Percutaneous cement discoplasty for the treatment of lumbar degenerative diseases: A system review and meta-analysis

Abstract:Lumbar degenerative disease is one of the main causes of low back pain in the elderly. [ 1 ] Common lumbar degenerative diseases include lumbar disc herniation, lumbar spinal stenosis, degenerative lumbar scoliosis, and lumbar spondylolisthesis. Degenerative lumbar disc herniation is one of the main causes of low back pain and leg pain in elderly patients, which brings heavy economic and health burdens to patients and society. [ 2 , 3 ] The disease is characterized by a gradual rupture of the fibrous annulus on the outside of the intervertebral disc, leading to morphological changes in the intervertebral disc, resulting in lumbar instability, and the herniated nucleus pulposus oppresses the nerve roots and dura mater of the same level. Generally, patients with lumbar disc herniation consider surgical treatment when conservative treatment such as traction, physiotherapy, or drug therapy is ineffective or progressive neurological dysfunction occurs. [ 4 ] For elderly patients, the choice of surgery for lumbar degenerative diseases is closely related to increased risk of complications and adverse outcomes. Surgical treatment usually involves decompression surgery and fusion techniques; however, standard fusion surgery in elderly patients is associated with a higher rate of complications, hospital length of stay, and readmission, [ 5 , 6 ] Although minimally invasive surgery can reduce risk and shorten hospital stay, it still cannot eliminate the inherent complications of fusion or internal fixation, especially in frail patients. [ 7 , 8 ] Therefore, it is necessary to find a surgical technology that can not only reduce the risk of operation but also effectively reduce the inherent complications of fusion or internal fixation. In recent years, percutaneous cement discoplasty (PCD) introduced by Varga et al seems to be a feasible choice. [ 9 ] By relying on the accurate guidance of medical imaging, polymethyl methacrylate is directly implanted into the diseased intervertebral disc to maintain the stability of the lumbar spine, expand the intervertebral space, and indirectly expand the intervertebral foramen to achieve local decompression, relieve pain and enhance the activity of patients. Fusini et al conducted a systematic review of PCD to explore the indications, clinical outcomes, and complications. [ 10 ] The purpose of this study was to comprehensively review the literature on percutaneous cement discoplasty, conduct a systematic review and meta-analysis, and establish the effect of intervention measures to evaluate the clinical effectiveness of PCD. Due to there being few reports on PCD, and all of them are retrospective studies, there is a high risk of methodological heterogeneity. To further evaluate the stability and reliability of the research results, we conduct sensitivity analysis and subgroup analysis of the results and analyze possible sources of heterogeneity.
