Abstract:Objective This study explores the experience of Israeli therapists who both worked with clients in emergency interventions during the third week following the Hamas attack on Israel on October 7, 2023, and were themselves exposed, to one extent or another, to the terrifying events. Method Open‐ended questionnaires were completed by 201 therapists during the third week following October 7. Using thematic analysis, therapists' reports of the themes their clients raised in therapeutic sessions were compared with their reports of the hardships they themselves experienced as individuals and professionals during the same period. Drawing on the concept of shared traumatic reality, the study considers how close the themes are, and how challenging it is to cope concurrently with a traumatic reality that is experienced both directly and indirectly. Results Seven main categories emerged from the responses to questions about both their clients and themselves: (1) uncertainty and worries about the near and more distant future; (2) overwhelming emotions; (3) physical sensations; (4) shattered meaning and loss of trust; (5) lack of routine; (6) self‐preservation; and (7) shared trauma. Conclusions The findings shed light on early reactions to a shared traumatic reality in the period closely following the trauma itself, indicating that professionals find it hard to differentiate between the personal, professional, and national levels when exposed to a large‐scale traumatic event they share with their clients.

Abstract:This qualitative study explored the mortality salience effect as it manifested in the dreams of Israelis in the aftermath of the Hamas terror attack on October 7, 2023, and during the ensuing war. Over a two-month period, a sample of 242 dreams was collected via an online survey. Two forms of thematic analysis, namely, inductive and deductive, were utilized. Viewed through the theoretical lens of terror management theory, the findings suggest that exposure to the attack's horrors and the resulting strong mortality salience were reflected in the participants' dream contents. This indicates that the anxiety-buffering roles of the three psychological coping mechanisms—cultural worldviews, self-esteem, and close personal relationships—have been compromised. The examination of collective dream content offers a glimpse into subtle human psychic processes and how they might be affected during times of national trauma that necessitate robust psychological mechanisms to cope with the heightened mortality salience.

Abstract:On October 7, 2023 (10/7), Hamas militants brutally attacked Israeli towns and villages surrounding the Gaza Strip, resulting in the killing of more than 1100 people, most of whom were civilians slaughtered in their own homes. The killings occurred under highly traumatic circumstances, including shooting, the burning of homes, hunting down people who attempted to escape, and severe sexual assault. Thus, many Israelis today experience a unique mix of both posttraumatic and severe grief reactions. Traumatic grief (TG) is often defined as a condition that results from the death of a significant other and includes symptoms similar to posttraumatic stress disorder (PTSD) that are specifically related to the deceased, such as intrusive thoughts and memories about the deceased and hypervigilance expressed by constantly looking for them or cues associated with them. However, whereas definitions, phenomenological descriptions, and clinical illustrations of PTSD are abundant and widely validated, TG has yet to receive formal diagnostic status. In this paper, we aim to reexamine TG in the context of post‐10/7 Israel. We argue that TG is a critical concept for clinicians working in Israel and other conflict‐exposed areas of the world today, as it accurately captures the painful mix of grief‐ and trauma‐related symptoms. We also suggest potential explanations for the lack of acknowledgment of TG as a formal diagnosis and discuss the possible role of historical events in the formation of new, relevant psychiatric diagnoses.

[ON SWORDS WAR - THE BATTLE FOR THE SOUL]

Abstract:The Iron Swords War broke out with an unprecedented attack of brutality, leaving soldiers, civilians, residents of the Israeli settlements surrounding Gaza, and participants of the Nova Festival severely traumatized. Israel was exposed to loss, bereavement, sexual abuse, kidnapping of civilians and soldiers, including children, women, and the elderly. Identification of bodies to be brought to honorable burial, while simultaneously organizing for combat and the ongoing war, resulted in physical and mental injuries. This prolonged war has also led to the evacuation of communities from the areas surrounding Gaza and from the cities and settlements on Israel's northern borders to safer locations, causing them to be displaced persons in their own country. At the outbreak of the war, the public mental health system was at an unprecedented low, lacking professional therapists, with long waiting lists for treatment across all domains of mental health care. Psychiatric hospitals, health funds' mental health clinics and non-governmental organizations (NGOs), and the military mental health care system have been working tirelessly to train therapists to seek more effective trauma treatment and to maximize treatment capacity. The activities resulting from this intensive effort, are reflected in the articles in this special issue. This issue, is primarily dedicated to saluting the efforts of mental health care providers, while looking forward and understanding that the mental damage from the war, the displacement, the captivity, the sexual abuse, and more, will need to be addressed and treated for many years to come. Many works were submitted for this special issue, and the number of articles accepted were beyond the normal range of the issue. We could not have faced the challenge of handling these materials without the support (and tireless leadership) of Mrs. Rena Kurs, to whom we are deeply grateful. We all look forward to the return of all the hostages and a return to focusing on health and normality.
Exposure to the 2014 Gaza War and Support for Militancy: The Role of Emotion Dysregulation

Abstract:How do wars shape emotions and attitudes in intractable conflicts? In two studies conducted in the aftermath of the 2014 Gaza War in the Middle East, we tested a new theoretical model wherein the ability to regulate emotions is central in determining the influence of war exposure on emotions (i.e., group-based humiliation) and support for militancy, through posttraumatic stress symptoms (PSS). Results supported our model: (a) higher exposure to the war predicted group-based humiliation in both studies and in Study 2 also greater support for militancy; in both studies, (b) higher exposure predicted more PSS only among participants high in emotion dysregulation, and, for them, (c) higher exposure predicted greater group-based humiliation, through increased levels of PSS. Results from Study 2 suggest that (d) group-based humiliation will ultimately lead to greater support for militancy. The findings' contribution to the different literatures and their integration is discussed.

Abstract:On October 7, 2023, terrorist organizations led by Hamas, launched an extensive attack on Israel. Within days following the initial attack, there was a clear need to provide psychological support to individuals who were exposed to the horrors of the October 7 attacks. Hence, the Tel Aviv Sourasky Medical Center opened the "Ichilov Otef Nefesh" clinic to provide psychological first aid to the first-line victims of the war. As the fighting spread, the services were extended to the entire population. The guiding principle of the intervention was that it is crucial in the acute phase after exposure to significant trauma, and its main purpose is to support the natural course of one's recovery processes. This case series describes three representative cases from among the hundreds of people who applied to the clinic. The intervention provided was guided by an integrated approach that promotes introspection and connectedness to one's environment and community, enhancing cognitive functions, emotional regulation and utilizing available resources to reduce distress. The main themes that emerged shifted between feelings of helplessness and a sense of agency; a breach in basic trust and renewed trust, guilt and self-compassion; anger, abandonment and alienation versus solidarity and connecting to the present and the future, despair versus hope and loss versus acceptance and connectedness. In face of these, the therapists endorsed techniques of validation, restoring self-agency, promoting a sense of self-agency and self-compassion, creating a link between the past and present, and finding connection and hope.

Abstract:On 7 October 2023, 3,000 Hamas terrorists from the Gaza Strip infiltrated Israel. Over 1,300 people were killed on that day, and over 240 were abducted to Gaza. On October 8, Israel declared war on Hamas. The current study delves into the prevailing phenomenon of spontaneous war weddings held in military settings during the war. Drawing from Terror Management Theory (TMT) and utilizing an ethnographic approach, we analyze published media reports of these weddings. We suggest that this phenomenon is a sign of three terror management anxiety buffer mechanisms activated in view of current national and personal mortality salience: the validation of cultural worldviews; the enhancement of self-esteem; and the pursuit of proximity, in the form of a sense of closeness to attachment figures. All three anxiety-buffer mechanisms contribute to the fortification of psychological defenses, the denial of death, and the maintenance of psychological equanimity in view of this incomprehensible threat.

Abstract:This study explores the phenomenon of memorial stickers commemorating victims of the October 7, 2023, massacre and subsequent Israel-Hamas war. Analyzing 600 stickers collected across Israel, we examine how these artifacts shape personal and collective memory of these tragic events. Using content analysis, visual data analysis, and ethnography of texts, we investigate the stickers' distribution, textual content, and visual elements. Three key findings emerged: (1) The widespread distribution of stickers expands commemoration beyond cemeteries, creating a larger community of remembrance; (2) Diverse textual content, from personal traits to universal messages, aims to keep the deceased's values alive in social awareness; (3) Visual elements balance public recognition with private mourning through strategic use of photographs, colors, and barcodes. Drawing on theories of collective memory and continuing bonds, we argue that these stickers symbolically bring the deceased into daily life and public spaces, contributing to the processing of personal and national trauma.

Abstract:Background: Public health research suggests that multiple factors contribute to the mental health sequalae of political violence, but the daily experiences and ongoing strategies of psychological survival during active warfare are not clearly understood. We analysed retrospective chronological diaries from Palestinian women in Gaza, occupied Palestinian territory, to explore their lives during Operation Protective Edge in 2014, when a series of aerial bombardments killed over 2000 Palestinians and left around 500 000 displaced from their homes. Methods: 20 Palestinian female teachers in Gaza were recruited via an intervention for trauma recovery 1 month after the end of Operation Protective Edge, in September, 2014. Following the experience sampling method, women were asked to chronicle specific memories and affective responses associated with their experiences. Data were imported into qualitative data analysis software, coded line by line, and analysed using content analysis, with special attention to the causes of mental distress associated with political violence. Findings: The women's narratives included memories of exposure to bombings, injuries, death, and destruction of vital infrastructure and systems. Their experiences of flight were described as exhausting and demoralising. Their temporary shelters lacked their basic needs for sanitation, food, and privacy. These experiences resulted in intense terror, grief, exhaustion, hopelessness, and isolation. Strategies of psychological survival included reframing; trying to appreciate "being able to escape death", a sense of solidarity within families and neighbourhoods, and pride and satisfaction found in a context of resistance and global attention to the suffering in Gaza. Interpretation: Our results reveal the importance of tracing the events of warfare, its psychosocial consequences, and the distinct patterns of emotional and logistical survival in Gaza. This study was limited by the representativeness of our study population and the small sample size. Future studies should explore this topic in larger populations. Limitations notwithstanding, in drawing out the unique contribution of reflective narratives of women survivors of war, our study highlights the need to solicit and analyse reflective and chronologically grounded narratives within global mental health epidemiology. Daily, we are bombarded with images of destruction and desperation, and a shared vision for peace and security feels both elusive and essential. This ongoing human rights catastrophe, saturated with intergenerational trauma, has fractured communities worldwide and within psychoanalysis; dialogue and recognition of the Other often feel impossible.

Healing the social self: how parents whose children were killed in terror attacks construct the experience of help

Abstract:This study focuses on expressed needs and structures of assistance received by Israeli parents whose children were killed in terror attacks. The loss takes place within a multi-systemic network that can be a healing force and/or a distorting factor in the grief process. The qualitative research paradigm employed privileges the knowledge of the parents themselves. In-depth interviews were conducted with 16 parents. (1) The primary criterion that determines the parents' attribution of helpfulness is perceived inclusiveness. (2) The subsystem of "family of the bereaved" is salient in the healing process. (3) There is a clear preference for the services provided by NGO's as opposed to governmental agencies. (4) The bereaved parents engage the symbolic level of the macro-system-the heritage of the Jewish people. The article concludes with the practical implications of the findings for the development and delivery of psychosocial services to parents bereaved in terror attacks.

Abstract:On April 29, 2021, during an Ultra-Orthodox annual communal event in Meron, a crowd rush resulted in the deaths of 45 individuals. Experiencing such events may intensify death proximity (subjective nearness to death, SNtD) and death anxiety, and increase distress. Furthermore, the experience of grief following the trauma may disrupt defense mechanisms that reduce death-related anxieties. Thus, we examined the mediating role of death anxiety on the association between SNtD and distress, and the possible moderating role of grief experiences on this model. Ultra-Orthodox Israeli Jews who experienced the Meron disaster (N = 168) responded to scales assessing demographics, SNtD, death anxiety, and psychological distress. Death anxiety mediated the SNtD-distress link and grief was a significant moderator. Specifically, for individuals low in grief, the association between high death anxiety and increased distress was nullified. Findings are discussed from the perspective of Terror Management Theory.

Abstract:This paper discusses the present Israeli-Palestinian war, with an emphasis on the destructive clash between two utopian visions: the vision of "Greater Israel" – including all Palestinian territories – of the far right in Israel, and the vision of destroying the state of Israel, fundamental in Hamas ideology. The dystopic result of this clash is explored from a psychoanalytic point of view, relying in part on Ogden's notion of historicity as achieved only in the depressive position, and Jessica Benjamin's notion of a "doer and done-to" mode which excludes recognition of the reciprocal interaction that is essential to that historicity. A brief review of the history of the Palestinian-Israeli conflict, lasting more than a hundred years, points to the constant reciprocal influences leading to its deterioration, and ends with the hope for a pragmatic compromise that would alleviate the suffering and traumatization on both sides.

The Holocaust changed forever mentalities and the way in which humankind perceived and related to history, challenging the very possibility of representation itself as it ushered in an obligation to conceive of memory and the recollection of history differently.

Abstract:This study explores the experiences of bereaved siblings in Israel, examining how different causes of death affect their psychological well-being. We recruited three groups of siblings who lost loved ones as a result of military service, terrorism, or civilian circumstances. A total of 159 bereaved siblings completed questionnaires measuring complications of grief (CG), posttraumatic stress disorder (PTSD), and world assumptions. Results indicated that siblings who experienced civilian loss exhibited the highest prevalence of CG, while no significant differences in PTSD levels were observed across the groups. Both CG and PTSD were associated with the world assumptions regarding the benevolence of the world and sense of self-worth. The study also examined how demographic variables influence siblings' well-being and coping with loss. Overall, the findings emphasize the importance of recognizing the unique experiences of bereaved siblings and underscore the necessity of tailored support to address their individualized needs.

Abstract:OBJECTIVE: We assessed in vivo symptom courses of early psychological responses during war and investigated the influence of exposure, gender, and a prior diagnosis of severe mental illness (SMI).METHOD: Participants were 181 highly exposed individuals from the general population and community psychiatric rehabilitation centers. A 30-day twice-daily Internet-smartphone-based intensive assessment two weeks into the 2014 Israel-Gaza war estimated peritraumatic symptom clusters, sense of threat, negative emotions and cognitions, and siren exposure during two periods that varied in exposure level. Piecewise growth curve modeling procedures were performed.RESULTS: We found different courses for most variables, gender, and SMI status. Women were more reactive two weeks into the war but reduced their reactivity level at a faster pace than males, reaching lower symptom levels one month later. Women's courses were characterized by arousal, negative emotionality, sense of threat, and reactivity to siren exposure. No-SMI men had a stable course followed by a significant reduction in arousal, negative emotions, avoidance, and perceived threat during a "return to routine" lower-level intensity period of the war. Individuals with SMI had higher reactivity levels at study onset; but while women with SMI improved over time, men with SMI worsened. SMI reactivity was characterized by negative cognitions, intrusions, and avoidance.CONCLUSIONS: Early reactions during prolonged exposure to war are variable, dynamic, and affected by exposure context. Symptoms, emotions, and cognitions develop differentially over time and are affected by gender and mental health status. The identification of various early stress courses should inform primary intervention strategies.

Abstract:Objectives: To delve into the phenomenon of memorial tattoos in Israel following the October 7, 2023, massacre, and the ensuing Iron Swords War utilizing Terror Management Theory. Methodology: A qualitative approach employing digital ethnography and visual content analysis was adopted to scrutinize 250 war tattoo images sourced from Israeli tattoo artists' Instagram pages and Facebook groups. Data collection spanned the initial four months of the war, from October 2023 to January 2024. Findings and conclusions: The analysis highlights existential anxieties stemming from the massacre and ongoing conflict, which are both reflected and addressed through the tattoos' content and meaning. Memorial tattoos serve as responses to mortality salience, depicting themes that adhere to the three anxiety-buffer mechanisms proposed by the theory: reinforcement of collective worldviews; enhancement of self-esteem; and seeking continued attachment relationships. Through these mechanisms, tattoo recipients invest efforts in giving meaning to the inconceivable events and their ongoing grief.
