Abstract:To mitigate outpatient care delivery inefficiencies induced by resource shortages and demand heterogeneity, this paper focuses on the problem of allocating and sequencing multiple medical resources so that patients scheduled for clinical care can experience efficient and coordinated care with minimum total waiting time. We leverage highly granular location data on people and medical resources collected via Real-Time Location System technologies to identify dominant patient care pathways. A novel two-stage Stochastic Mixed Integer Linear Programming model is proposed to determine the optimal patient sequence based on the available resources according to the care pathways that minimize patients' expected total waiting time. The model incorporates the uncertainty in care activity duration via sample average approximation. We employ a Monte Carlo Optimization procedure to determine the appropriate sample size to obtain solutions that provide a good trade-off between approximation accuracy and computational time. Compared to the conventional deterministic model, our proposed model would significantly reduce waiting time for patients in the clinic by 60%, on average, with acceptable computational resource requirements and time complexity. In summary, this paper proposes a computationally efficient formulation for the multi-resource allocation and care sequence assignment optimization problem under uncertainty. It uses continuous assignment decision variables without timestamp and position indices, enabling the data-driven solution of problems with real-time allocation adjustment in a dynamic outpatient environment with complex clinical coordination constraints.

What problem does this paper attempt to address?
### Problems Addressed by the Paper The paper aims to address the inefficiency in outpatient care delivery caused by the shortage of medical resources and the heterogeneity of demand. Specifically, the paper focuses on how to allocate and sequence multiple medical resources so that patients with scheduled clinical care can experience efficient and coordinated care, minimizing the total waiting time. ### Solution Overview 1. **Data Collection and Path Identification**: - Utilize Real-Time Location System (RTLS) technology to collect highly detailed location data of personnel and medical resources. - Identify major patient care paths through this data. 2. **Optimization Model**: - Propose a new two-stage Stochastic Mixed-Integer Linear Programming (SMILP) model to determine the optimal patient sequence based on available resources to minimize the expected total waiting time of patients. - The model handles the uncertainty of care activity durations through the Sample Average Approximation (SAA) method. - Use a Monte Carlo Optimization (MCO) procedure to determine the appropriate sample size to balance approximation accuracy and computation time. 3. **Experimental Results**: - Compared to traditional deterministic models, the proposed model can reduce patient waiting time by an average of 60% while maintaining acceptable computational resource requirements and time complexity. ### Main Contributions - **Innovation**: Propose a new two-stage SMILP model that effectively addresses the multi-resource allocation and care sequence assignment problem under uncertainty. - **Practicality**: Utilize RTLS data to provide real-time adjustment capabilities for resource allocation, suitable for dynamic outpatient environments with complex clinical coordination constraints. - **Efficiency**: Experimental validation shows that the model significantly improves efficiency in reducing patient waiting time. ### Keywords - Scheduling - Real-Time Location System Data - Care Path Management - Activity Duration Uncertainty - Stochastic Programming - Operations Research - Operations Management ### Conclusion The paper effectively addresses the inefficiency in care delivery in outpatient environments caused by resource shortages and demand heterogeneity by proposing a resource allocation and care sequence assignment framework based on RTLS data. This framework not only significantly reduces patient waiting time but also achieves efficient resource allocation and care coordination in dynamic environments.
