Abstract:Objective: Describe the ultrasound aspects of brain lesions during perinatal asphyxia at Charles de Gaulle pediatric teaching hospital of Ouagadougou,from January 1, 2019 to December 31, 2021. Patients and method: Retrospective cross-sectional study with a descriptive aim over a period of three years, from January 1, 2019 to December 31, 2021, including newborns with perinatal asphyxia who underwent transfontanellar ultrasound at the Charles de Gaulle pediatric university hospital center. . Results: We identified 233 patients meeting the inclusion criteria, out of a total population of 424 newborns suffering from perinatal asphyxia. The average age was 14 days with extremes of 00 days and 28 days; the sex ratio was 1.8. Respiratory distress was the main reason for consultation (42.9%). A predominance of patients classified in SARNAT stage I (68.7%) with a Blantyre score 5 (57.1%) was noted. At ETF, subependymal hemorrhage, ventricular dilatation and periventricular leukomalacia were the main lesions observed with a respective frequency of 62.3%, 30.2% and 26.4%. The parenchymal lesions appeared mainly hyperechoic, i.e. 56.6% of cases and electively involved the white matter (74.8%). Conclusion: The frequency of perinatal asphyxia was high in our series. The most common brain lesions at ETF were: subependymal hemorrhage, ventricular dilatation and periventricular leukomalacia. An MRI exploration would have made it possible to make a more refined lesion assessment. Strengthening prevention remains essential to reduce the incidence of perinatal asphyxia. RÉSUMÉ Objectif : Décrire les aspects échographiques des lésions cérébrales au cours de l’asphyxie périnatale au centre hospitalier universitaire pédiatrique Charles de Gaulle de Ouagadougou, du 1er janvier 2019 au 31 décembre 2021. Patients et méthode : Etude rétrospective transversale à visée descriptive sur une période de trois ans, du 1er janvier 2019 au 31 décembre 2021, incluant des nouveau-nés avec asphyxie périnatale et ayant bénéficié d’une échographie transfontanellaire au CHUP-CDG Résultats : Nous avions recensé 233 patients répondant aux critères d’inclusion, sur une population totale de 424 nouveau-nés souffrant d’asphyxie périnatale. L’âge moyen était de 14 jours avec des extrêmes de 00 jour et 28 jours ; le sex-ratio était de 1,8. La détresse respiratoire était le principal motif de consultation (42,9%). Une prédominance des patients classés en stade I de SARNAT (68,7%) avec un score 5 de Blantyre (57,1%) était notée. A l’ETF, l’hémorragie sous épendymaire, la dilatation ventriculaire et la leucomalacie périventriculaire étaient les principales lésions observées avec une fréquence respective de 62,3%, 30,2% et 26,4%. Les lésions parenchymateuses apparaissaient essentiellement hyperéchogènes, soit 56,6% des cas et intéressaient électivement la substance blanche (74,8%). Conclusion : La fréquence de l’asphyxie périnatale était élevée dans notre série. Les lésions cérébrales les plus fréquentes à l’ETF étaient : l’hémorragie sous épendymaire, la dilatation ventriculaire et la leucomalaccie périventriculaire. Une exploration IRM aurait permis de faire un bilan lésionnel plus affiné. Le renforcement de la prévention reste indispensable pour réduire l’incidence de l’asphyxie périnatale.
