Abstract:Background: Losing competent nurses due to turnover can have adverse effects on healthcare administration, service delivery, and organizational dynamics. Frequent staff turnover can hinder optimal nursing practices, but some nurses remain committed. Therefore, understanding factors that foster sustained engagement and retention is crucial to create a retention strategy. Objective: This study aimed to explore the current experience of Indonesian nurses who retain their jobs in the hospital setting. Methods: A phenomenological design was employed in this study, with ten nurses selected using purposive sampling. Semi-structured interviews were used to gather data from a private hospital in Batam, Indonesia, between January and March 2022. A conventional content analysis was used to analyze the data. Results: The participants had an average total work experience of 10.4 years and an average age of 35.6 years, ranging from 28 to 44 years. The majority of the participants were females, held bachelor's degrees, and were married. Three main themes were developed in this study: solid teamwork, adequate compensation, and professional career development. Conclusion: Retaining experienced nurses in hospitals is influenced by antecedents such as teamwork, pay, and career growth. These findings offer nursing and hospital managers insights into developing policies and strategies to improve hospital nursing retention programs in Indonesia and beyond.

Abstract:Aim To explore factors associated with job satisfaction between novice and experienced nurses. Background Job satisfaction is crucial for better quality of patient care and nurses' job engagement in clinical setting. However, job dissatisfaction is commonly reported from clinical nurses leading to poorer patient outcomes and high turnover rate. Job satisfaction at different professional stages may have different expectations. Knowledge about factors influencing job satisfaction of junior and senior nurses is inadequate. Design A phenomenological study was conducted using face-to-face individual interviews. Method Semi-structured interviews were conducted and audio-recorded. The Herzberg's Two-Factor Theory was used to understand the phenomena. Purposeful sampling was used to select eligible nurse participants who were working in hospital settings. Results Eight participants, consisting of four junior and four senior nurses, with clinical experience ranging from 1 to more than 25 years, were selected in this study. Four main themes related to job satisfaction were identified: "supportive working environment," "autonomy in practice," "professional training for competence enhancement," and "heavy workload and insufficient manpower." These themes highlighted the common aspects that contribute to job satisfaction among nurses according to the Herzberg's Two-Factor Theory. Conclusion The findings of this study contribute to our understanding of the factors that influence job satisfaction among both novice and experienced nurses. While four themes highlighting common aspects of their needs have been identified, it is important to acknowledge that novice and experienced nurses may have their own unique perspectives within these aspects. According to Herzberg's Two-Factor Theory, job satisfaction factors can be complex and diverse. To enhance job satisfaction among nurses, hospital policymakers and senior management should be attentive to identify factors that have an impact on job satisfaction of nurses at different stages of their clinical experience. Developing a strategic plan to meet the needs of nurses based on their experience levels should take into consideration the disparities in work environment and responsibilities, ultimately job satisfaction can be improved, leading to better overall patient care.

How Do We Approach Quality Care for Patients from Middle Eastern Countries? A Phenomenological Study of Korean Nurses' Experiences

Abstract:Purpose: Although more people from Middle Eastern countries are visiting South Korea for medical treatment, Korean nurses lack experience in treating them. Understanding and describing Korean nurses' experiences can help them provide quality care to these patients by enhancing their competency in culturally appropriate care. This study described the experiences of nurses who provide care to Middle Eastern patients in clinical settings in South Korea. Methods: We conducted a phenomenological study to describe nurses' experience of caring for patients from Middle Eastern countries. Ten nurses with prior experience in caring for these patients were recruited from a university-affiliated tertiary hospital. Semi-structured face-to-face interviews were conducted between May 1 and June 4, 2020. The transcribed data were analyzed using Giorgi's phenomenological method to identify the primary and minor categories representing nurses' experiences. Results: Four major categories (new experiences in caring for culturally diverse patients, challenges in caring for patients in a culturally appropriate manner, nursing journey of mutual agreement with culturally diverse patients, and being and becoming more culturally competent) and 11 subcategories were identified. Conclusion: Nurses experience various challenges when caring for Middle Eastern patients with diverse language and cultural needs. However, nurses strive to provide high-quality care using various approaches and experience positive emotions through this process. To provide quality care to these patients, hospital environments and educational programs must be developed that center on field nurses and students and support them in delivering quality care while utilizing their cultural capabilities.

Abstract:The world is facing various challenges in response to a growing aging population and increasing burden of chronic disease. A forecast has predicted that by 2030, 25% of Europe's population and 20% of America's population will be over 65 years old [ 1 ] while 26.19% of China's population will be aged 60 and older. [ 2 ] About 21% of the elderly aged ≥ 60 years in India have at least one chronic disease, [ 3 ] the latter of which affects up to 81.1% of older people aged ≥ 60 years in China, [ 4 ] which has dramatically increased the demand for nursing resources. Moreover, the COVID-19 pandemic has intensified nursing staff workloads. [ 5 ] Nowadays, the acute shortage of nurses and high turnover rates, including among novice nurses, are of great concern in many countries.

A Qualitative Study on the Experiences and Attributions for Resigned Nurses with Career Plateau

Abstract:OBJECTIVES:This study seeks to understand the experiences of nurses who resigned because of limited available career development opportunities and of the possible reasons for their resignation.METHODS:Semi-structured interviews were conducted with nine clinical nurses who experienced limitations in the development of their careers and consequently left their jobs to elicit their feelings and reasons for leaving. The content analysis was used to analyze the interview data.RESULTS:Two themes and four subthemes relating to the experiences and attributions of former nurses were identified. Nurses who experienced career plateau were disappointed with the current work and uncertainty about the future. The attribution of career plateau lies in unsatisfied personal needs and the absence of organizational support.CONCLUSIONS:Nurses had strong career plateau experiences before they left, and it caused a series of negative feelings in their practice. Establish a clear career development path and multi-dimensional support from the organization may be helpful to reduce the occurrence of career plateau, thus increasing the retention of nurses.

Abstract:Background High nurse turnover in hospitals is a worldwide problem with dire consequences for patient care such as increased mortality and a decrease in patient safety. A specific effort to retain nurses is urgently needed due to the many vacant nursing positions, especially across the Medical hospital departments. Aim To identify the principal deteriorating factors experienced by nurses relating to their intentions to leave the medical department at a university hospital. Methods A rapid qualitative research methodology was used. Participants comprised 22 registered nurses employed in five university hospital medical department units. The Rigorous and Accelerated Data Reduction (RADaR) technique was therefore used for data collection through qualitative semi‐structured interviews and analysis. COREQ was used for reporting the study. Results The RADaR analysis detected a downward spiral of five principal deteriorating factors influencing nurses' intentions to leave their position in the medical department. The factors were resignations from several nursing colleagues combined with too few and inexperienced nurses present during shifts, additional tasks assigned, a management refraining from improving the problematic issues, leading to decisive consequences for patient care and a declining feeling of professional care. Conclusions The lack of nurses in the department caused missed nursing care, which affected the nurses' job satisfaction and intentions to leave their positions. Future research initiatives must focus on evaluating successful interventions to maintain the nurses in the positions. Further knowledge, is also needed, to investigate how we can change the downward spiral to a story of retention success.

New Graduate Nurses' Experiences by Generation in South Korea: A Qualitative Meta-Synthesis

Abstract:Background: As the nursing organization environment evolves into one in which several generations work together, there are concerns about generational differences leading to conflicts. Purpose: A synthesis of qualitative studies was used to gain insight into the experiences and perceptions of new nurses working in South Korean hospitals and identify generational differences between nurses of Generations X and Y. Methods: Eight databases were searched systematically for qualitative studies published up to June 2020. The Critical Appraisal Skills Programme Checklist for Qualitative Research was used to evaluate the quality of the retrieved studies, and the data were synthesized thematically using the Thomas and Harden method. Results: Of the 2,482 studies retrieved, 19 were included in this meta-synthesis. The experience of new nurses were presented in the three analytical themes of work experience, interpersonal relationships, and adaptation, from which eight descriptive themes were derived: (a) dissatisfied with own work performance, (b) negative feedback on work outcomes, (c) irregular daily life, (d) colleagues who help adapt to work life or make work life harder, (e) feelings of sympathy toward peers, (f) disrespectful patients or caregivers, (g) effective reactions, and (h) ineffective reactions. Although the new nurses from Generations X and Y reported similar experiences during their first year of nursing, differences were found in their values and tendencies regarding work and interpersonal relationships. Compared with new Generation X nurses, new Generation Y nurses placed more value on the meaning of individual work contributions, intimacy with colleagues, and appropriate feedback. Conclusions/implications for practice: The findings of this study support nursing administrators implementing strategies that both acknowledge and reduce the impact of generational category on nursing teams. Furthermore, emphasizing communication and acknowledging ongoing change as generations progress may strengthen bonds that lead nurses to value each other and their work.
Understanding workforce experiences in the early career period of Australian midwives: insights into factors which strengthen job satisfaction

Abstract:Objective: The aim of this study was to explore the experiences of early career midwives in Australia and identify the organisational, work environment, personal factors and stressors that influence workforce participation. Design and setting: A qualitative study, using in-depth semi-structured interviews, was undertaken with midwives 6 - 7 years post-qualification. Qualitative content analysis identified key themes and sub-themes. Participants: Twenty-eight midwives from one Australian university (graduating years 2007 and 2008) were included. Their pre-registration education was via either a Bachelor of Midwifery (direct-entry) or a Gradate Diploma of Midwifery (post-nursing degree). Findings: Three themes were generated: (i) 'sinking and swimming'; (ii) 'needing a supportive helping hand'; and (iii) 'being a midwife … but'. The initial transition into midwifery was overwhelming for most participants, particularly when providing intrapartum care. Job satisfaction was strongly related to having a well-developed midwife-woman relationship in clinical care and being able to work to their full scope of practice. Dissatisfaction stemmed from remuneration concerns, inflexibility of rostering, high workloads, and poor managerial approaches. Experiences of bullying were ubiquitous. Factors inducing midwives to stay in their profession were not the absence of those that caused dissatisfaction. The midwife-woman relationship sustained their practice despite those factors that caused dissatisfaction. Key conclusions: Building strategies that strengthen job satisfaction in midwives is vital. Strategies that provide relational aspects of midwifery practice, ongoing support, rostering flexibility, induce psychological wellbeing, and address workplace bullying, may assist in the early career transition. Access to continuity of midwifery care models as new graduates is warranted. Implications for practice: Continued professional development and career progression strategies are needed for midwives to cultivate their midwifery skills and work to their potential.

Abstract:Purpose: While clinical practice is crucial for nursing students to acquire the skills needed to provide professional, high-quality nursing care, further studies on improving undergraduate nursing programs are needed to provide a supportive clinical learning environment for student nurses. This study aimed to understand nursing students' clinical experiences in newborn nurseries and neonatal intensive care units and to provide basic data for the establishment of strategies to promote effective clinical education.Methods: Interviews were held with 15 nursing students at J University who had clinical practice experience in the newborn nursery and neonatal intensive care unit. The collected data were analyzed using the phenomenological analysis method developed by Colaizzi (1978).Results: The nursing students' experiences were grouped into four categories: “expectations for and anxiety about clinical practice", "acquisition of a wide range of knowledge regarding neonatal nursing", "challenges faced in clinical practice", and "experiencing interpersonal changes".Conclusion: The current neonatal practice nursing education system provides students with positive learning experiences. However, the lack of practice opportunities, insufficient instruction, and the theory-practice gap were identified as major issues hindering students' learning needs. These study results are expected to provide basic data for curriculum development to improve undergraduate nursing education.
I Was Merely a Brick in the Game: A Qualitative Study on Registered Nurses’ Reasons for Quitting Their Jobs in Hospitals

Abstract:The aim was to explore why registered nurses (RNs) in Sweden choose to quit their jobs in hospitals, also in relation to experienced patient safety. Previous research has shown that nurse turnover, especially in hospital settings, is a serious challenge for society and health care globally. Insufficient staffing of RNs is linked to poorer patient outcomes and a general patient safety at risk. It is, therefore, important to continually explore how nurses describe their reasons for quitting their jobs. The study was conducted using a qualitative descriptive design, based on 11 semistructured interviews with RNs. The analysis generated four categories describing the results: Feeling that the profession is not valued; Psychological and physical symptoms related to work; An insufficient and unsupportive organization; and Unsatisfying leadership and teamwork. Specifically, the RNs participating in this study described a range of reasons for quitting, where the feeling of not being valued and treated as a respected and autonomous profession was a common thread throughout the results. RNs experienced that, overall, the insufficient work conditions, also resulting in lower patient safety, ultimately led to their decision to quit. The findings highlight the crucial need for employers to develop working conditions for RNs, to make sure that the profession is valued according to professional standards and provide the potential for autonomous nursing practice. To reduce nurse turnover, and instead attract and retain nurses, leadership and management in nursing need to be adjusted to meet the demands of a modern academic profession.

Abstract:Nurses in long-term care (LTC) and home and community care (HCC) are well acquainted with the high workloads and manifold job stressors that pervade these sectors, making it increasingly difficult to employ and retain a strong workforce. In response, nursing bridging education programs have been proposed to enhance recruitment and retention. While the efficacy of such programs is supported, the transition to practice experience of new nurse graduates is often associated with feelings of under-preparedness. Understanding the lived experience of nurses transitioning into new roles in LTC and HCC after completing a bridging program may highlight areas of emphasis for future retention efforts.

Effects of recognition of flexible work systems, organizational commitment, and quality of life on turnover intentions of healthcare nurses

Abstract:Background: It is necessary to adjust and mediate environmental, personal, and structural aspects to reduce the turnover rate among healthcare nurses, and awareness of flexible work systems, organizational commitment and quality of life are factors contained in this category. Objective: The purpose of this study was to identify the influence of awareness regarding flexible work systems, organizational commitment, and quality of life on turnover intentions among healthcare nurses. Methods: Two hundred and twenty-six healthcare nurses participated in the study. Data were collected from September 1 to October 1, 2018, and analyzed using SPSS/WIN 23.0 version. Results: The results in the first analysis revealed that being dissatisfied with work (β= 0.287) and having poor subjective health (β= 0.197) were significant influencing factors with a combined explanatory power of 14.9% (p< 0.001). In the second analysis, dissatisfied with work satisfaction (β= 0.143), organizational commitment (β=-0.389), and quality of life (β= 0.256) were significant influencing factors with a combined explanatory power of 29.5% (p< 0.001). Conclusion: It is necessary to reflect awareness of flexible work systems, organizational commitment and quality of life in interventions to reduce the turnover intention of healthcare nurses.
A qualitative descriptive exploration of the educational and career plans of early career neonatal nurses and midwives: An Irish perspective

Abstract:The scarcity of appropriately qualified nurses and midwives is a major obstacle in achieving an effective health system. Neonatal nurses and midwives require a high level of skill and education to fulfil their role. It is also an area that sees high staff turnover rates. For this study a descriptive qualitative approach was used to ascertain early career neonatal nurses' and midwives' experiences of further education, their future career plans, and their perceived facilitators and barriers to further education and career progression. After receiving ethical approval, twelve nurses and midwives were recruited across three tertiary level neonatal units in Ireland. Semi structured interviews were carried out and interview transcripts were subsequently analysed using Attride-Stirling's (2001) Thematic Networks to deduce themes from the data. Support and involvement, mentoring, and career progression and retention were the three main themes identified upon analysis of the data. The majority of participants identified definitive career plans but some felt their goals were unachievable in their current workplace. Consequently a large number of participants have plans to leave their employment in neonates and pursue a career in other areas of nursing. Staff appraisals and succession planning programmes may assist early career nurses and midwives in focusing on their individual career goals, leading to a greater uptake of further specialised education and improved retention of neonatal nurses and midwives.
New Nurse Turnover Intention and Related Factors in Japan and China: Focusing on Nursing Practice Environment and Burnout

Abstract:Background: With rapidly declining birth rates and aging populations worldwide, the demand for nursing care has increased in recent years. High turnover, an important cause of nurse staffing shortages, directly affects the quality of nursing care. However, no comparison studies on turnover intention in East Asia have been published. Purpose: The purpose of this study was to clarify the relationships among turnover intention, the work environment, and related factors among new nurses in Japan and China. Methods: A self-administered questionnaire survey was used to collect data from nurses employed for < 2 years. This questionnaire included participant characteristics, assessment of desire to become a nurse at the start of employment, current turnover intention, the Practice Environment Scale of the Nursing Work Index, and the Maslach Burnout Inventory-Human Services Survey. The relationship between current turnover intention and, respectively, number of hospital beds, strength of desire to become a nurse at the start of employment, level of participation in hospital affairs, emotional exhaustion (EE), personal achievement, and demographic variables was assessed. Results: The number of nurses who intended to leave the hospital was higher in Japan (74.1%) than in China (42.4%). However, of those expressing intention to leave, 46.6% of the nurses in China and only 5.0% of those in Japan indicated intention to quit the nursing profession altogether. In Japan, collegial nurse-physician relations and EE were related to turnover intention, whereas in China, number of hospital beds, strength of desire to become a nurse at the start of employment, participation in hospital affairs, EE, and personal achievement were all related to turnover intention. Conclusions: To enhance retention and reduce turnover intention among nurses, the strength of desire to become a nurse in China and the nurse-physician relationship and EE in Japan should be taken into particular consideration when designing retention strategies and nurse education programs.
Factors affecting turnover of pediatric nurses:a qualitative study

Abstract:Objective To find out factors that influence pediatric nurses to leave their jobs,and to provide information for taking countermeasures to stabilize nursing staff. Methods Fourteen pediatric nurses who voluntarily left their jobs were deeply interviewed regarding to their reasons of turnover,and data were analyzed by using phenomenological analysis. Results Three themes were extracted: unrelieved stress including dissatisfied work environment,imperfect management,unrecognized work,and unstable professional commitment;social and family supports;chances of working in other fields. Conclusion Pediatric nurses are experiencing various stressors.Nursing managers should concern about working status of employees and help them cope with stress to reduce turnover.
The Evolution of Phenomenology in Korean Nursing Research: A Scoping Review

Abstract:Purpose Phenomenological methods are used to capture human experience, and nursing research has long attempted phenomenological approaches in many studies. This study explored the 22-year trends (2001 to 2022) in phenomenological research within Korean nursing science and identified the types of journals where research is published, common phenomenological data analysis methods, phenomena of interest, and standards of rigor applied to phenomenological studies. Methods This scoping review followed the six stages recommended by Arksey and O'Malley's (2005) and utilized the PRISMA Extension for Scoping Reviews (PRISMA-ScR) for reporting guidance. A total of 4,354 articles acquired through systematic searches across of seven search engines PubMed, CINAHL, Embase, Research Information Sharing Service (RISS), Korea Citation Index (KCI), Korea Institute of Science and Technology Information (KISTI), and Koreanstudies Information Service System (KISS) were reviewed. Key search terms and inclusion and exclusion criteria were used as strategies to identify relevant articles. Results In the final review, 568 Korean phenomenological studies were included. Among the phenomenology research, 50.4% of the total work was performed between 2016 and 2020, and the Colaizzi method of study was the most common (62.9%). Most researchers were published in nursing journals (55.8%). Nurses constituted most participants (24.5%), followed by patients with illnesses (23.1%), and people of all ages. The primary focus included participants' experiences: the disease experience of the sick, job-related experience of the nurses, and learning-related experience of the nursing students. The median sample size of reviewed papers (i.e., 9) is relevant to phenomenological research saturation. The most rigorous studies applied Lincoln and Guba's criteria (54.0%). Conclusion Advancing phenomenological research in nursing entails giving priority to diverse perspectives, rigor and the foundational essence of phenomenology. Ensuring transparency and reliability requires reinforcing rigor through the integration of phenomenology, philosophy, and effective data methods.

Abstract:AIM:This paper reports a theoretical understanding of nurses leaving nursing practice by exploring the processes of decision-making by registered nurses in China on exiting clinical care.BACKGROUND:The loss of nurses through their voluntarily leaving nursing practice has not attracted much attention in China. There is a lack of an effective way to understand and communicate nursing workforce mobility in China and worldwide.DESIGN:This qualitative study draws on the constant comparative method following a grounded theory approach.METHOD:In-depth interviews with 19 nurses who had left nursing practice were theoretically sampled from one provincial capital city in China during August 2009-March 2010.RESULTS:The core category 'Mismatching Expectations: Individual vs. Organizational' emerged from leavers' accounts of their leaving. By illuminating the interrelationship between the core category and the main category 'Individual Perception of Power,' four nursing behaviour patterns were identified: (1) Voluntary leaving; (2) Passive staying; (3) Adaptive staying and (4) Active staying.

Abstract:Objective To explore the work experiences in nursing homes among nurses and to provide the referenced basis for managers to understand the needs of nurses and develop humanistic nursing management measures. Methods Descriptive phenomenological methodology was adopted in this study,a semi-structured interview was conducted with 9 nurses working in nursing homes.Data were analyzed based on Colaizzi′s phenomenological research method. Results Four themes were extracted including:heavy work tasks;high psychological pressure;strong demand in training;emotional experience of work. Conclusions Through in-depth interviews with 9 nurses working in nursing homes,we suggest that in order to promote the professional development of geriatric nursing, managers should change and update the concept of geriatric nursing and attach importance on professional study of nurse, and take appropriate intervention measures to lighten nurses' work pressure and adjust their bad mood, so as to arouse the enthusiasm of nurses.Finally,we can achieve the goal of stabilizing the nursing personnel and improving the quality of care in the nursing homes.
