Abstract:OBJECTIVE:To investigate the factors influencing the prognosis of patent ductus arteriosus (PDA) in very low birth weight (VLBW) infants.METHODS:A total of 194 VLBW infants who were admitted from January 2012 to December 2014 were enrolled as study subjects. According to cardiac ultrasound findings and treatment outcome, these infants were divided into non-PDA group, spontaneous closure group, pharmaceutical closure group, and surgical closure group. Their clinical and echocardiographic characteristics were analyzed.RESULTS:The spontaneous closure rate of PDA was 58.7%. The spontaneous closure group showed significantly higher gestational age, birth weight, and proportion of small-for-gestational-age infants than the pharmaceutical and surgical closure groups (P<0.05). The pharmaceutical and surgical closure groups had a significantly higher incidence rate of neonatal respiratory distress syndrome and a significantly higher proportion of infants who were given pulmonary surfactant (PS) than the spontaneous closure group (P<0.05). During different periods of time, the spontaneous closure group had a significantly smaller ductus arteriosus diameter than the pharmaceutical and surgical closure groups (P<0.05). The multivariate logistic regression analysis showed that gestational age, application of PS, and ductus arteriosus diameter at 48 hours were significantly associated with the prognosis of PDA. The major transductal flow pattern in the spontaneous closure group was closing pattern, while in the pharmaceutical and surgical closure groups, the main flow patterns were pulmonary hypertension and growing patterns within 48 hours and growing pattern on days 4 and 7.CONCLUSIONS:The VLBW infants have a high spontaneous closure rate of PDA. A decreased closure rate of PDA is associated with the lower gestational age and the application of PS. PDA with a large ductus arteriosus diameter and a growing or pulsatile flow pattern cannot easily achieve spontaneous closure.

Abstract:Transcatheter closure of congenital patent ductus arteriosus with the use of Rashkind PDA occluder system was undertaken in 5 cases, of whom cardiac murmur disappeared instantly in one case with no residual shunt detected in aortogram, while in the remaining cases cardiac murmur greatly decreased, only a small residual shunt was observed at the site of previous ductus arteriosus in aortogram and coloured Doppler echocardiogram. Cardiac murmur disappeared with no residual shunt detected on ultrasound Doppler examination in 2 cases 2 months later. No complication was found during the procedure and in the follow-up period. Occlusion of PDA by Rashkind PDA occluder system is a safe, effective, and relatively simple procedure.
A Follow-Up Study of the Efficacy of the Transcatheter Closure of Patent Ductus Arteriosus

Abstract:Objective:To assess the safety and efficacy of the transcatheter closure of patent ductus arteriosus(PDA).Methods:A follow-up study was conducted in patients with PDA underwent transcatheter closure in our hospital.All patients were assessd with physical examination,X-ray,and echocardiography before the operation.During the follow-up time,the patients' cardiac function and complications were evaluated.Results:175 patients underwent transcatheter closure had been followed up.The patients were followed up from 1 to 77 months after operation.Minimal residue shunts were found in 9 cases(5.14%) immediately after closure.3 cases(1.71%) were detected increased blood velocity in descending aorta or pulmonary aorta.At 1 month follow-up,no residue shunt was found.3 cases with increased blood velocity returned to normal during 1～5 years follow-up.The heart structure was significantly changed 1 week after operation.The mean value of left atria diameter,left ventricular end systolic diameter,left ventricular end diastolic diameter,left ventricular end systolic volume and left ventricular end diastolic volume measured by echocardiography before the operation were(31.6±7.8)mm,(33.0±7.6)mm,(51.6±10.6)mm,(40.3±19.5)ml,(118.6±32.5)ml and(77.7±38.1)ml,respectively.These values were decreased to(29.4±6.6)mm,(30.8±6.7)mm,(47.5±9.2)mm,(34.6±19.0)ml,(80.3±29.6)ml and(64.5±31.1)ml,respectively,1 week after operation(P 0.05).There was no significant difference between pre-operation and 1-week or 1-month after operation.There were no severe complications or deaths in this group of patients during follow-up.Conclusion:This study suggested transcatheter closure of PDA could improve cardiac function of these patients,and also confirmed the safety and efficacy of those during short-term and long-term follow-ups.

Abstract:OBJECTIVE:Decline in platelet count (DPC) in patients with congenital heart disease (CHD) undergoing percutaneous transcatheter closure (PTC) has seldom been reported. We sought to investigate the incidence, severity and predictors of DPC among patients with CHD undergoing PTC. METHODS:Patients with CHD undergoing PTC in our department were consecutively included. Platelet count was measured at admission and daily after PTC at least for 5 days. Bleeding events were observed after PTC. RESULTS:Among the whole population (N = 336), 77 (22.9%) patients had no DPC (< 10%), 238 (70.8%) developed minor DPC (10-49%) and 21 (6.3%) acquired severe DPC (> 50%).The mean length of stay in hospital was longer in the severe DPC group than in the no DPC group (4.4 +/- 0.6 vs. 8.4 +/- 4.5 days, P < 0.01). The rate of minor/minimal bleeding was significantly higher in the severe DPC group (42.9%) than in the no DPC (0.0%) and minor DPC (0.8%) groups (both P < 0.01). Multivariate regression revealed that occluder size, residual shunts and CHD types were independent predictors of severe DPC (all P < 0.05).The rate of severe DPC in patent ductus arteriosus (PDA, 12.6%) was higher than in the atrial septal defect (5.2%) or the ventricular septal defect (0.0%) groups (both P < 0.05).The value of DPC was much smaller in the control group (patients undergoing radiofrequency catheter ablation) than in the case group (4.6 +/- 9.7% vs. 22.3 +/- 16.5%, P < 0.001). CONCLUSION:DPC was frequently presented in CHD patients undergoing PTC and associated with increasing minor/minimal bleeding and longer hospital stay. PDA closure with large occluder size and residual shunt, has the highest risk for developing DPC.

Abstract:A patent ductus arteriosus (PDA) can be closed using various procedures, including transcatheter closure, video-assisted thoracoscopic surgery, and conventional surgery with division and suture. These procedures are feasible for treatment of PDA in a child, but are believed to be challenging when employed in an adult. The main danger is intraoperative bleeding. With a hemorrhage, inadvertent ductus damage/perforation during the isolated ductus, the traditional surgical approach was accomplished with considerable difficulty. This study reports a new procedure for these adult patients using an intrapericardial approach to the ductus. The operation was done as an emergency salvage procedure. Based on the results from the 4 cases of PDA, such technique is found to be simple, safe, and effective.

Abstract:We evaluated the transcatheter intervention of complex patent ductus arteriosus (PDA) in Chinese adults. Between January 2004 and April 2008, 112 adult patients (43 males, 69 females, mean age 31 +/- 19 years) underwent intervention. Coils were used for patients with small PDA, and Amplatzer duct occluders or China-made mushroom-shaped occluders were used for patients with moderate-to-large PDA. The success rate of transcatheter intervention was 93.8%, and 9 patients (8.0%) had small residual shunts. At the end of 12 months follow-up, the rate of residual shunts was 1.8%. Peak systolic pulmonary pressure decreased from 94 +/- 21 mm Hg preintervention to 58 +/- 20 mm Hg postintervention (P < .001). No severe procedure-related complications (including death, dislocation of occluders, stenosis of aorta or pulmonary artery) occurred. Some patients developed hemolysis or vascular access complications, all resolved by conservative therapy. Transcatheter intervention is an effective and safe treatment for adult PDA patients with complex anatomic or hemodynamic conditions.
Transthoracic echocardiography as an alternative major guidance to angiography during transcatheter closure of patent ductus arteriosus: technical feasibility and clinical relevance

Abstract:The conventional transcatheter closure of patent ductus arteriosus (PDA) requires femoral artery puncture and angiography for duct anatomic and shunting estimation. If such estimation can be replaced by transthoracic echocardiography (TTE), the procedure will be further simplified, with fewer invasions. This study aimed to examine whether TTE can serve as an alternative to aorta angiography and as a major guidance for transcatheter duct closure. The study enrolled 298 consecutive patients (71 males and 227 females) with PDA. In the study, TTE with combined two-dimensional echocardiography (2DE) imaging and color-coded flow imaging (CDFI) was performed to measure the minimal shunting width (MSW) as the estimated minimal duct size for selection of an Amplatzer duct occluder (ADO) and to monitor the transcatheter duct closure intraprocedurally. The MSW was validated against the duct-stretched diameter (SDD), against the minimal waist diameter of the conical part of a released occluder measured by X-ray spot picture after successful duct closure (SDC), and against the size of the finally used ADO (SADO). Good correlation was found between MSW and SDD [SDD (mm) = 1.31 MSW; r = 0.89; p < 0.01] and between MSW and SADO [SADO (mm) = 1.71 MSW; r = 0.88; p < 0.01]. Of 296 patients who received occlusion using MSW as the reference for selection of the occluder, SDC was attained in 288 (97.3%), 5 (1.7%), and 2 (0.7%) patients, respectively, at the first, second (1 ADO replacement), and third (2 ADO replacements) occluding attempt. Acute occluder dislodgement occurred in one patient (0.3%). At the 12-month follow-up assessment, no major complications were found, and the total immediate or 12-month SDC was 99.7%. Echocardiography as an alternative major guidance to angiography for transcatheter duct closure is technically feasible, and TTE guidance can further simplify the procedure, with fewer invasions and potential complications.
Assessment of the Transcatheter Closure of Patent Ductus Arteriosus in Children

Abstract:Purpose To evaluate efficacy of transcatheter occlusion with the coil for small size and with Amplatzer duct occluder(ADO) for moderate-to-large size patent ductus arteriosus(PDA). Methods A retrospective cohort study was couducted in a total of 112 pediatric patients with PDA with transcatheter closure from May 1998 to January 2006. Results The devices were successfully implanted in all 112 patients(100%).Coils were implanted in 19 and ADOs in the 93 patients.The minimal diameters of PDA in the ADO group were from 1.7 to 11.0(mean 3.53)mm,whereas those in the Coil group were from 1.0 to 3.3(mean 1.76)mm(P0.001).Eventually the devices were successfully implanted in all 112 patients(100%).The residual shunt rates during one year follow-up period were 31.6%(immediate),26.3%(24 hour),5.3%(1-3 months)and 5.3%(9-12 months) in the Coil group,compared with 21.5%,6.5%,2.2% and 1.2% in the ADO group,respectively.Early complications were found in two patients for hemolysis and arrhythmia.No hemolysis,recanalization and stenosis of left pulmonary or aorta were found during further follow-up.No patient required re-intervention to treat residual shunt. Conclusions Transcatheter PDA occlusion with appropriate occluder is generally a very effective and safe procedure for children.
Clinical Characteristics and Transcatheter Occlusion of Patent Ductus Arteriosus in Older Adults

Abstract:Objective: To study the clinical characteristics of patent ductus arteriosus in older adults, and evaluate the safety and efficacy of transcatheter occlusion in these patients. Methods: Clinical characteristics and transcatheter occlusion of patent ductus arteriosus in 19 patients over age 40 were retrospectively analyzed and compared to 76 patients under 40 years old. Results; Advanced New York Heart Association functional class (Ⅲ-Ⅳ) and pulmonary hypertension were more common in the older adult group than those in younger group (84.2% vs 50.0%, P0.05 and 31.6% vs 7.9%, P0.05, respectively). Eighteen in older adults group was successfully occluded with a peak systolic pulmonary pressure decreased from 49 ± 28 mmHg to 32 ± 12 mmHg(P 0.05) and the successful rate was equal to that of the younger group. The follow-up echocardiography showed complete PDA occlusion in all 18 patients within one week after occlusion and the peak systolic pulmonary pressure decreased to 26 ± 4 mmHg 6 months later. There were no major complications. Conclusion: Serious heart dysfunction and pulmonary hypertension were more common in the older adult patients with PDA and transcatheter occlusion is a safe and effective treatment for them.
Pulmonary artery in situ thrombosis due to patent ductus arteriosus: a case report

Abstract:Background: Pulmonary Artery in situ Thrombosis (PAIST) refers to a thrombus forming within the pulmonary arterial system, distinct from an embolus originating from elsewhere in the body (e.g., the deep veins of the lower extremities) and traveling to the lungs where it lodges and forms. Case presentation: We present a case of PAIST caused by the arterial ductus arteriosus. The patient primarily presented with dyspnea, and the chest pain dichotomy Computed Tomography Angiography(CTA) suggested that a nodular low-density filling defect was seen in the lumen of the left pulmonary artery trunk. Initially, pulmonary embolism (PE) was suspected. However, upon reevaluation of the imaging, it became apparent that the patient's pulmonary artery obstruction was closely associated with the ductus arteriosus. After admission, the patient was treated with sodium ampicillin (2.0 g Q12H) for infection, heparin sodium (5,000 IU Q12H) for anticoagulation, and metoprolol succinate extended-release tablets (23.75 mg QD) to correct cardiac remodeling, among other treatments. Subsequently, the patient underwent a cardiac surgery involving the ligation of the arterial duct, resection of pulmonary artery lesions, and open-heart surgery with extracorporeal circulation support. Postoperative examination of the pulmonary artery mass indicated coagulation tissue. The final diagnosis was "PAIST". Conclusion: Both PAIST and PE manifest as low-density filling defects in the pulmonary arteries. However, due to the relative unfamiliarity with PAIST, such findings are often initially attributed to PE.

Abstract:Objective To evaluate post-procedural clinical characteristics of preterm infants undergoing transcatheter patent ductus arteriosus (PDA) closure, including oxygenation/ventilation failure and cardiovascular compromise Study design Multicenter retrospective cohort study of preterm infants who were ≤2 kg at the time of percutaneous PDA closure between August 2018 and July 2021. Indices of cardiorespiratory stability were collected pre-closure, immediately post-closure, and subsequently averaged every 4 hours for the first 24 hours post-procedure. The primary outcome was incidence of post-transcatheter cardiorespiratory syndrome: composite of: (i) hemodynamic instability (defined by systemic hypotension, systemic hypertension, or use of new inotropes/vasopressors in the first 24 hours after catheterization and at least one of the following: (i)ventilation failure or (ii) oxygenation failure. Results A total of 197 patients were included with a median [IQR] age and weight at catheterization of 34 [25, 43] days and 1090 [900, 1367] grams, respectively. The primary composite outcome was reported in 46 (23.3%) patients and subcomponents of oxygenation and/or ventilation failure, systolic hypotension or systolic hypertension were noted in 81 (41.1%), 3 (1.5%) and 86 (43.6%) respectively. Logistic regression models showed weight at catheterization and respiratory severity score pre-closure to be significantly associated with post-transcatheter cardiorespiratory syndrome. Conclusion Post-transcatheter cardiorespiratory syndrome is characterized primarily by systemic hypertension and oxygenation failure, with a very low incidence of hypotension and need for inotropes.

Clinical Investigation on Patent Ductus Arteriosus of Premature Infants

Abstract:Objective To investigate the incidence rate, high risk factors and hemodynamic changes of patent ductus arteriosus (PDA) in premature infants, and to give suggestions abo ut clinical monitoring and management of PDA in premature infants. Methods Echocardiography was performed on 86 non-ventilated or weaned from ventilator-pr emature infants at 2 to 5 days of age,whose gestational age was 28 to 36 weeks. All premature infants diagnosed as PDA were followed up clinically and by Echoc ardiography until discharged. Results Twenty-two infants diagnosed as PDA at mean 3 days of age, mean gestational age was (33.l±2.0) weeks. Ductus in 16 infants (out of 20 infants) closed spontaneo usly when repeated echocardiography at mean 8.5 days of age. For 4 remaining PDA infants, ductus closed in 2 cases (l treated with indomethacin). One ductus reo pened because of sepsis, and 3 infants discharged with opened ductus at their 2l , 40 and 47 days of age respectively. Single and multiple Logistic analysis indi cated that the lower the birth-weight ,the higher the incidence of PDA (χ2=2. 8907 P=0.0891); neonatal asphyxia and suffered from severe diseases (neonat al respiratory distress syndrome, sepsis) were high risk factors of PDA (χ2= 4.3729 P=0.0365;χ2=11.6590 P=0.0006). Premature infants with PDA h ad good heart function,although their LA/AO ratio increased slightly (1.0810±0. 18 vs 1.00±0.07,P= 0.048).Conclusions PDA incidence at 3 days of life in 33 weeks premature infants is 25.6%, 85% PDA disappeares spontaneously during follow-up. Exploring risk factors for totally implantable venous access devices (TIVADs)-related thrombotic occlusion in the off-treatment period

Abstract:Abstract Totally implantable venous access devices (TIVADs) have been widely used for many years in the management of patients suffering from cancer. Thrombotic occlusion is the most common functional complication in the off-treatment period. This study aims to investigate the incidence of and risk factors for TIVADs-related thrombotic occlusion in patients with breast cancer. The clinical data of 1586 eligible patients with breast cancer with TIVADs at the Fourth Affiliated Hospital of Hebei Medical University from 1 January 2019 to 31 August 2021 were analysed. Thrombotic occlusion was confirmed by angiography with signs of partial or total occlusion. Thrombotic occlusion occurred in 96 (6.1%) cases. Multivariable logistic regression analysis showed that the insertion site of the catheter ( P = 0.004), size of the catheter ( P < 0.001), and indwelling time ( P < 0.001) were significant factors for thrombotic occlusion. Insertion in the right internal jugular vein, smaller catheter size and shorter indwelling time can lower the incidence of thrombotic occlusion in breast cancer patients with TIVADs in the off-treatment period.

Investigation of Coagulation System After Percutaneous Coronary Intervention

Abstract:Objective:The von Willebrand factor(VWF),thrombin,fibrinogen(FIB) and D-dimer were important factors in the process of thrombosis.We explored the extent of thrombosis and studied the changes of these factors after percutaneous coronary intervention (PCI). Methods:A total of 35 patients were enrolled,they were with low risk of unstable angina(UA) who received PCI in our hospital. VWF,thrombin-antithrombin(TAT),FIB and D-dimer were examined before PCI and 1 h,4 h,24 h and 48 h after PCI.VWF,FIB and D-dimer were analyzed by immunoturbidimetry assay,TAT was measured by ELISA. Results:After PCI,the levels of VWF,TAT and D-Dimer raised at 1 h,achieved peak level at 24 h and began to fall at 48 h. FIB fall down immediately after PCI,recovered to the base level at 48 h of time. Conclusion:The VWF and thrombin presented high level of aggregation activation after PCL The patients with high risk of thrombosis should take antiplatelet and anticoagulation therapy for at least 48h after PCI.

Abstract:【Objective】 To learn the status of the patent ductus arteriosus in term births and to investigate the necessity of early artificial intervention.【Methods】 During March 2007 to December 2007,1 437 term newborn infants were examined by Doppler echocardiography after 24 hours to 96 hours of the delivery,and the infants with isolated persistence of the arterial duct were followed in the next 3 months to ensure if the ductus arteriosus had closed.【Results】 Persistence of the arterial duct was detected in 318 term newborn infants.In the first 24 hours 59.3% ductus was still open,in 48 hours,72 hours,96 hours the scale was 27.1%,15.2% and 9%.During the follow up,304 cases spontaneously closed without any treatment,only 3 cases were diagnosed patent ductus arteriosus PDA.The incidence of PDA is 1 ∶ 475,with a male to female ratio of 1 ∶ 2.【Conclusions】 In term newborn infants ductal patency may be detected after 96 hours of age,having no significant symptoms.If asymptomatic,it shouldn't be think abnormal and needn't any treatment but follow up.
Comparative transcatheter occlusion of patent ductus arteriosus: multicenter collaborative study across pediatric and veterinary cardiology centers

Abstract:Introduction/objective: Transcatheter therapeutics have revolutionized treatment of patent ductus arteriosus (PDA). Greater understanding of corrective interventions across species can advance best practices, protocols, and outcomes while minimizing adverse events. The objective of this study was to describe characteristics and outcomes in children and dogs undergoing transcatheter PDA occlusion. Animals, materials and methods: This was a multicenter, retrospective cohort study from two pediatric and three veterinary centers. Demographics, procedural characteristics, and outcomes were assessed. Results: Data included 202 children and 106 dogs treated from July 2019 to June 2021. Forty-five (23%) children and 19 (18%) dogs had congestive heart failure prior to catheterization. Transvenous and transarterial approaches for deployment were most used in children and dogs, respectively. All children had percutaneous vascular access compared to 17 (16%) dogs. Intraprocedural anticoagulation was standard for children (100% of 165 reported), but not for dogs (2/103). The median (interquartile range) pulmonary ostium diameter in children was 2.2 mm (1.5-3.0 mm) and 3.0 mm (2.0-4.2 mm) in dogs when all body sizes were considered (P<0.001). Amplatzer™ Duct Occluder was most commonly used in children (64/202, 32%); the Amplatz® Canine Duct Occluder was used in dogs (96/100, 96%). Closure was manual compression in all children, whereas vessel ligation was most used in dogs (74/104, 73%). Successful device deployment was achieved in 197 (98%) children and 98 (93%) dogs (P=0.03). Major reasons for failure included device embolization in six (2.9%) children and PDA morphology concerns in four (3.8%) dogs. Conclusions: Transcatheter PDA occlusion is successful in children and dogs. Study data might be useful for optimizing transcatheter therapeutics and animal models for interventional cardiology.
Clinical efficacy of solo transcatheter closure of patent foramen ovale guided by transthoracic echocar-diography

Abstract:Objective:To observe the safety and efficacy of solo transcatheter closure of Patent Foramen Ovale (PFO) guided solely by transthoracic echocardiography (TTE).Methods:A total of 25 cases were retrospectively included who underwent the solo transcatheter closure of PFO guided by TTE in Peking University Third Hospital from December 2018 to December 2019. They were 20 females and 5 males with average age of (42.3±7.8) years. TTE was performed immediately after percutaneous closure. The follow-up interviews were conducted at 1 month, 3 months, and 6 months after the procedure, respectively. There were 6 patients with cryptogenic stroke (RoPE score＞6) and 19 patients with obstinate migraine. The presence of PFO was confirmed by preoperative transesophageal echocardiography (TEE). The diameter of PFO was 2-3 mm and the diameter of tunnel was 4-8 mm. The contrast-enhanced transcranial doppler (cTCD) was 2-3 grade.Results:All patients were successfully treated with the solo transcatheter closure of PFO guided by TTE. The mean procedural time (from puncture to sheath removal) was (32.1±7.5) mins. During the operation, 3 cases presented vagal reflex and 2 cases developed transient ventricular arrhythmia, which disappeared after conservative treatment. All patients were discharged the next day after operation without complications, e.g. pericardial effusion, peripheral vascular injury, or cardiac perforation. During the follow-ups, no patients suffered from pericardial effusion, occluder malposition, residual shunt, recurrence of headache and stroke. The cTCD is normal.Conclusion:The solo transcatheter closure of PFO guided by TTE is an effective procedure with high successful rate and low rate of complications.

Abstract:The question of preserving the patent ductus arteriosus (PDA) during the modified Blalock–Taussig shunt (MBTS) procedure remains controversial. The goal of this study was to investigate the effects of the PDA on the flow features of the MBTS to help with preoperative surgery design and postoperative prediction. In this study, a patient with pulmonary atresia and PDA was included. A patient-specific three-dimensional model was reconstructed, and virtual surgeries of shunt insertion and ductus ligation were performed using computer-aided design. Computational fluid dynamics was utilized to analyze the hemodynamic parameters of varied models based on the patient-specific anatomy and physiological data. The preservation of the PDA competitively reduced the shunt flow but increased total pulmonary perfusion. The shunt flow and ductal flow collided, causing significant and complicated turbulence in the pulmonary artery where low wall shear stress, high oscillatory shear index, and high relative residence time were distributed. The highest energy loss was found when the PDA was preserved. The preservation of PDA is not recommended during MBTS procedures because it negatively influences hemodynamics. This may lead to pulmonary overperfusion, inadequate systemic perfusion, and a heavier cardiac burden, thus increasing the risk of heart failure. Also, it seems to bring no benefit in terms of reducing the risk for thrombosis.

Abstract:(1) Background: To identify reasons for the persistence of surgical ligation of the patent ductus arteriosus (PDA) in premature infants after the 2019 Food and Drug Administration (FDA) approval of transcatheter device closure; (2) Methods: We performed a 10-year (2014–2023) single-institution retrospective study of premature infants (<37 weeks) and compared clinical characteristics and neonatal morbidities between neonates that underwent surgical ligation before (epoch 1) and after (epoch 2) FDA approval of transcatheter closure; (3) Results: We identified 120 premature infants that underwent surgical ligation (n = 94 before, n = 26 after FDA approval). Unfavorable PDA morphology, active infection, and recent abdominal pathology were the most common reasons for surgical ligation over device occlusion in epoch 2. There were no differences in demographics, age at closure, or outcomes between infants who received surgical ligation in the two epochs; (4) Conclusions: Despite increasing trends for transcatheter PDA closure in premature infants, surgical ligation persists due to unfavorable ductal morphology, active infection, or abdominal pathology.
