Abstract:Objectire:To investigate the clinical feasibility of dual-source CT enterography using dual-energy virtual non-enhanced CT (VNCT)technique.Methods:CT enterography were performed in 39 patients with dual source CT scan-ner,including conventional non-enhanced CT scan (CNCT),arterial and venous phase contrast-enhanced CT scan (CECT) with dual energy mode (100kVp/230mAs and 140kVp/178mAs).VNCT images of arterial and venous phase were obtained after being post-processed with Liver-VNC software.Using CNCT as a standard,the image quality of VNCT images were evaluated and scored with 1~4 grade scoring system (the higher score indicated better image quality:4 scores means per-fect,1 score means unsatisfactory for diagnosis).The radiation dose of CNCT,arterial and venous phase CECT was com-pared.Results:Of 39 patients,there were 25 cases with lesions and 14 cases of normal.Compared with CNCT images,all the lesions were also detected accurately by VNCT images,and there was no significant difference of size and extent of lesions between CNCT and VNCT.The scores of VNCT images were all higher than 2,there was no significant statistic difference between the score of two doctors,and the image quality of VNCT images met the requirement of diagnosis.The average dose length product (DLP)of conventional non-enhanced CT,arterial and venous phase CECT were (283.8750±57.1195), (289.3750±53.1543 )and (301.2917 ± 55.3346 )mGy · cm respectively,with no statistic difference (F = 0.615,P =0.544).Conclusion:Dual energy CT virtual non-enhanced images may replace true non-enhanced images in CT enterography and then can reduce radiation dose of examination.
Virtual non-contrast of liver from dual energy CT: a clinical application

Abstract:Objective To assess the virtual non-contrast liver CT from dual-energy CT for the clinical application. Methods In total, 51 patients were included in the study, and all patients underwent multi-phase liver CT on a dual-source CT. The True non-contrast liver CT (TNCT) was performed in a single-energy acquisition mode, but the arterial and portovenous liver CT (VNCT) were performed in a dual-energy mode of 110 kV and 140 kV respectively. The virtual non-contrast CT images were derived from the arterial data using liver virtual non-contrast software. Between the true non-contrast CT and the virtual non-contrast CT, the image quality, mean CT HU values in the liver and muscle, signal to noise (SNR), the radiation dose of volume CT dose index (CTDIvol) and dose length product (DLP) in a single phase and total examination were compared with t test. Results There was no significant difference in the detection of liver lesions between TNCT and VNCT. The CT Hu values of muscle on both TNCT and VNCT images were almost equal. The CT HU values of liver on VNCT images were higher than that on TNCT images and the difference was significant [61.32 ±6. 04 vs. (56. 85 ±4. 80) HU, t = -3. 927,P＜0.01]. There was also significant differenc of SNR between TNCT (11.28±2. 78) and VNCT (8.65 ± 1.56) images( t =-5.590,P＜0.01). The CTDIvol and DLP of single phase were (7.07 ±0.85) mGy and (155.11 ±respectively, but in TNCT the total CTDIvol and DLP reached (21.43 ± 2. 46 ) mGy and (469. 02 ±significance, but the total CTDIvol and DLP were significantly different (t = 16. 168 and 13. 132, P ＜0. 01). Conclusion With the consequent reduction in radiation dose, the VNCT can replace TNCT as an imaging protocol in multi-phase abdominal CT examination in clinic.

Abstract:Objective: This study aimed to evaluate virtual monochromatic images (VMIs) obtained using dual-layer dual-energy computed tomography (CT) for breast carcinoma. Methods: We retrospectively enrolled 28 patients with breast cancer who were pathologically diagnosed using dual-layer dual-energy CT. Virtual monochromatic images (40-200 keV) were generated. We compared CT number, image noise, contrast, and contrast-to-noise ratio (CNR) between VMIs with the highest CNR and conventional CT images. We performed qualitative image analysis between VMIs at optimized energy and conventional CT images. Results: Image noise of VMIs was not significantly different from that of the conventional CT images. As the x-ray energy decreased, CNR increased. The 40-keV VMIs were highest CNR and higher than that of the conventional CT images. In qualitative image analysis, the 40-keV images were significantly higher than conventional CT images. Conclusion: Both qualitative and quantitative analyses showed that the image quality of VMIs at 40 keV was significantly higher than that of conventional CT images.
Application of Computed Tomography Virtual Noncontrast Spectral Imaging in Evaluation of Hepatic Metastases: a Preliminary Study.

Abstract:OBJECTIVE:The objective was to qualitatively and quantitatively evaluate hepatic metastases using computed tomography (CT) virtual noncontrast (VNC) spectral imaging in a retrospective analysis. METHODS:Forty hepatic metastases patients underwent CT scans including the conventional true noncontrast (TNC) and the tri-phasic contrast-enhanced dual energy spectral scans in the hepatic arterial, portal venous, and equilibrium phases. The tri-phasic spectral CT images were used to obtain three groups of VNC images including in the arterial (VNCa), venous (VNCv), and equilibrium (VNCe) phase by the material decomposition process using water and iodine as a base material pair. The image quality and the contrast-to-noise ratio (CNR) of metastasis of the four groups were compared with ANOVA analysis. The metastasis detection rates with the four nonenhanced image groups were calculated and compared using the Chi-square test. RESULTS:There were no significant differences in image quality among TNC, VNCa and VNCv images (P > 0.05). The quality of VNCe images was significantly worse than that of other three groups (P < 0.05). The mean CNR of metastasis in the TNC and VNCs images was 1.86, 2.42, 1.92, and 1.94, respectively; the mean CNR of metastasis in VNCa images was significantly higher than that in other three groups (P < 0.05), while no statistically significant difference was observed among VNCv, VNCe and TNC images (P > 0.05). The metastasis detection rate of the four nonenhanced groups with no statistically significant difference (P > 0.05). CONCLUSIONS:The quality of VNCa and VNCv images is identical to that of TNC images, and the metastasis detection rate in VNC images is similar to that in TNC images. VNC images obtained from arterial phase show metastases more clearly. Thus, VNCa imaging may be a surrogate to TNC imaging in hepatic metastasis diagnosis.
Determining the Optimal Energy Level of Virtual Monoenergetic Images in Dual-Source CT for Diagnosis of Bowel Obstruction and Colitis

Abstract:Images from 64 patients undergoing an enhanced abdominal-pelvis scan at portal phase in dual-energy CT mode for the diagnosis of colitis or bowel obstruction were retrospectively analyzed. Acquisitions were performed on a third-generation dual-source CT (DSCT) 100/Sn150 kVp. Mixed images were generated, as well as virtual monoenergetic images (VMIs) at 40/50/60/70 keV. Objective image quality was assessed on VMIs and mixed images by measuring contrast, noise and contrast-to-noise ratio (CNR). Noise, smoothing and overall image quality were subjectively analyzed by two radiologists using Likert scales. For both patient groups, the noise decreased significantly according to the energy level from 40 to 60 keV by −47.2 ± 24.0% for bowel obstruction and −50.4 ± 18.2% for colitis. It was similar between 60 and 70 keV (p = 0.475 and 0.059, respectively). Noise values were significantly higher in VMIs than in mixed images, except for 70 keV (p = 0.53 and 0.071, respectively). Similar results were observed for contrast values, with a decrease between 40 and 70 keV of −56.3 ± 7.9% for bowel obstruction −56.2 ± 10.9% for colitis. The maximum CNR value was found at 60 keV compared to other energy levels and mixed images, but there was no significant difference with the other energy levels apart from 70 keV (−9.7 ± 9.8%) for bowel obstruction and 40 keV (−6.6 ± 8.2%) and 70 keV (−5.8 ± 9.2%) for colitis. The VMIs at 60 keV presented higher scores for all criteria for bowel obstruction and colitis, with no significant difference in smoothing score compared to mixed images (p = 0.119 and p = 0.888, respectively).

Abstract:Objective To qualitatively and quantitatively evaluate the feasibility of CT virtual non-contrast(VNC) spectral imaging in colon carcinomas. Methods Forty patients with pathologically proven colon carcinomas underwent gemstone spectral imaging(GSI) including non-contrast(TNC), hepatic arterial, portal venous and equilibrium phase acquisitions. The triphasic GSI dataset was sent to workstation and 3 sets of VNC images(including VNCa, VNCv, VNCe) were obtained by subtract iodine from iodine-water images in the triphasic enhanced GSI images, respectively. The quality of VNCs and TNC images was evaluated on a five-point scale. Interobserver agreement with regard to image quality was assessed using Cohen's kappa. The contrast-to-noise ratio(CNR) of carcinoma-normal colon wall in the TNC and VNCs images was calculated, and difference between these four datasets was compared with ANOVA analysis. Diagnosis accuracy of colon serosal invasion in TNC and VNCs was calculated according to operation findings and compared with chi-square test. The diagnosis accuracy of perigastric lymphadenectasis in VNCs was evaluated with comparison to TNC images. Results Interobserver agreement with regard to image quality was excellent(k>0.700). There was no significant difference among the image quality of TNC, VNCa and VNCv. The image quality of VNCe was significant worse than that of other three groups; Mean CNR of carcinoma- normal colon wall in the TNC and VNCs images was 0.51,0.87,0.75,0.49 respectively and that of VNCa and VNCv significant higher than that of TNC(P<0.05). No statistically significant difference was observed between VNCe and TNC(P>0.05). There was no significant difference between diagnosis accuracy of colon serosal invasion in TNC and VNCs. The diagnosis accuracy of perigastric lymphennopsy in VNCa, VNCv and VNCe was 100%, 100%, 100% respectively. Conclusion VNC images obtain from arterial phase and portal venous may be a surrogate for conventional non-contrast scan in colon carcinoma diagnosis.
Possible Contrast Media Reduction with Low keV Monoenergetic Images in the Detection of Focal Liver Lesions: A Dual-Energy CT Animal Study

Abstract:Objective: To investigate the feasibility of dual-energy CT for contrast media (CM) reduction in the diagnosis of hypervascular and hypovascular focal liver lesions (FLL). Subjects and methods: The Institutional Animal Care and Use Committee approved this study. VX2 tumors were implanted in two different segments of the liver in 13 rabbits. After 2 weeks, two phase contrast enhanced CT scans including the arterial phase (AP) and portal-venous phase (PVP) were performed three times with 24-hour intervals with three different concentrations of iodine, 300 (I(300)), 150 (I(150)) and 75 mg I/mL (I(75)). The mean HU and standard deviation (SD) were measured in the liver, the hypervascular portion of the VX2 tumor which represented hypervascular tumors, and the central necrotic area of the VX2 tumor which represented hypovascular tumors in 140 kVp images with I(300) as a reference standard and in monoenergetic images (between 40 keV and 140 keV) with I(150) and I(75). The contrast-to-noise ratio (CNR) for FLLs and the ratio of the CNRs (CNR(ratio)) between monoenergetic image sets with I(150) and I(75), and the reference standard were calculated. Results: For hypervascular lesions, the CNR(ratio) was not statistically different from 1.0 between 40 keV and 70 keV images with I(150), whereas the CNR(ratio) was significantly lower than 1.0 in all keV images with I(75). For hypovascular lesions, the CNR(ratio) was similar to or higher than 1.0 between 40 keV and 80 keV with I(150) and between 40 keV and 70 keV with I(75).. Conclusions: With dual-energy CT, the total amount of CM might be halved in the diagnosis of hypervascular FLLs and reduced to one-fourth in the diagnosis of hypovascular FLLs, while still preserving CNRs.
Deep learning-based reconstruction of virtual monoenergetic images of kVp-switching dual energy CT for evaluation of hypervascular liver lesions: Comparison with standard reconstruction technique

Abstract:Objective: To investigate clinical applicability of deep learning(DL)-based reconstruction of virtual monoenergetic images(VMIs) of arterial phase liver CT obtained by rapid kVp-switching dual-energy CT for evaluation of hypervascular liver lesions. Materials and methods: We retrospectively included 109 patients who had available late arterial phase liver CT images of the liver obtained with a rapid switching kVp DECT scanner for suspicious intra-abdominal malignancies. Two VMIs of 70 keV and 40 keV were reconstructed using adaptive statistical iterative reconstruction (ASiR-V) for arterial phase scans. VMIs at 40 keV were additionally reconstructed with a vendor-agnostic DL-based reconstruction technique (ClariCT.AI, ClariPi, DL 40 keV). Qualitative, quantitative image quality and subjective diagnostic acceptability were compared according to reconstruction techniques. Results: In qualitative analysis, DL 40 keV images showed less image noise (4.55 vs 3.11 vs 3.95, p < 0.001), better image sharpness (4.75 vs 4.16 vs 4.3, p < 0.001), better image contrast (4.98 vs 4.72 vs 4.19, p < 0.017), better lesion conspicuity (4.61 vs 4.23 vs 3.4, p < 0.001) and diagnostic acceptability (4.59 vs 3.88 vs 4.09, p < 0.001) compared with ASiR-V 40 keV or 70 keV image sets. In quantitative analysis, DL 40 keV significantly reduced image noise relative to ASiR-V 40 keV images (49.9%, p < 0.001) and ASiR-V 70 keV images (85.2%, p = 0.012). DL 40 keV images showed significantly higher CNRlesion to the liver and SNRliver than ASiR-V 40 keV image and 70 keV images (p < 0.001). Conclusion: DL-based reconstruction of 40 keV images using vendor-agnostic software showed greater noise reduction, better lesion conspicuity, image contrast, image sharpness, and higher overall image diagnostic acceptability than ASiR for 40 keV or 70 keV images in patients with hypervascular liver lesions.
Image quality evaluation of dual-layer spectral detector CT of the chest and comparison with conventional CT imaging

Abstract:OBJECTIVES: To evaluate image quality parameters of virtual mono-energetic (MonoE) and conventional (CR) imaging derived from a dual-layer spectral detector CT (DLCT) in oncological follow-up venous phase imaging of the chest and comparison with conventional multi-detector CT (CRMDCT) imaging.MATERIALS AND METHODS: A total of 55 patients who had oncologic staging with conventional CT and DLCT of the chest in venous phase were included in this study. Established image quality parameters were derived from all datasets in defined thoracic landmarks. Attenuation, image noise, and signal-/contrast- to noise ratios (SNR, CNR) were compared between CRDLCT and MonoE as well as CRMDCT imaging. Two readers performed subjective image analysis.RESULTS: CRMDCT showed significant lower attenuation values compared to CRDLCT and MonoE at 40-70keV (p≤0.05). Moreover, MonoE at 40-70keV revealed significantly higher attenuations values compared to CRDLCT (p<0.001). Noise was statistically lower in CRMDCT compared with CRDLCT and MonoE at 40keV (11.4±2.3 HU vs. 12.0±3.1 HU vs. 11.7±5.2 HU; p<0.001). In contrast, all MonoE levels showed significantly lower noise levels compared to CRDLCT (p<0.001). SNR was not significantly different between CRMDCT and CRDLCT (13.5±3.7 vs. 14.4±5.3; p>0.99). SNR values were significantly increased for MonoE at 40-80keV compared to CRMDCT and CRDLCT (p<0.001). CRDLCT and MonoE (40-70keV) from DLCT revealed significantly higher CNR values than CRMDCT (p<0.001). In subjective analysis, MonoE at 40keV surpassed all other image reconstructions except for noise in MonoE at 70 keV.CONCLUSION: In dual-layer spectral detector CT, MonoE at low keV showed superior image quality compared to conventional images derived from the same system and may therefore be added to clinical routine imaging protocols. Whether MonoE reconstructions yield additional diagnostic information is still unknown.

Abstract:Abstract Background Renal cell carcinoma (RCC) is the most common renal malignant tumour. We evaluated the potential value and dose reduction of virtual non-contrast (VNC) images and virtual monoenergetic images (VMIs) from dual-layer spectral CT (DL-CT) in the diagnosis of RCC. Results Sixty-two patients with pathologically confirmed RCC who underwent contrast-enhanced DL-CT were retrospectively analysed. For the comparison between true non-contrast (TNC) and VNC images of the excretory phase, the attenuation, image noise, signal-to-noise ratio (SNR) and subjective image quality of tumours and different abdominal organs and tissues were evaluated. To compare corticomedullary phase images and low keV VMIs (40 to 100 keV) from the nephrographic phase, the attenuation, image noise, SNR and subjective lesion visibility of the tumours and renal arteries were evaluated. For the tumours, significant differences were not observed in attenuation, noise or SNR between TNC and VNC images ( p > 0.05). For the abdominal organs and tissues, except for fat, the difference in attenuation was 100% within 15 HU and 96.78% within 10 HU. The subjective image quality of TNC and VNC images was equivalent ( p > 0.05). The attenuation of lesions in 40 keV VMIs and renal arteries in 60 keV VMIs were similar to those in the corticomedullary images ( p > 0.05). The subjective lesion visibility in low keV VMIs is slightly lower than that in the corticomedullary images ( p < 0.05). Using VNC and VMIs instead of TNC and corticomedullary phase images could decrease the radiation dose by 50.5%. Conclusion VNC images and VMIs acquired from DL-CT can maintain good image quality and decrease the radiation dose for diagnosis of RCC.

Clinical Feasibility of Dual-Layer CT With Virtual Monochromatic Image for Preoperative Staging in Patients With Breast Cancer: A Comparison With Breast MRI

Abstract:Objective: Dual-layer CT (DLCT) can create virtual monochromatic images (VMIs) at various monochromatic X-ray energies, particularly at low keV levels, with high contrast-to-noise ratio. The purpose of this study was to assess the clinical feasibility of contrast-enhanced chest DLCT with a low keV VMI for preoperative breast cancer staging, in comparison to breast MRI. Materials and methods: A total of 152 patients with 155 index breast cancers were enrolled in the study. VMIs were generated from contrast-enhanced chest DLCT at 40 keV and maximum intensity projection (MIP) with three-dimensional (3D) reconstruction was performed for both bilateral breast areas. Two radiologists reviewed in consensus the 3D MIP images of the chest DLCT with VMI and breast MRI in separate sessions with a 3-month wash-out period. The detection rate and mean tumor size of the index cancer were compared between the chest DLCT with VMI and breast MRI. Additionally, the agreement of tumor size measurement between the two imaging modalities were evaluated. Results: Of all index cancers, 84.5% (131/155) were detected in the chest DLCT with VMI, while 88.4% (137/155) were detected in the breast MRI (P = 0.210). The Bland-Altman agreement between the chest DLCT with VMI and breast MRI was a mean difference of -0.05 cm with 95% limits of agreement of -1.29 to 1.19 cm. The tumor size in the chest DLCT with VMI (2.3 ± 1.7 cm) was not significantly different from that in the breast MRI (2.4 ± 1.6 cm) (P = 0.106). Conclusion: The feasibility of chest DLCT with VMI was demonstrated for preoperative tumor staging in breast cancer patients, showing comparable cancer detectability and good agreement in tumor size measurement compared to breast MRI. This suggests that chest DLCT with VMI can serve as a potential alternative for patients who have contraindications to breast MRI.

Abstract:Objectives: To determine the optimal keV on dual-layer, dual-energy CT enterography (dlDE-CTE) and to investigate the added value of virtual monoenergetic images (VMIs) for the diagnosis of active Crohn's disease (CD). Methods: We collected 76 patients (including 45 CD patients) who underwent dlDE-CTE. CD was diagnosed using ileocolonoscopy. Conventional polychromatic images (PCI) were reconstructed using an iterative reconstruction algorithm at 120 kVp, and VMI at 40 keV (VMI40), 55 keV (VMI55), and 70 keV (VMI70). Contrast-to-noise ratio (CNR) was compared using Kruskal-Wallis test. Three radiologists independently reviewed PCI and subsequently combined PCI and the optimized VMI for the diagnosis of active CD using a 5-point scale. Multi-reader multi-case receiver operating characteristic analysis was performed. Results: Mean ± standard deviation of CNRs for both normal (13.6±6.5, 6.1±3.2, 2.0±2.1, 1.9±1.6; P<0.001) and abnormal (9.4±7.3, 6.5±4.8, 4.9±3.1, 3.7±2.3; P<0.001) bowels were significantly greatest on VMI40, followed by VMI55, VMI70, and PCI. When VMI40 were added to PCI, overall area-under-the-curve of the three radiologists was significantly improved from 0.891 to 0.951 for diagnosing active CD (P=0.009). Conclusions: The lowest monoenergetic images (VMI40) provided the best CNR on dlDE-CTE. Furthermore, the diagnostic performance for diagnosing active CD can be significantly improved with the addition of VMI40. Key points: • CNR for both normal and abnormal bowel walls is greatest on VMI 40 . • Subjective image quality on VMI 40 is better than those on PCI. • When VMI 40 images are added to PCI, radiologists' diagnostic performance can be improved.
Clinical application of virtual non-contrast CT in diagnosing rectal cancer with second-generation dual-source computed tomography: a preliminary study

Abstract:Objective To evaluate the clinical feasibility of dual-energy virtual non-enhanced CT in rectal cancer patients.Methods Forty-nine patients with rectal cancer proved by biopsy underwent conventional CT scan and pelvic dual-energy scan.The Liver VNC software was used to reform the virtual non-contrast CT(VNCT) at arterial and portal phase.The mean CT value,noise,image quality,and radiation dose for virtual non-contrast CT(VNCT) and true non-contrast CT(TNCT) imaging were compared.Results There was no statistical difference in mean CT value of all tissues except for external iliac artery and subcutaneous fat(P>0.05).The noise of VNCT images was lower than TNCT images(P< 0.01).The image quality of VNCT was lower than TNCT(P<0.01) but was still diagnostic.The reduction of radiation dose by omitting the TNCT scan was about(31.49±1.47)%.Conclusions VNCT scan could reduce radiation dose and may potentially replace TNCT for multi-phase rectum preoperative staging.

Abstract:Background: Colorectal cancer (CRC) is commonly assessed by computed tomography (CT), but the associated radiation exposure is a major concern. This study aimed to quantitatively and qualitatively compare the image quality of virtual non-contrast (VNC) images reconstructed from arterial and portal venous phases with that of true non-contrast (TNC) images in patients with CRC to assess the potential of TNC images to replace VNC images, thereby reducing the radiation dose. Methods: A total of 69 patients with postoperative pathologically confirmed CRC at the West China Hospital of Sichuan University between May 2022 and April 2023 were enrolled in this cross-sectional study. The CT protocol included the acquisition of TNC images, arterial and portal venous phase images; the VNC images were reconstructed from the two postcontrast phase images. Several parameters, including the CT attenuation value, absolute attenuation error, imaging noise [standard deviation (SD)], signal-to-noise ratio (SNR), and contrast-to-noise ratio (CNR), were measured in multiple abdominal structures for both the TNC and VNC images. Two blinded readers assessed the subjective image quality using a five-point scale. Interobserver agreement was evaluated using interclass correlation coefficients (ICCs). The paired t-test and Wilcoxon signed-rank test were used to compare the objective and subjective results between the TNC and VNC images. Individual measurements of radiation doses for the TNC scan and contrast scan protocols were recorded. Results: A total of 2,070 regions of interest (ROIs) of the 69 patients were analyzed. Overall, the VNC images exhibited significantly lower attenuation values and SD values than the TNC images in all tissues, except for the abdominal aorta, portal vein, and spleen. The mean absolute attenuation errors between the VNC and TNC images were all less than 10 Hounsfield units (HU). The percentages of absolute attenuation errors less than 5 and 10 HU in the VNC images from the arterial phase (VNCa) were 78.99% and 97.97%, respectively, while those from the portal venous phase (VNCp) were 81.59% and 96.96%, respectively. The absolute attenuation errors between the TNC and VNCa images were smaller than those between the TNC and VNCp images for tumors [VNCaerror: 2.77, interquartile range (IQR) 1.77-4.22; VNCperror: 3.27, IQR 2.68-4.30; P=0.002]. The SNR values and CNR values in the VNC images were significantly higher than those in the TNC images for all tissues, except for the portal vein and spleen. The image quality was rated as excellent (represented by a score of 5) in the majority of the TNC and VNC images; however, the VNC images scored lower than the TNC images. Eliminating the TNC phase resulted in a reduction of approximately 37.99% in the effective dose (ED). Conclusions: The VNC images provided accurate CT attenuation, good image quality, and lower radiation doses than the TNC images in CRC, and the VNCa images showed minimal differences in the CT attenuation of the tumors.
Virtual unenhanced CT imaging replaces the conventional unenhanced CT imaging in liver:a feasibility study

Abstract:Objective To assess the feasibility of virtual unenhanced CT imaging obtained by gemstone spectral imaging(GSI)replacing the conventional unenhanced scan in liver by comparing the image quality and radiation dose.Methods 41 patients with suspected liver disease underwent CT(GE DISCOVERY CT750HD)scan,including conventional unenhanced(CU)imaging and dual phase enhanced imaging with GSI mode.The reconstructed data were post-processed with GSI viewer for water base images,which were called virtual unenhanced(VU)imaging.The noise and artifact of CU images were compared with that of VU by two blinded radiologists using a four-point grade scale(excellent:1;bad 4).The effective radiation dose of a GSI dual-phase protocol(arterial and portal phases)was compared with that of a conventional triple phase protocol(CU,arterial and portal phases).Independent-samples t test(SPSS 13)was performed on these values between CU and VU images.Sensitivity and specificity were also compared by X2 test.Results A significant difference(P <0.01)in image noise was observed between VU(2.08±0.66)and CU(1.10±0.3).No significant difference in artifact was observed between VU(1.56±0.35)and CU(1.45±0.28)images.Both VU and CU images were acceptable.Diagnostic sensitivity of VU and CU was 65.67%and 62.69%,respectively(P >0.05).Specificity of VU and CU was all 100%.Effective radiation dose of dual-phase and triple-phase was(15.51±8.56)mSv,(19.50±4.08)mSv,respectively(P <0.01).The dose reduction achieved by omitting the unenhanced acquisition was 21.09%.Conclusion VU images can be obtained with similar image quality as CU.GSI dual-phase imaging in liver will be a good substitute since its radiation can be greatly reduced compared with traditional triple-phase imaging.
Dual-Energy CT for the Detection of Portal Vein Thrombosis: Improved Diagnostic Performance Using Virtual Monoenergetic Reconstructions

Abstract:Purpose: To investigate the diagnostic performance of noise-optimized virtual monoenergetic images (VMI+) in dual-energy CT (DECT) of portal vein thrombosis (PVT) compared to standard reconstructions. Method: This retrospective, single-center study included 107 patients (68 men; mean age, 60.1 ± 10.7 years) with malignant or cirrhotic liver disease and suspected PVT who had undergone contrast-enhanced portal-phase DECT of the abdomen. Linearly blended (M_0.6) and virtual monoenergetic images were calculated using both standard VMI and noise-optimized VMI+ algorithms in 20 keV increments from 40 to 100 keV. Quantitative measurements were performed in the portal vein for objective contrast-to-noise ratio (CNR) calculation. The image series showing the greatest CNR were further assessed for subjective image quality and diagnostic accuracy of PVT detection by two blinded radiologists. Results: PVT was present in 38 subjects. VMI+ reconstructions at 40 keV revealed the best objective image quality (CNR, 9.6 ± 4.3) compared to all other image reconstructions (p < 0.01). In the standard VMI series, CNR peaked at 60 keV (CNR, 4.7 ± 2.1). Qualitative image parameters showed the highest image quality rating scores for the 60 keV VMI+ series (median, 4) (p ≤ 0.03). The greatest diagnostic accuracy for the diagnosis of PVT was found for the 40 keV VMI+ series (sensitivity, 96%; specificity, 96%) compared to M_0.6 images (sensitivity, 87%; specificity, 92%), 60 keV VMI (sensitivity, 87%; specificity, 97%), and 60 keV VMI+ reconstructions (sensitivity, 92%; specificity, 97%) (p ≤ 0.01). Conclusions: Low-keV VMI+ reconstructions resulted in significantly improved diagnostic performance for the detection of PVT compared to other DECT reconstruction algorithms.
