Abstract:Objective:To assess the safety and efficacy of the transcatheter closure of patent ductus arteriosus(PDA).Methods:A follow-up study was conducted in patients with PDA underwent transcatheter closure in our hospital.All patients were assessd with physical examination,X-ray,and echocardiography before the operation.During the follow-up time,the patients' cardiac function and complications were evaluated.Results:175 patients underwent transcatheter closure had been followed up.The patients were followed up from 1 to 77 months after operation.Minimal residue shunts were found in 9 cases(5.14%) immediately after closure.3 cases(1.71%) were detected increased blood velocity in descending aorta or pulmonary aorta.At 1 month follow-up,no residue shunt was found.3 cases with increased blood velocity returned to normal during 1～5 years follow-up.The heart structure was significantly changed 1 week after operation.The mean value of left atria diameter,left ventricular end systolic diameter,left ventricular end diastolic diameter,left ventricular end systolic volume and left ventricular end diastolic volume measured by echocardiography before the operation were(31.6±7.8)mm,(33.0±7.6)mm,(51.6±10.6)mm,(40.3±19.5)ml,(118.6±32.5)ml and(77.7±38.1)ml,respectively.These values were decreased to(29.4±6.6)mm,(30.8±6.7)mm,(47.5±9.2)mm,(34.6±19.0)ml,(80.3±29.6)ml and(64.5±31.1)ml,respectively,1 week after operation(P 0.05).There was no significant difference between pre-operation and 1-week or 1-month after operation.There were no severe complications or deaths in this group of patients during follow-up.Conclusion:This study suggested transcatheter closure of PDA could improve cardiac function of these patients,and also confirmed the safety and efficacy of those during short-term and long-term follow-ups.
[Factors Influencing the Prognosis of Patent Ductus Arteriosus in Very Low Birth Weight Infants].

Abstract:OBJECTIVE:To investigate the factors influencing the prognosis of patent ductus arteriosus (PDA) in very low birth weight (VLBW) infants.METHODS:A total of 194 VLBW infants who were admitted from January 2012 to December 2014 were enrolled as study subjects. According to cardiac ultrasound findings and treatment outcome, these infants were divided into non-PDA group, spontaneous closure group, pharmaceutical closure group, and surgical closure group. Their clinical and echocardiographic characteristics were analyzed.RESULTS:The spontaneous closure rate of PDA was 58.7%. The spontaneous closure group showed significantly higher gestational age, birth weight, and proportion of small-for-gestational-age infants than the pharmaceutical and surgical closure groups (P<0.05). The pharmaceutical and surgical closure groups had a significantly higher incidence rate of neonatal respiratory distress syndrome and a significantly higher proportion of infants who were given pulmonary surfactant (PS) than the spontaneous closure group (P<0.05). During different periods of time, the spontaneous closure group had a significantly smaller ductus arteriosus diameter than the pharmaceutical and surgical closure groups (P<0.05). The multivariate logistic regression analysis showed that gestational age, application of PS, and ductus arteriosus diameter at 48 hours were significantly associated with the prognosis of PDA. The major transductal flow pattern in the spontaneous closure group was closing pattern, while in the pharmaceutical and surgical closure groups, the main flow patterns were pulmonary hypertension and growing patterns within 48 hours and growing pattern on days 4 and 7.CONCLUSIONS:The VLBW infants have a high spontaneous closure rate of PDA. A decreased closure rate of PDA is associated with the lower gestational age and the application of PS. PDA with a large ductus arteriosus diameter and a growing or pulsatile flow pattern cannot easily achieve spontaneous closure.

Abstract:Background:The optimal treatment strategy for patent ductus arteriosus (PDA) in extremely preterm infants is currently highly controversial. This study aimed to evaluate the association between PDA treatment and short-term outcomes among extremely preterm infants.Methods:This cohort study included all extremely preterm infants (≤27 and 6/7 weeks) who were admitted to hospitals participating in the Chinese Neonatal Network from January 2019 to December 2021, and were diagnosed to have PDA by echocardiogram. PDA treatment was defined as medical treatment and/or surgical ligation of PDA during hospitalization. Short-term outcomes included death, bronchopulmonary dysplasia (BPD), death/BPD, retinopathy of prematurity, necrotizing enterocolitis, and severe brain injury. Multivariate logistic regression was used to evaluate the association between PDA treatment and outcomes. Subgroup analysis were performed among infants with different respiratory support on 3 and 7 days of life.Findings:A total of 2494 extremely preterm infants with the diagnosis of PDA were enrolled, of which 1299 (52.1%) received PDA treatment. PDA treatment was significantly associated with lower risk of death (adjusted odds ratio, 0.48; 95% confidence interval, 0.38-0.60). The decreased risk of death was accompanied by increased risk of BPD and death/BPD. In subgroup analysis according to respiratory support, PDA treatment was associated with lower risk of death among infants who required invasive ventilation. However, the beneficial effect on death was not significant among infants who did not require invasive ventilation.Interpretation:PDA treatment was associated with reduced mortality in extremely preterm infants, but this beneficial effect was mainly present among infants who required invasive ventilation.Funding:This study was funded by the Shanghai Science and Technology Commission's Scientific and Technological Innovation Action Plan (21Y21900800) and the Canadian Institutes of Health Research (CTP87518).

Abstract:Transcatheter closure of congenital patent ductus arteriosus with the use of Rashkind PDA occluder system was undertaken in 5 cases, of whom cardiac murmur disappeared instantly in one case with no residual shunt detected in aortogram, while in the remaining cases cardiac murmur greatly decreased, only a small residual shunt was observed at the site of previous ductus arteriosus in aortogram and coloured Doppler echocardiogram. Cardiac murmur disappeared with no residual shunt detected on ultrasound Doppler examination in 2 cases 2 months later. No complication was found during the procedure and in the follow-up period. Occlusion of PDA by Rashkind PDA occluder system is a safe, effective, and relatively simple procedure.

Abstract:A patent ductus arteriosus (PDA) can be closed using various procedures, including transcatheter closure, video-assisted thoracoscopic surgery, and conventional surgery with division and suture. These procedures are feasible for treatment of PDA in a child, but are believed to be challenging when employed in an adult. The main danger is intraoperative bleeding. With a hemorrhage, inadvertent ductus damage/perforation during the isolated ductus, the traditional surgical approach was accomplished with considerable difficulty. This study reports a new procedure for these adult patients using an intrapericardial approach to the ductus. The operation was done as an emergency salvage procedure. Based on the results from the 4 cases of PDA, such technique is found to be simple, safe, and effective.

Abstract:We evaluated the transcatheter intervention of complex patent ductus arteriosus (PDA) in Chinese adults. Between January 2004 and April 2008, 112 adult patients (43 males, 69 females, mean age 31 +/- 19 years) underwent intervention. Coils were used for patients with small PDA, and Amplatzer duct occluders or China-made mushroom-shaped occluders were used for patients with moderate-to-large PDA. The success rate of transcatheter intervention was 93.8%, and 9 patients (8.0%) had small residual shunts. At the end of 12 months follow-up, the rate of residual shunts was 1.8%. Peak systolic pulmonary pressure decreased from 94 +/- 21 mm Hg preintervention to 58 +/- 20 mm Hg postintervention (P < .001). No severe procedure-related complications (including death, dislocation of occluders, stenosis of aorta or pulmonary artery) occurred. Some patients developed hemolysis or vascular access complications, all resolved by conservative therapy. Transcatheter intervention is an effective and safe treatment for adult PDA patients with complex anatomic or hemodynamic conditions.
Risk Factors of Long-Term Sequelae After Transcatheter Closure of Perimembranous Ventricular Septal Defect in Young Children

Abstract:BACKGROUND: Complications arising from transcatheter closure of perimembranous ventricular septal defects (pmVSD) in children, such as residual shunts and aortic regurgitation (AR), have been observed. However, the associated risk factors remain unclear. This study identified risk factors linked with residual shunts and AR following transcatheter closure of pmVSD in children aged 2-12 years.Methods and Results: The medical records of 63 children with pmVSD and a pulmonary-to-systemic blood flow ratio <2.0 who underwent transcatheter closure between 2011 and 2018 were analyzed with a minimum 3-year follow-up. The success rate of transcatheter closure was 98.4%, with no emergency surgery, permanent high-degree atrioventricular block, or mortality. Defects ≥4.5 mm had significantly higher odds of persistent residual shunt (odds ratio [OR] 6.85; P=0.03). The use of an oversize device (≥1.5 mm) showed a trend towards reducing residual shunts (OR 0.23; P=0.06). Age <4 years (OR 27.38; 95% confidence interval [CI] 2.33-321.68) and perimembranous outlet-type VSD (OR 11.94, 95% CI 1.10-129.81) were independent risk factors for AR progression after closure.CONCLUSIONS: Careful attention is crucial for pmVSDs ≥4.5 mm to prevent persistent residual shunts in transcatheter closure. Assessing AR risk, particularly in children aged <4 years, is essential while considering the benefits of pmVSD closure.

Transthoracic echocardiography as an alternative major guidance to angiography during transcatheter closure of patent ductus arteriosus: technical feasibility and clinical relevance

Abstract:The conventional transcatheter closure of patent ductus arteriosus (PDA) requires femoral artery puncture and angiography for duct anatomic and shunting estimation. If such estimation can be replaced by transthoracic echocardiography (TTE), the procedure will be further simplified, with fewer invasions. This study aimed to examine whether TTE can serve as an alternative to aorta angiography and as a major guidance for transcatheter duct closure. The study enrolled 298 consecutive patients (71 males and 227 females) with PDA. In the study, TTE with combined two-dimensional echocardiography (2DE) imaging and color-coded flow imaging (CDFI) was performed to measure the minimal shunting width (MSW) as the estimated minimal duct size for selection of an Amplatzer duct occluder (ADO) and to monitor the transcatheter duct closure intraprocedurally. The MSW was validated against the duct-stretched diameter (SDD), against the minimal waist diameter of the conical part of a released occluder measured by X-ray spot picture after successful duct closure (SDC), and against the size of the finally used ADO (SADO). Good correlation was found between MSW and SDD [SDD (mm) = 1.31 MSW; r = 0.89; p < 0.01] and between MSW and SADO [SADO (mm) = 1.71 MSW; r = 0.88; p < 0.01]. Of 296 patients who received occlusion using MSW as the reference for selection of the occluder, SDC was attained in 288 (97.3%), 5 (1.7%), and 2 (0.7%) patients, respectively, at the first, second (1 ADO replacement), and third (2 ADO replacements) occluding attempt. Acute occluder dislodgement occurred in one patient (0.3%). At the 12-month follow-up assessment, no major complications were found, and the total immediate or 12-month SDC was 99.7%. Echocardiography as an alternative major guidance to angiography for transcatheter duct closure is technically feasible, and TTE guidance can further simplify the procedure, with fewer invasions and potential complications.

Abstract:Objective To evaluate post-procedural clinical characteristics of preterm infants undergoing transcatheter patent ductus arteriosus (PDA) closure, including oxygenation/ventilation failure and cardiovascular compromise Study design Multicenter retrospective cohort study of preterm infants who were ≤2 kg at the time of percutaneous PDA closure between August 2018 and July 2021. Indices of cardiorespiratory stability were collected pre-closure, immediately post-closure, and subsequently averaged every 4 hours for the first 24 hours post-procedure. The primary outcome was incidence of post-transcatheter cardiorespiratory syndrome: composite of: (i) hemodynamic instability (defined by systemic hypotension, systemic hypertension, or use of new inotropes/vasopressors in the first 24 hours after catheterization and at least one of the following: (i)ventilation failure or (ii) oxygenation failure. Results A total of 197 patients were included with a median [IQR] age and weight at catheterization of 34 [25, 43] days and 1090 [900, 1367] grams, respectively. The primary composite outcome was reported in 46 (23.3%) patients and subcomponents of oxygenation and/or ventilation failure, systolic hypotension or systolic hypertension were noted in 81 (41.1%), 3 (1.5%) and 86 (43.6%) respectively. Logistic regression models showed weight at catheterization and respiratory severity score pre-closure to be significantly associated with post-transcatheter cardiorespiratory syndrome. Conclusion Post-transcatheter cardiorespiratory syndrome is characterized primarily by systemic hypertension and oxygenation failure, with a very low incidence of hypotension and need for inotropes.

Assessment of the Transcatheter Closure of Patent Ductus Arteriosus in Children

Abstract:Purpose To evaluate efficacy of transcatheter occlusion with the coil for small size and with Amplatzer duct occluder(ADO) for moderate-to-large size patent ductus arteriosus(PDA). Methods A retrospective cohort study was couducted in a total of 112 pediatric patients with PDA with transcatheter closure from May 1998 to January 2006. Results The devices were successfully implanted in all 112 patients(100%).Coils were implanted in 19 and ADOs in the 93 patients.The minimal diameters of PDA in the ADO group were from 1.7 to 11.0(mean 3.53)mm,whereas those in the Coil group were from 1.0 to 3.3(mean 1.76)mm(P0.001).Eventually the devices were successfully implanted in all 112 patients(100%).The residual shunt rates during one year follow-up period were 31.6%(immediate),26.3%(24 hour),5.3%(1-3 months)and 5.3%(9-12 months) in the Coil group,compared with 21.5%,6.5%,2.2% and 1.2% in the ADO group,respectively.Early complications were found in two patients for hemolysis and arrhythmia.No hemolysis,recanalization and stenosis of left pulmonary or aorta were found during further follow-up.No patient required re-intervention to treat residual shunt. Conclusions Transcatheter PDA occlusion with appropriate occluder is generally a very effective and safe procedure for children.
Left Ventricular Size and Function after Percutaneous Closure of Patent Ductus Arteriosus in Chinese Adults.

Abstract:Background: Left ventricular (LV) systolic dysfunction can occur after patent ductus arteriosus (PDA) closure and data in adult Chinese patients are lacking. Methods: We examined adult Chinese patients who underwent successful transcatheter PDA closure at Zhongshan Hospital. Echocardiographic studies were performed before closure, before discharge, and at 1, 3, 6, and 12 months after closure. A total of 430 patients were included between January 2010 and December 2016. Patients were divided into two groups based on LV end-diastolic diameter (LVEDD): Dilated LV Group: >56 mm (n = 191) and Non-dilated LV Group: <= 56 mm (n = 239). Results: LVEDD and LV ejection fraction (LVEF) were significantly decreased immediately after closure. Reductions in LVEDD (-10.5% +/- 7.1% vs. -4.6% +/- 7.0%, P < 0.001) and LVEF (-8.9% +/- 12.6% vs. -2.1% +/- 8.6%, P < 0.001) were greater in the Dilated LV Group. LV end-systolic diameter (LVESD) remained unchanged compared to levels before closure (-4.0% +/- 5.4%, P = 0.257; -2.6% +/- 5.4%, P = 0.201). 48 patients in the Dilated LV Group (25.1%) and 7 patients in the Non-dilated LV Group (2.9%) developed late LV systolic dysfunction. In multivariable analysis, LVEF >= 60%, LVEDD <63 mm, and mean pulmonary arterial pressure (mPAP) <29 mmHg were predictive of normal LV function after closure. Conclusion: Many adult Chinese patients developed early LV dysfunction after PDA closure and some patients developed late LV dysfunction. LVEF, LVEDD, and mPAP were identified as significant predictors of late LV systolic function. (C) 2020 Elsevier B.V. All rights reserved.
Percutaneous Transcatheter Closure of a Stented Patent Ductus Arteriosus in a Patient With Critical Pulmonary Stenosis: A Case Series

Abstract:INTRODUCTION: TStenting of the ductus is an alternative to surgical shunt in patients with critical pulmonary stenosis who remain ductal dependent following valvuloplasty. Stents close spontaneously after improvement of right ventricular function; however, in rare cases, stents fail to completely endothelialize, resulting in pulmonary overcirculation. CASE PRESENTATION: The first case was a 3-year-old who had critical pulmonary stenosis at birth, and the second case was a 3-year-old boy who at 5 months of age was managed for severe pulmonary valve stenosis. Both underwent stenting of the duct and percutaneous pulmonary balloon valvuloplasty. On follow-up, the patent ductus arteriosus (PDA) stent of both cases remained patent even after an adequate growth and improvement of the right ventricle, resulting in significant left-to-right shunting and development of increased pulmonary blood flow; hence, percutaneous transcatheter closure of the stented PDA was done. DISCUSSION: Stenting of the ductus allows significant and balanced growth of the pulmonary artery in patients with duct-dependent congenital heart diseases leading to adequate growth of the right ventricle. Persistence of patency of stent causing significant left-to-right shunting warrants closure of the PDA stent. Closure of the stent is challenging and requires a high degree of technical skill because of the difficulty to enter the partially closed stent. CONCLUSION: Percutaneous transcatheter closure of a stented PDA in patients with critical pulmonary stenosis can be a technically challenging procedure because of the difficulty to enter the stent due to the presence of intrastent endothelial proliferation. Abstract:In preterm newborns with extremely low birth weights, patent ductus arteriosus (PDA), which is defined as a remnant connection between the aorta and pulmonary artery after 72 hours of birth, is frequently linked to substantial morbidity and mortality. If left untreated, a hemodynamically significant PDA (hsPDA) increases the risk for bronchopulmonary dysplasia, necrotizing enterocolitis, and intraventricular hemorrhage among other morbidities, and can even lead to death. While instances of patent ductus arteriosus (PDA) resolving on their own are frequent, the primary approach for managing PDA closure in premature infants involves pharmacological interventions, commonly utilizing indomethacin, ibuprofen, or paracetamol. However, with these pharmacological treatment options, there is an increased risk of renal toxicity, gastrointestinal bleeding, and reopening of PDA among other complications. If pharmacological interventions are not successful or contraindicated, PDA can be closed via transcatheter closure or surgical ligation. As with any medically invasive procedure, it is not without risks and can lead to long-term complications. This review explores the different management options and the benefits and outcomes of conservative management vs. active management in order to get one step closer to standardizing the treatment for PDA. With so much controversy surrounding the best management option, there is a lack of evidence to support one treatment method superior to the other in reducing overall mortality, and this needs to be explored further.
Arrhythmias after transcatheter closure of perimembranous ventricular septal defects with a modified double-disk occluder: early and long-term results

Abstract:With the development of interventional techniques and devices, transcatheter closure of perimembranous ventricular septal defect has been widely performed. However, there has been a lack of long-term follow-up results about postoperative ECG changes of PmVSD patients. We report our experience of early and late arrhythmias after transcatheter closure of PmVSD with a modified double-disk occluder (MDVO). We performed a retrospective review of 79 patients (47 males, 32 females) between September 2002 and May 2007 who underwent transcatheter closure of perimembranous ventricular septal defect. Symmetric and asymmetric PmVSD occluders were used. The diameter of the evaluated defects ranged from 3 to 12 mm, as measured by TTE and 3 to 15 mm by left ventriculography. Most cases of PmVSD were treated successfully with a single procedure, resulting in a successful closure rate of 97% (77/79 patients). There was no death in any of the patients. After the operation, 79 patients were followed-up for a range of 10-76 months (35.3 ± 17.4 months). In this series, 11 cases of incomplete right bundle branch block and five cases of complete right bundle branch block occurred during the early period after operation. During long-term follow-up, these issues declined in prevalence to five and four cases, respectively. Moreover, reversible third-degree AVB occurred during closure or after the procedure, and two of the three patients with reversible AVB received a temporary heart pacemaker implantation. These patients recovered 1 h, 6 days, and 9 days later, respectively. During 10-76 months of follow-up, no complications occurred in any of the patients, including residual shunt, severe aortic valve, or tricuspid valve regurgitation. Device closure of perimembranous ventricular septal defects with a modified double-disk occluder (MDVO) resulted in excellent closure rates and acceptably low arrhythmia rates.
Long-term outcomes of transcatheter atrial septal defect closure: a single-center retrospective study

Abstract:Background: Transcatheter atrial septal defect (ASD) closure is the primary approach for treating ASD secundum; however, data on long-term outcomes remain limited. This study aimed to elucidate the prevalence of adverse outcomes following transcatheter ASD closure in a diverse population. Methods: This retrospective cohort study was conducted at the Songklanagarind Hospital and included patients who underwent transcatheter ASD closure between January 2010 and August 2021. Results: The study included 277 patients who completed follow-up for at least 1 year, with varying ages: <25 years (31%), 25-40 years (19%), 40-60 years (34%), and >60 years (16%). The median follow-up duration was 37 months (interquartile range: 20, 61). The overall mortality rate was 1.8%, and no deaths were attributed to device-related complications. Hospitalization due to heart failure occurred in 0.7% of the cases. Most patients improved or stabilized based on the New York Heart Association functional class. Adverse outcomes included new-onset atrial fibrillation (prevalence: 2.7%) and pulmonary hypertension (prevalence: 0.6%). The resolution of pulmonary hypertension varied among age groups, with 100% resolution in patients <25 years. Multivariate analysis identified male sex, overweight, and history of stroke to be significantly associated with adverse outcomes after transcatheter ASD closure. Conclusion: Transcatheter ASD closure was safe and effective, with age not being a limiting factor for success. Male sex, being overweight, and a history of stroke were associated with adverse outcomes. These findings contribute to our understanding of the long-term outcomes following ASD closure.

Abstract:(1) Background: To identify reasons for the persistence of surgical ligation of the patent ductus arteriosus (PDA) in premature infants after the 2019 Food and Drug Administration (FDA) approval of transcatheter device closure; (2) Methods: We performed a 10-year (2014–2023) single-institution retrospective study of premature infants (<37 weeks) and compared clinical characteristics and neonatal morbidities between neonates that underwent surgical ligation before (epoch 1) and after (epoch 2) FDA approval of transcatheter closure; (3) Results: We identified 120 premature infants that underwent surgical ligation (n = 94 before, n = 26 after FDA approval). Unfavorable PDA morphology, active infection, and recent abdominal pathology were the most common reasons for surgical ligation over device occlusion in epoch 2. There were no differences in demographics, age at closure, or outcomes between infants who received surgical ligation in the two epochs; (4) Conclusions: Despite increasing trends for transcatheter PDA closure in premature infants, surgical ligation persists due to unfavorable ductal morphology, active infection, or abdominal pathology.

[Effects of Transcatheter Closure of Perimembranous Ventricular Septal Defects: Intermediate and Long-Term Follow-Up of 68 Cases].

Abstract:OBJECTIVE:To evaluate the curative effects of transcatheter closure on perimembranous ventricular septal defects (PMVSDs) using unbalanced Amplatzer asymmetric ventricular septal defect occluder (AAVSDO).METHODS:The data of 68 patients of PMVSDs with a diameter of 6.7 mm (3 to 12 mm) and the diameter of ventricular septal rim below the aortic valve of 2.7 +/- 1.1 mm (1-5 mm), 27 males and 41 females, aged 15.6 +/- 11.5 (1.5-44), weighing 42.8 +/- 15.2 kg (10-72 kg), treated with AAVSDO, the diameter of which was 1-2 mm larger than the largest diameter of the defects determined by angled left ventriculography, from September 2002 to January 2005 were prospectively analyzed. The patients were followed up for 221 +/- 130 days (90 to 750 days).RESULTS:Seventy-one procedures were performed. The device was implanted successfully in 65 of the 68 patients (95.6%). The selected device diameter was 8.4 mm (4 to 14 mm). Device was lost in one patient during the procedure, which was successfully managed by recapturing the device with a snare device and redeploying it. During the procedure, transient complete left bundle branch block and right bundle branch block occurred in 6 and 5 patients respectively. On follow-up evaluation, transient junctional rhythm occurred in one patient, and accelerated ventricular rhythm in 1. After deployment of the device, the immediate complete closure rate was 43% (28/68), increased to 81.5% (53/68) on the day next to the procedure, and reached 100% 6 months after. One patient adopted surgical reparation because hemolysis occurred after the device implanted. The hospitalization time was 4.5 +/- 3.6 days (2-8 days). The X-ray exposure time was 14.8 +/- 10.7 min (6-48 min). The procedure time 72.6 +/- 38.7 min (35-186 min). One patient was diagnosed as with deep vein thrombosis because of right leg swelling at the seventh day after the procedure. The symptoms disappeared after anticoagulation treatment with low molecule weight heparin. During the scheduled long-term follow-up all patients were doing well. No episode of endocarditis, procedure-related death, or wire disruption was recorded.CONCLUSION:The initial and long-term follow-up results of transcatheter closure of PMVSDs are promising with high success and occlusion rates. Transcatheter closure of PMVSDs using AAVSDO appears to be the first line choice for suitable patients with such defects.

Abstract:Objective:To assess the regular change of cardiac form and function after transcatheter closure of patent ductus arteriosus with domestic occluder, the effect of perindopril on left cardiac remodelling and pulmonary hypertension.Method:Twenty patients whose sizes of PDA were more than 5mm, and age were older than seventeen years, were divided into treatment group and control group according to identical sex and age±5 years, two groups were regularly given Aspirin in the day before operation, 200 mg·d-1×1 moth, then 100 mg·d-1×5 moths, treatment group was also given perindopril 2 mg·d-1×6 moths. Echocardiographic and electrocardiographic examinations were performed before and after closure of PDA, cardiac volume and ejection fraction was measured by area-length method, pulmonary systolic pressure by continuous wave Doppler according to tricuspid reflow.Result:Fifteen patients without drug have no significant difference of right cardiac diameter,LVEF, FS between before and after closure (P0.05). The left cardiac diameter and volume, pulmonary diameter (PD), pulmonary systolic pressure (PSP) decreased significantly (P0.05) between before and 24 h or 3 months after intervention, whereas they had no significant difference between 6 months and 3 months after closure (P0.05). Left ventricular ejection fraction (LVEF) and left ventricular shorten fraction (FS) showed no significant difference during the follow-up after closure compared with those before closure (P0.05). After 6 month therapy with perindopril, the improvement of left ventricular end-diastolic and end-systolic diameter, end-diastolic volume, LVEF, FS, PD, PSP were significantly different between two groups (P0.05), baseline of both groups had no significant difference (P0.05).Conclusion:With 6 month follow-up, Perindopril could decrease PSP and PD, improve left cardiac function, size and morphology significantly after the transcatheter closure of PDA, better clinical symptoms.

Abstract:Objective To evaluate the safety and efficacy of transcatheter closure of post-surgery residual ventricular septal defect( VSD)and its mid-to long-term follow-up results. Methods The clinical data of patients with post-surgery residual VSD who underwent an attempt of percutaneous device closure from September 2002 to December 2013 in Childrenˊs Hospital of Fudan University were summarized. Results 21 patients( 11 males and 10 females ) with post-surgery residual VSD underwent an attempt of percutaneous device closure in our center. The mean age was 8. 8 years(2. 7-21 years)and the mean weight was 26. 3 kg(10. 5-53. 5 kg). Among these 21 patients,their previous surgery procedures were VSD repair in 13 cases,repair of tetralogy of Fallot( TOF)in 5 cases,repair of double outlet right ventricle( DORV)in 2 cases,and repair of complete transposition of great arteries( TGA)with VSD in 1 case. In addition,1 patient of post-TOF repair was associated with frequently premature ventricular contraction and ventricular tachycardia,2 patients of post VSD repair were associated with recoarctation of the aorta. The duration between previous surgery and transcathter device closure was 11-80 months. The mean diameter of VSDs in right ventricle( RV) side was 3. 73 mm(1. 6-6. 3 mm)by angiogram. There were multiple shunts in RV side in 4 patients,and one patient was left ventricle to right atrium shunt. The Qp/Qs ratio was 1. 53 ± 0. 23,and there was no pulmonary hypertension more than moderate. Amplatzer devices were successfully deployed in all cases except for 1 failure case due to a repeat complete block during the arterial-venous loop establishing,and 2 devices were used in 1 patient. The median device size was 6. 5 mm(4 -12 mm). A radio-frequency ablation and 2 balloon dilatations were successfully finished for the associated defects in the same procedure. There was no procedure-related adverse events. Mild residual shunt was showed by angiogram and transthoracic echocardiography( TTE ) immediate after closure in 3 patients. Three patients with moderate to severe tricuspid regurgitation before device closure improved significantly after closure. All patients were doing well clinically in a mean 22. 2 months(1 - 79 months)follow up. Minor arrhythmia was observed in 2 patients in the first year follow-up ECG including incomplete right bundle branch and first-degree atrioventricular block,and remained the same in another 6 months to 1 year follow-up. All 3 residual shunts disappeared in the follow-up echocardiography except for a 3. 4 mm residual shunt still existed in 1 patient with normal heart size and function. No new onset tricuspid or aortic regurgitation was showed. There was no device malposition or embolization,or thrombosis formation. Conclusion Transcatheter closure of post-surgery residual ventricular septal defect is safe,effective and less trauma. The mid-to long-term follow-up results of these cases are promising.
[Effects of transcatheter closure of extracardiac Fontan fenestration by Amplatzer duct or septal occluder in three patients]

Abstract:Objective: The purpose of this study was to report our experiences from the transcatheter closure of patent fenestration after total cavopulmonary connection (TCPC) with an extra cardiac conduit. Methods: Three patients (7, 14 and 8 years old) with various forms of functionally univentricular heart lesions received a total cavopulmonary connection with an extra cardiac conduit as a final reconstructive procedure. Transcatheter occlusion of the fenestration was accomplished using a 8/6 mm Amplatzer duct occluder in one patient, and 5 mm or 10 mm Amplatzer septal occluder in the other two patients. Residual shunting following occlusion was assessed using angiography and echocardiography. Results: Post total cavopulmonary connection with an extra cardiac conduit, diagnostic catheterization revealed normal pressures in the superior vena cava and pulmonary artery without obstruction at the site of the anastomosis. Angiography of the extra cardiac conduit confirmed the communication between the conduit and the atrium in all three patients and patients still suffered from cyanosis and low oxygen saturation. Immediate full occlusion of fenestration was obtained in all patients. Post closure, mean central venous pressure returned to normal accompanied with significantly increased oxygen saturation. Cyanosis was also significantly attenuated. There were no procedural complications or device failures at intra-hospital and during the 3 to 6 months follow up period. Conclusions: The Amplatzer septal or duct occluder device is a safe and effective strategy for the Fontan fenestration occlusion.
