Abstract:Personal identity is self-evidently important to us all. Identity is a philosophically complex subject, but there is some agreement that memory, embodiment and continuity are essential components. The sense of memory includes 'future memory', the kind of memory we would like to construct for ourselves as our lives proceed. While the sense of personal identity is internal to the individual, a sense of that person's identity exists in the minds of others. Extreme experiences threaten the element of continuity, because they may bring bodily changes as well as cognitive changes that challenge central values. Restoring or preserving continuity is a major task for survivors. The ways in which people experience discontinuity because of cancer illness, and the ways in which they manage this experience emerges from the narratives of the survivors of cancer and in the narratives of health care workers who look after them. People manage discontinuity by reference to stable 'anchor points' in their beliefs and values; by re-constructing versions of their pre-experience identities, drawing on past memory and finding ways to preserve a continuity between past memory, present experience and constructions of the future; by using the experience to develop established facets of identity; and by imbuing the experience with meaning and recognising the enlarged identity made possible by survival. Those who cannot achieve a sense of continuity may feel alienated from themselves, their friends and family. All these methods of management may be used by one person to negotiate the post-experience identity in its different social interactions. The experience of the survivor can be further understood by recognising the challenge posed by extreme experience to the sense of continuity of both embodied self and memory. A satisfactory discourse of survival has yet to enter the public domain. This lack adds to the burdens of survivors, including those who have survived cancer.

Abstract:How do metaphors shape and reflect patients' experience of cancer? How can repeated immersion in challenging poetry affect our understanding of cancer and other diseases? This essay addresses these two questions through an exploration of the varied and sometimes contradictory metaphors crafted by poet and seven-time cancer patient Judy Rowe Michaels. Although cancer is often regarded as an enemy intrusion to be eradicated, it is an illness in which the threat to life comes from within the self. Michaels's metaphors complicate the relationship between an embodied self and the cancerous cells the body produces, presenting multifarious responses to living with cancer. Defying expectations of a linear trajectory from symptom to diagnosis to treatment to recovery, these metaphors provide a means to understand the changing emotions; the changing relationship to illness, to wellness, and to self; and the changing status as patient (symptomatic, diagnosed, in treatment, in remission, in recurrence) that characterize the lived experience of cancer-and of other recurring or chronic diseases. Examining these poems demonstrates the value for practitioners, patients, and families of honing a poet's mode of understanding, as a vital complement to the mode of thinking that currently drives health care.

Abstract:Qualitative Health Research, Ahead of Print. This study feeds into ongoing discussions on the metaphors used by cancer patients. Its aim is to explore how women living with a history of breast cancer use metaphors to express and interpret the experience of cancer remission. Data were collected in interviews designed to capture a rich and metaphorical description of participants' experiences with breast cancer and what these experiences mean to them. Ten participants were recruited. An interpretative phenomenological analysis of the participants' narratives highlighted a central metaphor: the cancer trace in one's life. The participants had to adapt to four specific traces of cancer: (1) the identity trace, (2) the existential trace, (3) the bodily trace, and (4) the narrative trace. We discuss how cancer challenges one's sense of biographical continuity and initiates a search for a new way of being. We also discuss how the metaphor of the trace differs from the metaphor of the cancer hero living without any trace of cancer.

Abstract:The constantly escalating population of cancer survivors worldwide has prompted a focused exploration of their unique needs and experiences within the context of healthcare medicine. This review initiates its analysis inspired by Dr. Lidia Schapira's insightful keynote conference on the Survivorship 1.0 and Survivorship 2.0 Programs, shedding light on their implementation challenges and setting the stage for a comprehensive analysis of cancer survivorship initiatives. Within the transformed landscape of cancer care, patient-centric strategies embedded in cancer survivorship programs comprising vital elements such as connection, support, and education are presented. While placing cancer recurrence surveillance at the forefront, the review underlines concern regarding the potential oversight of the enduring impact on mental and physical health. Dr. Schapira's insights further extend into the exploration of mental health challenges faced by survivors, promoting an examination of diverse strategies to address these concerns. Furthermore, the discussion continues toward promising areas of research, notably Precision Medicine's role in de-escalating cancer therapies, and advocates for measures such as early cancer awareness and timely referrals to supportive services. Highlighting the significance of education, the role of online resources in enhancing the knowledge of healthcare practitioners and future generations in cancer care is then explored. The paper concludes by presenting some of the most prominent global current survivorship programs, identifying critical knowledge gaps in cancer care and projecting future developments aimed at delivering accurate and holistic care, improving the quality of life for survivors, and enhancing both mental and physical well-being. Drawing upon the insights from Dr. Schapira, this review lays the groundwork for a nuanced exploration of cancer survivorship and its multifaceted implications.

Abstract:This article presents and interprets illness narratives, told by patients treated for cancer. The aim is to discuss the meaning of cancer metaphors. The interpretation focuses on the relationships of metaphors to bodily experience and to social and cultural levels. The experience of cancer and cancer therapy obviously involve the patient's body. To comprehend what is happening is a process full of uncertainty and obscurity. Metaphors, such as cancer "eating," are widespread and persistent while the meaning of that metaphor today mirrors new scientific and lay explanations of cancer and cancer treatment. Personal newly created metaphors express threatening experiences in which fear of losing oneself and fear of death are involved. Ambivalent hope in relation to treatment and caregivers is expressed. Creating new metaphors involves imaginatively using the available concepts and metaphors. Narrative communication gives access to patients' experiences of an unwanted and painful physiologic process leading to forced embodiment of sickness. Patients want consolation and must overcome solitude by articulating experience, being listened to, and, in this way, recreating and strengthening identity.

Abstract:The use of metaphors to talk about cancer experiences has attracted much research and debate, especially in the case of military metaphors. However, questions remain about what metaphors are used by different populations for different aspects of the cancer experience. This scoping review aims to answer them.We searched PubMed, PsycINFO, CINAHL, Scopus and Web of Science databases. Eligible sources include peer-reviewed scientific research published in English between 2013 and 2023, investigating patterns of metaphor use from adult populations (age ≥18) for cancer-related topics, such as cancer itself, the general experience of being ill, treatment, and people and relationships.Out of 1929 articles identified, 30 met the criteria, spanning over different populations. While most papers focused on cancer in general, some focused on specific cancer types, such as breast cancer. Both spontaneous and elicited data were collected in ten languages: mostly English (N=12), Swedish (N=3) and Arabic (N=3). The identified metaphors were subsumed under various broad categories, including particularly violence and journey. Other categories include education and non-human animate entity for the cancer itself, confinement and deprivation and cleanliness for the general experience of being ill with cancer, Poison and gardening for cancer treatment, and distance for patients' social relationships.It was found that metaphors help to identify how patients describe experiences of vulnerability and empowerment. To provide patient-centred care, clinicians and researchers should avoid blanket conclusions about helpful or unhelpful metaphors, but consider the ways in which different metaphors are used by different populations in different contexts.
The role of narrative and metaphor in the cancer life story: a theoretical analysis

Abstract:Being diagnosed with cancer can be one of those critical incidents that negatively affect the self. Identity is threatened when physical, psychological, and social consequences of chronic illness begin to erode one's sense of self and challenge an individual's ability to continue to present the self he or she prefers to present to others. Based on the notion of illness trajectory and adopting a Ricoeurian narrative perspective, this theoretical paper shall explore the impact of cancer disease on identity and establish the crucial importance of metaphor in the narratives of life with cancer. Findings indicate that in cancer narratives the illness experience supplies the narrative structure with temporal and spatial meeting points that make the narrative comprehensible and meaningful. Cancer forces identity changes not only from having to endure the long-term physical and psychosocial effects of the disease, but also from inevitable existential questions about life's meaning. Improved medical knowledge today means improved ethnomedical practices. Metaphor can bridge the gap between the cancer experience and the world of technology and treatment, helping patients to symbolically control their illness.
Anomic aspects of recovery from cancer

Abstract:A cancer diagnosis is universally regarded as a traumatic event. Many equate it with a 'sentence of death'. But an increasing number of cancer patients are being cured. An exploratory study involving intensive interviews with recovered cancer patients indicated that the positive experience of being cured is often mixed with negative elements, including: (1) the withdrawal of the intensified social support which accompanied the diagnosis and early treatment; (2) ambivalence about the discontinuation of treatment; (3) anxiety about recurrence of the disease; (4) adjustment to permanent disabilities resulting from the disease or its treatment; (5) the need to resume life-oriented modes of thought after a successful adjustment to the ideal of death; (6) anger at perceived inadequacies in the handling of treatment; and (7) confusion about feelings of depression when the objective situation has improved. Durkheim's concept of anomie originally referred to a societal condition engendered by either positive or negative change. Srole and others adopted the term 'anomia' to refer to the social-psychological correlate of this condition, i.e. anomie as experienced by the individual. The present research suggests that the concept of anomia, and specifically, the anomia of good fortune, may be useful in studying the rehabilitation of cancer patients.
"I'm a Survivor": Aboriginal and Torres Strait Islander Cancer Survivors' Perspectives of Cancer Survivorship

Abstract:Background: Disparity in outcomes between Indigenous and non-Indigenous people after cancer diagnosis is multifactorial, including lower cancer screening participation, later diagnosis, reduced access and uptake of cancer treatment, higher rate of comorbidities, and barriers accessing the health system. Little is known about cancer survivorship experiences. Objective: The aim of this study was to explore Indigenous Australian cancer survivor's perspectives of cancer survivorship. Methods: Indigenous people who completed cancer treatment 6 months to 5 years before fieldwork were recruited from a tertiary hospital and remote primary health service for this qualitative study. Data collection was guided by yarning methods, a culturally appropriate method emphasizing storytelling. Data were interpreted using a social constructionist framework. Results: Thirteen women and 6 men were interviewed. Participants' past experiences contributed to their specific identity as survivors. Participants described factors affecting a positive transition from cancer patient to cancer survivor and the importance of ongoing family support in helping to manage survivorship. Finally, participants described a range of community support they received and provided to others and how this improved their cancer survivorship. Conclusion: Although a range of experiences are presented, this study provides evidence that the survivorship perspectives of Indigenous cancer survivors may be, in part, shared by non-Indigenous cancer survivors. Implications for practice: Acknowledging Indigenous cancer survivors' past experiences and how these influence their overall well-being is important for providing patient-centered and culturally appropriate care. Nurses and other healthcare professionals may use this knowledge to foster a range of coping strategies to assist Indigenous cancer survivors to live well.

Abstract:Thankfully, many more women are now surviving cancer due to advances in awareness, surveillance, and treatment. In fact, many oncology clinicians now consider cancer a chronic illness. This has meant that a growing number are living with the physical and psychological consequences of the disease. The psychological impact and emotions experienced by survivors are varied. Some reactions are more common than others, including the onset of depression, anxiety, and sexual dysfunction and feelings of abandonment, isolation, grief, and fear of recurrence. Body image and sexuality are also commonly impacted by cancer, particularly by women who experience treatment-induced menopause. Existential and spiritual concerns can also cause psychological consequences. In time, however, many women adjust to life as a cancer patient and find new meaning and purpose in their lives. The aim of this article is to highlight the key psychological issues faced by women who are in the chronic or survivorship phase of their cancer experience. The article also aims to emphasize the need for ongoing evaluation of the psychological status of these women during regular follow-up appointments. This will ensure that patient-centered supportive care is optimized.

Abstract:Many cancer patients tell their story to help others. This paper reports a study of 160 such stories, which were processed using a grounded theory methodology. The stories were rich in emotional words and in metaphors and these, along with self-talk and meaning-for-life, provided the commonest indications of the patients' well-being. Cancer patients, through the process of telling their story, help themselves and enhance their well-being. The method of active listening is described, with associated risks for professionals.

Abstract:Maintaining the continuity of the 'self' is crucial for finding meaning and ensuring psychological well-being. However, the traumatic encounter with cancer disrupts the normal flow of life stories, challenging coherence. Beyond its physical toll, cancer challenges one's sense of self, identity and overall psychological resilience. As personal narratives unfold, encompassing evolving life stories from one's reconstructed past to the present and envisioned future, inherently reflecting the dynamic conception of self, it becomes imperative to investigate how these narratives shape self-concept and resilience throughout the intricate emotional and psychological landscape of the cancer journey. This narrative review explores the intricate relationship among personal narratives, self-concept and psychological resilience in the context of cancer. The study draws on various theoretical constructs, encompassing self-concept, coping, narrative identity, and psychological resilience theories, to provide a comprehensive exploration of the psychological processes involved. Incorporating insights from both Western and Indian philosophical traditions, the review not only enriches the theoretical framework but also emphasizes the practical implications for healthcare practices. Integrating narrative resilience principles into medical education and advocating for culturally sensitive approaches are deemed essential for advancing patient-centred care. The study underscores both practical and theoretical contributions to psycho-oncology, emphasizing the need for personalized narrative interventions, empathetic healthcare strategies, longitudinal studies and innovative technologies that recognize and support the intricate interplay between personal narratives, self-concept and psychological resilience in the cancer survivorship journey.

Abstract:Objective: to analyze the meaning attributed by parents to the extended and permanent survival of childhood cancer. Method: qualitative narrative inquiry, developed with parents of adolescents and young adults who survived childhood cancer. Recruitment and data collection involved virtual and in-person approaches. The data were collected through semi-structured interviews. Data were analyzed according to reflective thematic analysis. Results: a total of ten parents were included in the study. Two thematic narrative syntheses were constructed: "Times of war"; and "Time of uncertain peace", with their respective sub-themes. The cancer diagnosis marks the beginning of times of war in the parents' lives. They experience cancer treatment as "highs and lows" with potential threats to their children's lives. After that, "Time of uncertain peace" are reached, and the balance of the family unit is reestablished. However, the fear of recurrence makes the family peace uncertain, and its maintenance requires constant vigilance and attention to the signs and symptoms of a possible new battle. Conclusion: the results highlight the experience of being a parent of a childhood cancer survivor and can be applied to develop models of care centered on the survivors' family.
Unveiling perspectives on the psychosocial impacts of childhood cancer survival on young adult survivors' reassimilation journey: A qualitative exploration

Abstract:Background: Approximately 82% of children with childhood cancer survive more than five years after diagnosis. Living as a cancer survivor elicits a new reality that can include psychosocial impacts. These psychosocial impacts interact collectively, especially regarding reassimilation, and are rarely explored. Objective: To explore the psychosocial impacts of surviving childhood cancer and reassimilation back into society in young adult survivors of childhood cancer. Methodology: Individual in-depth semi-structured interviews were conducted with childhood cancer survivors and explored psychosocial aspects associated with returning to work, school, and social environments after remission. Interpretive phenomenological analysis was conducted once interviews were manually transcribed. A group interview with survivors was held to discuss the study's findings and interpretation. Results: Individual interviews and the group interview revealed three major themes: outlook on reassimilating, outlook on coping, and outlook on cancer. Conclusions: This work is a first step to understanding how survivors' personal outlook on coping and healthcare system barriers play influential roles in reassimilation following cancer treatment. Survivors expressed the need for reliable survivorship information and improved communication with healthcare providers regarding what to expect, so they could feel prepared for life post-cancer. These aspects need to be explored more deeply through other qualitative studies.
Interpreting Cancer Survivors’ Perceptions of the Survivor Label Through Social Identity and Communication Accommodation Theories

Abstract:This study uses social identity theory and communication accommodation theory as lenses to examine former cancer patients' perspectives of the "survivor" label, replacement labels for their experience, and use of survivor services. Semi-structured interviews of 43 former cancer patients offer insight into their unique cancer experiences and explain how these events influence their perceptions of the term survivor. Coders used constant comparison methods to capture six themes related to the participants' impressions of the survivor label. When sharing perceptions of the survivor label, participants expressed language that illustrated convergence (It means everything to me), divergence (I don't like to be called anything), convergence and divergence (Part of me is happy … Part of me is kind of aggravated), and apathy (I have no feelings toward the label). Participants also generated new labels that captured their cancer experiences and six unique themes emerged from these responses. Most of the former cancer patients were aware of survivorship programs; however, relatively few used these programs regularly and cited reasons for nonuse explained by social identity theory.

Abstract:Abstract Purpose: To explore how Black adults who have been diagnosed with cancer perceive and interpret the term ‘cancer survivor’. Procedures: This study is a secondary analysis of qualitative data from a project identifying physical activity determinants among Black U.S. residents living past a cancer diagnosis. Semi-structured telephone interviews began with an inquiry into perceptions of the term ‘cancer survivor.’ Eligible participants were age 18 or older, self-identified as Black, and were living beyond a cancer diagnosis. Interviews were audio recorded and transcribed verbatim. Applying directed content analysis, 5 researchers independently applied a codebook generated from the interview guide to a subset of transcripts. The codebook was refined through iterative discussions to address coding discrepancies and achieve replicability. Remaining transcripts were then coded and a report of grouped narrative summaries was generated. Themes and subthemes were organized to present the meaning and significance of ‘cancer survivorship’ among the sample. The partnering community advisory board aided in and reviewed the interpretation of study findings. Results: Participants were 19 survivors of nine different types of cancer. Most participants were women (89.5%), 68.4% held a bachelor’s degree or greater, and the mean age was 56 (SD=12). Emergent themes describing perceptions of ‘cancer survivorship’ included 1) cancer ‘survivor’ terminology preferences, 2) outset of cancer ‘survivorship,’ 3) ‘survivor mentality’ and mental fortitude, and 4) advocacy for those affected by cancer. Many participants identified as ‘cancer survivors’ and preferred this to ‘in remission,’ but some preferred different terms or identities (e.g., warrior or thriver). Many believed survivorship began once an individual completed cancer treatment, but some felt survivorship began at cancer diagnosis. Survivor mentality was raised as a mindset providing some individuals the mental fortitude to complete cancer treatment, especially among those embracing a survivor identity during treatment. Some participants raised advocacy, providing support to others diagnosed with cancer, and sharing cancer information and their experience as part of what it means to be a survivor. Conclusions: Acceptance of the term “cancer survivor” and what it means to be a “survivor” vary among Black individuals living past a cancer diagnosis. The identities held by those diagnosed with cancer may have implications for their cancer experiences and outcomes. Many Black cancer survivors may perceive working to improve the lives of others with cancer as part of what it means to be a survivor. Partnering with Black cancer survivors in cancer prevention and control efforts may be mutually appealing and beneficial to improve the lives of individuals living past a cancer diagnosis. Abstract:Cancer survivors face significant psychological challenges post-treatment, which necessitates an understanding of the resilience mechanisms they employ to cope and thrive. This study aims to explore the psychological resilience of cancer survivors, identifying the emotional responses, coping strategies, support systems, and personal growth experienced during and after their recovery. This qualitative study employed semi-structured interviews with 26 cancer survivors who had completed treatment at least six months prior. The sample included a diverse group of participants in terms of age, gender, and cancer type. Data collection aimed at theoretical saturation and analysis was conducted using NVivo software, following a grounded theory approach to identify themes and sub-themes within the narratives. Four main themes were identified: Emotional Responses, Coping Strategies, Support Systems, and Reflections and Growth. Each theme encompassed several categories with specific concepts: Emotional Responses included initial reactions, coping during treatment, and post-treatment emotions; Coping Strategies covered personal strategies, reliance on support systems, and spiritual and philosophical approaches; Support Systems highlighted the role of family and friends, healthcare providers, and community networks; Reflections and Growth focused on changes in self-perception, impact on relationships, and shifts in life philosophy. The study underscores the complexity of the resilience process among cancer survivors, revealing a broad spectrum of emotional responses, adaptive coping mechanisms, and the crucial role of support systems. The findings emphasize the need for holistic approaches in oncological care that address not only the physical but also the psychological and social aspects of cancer survivorship.
"That bastard chose me": the use of metaphor in women's cancer blogs

Abstract:Metaphors play a significant role in how cancer is experienced and discussed. This study delves into the utilization of metaphors by women bloggers grappling with colorectal and gynecological cancers. By focusing on cancer types less represented in mainstream media, we aim to shed light on cancer cultures associated with body areas often considered taboo. Our findings reveal that widely recognized expressions and stories about cancer, such as the metaphors of battle and narratives promoting optimism and heroism in the face of illness, are deeply ingrained. However, a notable discovery is the prevalence of personification alongside these conventional expressions. Personifying cancer endows it with human characteristics, providing an outlet for bloggers to express their fear and frustration, including articulating feelings of sadness and anger, diverging from narratives centered on heroism and positive thinking. Furthermore, our analysis reveals a significant emphasis on death, underscoring that despite advancements in treatment, bloggers still perceive cancer as highly lethal. Personification can serve both detrimental and therapeutic purposes for bloggers and for societal perceptions of cancer survivorship, both reinforcing and opposing dominant Western discourses surrounding the illness. These findings enrich our understanding of cognitive and cultural tools used to describe cancer within contemporary Western society.

Abstract:Background Cancer threat is relevant to age, and the threat of a foreshortened life coupled with a lengthy treatment process negatively affects middle‐aged and older adults. Understanding the coping throughout the cancer experience in middle‐aged and older cancer survivors will help develop supportive care to promote their physiological and psychological coping effects. Objectives To explore the cancer coping experiences of middle‐aged adults aged 40–59 and older adults over 60. Design A descriptive phenomenological study was employed. Methods Face‐to‐face, in‐depth, semistructured interviews were conducted with 22 oncology patients in a tertiary university hospital aged 40 or above from August to October 2023. The interview data were analyzed using thematic analysis procedures. Results Five themes and 13 subthemes were formed through analysis: acceptance of cancer (considering cancer as chronic, believing in fate and attributing cancer to karma); having different information needs (desired to be truthfully informed, information‐seeking behaviour, information avoidance behaviour); getting families involved (developing dependent behaviours, feeling emotional support, family members suffering worse); striving to maintain positive psychological state (positive thinking, seeking peer support) and negative experience (undesirable, low self‐esteem). Conclusion Our study reveals that cancer survivors' attitudes towards having cancer have changed from a death sentence to a more positive perception of a chronic disease. Supportive programmes for developing coping strategies should consider the cultural traditions and religious beliefs, different information needs, involvement of family and promoting a positive psychological state while avoiding negative factors. Patient or Public Contribution Participants with experience of coping with cancer were involved in the semistructured interview.
