Abstract:This paper studies the real-life problems of outpatient clinics having the multiple objectives of minimizing resource overtime, patient waiting time, and waiting area congestion. In the clinic, there are several patient classes, each of which follows different treatment procedure flow paths through a multiphase and multiserver queuing system with scarce staff and limited space. We incorporate the stochastic factors for the probabilities of the patients being diverted into different flow paths, patient punctuality, arrival times, procedure duration, and the number of accompanied visitors. We present a novel two-stage simulation-based heuristic algorithm to assess various tactical and operational decisions for optimizing the multiple objectives. In stage I, we search for a resource allocation plan, and in stage II, we determine a block appointment schedule by patient class and a service discipline for the daily operational level. We also explore the effects of the separate strategies and their integration to identify the best possible combination. The computational experiments are designed on the basis of data from a study of an ophthalmology clinic in a public hospital. Results show that our approach significantly mitigates the undesirable outcomes by integrating the strategies and increasing the resource flexibility at the bottleneck procedures without adding resources.

Abstract:OBJECTIVE: Efficient scheduling of patient appointments on expensive resources is a complex and dynamic task. A resource is typically used by several patient groups. To service these groups, resource capacity is often allocated per group, explicitly or implicitly. Importantly, due to fluctuations in demand, for the most efficient use of resources this allocation must be flexible.METHODS: We present an adaptive approach to automatic optimization of resource calendars. In our approach, the allocation of capacity to different patient groups is flexible and adaptive to the current and expected future situation. We additionally present an approach to determine optimal resource openings hours on a larger time frame. Our model and its parameter values are based on extensive case analysis at the Academic Medical Hospital Amsterdam.RESULTS AND CONCLUSION: We have implemented a comprehensive computer simulation of the application case. Simulation experiments show that our approach of adaptive capacity allocation improves the performance of scheduling patients groups with different attributes and makes efficient use of resource capacity.

Abstract:To mitigate outpatient care delivery inefficiencies induced by resource shortages and demand heterogeneity, this paper focuses on the problem of allocating and sequencing multiple medical resources so that patients scheduled for clinical care can experience efficient and coordinated care with minimum total waiting time. We leverage highly granular location data on people and medical resources collected via Real-Time Location System technologies to identify dominant patient care pathways. A novel two-stage Stochastic Mixed Integer Linear Programming model is proposed to determine the optimal patient sequence based on the available resources according to the care pathways that minimize patients' expected total waiting time. The model incorporates the uncertainty in care activity duration via sample average approximation.We employ a Monte Carlo Optimization procedure to determine the appropriate sample size to obtain solutions that provide a good trade-off between approximation accuracy and computational time. Compared to the conventional deterministic model, our proposed model would significantly reduce waiting time for patients in the clinic by 60%, on average, with acceptable computational resource requirements and time complexity. In summary, this paper proposes a computationally efficient formulation for the multi-resource allocation and care sequence assignment optimization problem under uncertainty. It uses continuous assignment decision variables without timestamp and position indices, enabling the data-driven solution of problems with real-time allocation adjustment in a dynamic outpatient environment with complex clinical coordination constraints.

Abstract:A software agent may be a member of a Multi-Agent System (MAS) which is collectively performing a range of complex and intelligent tasks. In the hospital, scheduling decisions are finding difficult to schedule because of the dynamic changes and distribution. In order to face this problem with dynamic changes in the hospital, a new method, Distributed Optimized Patient Scheduling with Grouping (DOPSG) has been proposed. The goal of this method is that there is no necessity for knowing patient agents information globally. With minimal information this method works effectively. Scheduling problem can be solved for multiple departments in the hospital. Patient agents have been scheduled to the resource agent based on the patient priority to reduce the waiting time of patient agent and to reduce idle time of resources.

Abstract:This study is concerned with the determination of an optimal appointment schedule in an outpatient-inpatient hospital system where the inpatient exams can be cancelled based on certain rules while the outpatient exams cannot be cancelled. Stochastic programming models were formulated and solved to tackle the stochasticity in the procedure durations and patient arrival patterns. The first model, a two-stage stochastic programming model, is formulated to optimize the slot size. The second model further optimizes the inpatient block (IPB) placement and slot size simultaneously. A computational method is developed to solve the second optimization problem. A case study is conducted using the data from Magnetic Resonance Imaging (MRI) centers of Lahey Hospital and Medical Center (LHMC). The current schedule and the schedules obtained from the optimization models are evaluated and compared using simulation based on FlexSim Healthcare. Results indicate that the overall weighted cost can be reduced by 11.6% by optimizing the slot size and can be further reduced by an additional 12.6% by optimizing slot size and IPB placement simultaneously. Three commonly used sequencing rules (IPBEG, OPBEG, and a variant of ALTER rule) were also evaluated. The results showed that when optimization tools are not available, ALTER variant which evenly distributes the IPBs across the day has the best performance. Sensitivity analysis of weights for patient waiting time, machine idle time and exam cancellations further supports the superiority of ALTER variant sequencing rules compared to the other sequencing methods. A Pareto frontier was also developed and presented between patient waiting time and machine idle time to enable medical centers with different priorities to obtain solutions that accurately reflect their respective optimal tradeoffs. An extended optimization model was also developed to incorporate the emergency patient arrivals. The optimal schedules from the extended model show only minor differences compared to those from the original model, thus proving the robustness of the scheduling solutions obtained from our optimal models against the impacts of emergency patient arrivals.

Abstract:Hospitals are among the largest and most sophisticated service organizations and the most critical service delivery units in the health system. Due to the high risk of hospital services, the services provided must be of acceptable quality. Also, patient scheduling and timely receiving of services in medical centers can lead to patient satisfaction. In this study, a patient admission scheduling is modeled in which it is assumed that the staff is not always available. To enhance the quality of healthcare services and increases patient satisfaction, in this research, mathematical modeling is presented, in which resource capacity and treatment sequence constraints are considered in the proposed model. Furthermore, the uncertainty in the service quality parameter is taken into account, which makes the model more realistic. In this regard, first, a robust optimization approach based on the Bertsimas and Sim model is applied. Then, due to its Np-hardness, the multi-objective particle swarm optimization algorithm is proposed. Finally, the quality of the proposed algorithm is examined by comparing its results with the GAMS solver and the NSGA-II algorithm in small and large-size instances, respectively. Results indicate the proper performance of the proposed algorithm.

Abstract:An important challenge confronting healthcare is the effective management of access to primary care. Robust appointment scheduling policies/templates can help strike an effective balance between the lead-time to an appointment (a.k.a. indirect waiting time, measuring the difference between a patient's desired and actual appointment dates) and waiting times at the clinic on the day of the appointment (a.k.a. direct waiting time). We propose methods for identifying effective appointment scheduling templates using a two-stage stochastic mixed-integer linear program model. The model embeds simulation for accurate evaluation of direct waiting times and uses sample average approximation method for computational efficiency. The model accounts for patients' no-show behaviors, provider availability, overbooking, demand uncertainty, and overtime constraints. The model allows the scheduling templates to be potentially updated at regular intervals while minimizing the patient expected waiting times and balancing provider utilization. Proposed methods are validated using data from the U.S. Department of Veterans Affairs (VA) primary care clinics.

Abstract:The demand for outpatient services is rapidly growing, resulting in multifaceted challenges due to capacity limitations. This research aims to analyse the patient waiting time in a multi-specialty ophthalmic outpatient clinic using data analysis and discrete event simulation (DES). The patient arrivals, the duration for pre-consultation and post-consultation services are highly uncertain. At first, a linear regression analysis is performed using electronic health record (EHR) log data, and the significant factors that affect patient waiting time are found. Further, a discrete event simulation model of the outpatient clinic is built using FlexSim Healthcare software (5.3) and validated. Improvement scenarios, namely: 1) adding resources; 2) introducing fixed interval appointment scheduling; 3) combining scenarios 1 and 2, are proposed for reducing the patient waiting time and evaluated. From the simulation results, it is inferred that scenario 3 reduces the average waiting time of the patients to 12.45 minutes from 38.37 minutes.

Abstract:No-shows and non-punctual appointments have always been uncertain factors faced by managers of service-oriented enterprises and institutions, which usually lead to a low utilization rate of resources and a rapid decline in service satisfaction. Taking clinics as an example, this paper proposes a novel and efficient real-time sequencing strategy to minimize the cost associated with patient waiting time, provider idle time and overtime considering no-shows and unpunctuality. Four types of patient waiting time are considered for the first time, based on which the developed real-time sequencing strategy is used for scheduling the waiting patients when the provider becomes idle. Then, a biased random-key genetic algorithm is adopted to determine the number of patients on appointment slots and the length of each appointment slot. Extensive computational experiments show that the derived real-time sequencing strategy achieves a significant cost reduction compared with the famous FCFS (first come first served) and the state-of-art LAR (the larger of appointment time and real arrival time) rules.

Abstract:With the advancement of surgery and anesthesiology in recent years, surgical clinical pathways have changed significantly, with an increase in outpatient surgeries. However, the surgical scheduling problem is particularly challenging when inpatients and outpatients share the same operating room blocks, due to their different characteristics in terms of variability and preferences. In this paper, we present a two-phase stochastic optimization approach that takes into account such characteristics, considering multiple objectives and dealing with uncertainty in surgery duration, arrival of emergency patients, and no-shows. Chance Constrained Integer Programming and Stochastic Mixed Integer Programming are used to deal with the advance scheduling and the allocation scheduling, respectively. Since Monte Carlo sampling is inefficient for solving the allocation scheduling problem for large size instances, a genetic algorithm is proposed for sequencing and timing procedures. Finally, a quantitative analysis is performed to analyze the trade-off between schedule robustness and average performance under the selection of different patient mixes, providing general insights for operating room scheduling when dealing with inpatients, outpatient, and emergencies.

Abstract:This paper aims to optimise healthcare resource pre-positioning, patient scheduling, and patient transferring under uncertain demands and stochastic resource consumption. We propose a two-stage stochastic programming model that formulates the patient scheduling problem as a Markov decision process. To address complexities and uncertainties, we use Artificial Intelligence (AI) techniques to improve both model formulation and algorithmic performance. To tackle the limited data challenge, we introduce a Wasserstein distance-based ambiguity set and propose a two-stage distributionally robust optimisation (DRO) model, which derives a deterministic equivalent using the Lagrangian dual of non-anticipativity constraints. The solution process is accelerated with a scenario decomposition approach and the K-means clustering method. Both theoretical and numerical results demonstrate the consistency of the two-stage DRO model with the sample average approximation (SAA) method. The potential of AI to improve the model's performance is evident through the significant reduction in computation time achieved with the K-means clustering approach without compromising solution quality. Compared to the SAA method, the DRO model exhibits a considerable reduction in the waiting penalty cost for out-of-sample cases, ranging from 43.06% to 81.23% . Numerical results show that the proposed algorithm outperforms SAA methods and several benchmark policies in terms of computational efficiency and solutions.

Abstract:Bresol–a poly-herbal formulation, has been reported to be effective against bronchial asthma and allergic rhinitis in children. In vivo studies have supported the anti-histaminic and anti-anaphylactic action of bresol. However, the mechanism of action of bresol in modulation of inflammation has not been studied at the cellular and molecular level. The present study was aimed to elucidate the mechanism(s) of action of bresol at the cellular and molecular levels, using human monocyte leukemia cells. The effects of bresol on phosphodiesterase 4B (PDE4B) gene expression were analyzed using human monocytic U937 leukemia cells. The ability of bresol to stimulate cAMP formation in these cells, as well as its effects on mediators of inflammation like tumor necrosis factor-α (TNFα), nitric oxide (NO), and cycloxygenase-2 (COX-2) in lipopolysaccharide (LPS)-stimulated U937 cells, were also studied. The results here indicated that bresol exhibited potential anti-inflammatory properties by inhibiting LPS-induced PDE4B gene expression in the cells. Bresol also dose dependently activated cAMP formation, and inhibited TNFα, NO, as well as COX-2 formation in the LPS-stimulated cells. Based upon the results, we concluded that the reported anti-inflammatory activity of bresol might be attributed to its abilities to inhibit PDE4B and thus elevate cAMP levels in human monocytes. The anti-inflammatory effects of bresol might also be a result of the capacity of bresol to modulate the formation of TNFα, NO, and COX-2 in monocytes.
Evaluating Appointment Postponement in Scheduling Patients at a Diagnostic Clinic

Abstract:Diagnostic clinics are among healthcare facilities that suffer from long waiting times which can cause medical issues and lead to increases in patient no-shows. Reducing waiting times without significant capital investments is a challenging task. We tackle this challenge by proposing a new appointment scheduling policy for such clinics that does not require significant investments. The clinic in our study serves outpatients, inpatients, and emergency patients. Emergency patients must be seen on arrival, and inpatients must be given next day appointments. Outpatients, however, can be given later appointments. The proposed policy takes advantage of this by allowing the postponement of the acceptance of appointment requests from outpatients. The appointment scheduling process is modeled as a two-stage stochastic programming problem where a portion of the clinic capacity is allocated to inpatients and emergency patients in the first stage. In the second stage, outpatients are scheduled based on their priority classes. After a detailed analysis of the solutions obtained from the two-stage stochastic model, we develop a simple, non-anticipative policy for patient scheduling. We evaluate the performance of this proposed, easy-to-implement policy in a simulation study which shows significant improvements in outpatient indirect waiting times.

Abstract:Walk-in patients can take a large portion of outpatient visits in comprehensive hospitals. In current common practice, an even number of appointments is assigned to each slot and patients with appointment are given a strictly higher priority than walk-in patients. As a result, walk-in patients usually wait longer and incur noticeable dissatisfaction. To improve their satisfaction, we propose an integrated optimization approach for patient-centred outpatient scheduling. This approach aims to allocate appointments in line with the temporal variation of patient walk-ins, and further develop a time-invariant threshold-based prioritization scheme to balance the service priorities between the two types of patients in real time. To substantiate this approach, we develop a prospect theory-based model to quantify each patient's real-time satisfaction. We apply discrete-event simulation-based optimization to maximize the average patient satisfaction over the two types of patients. To verify our integrated optimization approach, we conduct a pilot in the endocrine department of Shanghai Sixth People's Hospital. Our results suggest that our approach substantially decreases the waiting time of walk-in patients without much compromise on the satisfaction of patients who made advance appointment.
Multi-period and multi-resource operating room scheduling under uncertainty: A case study

Abstract:Efficient planning and scheduling is essential for timely treatment of patients and improving the quality of operating room services and activities. In the present study, attempts are made to investigate a multi-period and multi-resource operating room integrated planning and scheduling problem under uncertainty. To this end, a mixed integer linear programming model has been developed for minimizing the tardiness in surgeries, overtime and idle time. Constraints related to human resources, equipment, as well as beds in pre-operative holding unit, recovery unit, ward and intensive care unit are taken into consideration. The durations of surgeries and recoveries are assumed uncertain, and a robust optimization approach has been used to manage the uncertainty. Due to the complexity of the model and the inability to solve large-scale problems, a metaheuristic method based on the genetic algorithm and a constructive heuristic approach have been proposed. After setting the parameters of the solution approaches using the Taguchi method, numerical experiments are performed based on various instances, and the results obtained from solving the mathematical model are compared to the results of the proposed metaheuristic and heuristic approaches. The results indicate that the proposed methods have a very good performance and the heuristic approach outperforms the genetic approach because the objective function of the proposed constructive heuristic is on average, about 19% better than the objective function of the genetic approach. A case study is also conducted in a hospital. The results obtained from the comparison of the proposed approaches with the hospital scheduling show that overtime and idle time are significantly improved in the proposed approaches.

Abstract:Appointment scheduling (AS) plays a crucial role in outpatient clinic management. Traditional methods involve patient grouping using pre-defined rules and scheduling based on these groups. However, pre-defined rules may not adequately capture the heterogeneity in patients' service times (i.e., consultation duration). Advanced machine learning (ML) methods can address individual-level heterogeneity but pose challenges for practical scheduling. To strike a balance, we propose a data-driven AS decision support system, Cluster-Predict-Schedule ( CPS ), integrating both supervised and unsupervised ML for efficient patient grouping and scheduling. The novelty of CPS lies in its adaptability to service time heterogeneity through a data-driven approach, determining patient groups based on data rather than pre-defined rules. Additionally, CPS includes a generic and efficient algorithm for generating appointment templates adaptable to any number of patient groups. Our system's efficacy is demonstrated using a real-world dataset. Evaluated by the weighted sum of patient wait times, physician idle time, and overtime, CPS achieves up to 15.0% cost reduction compared to the FCFA (first-call, first-appointment) scheme and over 4.7% savings against the common New/Return classification with traditional sequencing candidate ( TSC ) rules. In addition, CPS enhances outpatient operational efficiency without compromising fairness.

Abstract:An efficient appointment scheduling system has a defining role in controlling wait times and improving the productivity of a large variety of service systems. This study addresses the variability and length of wait times. We reduce them by a form of restricted central intake. We believe it is the first study that expands the appointment scheduling-sequencing model to include multiple sites and incorporate clients' flexibility and priorities, and solves the large-size scheduling-sequencing problem in a decentralized manner. To make the study more practical, it should be compatible with the multi-stakeholder environment and consider their independency. Furthermore, the problem is large-size as it combines all requests from a geographical region into one stream. Therefore, a decentralized distributed algorithm is applied to solve the amended model. The solution approach is an ADMM-based combination of dual decomposition and augmented Lagrangian relaxation. For the application of this approach, this paper focuses on the outpatient appointment system due to its importance. Early diagnosis and prevention play a crucial role in community health and health system quality. However, patients often experience significant wait times for various diagnostic technologies worldwide. The approach is examined by a real situation of MRI in Ontario, Canada. It has been shown that this study provides better workload balance across hospitals, better responding to demand fluctuations, and alleviates excessive wait times. The computational results also show that the proposed solution method can be satisfactory in terms of accuracy, running time, and applicability. The approach developed in this study can be applicable to many practical applications of timing and sequencing, such as outpatient surgery, other diagnostic testing, home healthcare, and physical and mental therapies, as well as in other service industries beyond healthcare, like public consultations, government services.

Abstract:Overcrowding in outpatient department of hospitals has created the need for analyzing patient flow for enabling better patient outcomes. Managing queues and minimizing delays will increase patient satisfaction and render better outcomes for both the patient and the hospital. Digitization has led to adoption of technology, although in its nascent stage of implementation across many hospitals in the country. In addition to technological intervention, a quantitative approach to managing patient queues will improve the operational efficiency. In this study, observation data pertaining to time stamps of patient arrivals, service duration and departure were recorded for patients consulting a specialist in the OPD unit. A questionnaire was also administered to staff in the OPD unit through a snowball sampling technique to understand their perception of technological practices adopted in that unit. Further, Monte Carlo simulation technique has been applied to analyze queueing patterns and key performance indicators have been computed to enable the hospital manage patient flow, adopt practices to optimize delays and thereby enhance service operational efficiency.

Abstract:In this study, we provided a scheduling procedure which is a combination of machine learning and mathematical programming that minimizes the waiting time of higher priority outpatients. Outpatients who request for appointment in healthcare facilities have different priorities. Determining the priority of outpatients and allocating the capacity based on the priority classes are important concepts that have to be considered in the scheduling of outpatients. In this study, two stages are defined for scheduling an incoming outpatient. In the initial stage, we employed and evaluated four distinct clustering techniques; K-means clustering, agglomerative hierarchical clustering, DBSCAN, and OPTICS clustering to classify outpatients into priority classes and suggested the best pattern to cluster the outpatients. In the second stage, we modeled the scheduling problem as a Markov Decision Process (MDP) problem since the arrivals are uncertain and the decisions are taken at the end of each day after observing total requests. Due to the curse of dimensionality, we used the fluid approximation method to estimate the optimal solution of the MDP. our methodology is employed in a data set of Shaheed Rajaei Medical and Research Center, and we represented how our model works in prioritizing and scheduling outpatients.

Abstract:With the increasing demand of patients for limited healthcare services, e.g., expert physicians in general hospitals and the long on-site waiting time of patients, a real-time admission control (AC) policy was developed considering both distinction and fairness among heterogeneous patients. The AC policy chooses the customer for the next service from the waiting area to minimize the expected total disutility according to the current system state. A continuous-time Markov decision process in a finite horizon was developed, and a myopic optimal policy was derived using a switching curve to discriminate patient admission based on patient types and waiting time. The effect of each parameter on the switching curve is analyzed, and managerial insights are discussed. In the numerical experiment, an empirical case in the Pediatric Department of Peking University Third Hospital is conducted using simulation models. Three commonly used scheduling policies and the myopic policy (MP) are tested in both Markovian settings and a real-world scenario. Simulation results confirm the effectiveness of the MP, because the total disutility of patients is significantly alleviated. Furthermore, the service experience of patients is expected to improve with the application of the AC policy.
