Abstract:Background: The purpose of this study was to develop and validate a positioning method with hand-guiding and contact position feedback of robot based on a human-robot collaborative dental implant system (HRCDIS) for robotic guided dental implant surgery. Methods: An HRCDIS was developed based on a light-weight cooperative robot arm, UR5. A three-dimensional (3D) virtual partially edentulous mandibular bone was reconstructed using the cone bone computed tomography images. After designing the preoperative virtual implant planning using the computer software, a fixation guide worn on teeth for linking and fixing positioning marker was fabricated by 3D printing. The fixation guide with the positioning marker and a resin model mimicking the oral tissues were assembled on a head phantom. The planned implant positions were derived by the coordinate information of the positioning marker. The drilling process using the HRCDIS was conducted after mimicking the experimental set-up and planning the drilling trajectory. Deviations between actual and planned implant positions were measured and analyzed. Results: The head phantom experiments results showed that the error value of the central deviation at hex (refers to the center of the platform level of the implant) was 0.79 +/- 0.17 mm, central deviation at the apex was 1.26 +/- 0.27 mm, horizontal deviation at the hex was 0.61 +/- 0.19 mm, horizontal deviation at the apex was 0.91 +/- 0.55 mm, vertical deviation at the hex was 0.38 +/- 0.17 mm, vertical deviation at the apex was 0.37 +/- 0.20 mm, and angular deviation was 3.77 +/- 1.57 degrees. Conclusions: The results from this study preliminarily validate the feasibility of the accurate navigation method of the HRCDIS.
Effect of Clinical Experience on Accuracy of Implant Placement Using Dynamic Navigation and Static Guidance: an in vitro Study

Abstract:This study aimed to investigate the accuracy of implants placed by clinicians with varying dental implant experience using dynamic navigation (DN) and static guidance (SG). Sixty identical custom-made drillable maxillary models were fabricated, missing the right central incisor (#8) and left first molar (#14) with simulated gingival tissue. Models planned with a dynamic navigation system and guided surgery software were randomly allocated to an experienced clinician, an inexperienced clinician, and a nonexperienced clinician. The accuracy of implant placement was evaluated by overlaying the preoperative virtual surgical plan with the postoperative Cone Beam Computed Tomography (CBCT) scan. Deviations between the placed and planned implants were quantified using a mathematical algorithm. Independent-sample t-tests revealed significant differences (p < 0.001) in angular deviation but not in coronal and apical deviations when comparing dynamic navigation with static guidance for all three clinicians. One-way ANOVA and Tukey post-hoc test found no significant differences between clinicians of varying dental implant experience in dynamic navigation and static guidance groups. The study concluded that the level of dental implant experience and surgical site do not significantly impact the accuracy of implant placement when either dynamic navigation or static guidance is used. Dynamic navigation produced less angular deviation in comparison with static guidance. This finding suggests dynamic navigation could benefit clinical settings, particularly for less experienced clinicians.
Surgical performance of dental students using computer-assisted dynamic navigation and freehand approaches

Abstract:Introduction: Nowadays, the training of implant placement has shifted from once entirely instructor-student teaching to the increasing use of computer-assisted simulation. Based on computerized virtual planning, dynamic navigation has been used for implant placement with higher accuracy than the traditional freehand protocol. However, whether dynamic navigation benefits to the training of dental students in implant placement remains controversial. This study aimed to compare the surgical performance of dental students in implant placement using computer-assisted dynamic navigation and freehand approaches. Materials and methods: A total of 20 dental students (6 males, 14 females, age: 25.6 ± 0.5 years) were enrolled in this study. With the traditional freehand approach (training 1) as the control protocol, computer-assisted dynamic navigation (training 2) was used in the training of dental students in implant placement. For each training, both the operating time (OT) of students and placement accuracy represented by the linear (at the implant platform, Dpl, and apex, Dap) and angular (Dan) deviations between the virtually planned and placed implants were recorded. Statistical comparisons were made between the two training protocols as well as male and female surgeons. Results: OT2 was around twice of OT1 (p < .0001), whereas Dan1 was almost three times of Dan2 (p < .0001). Dap1 and Dpl1 were significantly higher than Dap2 (p = .014) and Dpl2 (p = .033) respectively. Besides, male students showed statistically higher Dpl1 (p = .033) and Dan1 (p = .002) than females. No significant difference was found between male and female students in OT1, OT2, Dpl2, Dap1, Dap2 and Dan2. Conclusions: Within the limitations of this study, the use of computer-assisted dynamic navigation in the preclinical training could improve the surgical performance of the dental students in implant placement. The combination of dynamic navigation with the traditional preclinical surgical training may benefit to dental students and could be applied in dental education.
Accuracy and Self-Confidence Level of Freehand Drilling and Dynamic Navigation System of Dental Implants: An In Vitro Study

Abstract:Background and objective: The impact of the experience of the clinician on learning a new skill or equipment was still an intriguing subject. The goal of this research is to determine the accuracy level of a dynamic navigation system to that of freehand drilling by expert and novice practitioners with varied levels of experience. Additionally, the duration of the surgical procedure and the self-confidence level of the surgeons were also evaluated. Materials and methods: An analog impression of the patient was used to make 20 polyurethane simulation models of the maxilla. Five expert and five inexperienced surgeons prepared the site and placed the implants at random on ten models each. Two different techniques were used to insert dental implants: freehand and dynamic navigation systems. Dental implants were placed in Group 1 utilizing a computer-assisted dynamic navigation device. The implants in Group 2 were secured using free-hand drilling. The dental implants were inserted first in the maxillary right first molar, then in the maxillary right lateral incisor, and the maxillary left second premolar. Preoperative and postoperative CBCT scans were superimposed by employing the Evalunav software and contrasted. The coronal 3-D, apex 3-D, apex vertical depth, and angular deviations for both procedures were evaluated. A pre-tested self-confidence questionnaire was also administered to assess the self-confidence of the practitioners. The duration of the surgical time was also documented for each strategy. The t-test was used to measure the difference in accuracy and confidence levels between freehand and dynamic navigation systems among expert and novice surgeons using SPSS software (IBM Corp., Armonk, NY, USA). Results: A total of 60 implants were used (three insertion sites, two methods, and 10 practitioners). Each of the five expert and novice clinicians implanted 15 implants (five models each). Except for entry 3-D, there was a statistically significant difference between the two approaches in all of the primary outcome variables. The apex 3-D (5.89±1.08 mm) and apex vertical (2.08±1.27 mm) dimensions of the dynamic navigation system were significantly smaller than those of the freehand drilling approach (p<005). Dynamic navigation and freehand drilling had angular deviations of 7.16±1.76ᵒ and 9.06±2.18ᵒ, respectively (p=0.0004). The apex vertical deviation was reduced in the navigation technique (2.07±1.5 mm) than in the freehand drilling (2.86±1.4 mm) by experienced practitioners (p=0.04). The difference in time between the two procedures was determined to be statistically highly significant (p<0.001) by both expert and novice surgeons. Furthermore, when contrasting with experienced practitioners, novice practitioners had an overall increase in surgery time (p<0.001) for both approaches. Conclusion: The current in vitro study found that the dynamic navigation system enables more accurate implant placement than the freehand drilling technique, irrespective of the experience of the surgeons. However, this technique appears to benefit novice practitioners more, as they can profoundly minimize their deviations while accomplishing results comparable to those of expert surgeons.

Abstract:Objectives: Infrared dynamic navigation systems can be categorized into active and passive based on whether the surgical instruments can emit or only reflect light. This in vitro study aimed to compare the accuracy of implant placement and the learning curve of both active and passive dynamic navigation systems, using different registration methods.Methods: Implants (n = 704) were placed in 64 sets of models and divided into active (Yizhime, DCARER, Suzhou, China) and passive (Iris-Clinic, EPED, Kaohsiung, China) dynamic navigation groups. Both marker point-based registration (M-PBR) and feature point-based registration (F-PBR) were employed for the two groups. Based on preoperative and postoperative cone-beam computed tomography imaging, the coronal, midpoint, apical, and angular deviations were analyzed from 2D and 3D views. The operation time was recorded for each group.Results: The active dynamic navigation group exhibited significantly higher accuracy than the passive dynamic navigation group (angular deviation, 4.13 ± 2.39° versus 4.62 ± 3.32°; coronal global deviation, 1.48 ± 0.60 versus 1.86 ± 1.12 mm; apical global deviation, 1.75 ± 0.81 versus 2.20 ± 1.68 mm, respectively). Significant interaction effects were observed for both registration methods and four quadrants with different dynamic navigation systems. Learning curves for the two dynamic navigation groups approached each other after 12 procedures, and finally converged after 27 procedures.Conclusions: The accuracy of active dynamic navigation system was superior to that of passive dynamic navigation system. Different combinations of dynamic navigation systems, registration methods, and implanted quadrants displayed various interactions.Clinical significance: Our findings could provide guidance for surgeons in choosing an appropriate navigation system in various implant surgeries. Furthermore, the time required by surgeons to master the technique was calculated. Nevertheless, there are certain limitations in this in vitro study, and therefore further research is required.

Accuracy of Different Registration Areas Using Active and Passive Dynamic Navigation Systems in Dental Implant Surgery: an in Vitro Study.

Abstract:Objectives: To gauge the relative accuracy of the use of passive and active dynamic navigation systems when placing dental implants, and to determine how registration areas affect the performance of these systems.Materials and Methods: Eighty implants were assigned to be placed into 40 total resin mandible models missing either the left or right first molars using either passive or active dynamic navigation system approaches. U-shaped tube registration devices were fixed in the edentulous site for 20 models each on the left or right side. Planned and actual implant positions were superimposed to assess procedural accuracy, and parameters including 3D entry deviation, angular deviation, and 3D apex deviation were evaluated with Mann-Whitney U tests and Wilcoxon signed-rank tests.Results: Respective angular, entry, and apex deviation values of 1.563 +/- 0.977(degrees), 0.725 +/- 0.268 mm, and 0.808 +/- 0.284 mm were calculated for all included implants, with corresponding values of 1.388 +/- 1.090(degrees), 0.789 +/- 0.285 mm, and 0.846 +/- 0.301 mm in the active group and 1.739 +/- 0.826(degrees), 0.661 +/- 0.236 mm, and 0.769 +/- 0.264 mm in the passive group. Only angular deviation differed significantly among groups, and the registration area was not associated with any significant differences among groups.Conclusions: Passive and active dynamic navigation approaches can achieve comparable in vitro accuracy. Registration on one side of the missing single posterior tooth area in the mandible can complete single-tooth implantation on both sides of the posterior teeth, highlighting the promise of further clinical research focused on this topic.
Comparison of the Accuracy of Dental Implant Placement Using Dynamic and Augmented Reality-Based Dynamic Navigation: an in Vitro Study

Abstract:Background/purpose: Augmented reality has been gradually applied in dental implant surgery. However, whether the dynamic navigation system integrated with augmented reality technology will further improve the accuracy is still unknown. The purpose of this study is to investigate the accuracy of dental implant placement using dynamic navigation and augmented reality-based dynamic navigation systems. Materials and methods: Thirty-two cone-beam CT (CBCT) scans from clinical patients were collected and used to generate 64 phantoms that were allocated to the augmented reality based dynamic navigation (ARDN) group or the conventional dynamic navigation (DN) group. The primary outcomes were global coronal, apical and angular deviations, and they were measured after image fusion. A linear mixed model with a random intercept was used. A P value < 0.05 was considered to indicate statistical significance. Results: A total of 242 dental implants were placed in two groups. The global coronal, apical and angular deviations of the ARDN and DN groups were 1.31 +/- 0.67 mm vs. 1.18 +/- 0.59 mm, 1.36 +/- 0.67 mm vs. 1.39 +/- 0.55 mm, and 3.72 +/- 2.13(degrees) vs. 3.1 +/- 1.56(degrees), respectively. No significant differences were found with regard to coronal and apical deviations (P = 0.16 and 0.6, respectively), but the DN group had a significantly lower angular deviation than the ARDN group (P = 0.02). Conclusion: The augmented reality-based dynamic navigation system yielded a similar accuracy to the conventional dynamic navigation system for dental implant placement in coronal and apical points, but the augmented reality-based dynamic navigation system yielded a higher angular deviation. 2023 Association for Dental Sciences of the Republic of China. Accuracy assessment of dynamic navigation during implant placement: A systematic review and meta-analysis of clinical studies in the last 10 years

Abstract:OBJECTIVES: To evaluate the accuracy of dynamic computer-aided implant surgery (dCAIS) and compare it with static computer-aided implant surgery (sCAIS) and freehand implant placement (FH) in partially or fully edentulous patients.DATA: Studies that analyzed the accuracy of dynamic computer-assisted implant surgery in partially or fully edentulous patients.SOURCES: This meta-analysis included studies published in English and Mandarin Chinese from January 2013 to February 2023 from MEDLINE/PubMed, Embase, CENTRAL (Cochrane Central Register of Controlled Trials), and CNKI (China National Knowledge Infrastructure).STUDY SELECTION: Only clinical studies were included. Accuracy was the primary outcome. Seventeen studies met the inclusion criteria. A total of 2,025 implants were analyzed. Meta-regression was conducted to compare the six different navigation systems. GRADE (Grading of Recommendations Assessment, Development, and Evaluation) assessment was adopted as a collective grading of the evidence.CONCLUSIONS: Dynamic navigation is a clinically reliable method for implant placement. Significantly lower angular deviation was observed for dCAIS compared to both sCAIS and FH, while significantly lower global platform and apex deviations were displayed between dCAIS and FH. Overall, dynamic navigation allowed for higher accuracy compared to both sCAIS and FH in a clinical setting; however, additional large sample RCT studies should be conducted, and patient-reported outcome measures (PROMs) reported.CLINICAL SIGNIFICANCE: This systematic review analyzed the accuracy of dynamic computer-assisted implant surgery in partially or fully edentulous patients compared with static navigation. The results demonstrated that dynamic navigation could decrease implant placement angular deviations.

Accuracy of 3 Calibration Methods of Computer-Assisted Dynamic Navigation for Implant Placement: an in Vitro Study

Abstract:Statement of problem. Dynamic navigation for implant placement has been reported to be more accurate than freehand surgery. However, the accuracy of the calibration methods used for navigation in partially edentulous individuals with distal extensions remains unknown. Purpose. The purpose of this in vitro study on dental models was to evaluate the accuracy of 3 calibration methods of dynamic navigation for implant placement in the distal extension of partially edentulous arches. Material and methods. Eleven standardized polyurethane mandibular models with distal extensions were prepared. The left first molar, second molar, and second premolar from each model (33 tooth sites) were randomly assigned to 1 of the 3 calibration methods: U-shaped tube embedded with radiopaque markers, anatomic tooth cusps, and bone markers with the random number table method. Preoperative and postoperative cone beam computed tomography images were obtained for deviation analyses. The primary outcomes were 3-dimensional (3D) deviation at the implant platform and apex and angular deviation. Differences among the test groups were analyzed by using a 1-way analysis of variance (ANOVA) and the least significant difference (LSD) post hoc test (alpha=.05). Results. The mean +/- standard deviation 3D deviations were 0.78 +/- 0.34, 1.86 +/- 0.91, and 1.44 +/- 0.57 mm at the implant platform and 0.79 +/- 0.35, 2.19 +/- 1.01, and 1.49 +/- 0.50 mm at the apex in the U-shaped tube, tooth cusp, and bone marker groups, respectively. The 3D deviations at the implant platform and apex were significantly different among the groups (P<.01). The angular deviation was 1.36 +/- 0.54, 2.95 +/- 2.07, and 2.92 +/- 2.45 degrees, with no significant differences among the groups (P=.092). Conclusions. In the dynamic navigation of implant placement in the distal extension of partially edentulous arches, the U-shaped tube calibration with radiopaque markers was more accurate than the anatomic tooth cusp or bone marker calibration.

Abstract:Purpose This in vitro study aimed to compare the accuracy of dental implant placement in partially edentulous maxillary models using a mixed reality-based dynamic navigation (MR-DN) system to conventional static computer-assisted implant surgery (s-CAIS) and a freehand (FH) method. Methods Forty-five partially edentulous models (with teeth missing in positions #15, #16 and #25) were assigned to three groups (15 per group). The same experienced operator performed the model surgeries using an MR-DN system (group 1), s-CAIS (group 2) and FH (group 3). In total, 135 dental implants were placed (45 per group). The primary outcomes were the linear coronal deviation (entry error; En), apical deviation (apex error; Ap), XY and Z deviations, and angular deviation (An) between the implants' planned and actual (post-surgery) positions in the models. These deviations were computed as the distances between the stereolithographic (STL) files for the planned implants and placed implants captured with an intraoral scanner. Results Across the three implant sites, the MR-DN system was significantly more accurate than the FH method (in XY, Z, En, Ap and An) and s-CAIS (in Z, Ap and An), respectively. However, S-CAIS was more accurate than MR-DN in XY, and no difference was found between MR-DN and s-CAIS in En. Conclusions Within the limits of this study ( in vitro design, only partially edentulous models), implant placement accuracy with MR-DN was superior to that of FH and similar to that of s-CAIS. Statement of Clinical Relevance In vitro , MR-DN showed greater accuracy in implant positioning than FH, and similar accuracy to s-CAIS: it could, therefore, represent a new option for the surgeon. However, clinical studies are needed to determine the feasibility of MR-DN.

Accuracy of a dynamic navigation system for dental implantation with two different workflows and intraoral markers compared to static-guided implant surgery: An in-vitro study

Abstract:Purpose: To investigate the accuracy of a miniaturized dynamic navigation system with intraoral markers and two different workflows for dental implantation and to compare with static computer-assisted implant surgery (sCAIS) surgery. Materials and methods: Two operators performed a total of 270 implant insertions in polyurethane mandibular models under simulated clinical conditions. Implants were placed after CBCT-based virtual planning in three different groups: two workflows utilizing dynamic computer-assisted implant surgery (dCAIS; DG1: marker in CBCT; DG2: 3D-printed marker) and the others with sCAIS (TG: template guided). Postoperative surface scans were matched to the planning data and allowed an evaluation of the angular and spatial deviation between the planned and the actually achieved implant position. Descriptive statistics were followed by a Mixed Model Analysis to determine the influence of the operator, the method, and operating area on different accuracy parameters and the random effect of the model number. Results: The mean angular deviation ranged from 2.26° (DG1) to 2.96° (TG). The mean 3D deviation at the implant's tip ranged from 1.08 mm (TG) to 1.51 mm (DG2) and at the implant's base from 0.69 mm (TG) to 1.49 mm (DG2). The operator showed no significant influence on the accuracy. The method showed significant influence on singular parameters and the operating area on all spatial accuracy parameters. Conclusions: Dynamic navigation systems with intraoral markers enable accurate implant positioning, which is comparable to the static-guided implant surgery. 3D-printed markers provide less accurate results compared to prefabricated markers, attached before CBCT scan.
Accuracy of dental implant surgery using dynamic navigation and robotic systems: An in vitro study

Abstract:Objectives: To compare the accuracy of dental implant placement using a dynamic navigation and a robotic system. Methods: Eighty three-dimensional (3D) printed phantoms, including edentulous and partially edentulous jaws, were assigned to two groups: a dynamic navigation system (Beidou-SNS) group and a robotic system (Hybrid Robotic System for Dental Implant Surgery, HRS-DIS) group. The entry, exit and angle deviations of the implants in 3D world were measured after pre-operative plans and postoperative cone-beam computed tomography (CBCT) fusion. A linear mixed model with a random intercept was applied, and a p value <.05 was considered statistically significant. Results: A total of 480 implants were placed in 80 phantoms. The comparison deviation of the dynamic navigation system and robotic system groups showed a mean (± SD) entry deviation of 0.96 ± 0.57 mm vs. 0.83 ± 0.55 mm (p=0.04), a mean exit deviation of 1.06 ± 0.59 mm vs. 0.91 ± 0.56 mm (p=0.04), and a mean angle deviation of 2.41± 1.42° vs. 1 ± 0.48° (p<0.00). Conclusions: The implant positioning accuracy of the robotic system was superior to that of the dynamic navigation system, suggesting that this prototype robotic system (HRS-DIS) could be a promising tool in dental implant surgery. Clinical significance: This in vitro study is of clinical interest because it preliminarily shows that a robotic system exhibits lower deviations of dental implants than a dynamic navigation system, in dental implant surgery, in both partially and completely edentulous jaws. Further clinical studies are needed to evaluate the current results.

Methods and materials A total of 38 implants, which underwent the dynamic navigation , and 57 implants which underwent a static surgical guide were enrolled in the retrospective study. Coronal deviation, apical deviation, and angular deviation were compared between the DN and SSG groups, along with the different experience level of surgeons and implant sites in the DN group. Results There were no statistically significant differences between the DN and SSG groups, and the experience level of the surgeons and implant sites in the DN group. However, the apical deviation of the DN was slightly higher than the SSG group in the anterior teeth ( P = 0.028), and the angular deviation of DN was smaller than the SSG group in the molar. Conclusion Dynamic navigation can achieve accurate implant placement as well as the static surgical guide. Additionally, the experience level of the surgeon and implant site do not influence the accuracy of dynamic navigation, while the accuracy of DN seems higher than the SSG in molar.

Abstract:Objectives: To evaluate the accuracy of implant placement using a dynamic navigation system in fully edentulous jaws and to analyze the influence of implant distribution on implant position accuracy. Materials and Methods: Edentulous patients who received implant placement using a dynamic navigation system were included. Four to six mini screws were placed in the edentulous jaw under local anesthesia as fiducial markers. Then patients received CBCT scans. Virtual implant positions were designed in the planning software based on CBCT data. Under local anesthesia, implants were inserted under the guidance of the dynamic navigation system. CBCTs were taken following implant placement. The deviation between the actual and planned implant positions was measured by comparing the pre- and postsurgery CBCT. Results: A total of 13 edentulous patients with 13 edentulous maxillae and 7 edentulous mandibles were included, and 108 implants were placed. The average linear deviations at the implant entry point and apex were 1.08 +/- 0.52 mm and 1.15 +/- 0.60 mm, respectively. The average angular deviation was 2.85 +/- 1.20 degrees. No significant difference was detected in linear and angular deviations between the maxillary and mandibular implants, neither between the anterior and posterior implants. Conclusions: The dynamic navigation system provides high accuracy for implant placement in fully edentulous jaws, while the distribution of the implants showed little impact on implant position accuracy.
Accuracy and Efficiency of a Calibration Approach in Dynamic Navigation for Implant Placement: an in Vitro Study

Abstract:Background/purpose: Computer-assisted dynamic navigation surgery could provide accurate implant placement. However, its low efficiency was always criticized by dental surgeons. The purpose of this study was to evaluate the accuracy and efficiency of a calibration approach with reflective wafers in dynamic navigation for implant placement. Materials and methods: Eighty implants were placed in the standardized polyurethane mandibular models under dynamic navigation and divided into 2 groups according to the calibration methods (n = 40). The U-shaped tube (UT) group used a prefabricated U-shaped tube embedded with radiopaque markers. The reflective wafers (RW) group used a fixation with 3 round reflective wafers as markers. Postoperative cone beam computed tomography images were obtained for implants deviation analyses. The calibration time was used to evaluate the efficiency of the 2 methods. Results: Significant differences were found in the trueness and efficiency between the 2 groups (P < 0.05). The 3D deviations at the implant platform and apex were smaller in UT group (0.89 +/- 0.28 and 0.79 +/- 0.30 mm, respectively) than in the RW group (0.99 +/- 0.28 and 0.98 +/- 0.30 mm, respectively). The angular deviation was larger in the UT group (2.16 +/- 1.12 degrees) than in the RW group (1.53 +/- 0.88 degrees). The calibration approach of RW group was more efficient than the UT group (2.05 +/- 0.55 and 7.50 +/- 0.71 min, respectively). Conclusion: The calibration method of RW improved the efficiency significantly and achieved equivalent trueness with UT for dynamic navigation during implant placement. Abstract:Background: This prospective clinical study aimed to investigate a possible deviation between the digitally planned implant position and the position achieved using dynamic navigation. The aim of the study was to establish clinical effectiveness and precision of implantation using dynamic navigation. Methods: Twenty consecutive patients received an implant (iSy-Implantat, Camlog, Wimsheim, Germany). One screw implant was placed in one jaw with remaining dentition of at least six teeth. The workflow was fully digital. Digital implant planning was conducted using cone-beam computed tomography (CBCT) and an intraoral scan of the actual condition. Twenty implants were subsequently placed using a dynamic computer-assisted procedure. The clinical situation of the implant position was recorded using an intraoral scan. Using these data, models were produced via 3D printing, and CBCTs of these models were made using laboratory analogs. Deviations of the achieved implant position from the planned position were determined using evaluation software. Results: The evaluation of 20 implants resulted in a mean angle deviation of 2.7° (95% CI 2.2–3.3°). The 3D deviation at the implant shoulder was 1.83 mm (95% CI 1.34–2.33 mm). No significant differences were found for any of the parameters between the implantation in the upper or lower jaw and an open or flapless procedure (p-value < 0.05). Conclusion: The clinical trial showed that sufficiently precise implantation was possible with the dynamic navigation system used here. Dynamic navigation can improve the quality of implant positioning. In particular, the procedure allows safe positioning of the implants in minimally invasive procedures, which usually cannot be performed freehand in this form. A clinical benefit and effectiveness can be determined from the results.

Abstract:Background/purpose Pterygoid implant is a promising solution for patients with a partially or fully edentulous atrophic maxilla. However, whether dynamic navigation system will improve the accuracy of pterygoid implant surgery is still unknown. This study aimed to compare the accuracy of dynamic navigation and free-hand approaches in pterygoid implant placement in completely edentulous maxilla models. Materials and methods Twenty three-dimensional (3D)-printed edentulous maxilla models were assigned to two groups: the dynamic navigation system group and the free-hand group. Two pterygoid implants were planned in the bilateral pterygomaxillary area and then placed in each model. The entry, exit and angle deviations of the pterygoid implants were measured after pre- and post-operative cone-beam CT (CBCT) image fusion. Student's t test and Mann–Whitney U test were used. A P value < 0.05 was considered statistically significant. Results A total of 40 pterygoid implants were placed in 20 models. The comparison deviation of the dynamic navigation group and the free-hand group showed a mean (±SD) entry deviation of 0.93 ± 0.46 mm vs. 2.28 ± 1.08 mm (P < 0.001), an exit deviation of 1.37 ± 0.52 mm vs. 3.14 ± 1.82 mm (P < 0.001), and an angle deviation of 2.41 ± 1.24° vs. 10.13 ± 4.68° (P < 0.001). There was no significant difference in the accuracy with regard to the side factors between the navigation group and the free-hand group. Conclusion The dynamic navigation system has higher accuracy for pterygoid implant placement in a complete edentulous maxilla than the free-hand approach.

Abstract:Purpose: To compare the U-tube and cusp dynamic navigation system registration methods in the use of dental implant placement, and to assess the influence of the location of missing teeth on these registrations. Methods: 32 resin mandible models and 64 implants were utilized, with implants being placed using one of the two registration methods selected at random. Accuracy was measured through the superimposition of the final and planned implant positions. Angular deviation, 3D entry deviation, and 3D apex deviation were analyzed. Results: The overall mean 3D deviation was 1.089 ± 0.515 mm at the entry point and 1.174 ± 0.531 mm at the apex point, and mean angular deviation was 1.970 ± 1.042 degrees. No significant difference (p > 0.05) was observed when comparing these two registration methods. However, the U-tube method showed significant difference when assessing the location of missing teeth (without distal-extension absence and distal-extension absence), whereas cusp registration was unaffected. Conclusions: Both the U-tube and cusp dynamic navigation system registration methods are accurate when implemented in vitro. Besides, the cusp registration technique can also overcome several of the limitations of the U-tube approach and the accuracy of it was not influenced by the location of the missing teeth, highlighting it as a method worthy of further clinical research.

Abstract:Objective To assess the accuracy of dynamic computer-assisted implant surgery. Materials and methods An electronic search up to March 2020 was conducted using PubMed, Embase, and the Cochrane Central Register of Controlled Trial to identify studies using dynamic navigation in implant surgery, and additional manual search was performed as well. Clinical trials and model studies were selected. The primary outcome was accuracy. A single-arm meta-analysis of continuous data was conducted. Meta-regression was utilized for comparison on study design, guidance method, jaw, and systems. Results Ten studies, four randomized controlled trials (RCT) and six prospective studies, met the inclusion criteria. A total of 1,298 drillings and implants were evaluated. The meta-analysis of the accuracy (five clinical trials and five model studies) revealed average global platform deviation, global apex deviation, and angular deviation were 1.02 mm, 95% CI (0.83, 1.21), 1.33 mm, 95% CI (0.98, 1.67), and 3.59 degrees, 95% CI (2.09, 5.09). Meta-regression shown no difference between model studies and clinical trials (p = .295, 0.336, 0.185), drilling holes and implant (p = .36, 0.279, 0.695), maxilla and mandible (p = .875, 0.632, 0.281), and five different systems (p = .762, 0.342, 0.336). Conclusion Accuracy of dynamic computer-aided implant surgery reaches a clinically acceptable range and has potential in clinical usage, but more patient-centered outcomes and socio-economic benefits should be reported.
