while physical activity helps to prevent CC but not RC. Obvious differences exist in molecular carcinogenesis, pathology, surgical topography and procedures, and multimodal treatment. Therefore, we conclude that CC is not the same as RC. The term “CRC” should no longer be used as a single entity in basic and clinical research as well as other areas of classification.

Abstract:Liebe Leserinnen und Leser, schätzen Sie einmal, vor wie viel Jahren Platin-basierte Chemotherapie mit dem PEB-Regime Hodentumore zum Rollenmodell für heilbare metastasierte solide Tumoren machte. Sind es mehr als 20, 30 oder 40 Jahre? Es sind unglaubliche 46 Jahre, fast ein halbes Jahrhundert. Wenn wir die Entwicklung bei Urothelkarzinomen betrachten, so gab es hier mit Gemcitabine und Cisplatin ebenfalls langzeitig eine Behandlungskonstante. Allerdings mit dem Unterschied, dass diese hochgradig ineffektiv war und aktuell, nach "nur 24 Jahren", als Behandlungsstandart abgelöst wurde. Bei Prostatakarzinomen und Nierenzellkarzinomen änderten sich die Therapiestandards für metastasierte Stadien in den letzten Jahren nahezu jährlich. Heilbar wurden diese Tumoren hierdurch allerdings bisher nicht, vielmehr handelte es sich um Schrittinnovationen. Bei Hodentumoren gab es bei aller Konstanz in den letzten Jahren wichtige Verbesserungen bei der Diagnostik und Therapie, die wir zum Thema dieser Ausgabe machen. Klaus-Peter Dieckmann und Gazanfer Belge, die großen Verdienst bei der Evaluierung der microRNA-371a-3p (M371-Test) erworben haben, werden die Vor- und Nachteile dieses neuen Markers diskutieren. So viel sei verraten, dieser Tumormarker stellt einen für die Praxis wichtigen Fortschritt dar, muss jedoch mit Umsicht bewertet werden. Bei Tumormarkern ist die Zeit bei microRNA-371a-3p nicht stehen geblieben. Tim Nestler und Kollegen werden einen Ausblick auf aktuelle Entwicklungen und in die Zukunft geben. Die histopathologische Befundung von Keimzelltumoren hat sich in den letzten Jahren weiterentwickelt. Das Anforderungsprofil ist, wenn man beispielsweise die gegen Claudin 6 gerichtete CAR-T-Zelltherapie betrachtet, deutlich verändert. Worauf zu achten ist, diskutieren Felix Bremmer und Kollegen. Axel Heidenreich wird den Stellenwert der retroperitonealen Lymphadenektomie beim Seminom im klinischen Stadium IIA/B diskutieren. Ein Vorgehen, dass angesichts offensichtlicher Vorteile bereits jetzt außerhalb der bisherigen Leitlinienempfehlung in Deutschland regelmäßig durchgeführt wird. Wichtig hierbei aus meiner Sicht: Die Patienten sollten sehr gut über Vor- und Nachteile aufgeklärt werden, und der Eingriff sollte nur an Behandlungszentren durchgeführt werden. Es ist misslich, jedoch Fakt: Die Ergebnisse der bereits seit Jahren abgeschlossenen, weltweit durchgeführten TIGER-Studie (Konventionell dosierte oder Hochdosis-Salvagetherapie bei rezidivierten Hodentumoren) wurden bisher nicht veröffentlicht. Umso gespannter dürfen wir auf die Einschätzung von Christoph Oing und Marcus Hentrich zu dieser wichtigen Fragestellung sein. Ich verspreche Ihnen hochspannende Artikel zu den genannten Themen.
