Heart Failure Patients Mortality In Low Income Country Hospital From The Dominican Republic. Prospective Followup

Abstract:Introduction Cardiovascular diseases are the main cause of global morbidity and mortality, generating a high cost for the health system. HF programs achieve a reduction, not only in the death of these patients, but also in the number of hospitalizations and visits to the emergency room, thus improving their quality of life and reducing health costs. Heart failure programs pursue this goal through closer follow-up of patients, achieving early optimization of foundational therapy for heart failure. Objective To determine the impact on mortality of the creation of a heart failure clinic in a public hospital in the Dominican Republic in the period from August 2021 to August 2022. Methodology The demonstration of the clinical impact that this heart failure program achieves in the morbidity and mortality of these patients is what this research study pursues, through the correlation of comparative variables in a period before and after the creation of heart failure consultation. Results Patients studied was 104 and 69% corresponded to the male sex, the main comorbidities found were Hypertension (87.5%), Diabetes Mellitus 2 (66.34%) and Coronary artery disease (55.76%), the average left ventricle ejection fraction at the beginning of the registry was 34.18 ± 5.5 %, corresponding in 77.8% to the HF phenotype with rEF. In contrast, at the end of the study, the estimated average LVEF was 37.63 ± 7%, with the predominance of the rEF phenotype persisting. During follow-up, the rate of admissions and visits to the emergency room went from 1.58 to 1.14 and from 2.83 to 1.70, respectively, the mortality rate was 2.88% (n = 3). The morbidity and perception of quality of life by patients estimated by the KCCQ-12 questionnaire was 47.81 at the beginning of the registry and 78.93 at the end of it. Conclusion The implementation of a care and management program for patients with HF has a significant impact on the reduction of morbidity and mortality. In the one-year follow-up, this registry manages to show a reduction in the admission rate of 27.85%, a reduction in visits to the emergency room of 39.93%, and a reduction in mortality of 46.91%. Improvement is also observed in functional class and in the perception of quality of life by patients, where of 3.85 and 32.69% of patients in functional class IV and III; respectively, at the start of the record. The study is concluded with 0 patients in FC IV and only 7.69% in FC III. The implementation of similar programs will have a beneficial and significant impact on the morbidity and mortality of patients with heart failure.
